To explore individual variations in visibility of the fat planes between the pancreatic parenchyma and adjacent organs and blood vessels using the multi-detector-computed tomography (MDCT). Abdominal contrast-enhanced MDCT examinations of 520 consecutive adult individuals were retrospectively analysed by exploring the presence of visible fat planes between the healthy pancreas and the following surrounding structures: stomach, descending duodenum (D2), splenic, portal, superior mesenteric vein (SV, PV, SMV), inferior vena cava (IVC), and coeliac trunk, common hepatic and superior mesenteric artery (CT, HA and SMA). Spearman’s rank correlation coefficient (rS) was used to assess the correlation of individual gender, age, body mass and BMI, and visible fat planes towards particular surrounding structures. Fat planes between the pancreatic parenchyma and surrounding structures was visible as follows: stomach in 76%, D2 11.7%, SV 51.5%, PV 0%, SMV 28.8%, IVC 80.8%, CT 99.4%, HA 90.4% and SMA in 100% participants. 3. The significance of Enterobacteriaceae as a process hygiene criterion in yogurt production

Yogurt is one of the most popular fermented dairy products with a worldwide acceptance. There are many types of yogurt differing in flavor, physical and chemical properties. Yogurt is produced by adding bacterial culture of Lactobacillus delbrueckii subsp. bulgaricus and Streptococcus thermophilus to milk and cream products. During the period from January 2017 to December 2020, a total of 202 yogurts from different small and medium sized dairy plants were analyzed as part of HACCP self-control programs. The determination of Enterobacteriaceae was performed as an alternative indicator of good hygiene practice. The results showed that 21.29% of analyzed yogurts contained more than 10 CFU/g Enterobacteriaceae, which is the evidence of poor hygiene or inadequate processing, process failure and post-process contamination. Generally, dairy products are potential vehicles for microorganisms from the Enterobacteriaceae family. Good manufacturing practices and good hygiene practices must be followed throughout the production line thoroughly. The absence of classic foodborne pathogens does not indicate that the yogurt is fit for consumption, since other potentially pathogenic bacteria of the Enterobacteriaceae family could be present. Thus, rather than pathogen testing, using Enterobacteriaceae to monitor the effectiveness of implemented preventive prerequisite measures could offer a better view of the quality, sanitary conditions, and safety of yogurt products.

4. P131: Practice patterns of emergency department physicians administering naloxone for patients with suspected opioid overdose

Introduction: Naloxone is recommended for reversing opioid-associated respiratory depression. There is wide variability in emergency department (ED) practice patterns regarding naloxone use, dosing, and observation time post-administration. This study describes the naloxone practice patterns of ED physicians managing suspected opioid overdose patients. Methods: A retrospective chart review was conducted of adult patients (≥ 18 years) presenting to an academic tertiary care centre (consisting of two EDs with an annual census 150,000 visits) in 2017 with suspected opioid overdose who were administered naloxone in the ED. Patients were identified electronically and the following information was abstracted from patient charts: demographics, naloxone dosage and infusion initiation, disposition data, indications for naloxone administration, response to therapy, and adverse effects. Variability in initial and total dose was examined. Initial dose was also compared in those with cardiorespiratory compromise (CPR given, respiratory rate < 8, or desaturation below 89%) using independent samples median tests. Data was analyzed using standard descriptive statistics. Results: 113 patients met inclusion criteria. Indications for naloxone administration were: level of consciousness (50.5%), respiratory depression (4.0%), miosis (1.0%), a combination of factors (19.8%), or undocumented (24.8%). Median initial dose was 0.40 mg (IQR: 0.20-0.40 mg). Median total naloxone administered in the ED was 0.48 mg (IQR: 0.35-1.2 mg). The initial dose resulted in a response in 43.1% of patients, with 36.0% of responding patients later experiencing subsequent respiratory depression. 31% of patients received a naloxone infusion. Initial dose in patients with cardiopulmonary compromise was significantly different only comparing patients who received CPR versus those who did not (median 0.40 mg; IQR: 0.20-0.80 mg; P = 0.019). Four patients experienced emesis following naloxone. Median length of ED stay was 7.0 hours (IQR: 4.0-9.5 hours), and median hospital length of stay was 3.0 days (IQR: 1.0-5.0 days). Median ED observation time prior to discharge was 4.0 hours (IQR: 2.0-8.0 hours). Ultimate disposition home, to the ward, or to the intensive care unit was 47.1%, 42.2%, and 9.8% respectively (1.0% deceased). Conclusion: The dose and usage of naloxone by ED physicians in this study is variable. Further prospective studies are needed to determine the effective naloxone dosing strategy.

The presence of visible fat planes significantly correlated (p 0.001) with body mass for stomach (rFat planes around the pancreatic parenchyma in the MDCT scan was almost always visible towards the adjacent magistral visceral arteries and IVC, always invisible towards the PV, and variably visible towards the SV, SMV, stomach and duodenum depending on the individual body mass, BMI, age and gender.

Summary Application of two-step approach of evapotranspiration ( ET ) crop coefficients ( Kc ) to “approximate” a very complex process of actual evapotranspiration ( ETa ) for field crops has been practiced by water management community. However, the use of Kc , and in particular the concept of growing degree days (GDD) to estimate Kc , have not been sufficiently studied for estimation of evaporative losses from riparian vegetation. Our study is one of the first to develop evapotranspiration crop coefficient ( Kc ET ) curves for mixed riparian vegetation and transpiration ( TRP ) crop coefficients ( Kc TRP ) for individual riparian species as a function GDD through extensive field campaigns conducted in 2009 and 2010 in the Platte River Basin in central Nebraska, USA. Kc TRP values for individual riparian vegetation species [Common reed (Phragmites australis), Cottonwood (Populus deltoids) and Peach-leaf willow (Salix amygdaloides)] were quantified from the TRP rates obtained using scaled-up canopy resistance from measured leaf-level stomatal resistance and reference evapotranspiration. The Kc ET and Kc TRP curves were developed for alfalfa-reference ( Kcr ET and Kcr TRP ) surface. The seasonal average mixed riparian plant community Kcr ET was 0.89 in 2009 and 1.27 in 2010. In 2009, the seasonal average Kcr TRP values for Common reed, Cottonwood and Peach-leaf willow were 0.57, 0.51 and 0.62, respectively. In 2010, the seasonal average Kcr TRP were 0.69, 0.62 and 0.83 for the same species, respectively. In general, TRP crop coefficients had less interannual variability than the Kcr ET . Response of the vegetation to flooding in 2010 played an important role on the interannual variability of Kcr ET values. We demonstrated good performance and reliability of developed GDD-based Kcr TRP curves by using the curves developed for 2009 to predict TRP rates of individual species in 2010. Using the Kcr TRP curves developed during the 2009 season, we were able to predict the TRP rates for Common reed, Cottonwood and Peach-leaf willow in 2010 within 7%, 8% and 13% accuracy, indicating a good performance of the two-step approach proposed in this study for estimating TRP for riparian vegetation. The surface conditions of the riparian ecosystem need to be considered when using the two-step approach to estimate ETa or TRP rates of riparian plant communities. The results of this study provide important water use information and data for riparian vegetation that can be used for more robust hydrologic/water balance analyses.

In this paper is proposed mixed integer linear programming based approach for determining the optimal number, type, and location of remotely controlled and supervised automation devices in distribution networks where island operation of DGs is allowed. The proposed approach enables simultaneous consideration of different types of automation devices (sectionalizing switches, recloseres, fault passage indicators (FPIs)) taking into consideration all relevant costs that influence the selection of the best automation strategy. Simultaneously, the island operation of DGs, i.e. possibility of creating islands is considered in determining the optimal number, type and location of automation devices.

Introduction: Endometriosis is the presence of endometrial glands and stroma outside of uterine cavity. It may occur in the abdominal wall scar after the operation in which uterus was opened. In cesarean section scar it occurs in 0.4%. It is in 2/3 patients characterized with triad of: tumor, periodic pain associated with menses and history of cesarean section. The mechanism of endometriosis occurring in the cesarean scar is felt to be secondary to iatrogenic transplantation of endometrium or extrauterine decidual tissue into the incision during the cesarean section. Case outline. Forty years old patient with tumor 4,5x4 cm that appeared in abdominal wall scar one year after second cesarean section, followed by periodic pain and macroscopic changes associated with menses. First diagnosis was granuloma in the surgical scar, but as she had periodic symptoms, diferential diagnosis was endometriosis. Hormonal therapy with contraceptive drugs was ordered. As it was no improvement she was operated. The surgical excision of the tumor including fascia and muscle tissue was done. Sample revealed endometrium after histopathologic examination. Patient was completely recovered and without relapse of symptoms during follow up to date. Conclusion. When there is a tumor in the cesarean section scar or scar after the operation in which uterus or ovarial tube was opened, followed with periodical pain and macroscopic changes associated with menses, endometriosis should be considered. Surgical excision of the tumor is sufficient and patohistological examination confirms diagnosis. .

The aim of the study is detection and evaluation of the abdominal and gastrointestinal infective foci using 99mTc-ciprofloxacin (Laboratory for radioactive isotopes, Vinca).In total 21 patients with clinical suspicion on abdominal or gastrointestinal infection were investigated. In all the patients, planar liver/spleen scintigraphy was performed. Ciprofloxacin chloride (3.5 mg) was mixed with 555 MBq of 99mTc in 3 mL of physiological solution and incubated for 20 min. After slow i.v. injection in a cubital vein, dynamic acquisition (1 f/min) was performed during the first 60 min in the position of interest, followed by static acquisition (500,000 imp) anterior and posterior view, abdomen and pelvis after 1 h and 4 h in all patients. When necessary, additional scintigrams were acquired after 24 h. In all the patients with negative or equivocal findings of planar scintigraphy, emission computerized tomography (SPECT) was performed (60 positions, 6 degrees). Interpretation was made by three independent observers. Additional data were provided using clinical findings, ultrasonography, computed tomography and magnetic resonance imaging, laboratory analyses, and surgical or microbiological confirmation of infection.There were eleven true-positive findings, seven true negative, two were false negative while one was false positive due to intestinal obstruction. Sensitivity was 79%, specificity 91%, positive predictive value 92%, negative predictive value 77%, accuracy 84%.According to our results, scintigraphy with radiolabeled ciprofloxacin is a useful method for detection and assessment of exact localization of abdominal and gastrointestinal infections.

We present a case of incidental detection of thyroid carcinoma at the time of parathyroidectomy in patient on dialysis after cadaver renal transplantation.

The work reported in this paper covers some problems regarding radial distribution network planning. Beside the common optimization criteria (new facility installation, maintenance and operation costs), the cost of the energy not supplied caused by expected outages is introduces. The expected outage time is determined in the reliability analysis in which reliability indices SAIFI (system average interruption frequency index) and SAIDI (system average interruption duration index) are calculated. The influence of network topology and switching devices configuration on system reliability is also described. A program module has been developed as a proposal to solving the problem of radial distribution network planning in rural areas. The program module is based on Prim minimum spanning tree algorithm. It is intended to become a part of software tools collectively referred CADDiN (computer aided design of distribution networks).

Fibrin tissue adhesive (FTA) is an agent developed for achieving better hemostasis and adhesion of living tissue. FTA appears to have no tissue toxicity, promotes a firm seal in seconds to minutes, is reabsorbed in days to weeks following application, and appears to promote local tissue growth and repair. It can be used in various surgical procedures. It has been used preoperatively, perioperatively and postoperatively in abdominal surgical procedures. There were no side effects. Improvement of surgical hemostasis was obvious in all patients. Anastomotic leakages were closed in a shorter time without surgical intervention. Sero-lymphatic drainage after surgical procedures that include extensive lymph node dissections was less. Use of FTA in treatment of fistula in ano was successful. The data would indicate that use of FTA may be a good alternative in solving various conditions in every day clinical practice, although a bigger randomized series, and longer follow up is needed.

16. Discriminant analysis of pre- and intraoperatively detected prognostic factors influencing lymph node involvement in patients with colorectal carcinoma

The aim of this study was to establish whether, and to what extent, pre- and intraoperatively detected characteristics (demographic, anamnestic and laboratory data) and tumor characteristics can be used in the assessment of regional lymph node involvement in patients with colorectal carcinoma. The assessment also included the number of lymph nodes involved in patients with positive lymph nodes. Considering that the number of obtained lymph nodes widely varies in resected specimens, assessment parameters also included the percentage of the involved lymph nodes within the total population of lymph nodes.From 1992-1993, 46 patients with carcinoma of the rectum and sigmoid colon were studied, with a total number of 736 lymph nodes evaluated. Out of the total number of lymph nodes, 577 (78.4%) were benign and 159 (21.6%), malignant. Data were analyzed by multi-variant statistical methods, namely: discriminant analysis and multiple regression with the aid of SPSS/PC+ software.For this patient group, we evaluated the following potentially predictive factors for lymph node involvement: age; serum hemoglobin, albumin and alkaline phosphatase levels; weight loss; and the primary tumor localization characteristics: histologic type, macroscopic growth pattern and depth of tumor invasion of the bowel wall. We found that there was no difference in the prediction of regional lymph node involvement between analysis of the aforementioned parameters and analysis of the isolated discriminators only.A predictability likelihood of 83.78% greatly surpasses the acceptable error tolerance level of 5%. Correlation of demographic, anamnestic and laboratory data about the patient and the characteristics of the primary tumor cannot be used in distinguishing malignant lymph nodes from benign ones. These data cannot be the basis for exact intraoperative staging and thus cannot be significant criteria for decision-making about operative treatment modalities.

In this study, we present radioimmunoassay data describing the concentration of Vasoactive Intestinal Polypeptide (VIP) in both plasma and colonic biopsies, as well as immunostaining of VIPergic innervation in mucosal biopsies of normal subjects and patients with ulcerative colitis (UC).Thirty three patients with UC and 17 healthy subjects were investigated. All UC patients suffered from active disease. Fasting circulating levels of VIP in plasma as well as tissue concentrations were measured by radioimmunoassay. For the immunohistochemistry, polyclonal antibody against VIP and the streptavidin-biotin peroxidase complex technique were carried out.Overall plasma VIP concentrations in the UC patients were similar to those in the controls. Significantly decreased concentrations of VIP were found in UC of rectum compared to the normal tissue. However, both plasma VIP values and tissue concentrations were found to be significantly lower in patients expressing minimal or mild active disease according to clinical activity index (AI) and histological activity index (HAI), but marked increase of plasma VIP was clear in UC patients with moderate or severe AI and HAI. There was a trend towards increased tissue concentrations of VIP in the group of patients with moderate or severe AI and HAI. Our immunohistochemical analysis of VIP fibers and nerve cell bodies revealed consistently weaker VIP-immunoreactivity in the rectum in UC patients with minimal or mild HAI. Simultaneously, in the rectal biopsies from UC patients with moderate and severe disease, the fibers in the lamina propria and ganglion cells in the submucous plexus were markedly increased in density and in degree of immunostaining. Very strong immunoreactivity was also found in inflammatory cells of the lamina propria as well as in the epithelial layer of the biopsies from UC patients with obvious disease.Our study shows clearly the heterogeneity in the response of VIP plasma level as well as rectum concentration and distribution in UC patients at different stages of the active disease. The possible role of VIP in the colon suggests that further studies of the alterations of this gut peptide may be useful in the understanding of UC pathophysiology.

A group of 26 patients with the clinical diagnosis of Senile Dementia of Alzheimer’s type (SDAT) was randomly assigned in a double blind cross-over trial of pyritinol versus placebo. Psychiatric and neurological examination, psychometric testing and regional cerebral blood flow measurements were used to assess the effects of medication. Pyritinol had a beneficial effect on cognitive performance which was sustained during the long term follow up of one year.

Cellular immunity was examined by two methods in vitro: absolute number T and B lymphocytes (E.T. and E.A.C. rosettes) and by the lymphocyte transformation test, in 280 patients with psoriasis (83.5% with psoriasis vulgaris and 16.5% with other forms of psoriasis). No statistically significant changes were found in comparison with the control group.
