Abstract

Primary osseous Hodgkin's lymphoma is a very rare entity. Cases reported in the literature are limited with often insufficient initial exploration. We report a new case of a 24 years old patient with a diagnosis of primary osseous Hodgkin lymphoma of the lumbosacral region with extension to the soft tissues, without simultaneous lymph node involvement confirmed both by conventional and metabolic imaging. The patient received a combination chemotherapy (two courses BEACOPP® and four courses ABVD) followed by radiotherapy of the lombosacral region at the dose of 40Gy in 20 fractions. Fifteen months after the end of treatment, the patient was in complete remission.

Abstract

Resume Objectif Rapporter les aspects epidemiologiques, anatomo-cliniques, pronostiques et therapeutiques des sarcomes uterins (SU) a partir d’une serie retrospective de 14 cas tunisiens. Patientes et methodes Notre etude retrospective a porte sur 14 cas de SU, colliges de 1995 a 2008 dans le service de cancerologie-radiotherapie du CHU Farhat Hached de Sousse. Nous avons analyse les donnees epidemiologiques, anatomo-cliniques et les resultats therapeutiques. Le bilan initial avait comporte : anamnese, examen clinique, donnees de l’imagerie (radiographie du thorax, echographie abdominopelvienne dans 10 cas et TDM abdominopelvienne dans 7 cas), classification FIGO modifie. Ces patients ont eu un traitement par hysterectomie, radio- et ou chimiotherapie. Resultats Les 14 patientes avaient un âge moyen de 48,5 ans (15 a 78 ans) et n’avaient pas d’antecedent pathologique particulier, notamment irradiation anterieure ou exposition medicamenteuse dans l’enfance. Il s’agissait de leiomyosarcomes (LMS) dans 3 cas, tumeur mullerienne mixte (TMM) dans 7 cas (50 %) et d’un sarcome du stroma endometrial (SSE) dans 4 cas. Le diagnostic a ete pose en postoperatoire dans 6 cas et en preoperatoire sur biopsie de curetage biopsique dans 8 cas. Les SU etaient classes stade I dans 8 cas, II dans 3 cas et III dans 3 cas. L’hysterectomie a ete associee a une annexectomie dans 12 cas et a un curage iliaque bilateral dans un cas. Huit de nos patientes ont eu en adjuvant une radiotherapie pelvienne et 3 une chimiotherapie. Avec un recul moyen de 54 mois, nous avons observe 4 rechutes locales, des metastases pulmonaires dans un cas et une carcinose peritoneale dans un autre cas. Sept patientes sont vivantes en remission, 5 sont decedees de progression tumorale et 2 sont perdues de vue. Discussion et conclusion Les SU restent des tumeurs rares, tardivement diagnostiquees et de mauvais pronostic. Un traitement adjuvant par radio et/ou chimiotherapie doit etre discute pour les formes a haut risque et localement avancees.

Abstract

To report epidemiologic, anatomoclinical treatment and results about a Tunisian retrospective serie of 14 patients with uterine sarcoma (US).A retrospective study of 14 cases of uterine sarcoma treated in the Radiotherapy Unit of Farhat Hached Hospital of Sousse between 1995 and 2008. Epidemiologic and anatomoclinical features were assessed. A complete work-up including abdominal ultrasonography and abdominopelvic CT scan were perfomed in 7 and 10 cases, respectively.The median age was 48.5 years (15 to 78) without previous medical history of irradiation or prolonged drug exposition. There were 3 cases of leimyosarcoma, 7 cases of mixed Mullerian tumor and 4 cases of endometrial stromal sarcoma. The diagnosis was made postoperatively in 6 patients and after curettage in 8 cases. According to the classification of the International Federation of Gynecology and Obstetrics (FIGO), 8 patients were in FIGO stage I, 3 in stage II and 3 in stage III. Hysterectomy was associated with annexectomy in 12 cases. Bilateral pelvic lymphadenectomy was performed in one patient. Eight of our 14 patients underwent postoperative pelvic radiotherapy, associated with adjuvant chemotherapy in 3 cases. The evolution was marked by the occurrence of local recurrence in 4 patients who did not have adjuvant therapy with lung metastases in one case and peritoneal carcinomatosis in another case. With a medium follow-up from 54 months, 7 patients are free from disease, 5 died of their disease (after a mean of 24 months) and 2 patients were lost to follow-up.Uterine sarcomas are rare tumors with poor prognosis. Adjuvant therapy (radiotherapy and/or chemotherapy should be discussed due to the high risk of recurrence or metastases.

13. Glioblastome cérébral : résultats thérapeutiques d’une série tunisienne

Abstract

Objectif Rapporter les resultats du traitement du glioblastome dans un centre tunisien. Patients et methode Cent trente-quatre patients ont ete pris en charge pour un glioblastome dans un centre tunisien entre 1992 et 2011. Resultats L’âge moyen des patients etait de 50 ans (6–76 ans). L’imagerie preoperatoire a permis le diagnostic de glioblastome dans 122 cas. Une exerese jugee macroscopiquement complete a ete realisee chez 49 patients (36,6 %), confirmee par l’imagerie postoperatoire chez 34 (25,4 %). Tous les patients ont ete operes. Quinze deces ont ete deplores apres la chirurgie (11 %). Le traitement adjuvant chez les 119 patients restants a compris une radiotherapie dans seulement 58 cas, soit 44 % des patients. Une chimiotherapie a ete administree chez seulement 20 d’entre eux (14,3 %). Sur le plan evolutif, 112 tumeurs ont recidive ou poursuivi leur evolution (94 %). Le delai moyen de recidive etait de 13 mois. Nous avons constate que plus l’âge etait avance, plus la recidive survenait precocement (p = 0,009). L’exerese complete avec absence de residu sur l’IRM postoperatoire retardait la recidive de 7 a 33 mois (p Conclusion Le glioblastome est une tumeur cerebrale frequente, de pronostic defavorable. La prise en charge multidisciplinaire ameliore la probabilite de survie et retarde la recidive, notamment grâce aux nouvelles techniques de radiotherapie et la standardisation de la chimiotherapie par le temozolomide.

23. Lung Adenocarcinoma with Gingival Metastasis.

Abstract

Gingival metastasis of lung cancer is uncommon. We report the case of an 82-year-old male smoker admitted to the pulmonology department with right pleural effusion. A chest computed tomography (CT) scan showed an invasive right hilar tumour, adherent to the superior vena cava, pulmonary artery, main right bronchus, mediastinal pleura and pericardium with lymphangitic carcinomatosis of the right lung. Pleural biopsy revealed pleural metastasis of pulmonary adenocarcinoma, its primary lung origin being confirmed by immunohistochemistry. One month later, the patient developed an ulcerated polypoid gingival mass. Biopsy of this lesion showed a poorly differentiated carcinoma compatible with metastasis from the lung adenocarcinoma. The patient underwent irradiation of the gingival mass at a dose of 30 Gray, but his condition worsened rapidly and he was not fit for chemotherapy. He received palliative treatment and died 2 months after diagnosis of his metastatic lung cancer., Learning Points: Lung cancers mostly metastasize to the bones, liver, lymph nodes, brain, lung and adrenal glands, with adenocarcinoma being the most common histological type.Distant metastasis to the oral region is very uncommon but can be the first manifestation of a primary tumour.Oral metastasis can be mistaken for a benign lesion, so a biopsy should be taken for further analysis., Competing Interests: Conflicts of Interests: The Authors declare that there are no competing interests.

A complete work-up including abdominal ultrasonography and abdominopelvic CT scan were perfomed in 7 and 10 cases, respectively., Results: The median age was 48.5 years (15 to 78) without previous medical history of irradiation or prolonged drug exposition. There were 3 cases of leimyosarcoma, 7 cases of mixed Mullerian tumor and 4 cases of endometrial stromal sarcoma. The diagnosis was made postoperatively in 6 patients and after curettage in 8 cases. According to the classification of the International Federation of Gynecology and Obstetrics (FIGO), 8 patients were in FIGO stage I, 3 in stage II and 3 in stage III. Hysterectomy was associated with annexectomy in 12 cases. Bilateral pelvic lymphadenectomy was performed in one patient. Eight of our 14 patients underwent postoperative pelvic radiotherapy, associated with adjuvant chemotherapy in 3 cases. The evolution was marked by the occurrence of local recurrence in 4 patients who did not have adjuvant therapy with lung metastases in one case and peritoneal carcinomatosis in another case. With a medium follow-up from 54 months, 7 patients are free from disease, 5 died of their disease (after a mean of 24 months) and 2 patients were lost to follow-up., Discussion and Conclusion: Uterine sarcomas are rare tumors with poor prognosis. Abstract

Pineal metastases of solid tumors are rare. Primary cancer is generally absent at the diagnosis, for which a cerebral MRI is important.
