1. Effect of inbreeding on reproduction, weight and survival of Tazegzawt sheep lambs

Abstract: This study evaluated the effects of inbreeding on the reproduction, weight, and survival of Tazegzawt Kabylian lambs. The study focused on the growth performance of 67 lambs born during 2013 to 2015 at the level of the herd of INRAA experimental station of Bejaia. Because of the difficulty in finding breeding males, the rams used for mating came from individuals of the same herd. Analysis of the results indicated a decrease in reproductive parameters of 6 points for fertility, 16 points for prolificacy and 4 points for fecundity respectively between the first and third lambing. The mortality rate increased with inbreeding for the first, second and third lambing. This mortality was characterized by mortality pre-partum and post-partum, before the age of 10 days and before weaning. The appearance of anterior limb abnormalities and cases of blindness and inguinal hernia were observed. The change of males and the use of cryopreservation as a means of reducing inbreeding and for the preservation and conservation of this lesser known genetic potential thus becomes imperative.

Abstract: The JEM-EUSO (Joint Experiment Missions for the Extreme Universe Space Observatory) program aims at developing Ultra-Violet (UV) fluorescence telescopes for efficient detections of Extensive Air Showers (EASs) induced by Ultra-High Energy Cosmic Rays (UHECRs) from satellite orbit. In order to demonstrate key technologies for JEM-EUSO, we constructed the EUSO-Balloon instrument that consists of a ∼1 m 2 refractive telescope with two Fresnel lenses and an array of multi-anode photo-multiplier tubes at the focus. Distinguishing it from the former balloon-borne experiments, EUSO-Balloon has the capabilities of single photon counting with a gate time of 2.3 µs and of imaging with a total of 2304 pixels. As a pathfinder mission, the instrument was launched for an 8 h stratospheric flight on a moonless night in August 2014 over Timmins, Canada. In this work, we analyze the count rates over ∼2.5 h intervals. The measurements are of diffuse light, e.g. of airglow emission, back-scattered from the Earth's atmosphere as well as artificial light sources. Count rates from such diffuse light are a background for EAS detections in future missions and relevant factor for the analysis of EAS events. We also obtain the geographical distribution of the count rates over a ∼780 km 2 area along the balloon trajectory. In developed areas, light sources such as the airport, mines, and factories are clearly identified. This demonstrates the correct location of signals that will be required for the EAS analysis in future missions. Although a precise determination of count rates is relevant for the existing instruments, the absolute intensity of diffuse light is deduced for the limited conditions by assuming spectra models and considering simulations of the instrument response. Abstract: EUSO-Balloon is a pathfinder mission for the Extreme Universe Space Observatory onboard the Japanese Experiment Module (JEM-EUSO). It was launched on the moonless night of the 25th of August 2014 from Timmins, Canada. The flight ended successfully after maintaining the target altitude of 38 km for five hours. One part of the mission was a 2.5 hour underflight using a helicopter equipped with three UV light sources (LED, xenon flasher and laser) to perform an inflight calibration and examine the detectors capability to measure tracks moving at the speed of light. We describe the helicopter laser system and details of the underflight as well as how the laser tracks were recorded and found in the data. These are the first recorded laser tracks measured from a fluorescence detector looking down on the atmosphere. Abstract: EUSO-TA is a ground-based telescope, installed at the Telescope Array (TA) site in Black Rock Mesa, Utah, USA. This is the first detector to successfully use a Fresnel lens based optical system and multi-anode photomultipliers (64 channels per tube, 2304 channels encompassing a 10.6° × 10.6° field of view) for detection of Ultra High Energy Cosmic Rays (UHECR). The telescope is located in front of one of the fluorescence detectors of the TA experiment. Since its installation in 2013, the detector has observed several ultra-high energy cosmic ray events and, in addition, meteors. The limiting magnitude of 5.5 on summed frames (∼ 3 ms) has been established. Measurements of the UV night sky emission in different conditions and moon phases and positions have been completed. Abstract: Theories with extra dimensions at low Planck scale, offer the exciting possibility of mini black holes production in ultra high-energy particles interactions. In particular, cosmic neutrinos interaction can produce black holes deep in the Earth's atmosphere. These mini black holes then decay and produce ‘characteristic’ air showers. In this paper, we examine the properties of the mini black holes (mBH) air showers and compare them to the standard model (mSM) ones. We point out to some possible criteria that help distinguishing mBH air showers.

Abstract: In models with large extra dimensions, the interactions of ultra high energy particles can create mini black holes; this happens when ultra high energy cosmic rays interact with the nucleons of the Earth’s atmosphere. These mini black holes then decay in the atmosphere and produce characteristic air showers. In this work we compare the air showers from black holes decay with those of the standard model and discuss a possible criteria allowing the distinction between them.

An introduction to various ordering methods in median-based multidimensional filtering is given. The various 3-D median processing methods and the effect of the conversion from the RGB to the YUV domain were tested. The signal processing methods used for testing were noise reduction and interlaced-to-progressive conversion. The test results are given. >

Abstract: Oral and/or intestinal mucositis is a severe complication of hematopoietic SCT. Keratinocyte growth factor (KGF) has proven activity in the prevention of oral mucositis. We examined the efficacy of KGF in the prevention of intestinal mucositis. From January 2006 until December 2007, 35 consecutive patients underwent autologous SCT (auto-SCT) in our institution. A total of 15 consecutive patients who underwent auto-SCT from March 2007 to December 2007 received KGF for the prevention of mucositis and were included in the study group A, whereas 20 consecutive patients treated from January 2006 to March 2007, were included in the historical control group B. Abstract: Short-term fluctuations in systolic blood pressure (SBP) and heart rate (HR) and their inter-relationship were analysed in a group of normotensive middle-aged men (n= 16) using a multivariate autoregressive modelling technique. This study is the first to evaluate the beat-to-beat variability of SBP and HR in a group of real normotensive subjects. Direct intra-arterial blood pressure was registered together with ECG using an ambulatory tape recording technique (the Oxford method). Power spectrum density estimates (PSD) were used as a measure of the variability. PSDs were calculated over 3-min periods for four basic physiological conditions: during sleep and in the supine, sitting and standing positions. The inter-relationship between the blood pressure and heart rate variabilities was analysed using a closed-loop model. In agreement with results presented earlier in the literature, the beat-to-beat variation in SBP and HR was concentrated in three typical power spectrum regions: the high-frequency (HF = 0·15-0·35 Hz) region (respiration), the mid-frequency (MF = 0·075-0·15 Hz) region (vasomotor oscillation) and the low-frequency (LF = 0·02-0·075 Hz) region (thermoregulation). The variability changes considerably between different situations, especially that of the MF region. The variability was most prominent in the MF region and in the standing position. The variability was generally smallest in the HF region and in sleep. The results also demonstrate that the beat-to-beat variability in SBP and HR can considerably affect one another.

Abstract: Summary. The 24-hour variability in blood pressure and heart rate of 14 normotensive subjects was examined under ambulatory conditions with a continuous, invasive method. The results were analysed with 30-second means calculated from beat-to-beat values. The registered time series had three different types of variation: (1) a continuous, fast variation around the mean level; (2) a stepwise, permanent change to a new level; and (3) a slow, continuous rise or fall. Variability in blood pressure and heart rate was determined by the 80 percent range of variation calculated from the cumulative distribution as the difference between the 90th and 10th percentiles. The variability in blood pressure and heart rate was significantly greater during waking hours than during night-time. Variability followed a similar type of diurnal profile as the mean level. However, variability of diastolic pressure changed to a lesser extent than that of systolic pressure. Total 24-hour variability was on average 45 mmHg in systolic blood pressure, 24 mmHg in diastolic pressure and 40 b.p.m. in heart rate. Using night-time to describe the basal state, physical and mental activities in the evening and during daytime were found to raise variability in systolic pressure with 66%, diastolic with 8–16% and heart rate with 130–180%, respectively. The results show that in normotensive subjects the sleep-wake state and activity are the major factors that affect both the diurnal profile and the variability in blood pressure and heart rate.

Abstract: Color, phenolic content, and chemical age values of red wines made from Cretan grape varieties (Kotsifali, Mandilari) were evaluated over nine months of maturation in different containers for two vintages. The wines differed greatly on their anthocyanin profiles. Mid-IR spectra were also recorded with the use of a Fourier Transform Infrared Spectrophotometer in ZnSe disk mode. Analysis of Variance was used to explore the parameter’s dependency on time. Determination models were developed for the chemical age indexes using Partial Least Squares (PLS) (TQ Analyst software) considering the spectral region 1830–1500 cm−1. The correlation coefficients (r) for chemical age index i were 0.86 for Kotsifali (Root Mean Square Error of Calibration (RMSEC) = 0.067, Root Mean Square Error of Prediction (RMSEP) = 0,115, and Root Mean Square Error of Validation (RMSECV) = 0.164) and 0.90 for Mandilari (RMSEC = 0.050, RMSEP = 0.040, and RMSECV = 0.089). For chemical age index ii the correlation coefficients (r) were 0.86 and 0.97 for Kotsifali (RMSEC 0.044, RMSEP = 0.087, and RMSECV = 0.214) and Mandilari (RMSEC = 0.024, RMSEP = 0.033, and RMSECV = 0.078), respectively. The proposed method is simpler, less time consuming, and more economical and does not require chemical reagents.

Abstract: Photonic time-bin qubits are well suited to transmission via optical fibers and waveguide circuits. The states take the form 1/sqrt[(2)](α|0⟩+e^{iϕ}β|1⟩), with |0⟩ and |1⟩ referring to the early and late time bin, respectively. By controlling the phase of a laser driving a spin-flip Raman transition in a single-hole-charged InAs quantum dot, we demonstrate complete control over the phase, ϕ. We show that this photon generation process can be performed deterministically, with only a moderate loss in coherence. Finally, we encode different qubits in different energies of the Raman scattered light, paving the way for wavelength-division multiplexing at the single-photon level.

Abstract: In this study, the effects of skating and diagonal techniques on skiing load at various speeds were compared. When skiing at the same speed, subjects' heart rates were significantly lower when using the skating technique than when using the diagonal technique (p less than 0.05). At maximal speed, skating technique gave a significantly higher speed at the same heart rate (p less than 0.001) than diagonal technique. Above the anaerobic threshold (4 mmol.L-1) blood lactic acid levels remained significantly lower (p less than 0.005) when using the skating technique than when using the diagonal technique. The results suggest that at the same speed, skating technique loads aerobic and anaerobic energy production less than diagonal technique, and that the same maximal energy production gives a significantly higher skiing speed when using the skating technique than when the diagonal technique (p less than 0.001).

25. Converting Biomass and Waste Plastic to Solid Fuel Briquettes

Abstract: This work examines the production of briquettes for household use from biomass in combination with plastic materials from different sources. Additionally, the combustion characteristics of the briquettes in a common open fireplace were studied. It is clear that the geometry of the briquettes has no influence on the smoke emissions. When the briquettes have a small amount of polyethylene terephthalate (PET), the behavior in the combustion is steadier because of the increase of oxygen supply. The smoke levels are between the 3rd and 4th grades of the smoke number scale. Measuring the carbon monoxide emission, it was observed that the burning of the plastic in the mixture with biomass increases the carbon monoxide emissions from 10% to 30% as compared to carbon monoxide emission from sawdust biomass emissions which was used as a reference.

27. Treatment of a resistant case of schizoaffective disorder with lumateperone: A case report.

Abstract: To this day, there exists skepticism about the reliability and clinical utility of the diagnostic criteria and classification of schizoaffective disorder. In addition, the treatment of schizoaffective disorder, especially of treatment-resistant cases, has been minimally investigated. As a result, formulating official treatment guidelines for schizoaffective disorder has been challenging. We present a case of a 27-year-old female, diagnosed with schizoaffective disorder, bipolar type, for whom, for over 5 years, trials of traditional treatments, to include psychotherapy, pharmacotherapy, and electroconvulsive therapy, were either partially effective or discontinued due to intolerable side effects. The subsequent off-label use of lumateperone led to an adequate response. Lumateperone is an atypical antipsychotic, approved by the Food and Drug Administration for schizophrenia and bipolar depression in adults. Interestingly, it has a similar structure and mechanism of action to paliperidone, the only Food and Drug Administration-approved medication for schizoaffective disorder. 28. State of perinatal mental health care in the WHO region of Europe: a scoping review.

Abstract: Background: Although perinatal mental disorders are the most common health complication among women in the perinatal period, there is a huge gap in the implementation of related research findings in the health care system. We mapped the state of perinatal mental health (PMH) care in the WHO Europe region with aim to identify leading countries, which can serve as models for countries with less developed perinatal mental health care., Methods: Guidelines, policies, and documents related to screening and treatment services for PMH were searched as grey literature. Results were analysed to assess the status of PMH care in the WHO European countries and to identify gaps (absence of relevant service or documents). The state of perinatal mental health care was scored on a 0-5 scale., Results: The grey literature search resulted in a total of 361 websites. Seven countries (Belgium, Finland, Ireland, Netherlands, Sweden, UK, Malta) received full points for the presence of relevant PMH services or documents, while five countries received zero points. Most WHO European countries (48/53) have general mental health policies, but only 25 countries have policies specifically on perinatal mental health. Ten countries offer PMH screening, and 11 countries offer PMH service (of any type). Any PMH guidelines were provided in 23/53 countries., Conclusions: Perinatal mental health care is in its infancy in most WHO European countries. Abstract: Prenylated isoflavonoids are phytochemicals with promising antifungal properties. Recently, it was shown that glabridin and wighteone disrupted the plasma membrane (PM) of the food spoilage yeast Zygosaccharomyces parabailii in distinct ways, which led us to investigate further their modes of action (MoA). Transcriptomic profiling with Z. parabailii showed that genes encoding transmembrane ATPase transporters, including Yor1, and genes homologous to the pleiotropic drug resistance (PDR) subfamily in Saccharomyces cerevisiae were upregulated in response to both compounds. Gene functions involved in fatty acid and lipid metabolism, proteostasis, and DNA replication processes were overrepresented among genes upregulated by glabridin and/or wighteone. Chemogenomic analysis using the genome-wide deletant collection for S. cerevisiae further suggested an important role for PM lipids and PM proteins. Deletants of gene functions involved in biosynthesis of very-long-chain fatty acids (constituents of PM sphingolipids) and ergosterol were hypersensitive to both compounds. Using lipid biosynthesis inhibitors, we corroborated roles for sphingolipids and ergosterol in prenylated isoflavonoid action. The PM ABC transporter Yor1 and Lem3-dependent flippases conferred sensitivity and resistance, respectively, to the compounds, suggesting an important role for PM phospholipid asymmetry in their MoAs. Impaired tryptophan availability, likely linked to perturbation of the PM tryptophan permease Tat2, was evident in response to glabridin. Finally, substantial evidence highlighted a role of the endoplasmic reticulum (ER) in cellular responses to wighteone, including gene functions associated with ER membrane stress or with phospholipid biosynthesis, the primary lipid of the ER membrane. IMPORTANCE Preservatives, such as sorbic acid and benzoic acid, inhibit the growth of undesirable yeast and molds in foods. Unfortunately, preservative tolerance and resistance in food spoilage yeast, such as Zygosaccharomyces parabailii, is a growing challenge in the food industry, which can compromise food safety and increase food waste. Prenylated isoflavonoids are the main defense phytochemicals in the Fabaceae family. Glabridin and wighteone belong to this group of compounds and have shown potent antifungal activity against food spoilage yeasts. The present study demonstrated the mode of action of these compounds against food spoilage yeasts by using advanced molecular tools. Overall, the cellular actions of these two prenylated isoflavonoids share similarities (at the level of the plasma membrane) but also differences. Tryptophan import was specifically affected by glabridin, whereas endoplasmic reticulum membrane stress was specifically induced by wighteone. Abstract: High resistance towards traditional antibiotics has urged the development of new, natural therapeutics against methicillin-resistant Staphylococcus aureus (MRSA). Prenylated (iso)flavonoids, present mainly in the Fabaceae, can serve as promising candidates. Herein, the anti-MRSA properties of 23 prenylated (iso)flavonoids were assessed in-vitro. The di-prenylated (iso)flavonoids, glabrol (flavanone) and 6,8-diprenyl genistein (isoflavone), together with the mono-prenylated, 4'-O-methyl glabridin (isoflavan), were the most active anti-MRSA compounds (Minimum Inhibitory Concentrations (MIC) ≤ 10 µg/mL, 30 µM). The in-house activity data was complemented with literature data to yield an extended, curated dataset of 67 molecules for the development of robust in-silico prediction models. A QSAR model having a good fit (R 2 adj 0.61), low average prediction errors and a good predictive power (Q 2 ) for the training (4% and Q 2 LOO 0.57, respectively) and the test set (5% and Q 2 test 0.75, respectively) was obtained. Furthermore, the model predicted well the activity of an external validation set (on average 5% prediction errors), as well as the level of activity (low, moderate, high) of prenylated (iso)flavonoids against other Gram-positive bacteria. Recently, we showed that priming with reactive oxygen species (ROS) specifically stimulated the production of glyceollins in Rhizopus spp.-elicited soybean seedlings (ROS + R). In this study, we achieved diversification of the inducible subclasses of prenylated isoflavonoids in soybean, by additional stimulation of two prenylated isoflavones and one prenylated coumestan. This was achieved by using a combination of the relatively long-lived ROS representative, H 2 O 2 , with AgNO 3 prior to microbial elicitation. Microbial elicitation was performed with a live preparation of either a phytopathogenic fungus, Rhizopus spp. or a symbiotic bacterium, Bacillus subtilis. B. subtilis induced 30% more prenylated isoflavones than Rhizopus spp. in (H 2 O 2 + AgNO 3 )-treated seedlings, without significantly compromising the total levels of glyceollins, compared to (ROS + R)-treated seedlings. The most abundant prenylated isoflavone induced was 6-prenyl daidzein, which constituted 60% of the total isoflavones. The prenylated coumestan, phaseol, was also induced in the (H 2 O 2 + AgNO 3 )-treated and microbially elicited seedlings. Based on previously developed quantitative structure-activity relationship (QSAR) models, 6-prenyl daidzein and phaseol were predicted to be promising antibacterials. Overall, we show that treatment with H 2 O 2 and AgNO 3 prior to microbial elicitation leads to the production of promising antibacterial isoflavonoids from different subclasses. Abstract: Glyceollins are a class of antimicrobial prenylated pterocarpans produced in soybean seedlings upon fungus elicitation. Priming with reactive oxygen species (ROS) prior to elicitation with Rhizopus oligosporus/oryzae (R) was investigated for its potential to enhance glyceollin production. ROS-priming prior to R-elicitation (ROS + R) increased glyceollin production (8.6 ± 0.9 µmol/g dry weight (DW)) more than 4-fold compared to elicitation without priming (1.9 ± 0.4 µmol/g DW). Furthermore, ROS-priming was superior to two physical primers which were used as benchmark primers, namely slicing (5.0 ± 0.6 µmol glyceollins/g DW) and sonication (4.8 ± 1.0 µmol glyceollins/g DW). Subsequently, the robustness of ROS + R was assessed by applying it to another soybean cultivar, where it also resulted in a significantly higher glyceollin content than R-elicitation without priming. 34. Testing The Lamp-Post and Wind Reverberation Models with XMM-Newton Observations of NGC 5506.

Abstract: The lamp-post geometry is often used to model X-ray data of accreting black holes. Despite its simple assumptions, it has proven to be powerful in inferring fundamental black hole properties such as the spin. Early results of X-ray reverberations showed support for such a simple picture, though wind-reverberation models have also been shown to explain the observed delays. Here, we analyze new and old XMM-Newton observations of the variable Seyfert-1 galaxy NGC 5506 to test these models. The source shows an emission line feature around 6.7 keV that is delayed relative to harder and softer energy bands. The spectral feature can be modeled with either a weakly relativistic disk line or by scattering in distant material. By modeling both the spectral and timing signatures, we find that the reflection fraction needed to explain the lags is larger than observed in the time-averaged spectrum, ruling out both a static lamp-post and simple wind reverberation models.

Abstract: The TNM staging system for cancer was developed by Pierre Denoix in France in the 1940s and 1950s. The North American effort to standardize the TNM system for cancer staging was first organized in 1959 as the American Joint Committee for Cancer Staging and End-Results Reporting, which is now the American Joint Committee on Cancer (AJCC). The most recent edition of the AJCC Cancer Staging Manual, the eighth edition, was globally adopted on January 1, 2018. Previous editions of the manual have relied on anatomic methods of staging alone, which used population-based survival data to predict clinical outcomes. In the era of precision medicine, the major change in the eighth edition is the incorporation of prognostic biomarkers to more accurately predict clinical outcomes and treatment response on an individual basis, without relying solely on the anatomic extent of disease. Factors such as tumor grade, hormone receptor and oncogene expression, and multigene panel recurrence scores are now integrated with anatomic information to yield a final prognostic stage group, which will provide better stratification of patient prognosis. The purpose of this article is to review the major changes in the AJCC eighth edition for breast cancer staging, review anatomic TNM staging, familiarize the radiologist with prognostic biomarkers and prognostic staging, and identify key sites of disease that may alter clinical management. © RSNA, 2018.

Abstract: Rationale and Objectives: We present an optical mammography study that aims to develop quantitative measures of pathologic response to neoadjuvant chemotherapy (NAC) in patients with breast cancer. Such quantitative measures are based on the concentrations of oxyhemoglobin ([HbO 2 ]), deoxyhemoglobin ([Hb]), total hemoglobin ([HbT]), and hemoglobin saturation (SO 2 ) in breast tissue at the tumor location and at sequential time points during chemotherapy., Materials and Methods: Continuous-wave, spectrally resolved optical mammography was performed in transmission and parallel-plate geometry on 10 patients before treatment initiation and at each NAC administration (mean number of optical mammography sessions: 12, range: 7-18). Data on two patients were discarded for technical reasons. The patients were categorized as responders (R, >50% decrease in tumor size), or nonresponders (NR, <50% decrease in tumor size) based on imaging and histopathology results., Results: At 50% completion of the NAC regimen (therapy midpoint), R (6/8) demonstrated significant decreases in SO 2 (-27% ± 4%) and [HbT] (-35 ± 4 µM) at the tumor location with respect to baseline values. By contrast, NR (2/8) showed nonsignificant changes in SO 2 and [HbT] at therapy midpoint. We introduce a cumulative response index as a quantitative measure of the individual patient's response to therapy. Abstract: We present a study of the bilateral symmetry of human breast hemoglobin saturation maps measured with a broadband optical mammography instrument. We have imaged 21 patients with unilateral breast cancer, 32 patients with unilateral benign lesions, and 27 healthy patients. An image registration process was applied to the bilateral hemoglobin saturation (SO 2 SO2 ) images by assigning each pixel to the low, middle, or high range of SO 2 SO2 values, where the thresholds for the categories were the 15th and 85th percentiles of the individual saturation range. The Dice coefficient, which is a measure of similarity, was calculated for each patient’s pair of right and left breast SO 2 SO2 images. The invasive cancer patients were found to have an average Dice coefficient value of 0.55±0.07 0.55±0.07 , which was significantly lower than the benign and healthy groups (0.61±0.11 0.61±0.11 and 0.62±0.12 0.62±0.12 , respectively). Although differences were seen in a group analysis, the healthy patient Dice coefficients spanned a wide range, limiting the diagnostic capabilities of this SO 2 SO2 symmetry analysis on an individual basis. Our results suggest that for assessing the SO 2 SO2 contrast of breast lesions, it may be better to select a reference tissue in the ipsilateral rather than the contralateral breast.

Abstract: Although numerous studies have outlined the benefits of contrast material-enhanced magnetic resonance (MR) imaging for the detection of breast parenchymal findings, the assessment of lesions of the skin and superficial tissue at breast MR imaging has largely been overlooked. Such lesions, both benign and malignant, are commonly encountered at breast MR imaging, and their detection and diagnosis are often imperative. These lesions include superficial, locally extensive, inflammatory, and recurrent breast cancers; iatrogenic changes; sebaceous cysts; and less prevalent diseases such as granulomatous mastitis and angiosarcomas, among others. As MR imaging continues to be used with increasing frequency in both the screening and diagnostic settings, these lesions will be encountered more often; consequently, a better understanding of which lesions may be ignored and which require further evaluation will become increasingly important. Frequently, MR imaging alone may not clearly indicate the cause of lesions of the breast skin and superficial tissue. However, radiologists should thoroughly assess such lesions, taking into consideration the spectrum of possible causes. The evaluated reorganization involved the adoption of an AC nurse service model supported by the use of information and communication technology for storing and handling follow-up data., Design: Statistical analysis of AC follow-up data, personnel task distribution and resource use analysis, and survey of personnel experiences., Setting: The Paloheinä health station in Helsinki, Finland., Subjects: All 122 Paloheinä patients on oral AC treatment and the related Paloheinä personnel. Also, follow-ups from two comparison health stations from Helsinki were used., Main Outcome Measures: Laboratory P-INR and follow-up density distributions before and after follow-up reorganization for Paloheinä and comparison station patients, task distribution, and personnel resource use in treatment guideline assignment before and after reorganization and gathered personnel experiences., Results: No statistically significant changes (Pearson's chi-square test, p < 0.05) were observed in P-INR distributions, considering also the comparison stations. Adoption of the AC nurse model resulted in a 66%/34% task distribution between nurses and physicians in guideline assignment. Personnel resource use analysis showed a cost decrease of 1.61 euros per follow-up in guideline assignment. Users who responded to the survey were generally satisfied with the reorganization and its subjective impacts., Conclusion: Reorganization resulted in a potential reduction in cost of care per follow-up without a change in the observed quality of care in the Paloheinä setting. Personnel generally accepted the reorganization.

Abstract: Objective: In this study we sought to develop a comparative cost evaluation between conventional and new media, e.g. web, mobile communication technology and digital television, and near patient testing supported anticoagulant (ac) treatment follow-up in a primary health care setting., Method: The comparison was done for two patient groups, self-care and home-care patients, on oral ac treatment in the primary health care centre of the rural and sparsely populated municipality of Ikaalinen. In practise case analysis was used to develop cost functions from collected economic data, which were analysed to determine the break-even point in total cost between conventional and new media supported follow-up for the two patient groups., Results: In the home-care setting the break-even point is 14 patients; in the self-care setting new media supported follow-up is always more cost-effective., Conclusion: The results illustrate that the use of new media and near patient testing in ac treatment follow-up brings about an economic benefit even with a small number of patients in the Ikaalinen setting. However, the sensitivity of break-even to perturbations in the individual costs of the used economic models remains high. Still, when the economic benefits are considered together with the clinical and practical benefits shown to result from self-testing, self-management and use of new media technologies the new service models can be said to provide noticeable benefits both in terms of quality of care and economics in our specific setting.

Abstract: Objective: To determine the feasibility of a more personal anticoagulant treatment follow-up routine involving the use of a personal measurement device along with the new media (e.g. the Internet)., Design: A telephone interview., Setting: The municipality of Ikaalinen with a total population of 7795., Subjects: Forty patients on anticoagulant treatment who have personal responsibility for their treatment follow-up., Main Outcome Measures: Demographic data, data and opinions on anticoagulant treatment and data and opinions on personal health care and the new media., Results: More than two-thirds of the interviewed were generally pleased with the current AC treatment routine in the Ikaalinen area. Sixty-eight percent of the patients feel that they are able to determine the anticoagulant dosage from the follow-up test results and almost a third have independently changed their dosage. Approximately half of the interviewed were willing to take various health-related measurements at home, but approximately only 20% of the interviewed were ready to use health-related services; for example, related to the follow-up of anticoagulant treatment, through the new media., Conclusion: Part of the population receiving anticoagulant treatment is ready to move to a more personal treatment routine involving home measurements in follow-up tests. However, most interviewed patients were not interested in using the new media as part of the new routine.

Abstract: The primary objective of this study is to evaluate a sample of health-related services available on the internet. In addition, types of new services and service qualities that may be required in the future were estimated. A registered nurse using a questionnaire containing items about usability, quality, and other characteristics related to the subject carried out an evaluation of 100 health-related internet sites, and the results were processed to determine average qualities and graphical presentations for the studied parameters. The services studied differed greatly from each other both in terms of content and quality. A general observation was that sites of larger organizations were, typically, both in terms of presentation and content, more sophisticated than sites of smaller producers. A typical characteristic was the superficiality of the information presented. In conclusion, the internet is an emerging medium for health-related information, and standards for the production and presentation of health content for the internet are still in development. However, even at this early stage, the internet represents a valuable source of information for those in need of health-related information for both health care professionals and consumers.

Abstract: The effect of blood pressure and heart rate reactivity on respective variability in everyday life conditions was studied in a group of middle-aged, normotensive men (n = 22, mean age 39.3 years, range 35-45 years). Continuous intra-arterial tape recording was used to measure 24-h blood pressure and heart rate and the subjects completed a 24-h behavioural diary. The variability of blood pressure and heart rate was analysed using 30-s averages and cumulative distribution curves. The overall blood pressure and heart rate variability was calculated as the difference between the 90 and 10% levels of the cumulative distribution curves. Reactivity caused by different daily activities was calculated as the change from baseline level, defined as the 10% level determined from the cumulative distribution curve of waking hours. All regular activities were monitored. Large interindividual differences were found in variability and reactivity. The mean reactivity to different activities varied from 3.3 to 44.7 mmHg for systolic blood pressure, from 1.8 to 16.3 mmHg for diastolic blood pressure and from 2.0 to 46.0 beats/min for heart rate. The mean contribution of reactivity to variability varied between 21 and 74% for systolic blood pressure, from 19 to 58% for diastolic blood pressure and from 20 to 82% for heart rate. As expected, blood pressure and heart rate levels were significantly higher at work than at home. We conclude that the reactivity caused by daily activities has a pronounced influence on blood pressure level and variability during waking hours.

Abstract: The long-term aim in the INFORM Project is to develop, evaluate and implement a new generation of Information Systems for hospital High Dependency Environments (HDE-Intensive Care Units, Neonatal Units, Burns Units. Operating and Recovery Rooms, and other specialised areas). The distinguishing feature of the HDE is the very large amount of data that is collected through monitors and paper records about the state of critically ill patients; this has made the role of the staff a technical one in addition to a caring one. The INFORM System will integrate Decision Support with on-line, off-line and observed patient data and, in addition, will incorporate and integrate unit management features. In the Exploratory Phase of the Project, functional requirements have been set out. These are based on four components: conceptual model of the HDE; evaluation of existing HDE Information Systems; development of a novel software architecture using a Knowledge-Based Systems (KBS) methodology, and based on a critical review of KBS applied to the HDE: monitoring of appropriate leading-edge technological developments. The conceptual model has two components: a patient-related information model, and a department-related cost model. The patient-related model is identifying key and difficult areas of decision making. A key aspect of INFORM is integration of clinical Decision Support for these areas into the Information System through a layered software architecture. The lower layers are concerned with monitoring and alarming and the higher levels with patient assessment and therapy planning. The functionality and interconnection of these layers are being determined.

50. Short-term variability of systolic blood pressure and heart rate in normotensive subjects.

Abstract: Short-term fluctuations in systolic blood pressure (SBP) and heart rate (HR) and their inter-relationship were analysed in a group of normotensive middle-aged men (n = 16) using a multivariate autoregressive modelling technique. This study is the first to evaluate the beat-to-beat variability of SBP and HR in a group of real normotensive subjects. Direct intra-arterial blood pressure was registered together with ECG using an ambulatory tape recording technique (the Oxford method). Power spectrum density estimated (PSD) were used as a measure of the variability. PSDs were calculated over 3-min periods for four basic physiological conditions: during sleep and in the supine, sitting and standing positions. The inter-relationship between the blood pressure and heart rate variabilities was analysed using a closed-loop model. In agreement with results presented earlier in the literature, the beat-to-beat variation in SBP and HR was concentrated in three typical power spectrum regions: the high-frequency (HF = 0.15-0.35 Hz) region (respiration), the mid-frequency (MF = 0.075-0.15 Hz) region (vasomotor oscillation) and the low-frequency (LF = 0.02-0.075 Hz) region (thermoregulation). The variability changes considerably between different situations, especially that of the MF region. The variability was most prominent in the MF region and in the standing position. The variability was generally smallest in the HF region and in sleep. The results also demonstrate that the beat-to-beat variability in SBP and HR can considerably affect one another.
