Background Peri-appendiceal inflammation (PAI) is often found in patients with left-sided ulcerative colitis (UC) or proctitis. However, there is still no consensus on the clinical significance of PAI. This study aimed to identify the clinical significance of PAI in UC as a prognosis factor. Methods We retrospectively collected medical records and endoscopic results of patients with diagnosed left side UC or proctitis between January 2013 and December 2021. According to the presence of PAI, we divided the patients into PAI and non-PAI groups. Demographic features and clinical course of the two groups were collected and analyzed. Results A total of 107 patients (41 of left site UC, 56 of proctitis) were enrolled, of which 52 were in the PAI group and 55 were in the non-PAI group There was no difference in most of the demographic and clinical variables between the PAI and non-PAI groups. During the follow-up period of 51 months for the PAI group and 46 months for the non-PAI group, respectively, there was no difference in disease relapse rate (47.1% vs 49.1%, p=0.823), cumulative rate of disease relapse at 1, 3, and 5 years (10.7%, 26.2%, 34.8% vs 8.39%, 23.0%, 36.7%, p=0.823), rate of treatment escalation (59.6% vs 41.8%, p=0.066) and proximal disease extension (25% vs 25.5%, p=0.957) however, new occurrence of PAI during the follow-up period in the non-PAI group was associated with proximal disease extension (55.6% vs 19.6%, p=0.037) In this study, oral 5-aminosalicylate (ASA) was administered to most patients regardless of the extent of the disease. In patients with proctitis with PAI, oral 5-ASA did not affect relapse rate (37.1% vs 66.7%, p=0.550) and proximal disease extension rate (28.6% vs 33.3%, p=1.000). Conclusion PAI at diagnosis did not affect the course of the disease nor did oral 5-ASA in proctitis, however new occurrences of PAI during follow-up were associated with proximal disease extension.

Multisystem inflammatory syndrome in children (MIS-C) can occur in association with coronavirus disease 2019 (COVID-19). It is not easy to differentiate MIS-C from severe COVID-19 or Kawasaki disease based on symptoms. The aim of this study was to describe the clinical and laboratory characteristics of MIS-C.We searched PubMed/Medline for case series and reports of MIS-C published until June 20, 2020. From a total of nine articles involving 45 cases, various clinical and laboratory data were extracted. Each target case was evaluated by using different diagnostic criteria.The average age at onset of MIS-C was 8.6 years. In 80% of cases, the age of patients ranged from 5 to 15 years. Fever (100%) and shock (82%) were the most common presenting symptoms. Sixty percent of cases met the diagnostic criteria for typical or atypical Kawasaki disease. Biomarkers indicative of inflammation, coagulopathy, or cardiac injury were characteristically elevated as follows: ferritin (mean: 1,061 ng/mL), CRP (217 mg/L), ESR (69 mm/hr), IL-6 (214.8 pg/mL), TNFα (63.4 pg/mL), D-dimer (3,220 ng/mL), PT (15.5 s), troponin I (1,006 ng/L), and BNP (12,150 pg/mL). Intravenous immunoglobulin was administered in all target cases, and inotropic agents were commonly used as well. No case of death was observed.This study demonstrated that MIS-C is a serious condition that presents with fever, rash, as well as cardiovascular and gastrointestinal symptoms. Although it is challenging to differentiate MIS-C from Kawasaki disease or severe COVID-19, initiation of appropriate treatments through early diagnosis is warranted.

We aim to generate a catalogue of merging galaxies within the 5.4 sq. deg. North Ecliptic Pole over the redshift range $0.0 < z < 0.3$. To do this, imaging data from the Hyper Suprime-Cam are used along with morphological parameters derived from these same data. The catalogue was generated using a hybrid approach. Two neural networks were trained to perform binary merger non-merger classifications: one for galaxies with $z < 0.15$ and another for $0.15 \leq z < 0.30$. Each network used the image and morphological parameters of a galaxy as input. The galaxies that were identified as merger candidates by the network were then visually checked by experts. The resulting mergers will be used to calculate the merger fraction as a function of redshift and compared with literature results. We found that 86.3% of galaxy mergers at $z < 0.15$ and 79.0% of mergers at $0.15 \leq z < 0.30$ are expected to be correctly identified by the networks. Of the 34 264 galaxies classified by the neural networks, 10 195 were found to be merger candidates. Of these, 2109 were visually identified to be merging galaxies. We find that the merger fraction increases with redshift, consistent with literature results from observations and simulations, and that there is a mild star-formation rate enhancement in the merger population of a factor of $1.102 \pm 0.084$., Accepted to A&A, 26 pages, 20 figures, 8 tables, 3 appendixes, full tables 1 and 4 will be available on CDS

Disinfection of shared spaces has become essential to minimize the spread of various diseases. An efficient disinfection device that can simultaneously inactivate airborne bacteria and surface-adhered bacteria in an enclosed space is required.An air-passable plasma filter (APF) was developed and applied to a chamber model to evaluate the efficacy of zone disinfection.The 60 L chamber consisted of a nebulizer, circulation fans, temperature and humidity monitors, an air-sampling port with a sealed gate, airborne bacteria-trapping media, and a built-in fan for evaluation. After spraying each bacterial strain (Escherichia coli, Staphylococcus epidermidis, and Mycobacterium smegmatis) as a bioaerosol, airborne and surface-attached bacteria were quantified simultaneously to evaluate the zone-disinfection effect of APF.The operation of APF in the 60 L chamber showed a complete zone-disinfection effect for E. coli (10 min), S. epidermidis (10 min), and M. smegmatis (60 min) present in the air and on the walls at various locations. The time required to completely disinfect each of the airborne bacteria and surface-attached bacteria within the same space was different.APF has the potential to exhibit significant germicidal effects on various micro-organisms and can be an effective alternative for disinfection of enclosed spaces.

Independent component analysis (ICA) of multi-subject functional magnetic resonance imaging (fMRI) data has proven useful in providing a fully multivariate summary that can be used for multiple purposes. ICA can identify patterns that can discriminate between healthy controls (HC) and patients with various mental disorders such as schizophrenia (Sz). Temporal functional network connectivity (tFNC) obtained from ICA can effectively explain the interactions between brain networks. On the other hand, dictionary learning (DL) enables the discovery of hidden information in data using learnable basis signals through the use of sparsity. In this paper, we present a new method that leverages ICA and DL for the identification of directly interpretable patterns to discriminate between the HC and Sz groups. We use multi-subject resting-state fMRI data from $358$ subjects and form subject-specific tFNC feature vectors from ICA results. Then, we learn sparse representations of the tFNCs and introduce a new set of sparse features as well as new interpretable patterns from the learned atoms. Our experimental results show that the new representation not only leads to effective classification between HC and Sz groups using sparse features, but can also identify new interpretable patterns from the learned atoms that can help understand the complexities of mental diseases such as schizophrenia.

Hydrocarbon explosion and fire are typical accidents in the offshore oil and gas industry, sometimes with catastrophic consequences such as casualties, property damage and pollution. Successful engineering and design should meet both functional requirements associated with operability in normal conditions and health, safety, environmental and ergonomics (HSE&E) requirements associated with accidental and extreme conditions. A risk-based approach is best for successful design and engineering to meet HSE&E requirements. This study aimed to develop an advanced procedure for assessing the quantitative risk of offshore installations in explosions. Unlike existing industry practices based on prescriptive rules or qualitative approaches, the proposed procedure uses an entirely probabilistic approach. The procedure starts with probabilistic selection of accident scenarios. As the defining components of risk, both the frequency and consequences associated with selected accident scenarios are computed using the most refined technologies. Probabilistic technology is then applied to establish the relationship between the probability of exceedance and the physical values of the accident. Acceptance risk criteria can be applied to define the nominal values of design and/or level of risk. To validate and demonstrate the applicability of the proposed procedure, an example of its application to topside structures of an FPSO unit subjected to hydrocarbon explosions is detailed. The conclusions and insights obtained are documented.

BackgroundForeign bodies in the ear, nose and throat commonly necessitate emergency department visits.MethodThis retrospective study was conducted on emergency department visits from January 2010 to December 2019 to determine characteristics and clinical prognoses of ENT patients. Patients were divided into three groups according to foreign-body entry route; patient characteristics and clinical findings were compared between groups.ResultsOf 676 142 emergency department visits, 10 454 were because of ENT-related foreign bodies. The mean (± standard deviation) age of subjects was 24.0 (± 23.4) years, and 5176 patients were male (49.5 per cent). The most common entry route was the mouth (74.5 per cent). Most patients (97.1 per cent) were discharged after emergency treatment. Intensive care and in-hospital mortality occurred only in the mouth group.ConclusionClinical findings differ depending on foreign-body entry route. After emergency treatment, most patients were discharged; some cases presented serious complications.

This study was purposed to evaluate release of nickel and chromium ions and methyl methacrylate (MMA) monomers from functional appliances and their possible health effects.Study design: Twin-block appliances and Bionators were immersed in artificial saliva and kept in a thermal incubator. Control group was established as artificial saliva without appliances. Artificial saliva was analysed after 7 days, 30 days and 90 days. Inductively coupled plasma mass spectrometry and gas chromatography-mass spectrometry was used for detection of nickel and chromium ions and MMA monomers. MTT assays and cytokine array were performed.One way ANOVA with Tukey test and Dunnett's T3 for post-hoc analysis was used for evaluation of time-dependent changes and independent t-test was used for evaluation of MTT assay results.The results revealed that metal ions and MMA monomers are released from the appliances. Metal ion detection pattern was irregular and could not be analysed. Twin-block group showed significantly larger amount of MMA release. MTT assay revealed statistically significant but minimally reduced cellular activity on Bionator and twin-block groups compared to control groups. Cytokine array showed no or less inflammatory cytokine release on Bionator and twin-block groups.MMA monomer release was confirmed but the cytotoxic effect of functional appliance material release is minimal or negligible. General toxicity of the functional appliance from the MMA monomer release is likely to be minimal or negligible.

Metastatic breast cancer is an extremely complex disease with limited treatment options due to the lack of information about the major characteristics of metastatic disease. There is an urgent need, therefore, to understand the changes in cellular complexity and dynamics that occur during metastatic progression. In the current study, we analyzed the cellular and molecular differences between primary tumors and paired lung metastases using a syngeneic p53-null mammary tumor model of basal-like breast cancer. Distinct subpopulations driven by the Wnt- and/or STAT3 signaling pathways were detected in vivo using a lentiviral Wnt- and STAT3 signaling reporter system. A significant increase in the overlapping populations driven by both the Wnt- and STAT3 signaling pathways was observed in the lung metastases as compared to the primary tumors. Furthermore, the overlapping populations showed a higher metastatic potential relative to the other populations and pharmacological inhibition of both signaling pathways was shown to markedly reduce the metastatic lesions in established lung metastases. An analysis of the unique molecular features of the lung metastases revealed a significant association with immune response signatures. Specifically, Foxp3 gene expression was markedly increased and elevated levels of Foxp3 + Treg cells were detected in close proximity to lung metastases. Collectively, these studies illustrate the importance of analyzing intratumoral heterogeneity, changes in population dynamics, and the immune microenvironment during metastatic progression.

scFv-BM3 is a single-chain variable fragment (scFv) against aflatoxin B1 (AFB1 ) engineered by affinity maturation and site-directed mutagenesis, and thus has a 31-fold higher affinity than its wild-type. To apply scFv-BM3 to immunological detection of AFB1 , periplasmic expression in Escherichia coli was attempted to produce a functional form of scFv-BM3. scFv-BM3 accumulated as inactive aggregates in the cells. However, it was found that scFv-BM3 secreted into the culture medium had binding activity to AFB1 . Expression conditions for scFv-BM3 were further manipulated to enhance secretion into the culture medium. This extracellular secretion of functional scFv-BM3 was significantly improved by supplementation with Triton X-100 and optimization of expression conditions. The scFv-BM3 purified from the culture medium exhibited a typical antiparallel β-sheet structure and adopted a proper conformation to bind AFB1 with high affinity and specificity in various biophysical and biochemical analyses. SIGNIFICANCE AND IMPACT OF THE STUDY: Single-chain variable fragments (scFvs) are recombinant antibodies that are difficult to produce as a functional form in Escherichia coli. This study demonstrates the production of functional scFvs against aflatoxin B1 (AFB1 ) (scFv-BM3) using Escherichia coli by extracellular secretion. While periplasmic expression of scFv-BM3 resulted in formation of inactive aggregates in E. coli, the scFv-BM3 secreted into the culture medium adopted a properly folded structure for specific binding to AFB1 . This study promotes the application of functional scFv-BM3 to the immunological detection of AFB1 in biotechnology fields.

Atomic layer etching (ALE) typically proceeds through four sequential steps of surface modification, purging, removal of the modified surface, and a second purging. This serial process is repeated to achieve atomic-scale precision etching by removing material layer by layer. However, it is is challenging for ALE to play a bigger role in semiconductor fabrication due to its low productivity. Among various obstacles, the time-consuming purging steps between the surface modification and removal steps of the ALE cycle have been a major hurdle hindering the ALE process. In this work, we experimentally demonstrate a purgeless SiO2 ALE methodology in which the surface modification and removal steps are controlled solely by pulsed C4F8 injection into continuous Ar plasma. The working principle of this simple approach is based on the conventional fluorocarbon (FC) plasma SiO2 etching mechanism, where the SiO2 etch rate decreases to zero when the thickness of an FC film on the SiO2 is above a certain level. Here, a thick FC film is considered to act as a protective layer against residual FC radicals in the surface removal step, allowing the purging step between the surface modification and removal steps to be omitted. The proposed approach is expected to facilitate the improvement of ALE equipment costs and potentially lead to wider employment of ALE technology in semiconductor manufacturing.

Introduction Direct gaze is the most important mediator of social interaction and communication. Existing studies have evaluated eye movements of patients with schizophrenia by presenting stimuli using photographs or pre-recorded videos, but few directly investigated gaze avoidance in real-world situations. Objectives To investigate the correlation between gaze avoidance and psychopathology in patients with schizophrenia through eye movement measurements in real-life interpersonal situations. Methods We enrolled 52 clinically stable patients with schizophrenia. Psychopathology was evaluated using the Positive and Negative Syndrome Scale (PANSS), Hamilton Depression Rating Scale, and Hamilton Anxiety Rating Scale. After presenting a visual stimulus, eye movements were measured with Tobii Pro Wearable Glasses 2, and deep learning-based emotional recognition using the residual masking network was used for neutral stimulus verification. Statistical analyses were performed using Pearson’s correlation and regression analyses. Results Data of 45 participants with verified stimulus neutrality by deep learning image recognition were used for analysis. The first dwelling time was negatively correlated with the PANSS positive syndrome subscale (p=0.028), general psychopathology subscale (p=0.008), total score (p=0.008), 5-factor positive symptoms (p=0.035), and 5-factor depression/anxiety symptoms (p=0.008). The baseline-area of interest (AOI) pupil diameter change was positively correlated with PANSS 5-factor positive symptom scores (p=0.039). After adjusting for additional variables, the same items had a significant effect on the first dwelling time and baseline-AOI pupil diameter change. Conclusions Psychopathology, particularly positive symptoms, was associated with gaze avoidance and pupil diameter in patients with schizophrenia. Evaluating the characteristics of eye movements in patients with schizophrenia will enable better understanding of their symptoms. Disclosure No significant relationships.
