This paper describes a major update to the quasi-global, higher-frequency sea-level dataset known as GESLA (Global Extreme Sea Level Analysis). Versions 1 (released 2009) and 2 (released 2016) of the dataset have been used in many published studies, across a wide range of oceanographic and coastal engineering-related investigations concerned with evaluating tides, storm surges, extreme sea levels, and other related processes. The third version of the dataset (released 2021), presented here, contains double the number of years of data, and nearly four times the number of records, compared to Version 2. The dataset consists of records obtained from multiple sources around the world. This paper describes the assembly of the dataset, its processing, and its format, and outlines potential future improvements.

Warnings issued by meteorological or oceanographic agencies are a common means of allowing people to prepare for likely impactful events. Quantifying the relationships between ocean conditions and coastal impacts, such as shoreline change or flooding of coastal assets (e.g. flooded access points, overtopping of sea walls) is crucial for developing operational coastal hazard warnings. Existing studies have largely omitted empirical data, relying on modelling to estimate total water levels and impact potentials. It is well documented that site-specific conditions influence local morphodynamics and as such, detailed data related to the physical environment is a necessary component of these existing approaches. The capacity to collect these data is not always available, however, and so an alternative approach that does no rely on detailed modelling may be necessary in some instances to identify the conditions that lead to coastal impacts. We propose an alternative empirically based approach for isolating oceanic conditions that are conducive to impact along open coasts, using two case studies from Victoria, southeast Australia: Port Fairy and Inverloch. Oceanic conditions were defined using data obtained from a WAVEWATCH III (WW3) model hindcast, assessed against newly installed wave buoys, which evidenced variation in mean conditions between the two sites. We coupled impact-based data derived from citizen-science and social media to modelled and observational data, to identify the oceanic conditions that led to impacts. We found heterogeneity in the response of the case study locations to deviations from the local mean wave characteristics and still water levels. This paper demonstrates an approach through which impact-based thresholds for erosion could be developed for management applications and early warning systems.

Sea-level rise is causing coastal inundation events in estuaries, harbours, bays, and tidal rivers to happen more often as predictable daily high and low tides reach higher levels. This can lead to coastal inundation happening under benign weather conditions, as flood thresholds are exceeded due to tides alone without the influence of storm surges or other phenomena. As such, changes in frequency of this 'tide-only' inundation may be a useful metric to quantify the role that sea-level rise plays in modulating the risk of coastal inundation from high still water levels. Here we present a conceptual model for 'tide-only' inundation and propose a practical methodology to formulate tide-only inundation statistics: estimates, historical trends, and future projections. This enables this emerging natural hazard to be fully incorporated into new and existing coastal risk assessment frameworks and considered in coastal management and planning strategies at local and national levels. Further, it leads to a framework that can quantify the role that tides play in coastal flooding as sea levels rise.

As global mean sea level continues to rise, thresholds corresponding to coastal inundation impacts are exceeded more frequently. This paper aims to relate sea level rise (SLR) observations and projections to their physical on‐the‐ground impacts. Using a large coastal city as an example, we show that in Sydney, Australia, frequencies of minor coastal inundation have increased from 1.6 to 7.8 days per year between 1914 and present day. We attribute over 80% of the observed coastal inundation events between 1970 and 2015 to the predominantly anthropogenic increases in global mean sea level. Further, we find that impact‐producing coastal inundation will occur weekly by 2050 under high‐ and medium‐emission/SLR scenarios and daily by 2100 under high emissions. The proportion of tide‐only coastal inundation events (i.e., where no storm surge is required to exceed flood thresholds) will increase with SLR, such that most coastal inundation events, including those considered historically severe, will become a predictable consequence of SLR and astronomical tides. These findings are important for coastal managers as frequency, severity, and predictability of inundation impacts can all now be related to the amount of SLR (e.g., a planning allowance or SLR projection). By incorporating known historical inundation events, this allows contextualization, visualization, and localization of global SLR and the changing nature of future coastal inundation risk.

This study presents the first assessment of the observed frequency of the impacts of high sea levels at locations along Australia’s northern coastline. We used a new methodology to systematically define impact-based thresholds for coastal tide gauges, utilising reports of coastal inundation from diverse sources. This method permitted a holistic consideration of impact-producing relative sea-level extremes without attributing physical causes. Impact-based thresholds may also provide a basis for the development of meaningful coastal flood warnings, forecasts and monitoring in the future. These services will become increasingly important as sea-level rise continues.The frequency of high sea-level events leading to coastal flooding increased at all 21 locations where impact-based thresholds were defined. Although we did not undertake a formal attribution, this increase was consistent with the well-documented rise in global sea levels. Notably, tide gauges from the south coast of Queensland showed that frequent coastal inundation was already occurring. At Brisbane and the Sunshine Coast, impact-based thresholds were being exceeded on average 21.6 and 24.3 h per year respectively. In the case of Brisbane, the number of hours of inundation annually has increased fourfold since 1977.

Summary Toxic epidermal necrolysis (TEN) is a life-threatening, immune-mediated reaction, characterized by severe cutaneous and mucosal blisters and erosions. It often presents with flu-like symptoms, followed by a maculopapular, urticarial, purpuric or erythema multiforme-like eruption, which then evolves into blisters and sheet-like erosions. Presentation with pustules, however, is not well described in the English literature, and may lead to delayed diagnosis. We present two unusual cases of TEN that initially presented with pustular lesions.

This paper presents the problem definition for the second generation of benchmark structural control problems for cable-stayed bridges. The goal of this study is to provide a testbed for the development of strategies for the control of cable stayed-bridges. Based on detailed drawings of the Bill Emerson Memorial Bridge, a three-dimensional evaluation model has been developed to represent the complex behavior of the full-scale benchmark bridge. Phase II considers more complex structural behavior than phase I, including multi-support and transverse excitations. Evaluation criteria are presented for the design problem that are consistent with the goals of seismic response control of a cable-stayed bridge. Control constraints are also provided to ensure that the benchmark results are representative of a control implementation on the physical structure. Each participant in this benchmark bridge control study is given the task of denning, evaluating and reporting on their proposed control strategies. Participants should also evaluate the robust stability and performance of their resulting designs through simulation with an evaluation model which includes additional mass due to snow loads. A case of extensive periungual viral warts in an immunocompetent patient that failed to respond to standard treatments over a 10-year period is reported. A rapid and complete response to treatment was achieved with oral acitretin.

Lasers are frequently used to treat pigmented skin lesions; however, there is little published data from routine clinical practice. We performed a retrospective review of patients treated between January 2003 and January 2004 to describe the patients, methods used and the clinical outcome. The long-term result was determined by a patient questionnaire. In total, 67 patients with 76 pigmented lesions were treated (22 males and 45 females, mean age 22 years); 27% had Fitzpatrick skin types IV-VI. The Q-switched Nd:YAG, and erbium:YAG lasers were used in 75% and 24% of cases, respectively, with an average of 5.4 treatments/patient. The physician-rated outcome at the end of treatment was 'very positive' in 45% and 'mostly positive' in 30% of cases. The patient-rated outcome at an average of 21 months post-treatment was 'excellent' in 29.5%, 'good' in 25%, and 'fair' in 22.7% of the 44 cases. About 50% of patients with the three most common lesions (congenital naevi, other/unspecified naevi and naevus of Ota) achieved good or excellent long-term results.

Play is children's work: “… play, games and cognitive development are functionally related” (Sutton-Smith 1971, 258). Children eveywhere, in every time period, from every culture and economic class enjoy playing with toys. Toys are learning tools; whether handmade or commercially produced, they help children prepare for their adult lives. Children's toys are frequently small-scale representations of objects from the adult world.

Angiogenesis is the growth of new vessels from existing vessels. It is important in the physiological processes of wound healing, embryogenesis, and the female menstrual cycle and involved in pathologies such as diabetic retinopathy and rheumatoid arthritis (1). There is now abundant evidence that tumors are angiogenesis dependent. Unless tumors can stimulate angiogenesis, and generate their own blood supply, they fail to grow larger than 2-3 mm(3). The angiogenic status of a tumor can be assessed directly using immunohistochem istry on pathology sections (see Chapter 13 by Kilic and Ergün in this volume and Chapter 7 by Ranieri and Gasparini and Chapter 8 by Turner and Harris in the companion volume) to count the number of blood vessels within a given area (microvessel density [MVD]). MVD is a prognostic indicator in a number of tumors including breast, bladder, and prostate (2). Angiogenic capacity can also be measured by assaying the various stimulatory and inhibitory factors that regulate angiogenesis. This can be done either by determining the mRNA level using ribonuclease protection (see Chapter 16 by Jones et al. in the companion volume) or the protein by either Western blotting (see Chapter 11 by Blancher and Jones in the companion volume) or with one of the commercially available enzyme-linked immunosorbent assay (ELISA) kits. Again, levels of these factors have been shown to be prognostic in a number of tumors.

Many agricultural studies have identified that wheat yield is sensitive to seasonal rainfall and extreme high temperatures. We investigate the impact of extreme heat events, in particular on wheat yields in South-East Australia (SEA) and South-West Western Australia (SWWA).We define a 'heat-day' as a day where the daily maximum temperature exceeds the 1911–2013 90th percentile for the respective calendar month. We find that the number of heat-days has experienced statistically significant increases across most months across much of Australia, particularly in South Australia, Western Australia, the Northern Territory and Tasmania. The trends are especially marked in winter, including in key wheat-growing regions. The temperatures recorded on these hottest days have also shown a statistically significant increase over the last 100 years.We find that, while wheat yields are more strongly correlated with rainfall than with the number of heat-days, there is substantial evidence to suggest that during drought conditions wheat yields are sensitive to the number of heat-days recorded in August and September in SEA and September and October in SWWA. Extreme heat and rainfall have a stronger association with below-average yields than above-average yields.Extreme temperatures and rainfall in these regions are related to major Australian climate drivers which form the basis of seasonal prediction models and are important for natural variability and long-term climate change. Here we assess the degree to which wheat yields in both regions can be related to the El Niño Southern Oscillation (ENSO), Indian Ocean Dipole (IOD) and Southern Annular Mode (SAM). We find that positive IOD events and El Niño events are both associated with reductions in wheat yields in SEA, but that the co-incidence of these events have no additional wheat yield reductions than would be expected if either a positive IOD or El Niño event occurs. The average annual wheat yield loss associated with El Niño state and/or positive IOD state in SEA is estimated to around sixteen to twenty one per cent.This paper provides insights into the historical relationships between wheat yields, extreme heat and climatic modes of variability in Australia, and discusses the possibilities for changes in wheat yields under a future climate change scenario.

Progestogens are used clinically for contraception, to control excessive menstrual bleeding and to oppose oestrogen in hormone replacement therapy. The use of intrauterine levonorgestrel (LNG) is however, associated with endometrial atrophy and decidualisation of the stroma. In this study, we aimed to identify genes whose expression is modulated by LNG either alone or in combination with progesterone. Thus endometrial stromal cells were stimulated with progesterone, LNG or LNG and progesterone. Poly-A RNA was isolated and used to probe expression arrays. The expression of a number of genes was altered on exposure to LNG or LNG and progesterone. Alteration of expression patterns was confirmed using semi-quantitative RT-PCR and western blot analysis. In particular, the protease activated receptor-1 (PAR-1) gene that encodes a receptor for thrombin was down regulated. This is the first demonstration that PAR-1 is down regulated by the progestogen LNG in human endometrium. Alteration in the expression levels of this receptor may affect both growth and haemostatic activity within the endometrium and may account for the observed morphological effects seen in users of intrauterine LNG delivery devices.

This paper describes work in progress in the gViz project. We suggest four areas in which XML should be applicable: data representation, visualization presentation, visualization application description, and audit trail for project history. We present initial thoughts on these areas.

Purpose To establish the efficacy and safety of botulinum toxin in the treatment of Crocodile Tear Syndrome and record any possible complications. Methods Four patients with unilateral aberrant VII cranial nerve regeneration following an episode of facial paralysis consented to be included in this study after a comprehensive explanation of the procedure and possible complications was given. On average, an injection of 20 units of botulinum toxin type A (Dysport®) was given to the affected lacrimal gland. The effect was assessed with a Schirmer’s test during taste stimulation. Careful recording of the duration of the effect and the presence of any local or systemic complications was made. Results All patients reported a partial or complete disappearance of the reflex hyperlacrimation following treatment. Schirmer’s tests during taste stimulation documented a significant decrease in tear secretion. The onset of effect of the botulinum toxin was typically 24–48 h after the initial injection and lasted 4–5 months. One patient had a mild increase in his preexisting upper lid ptosis, but no other local or systemic side effects were experienced. Conclusions The injection of botulinum toxin type A into the affected lacrimal glands of patients with gusto-lacrimal reflex is a simple, effective and safe treatment.

Uterine leiomyomas are the most prevalent benign tumor type in women of reproductive age and are one of the most common indications for hysterectomy. The expression of five angiogenic factors, adrenomedullin (ADM), vascular endothelial growth factor (VEGF), acidic fibroblast growth factor, basic fibroblast growth factor, and platelet-derived endothelial cell growth factor/thymidine phosphorylase, were examined in 91 uteri collected throughout the menstrual cycle; 52 of which contained leiomyomata, and the remainder were normal controls. The microvascular density and endothelial proliferative indices were then determined for each of the uterine sections. ADM and VEGF were the most widely expressed angiogenic factors in the leiomyomas. Furthermore, the expression of ADM and VEGF in the endometrium and myometrium was up-regulated in leiomyoma-bearing uteri compared with controls. Although acidic fibroblast growth factor and basic fibroblast growth factor were expressed in leiomyomas and endometrium in all of the uterine samples examined, they were only expressed in the myometrium of leiomyomata-bearing uteri. Endothelial proliferation in leiomyomas was statistically greater than that of the myometrium and endometrium, both within and between uteri (P0.05). The vascular density in the myometrium but not the endometrium was significantly increased in leiomyoma-containing uteri (P0.05). Expression of ADM alone correlated directly with vascular density and endothelial cell proliferation index in leiomyomas and myometrium and may account for the high vascularity found in leiomyomas and the myometrium of leiomyoma-bearing uteri. As such, ADM is identified as a novel target for antiangiogenic therapy of these benign, clinically problematic uterine tumors.

To investigate whether adults with infantile strabismus but without latent nystagmus have abnormalities of horizontal motion detection.Eleven adult subjects with infantile esotropia but without latent nystagmus and 15 control subjects were required to detect the onset of motion and drift direction of a sinusoidal, spatial frequency grating that moved with linearly increasing velocity. The grating was presented monocularly in paracentral vision at an eccentricity of 16.5 degrees with a field size of 18 degrees. The contrast of the grating was just above contrast threshold for visibility.The mean velocity threshold for detection of motion was raised significantly in the patient group compared with the control group. Nine of the 11 subjects with infantile esotropia demonstrated directional asymmetry for the detection of motion. Thresholds were elevated more often when the grating was moving nasally in the squinting eye and temporally in the nonsquinting eye, and raised thresholds were more prevalent in the squinting eye.The findings indicate that in infantile esotropia, the presence of motion perception deficits are not always associated with the development of latent nystagmus. The predominance of nasally directed motion deficits in the squinting eye and temporally directed motion deficits in the nonsquinting eye was unexpected and may have been caused by abnormal development of cortical motion processing.

Ophthalmic artery perfusion pressures and retinal arteriolar calibres were studied in 10 human subjects when sitting, standing, and lying. Differing responses in perfusion pressure were found; autoregulatory responses were found in 13 of the 18 significant calibre changes on moving from sitting to standing and 13 of 22 significant calibre changes on moving from standing to lying. The remaining significant calibre changes were non-autoregulatory, the arterioles dilating with perfusion pressure increase and constricting with decrease.

A reduced-order version of the one-on-one aerial combat problem is studied as a pursuit-evasion differential game. The coplanar motion takes place at given speeds and given maximum available turn rates, and is described by three state equations which are equivalent to the range, bearing, and heading of one aircraft relative to the other. The purpose of the study is to determine those relative geometries from which either aircraft can be guaranteed a win, regardless of the maneuver strategies of the other. Termination is specified by the tail-chase geometry, at which time the roles of pursuer and evader are known. The roles are found in general, together with the associated optimal turn maneuvers, by solution of the differential game of kind. For the numerical parameters chosen, neither aircraft can win from the majority of possible initial conditions if the other turns optimally in certain critical geometries.

On receipt of warning, the air mobile aircraft-missile system performs a high acceleration takeoff followed by a supersonic dash to a "safe" distance from the launch site. At that time a subsonic long endurance mode is entered. The study objectives were to 1) determine technological aircraft and boost trajectory requirements, 2) provide initial definition of the aircraft and boost trajectory requirements, and 3) provide partial cost estimates for a fleet of aircraft which provides 200 missile on airborne alert. Three aircraft boost propulsion systems were studied: an unstaged cryogenic rocket, an unstaged storable liquid, and a solid rocket staged system. Aircraftgross weight was minimized for the combined dash and endurance mission profile. Using existing technologies, the resulting vehicles could accomplish the mission for gross weights of approximately one million Ib.

THERE is an ever-present demand for improvements in electric equipment. To improve the equipment, the engineer may try many new ideas. Some of these will yield results while others will be discarded as not being worth while. On equipment such as mercury-arc rectifiers many ideas have been successful and the improvements have been frequent and substantial. Usually, the engineer has so many new ideas to try that he does not take time to re-examine old ideas in the light of recent developments.

Light microscopy and electron microscopy were used to define morphological changes occurring in human corneal epithelium during surgery. No correlation was established between preoperative medication or peroperative fluids and corneal epithelial clouding. It is suggested that the changes we observed in relation to epithelial clouding were the direct result of disturbance to endothelial pump functions.
