BackgroundTocilizumab (TCZ) has shown to be effective for large vessel vasculitis including giant cell arteritis (GCA) and Takayasu arteritis (TAK) (1-5). However, LVV-GCA and TAK show different demographic and clinical features that may influence on TCZ therapeutic response.ObjectivesTo compare the effectiveness of TCZ in patients with LVV-GCA and patients with TAK.MethodsObservational multicenter study of patients with LVV-GCA and TAK who received TCZ. Outcome variables were: a) proportion of patients who achieved complete clinical improvement along with normalization of laboratory markers (CRP ≤0.5mg/dL and/or ESR ≤ 20 mm/1st hour) at 12 months b) complete improvement in imaging techniques. A comparative study between patients with LVV-GCA and TAK was performed.ResultsWe evaluated 70 LVV-GCA and 57 TAK patients who received TCZ. Main clinical and demographic characteristic are described in Table 1. Patients with TAK were younger, had longer disease duration, had received more commonly previous biologic therapy and were receiving higher doses of prednisone at baseline. Background:Tocilizumab (TCZ) has shown to be effective for large vessel vasculitis including Takayasu arteritis (TAK) (1-3). Most evidence in TAK comes from Asian patients. However, white patients seem to have different clinical and prognostic features.Objectives:Our aims were to: a) assess the efficacy and safety of TCZ in white patients with refractory TAK, b) determine if clinical improvement correlates with imaging outcomes, c) compare TCZ in monotherapy (TCZMONO) vs combined with conventional immunosuppressive drugs (TCZCOMBO)Methods:Multicenter study of white patients with refractory TAK who received TCZ.Outcomes variables were remission, glucocorticoid-sparing effect, improvement in imaging techniques, and adverse events. A comparative study between patients who received TCZMONO and TCZCOMBO was performed.Results:54 patients (46 women/8 men; median age 42.0 [32.5-50.5] years). TCZ was started after 12.0 [3.0-31.5] months since TAK diagnosis. Remission was achieved in 12/54 (22.2%), 19/49 (38.8%), 23/44 (52.3%) and 27/36 (75%) at 1, 3, 6 and 12 months, respectively. Prednisone dose was reduced from 30.0 [12.5-50.0] to 5.0 [0.0-5.6] mg/day at 12 months (Table 1). 10 (26.3%) of the 38 patients in whom an imaging follow-up test was performed showed no radiographic improvement after a median of 9.0 [6.0-14.0] months. 4 of them were in clinical remission.23 (42.6%) patients were on TCZMONO and 31 (57.4%) on TCZCOMBO: MTX (n=28), cyclosporine A (n=2), azathioprine (n=1). Patients on TCZCOMBO were younger (38.0 [27.0-46.0] vs 45 [38.0-57.0] years; p= 0.048), with a trend to longer TAK duration (21.0 [6.0-38.0] vs 6.0 [1.0-23.0] months; p= 0.08) and higher C-reactive protein (2.4 [0.7-5.6] vs 1.3 [0.3-3.3] mg/dL; p=0.16). Despite these differences, similar outcomes were observed in both groups (log rank p=0.862) (Figure 1). Background:More than 100 mutations of the NOD2 gene have been described, some of which have been related to diseases such as Crohn’s disease, Blau syndrome, early-onset sarcoidosis and NOD2-associated autoinflammatory disease (NAID), the latter of polygenic character. The inflammatory disease associated with a NOD 2 gene is characterized by periodic fever, dermatitis, polyarthritis, gastrointestinal symptoms and symptoms similar to those of the dry syndrome and currently associated with the NOD2 variants.Objectives:Describe the clinical characteristics and genetic variants of a cohort of patients in adulthood with autoinflammatory disease associated with the NOD2 gene with follow-up in a third level hospital in southern SpainMethods:Retrospective descriptive study of patients in adulthood with diagnostic suspicion of autoinflammatory disease and with genetic mutation in NOD2 gene. Patients with follow-up from 2013 to the present and in which another diagnosis of autoimmune disease (inflammatory bowel disease, sarcoidosis, etc.) were excluded. Data were obtained by review of medical records.Results:17 patients with alterations in the NOD2 gene were included. 14 cases (76.4%) were women. The mean age at diagnosis was 34.3 years (± 13.2). In the 17 patients, 19 mutations were found at the level of the NOD2 gene, the most frequent was with 9 cases (47.3%) the mutation in exon 4 (R702W), followed by mutation in exon 11 (1007finsC) with 5 cases (26.3%) and mutation in exon 4 (P268S) with 3 cases (15.4%). Other mutations obtained were mutation in exon 4 (R311w), mutation in exon 6 (M863V) and mutation in exon 11 (G908R) with a single case in each of them. In 6 cases (35.3%) there were first-degree relatives with a similar clinic, although not in all cases they were studied. Regarding the clinical manifestations, joint involvement (arthralgia and / or arthritis) with 12 cases (70.6%) was the most frequent followed by rash skin involvement with 10 cases (58.9%) and nonspecific abdominal pain with 9 cases (52.3%). Other less frequent manifestations were evening predominance fever and the elevation of acute phase reactants (CRP and ESR) with 8 cases (47.1%), oral thrush present in 5 patients (27.8%) and diarrhea with 4 cases (23.5%).Conclusion:The autoinflammatory disease associated with the NOD2 gene is an entity that can occur in adulthood and that can occur in a similar way to other auto-inflammatory diseases of a monogenic nature and in which the joint, cutaneous and abdominal involvement may predominate. We must maintain a high degree of suspicion and always include inflammatory bowel diseases in the differential diagnosis.Disclosure of Interests:None declared

Background:Tocilizumab (TCZ) was recently approved for Takayasu Arteritis (TAK) in Japan based on the results of the TAKT trial(1).However, data in clinical practice in Europe and America are scarce(2).Objectives:To assess efficacy and safety of TCZ in TAK of clinical practice in Spain.Methods:Observational, open-label multicentre study of 53 TAK patients treated with TCZ due to refractoriness or adverse events of previous therapy. Outcomes variables were improvement of clinical features, acute phase reactants and glucocorticoid-sparing effect.Results:53 patients (46w/7m); mean age, 40.6±14.6 years at TCZ onset. TCZ was started after a median of 12 [3.0-48.0] months from TAK diagnosis. In addition to systemic corticosteroids and before TCZ they received conventional immunosuppressant drugs (n=42) and biologic therapy (n=14). TCZ was prescribed as standard I.V. (n=42; 79.2%) or subcutaneous (n=11; 20.8%). The initial dose was 8 mg/kg/IV/4 weeks or 162 mg/SC/week, respectively. TCZ was used in monotherapy or combined with immunosuppressants (n=32; 60.4%): methotrexate (n=27), azathioprine (n=2), cyclosporine (n=3). Main clinical features at TCZ onset were: malaise (n=30),limb claudication (n=22), headache (n=18), fever (n=14), abdominal pain (n=10), and chest pain (n=9). Most of the patients experienced a rapid and maintained clinical, analytical improvement(TABLE).After a median follow-up of 18.0 [7.0-45.0] months, TCZ was discontinued in 20 patients due to: sustained remission (n=6), relapse (n=6), adverse event (n=5), gestation (n=3). Background Autoinflammatory syndromes are diseases that are characterised by increased inflammation mediated by cells and molecules of the innate immune system. Currently, new techniques have been developed that allow earlier diagnosis and treatment of these pathologies. Objectives Describe the clinical characteristics, genetic variants of different autoinflammatory diseases of a cohort of adult patients with follow-up in a 3rd level hospital Methods We carried out a descriptive cross-sectional study of adult patients with follow-up in reference hospital consultations with suspicion and/or diagnosis of autoinflammatory disease. Clinical, demographic, and treatment variables were characterised. A descriptive analysis was carried out by subgroup of pathology. The qualitative variables were expressed in frequency and percentages and the quantitative variables in median and interquartile range (IQR). The statistical software IBM SPSS v.18 was used for the analysis Results A total of 51 patients were included, 31 women (60.8%). Overall, the median age at diagnosis was 28 years (RI 15–40). The most used treatment was colchicine (60.8%), steroids (43.1%), FAMES (21.6%), anti IL-1 (17.6%). By subgroup of pathologies the most prevalent with 31 cases (60.78%), 16 women (51.6%) was familial Mediterranean fever (FMF) presenting 15 patients (48.4%) pathogenic mutations, 8 patients (25, 8%) low penetrance mutations and the rest (25.8%) genetic polymorphisms. The median age at diagnosis was 30.6 years (IQR16–44). The most common symptoms were fatigue (80.6%), acute phase reactants (APR) elevation (71%), fever and myalgias (67.7%), arthralgias (64%), abdominal pain (48.4%)), rash (31%), arthritis (29%), lymphadenopathy (29%), serositis (12.9%) With regard to TRAPS, 10 patients were diagnosed (19.6%), 7 women (70%), with a median age at diagnosis of 26.5 years (IQR 15–31.5). The most common symptoms were arthralgias, fatigue and myalgias (90%) with elevated APR, fever (80%), abdominal pain (70%), rash (60%) and arthritis (50%). There were 2 cases of women (4%) with Schnitzler syndrome, with arthritis symptoms, elevated APR and chest pain (in 1 case). There were 6 cases (11.8%) of disease associated with NOD 2 gene with a median age at diagnosis of 30 years (IQR 12.5–40), 4 were women (66.7%). The most common symptoms were arthralgias, fever and myalgias (83.3%) with increased APR, arthritis (66.7%), abdominal pain, oral thrush and rash (66.7%). In the disease associated with NLRP12 gene there were 2 cases (3.9%), both cases were women. The most common symptoms were arthralgias, arthritis, fever, fatigue, increased APR and serositis (in one case). There was a case of sickness associated with the NLRP3 gene (cryopyrinopathy) (2%) in a woman with fever and refractory pyoderma gangrenosum and another woman with a diagnosis of Muckle-Wells (2%) with clinical signs of deafness since childhood, conjunctivitis, fever and arthritis. There was a case of PFAPA (2%) in a woman with fever, pharyngitis, rash and lymphadenopathy. Conclusions In our series, the most frequent pathology was FMF followed by TRAPS. The most prevalent symptoms were systemic manifestations (fever, fatigue, APR elevation) and musculoskeletal manifestations. Disclosure of Interest None declared

Aldosterone facilitates cardiovascular damage by increasing blood pressure and through different mechanisms that are independent of its effects on blood pressure. In this respect, recent evidence involves aldosterone in the pathogenesis of metabolic syndrome. Although this relationship is complex, there is some evidence suggesting that different factors may play an important role, such as insulin resistance, renin-angiotensin-aldosterone system, oxidative stress, sodium retention, increased sympathetic activity, levels of free fatty acids, or inflammatory cytokines and adipokines. In addition to the classical pathway by which aldosterone acts through the mineralocorticoid receptors leading to sodium retention, aldosterone also has other mechanisms that influence cardiovascular tissue remodelling. Finally, overweight and obesity promote the adrenal secretion of aldosterone, increasing the predisposition to type 2 diabetes mellitus. Further studies are needed to better establish therapeutic strategies that act on the blockade of mineralocorticoid receptor in the treatment and prevention of cardiovascular diseases related to the excess of aldosterone and the metabolic syndrome.

The Strategic Plan for the Development of Internal Medicine in Andalusia arose from the need that the internal medicine doctors had to redefine the purpose and values of their specialty to cope with the numerous changes occurring in the health care area. The project was developed in three phases. First, the tendency of the health care system and current position of the specialty were analyzed. After, the internal and external opinions on the present-future of Internal Medicine were checked out. Finally, five strategic lines with their action plans were established. Specific objectives were defined within each line: results to be achieved, methodology according to action plan. After several years of collegial work in this initiative, very positive results have been achieved. We conclude that the Strategic Plan has been useful to better define the position of our specialty and to state which tools such as those mentioned are effective to cope with the new challenges that may occur in other groups.

A todos los pacientes se les propuso una entrevista, en la que se analizaron datos clinicos, el indice de Barthel (IB), la escala de Pfeiffer, la vulnerabilidad clinica (VC) y la escala de Gijon, asi como la mortalidad y las hospitalizaciones tras un ano de seguimiento. Posteriormente se realizo un analisis univariante y multivariante de los factores asociados a dichas variables. Resultados Los PP identificados fueron 806 (1,38% de la poblacion) y las categorias mas prevalentes fueron las de dano vascular. Fueron elegibles para hacer la entrevista un total de 662 (69%) pacientes. La mediana del IB fue 90 (0-100) y un 24% presento un IB Conclusiones La definicion de PP utilizada selecciona en AP a una poblacion fragil con una alta prevalencia de deterioro funcional, cognitivo, problemas sociofamiliares, VC y consumo de recursos sanitarios. Este deterioro multidimensional hace aconsejable la realizacion de una valoracion integral en la practica asistencial de estos pacientes.

Introduccion En el paciente pluripatologico (PPP) se desconocen los factores biopsicosociales que afectan o influyen sobre la empatia en la relacion medico-PPP. Nos propusimos determinar los factores propios del PPP, y los ajenos a el desde el punto de vista biopsicosocial que se asocian a una mejor relacion empatica con los profesionales sanitarios en una cohorte multicentrica con base poblacional de PPP. Material y metodos La cohorte multicentrica se genero en cuatro Zonas Basicas de Salud (tres urbanas y una periurbana) del area sanitaria del Hospital Universitario Virgen del Rocio, en Sevilla. La empatia se midio utilizando la escala de Likert (1-5) mediante entrevista (diferenciando entre medico de familia, enfermero/a de familia e internista de referencia), por un miembro del equipo investigador no relacionado con la asistencia del paciente. Tras ello, se procedio a realizar un analisis factorial con las variables asistenciales y se obtuvo como factor la empatia global (metodo de maxima verosimilitud oscilando la matriz de las tres variables resumidas entre 0,44 y 0,85). Los posibles factores predictores de la empatia global se escogieron de caracteristicas demografico-clinicas y asistenciales del PPP. La comparacion entre grupos se realizo mediante la «t» de Student y la prueba de ANOVA, y las correlaciones mediante el coeficiente de correlacion de Pearson. Posteriormente se realizo un analisis multivariante mediante regresion lineal y el dintel de significacion se establecio para una p Resultados Cuatrocientos sesenta y uno (69%) de los 662 PPP elegibles de la cohorte contestaron la entrevista. La empatia con el medico de familia, el enfermero/a de familia y el internista de referencia fue 4,39 ± 0,98, 4,49 ± 0,87 y 4,48 ± 0,48, respectivamente; y la mediana del factor empatia global fue 0,33 (-4,7-1,2). La empatia global se asocio con la accesibilidad telefonica al medico de familia (0,56 frente a 0,22; p Conclusion En la cohorte multicentrica de PPP, el nivel de empatia con los distintos profesionales asi como la empatia global fueron elevados. Esta ultima se asocio con el acceso telefonico al medico y al enfermero/a de familia, con el numero de farmacos prescritos y con una buena situacion de soporte sociofamiliar.

Introduccion El proceso de atencion a pacientes con enfermedad terminal se ha estructurado en torno a la Atencion Primaria y a las Unidades de Cuidados Paliativos, quedando en un segundo plano otros especialistas. Sin embargo, se desconoce el impacto y las caracteristicas clinicas y asistenciales de este grupo de pacientes en Medicina Interna. Nuestro objetivo consistio en evaluar estos parametros en diferentes areas de Medicina Interna de un hospital de tercer nivel. Material y metodos Estudio prospectivo observacional de los pacientes ingresados en Medicina Interna durante junio de 2003. Se estratifico a los pacientes en tres cohortes (paliativo, pluripatologico y general). Se analizo la incidencia, procedencia, caracteristicas clinicoevolutivas y consumo de recursos en los 12 meses previos al estudio. La comparacion univariada de variables clinicas entre la cohorte de paliativos, pluripatologicos y general se llevo a cabo utilizando los test de Chi cuadrado, Fisher, ANOVA y post-hoc y Kruskal-Wallis. Resultados Se incluyeron 52 pacientes (53,8% mujeres; edad media: 66,5 ± 15 anos) de la cohorte global de 339. La incidencia de pacientes paliativos fue 15,4/100 ingresos. Los pacientes procedieron de Servicios Especializados (57,6%), Urgencias (27%) y Atencion Primaria (10%). La estancia media fue de 14,5 (1-150) dias y la supervivencia del 63,5%. El 68,5% de los fallecimientos se produjo en el domicilio. Los pacientes de la cohorte de paliativos, comparativamente con la general, tuvieron menor Barthel basal (47,5 frente a 95; p Discusion Los pacientes con enfermedad terminal suponen un grupo significativo en nuestras areas de Medicina Interna, lo cual apoya el papel del internista en la asistencia a esta poblacion y le obliga a adquirir las competencias especificas para su manejo integral.

Approximately 50% of hypertensive patients are salt sensitive (they increase their Blood Pressure in response to sodium intake or volume expansion). Mechanisms underlying salt sensitivity are not completely elucidated although there is evidence that they may be genetically determined. The aim of this study is to establish the relation among some genetic polymorphisms of the renin-angiotensin system (RAAS) and the beta-3 subunit of the protein G and salt sensitivity. We studied 102 essential hypertensive patients, stage 1-2 and without target organ damage. Salt sensitivity was assessed by the rapid protocol of Weinberger. We determined by polymerase Chain reaction techniques the following polymorphisms: insertion/deletion (I/D) of the angiotensin-converting enzyme (ACE), A1166C of the angiotensin II type 1 receptor (AT1R), -344C/T and intron 2 conversion (IC) of the aldosterone synthase (CYP11B2), and C825T of the beta-3 subunit of the protein G (GNB3). 41 patients (40.19%) were salt sensitive. The distribution of the different polymorphisms was similar in both groups of patients, but subjects carriers of the W allele of the CYP11B2 IC polymorphism had a greater risk for salt sensitivity as compared with no carriers (37 of 41, 90.2% vs 4 of 41, 9.8%, OR 3.02, P

Background Patients infected with the human immunodeficiency virus (HIV) have an increased risk of cardiovascular disease due to increased inflammation and persistent immune activation. CD163 is a macrophage scavenger receptor that is involved in monocyte-macrophage activation in HIV-infected patients. CD163 interacts with TWEAK, a member of the TNF superfamily. Circulating levels of sTWEAK and sCD163 have been previously associated with cardiovascular disease, but no previous studies have fully analyzed their association with HIV. Objective The aim of this study was to analyze circulating levels of sTWEAK and sCD163 as well as other known markers of inflammation (hsCRP, IL-6 and sTNFRII) and endothelial dysfunction (sVCAM-1 and ADMA) in 26 patients with HIV before and after 48 weeks of antiretroviral treatment (ART) and 23 healthy subjects. Results Patients with HIV had reduced sTWEAK levels and increased sCD163, sVCAM-1, ADMA, hsCRP, IL-6 and sTNFRII plasma concentrations, as well as increased sCD163/sTWEAK ratio, compared with healthy subjects. Antiretroviral treatment significantly reduced the concentrations of sCD163, sVCAM-1, hsCRP and sTNFRII, although they remained elevated when compared with healthy subjects. Antiretroviral treatment had no effect on the concentrations of ADMA and sTWEAK, biomarkers associated with endothelial function. The use of protease inhibitors as part of antiretroviral therapy and the presence of HCV-HIV co-infection and/or active HIV replication attenuated the ART-mediated decrease in sCD163 plasma concentrations. Conclusion HIV-infected patients showed a proatherogenic profile characterized by increased inflammatory, immune-activation and endothelial-dysfunction biomarkers that partially improved after ART. HCV-HIV co-infection and/or active HIV replication enhanced immune activation despite ART.

To determine the prevalence of relevant drug-drug interactions (DDIs) and associated predictor factors in a sample of patients with multiple complex chronic diseases (polypathological patients) receiving multiple drug therapy. Our secondary objective was to determine the acceptance of a drug interaction reporting program with recommendations addressed to the prescribing physicians.A cross-sectional study performed in three primary care centres assigned to a teaching hospital. All patients with 2 or more chronic diseases and treated simultaneously with 5 or more drugs were recruited in the study. DDIs were detected by using Drug-Reax System((R)) (Micromedex) program, the Drug Data Base (Bot) Spanish General Council of Official Colleges of Pharmacists or literature search when needed. Those DDIs which, according to the opinion of the pharmacist investigators, required any intervention were considered relevant. Acceptance of the reported DDI recommendations was evaluated by means of a survey addressed by primary care physicians ("acceptable," pertinent recommendation to modify treatment).A total of 283 polypathological polymedicated patients were included. Mean age was 74.5 years (range 43-100 years). Mean number of diseases per patient was 2.5 and prescriptions 9.7). Out of a total of 2748 drug prescriptions, 1053 DDIs in 250 patients (96.5%) were identified. Of these, 45% were filtered as relevant DDIs. The presence of ischemic heart disease, two or more hospital admissions and having received 7 or more prescriptions were associated with the presence of DDIs. 177 informs containing 473 recommendations about DDIs were sent to primary care physicians from our Pharmacy Department. 339 recommendations were answered by primary care physicians, and 84% were favourably accepted.Almost every polypathological polymedicated patient is exposed to at least one DDI and about a 60% would require any intervention. Appropriate filtering and personalising recommendations in a collaborative way may represent an adequate manner to improve the risk-benefit ratio of the drug prescriptions.

The Strategic Plan for the Development of Internal Medicine in Andalusia arose from the need that the internal medicine doctors had to redefine the purpose and values of their specialty to cope with the numerous changes occurring in the health care area. The project was developed in three phases. First, the tendency of the health care system and current position of the specialty were analyzed. After, the internal and external opinions on the present-future of Internal Medicine were checked out. Finally, five strategic lines with their action plans were established. Specific objectives were defined within each line: results to be achieved, methodology according to action plan. After several years of collegial work in this initiative, very positive results have been achieved. We conclude that the Strategic Plan has been useful to better define the position of our specialty and to state which tools such as those mentioned are effective to cope with the new challenges that may occur in other groups.

To analyze clinical, functional, mental, sociofamiliar, and evolutional characteristics of pluripathological patients (PP) in Primary Health Care setting.Prospective, multiinstitutional cohort study in four Primary Health Care Institutions by active identification of PP from a computerized registry using the Spanish Andalusian Health Care Council criteria. A clinical interview was proposed to all identified patients. The clinical data, Barthel index (BI), Pfeiffer scale, clinical vulnerability (CV), sociofamiliar features by the Gijon scale, and 1-year admissions and mortality were analyzed. An univariant and multivariant analysis was performed in order to know the risk factors associated to previously described variables.Overall, 806 PP were detected (1.38% of the population). Cardiovascular categories were the most prevalent. A total of 662 patients (69%) were eligible for the interview. Median BI was 90 (0-100), and 24% of patients had severe functional impairment (BI60). Twenty-nine percent of them had been admitted to hospital at least once in the last 3 months. Patients with more functional impairment and CV were older, having more defining categories, especially E category. A total of 174 patients (37.75%) had cognitive impairment. This group was older, with more functional impairment, and worse sociofamiliar support. One-year mortality was 6.1%, and was correlated with CV and older age.The definition of PP used selects in the Primary Care setting a population with a high level of multidimensional frailty having a high prevalence of functional, cognitive deterioration, sociofamiliar problems, CV and consumption of health care resources. Due to this multidimensional deterioration, it is recommendable to make an integral evaluation in the health care practice of these patients.

The biopsychosocial factors that affect or influence empathy in the doctor-patient with multiple disease (PMD) relationship are unknown. This study aimed to determine the patient's own and external factors (from a biopsychosocial point of view) associated to a better empathic relation with health professionals in a multicenter cohort obtained from a population basis.The multicenter cohort was obtained from four basic health zones, 3 urban and one periurban, from the province of Seville (Southwestern Spain). Empathy was measured with the Likert scale (1-5), by means of an interview (differentiating between family doctor, family nurse and reference internist), by a member of the research team who was not related with patient's care. After that, a factorial analysis was performed, obtaining global empathy as a factor (maximum likelihood method, the summarized three variable matrix, ranging from 0.44 to 0.85). The possible predictive factors of global empathy were chosen from clinical-demographic care features of PMD. Comparisons between groups were performed with the Student's t and ANOVA tests. Correlations were with Pearson correlation quotient. Finally a multivariant analysis was done by linear regression and p0.05 was considered statistically significant.Four hundred and sixty (69%) of 662 eligible PMD from the cohort answered the interview. Empathy with family doctor, family nurse and reference internist was 4.39 +/- 0.98; 4.49 +/- 0.87 and 4.48 +/- 0.48, respectively. The median of the global empathy factor was 0.33 (-4.7-1.2). Global empathy was associated to having telephone access to the family doctor (0.56 vs 0.22; p0.0001) and to the family nurse (0.58 vs 0.24, p = 0.05), and was inversely correlated with global Gijon scale (r -0.100; p = 0.031) and with two of its dimensions: support (r -0.145; p = 0.002), and income (r -0.167; p0.0001). In the multivariant analysis, social evaluation measured by the Gijon index (p = 0.001) and number of drugs prescribed (p = 0.004) independently predicted global empathy.In the PMD multicenter cohort, both empathy level with the different health professionals and global empathy were high. The latter was associated with having telephone access to the family doctor and nurse, with number of drugs prescribed and with a good social and family support status.

Incidence, clinical features, and outcome of heart failure in patients with other chronic pathologies have been scarcely evaluated. The aim of the present study was to prospectively assess these issues, and the prognostic and factors associated to functional deterioration in a cohort of pluripathologic patients (PP) with heart failure (HF), attended in areas of Internal Medicine of a tertiary teaching hospital in the south of Spain.Prospective observational study of all patients, attended in Internal Medicine areas of a tertiary teaching hospital, during June 2003. Patients were stratified in two cohorts: PP with HF as main category (PP-HF), and PP with no HF. Patients with two or more chronic diseases, distributed into seven categories (defined by a panel of experts) were considered PP. Incidence of PP-HF, functional evaluation (at baseline, at admission, and at discharge), and burden of hospital care (by means of urgent and programmed assistances, as well as episodes of hospitalization) in the last 12 months were analyzed. Chi-square, Fisher, "t" Student or U-Mann-Whitney and Rho de Spearman test were used for group comparisons. A multivariate analysis of predictors of survival and functional deterioration (fall in Barthel's scaleor = 10 points between baseline-discharge values) was performed in the PP-HF cohort. A p0.05 was considered significant.132 pluripathologic patients (55 in PP-HF, and 77 in PP cohort) were included, from a global cohort of 339. Global incidence of PP-HF was 38,9/100 admissions. Mean age of PP-HF patients was 78, 50.9% were females; mortality rate and mean hospital stay were 23.6% and 12.2 days, respectively. Patients of PP-HF cohort compared to those of PP, were older (78 +/- 9.5 vs 73 +/- 10.8; p0.005), and suffered more chronic diseases (p = 0.0001). Functional abilities (at baseline, at admission, and at discharge), mean hospital stay, mortality, and burden of care in the previous 12 months were similar. Better functional abilities (OR: 1.136 [0.94-1.842]; p = 0.055), and less associated chronic diseases (OR: 0.072 [0.006-0.943], p = 0.045) were independently associated to survival; while older age (OR: 1,217 [1.016-1.457]; p = 0.03), and a poorer functional status at baseline (OR:1.80 [1.019-1.144]; p = 0.01) were associated to functional deterioration.Heart failure prevalent disease in pluripathologic patients. Specific factors associated to survival were gender and less chronic conditions; while those associated to functional deterioration during hospital stay were age and a poor functional status at baseline.

The medical assistance to patients with terminal diseases has been structured with Primary Care and Palliative Care Units as protagonists, and with other specialties as secondary roles. Nevertheless the impact of this group of patients in Internal Medicine areas is unknown. Our objective was to evaluate these parameters in the area of Internal Medicine of a tertiary teaching Hospital.Prospective observational study of all patients attended in Internal Medicine areas during June 2003. Patients were stratified in three cohorts (palliative, pluripathologic, and general). Incidence of palliative patients, origin, clinical features, and burden of hospital care in the last 12 months were analyzed. Univariate analysis of the clinical differences between the palliative and the pluripathologic, and general cohorts was performed, using Chi-square, Fisher, ANOVA and post-hoc tests and Kruskal-Wallis test.52 (53.8% women; mean age 66.5 +/- 15 years) were included from the global study cohort of 339 patients. Incidence of palliative patients was 15.4/100 admissions. The patients were admitted from other specialties (57.6%), Emergency department (27%), and Primary Care (10%). Mean hospital stay was 14.5 (1-150) days, and survival 63.5%. The 68.5% of deceases occurred at home. Patients of palliative cohort, with respect to general cohort had less functional ability at baseline (47.5 vs 95; p0.0001), admission (40 vs 75; p0.0001), and at discharge (20 vs 75; p0.0001), and more functional deterioration during hospital stay (mean fall in Barthel's values at baseline-discharge of 27.5 vs 20 points; p0.003). There were no differences in the burden of hospital care in the previous 12 months. With respect to the cohort of pluripathologic patients, palliative patients were younger (66.5 +/- 15 vs 75 +/- 11 years; p = 0.001) and had similar functional limitations at baseline (47.5 vs 45), admission (40 vs 20) and at discharge (20 vs 20).Patients with terminal diseases are prevalent in the clinical setting in areas of internal medicine. These data support the role of the internist in palliative care proceedings, and prompt internists to acquire enough specific abilities to manage competitively these population.

Background A progressive dissatisfaction has been noted among many internists, and the future of the specialty represents a matter of concern. The objectives of this study were to analyze career satisfaction among 182 internists from 34 hospitals in the south of Spain and to assess the opinion of 47 other health care professionals who frequently interact with internists on the present and future role of the specialty. Methods Ours was a multi-institutional, cross-sectional survey of internists that focused on demographics/practice characteristics, education/research, career satisfaction, and opinions regarding the future of internal medicine. Also included were interactive, interdisciplinary focus groups of family physicians, sub-specialists, and managers. The internists were asked to complete a questionnaire that contained items and factors that were measured on a 5-point Likert scale and quantitatively analyzed; the opinions of the focus groups were qualitatively analyzed. Results The internists surveyed dedicated most of their time to treating hospitalized patients rather than to diagnostic procedures and technical assessment. Some 54% and 57% of the internists pursued continuing medical education and research, respectively, in their free time. The internists were satisfied with the content of their work and with their interpersonal relationships, but they were dissatisfied with their physical/instrumental environment, management policies, and degree of work stability. No differences were detected in relation to age, gender, community factor, or professional category. With regard to the future of the specialty, the main opportunity detected by internists was a closer collaboration with primary care. An analysis of the strengths, weaknesses, future opportunities and threats to internal medicine expressed by the seven focus groups was highly concordant and added worthwhile information and clear proposals for the development of the specialty. Conclusions Internists in the south of Spain were satisfied with the content of their work and dissatisfied with health care management and job stability. The future of internal medicine was felt to lie in a deeper collaboration between internists and their colleagues in primary care.

To study 11 beta-hydroxysteroid dehydrogenase activity in Cushing's syndrome.Measurements of free cortisol, cortisone, tetrahydrocortisol and tetrahydrocortisone in 24 h urine samples of patients with Cushing's syndrome and controls.The cortisol to cortisone and tetrahydrocortisol to tetrahydrocortisone relationship was significant (in controls, r = 0.70; p0.0001, and r = 0.75; p0.0001) respectively, but it was not in patients with Cushing's syndrome.11 beta-hydroxysteroid dehydrogenase activity is decreased in patients with Cushing's syndrome.

To know the prevalence of phenotypic dyslipidemias and their clinical and metabolic characteristics in recently diagnosed hypertensive patients.Consecutive study of 158 essential hypertensive patients without previous pharmacological treatment.69.6% of the patients had some kind of dyslipidemia, being the isolated increase of Lp(a) (27.3%) the most prevalent and the hyperapobetalipoproteinemia the less (10.0%). Age, sex, smoking, alcohol consumption, uric acid, systolic and pulse pressure and serum glucose were different among phenotypes.Essential hypertensive patients have high and heterogeneous prevalence of dyslipidemias.
