Aim. To study features of endogenous intoxication syndrome (EI), oxidative stress (LPO) and antioxidant defence (AOS) in patients with acute lower limb ischemia (LLI).Materials and methods. Some parameters of EI, LPO and AOS were studied. EI syndrome was studied in 86 patients with LLI who underwent complex examination and treatment in 2020-2021 in the Department of Vascular Surgery of the RNCfCS. POL and AOS parameters were studied in 14 patients out of 86. There were 51 (59.3%) men and 35 (40.7%) women, mean age was 56.0±5.8 years. Acute occlusion of the iliofemoral segments was noted in 33 (38.4%) patients, in 32 (37.2%) - the femoral segment, and in 21 (24.4%) - the popliteal and femoral vessels. In all cases, ischemia had a unilateral localization - on the right in 59 (68.6%) cases and on the left in 27 (31.4%) cases. In 52 (58.1%) cases, there were clinical signs of grade II b ischemia, and in 36 (41.9%) cases - grade III A (according to V.S. Saveliev, 1987). The average duration of ischemia was 77.9±28.3 hours.Results and discussion. In all cases of LLI before revascularization, there were signs of acute inflammatory reaction and EI syndrome in the form of increased ESR (42.3±7.1 mm/h), CRP concentration (16.1±4.2 mg/L), fibrinogen (8.5±0.9 g/L) and leukocytosis (12.7±1.5×109/L). Also, all patients had a slight elevation of blood glucose against the background of EI (7.5±1.8 mmol/L), a significant increase of creatinine concentration (128.9±6.1 µmol/L), urea (9.2±0.6 mmol/L) and aminotransferases as compared to normal indices. Results of LPO and AOS indices study in 14 patients showed that in LLI, malondialdehyde (MDA) increases 2.6-fold (3.9±0.1 μmol/L), diene conjugate increases 3.7-fold (0.77±0.04 units a/mL). However, after the elimination of ischemia, there was no significant decrease for 12.2±2.4 days, which confirms the presence of reperfusion syndrome. AOS indices - superoxide dismutase (20,8±1,0 u/l) and catalase (128,9±11,2 μl/l) increased 1,2 and 1,1 times in the examined patients cohort, respectively. Statistically significant associations were established between the following indices: Ischemia level and urea (r=0.89), Ischemia level and white blood cell count (r=-0.82), Hb and Er (r=0.99), Hb and creatinine (r=0.79), Hb and urea (r=0.79), Er and urea (r=0.79), creatinine and Er (r=0.90), ACTV and creatinine (r=-0.77), creatinine and urea (r=0.90).Conclusion. Acute lower limb ischemia is accompanied by ischemia-reperfusion damage manifested by endogenous intoxication syndrome, impaired glucose metabolism and activation of lipoperoxidation processes and antioxidant system. After the elimination of acute ischemia, there is not always a significant decrease in lipid peroxidation and some inflammation parameters. Therefore, prolonged use of antioxidants in the reperfusion period is recommended.

Aim. To optimize the surgical treatment of recurrent postoperative ventral hernias using synthetic alloplastic materials. Material and methods. The study was conducted among 139 patients with recurrent postoperative ventral hernias. According to the indications, all patients underwent abdominoplasty using mesh biologically inert alloprostheses using the “onlay” method and according to the original method with the displacement of the vaginal leaves mm. recti abdominis. Results and discussion. Recurrence of ventral hernias after the first year of follow-up was detected in 6 (4.3 %) patients. The causes of relapses in large hernias are dystrophic processes in the soft tissues of the anterior abdominal wall, caused both by the activation of fibroblasts upon contact with the polypropylene mesh, and directly by the defect of the mesh as a result of its rupture. In the event of a recurrence, reconstructive surgeries were performed in the scope of a modified technique with component separation to eliminate additional tissue tension. Conclusion. The developed method of surgical treatment of postoperative ventral hernias involves dissection of the anterior walls of the sheath of the rectus abdominis muscles along the the muscles, suturing the aponeurotic edges of the defect, connecting the rectus muscles along the midline, followed by fixing the mesh implant to the outer edges of the dissected aponeurosis of the rectus abdominis muscles, which makes it possible to prevent the occurrence of cicatricial degeneration and defect of functional activity, as demonstrated by monitoring the function of the rectus abdominis muscles.

Neutrophils fight with invading pathogens through various mechanisms including degranulation, phagocytosis, and the release of neutrophil extracellular traps (NETs). This study aimed to determine the impact of a synthetic formyl-peptide (FMLP) on human neutrophils in vitro, and to determine the role of mitoxantrone (MTX), a pharmacological blocker of mitochondrial Ca2+ Uniporter (MCU), on FMLP-induced alterations. Isolated neutrophils and a whole-blood preparation of neutrophils were pre-treated with MTX and then stimulated with FMLP. Field’s-stained smears and brightfield microscopy were employed for morphological characterization and quantification of neutrophils. The release of cell-free DNA (cfDNA) was also measured for determining neutrophil damage. Our data demonstrated degenerative changes in neutrophils and a greater cfDNA release upon stimulation with FMLP which was negatively associated with the presence of resting platelets in whole blood preparation. Interestingly, MTX pre-treatment significantly reduced FMLP-triggered neutrophil damage and cfDNA release. Metformin, a known inhibitor of NETs formation, also decreased the FMLP-induced changes in neutrophils. In addition to confirming the degenerative potential of FMLP, this study reveals a novel contribution of MCU in regulating FMLP-induced morphological alterations in human neutrophils.

Objective: to study the prevalence of latent tuberculosis infection (LTBI), risk factors for LTBI to develop into active tuberculosis, the efficacy of preventive treatment in those with LTBI among labor migrants, residing in the Republic of Tajikistan.Subjects and methods. In the Republic of Tajikistan, 500 residents were followed up as Main Group, they all were labor migrants for more than 3 months and returned to their homeland within one year before inclusion in this study. All of them participated in a questionnaire survey to identify risk factors for developing tuberculosis and had the examination that included Mantoux test with 2 TE and chest X-ray. Comparison Group (CG) consisted of 500 citizens of compatible gender and age who did not travel as labor migrants; they all participated in the same survey and had the same examination.Results. The positive results of Mantoux test with no abnormalities on X-ray were observed in 40% of people in Main Group and 33.4% of people in Comparison Group.The following factors were found to be more frequent in Main Group versus Comparison Group, respectively: frequent and continuous exposure to a TB case – 78.2 and 23.1% (p < 0.01); no tuberculosis prevention – 83.1% and 48.4% (p < 0.01); poor accommodation and work conditions 87.9 and 13.6% (p < 0.01); inadequate diet and lack of proper clothes – 79.4 and 24.3% (p < 0.01); limited access to medical care – 85.4 and 22.6% (p < 0.01). MG was found to have a higher summarized risk coefficient (r = 4.75) versus CG (r = 2.0), p < 0.01. During 2–3-year follow-up of those with LTBI who had no preventive treatment with isoniazod, tuberculosis was diagnosed in 44.4% of people from MG and 18.5% of people from CG. The introduction of the integrated approach to tuberculosis control among people preparing for labor migration allowed reducing the proportion (from 19.7 to 13.7%) of labor migrants in the structure of TB incidence.

Experimental and numerical investigation was conducted to study the thermo-hydraulic characteristics in circular tube fitted with conical spring inserts. The dimensions of circular copper tube are 100cm, 2.2cm and 2.4 cm which represent the length, inner and outer diameters respectively. The length of insert was 15cm, while the conical spring has 16mm diameter gradually decreases to 6mm with the length and the pitch between the coils was 20mm. The diameter of wire used to fabricate the conical spring insert was 4mm. Those inserts were arranged into eight categories (A1 to A8). In the numerical part, the governing equations were solved by using commercial ANSYS-Fluent package (2019 R3) with the assistance of solid works and Gambit software program. The finite volume approach was used in solving the equations to predict the pressure flow, heat transfer and temperature distribution along the tube. Experimental work included design and built a test platform for the experimental measurements. The aim of experimental study was to predict the behavior of heat transfer and pressure in the tube with two cases, plain tube and inserted tube at the effect of different parameters. All experimental tests were conducted at turbulent flow fully developed. The range of Reynolds number and heat flux were (3542-7522) (4549-10576 W/m^2) respectively. The results showed that the best augmentation in heat transfer was by using a category A5 of conical spring insert compared with plain tube. Results show good agreement between the numerical and experimental results with an error of 8%. Also results showed that the Nusselt number increases as the Reynolds number increasing.

A new method is described for detection of pork and lard as adulterants in processed beef and mutton mixtures. The unsaturated triglycerides in the fat are ozonized and then derivatized. The mixture of derivatized and saturated triglycerides is analyzed by liquid chromatography using a reverse-phase column and a UV detector. Pork fat has larger amounts of triglyceride containing saturated fatty acid at the C-2 position than does the fat of other meat. The ratio of triglyceride containing saturated fatty acid vs triglyceride containing unsaturated fatty acid at the same (C-2) position (SSU/SUS) in a sample is compared with those of pure meats. The presence of pork in the sample causes the ratio to increase compared with ratios for pure beef or mutton. The increase in the SSU/SUS ratio is significant for the addition of 1% pork in beef. In the case of mutton, the addition of 3% pork causes a noticeable change. The method is reliable and is also applicable to samples containing only fat. Processing (heating or cooking) does not affect the ratios.
