This paper presents interdisciplinary consensus on the use of protocols of high-intensity magnetic stimulation for the treatment of pelvic floor muscles dysfunction with anal incontinence in Russia.AIM: to highlight the discussion and the decision-making on the basis of an interdisciplinary consensus on the use of a new algorithm of peripheral and transcranial magnetic stimulation in the treatment of pelvic floor muscles dysfunction with the clinic of anal incontinence.RESULTS: the adoption of this consensus can serve as a basis for further research of this problem and optimize the results of treatment of patients with pelvic floor muscle dysfunction with anal incontinence. The data may be interesting for a wide range of medical specialists: general practitioners, gastroenterologists, coloproctologists, surgeons, neurosurgeons, gynecologists, urologists — anyone who encounter with manifestations of this disorder in a routine practice and chooses diagnostic and treatment options.CONCLUSION: protocols for the treatment of anal incontinence using high-intensity magnetic stimulation aimed at improving the quality of treatment of patients with anal incontinence are based on the Russian experience of using the methods discussed and the analysis of the results obtained are validated.

Surgical treatment of rectal cancer and sphincter-preserving low anterior resection results to evacuation disorders («low anterior resection syndrome» - LARS). There are no clinical recommendations for the treatment of patients with LARS as well as a rehabilitation program for them.To develop a rehabilitation program for patients with low anterior resection syndrome.The investigation was performed at 2 stages. During the first stage, 29 patients with LARS (17 (58.6%) men, mean age of the participants' 61.5±9.5 years), 12 (41.4%) women (mean age 61.2±7.8 years) were examined and received the course of conservative treatment with the use of biofeedback-therapy performed by the standard protocol. On the second stage, 17 patients (mean age 61.4±12.7 years) - 9 (52.9%) men, 8 (47.1%) women received biofeedback therapy in combination with tibial neuromodulation (TNM). Functional state of the rectum and the locking apparatus in all patients were evaluated by anorectal manometry: sphincterometry and studies of the reservoir function of the rectum before and after treatment.The developed complex of rehabilitation measures led to improved treatment results for patients with rectal cancer by improving the quality of life after low anterior resection, reducing the manifestations of LARS (by 47.8%). Stable positive results of treatment were maintained in 36.4% of patients, positive dynamics from the treatment according to the LARS scale decreased slightly in 54.5%, deterioration of indicators in 3-6 months after conservative rehabilitation was registered in 9.1% of cases. At the first stage maximal squeeze pressure improvement was reached in the whole cohort (Rehabilitation program for patients with low anterior resection syndrome should include: 1) biofeedback therapy to improve the holding function; 2) biofeedback therapy aimed at improving the reservoir function and sensitivity of the rectum to filling; 3) tibial neuromodulation. This program may help improving the contractility of the anal sphincter and reservoir function of the rectum, as well as the appearance of the urge to defecate.После сфинктеросохраняющей низкой передней резекции у значительной части больных раком прямой кишки отмечаются жалобы на проблемы эвакуации, а также удержание кишечного содержимого, объединенные в «синдром низкой передней резекции» (СНПР). На сегодняшний день нет клинических рекомендаций по лечению и реабилитации больных с СНПР.Разработать программу реабилитации больных с СНПР.На первом этапе работы были обследованы и пролечены 29 больных с СНПР: 17 (58,6%) мужчин (средний возраст 61,5±9,5 года), 12 (41,4%) женщин (средний возраст 61,2±7,8 года). На 2-м этапе были обследованы и получали консервативное лечение 17 больных (средний возраст 61,4±12,7 года): 9 (52,9%) мужчин, 8 (47,1%) женщин. Функциональное состояние низведенной кишки и запирательного аппарата оценивали с помощью методов аноректальной манометрии: сфинктерометрии и исследования резервуарной функции низведенной кишки до и после лечения (у 17 больных на втором этапе). Пациентам был назначен курс 10-дневной консервативной реабилитации методом БОС-терапии стандартным протоколом на первом этапе и протоколом комплексной БОС-терапии на втором этапе, а также тибиальной нейромодуляции.Разработанный комплекс реабилитационных мероприятий привел к улучшению результатов лечения больных раком прямой кишки за счет повышения качества жизни после низкой передней резекции, снижая проявления СНПР на 47,8%. Устойчивые положительные результаты лечения сохранялись у 36,4% больных, положительная динамика от проведенного лечения, по данным шкалы СНПР, несколько снижалась у 54,5%, ухудшение показателей через 3—6 мес после консервативной реабилитации регистрировалось в 9,1% случаев. Полученные данные позволили рекомендовать повторный курс лечения 63,6% пациентам. Авторами был модифицирован и оптимизирован специальный комплекс ЛФК для реабилитации больных как в лечебном учреждении, так и в домашних условиях. Приведенные упражнения направлены на улучшение функционального состояния мускулатуры тазового дна и сфинктерного аппарата.В реабилитационную программу больных с СНПР следует включать: 1) БОС-терапию (биофидбэк) для улучшения функции держания; 2) БОС-терапию, направленную на улучшение резервуарной функции и чувствительности низведенной кишки к наполнению; 3) тибиальную нейромодуляцию. Проведение этой программы может способствовать улучшению сократительной способности анального сфинктера и резервуарной функции низведенной кишки, а также появлению позыва на дефекацию.

The subject of the article is the phenomenon of poverty in light of the problems of excessive economic inequality, which is the main source of formation of Russian poverty, and indicative of institutional analysis which involves a critical revision of social-economic policy of the state instead of the official way of dealing with this problem without considering the reasons of its formation (targeting of poverty). The aim of the work is to update the cause-and-effect factors of poverty formation in Russia, the main of which is a critically high level of inequality with an assessment of the role, nature and causes of its causes. The hypothesis of the study is the thesis that the fight against poverty is ineffective based only on increasing the number and level of social payments and social programs, without taking into account the causes of poverty, thus not affecting any socio-economic processes for its formation. Meanwhile, both the indicative and institutional approach to the analysis of the main causes of poverty shows the dominant role of excessive inequality in the formation of the problem of Russian poverty. In turn, inequality is caused by the inefficiency of the state’s distributive and redistributive policies, caused by the lack of scientific conditionality and systematic political decisions.

The processes of swelling of poly(acrylic acid) ferrogels prepared via radical polymerization in an aqueous suspension of ferric oxide nanoparticles with the weighted average size of 23.5 nm obtained by laser evaporation method and stabilized by chitosan (М = 5.3 × 105 and degree of deacetylation of 62%) are studied. The swelling of washed ferrogels depends on the content of chitosan and decreases abruptly at a polymer concentration exceeding 1 g/L. At a chitosan concentration above 1 g/L, the chemical network of poly(acrylic acid) is formed on the fluctuation network of chitosan in solution. As pH increases, these ferrogels are contracted owing to formation of an interpolymer complex of chitosan with poly(acrylic acid) subchains.

Subject of consideration of article are the difficult systems and problems of their management which striking example is the public sector. The purpose of work is specification of complexity as independent object of the management demanding including, increases in a role and improvement of mechanisms of coordination in management of difficult system. Options of consideration of difficult system and elements of its complexity with allocation of four of its main types are stated: difficulties of plurality of affine components, their behavior and communication and cognitive difficulties in article.

Object of research are the tax concessions as the most important instrument of state regulation. The purpose of work is formation of measures of improvement of tax incentives on the basis of the analysis of their modern problems and approaches of use for spatial development of the country. In article an assessment of the central role of fiscal regulation which major part are tax incentives is given. Two major directions of spatial development of the country are considered (creation of effective regional and interregional areas of innovatively focused advancing development and mitigation of social inequality of regions, investments into the human capital) and problems of their modern realization are revealed. The analysis of measures of tax incentives of two main directions of realization of spatial strategy of Russia aimed at providing is carried out.

according to Russian and international experts, the East Siberian taiga is not only practically the only natural ecosystem in the country, but is also one of the "global ecoregions" that have been identified by WWF as the most valuable territories in terms of preserving biodiversity on a global scale. At the same time, the current use of this territory is characterized as extremely depleted, due to the high level of illegal logging, the scale of forest fires and the current nature of forest management and reforestation. One of the reasons for this is the extremely low efficiency of forest resources management against the background of high corruption of the authorities in this area, which creates additional risks and losses. In these circumstances, it seems necessary to create large-scale environmental conservation project of the East Siberian taiga, perhaps in conjunction with WWF, which implements, including the biodiversity conservation project in the Altai-Sayan Ecoregion. The principles and approaches of the latter within the framework of the forest program can serve as a basis for conservation measures of the East Siberian taiga. The article analyzes the problems and conditions for implementing the main principles of the forest program of the above-mentioned WWF project in relation to the East Siberian taiga and suggests additional measures to protect the unique Ecoregion.

In article the analysis of modern problems of forest exploitation on the example of the Irkutsk region as leading region in the field of stocks and wood procurement is given. Extensive forest exploitation in the region at catastrophically low volume of reforestation is followed by injurious destruction of the wood because of its illegal cabin, big losses of the wood from the fires, etc. that modern forest exploitation as «catastrophically exhausted» is characterized. Creation of such situation with forest exploitation is result of the state control system of the wood, both on legislative, and on executive levels. The scale and depth of problems shows a condition of a failure of the state which has a number of identifiers, including a set of characteristics offered in article. Change of a situation by the author contacts introduction of the adjusting mechanisms by means of system improvement of social management.

The article considers problems of forest utilization in Irkutsk oblast in the light of managing forest as federal property. General and particular reasons for the crisis situation in the region's forest products sector are identified as well as opportunities for efficient use of state property.

The covariant Lagrangian of the interaction of an electromagnetic field with polarizable spin hadrons has been constructed invoking sequentially the principle of correspondence between relativistic electrodynamics of moving media and relativistic quantum field theory and taking into account the cross-symmetry condition. It has been shown that for 1/2-spin hadrons, to the third order in radiation frequency, the contribution to the effective Lagrangian is from the spin polarizabilities.

The influence of high-, low- and very-low-density lipoproteins on platelet aggregation was examined in coronary patients and normal subjects. Preincubation with high-density lipoproteins was shown to drastically inhibit platelet aggregation capacity both in normal subjects and coronary patients. Preincubation with a mixture of low- and very-low-density lipoproteins had no significant effect on platelet aggregation. The suppression of platelet aggregation by high-density lipoproteins was the most obvious in washed platelets.

The effects of trifluoperazine and haloperidol on protective activity of phenazepam were studied during seizures in mice treated with pentylenetetrazole (an index used for the appraisal of tranquilizing activity) as was their action on the phenazepam-induced depression of the test potential in the recovery cycles of somatosensory primary response. Trifluoperazine administered in doses of 0.1-0.5 mg/kg potentiated anticonvulsant action of phenazepam but did not change its effects on the recovery cycles of primary response. Haloperidol also potentiated anticonvulsant action of phenazepam. However, the doses administered were 4-5-fold lower than the tranquilizing doses of haloperidol. The drug also increased the depression of the test potential in the somatosensory recovery cycle, caused by phenazepam. This suggests an increment of GABA-positive effect of the tranquilizer. It appears that higher activity of haloperidol in elevating anticonvulsant and, probably, anxiolytic effects of phenazepam is determined by interaction of these drugs on the level of cortical GABA-ergic receptors.

Variations in platelet aggregation activity before and after the control of lipid metabolism with lecithin linoleate (essential preparation, 1800 mg EPL daily) were examined in coronary patients with pronounced hyperlipidemia. A considerable change was noted in the ratios of LP fractions, that is, rising HDLP in the presence of decreasing total plasma lipids, Changes in plasma lipid composition were accompanied with a reduction of platelet aggregation activity that persisted for 3 months following the discontinuation of the drug. Correlation and regression analyses of relationships between platelet aggregation and HDLP levels were carried out.
