The use of the method of expert assessments is an indispensable element of the search for reserves to improve the quality and safety of medical activities for the realization of strategic tasks in the field of population conservation and improvement of health indicators of the Russian population.The purpose of the study was to assess the factors influencing the appeal for drug treatment of alcohol–associated diseases in state medical organizations on the model of a large agro-industrial region.Materials and methods. Anonymous sociological survey was conducted in 2022. 135 doctorsspecialist in the field of “Psychiatry-narcology” working in outpatient or inpatient units of the public health system of the Krasnodar region. The following methods were applied in this study: sociological, statistical, content analysis, expert assessments.Results. The representative group of experts was characterized by a predominance of female doctors (57,0 %), persons aged 40–49 years (25.2 %) with a qualification category (76.3 %), with more than 15 years of work experience in the specialty “Psychiatry-narcology” (58.5 %). The organization of drug treatment in outpatient settings was rated “good” and “excellent” by 54.8 % of respondents, in inpatient – 71.1 %. According to experts, the factors of legal restrictions, public censure, as well as defects in the self-assessment of the nosological status of patients and the lack of general medical literacy of the population had the greatest impact on the decrease in the incidence of drug treatment for alcohol-related pathology.Conclusion. The potential aspects of possible directions for selective influence in order to increase the appeal of the population with addictive alcohol disorders to specialized professionals and the preservation of human resources were identified.

Aim: bring to the attention of practitioners indications for anti-Helicobacter therapy, methods and procedure for diagnostics and eradication therapy of Н. pylori infection.Key points. Chronic gastritis caused by Н. pylori infection, including asymptomatic persons, may be considered as an indication for eradication therapy of Н. pylori as etiological therapy and opportunistic screening for gastric cancer prevention. Indications, for obligatory anti-Helicobacter therapy include peptic ulcer, gastric MALT lymphoma, early gastric cancer (EGC) with endoscopic resection. H. pylori primary diagnostics methods include 13C-urea breath test, H. pylori stool antigen lab test, rapid urease test and serological method. The serological method cannot be used after anti-Helicobacter therapy.In Russia H. pylori strains' resistance to clarithromycin does not exceed 15 % in most regional studies. The first line therapy for Н. pylori infection eradication is the standard triple therapy including a proton pump inhibitor (PPI), clarithromycin and amoxicillin, enhanced with bismuthate tripotassium dicitrate. A classic four-component therapy based on bismuthate tripotassium dicitrate or quadrotherapy without bismuth drug products which includes PPI, amoxicillin, clarithromycin and metronidazole, may be used as alternative to the first line eradication therapy. The standard triple therapy may be prescribed for 14 days only in those regions, where it has been proven to be effective. Quadrotherapy with bismuthate tripotassium dicitrate is also used as main second line therapy in case of standard triple therapy, bismuth enhanced standard triple therapy or combined therapy failure. Another second line therapy includes PPI, levofloxacin and amoxicillin, to which a bismuth-containing drug product may be added. The third line therapy is selected individually based on previously used treatment settings.Conclusion. In each case of H. pylori infection the decision for eradication therapy should be made, which is especially relevant as eradication of H. pylori has been recognized as an effective measure for the prevention of gastric cancer.

Introduction: The policy of mortality decline of the population of Russia, pursued by the state, requires consideration of the regional peculiarities of mortality in territorial subjects of the Russian Federation by gender and age. The regional situation on male and female mortality also should be considered when developing the measures directed to prevent various diseases including infectious ones. In this regard, the studies of gender and age indicators of mortality during the periods of infectious pandemics become relevant.Objective: The analysis of gender and age rates of COVID-19 mortality in 2021 in order to substantiate the best management decisions in the sphere of the organization of medical care and statistical registration of lethal outcomes during the pandemic spread of dangerous viral infections.Material and methods: The main source of information on mortality in 2020 is data of the State Statistics of Mortality of Rosstat. The data on mortality in 2021 are presented in the records on death cases in Federal State Information System “Unified State Register of Registration of Acts of Current State”. To evaluate the obtained data, simple (unweighted) arithmetic mean values, specific indicators, a root mean square (standard) deviation of the mean values, coefficient of variation were used (a measure of the spread of attribute values – the ratio of a standard deviation to the arithmetic-mean).Results: In 2021 17.3% of total number of all deaths had COVID-19 as an initial cause of death. 248,134 cases of those were women (58.5%), 176,116 cases were men (41.5%). The mortality rate from COVID-19 increased by an average 1.7 times upon transition from one five-year-old age group to another, 85% of those who died from COVID-19 were aged 60 years and older. In 2021 the coronavirus infection of COVID-19 contributed significantly to female mortality more, than male mortality, which is inconsistent with the data on gender structure of COVID-19 mortality in other countries, as well as with the widespread belief that men are more vulnerable to this infection.Conclusions: Growth rates of COVID-19 mortality depending on age resembles the aging rate, reflecting the biological nature of mortality. Total excess mortality is the better basis for assessment of changes in the population health during pandemics, than mortality from COVID-19 alone. The organization of systematic checks of correctness of coding the causes of death at the state registration of death is necessary.Restrictions: Data of the Federal State Information System “Unified State Register of Registration of Acts of Current State” can differ from data of Rosstat which in 2022 became available to professional researchers later, than data of the Federal State Information System “Unified State Register of Registration of Acts of Current State”.

Background. The population prevalence of alcohol abuse-associated drug-related diseases bears high social impact. This indicator holds special value both as a potential indirect estimator of the quality of life, availability and efficiency of drug addiction treatment, a well as parameter for qualitative prognostic models of social and economic development. The burden of alcohol-associated drug diseases is typically ambiguous in prevalence estimation, both across Russian Federation and worldwide.Objectives. A study of the alcohol abuse-associated drug-related morbidity prevalence in Krasnodar Krai for period 2000–2020.Methods. A retrospective descriptive study included legal-paper data of the “Information on Drug-Related Disorders” federal statistics survey (Form 11) of Krasnodar Krai, years 2000-2020, describing the re-registration rate of alcohol use-associated drug disorders. The inclusion criterion was an established drug-related disease among all age cohorts. The main study indicators were regional prevalence values of alcoholic psychosis, alcohol dependence syndrome and harmful alcohol use relative to gender, area and age.Results. Prevalence trends in alcohol use-related drug pathology were more favourable in Krasnodar Krai over Southern Federal District and country-wide. The decline rate among males was significantly higher (22.1-fold) vs. the female population (3.0-fold). The prevalence of alcohol use-related drug pathology remained higher in urban vs. rural areas, with higher rural vs. urban decline rates. Over the entire study period, the 40–59 years-age population was leading by the incidence of overall alcohol use-related drug pathology and, separately, of alcoholic psychosis and alcohol dependence syndrome. Highest harmful alcohol use values were registered for 20–39-year population.Conclusion. The revealed dynamics of legal-registered alcoholic drug pathology prevalence has a multifactorial origin. The changes are conditioned by improvement in the narcological aid institutional regulation within the state guarantee programme, federal and regional preventive measures, current progress in drug therapy, regional demography, as well as underreporting of alcohol-associated drug diseases due to a missing strict vertical statistic registration at the level of any-type medical institutions.

Hyperammonemia is an acute or chronic intoxication with ammonia and ammonium associated with elevated ammonia levels in serum due to either its increased production and/or decreased detoxification. Hyperammonemia can result from a variety of causes and clinically presents with unspecific signs and symptoms, including asthenia, encephalopathy, liver steatosis or fibrosis, and sarcopenia. With impaired liver function, hyperammonemia most frequently manifests in (micro)encephalopathy. Thus in case of unexpect change in mental status hyperammonemia must be excluded as fast as possible. An express method of photometric assay is informative enough to determine the ammonia levels. The following hyperammonemia classification is proposed: a) by ammonia levels (normal level: ≤ 60 μmol/L; mild (Grade 1): ≤ 100 μmol/L; moderate (Grade 2): ≤ 200 μmol/L; and severe (Grade 3): > 200 μmol/L); b) by etiopathogenesis (hereditary (congenital), functional (physiological), acquired (hepatic, extrahepatic, mixed)); c) by clinical presentation (transient, recurrent or persistent, constant (stable, without treatment), covert). Treatment for hyperammonemia is aimed at treating the primary disease and includes a diet that is restricted in animal protein but contains sufficient vegetable protein, limited physical activities, and use of intestinal non-absorbable antibiotics (rifaximin- alpha) as well as pre- and probiotics. L-ornithine- L-aspartate (LOLA) is a baseline therapeutic product administered in a number of scenarios to correct the level of hyperammonemia.

Introduction. One of the most significant issues that require close attention in the treatment and rehabilitation of patients with coronovirus infection is the analysis of the nutritional status of patients and the development of approaches to nutritional support for patients.Aim of study. Analysis of nutritional status of patients infected with COVID-19 and studying the effectiveness of specialized products dietary therapeutic and preventive nutrition during illness and recovery period.Materials and methods. A survey of 283 patients with mild and moderate severity was conducted. The survey showed a significant change in eating behavior in patients during the disease. To assess the effectiveness of nutritional support during the disease and during the recovery period, 36 individuals took a specialized product of dietary therapeutic and preventive nutrition “Detoxifying Kissel” LEOVIT DETOX daily after the diagnosis of COVID-19, both throughout the entire period of the disease, and within 2 weeks after the disease and going to work.Results and discussion. The survey showed the presence of a significant range of eating disorders in patients with coronavirus disease. It was found that in 90% of the respondents during the day, the main meal was 3 or more times. During the disease, the number of main meals decreased in 40% of individuals, and only in 4% of patients this figure increased. When using nutritional support with dietary therapeutic and prophylactic foods during the disease, patients noted a significant decrease in weakness and fatigue, temperature fluctuations, fears, anxiety, suspiciousness and other symptoms began to disappear. The continuation of the intake in the post-ovoid period of the use of nutritional support with the dietary therapeutic and preventive food product “Detoxifying Kissel” LEOVIT DETOX, contributes to a faster recovery of patients.Conclusion. The use of the dietary therapeutic and preventive food product “Detoxifying Kissel” LEOVIT DETOX is an effective method of nutritional support both during the disease and in the post-ovoid period. Long-term use of detoxification products (at least 3–6 months) after the disease is recommended.

The pandemic of a new coronavirus infection caused the need to reorganize the work of emergency medical services (EMS) due to the high contagiousness and mortality, multiple organ lesions, difficulties in diagnosis and the lack of protocols for providing medical care to patients at the prehospital stage.The purpose and objectives of the study is to assess the activities of the ambulance service in the context of a new coronavirus infection in Krasnodar.Materials and methods: data of the information base of the integrated automated system for managing the activities of the ambulance station SBIHC KEH of the Ministry of Health Care of the Krasnodar Region in Krasnodar.Results. А differentiated approach and algorithm was developed for assessing the severity of the condition in patients with COVID-infection and ARVI, their routing and organization of treatment in Krasnodar.Conclusion: an assessment of the work of the ambulance service in combination with the measures taken during the observation period will ultimately allow organizing high-quality medical care to the population.

Background . Cancer care involves a multidisciplinary approach to diagnosis and treatment of patients. A complex interaction of actors in the deployment of oncology services dictates usage of modern management technologies for improving the quality and efficiency of patient care through processes optimisation. Objectives. Provision of recommendations for patient routing based on research into the oncology service inter-level information exchange, integration of the process approach and electronic services. The lack of a unified information space with a regional oncology service has been explained. Measures are proposed for the patient routing optimisation as part of establishing a unified digital oncology service circuit. Methods . The oncology service was effectively remodelled through functional and information engineering of electronic services and the process management integration to establish a horizontal decision flow between facilities and employees on a process level bypassing the supervisor coordination. Statistical approaches were used to analyse the oncological patient population. Results . The following electronic services have been implemented: specialised patient referral routing, telemedicine and teleradiology. A comprehensive information framework has been created comprising medical, laboratory and radiological information subsystems integrated through regional electronic services of the unified state healthcare information platform. The goals, objectives, general principles, architecture and expected social economic impact on healthcare of Krasnodar Krai have been defined. Conclusion . Use of electronic services ensures an improved quality of specialised care and effective routing of patients. We perceive prospects of the integrated information platform in the extension and improvement of its subsystems’ functionality and content, sourcing more data providers and the circuit expansion to the federal and regional levels.

The purpose of this study was to analyze results of simultaneous septoplasty and functional endoscopic sinus surgery in children. The study included 341 children who underwent endoscopic functional sinus surgery. The average age of patients was 11.2 ± 0.4 years. The follow – up period was 33.06 ± 6.9 months. Endoscopic septoplasty was performed simultaneously with FESS (functional endoscopic sinus surgery) in 72 children (21.1%). A comparative analysis of the results of FESS and endoscopic septoplasty showed a statistically significant association for 15 indicators on the SNOT 20 scale (p < 0.05). Significant differences were found both in general symptoms: sleep disturbance, cough, irritability, dizziness, decreased concentration, fatigue, emotional depression, sadness, grief, and local: nasal obstruction, post-nasal drip, hyposmia, mucosal edema, secretory otitis media, rhinorrhea, dry throat. Correlation analysis of the results of FESS and endoscopic septoplasty in relation to the child’s age did not reveal statistically significant differences: chi2 = 3.42 p = 0.33. The results of the study showed the efficacy of single-stage septoplasty when performing endoscopic rhinosinus surgery in childhood in improving local and general symptoms and a low degree of complications in the postoperative period.

To study the microstructure of intrahemispheric connections between cortical areas of different hierarchical levels, horseradish peroxidase was microionophoretically injected into the vertical columns of areas 19, 21a. Retrogradely labeled cells in areas 17 and 18 were identified and reconstructed on the unfolded cortical surface. It was found that the columns of areas 19, 21a, in addition to the inputs from the zones of contralateral visual hemifield projection in areas 17, 18, also receive afferentation from the transition zone 17/18, which partly represents ipsilateral hemifield. The distance between these zones of afferentation depended on the distance of the injected column from the projection of the central vertical meridian of the visual field and coincided with the visual projection maps of areas 17, 18. Revealed changes in the position of afferent cells in the transition zone 17/18 were also dependent on the azimuth of the columns. It indicates the transition zone 17/18 division into two retinotopically ordered subzones related to areas 17 and 18. It is assumed that the convergence of inputs from areas 17, 18 makes it possible for the neurons of areas 19, 21a to tune for more complex features in the configuration of images and the spatial location of objects in comparison with neurons in areas 17, 18.

The tissue reaction of pig skin to implantation of decellularized and recellularized dermal matrices on a formed wound defect was evaluated by histological methods on days 2, 5, 8, 16, and 20 after surgery. Differences in tissue response to different matrices were identified. In experimental wounds coated with decellularized dermal matrices, we observed the formation of a scar tissue, which required autodermoplasty on day 12 of the experiment. In wounds coated with recellularized dermal matrices, all layers of the skin completely recovered by day 20 after surgery with the formation of full dermal and epidermal layers. Our findings suggest that reparative morphological changes in the wound depend on the presence of fibroblasts in the implanted dermal matrix.

Study of postimplantation immune response to decellularized matrices has great importance for assessing biocompatibility of tissue-engineered structures based on them, since inflammatory process and excessive production of inflammatory mediators lead to complications and implant rejection. The aim of this research: serum cytokine profile studying after subcutaneous implantation of decellularized esophagus matrix in rats. Experimental data were obtained on male Wistar rats aged 5-6 months (n=55). Rats were divided into 4 groups: two control groups, experimental and comparison group. Control group 1 consisted of conditionally healthy rats (n=10), control group 2 - shame-operated animals (incision in scapula without implantation, n=15). In experimental group (n=15), rats underwent subcutaneous implantation of decellularized esophagus fragments; in group 2 (n=15) - native esophagus fragments. Peripheral blood sampling and fragment explantation were performed on 7th, 14th and 21st experimental days. Serum samples were tested for IL1a, IL2, IL4, IL17A, TNFa, IFNy, GM-CSF content by ELISA. Explanted native esophagus and decellularized esophagus fragments were subjected to histological analysis. On 7th experimental day, significant increase in IL1 a content was observed in rats with implantation of decellularized esophagus fragments. IL17A, IFNy, GM-CSF content significantly decreased. On 14th day, IL17A concentration sharply decreased in comparison with value on 7th experimental day and control 1. IL1 a and IFNy concentration decreased in comparison with control group 1 values and 7th day respectively. On 21st day, dynamics of decrease in IL17A, IFNy, IL1 a content in this rat group was revealed. Thus, it was found change in concentrations of studied cytokines corresponds to regeneration histomorphological picture in group that underwent implantation of acellular matrices against of active inflammatory reaction in comparison group. Concentrations of IL1 a, IL4, IL17A, IFNy reflect positive dynamics of wound healing process and absence of decellularized matrix rejection.

Sore throat is a common complaint among children and adults. In addition to infection, causes of sore throat include non-infectious pathological conditions. Among possible causes are viruses, bacteria, fungal flora. Identification of etiology significantly affects the disease management. The prescription of systemic antimicrobial agents is justified, if bacterial etiology is confirmed or a child develops complications; the presence of β -hemolytic streptococcus is an absolute indication for antibiotic therapy. Increasingly greater attention is now paid to topical agents in the treatment of bacterial tonsillopharyngitis, in particular, to the drugs that combine the antiseptic and antibiotic properties. The article provides data on the use of a combination drug containing antibacterial substance gramicidin S and antiseptic component cetylpyridinium chloride that proved its efficacy, safety, and low risk of side effects in pediatric and adult patient population. The authors present their own clinical observations of the successful use of this combination drug in paediatric practice. Clinical case 1 . A 15-year-old child K. with a polyvalent drug allergy to antibacterial drugs. Moderately grave condition, subfebrile temperature 37.8 oC; submandibular lymph nodes increased up to 2.0 cm with expressed tenderness; preliminary diagnosis is acute tonsillopharyngitis; Infectious mononucleosis?». Pharyngoscopy showed bright hyperemia of edematous mucous membrane, hyperemia of enlarged palatine tonsils, visualized white plaque; single vesicular rashes on the anterior palatine arches and uvula. The patient received active etiotropic, pathogenetic and symptomatic therapy. Instead of systemic antibacterial therapy, a combination drug based on gramicidin S and antiseptic component cetylpyridinium chloride was chosen. The patient was discharged from the hospital on Day 10 of hospitalization, the child had no complaints, the general condition returned to normal. Clinical case 2 . A 6-year-old child was diagnosed with acute pharyngitis, degree II palatine tonsils hypertrophy, degree II nasopharyngeal tonsil hypertrophy, submandibular and cervical lymphadenitis. The laboratory test results showed absence of β -hemolytic streptococcus. The combination drug based on gramicidin S and antiseptic component cetylpyridinium chloride was prescribed, the pharyngotonsillitis symptoms disappeared on Day 7 of observation. Clinical observations demonstrate the possibility of using the combination drug for the complex treatment of acute tonsillopharyngitis of any etiology, including those that develop in childhood.

Relevance. To study the incidence of different types of resorption of multirooted primary teeth, to specify indications for deciduous molar extraction to prevent eruption abnormalities of permanent posterior teeth in mixed dentition.Materials and methods. Root resorption of 375 multirooted primary teeth (166 first primary molars and 209 second primary molars) was studied on panoramic X-rays of 60 children (30 girls and 30 boys) aged between 7 and 15. Illustrated classification by T.F. Vinogradova (1967) improved by authors was used to determine type and degree of root resorption of multi-rooted primary teeth. Received data were described with absolute values of number of cases and percentage. Chi-square was used to detect differences in sign incidence rate between groups, pResults. There were no statistically significant gender differences (p>0,05) in type and degree of root resorption of multirooted primary teeth. Type A resorption prevailed and constituted 53.3% of all primary molars. Disturbances in root resorption of multirooted primary teeth in mixed dentition were related to health condition of primary teeth. Transition of even resorption to unven was considered a risk factor of delayed eruption and aberrant position of permanent teeth, and indication for extraction of a primary molar in question. Conclusions. 1) Even root resorption (type A) was detected in 53.3% of primary molars in mixed dentition by orthopantomography. 2) Transition from even resorption of primary molar roots to uneven resorption was associated with eruption deviations and delayed premolar eruption. 3) Timely extraction of primary molars with uneven root resorption facilitated correct eruption of premolars and increased effectiveness of secondary prevention of malocclusion in children.

The academic potential of the Kuban State Medical University has been formed as a result of close interaction between university and academic science. Such collaboration encourages the development of highly qualifi ed academic and teaching staff, increases the importance of conducting fundamental studies and forms an information basis for boosting research and educational processes in the university. Over a century, academicians and corresponding members of the Russian Academy of Sciences (RAS) have contributed to the development of the Kuban State Medical University by taking part in fundamental research projects and creating scientifi c schools. The Kuban State Medical University has received ongoing intellectual support from representatives of academic science both in the formation of long-term developmental strategies and implementation of individual scientifi c projects. The collaboration between RAS and the university has been carried out under different socio-economic and political conditions. The Kuban State Medical University will continue implementation of fundamental research projects in close interaction with the Russian Academy of Sciences (RAS) in the foreseeable future. This article presents a brief biography of 15 RAS academicians and corresponding members having made a signifi cant contribution to the development of the Kuban State Medical University, which is celebrating its 100th anniversary this year.

This paper presents an experimental study of diffusion jet combustion of gas at different flow rates of CH4–N2 and a constant flow rate of coal. Blow-off curves for a diffusion flame for different nozzle diameters are obtained. It is shown that an increase in the the nozzle diameter causes a decrease in the minimum residual flow rate of the fuel gas at which stable combustion is possible. As coal is supplied, the residual flow rate of the fuel gas also becomes slightly lower. The fluorescence of OH, which makes it possible to analyze the position and dynamics of the flame front is recorded.

The results of diagnostics and complex treatment of dentoalveolar anomalies during the early shift bite in children living in the city of Krasnodar and the territories of the Krasnodar Territory with different levels of socio-economic development and the effectiveness of the health care system were studied. The prevalence and structure of dentoalveolar anomalies is established, the risk factors for their occurrence and progression are specified. Comprehensive rehabilitation and correction of the functional state of the dentoalveolar apparatus led to a noticeable change in the structure of the RFA in children who turned during the period of an early shift bite.

Aim. To evaluate the level of staffing by general practitioners (GP) in medical organizations in Krasnodar Krai.Materials and methods. Indicators characterizing the level of staffing by GPs in Krasnodar Krai hospitals were analysed using data from the state statistical observation form No. 30 “Information about a medical organization” for the 2009–2019 period. According to the data derived from the Krasnodar Krai Patient Register as of 01.01.2020, all GPs were divided into groups in terms of the ratio of the number of served population and the recommended values. Using the data of the Regional Register of Medical Workers as of 01.01.2020, the age composition of GPs was analysed.Results. As of 01.01.2020, Krasnodar Krai featured 228 medical organizations subordinate to the Krasnodar Krai Ministry of Healthcare. Of these, 58 have full-time GP positions. The overall number of GP positions comprises 294, which are currently taken by 198 specialists (1.1% of the total number of doctors and 1.9% of doctors providing medical care on an outpatient basis). Over the studied period, stable indicators of the provision of the Krasnodar Krai population by GPs were noted; however, the level of staffing by GPs in hospitals was decreasing. This indicates a growing trend in the lack of GPs. An analysis of the actual load of 198 GPs showed that, on average, 1 GP serves 1219 people. For 26.8% of GPs, the number of served population increased the recommended values by 1.5 times or more. As a result, over 100,000 adults in the region are served under the conditions of excessive workload on GPs. The average age of GP is 50.1 ± 0.9 years. Currently, more than 25% of GPs are working retirees by age. In the medium term, this value may exceed 50%. Considering specialists who are about to retire, the estimated need for GPs is 146 doctors.Conclusion. In order to provide affordable and high-quality primary health care to the population in Krasnodar Krai, the number of GPs should be increased. In addition, measures should be taken to raise the attractiveness of work in Krasnodar Krai hospitals.

Tonsillopharyngitis is a common disease among pediatric patients. Visits associated with sore throat account for 6 to 8% of all visits to pediatricians and general practitioners. Tonsillopharyngitis can have both infectious and non-infectious etiologies. In pediatric practice, most cases of upper respiratory tract infections are of viral origin, especially in children of early and preschool age. In most cases, uncomplicated course and non-infectious etiology of the disease do not require the administration of antibacterial drugs. The bacterial nature of the disease can be distinguished from the viral one using the data of culture analysis and rapid test for group A β-hemolytic streptococcus (GABHS) or Streptococcus pyogenes . The etiotropic therapy of streptococcal tonsillopharyngitis is antibiotic therapy. In GABHS, antibiotic therapy shortens the duration and reduces the severity of symptoms, reduces the Likelihood of purulent complications, post-streptococcal diseases and prevents the generalization of streptococcal infection. The use of modern antiseptic herbal medicines may be an additional and alternative line of therapy. The authors presented a literature review of the clinical trials results of a modern herbal medicine and provided the results of their own case of the combined treatment of a 10-year-old child with a diagnosis: acute moderate tonsillopharyngitis, bilateral cervical lymphadenitis; acute rhinitis; chronic adenoiditis, exacerbation; degree 2 adenoids. Antibacterial therapy included amoxicillin: oral: 100 mg/kg/day in 2 divided doses for 10 days, combined with an anti-inflammatory and antiseptic herbal medicine due to the complicated course of the disease in the form of concomitant regional lymphadenitis. The course of therapy resulted in clear positive changes: overall well-being improved as early as on the second day of treatment, sore throat and submandibular region ceased to cause pain by 5th day of treatment, the temperature became normal by 5th day of the disease. This clinical observation shows the possibility to introduce an anti-inflammatory herbal medicine to the complex treatment of acute tonsillopharyngitis in children, including cases of complicated course of the disease.

Despite advancements in modern surgery in the treatment of cutaneous injuries, the search for new methods of ensuring faster and more effective wound healing appears especially urgent today. Tissue engineering is undoubtedly of interest when it comes to developing such technologies. Objective : to determine the optimal protocol for obtaining a decellularized dermal matrix scaffold for subsequent development of tissue-engineered skin. Materials and methods . One Landrace piglet was used as the experimental animal. After preliminary skin treatment with dermatome, 0.3 cm thick samples were taken. Two decellularization protocols were considered: protocol No. 1 was based on the use of Triton X-100 and deoxycholate, protocol No. 2 was only based on deoxycholate. There were 5 processing cycles in total for the 2 protocols. After treatment, acellular matrix scaffolds were examined through histological analysis and quantitative determination of DNA concentration. Next, static recellarization of the matrix scaffolds was carried out with porcine dermal fibroblasts. After that, the matrix scaffolds were tested for cytotoxicity using XTT test and differential staining test to differentiate between live and dead cells. Results . Comparative analysis of the two protocols for porcine dermis decellularization showed that both protocols effectively remove cells and nuclear material, while maintaining the architectonics of the intercellular substance intact, since fibrous structures are not destroyed. But when assessing the biocompatibility of matrix scaffolds based on analysis of cell viability according to data obtained from XTT test and cell–matrix adhesion, the matrix scaffold processed under protocol No. 1, shows advantages. Conclusion . In this study, a decellularization protocol based on Triton X-100 and deoxycholate was noted. The results obtained mark the first stage towards developing a tissue-engineered skin.

Background & Aims Due to the poor eradication rates of standard triple therapy, the addition of bismuth salts has been proposed for first-line eradication of Helicobacter pylori. We assessed the effectiveness and safety of the combination of bismuth and the standard, clarithromycin-containing triple therapy in eradication of H pylori infection, using data from a large multi-center registry. Methods We performed an interim analysis of data from the European Registry on H pylori Management, a prospective trial registering clinical data and outcomes from infected patients from 27 countries in Europe since 2013. We extracted data on 1141 treatment-naive patients who received first-line treatment with bismuth salts (240 mg) and a proton pump inhibitor (57% received esomeprazole, 18% received omeprazole, 11% received pantoprazole, and 14% received rabeprazole), amoxicillin (1 g), and clarithromycin (500 mg), all taken twice daily. Results Intention to treat and per-protocol rates of eradication were 88% and 94%, respectively. Intention to treat eradication increased to 93% in patients who received 14-day treatments. Adverse events occurred in 36% of patients; 76% of these events were mild, with a mean duration of 6 days. In multivariate analysis, eradication was associated with treatment compliance (odds ratio [OR], 13.0), a double dose (equivalent to 40 mg omeprazole) of proton pump inhibitor (OR, 4.7), and 14-day duration of treatment (OR, 2.0). Conclusions In an analysis of data from a large multi-center registry, we found the addition of bismuth to 14-day standard triple therapy with clarithromycin and amoxicillin to eradicate H pylori infection in more than 90% of patients, based on intention to treat analysis, with an acceptable safety profile and level of adherence. Forty-seven patients with overlap between irritable bowel syndrome with constipation and (IBS-C) and postpradial distress syndrome (PDS) complying with Rome III criteria were treated with pinaverium in combination with itopride and 58 patients with IBS-C-PDS overlap were treated with pinaverium for 4 weeks. Clinical symptoms and quality of life by SF-36 were estimated at baseline and after 4 weeks of treatment. Pinaverium and Itopride combination therapy significantly improved both IBS-C and PDS-like symptoms, and SF-36 scores when compared to Pinaverium monotherapy over a 4-week period.

The paper describes the design of an experimental setup for plasma gasification of organic waste with its discrete supply to the loading unit. The experimental results of determining the percentage composition of synthesis gas and its calorific value depending on time for gasification of wood sawdust and polyethylene granules are presented. The amount of methane in synthesis gas composition has been calculated depending on temperature and oxygen flow rate. It is shown that when the waste is fed in separate portions, the synthesis gas composition, its yield and calorific value change over time significantly.
