Background and Aim. Tracheal stenosis is a common complication of intubation with or without subsequent tracheotomy whose management remains poorly defined. Over 600 post-intubation tracheal stenoses have been treated in our centre since 1982: the aim of this study was to determine the safety and efficacy of our endoscopic approach. Methods. This retrospective study includes 113 new cases treated between 1998 and 2001. We chose this period to have a standardised technique and a significant number of cases with a sufficiently long follow up (28-78 months). Forty patients who did not meet the criteria for “true stenosis” (granulomas, pseudoglottic stenosis, etc.) were excluded from the study. Results. 73 patients (50±21 years) entered the study: 13 (18%) web-like and 60 (82%) complex stenoses. Most web-like stenoses were successfully treated with Laser Assisted Mechanical Dilation (LAMD) alone; among complex stenoses LAMD was sufficient to treat 13 patients (22%), whereas 47 patients (78%) required stent placement: 22 had their stent removed after one year and did not require any further therapy, 13 inoperable patients required permanent stent and 12 were referred to surgery after failure of multiple endoscopic treatments. No permanent complications secondary to endoscopic treatment were observed. 48 patients (66%) obtained a stable, good result with the endoscopic procedure, 13 (18%) required a permanent stent while 12 patients (16%) were referred to surgery. Conclusions. Our results indicate that the endoscopic treatment of post-intubation tracheal stenoses performed in an expert setting can be considered a safe first-line therapy, leaving some selected cases and the relapsing stenoses, for surgical resection.

Patients with severe mental illness are falsely characterized as aggressive by the media, perpetuating stigma. While exaggerated, some patients with severe mental illness are more aggressive without treatment. Clozapine may have a unique anti-aggressive effect in patients with schizophrenia-related disorders, independent of antipsychotic or sedative effects. Limited data in forensic and involuntary committed patients is currently available.This study evaluates clozapine's effects on hostility and aggression in court-ordered Black patients.This study analyzes a subgroup of Black patients from a larger prospective 24-week open-label clozapine study. All patients were involuntarily committed and enrolled from two participating state psychiatric hospitals. The primary outcome measured was total use of 'as needed' (PRN) or 'immediate need' (STAT) medications for aggression/hostility. Secondary outcomes included number and duration of seclusion and restraint (S/R) episodes, and changes in Brief Psychiatric Rating Scale (BPRS) hostility factor score.Sixty-nine patients were included in our analysis. Significant reductions were noted in PRN/STAT medication use over time (χTreatment with clozapine effectively reduced hostility and aggression within this cohort of involuntarily committed Black patients with mental illness compared to baseline. Specifically, it helped lower the total number of PRN/STAT medication administrations and improved clinician-rated hostility factor scores on the BPRS. Our findings are pertinent as data in forensic settings is lacking and Black patients have been infrequently included in large prospective clinical trials with clozapine.NCT02404155.

Nanostructured thin films have attracted a considerable interest due to their unique size-dependent physicochemical properties, which are appealing for a multitude of applications. A thin film is traditionally defined as a material layer ranging from few nanometers to several micrometers in thickness. Due to their width, thin films are often used to coat surfaces or layers previously deposited. However, recent technological breakthroughs have enabled the development of freestanding thin layers, extending the application perspectives. Depending on both the target applications and the deposition technique used, nanostructured films can be designed and developed by tuning their atomic-molecular 2D and/or 3D aggregation, thickness, crystallinity, and porosity, as well as their optical, mechanical, catalytic, and conductive properties. The resulting structures can be exploited in electronic devices, electronic semiconductor devices (superconductive, semiconductive, magnetic, thermal, and insulating layers), energy storage and conversion (battery, solar, and fuel cells), optical and protective coating, and functional tools (catalysts, sensors, and biosensors). This book contains 27 scientific articles covering most of the recent advances in designing and preparing nanostructured films for target functions and applications, focusing on the improvements resulting from adjustments of the fabrication processes and the materials employed. The emphasized advances in thin film growth technologies here reported are noteworthy because they addressing environmental issues such as the use of eco-friendly materials and procedures, more efficient adsorption/detection systems, alternative energy applications, and removal of pollutants or toxic substances. To conclude, this book reports an overview on the design, synthesis, and applications of thin films of different natures and nanostructures, highlighting the latest advances as well as the challenges to be tackled, especially from an environmental standpoint. We hope that it can be useful and inspiring for a vast audience from academia and industry.

Proton exchange membrane fuel cells (PEMFCs) and water electrolysers (PEMWEs) are electrochemical devices for energy conversion and storage at the base of the sustainable and clean hydrogen economy. In order to face the challenges of durability under operation and cost that are slowing down their real implementation, novel micro/nanostructured materials based on electrospun fibers have been developed. Several chemical compositions and morphologies have been employed to prepare nanocomposite membranes and electrodes comprising the core of PEMFCs and PEMWEs. This chapter gives an overview on the recent advances on electrolyte membranes, electrodes, electrocatalysts and their supports based on electrospun polymers and inorganic materials applied in low-temperature fuel cells and electrolysers. Novel challenges to overcome and future trends in the development of these promising materials are also discussed.

ARGO-YBJ is a “full coverage” air shower detector consisting of Resistive Plate Chambers(RPCs) at the Yangbajing High Altitude Cosmic Ray Laboratory (Tibet, China) at 4300 m a.s.l. (lat=30.11° N, long=90.53° E). Using the data collected with a carpet of RPCs (1900 m 2 , about 1/3 of the whole ARGO-YBJ detector), the cosmic ray shadowing effect due to the Moon was studied. The 50% angular resolution is found to be ∼1.2° with the Chess-board method and the Moon shadow with a significance of 4.9 σ is found displaced by 0.7° westward and 0.5° northward with respect to the expected position by the equi-zenith angle Method.

The ARGO-YBJ experiment has been designed to detect air shower events over a large size scale and with an energy threshold of a few hundreds GeV. The building blocks of the ARGO-YBJ detector are single-gap Resistive Plate Counters (RPCs). The trigger logic selects the events on the basis of their hit multiplicity. Inclusive triggers as well as dedicated triggers for specific physics channels or calibration purposes have been developed. This paper describes the architecture and the main features of the trigger system.

The ARGO-YBJ experiment is designed for the detection of Extensive Air Showers (EAS) in the primary energy range 100 GeV–10 TeV. Its full-coverage feature allows to lower the energy threshold about one order of magnitude with respect to sampling EAS detectors. Here the performance of the RPC array is described and the current status of the experiment is presented.

Abnormal electrophysiological activity in the striatum, which receives dense innervation from the cerebral cortex, is believed to set the stage for the behavioral phenotype observed in Huntington's disease (HD), a neurodegenerative condition caused by mutation of the huntingtin (mhtt) protein. However, cortical involvement is far from clear. To determine whether abnormal striatal processing can be explained by mhtt alone (cell-autonomous model) or by mhtt in the corticostriatal projection cell–cell interaction model, we used BACHD/Emx1–Cre (BE) mice, a conditional HD model in which full-length mhtt is genetically reduced in cortical output neurons, including those that project to the striatum. Animals were assessed beginning at 20 weeks of age for at least the next 40 weeks, a range over which presymptomatic BACHD mice become symptomatic. Both open-field and nest-building behavior deteriorated progressively in BACHD mice relative to both BE and wild-type (WT) mice. Neuronal activity patterns in the dorsal striatum, which receives input from the primary motor cortex (M1), followed a similar age progression because BACHD activity changed more rapidly than either BE or WT mice. However, in the M1, BE neuronal activity differed significantly from both WT and BACHD. Although abnormal cortical activity in BE mice likely reflects input from mhtt-expressing afferents, including cortical interneurons, improvements in BE striatal activity and behavior suggest a critical role for mhtt in cortical output neurons in shaping the onset and progression of striatal dysfunction.

⇓The purpose of this European Respiratory Society/American Thoracic Society (ERS/ATS) Task Force position paper is to define the field of interventional pulmonology as well as to describe the minimal requirements in methodology and logistics, the indications, contraindications, outcome and training requirements for each interventional procedure. This position paper has been prepared by a group of European and North American experts. The ARGO-YBJ experiment has two kinds of signals in the shower working mode which allows coverage of the energy region from TeV to PeV region. One is the digital strip pattern, another is so-called ‘big pad’ mode, which is the analog signal counting the pulse height on half of an RPC, proportional to the number of hitting particles. In this paper by using the Monte Carlo simulation method the ARGO-YBJ sensitivity to the cosmic ray composition is discussed, by using the ‘big pad’ signal for measuring the number of particles detected close to the shower core.

In ground-based cosmic ray experiments, the equalization of the transit time of signals coming from different parts of the apparatus is of crucial importance for the improvement of the angular resolution and accuracy. In the ARGO-YBJ experiment, this is achieved using the Characteristic Plane Method with conical correction, studied by both Monte Carlo simulation and real data and also checked with manual absolute calibration on a portion of the detector. By introducing conical correction in primary direction reconstruction, the systematics error existing in the off-line calibration with planar fit is successfully removed. Two subsequent construction phases of the detector have been considered: ARGO-42 and ARGO-104. During the calibration of ARGO-104, events with more than 1000 hits and with the core reconstructed inside the carpet were used in order to achieve a good reconstruction of the shower, that is the arrival direction and the core position. The results of the calibration concerning the two configurations, ARGO-42 and ARGO-104, are compared and discussed.

Within a Collaboration Agreement between INFN and CAS (Chinese Academy of Science), the ARGO-YBJ experiment is completely installed at the YangBaJing Cosmic Ray Laboratory (4300 m a.s.l., Tibet, P.R. China). ARGO-YBJ is a plane detector, with a total detection area of ∼ 6500 m 2 , made of Resistive Plate Chambers (RPCs) which provide a detailed space-time picture of the charged component of the extensive air showers. At present the detector is completely mounted and the central carpet of 5800 m 2 is operating under a multiplicity trigger N p a d ⩾ 20 , the trigger rate is about 3.5kHz and the data flow is 5MB/s. A 0.5 cm lead converter will be mounted on the detector in summer 2007.

The ARGO-YBJ experiment is a large area full coverage detector operating at high mountain altitude (4300 m a.s.l., Tibet, China, ∼6700 m2 active surface). In scaler mode, the particle rate is recorded at fixed time intervals (0.5 s), allowing the achievement of an energy threshold as low as E t h ∼ 1 GeV for the photons. This result, together with the estimate of the effective area as a function of the primary energy, has been obtained using the simulation code CORSIKA6201 for the shower development in the atmosphere and a dedicated and detailed detector simulation, ARGOG, based on GEANT3.21. With this simulation chain, and using the known primary proton and helium spectra, we obtained counting rates consistent with the measured ones, making us confident in the expectation of the detector performance as far as the photon shower detection is concerned, which is of crucial importance to determine the capability of the ARGO-YBJ experiment to detect gamma-ray bursts (GRBs).

The ARGO-YBJ detector is an extensive air shower array consisting of a carpet of Resistive Plate Chambers (RPCs). Very high energy (VHE) gamma ray astronomy is the main scientific goal of this Chinese-Italian experiment. High altitude and full coverage ensure a low energy threshold (few hundreds of GeV for primary photons), high duty-cycle and large field of view allow a continuous sky survey in the declination range from −10° to +70°. Also many features of the high energy cosmic rays are studied by ARGO-YBJ exploiting the unprecented shower reconstruction.

The ARGO-YBJ experiment has been designed to decrease the energy threshold of typical Extensive Air Shower arrays by exploiting the high altitute location (Tibet, P. R. of China, 4300 m a.s.l.) and the full coverage.The lower energy limit of the detector (a few GeV) is reached with the single particle technique recording the counting rate at fixed time intervals. We present first results concerning the search for emission from Gamma-Ray Burst in coincidence with satellite detection.

The ARGO-YBJ experiment is currently under construction at the Yangbajing Cosmic Ray Laboratory (4300 m a.s.l.).The detector consists of a central carpet, 74 × 78 m2, made of a single layer of Resistive Plate Counters (RPCs), and surrounded by a partially instrumented guard ring for a total instrumented area of about 6700 m2. The digital read-out, performed by means of pick-up electrodes 6.7 × 62 cm2 (strip), allows to measure the charged particle number of small size air showers. The technique of counting the number of fired strips on the ARGO carpet corresponds to operate in the 10÷500 TeV energy region where both direct and indirect measurements on the primary cosmic radiation have been performed. Many composition models have been proposed by different experiments. In this work we discuss the ability of the ARGO detector to discriminate among some models.

This report refers to a case occurring in a young woman and presenting as a persistent airway obstructive syndrome. With regard to histopathologic characterization, the present case can be classified as a fibrous histiocytic subtype.

Here we report on the results concerning absorbed current, single counting rate, efficiency and time resolution. Environmental data concerning the operating temperature inside the ARGO experimental hall are also reported.

An RPC carpet covering similar to 10(4) m(2) (ARGO-YBJ experiment) will be installed in the YangBaJing Laboratory (Tibet, People's Republic of China) at an altitude of 4300 m a.s.l. A test-module of similar to 50 m(2) has been put in operation in this laboratory and about 10(6) air shower events have been collected. The increasing number of intubations and percutaneous tracheotomies performed, together with the extended indication towards invasive mechanical ventilation, the prolonged time of survival and, as a consequence, increasing number of days spent in Intensive Care Units have determined an enlarged number of traumas of the upper airways at the bases of the increasing incidence of tracheal stenoses. About 2-3% of patients who undergo intubations and/or tracheotomy develop tracheal stenosis [1] (Level of evidence: IIb).

We have developed a facile synthesis route to 1D structures of ultrathin polycrystalline hematite and goethite-hematite core-shells. One-dimensional structures of pure hematite and the goethite-hematite core-shell with very small diameters were synthesized in aqueous solution at low temperature with a simple, rapid method based on the oxidation of Fe3O4 nanoparticles without using surfactants.

Background: The potential of autofluorescence bronchoscopy (AFB) to detect precancerous lesions in the central airways and its role in lung cancer screening is uncertain. A study was undertaken to evaluate the prevalence of moderate/severe dysplasia (dysplasia II–III) and carcinoma in situ (CIS) using a newly developed AFB system in comparison with conventional white light bronchoscopy (WLB) alone. Methods: In a prospective randomised multicentre trial, smokers ⩾40 years of age (⩾20 pack-years) were stratified into four different risk groups and investigated with either WLB+AFB (arm A) or WLB alone (arm B). Results: 1173 patients (916 men) of mean age 58.7 years were included. Overall (arms A and B), preinvasive lesions (dysplasia II–III and CIS) were detected in 3.9% of the patients. The prevalence of patients with preinvasive lesions in the WLB arm was 2.7% compared with 5.1% in the WLB+AFB arm (p = 0.037). For patients with dysplasia II–III, WLB+AFB increased the detection rate by a factor of 2.1 (p = 0.03), while for CIS the factor was only 1.24 (p = 0.75). The biopsy based sensitivity of WLB alone and WLB+AFB for detecting dysplasia II–III and CIS was 57.9% compared with 82.3% (1.42-fold increase). The corresponding specificity was 62.1% compared with 58.4% (0.94-fold decrease). Conclusions: This first randomised study of AFB showed that the combination of WLB+AFB was significantly superior to WLB alone in detecting preneoplastic lesions. Our findings do not support the general use of AFB as a screening tool for lung cancer, but suggest that it may be of use in certain groups. The precise indications await further study.

Laser bronchoscopy is a usually well tolerated procedure for the treatment of obstructive lesions on the tracheobronchial tree, with a very low morbidity and mortality rate. Cardiovascular complications, including atrial and ventricular arrhythmias, and myocardial ischemia, have only rarely been reported during laser bronchoscopy. Cardiac arrhythmias during such a procedure are usually well tolerated but occasionally may be life threatening. Here we report a case of a young, female patient affected by Pulmonary Tuberculosis with a cicatricial stenosis of the left main bronchus who developed an episode of prolonged cardiac arrest due to ventricular fibrillation (and no signs of acute myocardial ischemia) during rigid broncoscopic laser-therapy. Underlying Coronary Artery Disease and other cardiac abnormalities were also excluded by subsequent cardiovascular examination. The clinical implications are also discussed.

In this paper we consider an extension of the wavelet transform leading to the construction of wavelets with arbitrary bandwidth. The new wavelets are complete, orthonormal and dyadic; nevertheless their bandwidth is not constrained to be one octave, rather it may be designed by selecting a set of parameters. The construction of the new bases starts in the discrete-time domain, exploiting properties of the Laguerre transform. Furthermore, we provide a procedure to define continuous-time warped wavelets. The flexibility of the bandwidth allocation allows for more and improved applications of the wavelet transform, such as signal coding, the design of auditory model based filterbanks and transient detection in pseudoperiodic signals, pointed out in the paper.

We show that the dyadic wavelet transform may be generalized to include non-octave spaced frequency resolution. We introduce orthogonal and complete wavelets whose set of cutoff frequencies may be adapted, in the simplest case, by changing a single parameter. The novel wavelets and the FWWT transform computational structure are obtained via an intermediate Laguerre representation of the signal. The warped wavelets are related to the ordinary wavelets by means of frequency transformations and orthogonalizing filtering. The classical sampled filter bank theory is extended to include frequency dependent upsampling and downsampling operators and dispersive delay lines. The FWWT frequency band flexibility may be exploited in order to adapt the wavelet transform to signals.

Few and conflicting data are available regarding the changes of plasma potassium concentration during acute respiratory acidosis in human beings. This study compares the acute changes in plasma potassium concentration in acutely hypercapnic patients and in non-hypercapnic patients during general anaesthesia.Thirty-three patients undergoing interventional rigid bronchoscopy were studied. Ventilation of the lungs was randomly conducted using either spontaneous-assisted ventilation or intermittent negative-pressure ventilation. All patients received the same anaesthetic protocol. Arterial blood gases and osmolality, and plasma concentrations of glucose, sodium, potassium and chloride were measured.Intraoperatively, PaCO2 was higher during spontaneous-assisted ventilation than during intermittent negative-pressure ventilation (9 +/- 1.8 vs. 5.4 +/- 1.2 kPa, P0.001) and the pH was also lower during spontaneous-assisted ventilation than during intermittent negative-pressure ventilation (7.24 +/- 0.07 vs. 7.4 +/- 0.08, P0.001). Plasma potassium concentration remained similar in both groups (3.8 +/- 0.2 mmol L(-1) with spontaneous-assisted ventilation vs. 3.7 +/- 0.4 mmol L(-1) with intermittent negative-pressure ventilation).Acute respiratory acidosis does not affect plasma potassium concentration.
