Nepalese agriculture insurance market is dominated by livestock sector and cattle are the most popular class of animal in livestock production system. This study was conducted to identify the determining factors and impact of household income on dairy cattle insurance. This study was based on the field survey conducted in Chitwan and Nawalparasi districts of Nepal. A total of 160 households were sampled using a stratified simple random sampling technique. Data were collected using a pre-tested interview schedule and focus group discussion (FGD) in 2017. The result of t-test showed annual household income, income from milk sell, and average cattle holding significantly higher in cattle insurer farmers than that of non-insurers. A probit model was used to assess the factors affecting the decision for the adoption of cattle insurance. The probit model revealed that cattle breed, access to loan, income from livestock, and number of cattle had a positive and significant relationship with the adoption of cattle insurance whereas, household size and district dummy had a negative and significant relationship with the adoption of cattle insurance. Income regression function revealed that the adoption of insurance, the number of cattle, and the district of the respondents had a positive and significant relation with income from livestock. The instrumental variable model adopted to combat endogeneity bias showed that adoption of insurance and district of the respondents had a positive and significant relation with income from livestock. Although the results are farmers specific, findings have implications for policymakers regarding promotion of improved breeds and facilitation of farm credit.

Fourteen different maize genotypes were screened for their relative resistance to maize stem borer Chilopartellus (Swinhoe) under glass house condition at National Maize Research Program, Rampur, Chitwan during two consecutive years 2018 and 2019. The design of the experiment was completely randomized design having 2 replications. The unit plot size was one row of one meter length with 50 cm row to row and 20 cm plant to plant spacing. Two second instar larva of C. partellus (Swinhoe) per plant were released two times, first at 8th leaves (vegetative) and second at 12th leaves (Before tasseling) stages. Insect damage parameters (IDPs) viz. dead heart percentage, damage % at vegetative and foliar stage, foliar injury rating (1-9) at vegetative and before tasseling stage, tunnel length (cm), exit hole number and yield per plot were recorded to study the germplasm susceptibility level. The result revealed that IDPs and yield varied significantly among the tested genotypes. Over the years, IDPs like dead heart percentage (5.17%), percent damage atpre-tasseling stage (10.6%), foliar injury level (2.25), tunnel length (10.15 cm) and exit holes (2.32) were found lower with high yield (2235 g/plot) in RML-95/ RML-96 followed by RML- 86/RML-96. Two pipeline maize hybrids RML-95/RML-96 and RML-86/RML- 96 with least damaged by maize stem borer in both years will be useful for maize stem borer resistance source. Bangladesh J. Agril. Downy mildew caused due to Peronospora fagopyri is the most important fungal pathogen that contributed to significant yield loss in buckwheat genotypes. Efficacy of six fungicides namely SAAF (Carbendazim 12% + Mancozeb 63%) @ 3 g/l, Krilaxyl (Metalaxyl 8%+ Mancozeb 64% WP) @ 2 g/l, Bavistin (Carbendazim 50% WP) @ 2g/l, Hexazol (Hexaconazol 5% SC) @ 2 g/l of water, Dithane M-45 (Mancozeb 75% WP) @ 2.5 g/l and Cyclon (Tricyclazol 75% WP) @ 2 g/l of water against one control with three replicates were tested at Hill Crops Research Program, Kabre, Dolakha during summer season of two consecutive years 2019 and 2020 against downy mildew of buckwheat. First spray was given just after the appearance of disease symptom in the field. Two sprays were given at an interval of 10 days. Data were recorded before every spray using 1-9 scoring scale on 10 randomly tagged plants/plot. All the fungicides were found to be effective in controlling the disease. SAAF was the most effective in reducing the disease severity (≤30%) and enhancing yield, followed by Dithane M-45 when compared to the unprotected control with highest disease severity (>75%). The use of these fungicides is recommended in an integrated disease management strategy, incorporating host resistance and cultural practices.

This paper examines the impact of microfinance in agriculture and livestock in Dhading and Chitwan districts of Nepal. Municipalities within the districts were selected purposefully, and 60 respondents from each district were selected by simple random sampling method to comprise 120 respondents. The study showed significantly higher percentage of people participating in microfinance had self-sufficient food production. Probit regression analysis was done to estimate the impact of different predictor variables on farmer’s participation in microfinance. Although the production, income and gross margin of rice and wheat and gross margin of livestock was found statistically higher among the farmers participating in microfinance, the expense for rice and wheat was also found significantly higher among the farmers participating in microfinance. The result of Probit regression analysis showed six variables namely age of household head, caste/ethnicity, education of household head, agricultural credit, livestock unit (LSU), and annual household income were statistically significant for farmer’s participation in microfinance.

Background Type 2 diabetes is an escalating health problem in Nepal and it holds a strong bidirectional Inter relationship with periodontitis. However, lack of its knowledge and motivation among patients may deteriorate and complicate their condition. Objective To assess knowledge among general population regarding the relation between diabetes and periodontitis and to evaluate the role of dentists and physicians in suggesting the diabetic patients for oral health care. Method A cross-sectional study was conducted among the patients visiting Department of Periodontics, Gandaki Medical College. A total 422 patients were interviewed using a set of pretested questionnaire and at the end they were counselled about the impact of diabetes mellitus on periodontal status and vice versa. Result Majority of the patients 257 (60.9%) had no idea about the interrelation between periodontitis and diabetes mellitus. Almost all the patients 363 (86.02%) assumed that poor oral health cannot increase the risk of developing diabetes and amongst those who had knowledge about this relation, the major source of information was their diabetic friends and relatives 46 (10.9%) and other sources such as syllabus 46 (10.9%). Only handful of patients 30 (7.10%) were informed about the relation by their dentist. Ironically, none of them were provided information about the interrelation by their treating physician. Conclusion Overall, only few participants had knowledge about the bidirectional relation between periodontitis and diabetes. Hence, our findings support the greater need for more targeted and specific health education along with close collaboration between dentists and physicians.

Drought is one of the major constraints to wheat production, and it is becoming a more serious problem in many wheat-growing regions of the world. It causes a severe reduction in wheat crop growth and productivity. Water stress at critical growth stages namely tillering, grain filling and flowering stages causes serious yield loss. Morphological, physiological, biochemical, and molecular changes are seen as a result of drought stress in wheat. When plants are exposed to drought stress, they use three basic survival strategies, stress avoidance, escape, and tolerance. Growing drought tolerant varieties and applying agronomic management to create innovative water-use approaches is important for drought management. This review summarizes the effects of drought stress on wheat growth and productivity. This review could be useful for wheat researchers and growers for making the right decision on drought management in wheat.

A study was carried out to assess farmers’ knowledge on insect pests of citrus and Integrated Pest management in citrus zone, Gulmi district. Survey sample size of 105 was taken from Dhurkot Rural Municipality, Chatrakot Rural Municipality and Resunga Municipality of the Gulmi district. Respondents were selected using simple random sampling technique and interviewed. The primary data were collected using pretested questionnaire. The knowledge on citrus insect pest is significantly associated positively with total citrus cultivation area (p=0.1%). The average years of engagement of 23.3 years and average mandarin cultivation area of 0.35 ha gives insight to great potentiality of farmers adopting integrated pest management. The result shows insect’s severity as one of the major problems with major insect pest of citrus as fruitfly, green stink bugs, white grubs, leaf miner, aphid and rent ants. Although (74.3 %) of the total respondents were found to have knowledge about term “IPM technology”, only (35.2%) practiced IPM practices till date. The positive attitude and perception of citrus growers towards IPM technology in the study area depicts great scope for profitable citrus production on a sustainable basis.

There is a need to increase the yield potential of barley by utilizing improved cultivars. So, the present study was done to assess the performance of barley genotypes for phenological and yield attributing traits. A field study was carried out at Lumle, Kaski in Nepal involving seven barley genotypes during two growing seasons in 2018/2019 and 2019/2020, employing RCB design replicated three times. The results, combined for the years, showed significant differences for all traits except effective tillers/m2 and thousand grains weight. The grain yield was highest for B86122-1-0K-3 (2.756 t/ha and was at par with the genotypes Xveola-28 (2.411 t/ha) and B90K-024-1-1-2-0K (2.350 t/ha) and significantly higher than the Bonus (standard check) (1.852 t/ha). These superior genotypes should be further evaluated at on-station and on-farm conditions of western hills of Nepal so that low and stagnating barley yield in Nepal can be improved

In this study, wavelet analysis and traditional cross-correlation analysis (TCA) have been employed to explore the temporal characteristics of the Air Quality Index (AQI) (PM2.5) and its relationships with meteorological factors, air temperature, and rainfall in the capital city of Nepal, Kathmandu, during 2017 - 2020. The temporal variation of monthly averaged (AQI) (PM2.5) revealed that the air quality was unhealthy in winter, improved in summer, and subsequently became unhealthy again in autumn and spring. We also noticed that the lowest AQI (PM2.5) was recorded in 2020, which is due to the COVID-19 pandemic lockdown. By analyzing the time-frequency relationship of (AQI) (PM2.5) with air temperature and rainfall, using Cross Wavelet Transform (XWT), we observed the interannual, seasonal, and monthly common periodicity between them. The coherency of XWT is then tested by using Wavelet Transform Coherence (WTC), which showed a negative correlation of AQI (PM2.5) with rainfall and air temperature, while an anomaly was seen among AQI (PM2.5) and temperature as they were positively associated for a few months in 2017, 2018, and 2020. Moreover, the results of cross-correlation and multiresolution analysis (MRA) are consistent with wavelet analysis, indicating the driving effect of air temperature and rainfall on (AQI) (PM2.5).

Myeloproliferative neoplasms (MPNs) constitute a group of hematologic clonal diseases that affect one or more myeloid lineages with abnormal proliferation. It is rare disease entity and incidence is about 1.15 to 4.99/100 000 person-years among hematological neoplasms for all subtypes of MPNs combined. Patients who present with hepatosplenomegaly, hyperleukocytosis with monocytosis should have routine tests along with bone marrow morphology possibly biopsy, quantiferon TB Gold in tube test, Dengue fever IgM, IgG, NS-1 antigen, cytogenetics t(9;22), BCR cABL fusion gene, JAK-2 V617F, MPL mutations, CALR gene test done along with karyotyping and flowcytometry to evaluate and establish diagnosis towards management.

Introduction: The prevalence of Delirium tremens (DT) is around 5-12%. Very few studies have looked into the prevalence of DT in general population and has shown varying results. The study aims at finding the prevalence of delirium tremens among alcohol dependents (AD) and a comparison of profiles of alcohol withdrawal syndrome (AWS) with DT and AWS without DT. Material And Method: A case- control study was conducted in patients diagnosed as Alcohol dependent syndrome and admitted in Psychiatry in-patient ward for detoxification. Socio demographic and clinical variables were assessed and compared between alcohol dependents who developed DT and who did not developed DT. Results: 24% of the patients with AD had/developed delirium tremens. The multivariate regression analysis model shows variables like Age>50years (AOR= 75.7,95% C.I=7.8-730), Caste(Janjati: AOR=4.2, 95% C.I=1.329-13.23,) and unemployment(AOR=54.2, 95%C.I=3.8-765.254),longer length(years) of alcohol consumption >20 years(AOR= 305,95%CI=3.4-2711),presentation within 48-72 hrs since the last intake of alcohol (AOR=923,95%CI=38-22333),absence of nicotine consumption(AOR=40,95%C.I.=1.66-967) were significantly associated with development of delirium tremens. The physical properties such as dust color temperature, dust mass, visual extinction, and Planck function with their distribution in the core region of two far-infrared cavities, namely FIC16-37 (size ~ 4.79 pc x 3.06 pc) located at R.A. (J2000): 16h 33m 57.25s & Dec. (J2000): -37d 47m 04.3s, and FIC12-58 (size ~ 22.54 pc x 14.84 pc) located at R.A. (J2000): 12h 52m 50.08s & Dec. (J2000): -58d 08m 55.02s, found within a galactic plane -10o to +10o nearby Asymptotic Giant Branch (AGB) stars namely AGB15-38 (R.A. (J2000): 15h 37m 40.74s & Dec. (J2000): -38d 20m 24.6s), and AGB12-57 (R.A (J2000): 12h 56m 38.50s & Dec. (J2000): -57d 54m 34.70s), respectively were studied using Infrared Astronomical Satellite (IRAS) survey. The dust color temperature was found to lie in the range of 23.95 ± 0.25 K to 23.44 ± 0.27 K with an offset about 0.5 K for FIC16-37, and 24.88 ± 0.27 K to 23.63 ± 0.98 K with an offset about 1 K for FIC12-58. The low offset in the dust color temperature indicated the symmetric distribution of density and temperature. The total mass of the cavities FIC16-37 and FIC12-58 were found to be 0.053 M☉ and 0.78 M☉, respectively. The contour plots of mass distribution of both of the cavities was found to follow the cosmological principle, suggesting the homogeneous and isotropic distribution of dust masses. The plot between temperature and visual extinction showed a negative correlation, suggesting that higher temperature has lower visual extinction and vice-versa. The distribution of Planck function along major and minor diameters of both of the cavities was found to be non-uniform, indicating oscillation of dust particles to get dynamical equilibrium. It further suggested the role of pressure-driven events nearby both cavities and suggested that dust particles are not in thermal equilibrium along the diameters.

Hybrid is the most economical option to boost up the grain yield of maize, and slowly it is gaining popularity among the farmers of Nepal. In order to identify the potential hybrids suitable for Terai and Inner Terai regions, a set of experiment was conducted on hybrid maize developed by National Maize Research Program (NMRP), Rampur in Coordinated Variety Trials (CVTs) during the winter season of 2014/15 and 2015/16. The experiments were done by using Randomized Complete Block Design (RCBD). Each treatment was replicated thrice for each site at Rampur, Belachapi, Tarahara, and Parwanipur. Over the years, genotypes RML-83/RL-197 and RML-4/RL-111 yielded higher than other tested genotypes in Tarahara. Similarly, RL-180/RL-105, RML-87/RL-105, Dekalb double and Rampur Hybrid-6 produced higher grain yield at Belachapi during 2014/15. Genotype RML-4/RML-111 followed by RML-98/RL-105, and Rampur Hybrid-6 yielded higher at Parwanipur during 2015/16. In the case of Rampur, genotypes RML-98/RML-105 had produced higher yield in both the years whereas RML-5/RL-105 during 2014/15, and Rampur Hybrid-2 followed by RML-55/RL-105 were the superior genotypes in terms of grain yield during 2015/16. Those hybrids with higher grain yield in CVTs will be upgraded to Coordinated Farmers Field Trial on Hybrid (CFFTH) and these hybrids might be the potential future hybrids for Terai and Inner Terai of Nepal.

Importance and benefits of denture labeling have been widely described in forensic and dental community. It helps in identification and prevents the probability of losing dentures in geriatric edentulous patient. Several methods have been described in literature for the purpose of labeling denture. The article fully describes an inclusive method which is simple, economical, permanent and is done with equipment readily available in a laboratory.

Introduction: Both diabetes and depression are major public health problems. Depression is a frequent comorbidity associated with diabetes. This study was aimed to find the prevalence of depression among patients with diabetes and to identify various factors associated with it. Material And Method: This is a cross-sectional study of diabetic patients visiting Internal medicine OPD of Universal College Of Medical Sciences, Bhairahawa, Nepal and referred for psychiatric evaluation. Sample size of 137 was included in 6 months period. Face to Face interview was conducted to gather socio-demographic and clinical profile of patients. Patient Health Questionnaire-9 was used to identify and classify depression. Frequency, Percentage, Chi-square test, Multivariate regression were applied. Results: The overall prevalence of depression was 27%. Depression was found to be significantly higher among female patients (p=0.009), patients suffering from diabetes mellitus for more than 15 years (p=0.054), patients under insulin therapy (p=0.034) and patients whose blood sugar level was uncontrolled(p=0.020) even with treatment. On regression analysis, treatment modality, blood sugar level, gender and treatment modality were found to be independent predictors of depression among diabetic patients. Twelve common buckwheat (Fagopyrum esculentum Moench) genotypes received from Hill Crops Research Program, Kabre, Dolakha were evaluated for resistance against major foliar fungal diseases and high yielding traits at the research field of National Maize Research Program, Rampur, Chitwan during winter seasons of two consecutive years 2017 and 2018. The design of the experiment was Randomized Complete Block having three replications. The unit plot size was 3m x 2m with 25cm row to row spacing and continues plant to plant spacing was maintained and net harvested plot was 6 sq meters. The experiment was planted at last week of October in both years. The recommended dose of fertilizer was 30:30:0 for N:P:K kg ha-1 respectively and seed rate 50 kg ha-1. Marsupialization is a conservative surgical approach for the surgical management of large odontogenic lesions. Such procedure often results in a post-surgical defect in the form of an intraoral opening. The patency of such defect, if not maintained can result in food accumulation which may lead to infection and thus delayed healing of the defect. Obturator is one of the modalities to facilitate closure of the resultant defect. This report presents a case where an acrylic plug obturator retained with clasp, was provided to the patient for the interim period following marsupialization on the mandibular left posterior region.

Stalk rot of maize (Zea mays L.) is becoming a serious threat in tropical and subtropical maize growing regions of Nepal. To identify the sources of disease resistance in maize genotypes, a field experiment was conducted under natural epiphytotic condition during the summer season of 2016 and 2017 with thirty genotypes in a randomized complete block design in two replications. Statistical analysis showed that percent disease incidence (PDI) and grain yield were highly significant among the tested genotypes. Most of the maize genotypes were resistant to moderately resistant, only few were susceptible in both years, indicating good sources of resistance in the available genotypes. During 2016, the highest PDI was found in Arun 4 (33.17%) followed by Rampur 27 (20.10%) and Arun 2 (20.06%) whereas TLBRS07F14 and TLBRS07F16 showed no disease incidence. Similarly, the highest and least grain yielders were identified as Rampur Hybrid 6 (6.77 t/ha) and Arun-4 (2.15 t/ha) respectively. In 2017, highest PDI was observed in Arun-2 (24%) followed by Arun-4 (22%) and no disease incidence was seen in TLBRS07F14. Highest grain yield was found in RAMS03F08 (7.38 t/ha) followed by Manakamana-3 (7.37 t/ha) and the lowest was recorded in Arun 4 (2.60 t/ha). These resistant genotypes can be utilized in national breeding program to develop stalk rot tolerant high yielding maize genotypes in future.

Population dynamics of lentil aphid Aphis craccivora (Hemiptera: Aphididae) was assessed in relation with climatic parameters at the research field of National Maize Research Program (NMRP), Rampur, Chitwan during winter season of two consecutive years 2016 to 2018. The experiment was organized in randomized complete block design consisting 20 lentil varieties with three replications. The crop was sown during last week of November in both the years. The daily meteorological parameters like maximum temperature (Tmax), minimum temperature (Tmin), relative humidity (RH) and rainfall (Rf) were recorded at the meteorological station located in NMRP, Rampur, Chitwan and then converted into weekly basis as the standard meteorological week (SMW) with correspondence to weekly population of aphid. The incidence of aphid was started from 2nd SMW of January (2 aphid/plant/10 cm apical twigs) during both experimentation years. Initially the population was low and gradually increased and reached to its peak (49 aphid/plant/10cm apical twigs) on 9th SMW i.e. first week of March with correspondence to weather parameters viz. maximum and minimum temperature (°C), relative humidity (%) and rainfall (mm) were 30.80, 15.34, 67.72 and 0, respectively over the years. The aphid population had significant positive correlation with Tmax (r= 0.94) while the Tmin showed highly significant correlation (r=0.99). The relative humidity (RH) had non significant negative correlation (r= -0.90) and rainfall (Rf) showed non significant negative impact (r= - 0.15) with aphid population. Introduction: Non-adherence to appropriately prescribed medication for psychiatric disorders prevents patients from realizing the full benefits of their treatment and negatively impacts on individuals, their families and the healthcare system. Understanding and reducing non-adherence is therefore a key challenge to quality care for patients with psychiatric disorders. This finding highlights the prevalence, consequence of non-adherence and barriers to adherence. Therefore, the main objective of this study was to determine the incidence and factors associated with medication non-compliance among patients suffering from schizophrenia and bipolar affective disorders. Material and Method: This cross-sectional study included 94 schizophrenia and 76 bipolar affective disorder subjects who were visited psychiatric OPD of tertiary level hospital and disproportionate stratified random sampling method. An anonymous socio-demographic as well contributing factor questionnaire pre tested, semi structured pro forma by interview method and a clinical proforma designed by Morisky, the 8-item Morisky Medication Adherence Scale (MMAS-8) were used for data collection. Results: The prevalence of non-compliance was 88%, the independent predictors of non-compliance were; seeking for traditional/spiritual treatment, weight gain as side effects of drugs , increased number of medication (p=0.038), beginning of medication start (p=0.057), unemployment (p=0.059), low level of insight/education (p=0.044), male as gender (p=0.016). Conclusion: Based on the outcome of the study, we recommend the development of psycho-educational programmes on compliance and the active involvement of the relatives and significant others in the management of patients of mentally ill patients.

Introduction: Anxiety is a vague, uneasy feeling, the source of which is often non-specific but known to cause abnormal hemodynamic changes as a consequence of sympathetic, parasympathetic and endocrine stimulation. Most patients awaiting elective surgery experience anxiety. The degree, to which patient develops anxiety depends on many factors like age, gender, type and extent of the proposed surgery, previous surgical experience, and personal susceptibility to stressful situations. This study was conducted to find out the prevalence of preoperative anxiety and factors contributing to it. Material and Method: This is a cross-sectional study done among 74 pre-operative patients admitted the day before elective surgery in Department Of Surgery Universal College Of Medical Sciences Teaching Hospital, Bhairahawa. Socio-demographic as well contributing factor questionnaire, semi structured pro forma by interview method and Beck Anxiety Inventory Scale (BAI) Nepali Version were used to collect the data. Results: The findings of the study revealed that 90.54%, had very low level of anxiety and 2.70% of patients had severe pre-operative anxiety. The bivariate logistic regression analysis showed statistically significant association between family income (p= 0.004, AOR=2.03, CI= 0.009-2.567), family support (p< 0.001, AOR= 2.34, CI= 0.003-3.368), expected duration of hospital stay (p= 0.049, AOR=8.889, CI= 78.051-78.051), clarity on given information by health care providers (p< 0.001, AOR= 53.33, CI=7.165-396.99), staffs friendliness (p< 0.001, AOR= 21.01, CI= 3.450-127.82), fear of nil per oral (p= 0.015, AOR= 2.32, CI= 0.26-3.67) and level of pre-operative anxiety. Conclusion: It is concluded that varying degree of pre-operative anxiety were found in patients undergoing surgery. Providing adequate information about Peri-Operative procedure can help in reducing these anxiety.

Introduction: Consultation Liaison Psychiatry is the fusion of Psychiatry and general medicine. The presence of mental disorders in physically ill people is often associated with poor medication compliance, slower or incomplete recovery, poor participation in the rehabilitation program, increased mortality and the high psychiatric comorbidity to other medical disorders in hospitalized patients significantly increases their length of hospital stay. Therefore, mental health professionals have important role to provide holistic care to people with various physical illnesses. Material and Method: This is a hospital- based, cross- sectional, descriptive study conducted for the period of six months, from 1st Baisakh, 2076 (14th April, 2019) to 30th Asoj, 2076 (17th October, 2019) at the department of Psychiatry at Gandaki Medical College, Pokhara, Nepal. This study was conducted to diagnose the various psychiatric disorders among the in-patients suffering from other physical illness, which were referred from other medical specialties. The psychiatric evaluation and the diagnosis were made by the consultant psychiatrists. Results: The total number of cases enrolled were163, out of which male were 96 (58.8%) and female were 67 (41.2%) in numbers. The age of the patients ranged from 6 years to 90 years with the mean age of 41.17 ± 18.57 years. Majority of the in- patient cases referred for psychiatric assessment were from the Internal medicine (42.3%), followed by ICU (23.3%), Surgery (12.8%), Orthopedics (9.8%) and Gynecology and Obstetrics (7.3%) specialty. Among the in- patient referral cases, the most common psychiatric diagnosis was substance dependence syndrome (39.2%), followed by anxiety disorder (17.1%), delirium due to general medical condition (12.2%), depression (6.7%), seizure disorder (6.7%) and intentional self harm (5.5%). Conclusion: Under Consultation Liaison Psychiatry, the most common diagnoses to look for are the substance dependence syndrome, anxiety disorder, delirium, depression, seizure disorder, self harm etc.

Indwelling urinary catheters (IDCs) are a common invasive device in hospitalised patients. Their use is associated with increased risks of developing catheter associated urinary tract infections (CAUTI), and blood stream infections (BSI).To examine the characteristics and outcomes of adult inpatients with an IDC inserted in hospital and identify risk factors for developing CAUTI and BSI.We performed a retrospective observational study of 430 patients with IDC admitted to medical and surgical units of a leading (tertiary) hospital between Nov 2019 till April 2020. Multiple logistic regression analysis was performed to determine independent risk factors for developing urinary tract infection and blood stream infection.The prevalence of CAUTI in our study was 7.4%. Results of multiple logistic regression indicated that with each day of IDC in situ, the likelihood of UTI development increased by 9% (OR 1.09; 95% CI 1.00 to 1.18; p = 0.03). Age, gender, and catheter reinsertion were not associated with UTI development.Longer duration of IDC was associated with elevated risk of developing CAUTI. CAUTI rates were higher than some of those previously published. There was no statistical significance in frequency of CAUTI between medical and surgical patients. No statistically significant variables that contributed to the development of BSI were found. Interventions targeted at reducing catheter days should be used to improve CAUTI rates.

We revisit an old subject to discuss relationships between the dynamics for particles subjected to potentials and the dynamics for particles moving freely on background geometries, in the context of non-relativistic quantum mechanics. In particular, we illustrate how selected geometries can be used to regularize singular potentials. We also compute scattering amplitudes for quanta incident on a static non-relativistic wormhole.

District Health System Management is a comprehensive study of the health system of the district as a unit of national health system. The report of District Health System Management has been prepared as per the MBBS 4th year (Third phase) curriculum of Tribhuvan University (TU), Institute of Medicine (IOM), Nepal after the field placement of our group in Gorkha and Kaski districts. We prepared regional health directorate profile, regional tuberculosis center profile, district profile, district health profile of Kaski, hospital profile of Western Regional Hospital, Kaski, and critical analysis on human resources of the Aampipal hospital. We prepared five year plan on ICU services of Gorkha District Hospital and conducted epidemiological study on COPD in WRH, Pokhara. The techniques used in this study were observation, interviews, interactions, participation, secondary data retrieval, analysis and presentation using specific tools and guidelines devised for the same. The field practice proved to be a milestone in enabling the students to develop aptitude in the fields of management, administration and communication in different health set-ups of the country. It is in fact once-in-a lifetime opportunity for the medical students to imbibe the practicality of management skills at various levels.

Introduction: Ageing is a normal phenomenon. Various studies shows that there is a sharp rise in the elderly population in the last few years. Prevalence of depression in elderly is found to around 13% in the community sample in various countries. There is a dearth of study related to psychiatric illness in elderly in Nepal. This study was aimed to find the prevalence of depression in elderly and see for various factors associated with elderly depression. Material And Method: This was a cross-sectional study conducted at Siddharthnagar Municipality, Bhairahawa, Rupandehi. 316 elderly were selected by using systematic random sampling technique. A Semi- structured interview schedule was developed to collect Socio-demographic data. The Short Form Geriatric Depression Scale (SF-GDS) was used to find out the prevalence of depression among elderly people. Ethical Approval was obtained from the Institutional Review Committee, Chitwan Medical College and permission was obtained from Siddharthnagar Municipality, Bhairahawa, Rupandehi. The collected data was entered in Epidata 3.1 and the data was exported and analyzed in IBM SPSS 20. Results: There were a total of 316 samples in this study. The mean age of respondents was 68.47 years of age. Majority of the respondents (54.1%) were female and living in joint Family (72.2%). Almost 45.9% respondents were from upper caste groups, 90.5% of respondents were Hindus. Among the married (100%) respondents, 54.6% of respondents were living with spouse and 3.2% were divorced and separated from spouse. Depression was present in 49.4% of respondents were depressed. Among depressed respondents, 46.7% of respondents were having mild depression, 39.2% moderate depression and 14.1% severe depression. Conclusion: The finding of this study concluded that depression among elderly people was a substantial problem in Siddharthnagar Municipality, Bhairahawa, Rupandehi.

Introduction: Despite the increasing prevalence of diabetes, lack of awareness about the disease, its consequences and lack of quality care in diabetes lead to delayed presentation to health care giver that might be associated with more number of complication and co-morbid condition of the disease which may adversely affect the outcome of the patient. Therefore we undertook this study to determine the common symptoms that lead to admission of patients suffering from diabetes along with the complications they present and their outcome.Methods: We analyzed data from the medical records available among admitted patients with diabetes over a period of 6 months.Results: Among 200 subjects with mean age of 52 years, majority had Type 2 diabetes mellitus, 53% were males and more than half had some other comorbidities. Most of them, 68% had microvascular complications, and 27% of them had some form of macrovascular complications.Conclusions: Despite of continuous effort from various organizations, micro and macrovacular complications are very high among diabetic patients, so more emphasis has to be given on prevention of these complications at various level of health care delivery system.Jour of Diab and Endo Assoc of Nepal 2018; 2 (1): 4-8
