Abstract

Cerebrovascular accident is a frequent cause of death and disability in most parts of the globe. The incidence of cerebrovascular diseases increases with age and the number of strokes is projected to increase as the elderly population grows. This study was undertaken to determine the type of stroke and also to relate the risks factors associated with stroke. Hundred consecutive patients presenting with stroke in the emergency department / neurology clinic were subjected to computed tomography scan of the brain. Sixty – four patients had ischemic infarct and 36 had intracranial hemorrhage which included 2 patients with subarachnoid hemorrhage and 1 with underlying cavernous angioma. Ischemic stroke was thus commoner than hemorrhagic stroke. Previous infarct or lacunes were noted in 25 % of the patients presenting with stroke. Risk factors were present in most (77 %) of the patients presenting with stroke. Among the various risk factors, the single most common was smoking seen in 38 cases, followed by hypertension in 28 cases, obesity in 8 cases and diabetes in 3 cases. Twenty- one patients had multiple combinations of the above risk factors. Among the multiple risk factors, combination of alcohol and smoking was the commonest seen in 11 cases, followed by hypertension and obesity in 8 cases and alcohol with obesity in 2 cases. 2. Enlargement of the Pancreas in Children Diagnosed with Acute Pancreatitis: An Approach Based on P/V Ratio

Abstract

Background Acute pancreatitis is a reversible inflammation of the pancreatic parenchyma. Enlargement of the pancreas is often envisaged in such conditions. This study evaluates P/V ratio, as a marker to decide pancreatic enlargement in disease condition. Purpose The aim of this study was to develop imaging-based diagnostic criterion for acute pancreatitis in children based on P/V ratio. Material and Methods This study included 37 children with acute pancreatitis and 283 children asymptomatic for pancreatic disorders, from a single hospital center. The age of children ranged between 2 and 18 years. P/V ratio, which is the ratio of greatest anteroposterior dimension of the head, body, and tail of the pancreas relative to the transverse lumbar vertebral body, was obtained for each child through ultrasonographic examination. Age-adjusted receiver operating characteristics (AROC) analysis was performed on P/V ratio at presentation for each pancreatic region, and the sensitivity at 90% specificity, the threshold errors, and the corresponding cutoffs were obtained. The enlargement assessment was also done after clinical recovery by referring to the cutoffs of respective regions. Results AROC analysis for males and females resulted into a maximum sensitivity of 83.33 and 81.67%, respectively, at 90% specificity for head. The error thresholds for both the groups were same, i.e., 0.098, indicating that 90% of the observations had errors less than the threshold. The corresponding P/V ratio cutoff for males and females was 0.43 and 0.42, respectively. Conclusion Radiologists and clinicians can refer a cutoff value of 0.4 for each region, along with hypoechogenicity, to decide about enlargement of the pancreas in acute pancreatitis condition.

Abstract

Abstract DUNE is a dual-site experiment for long-baseline neutrino oscillation studies, neutrino astrophysics and nucleon decay searches. ProtoDUNE Dual Phase (DP) is a 6 $$\times $$ × 6 $$\times $$ × 6 m $$^3$$ 3 liquid argon time-projection-chamber (LArTPC) that recorded cosmic-muon data at the CERN Neutrino Platform in 2019–2020 as a prototype of the DUNE Far Detector. Charged particles propagating through the LArTPC produce ionization and scintillation light. The scintillation light signal in these detectors can provide the trigger for non-beam events. In addition, it adds precise timing capabilities and improves the calorimetry measurements. In ProtoDUNE-DP, scintillation and electroluminescence light produced by cosmic muons in the LArTPC is collected by photomultiplier tubes placed up to 7 m away from the ionizing track. In this paper, the ProtoDUNE-DP photon detection system performance is evaluated with a particular focus on the different wavelength shifters, such as PEN and TPB, and the use of Xe-doped LAr, considering its future use in giant LArTPCs. The scintillation light production and propagation processes are analyzed and a comparison of simulation to data is performed, improving understanding of the liquid argon properties.

Abstract

Urticaria is an autoimmune disease and many patients are suffering from it. This research aims to investigate the development and characterization of an Ebastine-loaded transfersomal nanogel for the enhancement of bioavailability in the treatment of urticaria. The flexible transfersomes, consisting of the drug Ebastine, soya lecithin, and edge activator Tween 80, were prepared using the thin-film hydration method. The transfersomal nanogel was formulated by using the dispersion method and a suitable concentration of the gelling agent Carbopol 934. The transfersomes and their gel were evaluated for various parameters. The Ebastine-loaded transfersomes showed the highest entrapment efficiency, up to 79.92%. The polydispersity index (PDI) of the transfersomes was determined to be 0.103, and the zeta potential was determined to be −18.9 mV, indicating that the formulation was stable. The drug content of the transfersome gel was found to be 83.67%. The transfersomal gel formed using 1% Carbopol 934 showed the best results, showing in vitro release for up to 8 h and following a zero-order kinetic model. As per the microbial studies conducted, the Ebastine transfersomal gel has a good anti-microbial effect against S. aureus. These vesicular transfersomes are more flexible than other vesicular systems, making them excellent for skin penetration. In the future, this will be the best possible approach for the delivery of drugs via the transdermal route.

Abstract

Introduction: The maxillary sinuses (MS) are of particular importance to dentist because of their close proximity to the teeth and their associated structures, so increased risk of maxillary sinusitis has been reported with periapical abscess, periodontal diseases, dental trauma, tooth extraction, and implant placement. Complications of MS are related to its anatomic and pathologic variations. Thus, study was conducted to assess the prevalence of anatomic variations in MS by using cone-beam computerized tomography (CBCT). Aims and Objectives: To determine different anatomical variations in MS by using CBCT. Materials and Methods: CBCT scans of 150 subjects were collected between the age group of 18 years to 70 years and were analyzed for MS anatomical variation. Statistical Analysis: The distribution of age, sex, reasons for CBCT, and dimensions of sinus calculated using descriptive statistics and distribution of other anatomic findings using Chi-square test. Results: Prevalence of obstructed ostium is 23.3% and septa is 66.7%. Average height, width, and antero-posterior (A-P) dimensions for right MS are 34.13 mm, 26.09 mm, 37.39 mm and that of left MS are 33.24 mm, 26.11 mm, 37.72 mm respectively. Average distance between lower border of ostium to sinus floor in right MS is 32.17 mm and that of left is 32.69 mm. Average diameter of ostium in right MS is 1.88 mm and that of left is 1.67 mm. Conclusion: Study highlights the importance of accurate assessment of MS and its variations in order to properly differentiate the pathologic lesions from anatomic variations avoiding unnecessary surgical explorations.

Abstract

Flavonoids and polyphenolic compounds play a key role in wound healing cycle modulation. Propolis, a natural bee product, has been widely reported as an enriched source of polyphenols and flavonoids as important chemical constituents and for its wound healing potential. The goal of this study was to develop and characterize a propolis-based polyvinyl alcohol (PVA) hydrogel composition with wound healing potential. To understand the impacts of critical material attributes and process parameters, formulation development was carried out using a design of experiment approach. A preliminary phytochemical analysis of Indian propolis extract showed the presence of flavonoids (23.61 ± 0.0452 mg equivalent of quercetin/g) and polyphenols (34.82 ± 0.0785 mg equivalent of gallic acid/g), both of which aid in wound healing and skin tissue regeneration. The pH, viscosity, and in vitro release of the hydrogel formulation were also studied. The burn wound healing model results revealed significant (p < 0.0001) wound contraction by propolis hydrogel (93.58 + 0.15%) with rapid re-epithelialization relative to 5% w/w povidone iodine ointment USP (Cipladine®) (95.39 + 0.16%). The excision wound healing model confirms significant (p < 0.0001) wound contraction by propolis hydrogel (91.45 + 0.29%) with accelerated re-epithelialization comparable to 5% w/w povidone iodine ointment USP (Cipladine®) (94.38 + 0.21%). The developed formulation offers promise for wound healing, which may be investigated further for clinical research.

Abstract

The purpose of this review article is to characterize all of the parameters regarding the types, polymers used, and release kinetics of matrix tablets. Matrix system was the earliest oral extended-release platform for medicinal use. Matrix tablets are most commonly used methods to modulate the release profile of drugs. They are much desirable and preferred for such therapy because they offer better patient compliance, maintain uniform drug levels, reduce dose and side effects, and increase safety margin for high potency drugs. Hydrophilic polymer matrix systems are widely used for designing oral controlled drug delivery dosage forms because of their flexibility to provide a desirable drug release profile, cost effectiveness, and broad regulatory acceptance. However, the use of hydrophilic matrix alone for extending drug release for highly water-soluble drugs is restricted due to rapid diffusion of the dissolved drug through the hydrophilic gel network. For such drugs it becomes essential to include hydrophobic polymers in the matrix system. This leads to the conclusion that matrix tablets seem to be most promising when developing an oral controlled release formulation.

Physico-chemical parameters have been analyzed by standard method. The Field observations reveal that enhancement in human activities water deterioration also increased. It is all due to indiscriminate behaviour of the human being. Many human activities necessary for survival but it declined water quality. Study also reveals that in the study area due to deterioration of water quality phytoplankton abundance has been observed. At the dadh, Ashwi, Punatgaon, Toka due to phytoplankton water is not fit for drinking purpose. To analyze Phytoplankton abundance and Physio-chemical characteristics of water is the main aim of the research with remedial measures to mitigate the deterioration and related consequences in future.

Abstract

Because chitosan is biodegradable, biocompatible, non-toxic, and mucoadhesive, it is commonly used in the formulation of nasal drug delivery nanoparticles employing polyelectrolyte complexes. However, chitosan's lower solubility in aqueous and alkaline conditions limits its use in the pharmaceutical and biomedical fields. This needs the development of improved chemically altered chitosan mimics that can overcome the solubility barrier. Although Levodopa is an alternative in the treatment of Parkinson's disease, it has a low oral bioavailability and very low brain absorption due to its extensive degradation by aromatic amino acid decarboxylase in the peripheral circulation. As a result, levodopa and carbidopa, a peripheral amino acid decarboxylase inhibitor, are given together. The nose to brain medication delivery of levodopa desolate via the olfactory pathway and the trigeminal neurons has been investigated in an effort to improve brain uptake and avoid degradation of levodopa in peripheral circulation and the use of carbidopa in combination. Ionic interactions mediate charged functional groups of former polysaccharides at dissimilar pH situations (pH 5, 8, 10, and 12) and identify polyelectrolyte complexes based on chitosan and pectin during physicochemical (particle size and zeta potential) and solid-state characterizations.

Prompt recognition and immediate treatment with dantrolene can effectively treat the fatal syndrome. Family education and genetic counseling should be encouraged.

Nematodes are probably the most widespread and abundant group of animal often occurring in huge numbers in very diverse environments. The largest number of helminthic parasites of humans belongs to the phylum nematoda and Ascaris lumbricoides is the most common of human helminthic parasites. Commonly known as roundworm, Ascaris lumbricoides is cosmopolitan, especially prevalent in the tropical countries like India. The adult roundworm commonly lives in small intestine, retaining its position by the virtue of its own muscle tone. Eggs of roundworm are bile stained, appearing yellowish brown in colour. Roundworm is a restless wanderer, showing great inquisitiveness as it tends to move itself from the original site, and get into any aperture it may find on its way. So it’s quite possible to encounter this commonest and largest intestinal nematode of humans at some odd sites in the body. Here, we are presenting a very rare case of finding colourless (i.e., non bile stained) eggs with a female roundworm isolated from an ectopic site.

Abstract

Lower status of women is a stain on any society. The problem is becoming glaring in India in the wake of atrocities caused against women ‘from womb to tomb’. Even in this twenty first century, the impact of manuvadi ideology is getting evidently felt. ‘A thinking woman spoils everything’ is still the mindset. Maladies like honour killing, bride burning are still gloating over the security of women in the wake of the internalized mindset of patriarchal dictum, viz. ‘A woman does not deserve any freedom’ (Na stri swatantryam arhati). Since time immemorial, she was always taken for granted though she had never remained silent. The depiction of Draupadi in various writings underscores woman’s protest against male dichotomy and her refusal to live by disabling definitions that mark women as inferior.

38. Ultrasonography based imaging criterion to ascertain pancreatic enlargement in pediatric acute pancreatitis

Abstract

Introduction: Imaging studies have shown enlargement of pancreatic parts in children diagnosed with acute pancreatitis. The purpose here is to develop imaging based diagnostic evaluation criterion for acute pancreatitis in children. Materials and Methods: This study included 62 children of acute pancreatitis in the age range of 0.33 to 13 years, as reported in a single hospital center (1994-2019). A study was planned including 1116 normal healthy children in the age range of 0.16 to 18 years for pancreatic evaluation during 2016-17. Ultrasonography based measurement of three pancreatic parts were obtained for each individual in disease and normal groups. Age-adjusted receiver operating characteristics curve analysis was performed on each pancreatic part independently to derive respective cut-offs using a training set. These cut-offs were further referred to dichotomize the measurement data for each individual and was subjected to multiple logistic regression with presence/absence of acute pancreatitis as dependent variable. A probability score and accordingly the cut-off were obtained indicating a collective impression of enlargement of pancreas in disease condition independently for males and females. Result: On test data, the accuracy of age-adjusted cut-offs for three parts was near 80% for males, while it ranged between 81-85% for females. ROC analysis of probability score resulted into threshold value of 0.024 for males and 0.044 for females, with sensitivity of 94.11% and 90.91% respectively. The classification accuracy of score derived for males and females was nearly same (83%). Conclusion: The extent of enlargement of pancreas in acute pancreatitis in children can be determined using the MLR method along with hypoechogenicity. Abstract

One-dimensional iron hydroxide@cadmium hydroxide nanostructures have been prepared by sequentially converting cadmium hydroxide into iron hydroxide via simple and cost-effective cation exchange reaction based on solubility product to design electrochemical supercapacitive electrode. Surface architecture of iron hydroxide@cadmium hydroxide nanostructures in the form of core–shell enables easy and effective electrochemical reactions which have been studied via electrochemical cyclic voltammetry and charge–discharge studies. Interestingly, iron hydroxide@cadmium hydroxide electrode exhibits specific capacitance of 368 F g−1 at current density of 0.5 A g−1 along with electrochemical impedance analysis. Capacitance retention performed at the scan rate of 50 mV s−1 for 5000 cycles results in 52% capacity of its initial.

Abstract

Photovoltaic (PV) systems propose attractive alternative source of generation because these can be placed near to the load centers when compared with other renewable source of generation. It is therefore rooftop PV is the center of attraction for majority PV systems. The rooftop PV system in general is grid connected and supports the off-grid load with battery backup. The designed system must ensure total evacuation of generated power and with high efficiency of conversion, and utilizes the resource adequately to maximize the utilization of energy. This paper proposes single phase synchronous reference frame (SRF) theory based current controlled PWM controller for the voltage source converter (VSC) to realize maximum generated power evacuation by maintaining the DC link voltage constant without battery support, low THD sinusoidal line synchronized current output, and limited reactive power compensation based on the unutilized capacity of the inverter. PV power is being tracked always at MPP through incremental conductance (IC) method. MATLAB based simulation results shows the efficient working of rooftop PV with proposed control algorithms in grid connected mode with limited reactive power conditioning.

Abstract

Background: Aim of the study was to know the seroprevalence of TORCH infection in antenatal women with bad obstetric history and to co relate the same with different clinical presentations. Materials and Methods: IgM antibodies for TORCH infection were analyzed in the serum samples of 172 Bad obstetric history (BOH) cases attending Ante Natal clinic by using ELISA according to manufacturer’s instructions. Result: Seroprevalence for T.gondii, Rubella, CMV and HSV-2 was found to be 12.79%, 11.62%, 04.65% and 02.32% respectively. Highest seropositivity in spontaneous abortions (16.25%) and early neonatal deaths (8.33%) cases was seen with T.gondii infection. In cases with history of IUD (09.37%) and congenitally malformed babies (22.22%) maximum seropositivity was found for Rubella. History of still births (25%) was equally associated with T.gondii and Rubella infection. Conclusion: There is a need of screening programmes during antenatal visits to reduce repeated pregnancy wastages in our country. Early detection of IgM by Enzyme-linked immunosorbent assay is useful for diagnosis of TORCH infection to reduce adverse fetal outcome. Considering the current situation of COVID-19 this project provides medical assistance and sanitization. This project also provides a source of information as well as entertainment to the patients. Searching Google and Wikipedia based on users' voice input and giving back the reply through robotic voice with further interactive questions by the machine. It distributes the medicines i.e., morning and evening pills to the patient according to the request. According to the voice input it moves forward, backward, turns left and right. It also detects the hand through IR SENSOR & Dispenses the sanitizer to the patient.

Euphorbia sahyadrica (Euphorbiaceae), a new succulent species belonging to Euphorbia subg. Euphorbia sect. Euphorbia is described and illustrated from the Raigad district of Maharashtra, India. It is related to E. nivulia but differs in its shrub-like habit, bark with closely-packed horizontal branch scars, elliptical-shaped leaf with short petiole and easily visible lateral veins, protruding tubercles with two pairs of spines per spine shield, bisexual cyathia in the central sessile position on the cyme, and capsules that have laterally tapering cocci without bulging suture lines between the cocci.

Abstract

Thin films of Zn1–xMnxO (x = 1–4 at %) were applied onto a glass substrate by low-cost chemical bath deposition (CBD). The films were characterized by XRD, UV–VIS absorption spectra, SEM, and vibrating sample magnetometry—with special emphasis on the influence of dopant concentration x. Structural and optical properties of deposited films imply that the Mn2+ ions substituted Zn2+ ions without changing the wurtzite structure of ZnO. No impurity phases were detected in XRD patterns. The band gap in ZnO was found to increase with increasing x. Magnetic measurements showed that Mn-doped ZnO samples exhibited ferromagnetic behavior at room temperature, which tentatively was associated with the replacement of Zn2+ ions by Mn2+ ions in the ZnO lattice.

Abstract

High Doppler valve gradient is generally suggestive of valve thrombosis. However, it should be corroborated with the finding of restricted leaflet movement to confirm the diagnosis. In the present case, abnormally high gradient was not associated with limited leaflet movements or any valve thrombus.

Abstract

The degradation of nitroaromatics/toxic energetic compounds contaminated water is a major cause of concern. W-doped TiO2 nanoparticles (NPs) were synthesized in ionic liquid, ethyl methyl imidazolium dicyanamide (EMIM-DCA) by a solvothermal method. The developed NPs were sintered at 500 °C and characterized by UV-Vis-DRS, FT-IR, FE-SEM, XRD, XPS, and BET techniques. The 30–40-nm-sized NPs were subjected to photocatalytic degradation of the toxic energetic compound, tetryl (2,4,6-trinitrophenylmethylnitramine) under UV-Vis light. Various operating parameters such as the effect of concentration of catalyst, pH of feed phase, oxidizing agents, and recycling of catalyst were studied in detail. For the first time, the degradation-mechanism pathway and kinetics of tetryl were evaluated. The degradation products were precisely analyzed by using HPLC, GC-MS, and TOC techniques. The USEPA has prescribed a drinking water limit of 0.02 mg L−1, and it was found that 0.5 g of 4% W-TiO2 could totally degrade tetryl (50 mg L−1) within 8 h. The kinetic rate constant of 4% W-TiO2 was 0.356 h−1, whereas pure TiO2 showed 0.207 h−1.
