1. Histopathological Analysis of Endometrial Biopsy in Gandaki Medical College Teaching Hospital, Pokhara, Nepal

Abstract

Objective: To study the spectrum of histopathological diagnosis of endometrial lesions and their distribution according to age. Methods: All the endometrium samples obtained by the procedure of dilatation and curettage and hysterectomy sent for histopathological examination at Pathology Department of Gandaki Medical College Teaching Hospital, Pokhara, Nepal. The study duration was total 12 months ranging from July 2016 to June 2017. All the endometrial samples were processed, sectioned at 4 - 6 μm and stained with routine H & E stain. Patient’s data including age, sex, procedure of the biopsy taken and histopathological diagnosis were noted. A pathologist, using Olympus microscope, reported the slides. Cases were reviewed by a second pathologist whenever necessary. Results: A total of 128 cases were studied. The most common histopathological diagnosis was proliferative endometrium (28.9%) followed by disorder proliferative endometrium (15.65%). Most of the patients were in age group 36 - 45 years comprising 32.03%. Hydatidiform mole comprised of 7.03% and among Hydatidiform mole, partial mole was more common. Dilatation and curettage (82.8%) was the common procedure in compare to hysterectomy for the evaluation of endometrial lesions. Conclusions: In this study, we observed a variety of endometrial lesions. Most of them are benign; among benign, proliferative endometrium was the common histopathological diagnosis followed by disorder proliferative endometrium. Most common presenting age group was found to be at 36 - 45 years. In evaluation of hydatidiform mole, partial mole was more frequent in compare to complete mole. Conventional dilatation and curettage is the preferred method in developing countries with limited resource to screen endometrial lesion and therefore biopsy should be sent for histopathological examination. Thus histopathological examination of routinely stained hematoxylin and eosin is readily available and widely accepted standard technique for evaluation of the endometrial lesions.

Abstract

District Health System Management is a comprehensive study of the health system of the district as a unit of national health system. The report of District Health System Management has been prepared as per the MBBS 4th year (Third phase) curriculum of Tribhuvan University (TU), Institute of Medicine (IOM), Nepal after the field placement of our group in Gorkha and Kaski districts. We prepared regional health directorate profile, regional tuberculosis center profile, district profile, district health profile of Kaski, hospital profile of Western Regional Hospital, Kaski, and critical analysis on human resources of the Aampipal hospital. We prepared five year plan on ICU services of Gorkha District Hospital and conducted epidemiological study on COPD in WRH, Pokhara. The techniques used in this study were observation, interviews, interactions, participation, secondary data retrieval, analysis and presentation using specific tools and guidelines devised for the same. The field practice proved to be a milestone in enabling the students to develop aptitude in the fields of management, administration and communication in different health set-ups of the country. It is in fact once-in-a lifetime opportunity for the medical students to imbibe the practicality of management skills at various levels.

Abstract

Introduction: Ocular trauma is an important cause of blindness and ocular morbidity throughout the world. The present study was done to establish the common causes of ocular trauma in a Tertiary Care Hospital of Western Nepal. Methods: In this prospective study, all the patients with ocular trauma visiting eye Out Patient Department (OPD) and Emergency Department of Gandaki Medical College Teaching Hospital (GMCTH) from June 2015 to June 2016 were included. A complete history and detailed ophthalmological evaluation was done. Results: Over a period of one year, 226 patients attended to the OPD and Emergency Department of GMCTH. Mean age of patients was 30.41 ±15.7 years. Males were 70.8% and females 29.2%. About 82.5% patients had visual acuity better than 6/18 while 17.7% had visual acuity

Abstract

Community health diagnosis is a comprehensive assessment of health status of the community in relation to its social, physical and biological environment. The purpose of community health diagnosis is to define existing problems, determine available resources and set priorities for planning, implementing and evaluation health action, by and for the community. A cross sectional descriptive study was done at Dhampus VDC from 1 Sept 2016 AD (16th Bhadra, 2073 BS) to 16 Sept 2016 AD (31st Bhadra, 2073). The data was collected by questionnaires, anthropometric measurements, interviews and secondary data. The community health diagnosis field visit gave us the opportunity to build our team spirit and taught us the arts of problem solving. Abstract

Introduction: Swelling of the thyroid gland is a commonly encountered clinical problem in all age groups whether benign or malignant. Fine Needle Aspiration Cytology (FNAC) of the thyroid lesions along with hormonal function test helps in the proper preoperative assessment. Ultimately it changes clinical management and improves the patient outcome. Objectives: To describe the cytomorphological features of palpable thyroid nodules using fine needle aspiration cytology (FNAC) along with the assessment of thyroid hormonal status of the patient. Methods: The study was conducted in the Department of Pathology of Gandaki Medical College and Teaching hospital from January 2017 to December 2017 and included 50 patients with thyroid lesions. Cytological assessment was done using FNAC along with serological assessment of thyroid hormones. Results: In the study 50 cases of thyroid swelling were included and evaluated by cytological and hormonal analysis which comprises of 10% males and 90% females with a with a female to male ratio of 9 : 1. Maximum number of cases was seen in the age range 41 - 60 years (46%), mean age being 44.6 years. The cytological diagnosis comprised colloid goiter (58%), lymphocytic thyroiditis (16%), Hashimoto thyroiditis (10%). Thyroid hormone analysis showed 66% euthyroid, 22% hypothyroid and 12% hyperthyroid. Conclusions: The study showed that FNAC and TFT profile both are essential for the proper management of thyroid lesions. Abstract

Background: Drugs and therapeutic agents exposure is essentially universal for persons, young and old in the Philippines. In the hospital, many drugs and therapeutic agents are being given to the patients according to severity of the illness. The evidence has suggested that complex medication combination increase the risk for adverse drug events. The most common results of many drugs and therapeutic agents are increased adverse drug reactions, drug-drug interactions and higher costs. Objectives: To assess the prevalence and patterns of exposure to drugs and therapeutic agents among pediatric inpatients. Methods: Study Design: Retrospective study. Setting: Pediatric inpatients in the Fatima University Medical Center. Patients: A total of 1075 patients younger than 19 years, excluding healthy newborn, hospitalized in 2011, representing a part of pediatric inpatients in the Philippines. Results: The most common exposure was with D5.3NaCl, paracetamol, cefuroxime, salbutamol, zinc sulphate. Most of the pediatric inpatients received five or less number of medication varied by age and length of stay in the hospital. Pediatric inpatients were exposed to numerous drugs and therapeutic agents, especially patients with dengue III. Conclusions: A large portion of pediatric inpatients are exposed to numerous drugs and therapeutic agents, especially patients with Dengue III. Abstract

Community diagnosis is defined as a comprehensive assessment of the state of an entire community in relation to its social, economic, physical and biological environment. It is a process of examining the patterns of disease or health status in the community in order to promote health, prevent disease and manage health services for the community people. As per the curriculum of Tribhuwan University, B.Sc. Nursing first year, we were provided an opportunity to fulfill the practicum of community health nursing in the assigned area Shanti Tole and Teentara Tole, Pokhara, Lekhnath, Ritthepani-27 from 18th June to 14th July 2017. Total population of Ritthepani was 4529, male population was 2100 and female population was 2429. Each of the 13 students was assigned with 10 different families for community diagnosis in order to promote the health of the individual, family and community. The main objective of community diagnosis was to find out the health related aspects and to provide preventive, promotive, curative and rehabilitative services to the individual and the community as a whole. All the findings were presented among the key members of the community. On survey, total population of 130 houses was found 565, among them 296 were male population and 269 were female population. Sanitation of the community was inappropriate. People were not using the health facilities effectively so they were found suffering from different kinds of diseases most commonly gastritis. The common health seeking practice was going to governmental hospital and traditional healers.

Abstract

Introduction: Scrotal pain is a common urological condition. Common causes of scrotal pain are testicular torsion, epididymitis, epididymo-orchitis and occasionally due to post vasectomy pain, varicocele, testicular trauma etc. The objective of our study is to review the different disease patterns in patients presenting with scrotal pain in a tertiary care hospital. Methods: Patients presenting with scrotal pain to the out-patient clinic and the emergency department were included in the study. All patients were subjected to urinalysis and ultrasound of scrotum with color Doppler study when needed. Descriptive variables were studied and analyzed. Results: Eighty three patients were enrolled in the study during the period of one year. Age ranged from 13 to 72 years; most of the patients were less than 40 years and more than 20 years. Among this age group non-specific orchalgia was very common. Isolated epididymitis was seen in only 4 (4.81%) patients. Epididymo-orchitis was found in 16 (19.27%) patients. Torsion of testis was found in 6 patients (7.22%). The major bulk of the patients presenting with scrotal pain were nonspecific which was found in 39 (46.98%) and exact cause could not be found in them. Total of 15 (18%) patients had acute presentation. Conclusion: Management of Scrotal pain can be simple or at times organ threatening, proper diagnosis and treatment by clinical judgment and necessary investigation is very important. Abstract

Introduction Depression is a widespread problem that affects individuals of all ages. This study looks at the use of omega-3 polyunsaturated fatty acids (PUFAs) as an additional therapy for depression in people of different ages. Depression has an impact on everyone, from youth to the elderly, causing therapeutic concerns such as treatment resistance and recurrence. Omega-3 PUFAs, which may be found in fish and flaxseed, are important because of their impact on neurochemistry, inflammation, and neuroprotection. While pharmacotherapy, including antidepressants, has proven beneficial for many, the likelihood of remission and recurrence remains substantial. In recent years, there has been a growing interest in the potential role of omega-3 polyunsaturated fatty acids (n-3 PUFAs) in mitigating depressive symptoms. The primary constituents of n-3 PUFAs are eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA). Understanding the potential of omega-3 PUFAs across the lifespan can help address the multifaceted challenges posed by depression and improve mental health outcomes for diverse age groups. Objectives This review aims to assess the role of omega-3 fatty acids in depression treatment across different age groups: children and adolescents, adults (18–60), and the elderly (60+). It investigates the effectiveness and potential differences in omega-3 supplementation among these age cohorts. Methods A comprehensive literature search was conducted from 2003 to 2023 using PubMed, Google Scholar, and EMBASE, using specific keywords. Studies with inadequate age group information or Omega-3 intervention were excluded. Results In children and adolescents, several studies indicate a positive association between omega-3 supplementation and improved depressive symptoms. In adults, results are mixed, with some studies showing benefits while others do not. In the elderly, omega-3 PUFAs appear to have a more consistent positive effect on depression. In contrast, a consistent positive association was observed in the geriatric population, suggesting that Omega-3 PUFAs may hold particular promise in the treatment of depression among older adults. However, variations in methodology, dosage, and study populations contribute to these mixed findings. Conclusions Omega-3 PUFAs show promise as an adjunct therapy for depression across different age groups. Further research with standardized methodologies and larger sample sizes is needed to clarify their role and establish optimal dosage guidelines. Omega-3 PUFAs should be considered as a potential complement to conventional depression treatments, emphasizing the need for personalized approaches in depression management. Abstract

Introduction Addiction medicine is becoming more of an issue as addiction-related problems continue to plague people all over the globe, resulting in serious health consequences. Addiction has become increasingly prevalent in recent years, as have addiction-related disorders. For efficient care and improved patient outcomes, this growing pandemic requires early and precise identification. In the field of addiction medicine, artificial intelligence (AI) looks to be a feasible tool. This systematic review examines the current state of research on the use of AI in addiction medicine, including a variety of AI techniques, their efficiency compared to conventional diagnostic methods, and their potential influence on addiction therapy. While AI has great potential for transforming addiction treatment, further research is needed to assess its use fully. Objectives The objective of this review is to assess the current state of research on the use of artificial intelligence in addiction medicine, focusing on its diagnostic efficacy and potential for revolutionizing addiction therapy. Methods To evaluate the effectiveness of AI in addiction medicine, we conducted an extensive search of the PubMed database. Our search encompassed articles published in the English language from January 2013 to March 2023. Inclusion criteria encompassed studies reporting the utilization of AI for addiction diagnosis in human patients. Results The initial PubMed search produced 100 papers, of which 15 were included after meticulous analysis and screening. These studies assessed diverse types of data, including patient records and behavioral patterns, employing various AI techniques, such as machine learning and deep learning. The findings indicate that AI can accurately and swiftly identify addiction-related issues, boasting high sensitivity and specificity rates. Additionally, AI demonstrates potential in identifying specific addiction subtypes and forecasting patient outcomes. Nevertheless, these studies also underscore certain limitations of AI, such as the requirement for extensive data and susceptibility to overfitting. Conclusions Artificial intelligence holds the potential to revolutionize addiction medicine by enabling faster and more precise diagnostics, pinpointing specific addiction subtypes, and predicting patient outcomes. However, further research is imperative to validate AI’s efficacy across diverse patient populations and address challenges related to data accessibility, communication, and integration into clinical practice. 12. Cannabinoids Hyperemesis Syndrome – An Urgent Call for Timely Diagnosis, Management, and Future Directions– A Case Report and Review of the Updated Literature

Abstract

Introduction Cannabinoid Hyperemesis Syndrome (CHS) is distinguished by a pathognomonic cyclic pattern of hyperemesis characterized by recurring episodes of severe vomiting every few weeks to months, as well as obsessive thoughts and compulsive behavior, such as a proclivity to take frequent hot baths or showers. It is largely accepted as the most commonly used illicit drug in the United States, with estimates ranging from 42% to 46% lifetime consumption. Despite greater awareness of CHS, practitioners continue to lack comprehension, resulting in an unfortunate delay in patient identification and treatment. Objectives The aim of this article is to bring attention to CHS in order to enable clinicians, and more specifically, addiction medicine specialists and psychiatrists, to diagnose it as quickly as possible and thus avoid unnecessary additional invasive examinations and investigations. This will save the patient’s time, prevent financial burdens and mental health stresses, and increase their overall quality of life. Methods A thorough screening and data extraction of the relevant articles was conducted using PubMed, Cochrane, and Embase. Databases were used to search for articles on CHS published between January 2021 and September 2023, yielding relevant articles. Keywords used were “hyperemesis”, “cyclical vomiting,” “cannabis” and “cannabinoid”. Results We present a case of 20-year-old teens who came to emergency with severe dehydration and vomiting of more than 40 episodes at home. He had multiple admissions for abdominal pain, nausea, and vomiting in the past and was evaluated and diagnosed with gastritis, PUD, and H. pylori infection. A more detailed medical history revealed a frequent use of cannabis over the past few years and symptoms manifestation and worsening is associated with the use of cannabis. After the complete cessation of cannabis, there have been no new symptomatic episodes reported in the patient and the patient is stable clinically. Conclusions Cannabinoid Hyperemesis Syndrome (CHS) is a serious health hazard that requires immediate discovery and treatment. Despite the widespread use of cannabis, CHS is often misdiagnosed, resulting in unnecessary medical treatments and complications for patients. Given their special knowledge of linking chronic cannabis use to this syndrome, this case report and literature review highlight the critical role of addiction medicine experts and psychiatrists in quickly detecting and treating CHS. Early detection and treatment, particularly complete cannabis abstinence, are critical in alleviating symptoms, minimizing recurrent hospitalizations, and ultimately improving patients’ overall quality of life. Abstract

ABSTRACT: The combined effects of the in ovo injection of commercial Marek's disease vaccine (MDV) and various levels of 25-hydroxyvitamin D3 (25OHD3) on the hatch variables, immunological measurements, and gene expression of Ross 708 hatchling broilers were investigated. A total of 5 in ovo injection treatments that were applied at 18 d of incubation (doi) included: 1) noninjected (control); or a 50 μL solution volume of 2) MDV alone; or MDV combined with 3) 0.6 μg of 25OHD3; 4) 1.2 μg of 25OHD3; or 5) 2.4 μg of 25OHD3. At hatch, hatchability of set and live embryonated eggs, hatchling body weight, hatch residue analysis, serum IgY and alpha-1 acid glycoprotein (AGP) concentrations, and the expression of genes related to immunity (INFα, INFβ, INFγ, TLR-3, and TLR-21) and vitamin D3 activity (1 α-hydroxylase, 24 hydroxylase, and vitamin D receptor) were determined. No significant treatment differences were observed for hatchability of set and live embryonated eggs, or for serum IgY and AGP concentrations. However, hatchling body weight was higher when MDV was combined with either 1.2 or 2.4 μg of 25OHD3 than when MDV was provided alone or in combination with 0.6 μg of 25OHD3. Also, in comparison to the noninjected treatment group, the expression of the genes for 1 α-hydroxylase and 24 hydroxylase was improved when MDV was combined with either 1.2 or 2.4 μg of 25OHD3. Lastly, expression of the genes linked to viral detection (TLR-3) and antibody production (INF-β) was increased in those treatments that contained any level of 25OHD3. These results indicate that in comparison to controls, the effects of MDV were observed to be greater on hatchling BW and splenic gene expression when it was administered in combination with the 1.2 or 2.4 μg doses of 25OHD3. Further research is needed to determine the posthatch effects of the administration of various levels of 25OHD3 in combination with MDV.

19. Effect of Detasseling and Defoliation in the Yield of Sweetcorn in Khotang District of Nepal

Abstract

As maize is a C4 plant it is expected to have greater productivity however, its production and productivity are not found to its potential. Therefore, to evaluate the effect of detasseling and defoliation to increase the productivity, a field experiment was conducted on sweetcorn at Khotang, Nepal in 2021. The study was executed in Randomized Complete Block Design (RCBD) with three replications and seven treatments. The results revealed that number of kernels (608.46), number of rows (15.20), 1000 grains weight (210.68gm) were recorded significantly higher in the plot where 50% plants were detasselled. It clearly shows that 50% detasseling is one of the best options to enhance the yield of sweetcorn followed by detasseling + defoliation of all leaves leaving three leaves below the ear.

20. Health Promoting Lifestyle during Covid-19 Pandemic among Residents of Gandaki Province, Nepal

Abstract

Background Lifestyle is an important factor to individual’s health and quality of life. Although, the emergence of Corona virus disease 2019 (COVID-19) has forced billions of people to change their lifestyle overnight. Objective To identify health promoting lifestyle during COVID-19 pandemic among the residents of Gandaki Province, Nepal. Method Descriptive cross sectional web based study was conducted among 386 respondents from Gandaki Province. Adults from 18 years and above, who could fill the form online were included using purposive and snowball sampling technique. Data were collected using Health Promoting Lifestyle Profile II (HPLP II) scale and analysed using descriptive (frequency, percentage, median) and inferential statistics (Mann Whitney U test and Kruskal Wallis test). Result Only 18.7% of the respondents had an excellent health promoting lifestyle followed by good health promoting lifestyle among 60.4%. Highest median score was observed in spiritual 27(10-36), and interpersonal relationship subscale 27(9-36). Whereas, the lowest was observed in physical activity 21(8-32) followed by stress management 24(10-32). Statistically significant difference was observed in age and perception of own health status with spirituality; gender, chronic illness and perception of own health status with interpersonal subscale and age, gender, marital status, occupation and perception of own health status with physical subscale. Conclusion The study concludes the need of health education and awareness to promote healthy lifestyle behaviours with a particular emphasis on indoor physical activities and stress management skills with specific consideration to female and elderly.

In this paper we describe the development, testing, and characterization of three low-emission rate AmLi neutron sources. The sources are used to calibrate the nuclear recoil response of the LUX-ZEPLIN (LZ) dark matter experiment. The sources' neutron emission rate was measured using $^{3}$He proportional tubes. The sources' gamma emissions were characterized using a high-purity germanium (HPGe) detector. 22. Pattern of occlusal contacts in eccentric position of mandible in dental students

Abstract

Introduction: Canine protection and group function are the two stand out occlusal schemes observed during laterotrusive movement of the mandible. The study aims to evaluate the frequency of these occlusal schemes in Nepalese dental students. Materials and methods: A cross-sectional study was conducted among dental students in People’s Dental College and Hospital, Kathmandu, Nepal after the ethical approval from PDCH-IRC. Sixty four undergraduate students aged 21-30 years, were examined for the occlusal contact patterns in lateral mandibular movements using 12μm shim stock. The data were analyzed using SPSS software (version 21) Result: On 1mm laterotrusive position on mandible, 43 subjects (67%) showed group function, 15 subjects (24%) canine guided and 6 subjects (9%) non specified occlusion. On 3mm laterotrusive movement, 31(48%) subjects showed group function and 27 subjects (42%) showed canine guided occlusal scheme. The most frequent mediotrusive contact was the second molar followed by the first molars during both 1mm and in 3mm excursion Conclusion: Majority of occlusal schemes of the study population were group function followed by canine protection and non specified occlusion. Canine protected occlusion scheme was increased on 3mm lateral excursion. Most of mediotrusive side contact seen on 2nd molar on 1mm.

Abstract

The spread of Fake News during this global pandemic COVID-19 has dangerous consequences on economy and health of public. From origin of virus, spread, self-medication to hoaxes on vaccination, it created more panic than the fatality of the virus. For better infodemic preparedness and control, it is necessary to mitigate fear among people, manage rumours, and dispel misinformation. A survey on Fake News during COVID-19 was made by Poynter Fact Check institute. It stated that major chunk of the fake news on COVID-19 originated majorly in Brazil, India, Spain, and the United States. Fake news menace is severe in countries where the trust on online media is high such as Brazil, Kenya and South Africa. Based on these observations, this study provides preliminary insight on the co-relation of the spatial and temporal meta-information of the news like the news source country, the name of the countries specified in the news, and date of publish of news to the credibility of news. The main contribution of this study is to analyse the impact of spatial and temporal information features for classification of fake news, which to the best of our knowledge has not been explored yet. Also, these features are directly not available in any news article available online. Hence, these features are handcrafted. Meta-data of the news article such as origin of news is considered. Additional spatial information is extracted from the news article using NER tagging. Temporal information such as date of origin of news is given as an input to the LSTM model. These features are given as an input to Long Short-Term Memory (LSTM) model along with GloVe vectors and word length vector. A comparative analysis for accuracy is tested of the models with and without spatial and temporal information. The model with spatial and temporal information has achieved noteworthy results in fake news detection. To ensure the quality of prediction, various model parameters have been tuned and recorded for the best results possible. In addition to accuracy, the spatial and temporal information for fake news detection offers several other important implications for government and policy makers that will be instrumental in simulating future research on this subject.

Immunomodulatory effect of different statin regimens on regulatory T-cells among patients with acute coronary syndrome: a systematic review and network meta-analysis of randomized clinical trials Introduction In patients with acute coronary syndrome (ACS), studies suggest that statin therapy, besides its role in reducing the progression of atherosclerosis through its pharmacological effect, plays a pivotal role in controlling the pathogenesis of ACS through the anti-inflammatory effects of suppressor regulatory T cells (Tregs). Purpose We conducted a network meta-analysis (NMA) to determine the effects of low-dose conventional statin therapy (CST) (20 mg/day or less) and high-dose intensive statin therapy (IST) (40 mg/day or more) on the frequency of Tregs and their associated cytokines (IFN-γ, IL-10, TGF-β), compared to placebo. Methods The PubMed, Cochrane Library, and EMBASE databases were searched for randomized clinical trials (RCTs) to identify relevant articles published until June 2021. We pooled data extracted from the included studies using the standardized mean difference (SMD). A random-effects model was used to conduct this NMA. Heterogeneity was evaluated using Cochran's Q- and the I2-test. GRADE(Grading of Recommendations, Assessment, Development and Evaluations) was used to assess the study's quality. Data analysis was conducted using R software. Results A total of 505 patients were enrolled in the 5 RCTs. The NMA indicated a significant increase in Treg frequency in the CST group compared with the control group (SMD 1.77; 95% CI: 0.77 – 2.76; P-value = 0.0005) and a larger increase in the Treg frequency associated with the IST group compared with the control group (SMD 2.12; 95% CI: 1.15 – 3.10; P-value < 0.0001). However, there was significant heterogeneity and inconsistency among the included studies (τ2 = 0.6096; τ = 0.7808; I2 = 91.2% [80.5%; 96.0%]). When compared to the placebo, both CST and IST increased levels of secreted IL-10 (SMD 2.69; 95% CI: 2.07 – 3.31; P-value < 0.0001 and (SMD 2.14; 95% CI: 1.76 – 2.52; P-value < 0.0001), respectively. In comparison to the control group, CST was associated with increased levels of TGF-β (SMD 3.83; 95% CI: 0.63 – 7.0; P-value = 0.0189). This association was not seen in the IST group. IFN-γ levels decreased significantly in both the IST and CST groups (SMD -1.52; 95% CI: -1.94 – -1.10; P-value < 0.0001) and (SMD -2.34; 95% CI: -2.73 – -1.95; P-value < 0.0001) respectively. Conclusions Our study suggests that both high and low dose statin groups showed increased Treg frequency compared to the placebo group. IST showed larger benefits. Statin therapy also increased IL-10 and TGF-β cytokine levels and decreased IFN-γ levels. Therefore, statins have the potential to be the main treatment to reduce the incidence of cardiovascular events and improve cardiac function in patients with ACS through immunomodulatory effect on Treg.

Abstract

Aria is a plant hosting a $${350}\,\hbox {m}$$ 350 m cryogenic isotopic distillation column, the tallest ever built, which is being installed in a mine shaft at Carbosulcis S.p.A., Nuraxi-Figus (SU), Italy. Aria is one of the pillars of the argon dark-matter search experimental program, lead by the Global Argon Dark Matter Collaboration. It was designed to reduce the isotopic abundance of $${^{39}\hbox {Ar}}$$ 39 Ar in argon extracted from underground sources, called Underground Argon (UAr), which is used for dark-matter searches. Indeed, $${^{39}\hbox {Ar}}$$ 39 Ar is a $$\beta $$ β -emitter of cosmogenic origin, whose activity poses background and pile-up concerns in the detectors. In this paper, we discuss the requirements, design, construction, tests, and projected performance of the plant for the isotopic cryogenic distillation of argon. We also present the successful results of the isotopic cryogenic distillation of nitrogen with a prototype plant.

26. Effect of Crop Establishment Methods on Performance of Rice in Rupandehi, Nepal

Abstract

An on-station experiment was conducted to assess the growth, productivity and profitability of rice under various crop establishment methods at the agronomy farm of Paklihawa Campus, Rupandehi, Nepal in 2019. The experiment was laid out in Randomized Complete Block Design with five treatments and four replications. The treatments included: Traditional Transplanting–Random (TTR), Traditional Transplanting–Line (TTL), System of Rice Intensification (SRI), Wet Direct Seeded Rice-Line (WDSRL) and Wet Direct Seeded Rice-Broadcasting (WDSRB). The results showed that plant height, number of tillers at 40 DAT, number of grains per panicle, 1000 grain weight, grain yield, straw yield and grain sterility percentage were significantly influenced by crop establishment methods, whereas number of tillers at 60 DAP, number of effective tillers at harvest, panicle length and harvest index were not significantly different among the treatments. TTR method produced the highest grain yield (4.58 t/ha), straw yield (7.25 t/ha) and benefit-cost ratio (3.28) followed by TTL. Hence, traditional transplanting method of rice establishment was found better than WDSR and SRI in terms of productivity and profitability. Direct seeding and SRI could also be an attractive alternative to transplanting because of lower labor requirement and cost of production

Abstract

District Health System Management is a comprehensive study of the health system of the district as a unit of national health system. The report of District Health System Management has been prepared as per the MBBS 4th year (Third phase) curriculum of Tribhuvan University (TU), Institute of Medicine (IOM), Nepal after the field placement of our group in Gorkha and Kaski districts. We prepared regional health directorate profile, regional tuberculosis center profile, district profile, district health profile of Kaski, hospital profile of Western Regional Hospital, Kaski, and critical analysis on human resources of the Aampipal hospital. We prepared five year plan on ICU services of Gorkha District Hospital and conducted epidemiological study on COPD in WRH, Pokhara. The techniques used in this study were observation, interviews, interactions, participation, secondary data retrieval, analysis and presentation using specific tools and guidelines devised for the same. The field practice proved to be a milestone in enabling the students to develop aptitude in the fields of management, administration and communication in different health set-ups of the country. It is in fact once-in-a lifetime opportunity for the medical students to imbibe the practicality of management skills at various levels.

Abstract

Methods: All the endometrium samples obtained by the procedure of dilatation and curettage and hysterectomy sent for histopathological examination at Pathology Department of Gandaki Medical College Teaching Hospital, Pokhara, Nepal. The study duration was total 12 months ranging from July 2016 to June 2017. All the endometrial samples were processed, sectioned at 4 - 6 μm and stained with routine H & E stain. Patient’s data including age, sex, procedure of the biopsy taken and histopathological diagnosis were noted. A pathologist, using Olympus microscope, reported the slides. Cases were reviewed by a second pathologist whenever necessary. Results: A total of 128 cases were studied. The most common histopathological diagnosis was proliferative endometrium (28.9%) followed by disorder proliferative endometrium (15.65%). Most of the patients were in age group 36 - 45 years comprising 32.03%. Hydatidiform mole comprised of 7.03% and among Hydatidiform mole, partial mole was more common. Dilatation and curettage (82.8%) was the common procedure in compare to hysterectomy for the evaluation of endometrial lesions. Conclusions: In this study, we observed a variety of endometrial lesions. Most of them are benign; among benign, proliferative endometrium was the common histopathological diagnosis followed by disorder proliferative endometrium. Most common presenting age group was found to be at 36 - 45 years. In evaluation of hydatidiform mole, partial mole was more frequent in compare to complete mole. Conventional dilatation and curettage is the preferred method in developing countries with limited resource to screen endometrial lesion and therefore biopsy should be sent for histopathological examination. Thus histopathological examination of routinely stained hematoxylin and eosin is readily available and widely accepted standard technique for evaluation of the endometrial lesions. Abstract

We present gravitational-arc tomography of the cool-warm enriched circumgalactic medium (CGM) of an isolated galaxy (``G1'') at $z \approx 0.77$. Combining VLT/MUSE adaptive-optics and Magellan/MagE echelle spectroscopy we obtain partially-resolved kinematics of MgII in absorption and [OII] in emission. The unique arc configuration allows us to probe 42 spatially independent arc positions transverse to G1, plus 4 positions in front of it. The transverse positions cover G1's minor and major axes at impact parameters of $\approx 10-30$ kpc and $\approx 60$ kpc, respectively. We observe a direct kinematic connection between the cool-warm enriched CGM (traced by MgII) and the interstellar medium (traced by [OII]). This provides strong evidence for the existence of an extended disc that co-rotates with the galaxy out to tens of kiloparsecs. The MgII velocity dispersion ($\sigma \approx 30-100$ km s$^{-1}$, depending on position) is of the same order as the modeled galaxy rotational velocity ($v_{\rm rot} \approx 80$ km s$^{-1}$), providing evidence for the presence of a turbulent and pressure-supported CGM component. We regard the absorption to be modulated by a galactic-scale outflow, as it offers a natural scenario for the observed line-of-sight dispersion and asymmetric profiles observed against both the arcs and the galaxy. An extended enriched co-rotating disc together with the signatures of a galactic outflow, are telltale signs of metal recycling in the $z\sim 1$ CGM., Comment: MNRAS accepted. Abstract

Proportional electroluminescence (EL) in noble gases is used in two-phase detectors for dark matter searches to record (in the gas phase) the ionization signal induced by particle scattering in the liquid phase. The "standard" EL mechanism is considered to be due to noble gas excimer emission in the vacuum ultraviolet (VUV). In addition, there are two alternative mechanisms, producing light in the visible and near infrared (NIR) ranges. The first is due to bremsstrahlung of electrons scattered on neutral atoms ("neutral bremsstrahlung", NBrS). The second, responsible for electron avalanche scintillation in the NIR at higher electric fields, is due to transitions between excited atomic states. In this work, we have for the first time demonstrated two alternative techniques of the optical readout of two-phase argon detectors, in the visible and NIR range, using a silicon photomultiplier matrix and electroluminescence due to either neutral bremsstrahlung or avalanche scintillation. The amplitude yield and position resolution were measured for these readout techniques, which allowed to assess the detection threshold for electron and nuclear recoils in two-phase argon detectors for dark matter searches. Abstract

Aria is a plant hosting a$${350}\,\hbox {m}$$350mcryogenic isotopic distillation column, the tallest ever built, which is being installed in a mine shaft at Carbosulcis S.p.A., Nuraxi-Figus (SU), Italy. Aria is one of the pillars of the argon dark-matter search experimental program, lead by the Global Argon Dark Matter Collaboration. It was designed to reduce the isotopic abundance of$${^{39}\hbox {Ar}}$$39Arin argon extracted from underground sources, called Underground Argon (UAr), which is used for dark-matter searches. Indeed,$${^{39}\hbox {Ar}}$$39Aris a$$\beta $$β-emitter of cosmogenic origin, whose activity poses background and pile-up concerns in the detectors. In this paper, we discuss the requirements, design, construction, tests, and projected performance of the plant for the isotopic cryogenic distillation of argon. We also present the successful results of the isotopic cryogenic distillation of nitrogen with a prototype plant.

Abstract

Amyloidosis represents a heterogeneous group of disorders characterized by the deposition of amyloid protein in various target organs of the body. Amyloidosis is classified as systemic or local. Larynx is the common site for localized amyloid deposition and patient usually presents with hoarseness of voice. Because of the similar clinical and imaging features to malignant laryngeal mass, diagnostic confusion occurs and histopathological confirmation is usually needed. Although laryngeal tracheo-bronchial amyloidosis is rare, it should be considered among the differentials of benign laryngeal tumors.

Abstract

This article presents an analysis and the resulting limits on light dark matter inelastically scattering off of electrons, and on dark photon and axion-like particle absorption, using a second-generation SuperCDMS high-voltage eV-resolution detector. The 0.93 gram Si detector achieved a 3 eV phonon energy resolution; for a detector bias of 100 V, this corresponds to a charge resolution of 3% of a single electron-hole pair. The energy spectrum is reported from a blind analysis with 1.2 gram-days of exposure acquired in an above-ground laboratory. With charge carrier trapping and impact ionization effects incorporated into the dark matter signal models, the dark matter-electron cross section $\bar{\sigma}_{e}$ is constrained for dark matter masses from 0.5--$10^{4} $MeV$/c^{2}$; in the mass range from 1.2--50 eV$/c^{2}$ the dark photon kinetic mixing parameter $\varepsilon$ and the axioelectric coupling constant $g_{ae}$ are constrained. The minimum 90% confidence-level upper limits within the above mentioned mass ranges are $\bar{\sigma}_{e}\,=\,8.7\times10^{-34}$ cm$^{2}$, $\varepsilon\,=\,3.3\times10^{-14}$, and $g_{ae}\,=\,1.0\times10^{-9}$., Comment: 5 pages + title and references, 3 figures and 1 table

Abstract

We present an analysis of electron recoils in cryogenic germanium detectors operated during the SuperCDMS Soudan experiment. The data are used to set new constraints on the axioelectric coupling of axionlike particles and the kinetic mixing parameter of dark photons, assuming the respective species constitutes all of the galactic dark matter. This study covers the mass range from 40 eV/c2 to 500 keV/c2 for both candidates, excluding previously untested parameter space for masses below ∼1 keV/c2. For the kinetic mixing of dark photons, values below 10−15 are reached for particle masses around 100 eV/c2; for the axioelectric coupling of axionlike particles, values below 10−12 are reached for particles with masses in the range of a few-hundred eV/c2.

Abstract

We reanalize data collected with the DarkSide-50 experiment and recently used to set limits on the spin-independent interaction rate of weakly interacting massive particles (WIMPs) on argon nuclei with an effective field theory framework. The dataset corresponds to a total (16660 $\pm$ 270) kg d exposure using a target of low-radioactivity argon extracted from underground sources. We obtain upper limits on the effective couplings of the 12 leading operators in the nonrelativistic systematic expansion. For each effective coupling we set constraints on WIMP-nucleon cross sections, setting upper limits between $2.4 \times 10^{-45} \, \mathrm{cm}^2$ and $2.3 \times 10^{-42} \, \mathrm{cm}^2$ (8.9 $\times 10^{-45} \, \mathrm{cm}^2$ and 6.0 $\times 10^{-42} \, \mathrm{cm}^2$) for WIMPs of mass of 100 $\mathrm{GeV/c^2}$ (1000 $\mathrm{GeV/c^2}$) at 90\% confidence level., Accepted by PRD

Abstract

We present limits on spin-independent dark matter-nucleon interactions using a $10.6$ $\mathrm{g}$ Si athermal phonon detector with a baseline energy resolution of $\sigma_E=3.86 \pm 0.04$ $(\mathrm{stat.})^{+0.19}_{-0.00}$ $(\mathrm{syst.})$ $\mathrm{eV}$. This exclusion analysis sets the most stringent dark matter-nucleon scattering cross-section limits achieved by a cryogenic detector for dark matter particle masses from $93$ to $140$ $\mathrm{MeV}/c^2$, with a raw exposure of $9.9$ $\mathrm{g}\cdot\mathrm{d}$ acquired at an above-ground facility. Abstract

Community health diagnosis is a comprehensive assessment of health status of the community in relation to its social, physical and biological environment. The purpose of community health diagnosis is to define existing problems, determine available resources and set priorities for planning, implementing and evaluation health action, by and for the community. A cross sectional descriptive study was done at Dhampus VDC from 1 Sept 2016 AD (16th Bhadra, 2073 BS) to 16 Sept 2016 AD (31st Bhadra, 2073). The data was collected by questionnaires, anthropometric measurements, interviews and secondary data. The community health diagnosis field visit gave us the opportunity to build our team spirit and taught us the arts of problem solving. Abstract

Community diagnosis is defined as a comprehensive assessment of the state of an entire community in relation to its social, economic, physical and biological environment. It is a process of examining the patterns of disease or health status in the community in order to promote health, prevent disease and manage health services for the community people. As per the curriculum of Tribhuwan University, B.Sc. Nursing first year, we were provided an opportunity to fulfill the practicum of community health nursing in the assigned area Shanti Tole and Teentara Tole, Pokhara, Lekhnath, Ritthepani-27 from 18th June to 14th July 2017. Total population of Ritthepani was 4529, male population was 2100 and female population was 2429. Each of the 13 students was assigned with 10 different families for community diagnosis in order to promote the health of the individual, family and community. The main objective of community diagnosis was to find out the health related aspects and to provide preventive, promotive, curative and rehabilitative services to the individual and the community as a whole. All the findings were presented among the key members of the community. On survey, total population of 130 houses was found 565, among them 296 were male population and 269 were female population. Sanitation of the community was inappropriate. People were not using the health facilities effectively so they were found suffering from different kinds of diseases most commonly gastritis. The common health seeking practice was going to governmental hospital and traditional healers.

Abstract

The experiment was conducted to study the comparative performance of Gririaja birds reared on different feeding regime in Nepal. A total of 384 Giriraja birds of aged 40 weeks were assigned to four treatment groups in four replications with 24 birds (20 female and 4 male) per replication over a period of 180 days. The birds of control group were provided with 130g of feed in confinement. In group 1, 10% feed was replaced by green seasonal forage mainly oat and birds were kept on scavenging, while in group 2, 10 % feed was replaced by similar forage but the birds were in confinement. However, in group 3, birds were provided 10 % less feed on scavenging without forage supplementation. The result showed that the feed consumption required for dozen of egg production was lower (P

Abstract

Contraceptive use and fertility rates vary substantially among developing countries. An important factor, which affects the fertility of any population, is Contraceptive. Contraceptive use varies by age. Nepal over the past 15 years show an impressive increase in the use of modern contraceptive methods from 26 percent in 1996 to 43 percent in 2011. Objectives of the is to assess the knowledge, attitude regarding family planning and the practice of contraceptives among the married women of Dhabi VDC of Eastern Nepal. A descriptive cross-sectional observational study was done in Dhabi VDC. Total of 300 married women age 15-49 sample were taken from family planning center situated in Dhabi. Knowledge, attitude and practice on contraceptives were evaluated with the help of a predesigned questionnaire. Descriptive analysis was done by using SPSS 11.5 software to obtain frequencies and percentages. Out of 300 interviewed women, the mean age was 27.94 years, 98% had heard about Family planning method. Radio was the main Source of family planning information. Regarding the usage of contraceptive methods, about 79.3% had ever used and 63.3 had current using some sort of contraception, among the method used Injectables were the commonly used methods About 71% of married women other child in the further. Despite the knowledge of all family planning methods majority of the women used Injectables. Easily accessible and easily to use were the main reasons for choosing Injectables methods.

The fraction of 218Po that is charged is found to be 0.37 ± 0.03 and the mobility of 218Po+ is (8.6 ± 0.1) × 10−4 cmVs2.

Abstract

We investigated the structural, morphological, and optical properties of Fe-doped ZnO transparent thin films spin coated on ordinary and FTO glass substrates using X-ray diffraction (XRD), scanning electron microscopy (SEM), and spectrometric reflectometry (SR) techniques. The XRD spectra showed the polycrystalline films with a preferential orientation along the c-axis and without any impurity phases. SEM images revealed that the randomly oriented matrix shaped nanoparticles of ZnO doped with 1% Fe (with respect to Zn) transformed into cylindrical long grains at higher dopant concentration (4% Fe). The surface reflectance of the Fe-doped ZnO film, particularly in the wavelength range of 400–650 nm, was approximately two fold higher than the ones with un-doped ZnO film. However, the transmittance of the doped film decreased only by ~10%. The band gap, derived with the aid of measured transmittance curves, of un-doped ZnO thin film ca. 3.26 eV was decreased to ca. 3.21 eV when Fe ions were doped into the film and was independent to the dopant concentration.

Abstract

The Cryogenic Dark Matter Search (CDMS II) experiment aims to detect dark matter particles that elastically scatter from nuclei in semiconductor detectors. The resulting nuclear-recoil energy depositions are detected by ionization and phonon sensors. Neutrons produce a similar spectrum of low-energy nuclear recoils in such detectors, while most other backgrounds produce electron recoils. The absolute energy scale for nuclear recoils is necessary to interpret results correctly. The energy scale can be determined in CDMS II silicon detectors using neutrons incident from a broad-spectrum $^{252}$Cf source, taking advantage of a prominent resonance in the neutron elastic scattering cross section of silicon at a recoil (neutron) energy near 20 (182) keV. Results indicate that the phonon collection efficiency for nuclear recoils is $4.8^{+0.7}_{-0.9}$% lower than for electron recoils of the same energy. Comparisons of the ionization signals for nuclear recoils to those measured previously by other groups at higher electric fields indicate that the ionization collection efficiency for CDMS II silicon detectors operated at $\sim$4 V/cm is consistent with 100% for nuclear recoils below 20 keV and gradually decreases for larger energies to $\sim$75% at 100 keV. Abstract

Future direct searches for low-mass dark matter particles with germanium detectors, such as SuperCDMS SNOLAB, are expected to be limited by backgrounds from radioactive isotopes activated by cosmogenic radiation inside the germanium. There are limited experimental data available to constrain production rates and a large spread of theoretical predictions. We examine the calculation of expected production rates, and analyze data from the second run of the CDMS low ionization threshold experiment (CDMSlite) to estimate the rates for several isotopes. We model the measured CDMSlite spectrum and fit for contributions from tritium and other isotopes. Using the knowledge of the detector history, these results are converted to cosmogenic production rates at sea level. The production rates in atoms/(kg$\cdot$day) are 74$\pm$9 for $^3$H, 1.5$\pm$0.7 for $^{55}$Fe, 17$\pm$5 for $^{65}$Zn, and 30$\pm$18 for $^{68}$Ge., 14 pages, 10 figures, 5 tables. v5 contains the extended data release (and documentation) of the CDMSlite Run 2 data as ancillary files

Abstract

We present the first limits on inelastic electron-scattering dark matter and dark photon absorption using a prototype SuperCDMS detector having a charge resolution of 0.1 electron-hole pairs (CDMS HVeV, a 0.93 gram CDMS HV device). These electron-recoil limits significantly improve experimental constraints on dark matter particles with masses as low as 1 MeV/$\mathrm{c^2}$. We demonstrate a sensitivity to dark photons competitive with other leading approaches but using substantially less exposure (0.49 gram days). Abstract

We report the result of a blinded search for weakly interacting massive particles (WIMPs) using the majority of the SuperCDMS Soudan data set. With an exposure of 1690 kg d, a single candidate event is observed, consistent with expected backgrounds. This analysis (combined with previous Ge results) sets an upper limit on the spin-independent WIMP–nucleon cross section of 1.4×10−44 (1.0×10−44) cm2 at 46 GeV/c2. These results set the strongest limits for WIMP–germanium-nucleus interactions for masses >12 GeV/c2.
