Goat (Capra hircus) is important meat animal of the country. Fattening performance of different breeds of goats with common mid hill fodder trees was conducted in Completely Randomized Design with 4×5 factorial arrangements at Goat Research Station, Tanahun. This study was carried out at Goat Research Station, Bandipur, Tanahun, Nepal to evaluate the different factors affecting the growth performance of pure Boer goat since its arrival on 2015. A total of 225 kids of Boer goat born from 218 kidding does recorded between 2015 and 2019 were used for analysis of birth weight, weaning weight, eight month weight and yearling weight in response to sex and type of birth. The overall least square means for birth weight is 3.246 ± 0.056 kg, weaning weight is 17.86 ± 0.105 kg, eight month weight is 28.66 ± 0.2.6 kg and yearling weight is 45.10 ± 0.218 kg. The least square means across sex for birth weight, weaning weight, eight month weight and yearling weight were 3.368 ± 0.090 kg, 18.42 ± 0.226 kg, 31.69 ± 0.415 kg and 49.29 ± 0.707 kg for male and 3.136 ± 0.068 kg, 17.43 ±0.150 kg, 26.35 ± 0.378 kg and 42.18 ± 0.778 kg for female respectively. Sex and type of birth have significant effect on birth weight and weaning weight, while for eight month weight and yearling weight, sex have significant effect but no significant effect on type of birth. In conclusion, the overall growth performance of Boer goat in Goat Research Station was to the expectation and may increase on improvement of management and environmental effects. Bang. J. Anim. Aim To provide an insight on the UK- Surgical Training Pathways for the International Medical Graduates (IMGs) from Lower- & Middle-Income Country (LMIC's). To eliminate the barriers for the effective and safe patient care. Method A webinar “PLAB & Beyond” was organised by Nepalese Doctors Association-UK on 11thDec.2021 to provide an insight on various aspects of NHS and Training application process for Aspiring Surgeons from LMIC's. Virtual sessions were designed to share the first-hand experience by IMG's on varied NHS roles. A tailored session on Surgical Training pathways and application process was delivered by the IMG currently on the General Surgery Training Pathway. Results There were 70 participants of various grades joining from 3 different continents. The pre-webinar questionnaire suggested that almost 85% of the IMG's were not oriented regarding the General Surgery Training Pathways. Only 30% had a very limited idea about the Surgical-Portfolio Scoring system. 55% thought it was impossible to get into the Surgical Training Pathway in the UK for an IMG. Post webinar feedback suggested that majority of the attendees (∼95%) acknowledged that the webinar enhanced their understanding of NHS and Surgical Training Pathway meeting the aims & learning outcomes. Conclusion As IMGs from LMIC's contribute to major work force in the NHS, proper orientation to the training pathways must be added during local hospital induction. These session in broader sense will also aid to provide improved and equitable surgical care across international health system advocating Global Surgery, the ‘Neglected stepchild of global health’.

The molecular rules driving TCR cross-reactivity are poorly understood and, consequently, it is unclear the extent to which TCRs targeting the same Ag recognize the same off-target peptides. We determined TCR–peptide–HLA crystal structures and, using a single-chain peptide–HLA phage library, we generated peptide specificity profiles for three newly identified human TCRs specific for the cancer testis Ag NY-ESO-1157–165–HLA-A2. Two TCRs engaged the same central peptide feature, although were more permissive at peripheral peptide positions and, accordingly, possessed partially overlapping peptide specificity profiles. The third TCR engaged a flipped peptide conformation, leading to the recognition of off-target peptides sharing little similarity with the cognate peptide. These data show that TCRs specific for a cognate peptide recognize discrete peptide repertoires and reconciles how an individual’s limited TCR repertoire following negative selection in the thymus is able to recognize a vastly larger antigenic pool.

Supplementation of concentrate feed during late pregnancy of doe is an approach to improve milk yield and kid performance after kidding. The experiment was conducted to find out appropriate level and duration of supplementation to the pregnant doe. The data of the experiment were analyzed applying Completely Randomized Design (CRD) with factorial arrangements with three replications. First factor used in the experiment was the duration of concentrate feeding (15 days, 30 days, 45 days and 60 days) and second was amount of concentrate supplemented (2.00%, 1.75%, 1.50% and 1.25% of their body weight). Birth weight, weaning weight, milk yield and kid mortality in different groups were recorded. Birth weight and weaning weight were found higher in 60 days supplemented group i.e. 2.3±0.23 kg and 10.94±0.41 kg, respectively. Both birth weight and weaning weight were also found higher with 2 percent concentrate feeding group than others i.e. 2.1±0.23 kg and 9.64±0.42 kg, respectively. Milk yield recorded for fourth week shows that higher milk yields were found in 2 percent concentrate supplemented group with 60 days time period. The result showed that by increasing the level of concentrate during late gestation period of does increased the birth weight, weaning weight of kids and increased milk yield of does. So the higher level of concentrate supplementation is required during 45 to 60 days of late gestation in does. Bang. J. Anim. Sci. 2019. 48 (1): 23-28

Objective To evaluate the clinical significance of CA19-9 in patients with ovarian mature cystic teratoma (MCT). Materials and methods A retrospective study was performed on 65 patients with pathologically-confirmed MCT and 80 patients with benign epithelial ovarian tumors. Serum tumor markers for all patients and tissue CA19-9 for MCTs were measured. The relationships between clinical characteris- tics of MCTs and CA19-9, as well as the correlation between serum and tissue level of CA19-9 in MCTs, were evaluated. Results The mean serum level of CA19-9 in MCTs was significantly higher than that in benign ovarian epithelial tumors (49.9 ± 73.4 IU/ml vs. 17.08 ± 24.8 IU/ml). CA19-9 was the only tumor marker with a mean serum level above the cut-off value and the elevation rate was 30.76% in MCTs. The positive tissue expression rate of CA19-9 in MCT patients were 50.9% and were higher than that of preoperative serum levels (50.9% vs. 32.7%). Conclusion Serum CA19-9 has the highest positivity rate among other tumor markers in MCT. Elevated serum CA19-9 is not an uncommon finding MCT and could be used as a marker in the differential diagnosis of MCT in patients with pelvic mass.

Tuberculosis is the leading cause of deaths among HIV patients. In this study, we estimated the prevalence of pulmonary tuberculosis (PTB) and identified the factors/co-morbidities associated with active PTB in HIV-infected people visiting the national public health laboratory to assess their eligibility to receive highly active antiretroviral therapy.A cross-sectional study was conducted to measure the prevalence of pulmonary tuberculosis. Data on probable risk factors in patients with and without PTB were compared, calculating the odds ratio as a measure of association. Factors showing significant association in univariate analyses were included in a stepwise backward logistic regression model to adjust for confounding.The prevalence of pulmonary tuberculosis was 32.4 % (95% confidence interval (CI) 30.25-34.56). In the univariate analysis, patients with PTB were more likely to be older, married, and have a longer duration since the diagnosis of HIV, diarrhoea, parasitic infection, lower CD4 T-cell counts, and lower CD4/CD8 ratio. However, the backward stepwise logistic regression revealed that only the CD4 T-cell count200/μL (AOR 11.69, 95% CI 6.23-21.94), CD4 T-cell count 200-350/μL (AOR 2.52, 95% CI 1.30-4.89), diarrhoea (AOR 2.77, 95% CI 1.78-4.31), parasitic infection (AOR 3.34, 95% CI 2.02-5.50) and 'sex with partner' as probable modes of transmission (AOR 0.44, 95% CI 0.20-0.93) were independently associated with pulmonary tuberculosis.A high prevalence of pulmonary tuberculosis was observed. Participants with tuberculosis were significantly more likely to have lower CD4 counts, diarrhoea, and parasitic infections. HIV treatment programmes should consider these factors for better outcomes.

In Rio Grande do Sul (RS), as in Pais Vasco (PV), some kidneys are retrieved or offered and not accepted for transplantation. This study aimed to evaluate the profile of the available kidneys and the reasons for them not being accepted in the 2 regions, and to compare the characteristics of the organs and reasons for refusal. All of the kidneys retrieved or offered in RS in December 2012 and in PV from September to December 2012 were evaluated. Data were collected from each local donation registry. There were 61 kidneys available in RS and 61 in PV in the study period. Of these, 16 kidneys (26%) in RS and 27 (44%) in PV were not implanted. The age of the donors was higher in PV (59 years) than in RS (45 years; P = .000), as was the age of the donors of accepted kidneys (62 and 41 years old, respectively; P = .000). The proportion of donors considered to be “extended criteria” was higher in PV (78%) than in RS (47%; P = .001), and the refusal rate of the kidneys from these donors was the same in the 2 regions. The reasons for not using the kidneys in RS and in PV were similar and absolute. It is concluded that there is no organ waste in the 2 regions, but that the offer of kidneys can be expanded in RS by considering elderly donors for evaluation, even if this means a higher number of refused organs.

The composites of (1-x) La0.7Ca0.2Sr0.1MnO3 + (x) MgO with x = 0.0, 0.05, 0.10 (abbreviated as LCSMO - MgO) were synthesized by conventional solid-state reaction method. X-ray diffraction (XRD) data infers that manganite LCSMO has an orthorhombic structure (Pnma space group) while to that MgO compound crystallizes in cubic structure (space group Fd3m). XRD shows the coexistence of MgO and La0.7Ca0.2Sr0.1MnO3 phases in doped composites through their characteristic peaks. The magnetoresistance (MR) in this system increases with increasing MgO content, showing a maximum value (91% at 50K and at lower fields) for (0.90) LCSMO - (0.10) MgO sample, which is larger than the largest MR value of pure La0.7Ca0.2Sr0.1MnO3. (44% at 50K).

Highly active anti-retroviral therapy (HAART) has been freely available in Nepal since 2004. In the present longitudinal study, we followed two distinct cohorts of human immunodeficiency virus-infected participants, those receiving HAART and those under assessment of eligibility for HAART, during the period 2005-2007 in Kathmandu, Nepal. The median change in CD4+ T-cell count among participants receiving HAART after 12 months of the initiation of therapy was +118 T cells/μl (95% confidence interval [CI], +91 to +145 T cells/μl) and that among participants under assessment of eligibility for HAART was -74 T cells/μl (95% CI, -103 to -44 cells/μl). However, the median CD8+ T-cell count after 12 months remained stable in both the cohorts (P0.05). The CD4+/CD8+ T-cell ratio increased from 0.16 to 0.26 after 12 months of therapy (P0.001). The multivariate regression model revealed that participants30 years of age, and injection drug users had significantly lower increases in the CD4+ T-cell count in response to therapy. We observed a high proportion of loss to follow-up after 12 months of therapy; however, the associated factors were unknown. In conclusion, we observed a significant improvement in the CD4+ T-cell count in participants receiving HAART; however, the CD4+/CD8+ T-cell ratio remained0.5 after 12 months of treatment.

Hearing loss is the most common sensory deficit in the elderly, and is becoming a severe social and health problem. Presbycusis is the result of aging which can lead to communication problems compromising the quality of life (QoL).Since the elderly population is increasing worldwide, presbycusis is showing a similar trend. This study intended to identify the impact of hearing loss in the social life of the elderly. A total of 70 elderly patients attending Nepal Medical College Teaching Hospital who were found to have sensorineural hearing loss (SNHL) on pure tone audiometry (PTA) were recruited for this study.None of the patients had used hearing aids in the past. To assess their handicap due to hearing impairment a Hearing Handicap Inventory for the Elderly (HHIE) questionnaire was used and patients were graded as: no handicap, mild to moderate handicap and significant handicap. Pure-tone averages (PTA) were calculated for the thresholds at 0.5, 1 and 2 kHz in each ear. Patients with their PTA values between 26 to 40 dBHL were interpreted as having mild SNHL, those between 41 to 55dBHL as moderate SNHL and those above 55 dBHL as severe SNHL. Out of 70 patients, 65 had some degree of handicap ranging from mild to severe. The severity of handicap was significantly associated with the degree of hearing loss in both ears.

Background Disc diffusion technique is the routine susceptibility testing procedure for isolates of enteric fever, the most common clinical diagnosis among febrile patients in Nepal. Objective To evaluated the current fluoroquinolones (FQs) susceptibility criteria and nalidixic acid screening test in Salmonella enterica serovar Typhi and Paratyphi A. Methods S. Typhi and Paratyphi A strains isolated from 443 suspected enteric fever patients visiting National Public Health Laboratory (NPHL) during April through October 2008 were analyzed. All isolates were confirmed by standard microbiological procedures including serotyping. Antibiotic susceptibility testing was performed by using Kirby Bauer disc diffusion method and Clinical and Laboratory Standards Institute (CLSI) approved interpretive criteria. Agar dilution method was used to determine Minimum Inhibitory Concentration (MIC) of ciprofloxacin, ofloxacin and nalidixic acid. Result Out of 41 Salmonella isolates, 80.49% were nalidixic acid resistant, with S. Paratyphi A showing higher resistance rate (88.23%) compared to S . Typhi (75%). The difference in both MIC and zone diameter in nalidixic acid susceptible and nalidixic acid resistant isolates was found to be significant (P < 0.001) and decreased susceptibility to FQs was strongly correlated (sensitivity and specificity of 100%) with resistance to nalidixic acid. Regression analysis of MIC against zone diameter based on the current CLSI recommended guidelines suggests that accommodation of current susceptible and resistant MIC requires increase in the zone diameter of ciprofloxacin and ofloxacin. Conclusion Before using these drugs for management of enteric fever, appropriate identification of Salmonella isolates with reduced susceptibility to FQs is essential to limit the possible treatment failure and development of highly resistant strains. The current FQs susceptibility break point criteria for Salmonella need re-evaluation. Cholera occurs in sporadic cases and outbreaks in Nepal each year. Vibrio cholerae O1 (n = 522) isolated during 2007-2010 from diarrheal patients at 10 different hospital laboratories in Nepal were characterized. Biochemical and serologic identifications showed that all the isolates belonged to serogroup O1, El Tor biotype. Except 72 isolates of Inaba serotype isolated in the year 2007, all the remaining isolates were of Ogawa serotype. All isolates were resistant to nalidixic acid and furazolidone. Resistance to tetracycline, ciprofloxacin, erythromycin and co-trimoxazole were 21, 4, 16 and 90 % respectively. Seventy-seven of these isolates were selected for further characterization for ctxB gene and MLVA typing. Two different variants of classical type cholera toxin were observed. Ogawa strains from 2007 and 2010-Western Nepal outbreak harbored CTX-3 type cholera toxin, whereas Inaba serotypes in 2007 and the remaining Ogawa serotypes in 2008-2010 harbored CTX 3b-type toxin. MLVA analysis showed circulation of four different groups of altered V. cholerae O1 El Tor strains. Two different profiles were seen among 2007 Inaba (9, 3, 6, x, x) and Ogawa (10, 7, 6, x, x) isolates. The MLVA profile of 2008 and 2009 Ogawa isolates were similar to those of Inaba strains of 2007. Isolates from 2010 also showed three different MLVA profiles; profile 9, 3, 6, x, x in 3 isolates, 11, 7, 6, x, x among 2010 Western Nepal outbreak strains and profile 8, 3, 6, x, x among isolates from Butwal and Kathmandu.

A comparative study of one hundred and five mature placentas from Kathmandu Maternity Hospital in Nepal and 4764 mature placentas as well as 325 premature placentas in Tokyo was undertaken. In general, the infants and the placentas in the Nepalese group weighed significantly less in their mean values than those in the Japanese group. Their weights corresponded to those at 35 – 37 weeks of gestation in the Japanese group. The gross characteristics of the Nepalese placentas were represented by extensive lesions such as extrachorial placenta, marked subchrionic fibrin, chorionic cysts and abnormal insertion of the umbilical cord. On histologic examination, the Nepalese placentas showed a high incidence of inflammation as well as intensive crowding of villi associated with narrowness of the intervillous space. It is suggested that such low weights of the fetus and the placenta in the Nepalese group may result from a reduced functional capacity of the feto-placental unit affected by specific environmental factors in Nepalese women.

We report on two years of Japanese encephalitis (JE) surveillance in Nepal and the implications for a national immunization strategy. From May 2004 to April 2006, 4,652 patients with encephalitis were evaluated. A serum or cerebrospinal fluid specimen was collected from 3198 (69%) patients of which 1,035 (32%) were positive by Japanese encephalitis IgM ELISA. Most cases (N = 951, 92%) were from the 24 Terai districts (i.e., southern plains, 12.3 million persons) with the majority (N = 616, 65%) from four western Terai districts (population = 1.8 million). The case fatality ratio was 14.7% and 6.3% and the proportion of cases under 15 years old was 52% and 62% in the four western and 20 non-western Terai districts, respectively. Japanese encephalitis immunization targeting residents one year of age and older in the western districts and one through 14 years old in the non-western Terai districts may have reduced Japanese encephalitis cases by 84% and deaths by 92%, nationally.

A mild and efficient synthesis of bis(indolyl) methanes by the reaction of indoles with various aldehydes at room temperature in the presence of a catalytic amount of a La(NO3)3 · 6H2O afforded the corresponding bis(indolyl) methanes in excellent yields under solvent‐free conditions.

A mild and highly efficient stereoselective reaction of 3,4,6-tri-O-acetyl-D-glucal with a variety of nucleophiles, viz. alcohols, phenols, thiols, thiophenols, and allyl trimethyl silane (TMS), in the presence of 5 mol% of lanthanum(III) nitrate hexahydrate under solvent-free conditions yielded the corresponding 2,3-unsaturated glycopyranosides (pseudoglycals) in excellent yields.

A mild and highly efficient synthesis of chiral tetrahydroquinolino pyranose derivatives is described. The reaction of a chiral α,β-unsaturated aldehyde containing a hydroxyl group with different aryl amines using lanthanum(III) nitrate hexahydrate as a catalyst afforded corresponding chiral tetrahydroquinolinated pyranosides in excellent yields. © 2008 Wiley Periodicals, Inc. The composites of (1-x) La0.67Ba0.33MnO3 (LBMO) + xBaTiO3 (BTO) (x = 0, 0.25 and 1.0) were synthesized by conventional solid-state reaction method. Rietveld refinement was employed to characterize the structural information of the prepared ceramics. The result of the Rietveld refinement of X-ray powder diffraction of La0.67Ba0.33MnO3 and BaTiO3 shows that these compounds crystallize in rhombohedral (R3c) and tetragonal (P4mm), respectively. The structural parameters and the reliability factors for the LBMO-BTO composite ceramics were successfully determined by the Rietveld refinement. At room temperature, Raman active phonon modes predicted by the group theory were observed only in BaTiO3 and composite sample. Pure LBMO does not show any Raman active Phonon mode at room temperature.

Melting ice is one of the concerns of world climate scientists and planners. It is the gradual change of ice in the liquid form caused by increase in atmospheric temperature. Global Warming is the increase in atmospheric temperature caused by greenhouse effects (C02, CH4, N20 and CFCs). Increase in temperature of atmosphere by 1oC that has been occurred since 1860 AD (IPCC, 1996). Scientists predicted that the Earth would warm by 1.4 to 5.8oC by 2100. In case of Nepal, increase of temperature was recorded 0.06oC per year (DHM). Sea level rise causes by melting ice is recent warning sign. The Times Center, 2007 estimated sea level rise would be 51 -140 cm by 2100 AD. It affects wide variety of ecosystem, water resources, agriculture, life and livelihoods of people in the world. However, its impact is largely unpredictable and uncertainty. There were series of glacial outburst in Nepal since 1964, destroying ecosystem of surrounding region caused by global warming. Therefore, IPCC member's countries with United State of America (USA) must act quickly to reduce greenhouse gasses to overcome threat of melting ice. A simple, convenient and general method has been developed for the synthesis of α-aminonitriles by a one-pot three component condensation of aldehydes, amines and trimethyl silyl cyanide in acetonitrile in the presence of a catalytic amount of La(NO 3 ) 3 ·6H 2 O or GdCl 3 ·6H 2 O.

Chemical investigations of the ascidian Didemnum obscurum has resulted in the isolation of four new lamellarin alkaloids, lamellarin-ζ ( 1 ), lamellarin-η ( 2 ), lamellarin-ϕ ( 3 ) and lamellarin-χ ( 4 ) along with seven known lamellarins, lamellarin-K ( 5 ), lamellarin-I ( 6 ), lamellarin-J ( 7 ), lamellarin-K triacetate ( 8 ), lamellarin-L triacetate ( 9 ), lamellarin-F ( 10 ) and lamellarin-T diacetate ( 11 ). The structures of the compounds 1–11 were established by detailed analysis of NMR spectral data. Cytotoxic activity of the isolates has been done against coloractal cancer cells.
