Purpose: To evaluate pregnancy outcomes following embryo transfer in patients with endometrial carcinoma (EMCA) or endometrial hyperplasia (EH) who elected for fertility-sparing treatment (FST). Methods: This retrospective cohort study at a large urban university-affiliated fertility center included all patients who underwent embryo transfer after fertility-sparing treatment for EMCA or EH between January 2003 and December 2018. Primary outcomes included embryo transfer results and a live birth rate (defined as number of live births per number of transfers).Results: There were 14 patients, 3 with EMCA and 11 with EH, who met criteria for inclusion with a combined total of 40 embryo transfers. An analysis of observed outcomes by sub-group, compared to the expected outcomes at our center (patients without EMCA/EH matched for age, embryo transfer type and number, and utilization of PGT-A) showed that patients with EMCA/EH after FST had a significantly lower live birth rate than expected (Z = -5.04, df =39, p < 0.01). A sub-group analysis of the 14 euploid embryo transfers resulted in a live birth rate of 21.4% compared to an expected rate of 62.8% (Z = -3.32, df = 13, p < 0.001).Conclusions: Among patients with EMCA/EH who required assisted reproductive technology, live birth rates were lower than expected following embryo transfer when compared to patients without EMCA/EH at our center. Further evaluation of the impact of the diagnosis, treatment and repeated cavity instrumentation for FST is necessary to create an individualized and optimized approach for this unique patient population

Background: Key challenges of biopsy-based determination of prostate cancer aggressiveness include tumour heterogeneity, biopsy-sampling error, and variations in biopsy interpretation. The resulting uncertainty in risk assessment leads to significant overtreatment, with associated costs and morbidity. We developed a performance-based strategy to identify protein biomarkers predictive of prostate cancer aggressiveness and lethality regardless of biopsy-sampling variation. Methods: Prostatectomy samples from a large patient cohort with long follow-up were blindly assessed by expert pathologists who identified the tissue regions with the highest and lowest Gleason grade from each patient. To simulate biopsy-sampling error, a core from a high- and a low-Gleason area from each patient sample was used to generate a ‘high' and a ‘low' tumour microarray, respectively. Results: Using a quantitative proteomics approach, we identified from 160 candidates 12 biomarkers that predicted prostate cancer aggressiveness (surgical Gleason and TNM stage) and lethal outcome robustly in both high- and low-Gleason areas. Conversely, a previously reported lethal outcome-predictive marker signature for prostatectomy tissue was unable to perform under circumstances of maximal sampling error. Conclusions: Our results have important implications for cancer biomarker discovery in general and development of a sampling error-resistant clinical biopsy test for prediction of prostate cancer aggressiveness.

Too many special needs students leave school without being able to compete effectively for jobs which require problem solving, skilled literacy, knowledge of technology, and at least adequate interpersonal skills. In this article, key problem solving and decision making skills in which special needs students are deficientare outlined and evidence is presented for the importance of teaching these skills. A specific procedure called the Personal Problem Solving Guide, utilizing readily available personal computer technology to teach children social decision making and problem solving, is described. The Guide provides formats that allow for constructive behavior in discipline situations, as well as anticipatory guidance for managing conflicts, thinking about health choices, and reviewing everyday interpersonal and academic situations before taking impulsive action. BACKGROUND AND PURPOSE Glucagon-like peptide-1 (GLP-1) is secreted from enteroendocrine L-cells after food intake. Increasing GLP-1 signalling either through inhibition of the GLP-1 degrading enzyme dipeptidyl-peptidase IV or injection of GLP-1-mimetics has recently been successfully introduced for the treatment of type 2 diabetes. Boosting secretion from the L-cell has so far not been exploited, due to our incomplete understanding of L-cell physiology. Elevation of cyclic adenosine monophosphate (cAMP) has been shown to be a strong stimulus for GLP-1 secretion and here we investigate the activities of adenylate cyclase (AC) and phosphodiesterase (PDE) isozymes likely to shape cAMP responses in L-cells. EXPERIMENTAL APPROACH Expression of AC and PDE isoforms was quantified by RT-PCR. Single cell responses to stimulation or inhibition of AC and PDE isoforms were monitored with real-time cAMP probes. GLP-1 secretion was assessed by elisa. KEY RESULTS Quantitative PCR identified expression of protein kinase C- and Ca2+-activated ACs, corresponding with phorbolester and cytosolic Ca2+-stimulated cAMP elevation. Inhibition of PDE2, 3 and 4 were found to stimulate GLP-1 secretion from murine L-cells in primary culture. This corresponded with cAMP elevations monitored with a plasma membrane targeted cAMP probe. Inhibition of PDE3 but not PDE2 was further shown to prevent GLP-1 secretion in response to guanylin, a peptide secreted into the gut lumen, which had not previously been implicated in L-cell secretion. CONCLUSIONS AND IMPLICATIONS Our results reveal several mechanisms shaping cAMP responses in GLP-1 secreting cells, with some of the molecular components specifically expressed in L-cells when compared with their epithelial neighbours, thus opening new strategies for targeting these cells therapeutically.

Conclusions: These studies reveal no evidence for an allosteric trapping mechanism and indicate that all PARP inhibitors examined trap PARP1 via catalytic inhibition. The potency of PARP inhibitors with respect to trapping and catalytic inhibition is linearly correlated in biochemical systems. In cells, trapping potency is related to concentrations required for potentiation in animal models. Detection of PARP trapping in cells requires supraphysiologic conditions that exhaust cellular NAD and PAR, exceed concentrations required to elicit synergism and are not tolerated in vivo. In addition trapping potency appears to be inversely correlated with tolerability. Quantitation of the degree of trapping that is tolerable and is required for therapeutic benefit is under active investigation. Disclosures: All authors are employees of AbbVie. Synthesis and processing of nanostructures will employ a diverse array of material types—organic, inorganic, and biological—well beyond examples already realized. The driving forces will be creativity, applications, opportunities, and economics in broad areas of science, medicine, and technology. Increasing emphasis will be placed on synthesis and assembly at a very high degree of precision, achieved through innovative processing. The result will be control of the size, shape, structure, morphology, and connectivity of molecules, supermolecules, nano-objects and nanostructured materials and devices. Integration of top-down physical assembly concepts with bottom-up chemical and biological assembly concepts may be required to create fully functional nanostructures that are operational at mesoscopic scales. The combination of new nanoscale building blocks and new paradigms in assembly strategies will provide nanostructured materials and devices with new, unprecedented capabilities limited only by our imagination.

White mural thrombosis has been very rarely observed in the rabbit's aorta in experimental cholesterol atherosclerosis. Apparently cholesterol atherosclerosis by itself is not the appropriate damage or alteration required to trigger or initiate thrombosis in rabbits. It appeared to be of interest to determine if the hyperlipidemic state of the rabbit fed cholesterol could modify the amount of white thrombus formed after thrombosis was initiated by insertion of a standard length of polyethylene tube into the abdominal aorta. Twenty rabbits fed 0.5 gm cholesterol per day for 24 days developed severe hyperlipidemia. Polyethylene tubes were inserted into their abdominal aortae on the 24th day and rabbits were sacrificed on the 28th day. In this group, 61.1 mg ± 17.1 S.D. fresh wet weight of white thrombus or 17.0 mg ± 4.6 S.D. freeze-dried weight were found. In comparison, 20 normolipidemic control rabbits showed 67.2 mg ± 4.6 S.D. fresh weight or 15.7 ± 8.8 freeze-dried weight (P > 0.5). It is concluded that hypercholesterolemia induced by feeding cholesterol to rabbits does not modify the amount of white thrombus induced to form by a foreign plastic surface.

Four large solar thermal electric plants, 15–30 MWe in scale, using Luz parabolic trough collectors are operating in Southern California, U.S.A. This paper reviews the collector designs, system configurations, and plant operation. In addition, the financing structure of these investor-owned projects is outlined. Performance of the latest 30 MWe plants is presented and discussed. Finally, plans for the next three systems, the third generation of the collector technology, and advancement in system design and plant size are summarized.

A study of the patient register at Kopata Medical Centre, was undertaken to determine its accuracy. A 10% sample gave an inflation rate of 4.2% and an overall accuracy of 92.4%. There had been a great improvement in the accuracy over the last seven years and we conclude this is mainly due to considerable use of the register for patient recall.

In at least 80 per cent of the patients seen by the cardiologist, anxiety prolongs and intensifis the physical disorder. The prognosis depends largely on the ability of the physician to control the anxiety factor, as well as the somatic disease For ambulatory patients, meprobamate is believed the most suitable of the ataractic agents for this purpose. A series of 41 adult patients who had survived one or more episodes of coronary thrombosis, or were suffering from coronary insufficiency, labile essential hypertension, or cardiac neurosis, received meprobamate in initial doses usually of 400 mg, three or four times a day, in conjunction with the appropriate specific therapy. On control of the acute symptoms dosage was halved, and administered at longer intervals. Medication was continued from two months to one year. On control of the emotional complications, treatment in every case was less intensive and prolonged than ordinarily would have been expected. The postcoronary and hypertensive patients were less apprehensive and excitable, and convalescence was more rapid. Drowsiness in the first few days of medication occurred in a few but subsided with no reduction in dosage. Meprobamate is essentially nontoxic and therefore well suited for prolonged treatment in chronic disorders with emotional complications.
