7. Metabolic Risk Factors in Children with Urinary System Stones.

Abstract

Objective: To diagnose, treat, and prevent stone recurrence, it is important to determine the metabolic risk factors that play a role in developing urinary system stone disease in children. This study assessed children with urinary system stones’ clinical, radiological, and metabolic characteristics. Material And Methods: A retrospective study was conducted on the records of pediatric patients who applied to our pediatric outpatient nephrology clinic for various reasons between February 2018 and December 2023 and were diagnosed with urinary system stones. Results: Of the 122 patients with a mean age of 4.40±4.16 years (1 month-17 years), 63 (51.6%) were boys and 59 (48.4%) were girls. In 61.4% of the children, a family history was identified. The most common presenting symptom was abdominal/flank pain or restlessness (47.5%). In 25.4% of the patients, the stones were ≤3 mm (microlithiasis), and most stones were in the upper system. One or more metabolic abnormalities have been detected during urine analysis for 58.2% of patients. The most frequent metabolic abnormalities were hypercalciuria (20.5%) and hypocitraturia (17.2%). In 74.6% of patients, the size of stones decreased or completely disappeared with medical treatment based on underlying metabolic abnormalities, and in 17.2%, they did not change at all. Only eight (6.6%) patients required interventional procedures. Conclusion: Metabolic causes should be investigated first in all children with urinary tract stones. Special medical treatments designed to alter metabolism reduce the need for invasive stone procedures. [ABSTRACT FROM AUTHOR]

Abstract

Megalourethra is a rare congenital anomaly of the urethra that develops as a consequence of dysgenesis of the corpus cavernosum and corpus spongiosum. Herein, we present three congenital abnormalities in one newborn ie; Down syndrome, posterior urethral valve (PUV) and scaphoid megalourethra. The patient was treated successfully by ablation PUV and reduction uretrophlasty. The postoperative period was uneventful and the renal functions could be preserved in the long term. Abstract

Objective: To elucidate the mechanism of a possible protective effect of montelukast against testicular ischemia/reperfusion (I/R) injury. Material and methods: Fifty-one adult male Wistar-Albino rats were randomly assigned into 6 groups; sham + saline (S), sham + montelukast (M), I/R + S, I/R + S 30', I/R + M and I/R + M 30'. Saline or montelukast (10 mg/kg) was intraperitoneally administered 30 minutes prior to (S 30', M 30') and during detorsion (I/R + S, I/R + M) in the I/R groups. The I/R groups underwent 2 hours of ischemia followed by 4 hours (early-term) of reperfusion in unilateral testes. Half of the rats underwent 24 hours (late-term) of reperfusion to investigate long-term effects. Testicular tissue samples were examined for biochemical and histopathological parameters. Germ cell apoptosis was evaluated using apoptosis-activating factor 1 (Apaf-1). Inducible nitric oxide synthase (iNOS) activity was analyzed in late-term reperfusion groups. Spermatogenic functions were assessed for each testis based on the Johnsen criteria. Results: Unilateral I/R caused a significant increase in serum TNF-α levels in the early-term group compared to the sham groups. Malondialdehyde levels and myeloperoxidase activity were found to be elevated in the I/R groups and accompanied with a significant decrease in glutathione levels when compared to the sham groups. I/R significantly increased iNOS activity and germ cell apoptosis compared to the sham groups. Montelukast treatment significantly reversed all of these parameters and achieved comparable results with the sham groups. Finally, spermatogenic indices were similar for the bilateral testes between all groups. Conclusion: Montelukast exerts protective effects against testicular I/R injury by inhibiting neutrophil activity, reversing the oxidative stress markers, decreasing iNOS activity and attenuating apoptosis. Abstract

Objective: To report our experience with transcatheter embolization (TCE) for iatrogenic renal hemorrhage at our institution. Materials and methods: A total of 18 patients underwent superselective embolization for iatrogenic renal hemorrhage from 2005 to 2010 at our hospital. Patient age, gender, Clavien classification, transfusion units, indications for TCE and embolic material were noted. The numbers of TCE and percutaneous nephrolithohomy (PNL) performed during the years of the study were recorded. The data are represented as the mean±standard deviation. Results: Of 18 patients (12 male, 6 female), the mean age was 39.3±12.0 years (range 18 to 65). The procedures leading to massive renal hemorrhage were renal biopsy (n=2), nephrostomy placement (n=1), partial nephrectomy (n=3) and PNL (n=12). In total, there was 3 performances of TCE in the first two years, 9 in the second two years and 6 in the remaining two years. Accordingly, the numbers of PNLs performed during these periods were 92, 208 and 143, respectively. The mean number of transfusion units prior to TCE was 2.9±2.2. The Clavien grade was IIIa for 17 patients and IVa for one patient. Technical success was achieved in 94.4% of the patients, whereas clinical success was achieved in 83.3%. Conclusion: TCE is a safe and effective procedure for the treatment of iatrogenic renal hemorrhage. PNL is the most common procedure leading to the need for TCE because of its frequent performance at our institution. [ABSTRACT FROM AUTHOR]
