Hepatit serolojileri, hastaların demografik, klinik ve laboratuvarbulguları kayıt edildi. HBV, HCV olan hastalar ile aynı dönemde nakil olmuşHBV, HCV olmayan hastalarla kontrol grubu oluşturularak verilerikarşılaştırıldı. Bulgular : Toplam 872 hastanın 15 ‘inde (%1,7) HBV (9E,ortalama yaş 42±11 yıl, ortalama izlem süresi 56,6±26 ay), 20'sinde(%2,3) HCV(15E, ortalama yaş 50,6±14,9 yıl, ortalama izlem süresi 46,6±28,2 ay), olduğugörüldü. Kontrol grubuna 35 hasta (23E, ortalama yaş 47±10,4 yıl, ortalamaizlem süresi 51,4±24,3 ay) alındı. Gruplar arasında yaş, cinsiyet ve takipsüresi açısından anlamlı fark saptanmadı (sırasıyla p:0.12, 0.62,0,52). HBVhastalarının %53,3'ünde , kontrol grubunun ise %60'ında indüksiyon tedavisiolarak Anti timosit globulin(ATG) kullanılırken HCV hastalarının %75'indeSimulect kullanımı saptandı. Introduction: Management of COVID-19 in kidney transplant recipients should include treatment of the infection, regulation of immunosuppression, and supportive therapy. However, there is no consensus on this issue yet. This study aimed to our experiences with kidney transplant recipients diagnosed with COVID-19. Material and Methods: Kidney transplant recipients diagnosed with COVID-19 from five major transplant centers in Istanbul, Turkey, were included in this retrospective cohort study. Patients were classified as having moderate or severe pneumonia for the analysis. The primary endpoint was all-cause mortality. The secondary endpoints were acute kidney injury, the average length of hospital stay, admission to intensive care, and mechanical ventilation. Results: Forty patients were reviewed retrospectively over a follow-up period of 32 days after being diagnosed with COVID-19. Cough, fever, and dyspnea were the most frequent symptoms in all patients. The frequency of previous induction and rejection therapy was significantly higher in the group with severe pneumonia compared to the moderate pneumonia group. None of the patients using cyclosporine A developed severe pneumonia. Five patients died during follow-up in the intensive care unit. None of the patients developed graft loss during follow-up. Discussion: COVID-19 has been seen to more commonly cause moderate or severe pneumonia in kidney transplant recipients. Immunosuppression should be carefully reduced in these patients. Induction therapy with lymphocyte-depleting agents should be carefully avoided in kidney transplant recipients during the pandemic period., NA

Amaç: Bu çalışmada Hepatit B ve Hepatit C virüsleriyle karşılaşmış ve böbrek nakli yapılmış hastaların böbrek fonksiyon testlerinin, karaciğer enzimlerinin ve kan değerlerinin Hepatit B ve Hepatit C virüsleriyle karşılaşmamış böbrek nakli olmuş hastalarla karşılaştırılması amaçlanmaktadır. Gereç ve Yöntem: Araştırma, İstanbul Bilim Üniversitesi Şişli Florence Nightingale Hastanesi Genel Cerrahi Anabilim Dalında 2008-2016 yılları arasında böbrek nakli yapılmış 872 hasta dosyası retrospektif olarak taranıp hepatit B ve hepatit C virüslerinin hasta ve greft sağ kalımına etkileri incelenmiştir. Bulgular: Araştırmaya dahil edilen hastaların HBV(+) grubundakilerin %86,7'si, HCV(+) grubundakilerin tamamı ve kontrol grubundakilerin %97'si için greft tipinin canlı olduğu belirlendi. Hastalık nedenleri içinde; hipertansiyon HBV(+) hastalarda HCV(+) hastalara, kontrol grubu ise hem HBV(+)hem de HCV(+) hastalara göre anlamlı olarak daha yüksek bulunmuştur. Hastaların en son kotrol BUN değerine bakıldığında, HCV(+) grubu kontrol grubundan anlamlı olarak yüksek bulundu. Hastaların 5.yıldaki AST değerine bakıldığında, HCV(+) grubun kontrol grubundan anlamlı olarak yüksek bulundu. Hastaların taburcu oldukları an ki ürik asit değerine bakıldığında, HBV(+) grubunun ortalama HCV(+) grubundan anlamlı olarak yüksek bulundu. Sonuçlar: Bulgularımız böbrek transplantasyonu hastalarında hepatit B ve hepatit C virüs enfeksiyonun böbrek fonksiyonları üzerinde etkisi olmadığı gibi nakil sonrasında gelişebilecek komplikasyonlar üzerindede etkisi yoktur. Dolayısıyla Hepatit B ve hepatit C virüs enfeksiyonunun hasta sağ kalımına ve greft sağ kalımına etkisi olmadığı düşünülebilir. Objective: The aim of this study is to compare the renal function tests of renal transplantation patients with hepatitis B and hepatitis C viruses and liver enzymes and blood donors in patients with renal transplantation who were not exposed to Hepatitis B and Hepatitis C viruses.Materials and Methods: The study was carried out retrospectively in 872 patient files of renal transplantation performed between 2008 and 2016 at Istanbul Bilim University Sisli Florence Nightingale Hospital General Surgery Department. The effects of hepatitis B and hepatitis C virus on patient and graft survival were investigated. Findings: The patients included in the study were 86.7% of the HBV (+) group, all of the HCV (+) group and 97% of the control group were alive. In hypertension, HBV (+) HCV (+) is significantly higher in the experimental group. In addition, the control group was significantly higher than HBV (+) and HCV (+). When the BUN value of the patients was examined, the HCV (+) group had the highest other groups. HCV (+) group was significantly higher than control group. When the AST value of the patients at 5th year was examined, it was seen that the HCV (+) group had the highest other groups. HCV (+) group was significantly higher than control group. When the current uric acid value of the patients were discharged, it was seen that the HBV (+) group had the highest other groups. HBV (+) group was significantly higher than the mean HCV (+) group.Conclusions: Our findings showed that hepatitis B and hepatitis C virus infection did not affect survival and graft survival in patients with renal transplantation. 70
