Tarım ürünlerinin kurutulması ve mekân ısıtması gibi uygulamalarda ihtiyaç duyulan sıcak hava, hava ısıtıcılı güneş kollektörleri ile elde edilebilmektedir. Suya göre, havanın ısı transfer katsayısı daha düşüktür. Bu sebeple hava ısıtıcılı güneş kollektörlerinde yüzey alanı artırılmış depolama malzemeleri kullanılarak hem daha yüksek sıcaklıklarda hava elde edebilmekte hem de enerjinin depolanması sağlanarak kollektörün toplam verimi artırılabilmektedir. Ateş tuğlaları gibi yüksek sıcaklıklara dayanıklı malzemeler, düşük ısı iletkenliğine ve yüksek ısı kapasitelerine sahiptirler. Bu çalışmada, depolama ve ısıl enerjinin tekrar kullanımında bir tür refrakter olan ateş tuğlasına ait parçalar kollektör içerisine yerleştirilerek, kollektörün verimi üzerine etkileri incelenmiştir. Hava ısıtıcılı güneş kollektörüne farklı miktarlarda ateş tuğlası parçaları uygulanmış, ışınım altında ve soğutma sırasında ısıl verimleri karşılaştırılmıştır., The hot air needed in applications such as drying agricultural products and space heating can be obtained with air-heated solar collectors. Air has a lower heat transfer coefficient than water. Using storage materials with increased surface area in air-heated solar collectors provides both obtaining higher temperatures output air and increased the total efficiency of the collector by storing energy. Refractory materials such as fire bricks have low thermal conductivity and high thermal capacities. In this study, the effects of refractory fire-brick fragments which are a kind of refractory in storage and reuse of thermal energy on collector efficiency were investigated. Fire brick fragments of different densities were applied into the solar air heater collector and their thermal efficiencies were compared during under radiation and cooling.

Artificial intelligence is seeking character in our mind, sometimes as a role of savior or sometimes taking over the world. But AI offers beneficial solutions, because its goal settings determining by the user yet. It is possible to come across artificial intelligence algorithms in different fields such as land and air vehicles, personal or commercial products, and no doubt about that it will continue to exist in many areas with its increasing effect in the future.In this study, firstly, the historical convolution and development of the concept of artificial intelligence and its technologies are examined. Then, examples of how artificial intelligence systems are used in natural and social science are given. Artificial intelligence systems find an important application field in the natural and human sciences. The scope of the study, used artificial intelligence systems in the natural and human sciences have been evaluated in detail and a projection has been work on the future of this technology. As with many technologies, artificial intelligence systems can be used for the service of humanity and nature, as well as for combat technologies, unlimited exploitation of nature and controlling people. In the conclusion of the study, predictions are artificial intelligence systems on how can be used to solve many problems (global warming, environmental pollution, air pollution, revenue and resources inequality etc.)., İnsan, tarih boyunca duyularıyla hissettiği her şeye hükmetmeye çalışmıştır. Düşüncenin işleyişi, zihnin gizemli duruşu ve metafizik varlıkların olasılığı gibi düşünceler ise, sanatçıları, bilim insanlarını ve hatta teologları etkilemiştir. İnsanın kendi gibi düşünen bir makine yaratma hayalinin temeli, karşılaşılan problemlerin çözümüne bir yardımcı arayışıdır. Yapay zekâ, bilim kurgu olarak kimi zaman kurtarıcı rolünde, kimi zaman ise dünyayı ele geçirme hedefiyle, zihinlerimizde bir karakter arayışına girmiştir. Fakat yapay zekâ, amacını henüz kullanan kişinin belirlediği bir araç olması sebebiyle, günümüzde fayda sağlayan çözümler sunmaktadır. Kara ve hava araçları, kişisel veya ticari ürünler gibi farklı alandaki teknolojilerde, yapay zekâ algoritmalarına rastlamak mümkündür ve şüphesiz ki gelecekte de artan etkisiyle birlikte birçok alanda varlığını sürdürmeye devam edecektir.Bu çalışma kapsamında öncelikle, yapay zekâ kavramının ve teknolojilerinin tarihsel kökeni ve gelişimi incelenmiştir. Daha sonra yapay zekâ sistemlerinin teknoloji ve sosyal alanlarda nasıl kullanıldığına dair örnekler verilmiştir. Yapay zekâ sistemleri doğa ve insan bilimlerinde önemli uygulama alanları bulmaktadır. Çalışma kapsamında doğa ve insan bilimleri alanında kullanılan yapay zekâ sistemleri ayrıntılı bir şekilde değerlendirilmiş ve bu teknolojinin geleceği konusunda bir projeksiyon oluşturulmaya çalışılmıştır. Birçok teknolojide olduğu gibi yapay zekâ sistemleri de insanlığın ve doğanın hizmetinde kullanılabileceği gibi savaş teknolojileri, doğanın sınırsızca sömürülmesi ve insanların kontrol edilmesi amaçlarıyla da kullanılabilir. Çalışmanın sonuç kısmında yapay zekâ sistemlerinin, bugün yaşadığımız birçok sorunun (küresel ısınma, çevre kirliliği, hava kirliliği, gelir ve kaynakların dağılımdaki adaletsizlik vd.) çözümünde nasıl kullanılabileceğine dair öngörülerde bulunulmuştur.

Bu tez çalışmasında, kompostlaştırma sistemleri için egzoz gazı içeriğini temelalarak yığın içerisindeki O 2 konsantrasyonunun tahminlenebilmesini sağlayanproses kontrol ve yönetim cihazı geliştirilmiştir. Denemelerin yürütülmesindegerekli kontrolü ve elde edilen verilerin dijital ortama kaydedilmesini sağlayanözgün bir otomasyon sistemi oluşturulmuş ve programlanmıştır. Denemesüresince ölçülen sıcaklık verileri tüm reaktörlerde başarılı bir kompostlaştırmaişleminin gerçekleştiğini göstermiştir. Bu durum, çalışmanın asıl amacı olan,yığın içi oksijen seviyelerini tahmin etmek için yapılacak modelleme çalışmalarıiçin kullanılacak verilerin, başarılı bir kompostlaştırma işlemini temsil edeceğinigöstermiştir.Yürütülen denemelerde Rutgers' havalandırma stratejisi uygulanarak sığırgübresi ve saman karışımının kompostlaştırılması sağlanmıştır. 60 litrelik aktifhacme sahip 5 farklı kompost reaktöründe %0, 20, 40, 60 ve 80 oranında egzozgazı geri dönüşümü yapılarak, her geri dönüşüm oranı için farklı bir modelgeliştirilmiştir. Bu modeller otomasyon sisteminin, farklı sistemler ve prosesleriçin kullanılabilirliğini arttıracak niteliktedir. Denemelerde 5 farklı reaktör içinbelirlenen reaktör içi O 2 seviyesini tahmin eden modellerin r 2 değerleri sırasıyla%98.94, %98.47, %95.75, %98.74, %97.09 olarak hesaplanmıştır.Sonuç olarak; reaktör denemelerinden elde edilen tahmin modelleri, PLC tabanlıotomasyon sisteminin yazılımına dahil edilerek, kompostlaştırma sistemleri içinfan kontrolleri, sıcaklık ölçümü ve sıcaklık kaydı yapan, aynı zamanda egzozgazının oksijen konsantrasyonunu analiz ederek yığın içerisindeki oksijenkonsantrasyonunu tahmin edebilen bir proses kontrol sistemi geliştirilmiştir.Anahtar Kelimeler: Kompost, Otomasyon, Yığın İçi O 2 konsantrasyonu, EgzozGazı, Modelleme. In this thesis study, a process control and management device has beendeveloped for the composting systems to estimate the O 2 concentration in thepile based on the exhaust gas content. A unique automation system has beencreated and programmed which enables the necessary control and recording ofthe data obtained in the execution of the experiments in the digitalenvironment. The temperature data measured during the experiment showedthat a successful composting process was performed in all reactors. This showsthat the data to be used for modeling studies to estimate the oxygen levels in thestack, which is the main goal of the study, will represent a successfulcomposting process.Rutgers' ventilation strategy was applied to make composting of cattle manureand straw mixture. A different model was developed for each recycling rate byexhaust gas at 0, 20, 40, 60 and 80% in 5 different compost reactors with anactive volume of 60 liters. These models will increase the usability of theautomation system for different systems and processes. The r 2 values of themodels estimating the intra-reactor O 2 level for 5 different reactors in theexperiments were calculated as 98.94%, 98.47%, 95.75%, 98.74%, 97.09%,respectively.As a result; the prediction models obtained from the reactor experiments havebeen incorporated into the software of the PLC based automation system todevelop a process control system that can estimate the oxygen concentration inthe pile by analyzing the oxygen concentration of the exhaust gas, fan control,temperature measurement and temperature recording for composting systems.Keywords: Compost, Automation, O 2 concentration in the Pile, Exhaust Gas,Modeling. 70

Özet 1 :Ülkemizde ve dünyada atık miktarlarıhızla artmakta ve toplanan atıklar çevre ve insan sağlığı için risk oluşturmaktadır.Toplanan atıklar içerisinde organik kısımlar büyük bir yer tutmaktadır.Kentsel, tarımsal ve sanayi kökenli organik atıkların değerlendirilmesindekompost ve biyogaz teknolojileri yaygın olarak kullanılmaktadır.Kompostlaştırma işleminde organik atık yığınları içerisinde oksijenli şartlarsağlanarak mikroorganizmaların bu atıkları ayrıştırması ve kompost üretmesisağlanır. Üretilen kompost toprak düzenleyici/bitki besleme materyali veyamantarcılık işletmelerinde mantar yetiştirme ortamı olarak kullanılmaktadır.Ülkemizde kentsel ve tarımsal atıklardan kompost üretimi yapılmaktadır. Ancakkompost üretim sistemleri konusunda pratiğe aktarılmış veya ticari ürünedönüştürülmüş teknoloji konusunda sıkıntılar yaşanmaktadır.Bu çalışmada ülkemizde kompostsektörünün ihtiyaç duyduğu kapalı sistemler için yerli teknoloji üretilmesihedeflenmiştir. Çalışma kapsamında işlem parametrelerini belirlemek amacıylareaktör denemeleri yapılmış ve bu denemelerin sonuçları doğrultusunda prototipsistemin tasarımı gerçekleştirilmiştir. Reaktör denemelerinde egzoz gazınınoksijen içeriğini kullanarak, kompost matrisindeki oksijen içeriğini tahmin eden 5farklı denklem geliştirilmiştir. Composting and biogastechnologies are commonly used for utilization of residential, agricultural andindustrial wastes. Microorganisms degrade wastes and let transform to compostwhile maintaining the oxygenated conditions in a pile of organic wastes. Thecompost produced is used for soil conditioner, soil nutrients, and substratesfor mushroom growers. Compost is produced from residential and agriculturalwastes in Turkey. However, the technologies developed for compost production inTurkey have their own problems in this sector.In this study, it is aimed to producedomestic technologies for closed systems for composting sector in Turkey and todevelop process control equipment for mobile tunnel composting system with airrecirculation. Within the study, experiments in reactors were carried out anddesign of prototype was realized through the direction of the results ofexperiments. In this study, the contributions of the Turks to the history of science are explained. Turks have established many civilizations. They were interested in different branches of science. The scope of the study summarizes the scientific areas and the contributions of the Turks to history. The life story and interests of the Turkish scientists who have an important place in the history of science are explained., Bu çalışmada Türklerin bilim tarihine sunduğu katkılar anlatılmıştır. Türkler birçok medeniyet kurmuştur. Kurdukları medeniyetler içerisinde farklı bilim dallarına ilgi duymuşlardır. Çalışma kapsamında, Türklerin tarih içerisinde ildi duyduğu bilim alanları ve bula alanlara sunduğu katkılar özetlenmiştir. Bilim tarihinde önemli yere sahip Türk bilim insanlarının yaşamları ve ilgi alanları anlatılmıştır.

Amaç: Alt konka hipertrofilerinde radyofrekans cerrahisinin uzun dönem sonuçlarını araştırmak. Yöntem ve Gereçler: Alt konka hipertrofilerinde radyofrekans ile tedavi edilen hastaların kayıtları retrospektif olarak incelenmiş, preoperatif ve işlem sonrası erken dönemde (2 ay) doldurulmuş olan 10 cm’lik vizüel analog skala (VAS) skorları kaydedilmiştir. Çalışmaya alınan hastalara aynı VAS uzun dönemde uygulanmış, tekrarlı ölçümler için varyans analizi yapılarak preoperatif, kısa dönem ve uzun dönem VAS skorları karşılaştırılmıştır. Bulgular: Çalışmaya 89 hasta alınmıştır. Ortalama takip süresi 30,5 ± 5,8 ay olarak bulunmuştur. Preoperatif VAS skoru ortalama 8,6 ± 1 iken kısa dönem ve uzun dönem VAS skorları sırası ile 4,2 ± 2,3 ve 4,7 ± 2,7 olarak bulunmuştur. Kısa ve uzun dönem VAS sonuçları arasında anlamlı fark bulunamamıştır (p=0,29). Sonuç: Alt konka hipertrofilerinin tedavisinde uzun dönem sonuçları göz önüne alındığında etkili bir tedavi yöntemi olarak değerlendirilmiştir. Objective: To report long-term efficacy of radiofrequency surgery in treatment of inferior turbinate hypertophy. Material and Methods: Reports of patients, who were treated with radiofrequency for inferior turbinate hypertrophy, were retrospectively reviewed. Preoperative and short-term (2 months) postoperative 10 cm visual analog scales (VAS) for evaluation of nasal obstruction were analyzed. All patients that were included into the study were given the same VAS. Preoperative, short-term and long-term VAS scores were compared by performing repeated measures variance analysis. Results: Eighty nine patiens were included into the study. The mean follow-up was 30.5 ± 5.8 months. The mean preoperative VAS score was 8.6 ±1 while the short-term and long-term scores were 4.2 ± 2.3 and 4.7 ± 2.7 respectively. There was no statistically significant difference between short-term and long-term VAS scores (p=0.29). Conclusion: Radiofrequency is effective in treating inferior turbinate hypertrophy with sustained benefit at long-term follow-up.

Amaç: Bu çalışma, tonsillektomi yapılan hastalarda baktereminin süresini ve spektrumunu araştırmak ve baktereminin, akut faz reaktanları, tonsil kültürleri, ateş varlığı ile ilişkisini incelemek üzere tasarlanmıştır. Çalışma Planı: Prospektif klinik çalışmadır. Hastalar ve Yöntemler: Tonsillektomi yapılan (15 erkek, 16 kadın) toplam 31 hasta çalışmaya dahil edilmiştir. Tonsillerin yüzeyinden ve derin dokularından sürüntü yapılarak kültürler alınmıştır. Tüm hastalardan preoperatif dönemde ve ameliyattan sonraki 1., 3. ve 5. saatlerde venöz kan örnekleri, kan kültürü için alınmıştır. Preoperatif ve postoperatif eritrosit sedimentasyon hızı ve C-reaktif protein seviyeleri karşılaştırılmıştır. Kültür sonuçları ve bakteremi varlığı araştırılmıştır. Bulgular: Tonsillektomi sonrası bakteremi sadece üç hastada tespit edilmiştir. On yedi (%54,8) hastanın tonsil derin doku kültürlerinde üreme olduğu görülmüştür. Bakteremi gelişen üç hastada tonsil yüzey ve derin doku kültürlerinden izole edilen mikroorganizmalar ve bakteremi arasında her hangi bir ilişki bulunamamıştır. Akut faz reaktanlarının preoperatif ve postoperatif seviyeleri arasındaki fark çalışma grubumuzda anlamlı değildir. Sonuç: Baktereminin geçici olması ve düşük oranlarda tespit edilmesi; tonsillektomi sonrası rutin antibiyotik kullanımının gereksiz olduğu ve sadece yüksek riskli hastalarda kullanılabileceği sonuçlarını çıkarmıştır. Objectives: This study was designed to investigate the duration and spectrum of bacteremia and its relation with acute phase reactants and fever in patients who underwent tonsillectomy. Study Design: A prospective clinical trial Patients and Methods: Thirty one patients (15 male, 16 female) who underwent tonsillectomy were included. Swab cultures from surface and core of tonsils were performed. All patients underwent venous blood sampling preoperatively and 1, 3 , 5 hours after the operation for blood cultures. Preoperative and postoperative erythrocyte sedimentation rates and C Reactive Protein levels were compared. Results of bacteriological research and bacteremia were investigated. Results: Post-tonsillectomy bacteremia was determined only in three patients. In 17 (54.8%) patients various microorganisms were isolated from cultures of tonsil core. There was no relation between the microorganisms which were isolated from tonsil core or tonsil surface and the bacteremia in three patients. The preoperative and postoperative difference between the levels of acute phase reactants was not found to be significant in our study group. Conclusion: The transient pattern and low ratios of bacteremia after tonsillectomy indicates that routine antibiotic use is unnecessary and may be used only in high risk patients.

Study Design: An animal experiment using a tracheal stent model. Methods: Twenty Wistar albino rats were divided into two groups; a heparin group (n=10) that received 210 U/kg/d heparin sodium, and a saline group (n=10) that received 0.1 mL of 9% NaCl. Stents of 1 cm in length made of 8F feeding tube were placed into the tracheas of rats in both groups and stayed in place for 10 days. Ten days after removal of the stents, the rats were sacrificed and the tracheas were harvested. Histological evaluations of the tracheas were performed with respect to inflammatory parameters. Results: We observed significantly milder inflammation in the heparin group compared to the saline group in terms of inflammatory cell count, fibroblastic proliferation, edema, and vascularity at the site where the tracheal incision was made (P.05). Conclusions: We could demonstrate that heparin does inhibit fibroblast proliferation, inflammatory cell count, edema, and angiogenesis in this animal model. We believe that future studies can elucidate on laryngotracheal wound healing as well as their molecular mechanisms.

WOS:000307083900008 Objective: In this study we aimed to investigate the psychological status of tinnitus patients as well as the sleep quality and their relation to handicap caused by tinnitus using various scales. Additionally, serum melatonin levels and the effectiveness of melatonin treatment were studied. Study Design: Prospective, double blind, randomized controlled trial Materials and Methods: Patients were divided randomly into two groups as study (melatonin, n=13) and placebo (control, n=11) groups. Tinnitus Handicap Inventory (THI), Symptom Check List (SCL), Hospital Anxiety and Depression Scale (HAD), Beck Depression Inventory (BDI), Pittsburg Sleep Qualty Index (PSQI) were applied. After filling the scales and giving the venous blood sample for melatonin measurements the patients were instructed to take one tablet (placebo or 3 mg melatonin) before sleep every night. After 8 weeks, second order scales were filled and melatonin measurements were repeated. Results: Serum melatonin levels were between 1-260 pg/ml; the mean was 38,7 pg/ml. The correlation of melatonin levels with THI and tinnitus duration was not significant. THI was found to correlate with different measures of the PSQI, HAD, and BDI in both groups. Statistical analysis failed to show any significant difference within and between groups in respect of anxiety, depressive symptoms and sleep as well as melatonin and handicap levels. When the groups were assessed according to the THI severity (mild/moderate to severe;THI2); in the control group there was significant differences in PSQI(1) and PSQI(1) (p=0.0008, p=0.18), NAD(1), HAD(2) (0.002, 0.03), HAD Depression(1-2) (0.0, 0.006) BDI (p=0.007) PSQI(2 sleep) (disturbance) (P=0.018) parameters. However, in the melatonin group it was found that there were significant differences in SCL2 (sleep) (latency), PSQI(2) (sleep) (duration) and total PSQI(2) parameters. (p=0.022, 0.027, 0.006 respectively) Conclusion: Patients with higher handicap may benefit melatonin in respect of sleep latency and duration as well as sleep quality comparing with the patients taking placebo. Moreover, melatonin efficiency may be related to its antidepressive effect.

Tarımın ülke ekonomisinde önemli bir yeri olan Türkiye gibi devletlerdehasat öncesinde tahıl ekim alanlarının ve rekolte tahminlerinin hızlı ve ekonomik biryöntem olan uzaktan algılama ile belirlenerek tarım politikasında öncelikli adımlarınatılması hiç süphesiz ki faydalı olacaktır.Yapılan bu çalısmada Ankara il sınırları içinde yer alan Sereflikoçhisarilçesine ait 12.05.2005 tarihli SPOT 2 ve 16.05.2005 tarihli Landsat 5 TM uydugörüntüleri kullanılarak, 2005 yılına ait klasik yöntemlerle elde edilen ve lçe TarımMüdürlügü'nden alınan ekim alanları ile uydu görüntülerinin sınıflandırılmasısonucu elde edilen ekim alanları karsılastırılmıs ve rekolte tahmini yapılmıstır.Arpa ve bugday Sereflikoçhisar için ana tahıl ürünleridir. Bu tarımsalürünlerin uydu görüntüleriyle ayrımı spektral yakınlıklarından dolayı zor birproblemdir ve iyi seçilmis veri setlerinin kullanılması gerekir. Egitim setindeki yanlısörneklerin sınıflandırmayı önemli oranda etkiledigi tespit edilmistir. En çokbenzerlik (maximum likelihood) sınıflandırıcısı yardımıyla çalısma alanınınsınıflandırması %93 genel dogrulukla gerçeklestirilmistir. NDVI, OSAVI ve SAVIindeksleri kullanılarak ürün rekolte tahminleri yapılmıstır. Üç yöntemle de birbirineyakın degerler elde edilmistir. En iyi rekolte tahminleri NDVI ve OSAVI yöntemi ileelde edilmistir. Arpa ve bugday rekolteleri NDVI ile %99, OSAVI ile %99, SAVI ilearpa %94, bugday %98 dogrulukla hesaplanmıstır. The determination of the extent of agricultural crops and the crop yieldestimation before harvesting are very important issues for state planing, especially inTurkey. The use of remote sensing technology for this purpose provides a rapid andcost-effective solution with superior advantages.In this study, SPOT 2 dated on 12th May 2005 and Landsat 5 dated on 16thMay 2005 satellite images of Sereflikoçhisar in the city of Ankara were used. Withthe use of remotely sensed data, vegetation indices was evaluated by comparing thecrop yield estimates obtained from the classification images of the study area.Barley and wheat are the main agricultural crops in Sereflikoçhisar. Due tothe spectral proximity, discrimination or delineation of these crops by satelliteimages is a difficult problem and requires well prepared and representative data sets.It is obsevered that atypical or mixed pixel samples have considerable negative effecton classification results.With the use of maximum likelihood classifier, classification of testingsamples resulted in 93% general accuracy. Yield estimation has been carried outusing NDVI, OSAVI and SAVI indices. With the three methods yield estimationswere determined for each pixel. Whilst accuracies for barley and wheat were equal99% for NDVI and OSAVI, accuracy were 94% for barley and 98% for wheat inSAVI estimation. The best results were produced using NDVI and OSAVI methodsin that barley and wheat yields have been calculated with 99% accuracy assesment. 96

IP yönlendirme kararının verilebilmesinde temel girdi, IPadresinden hareketle ilgili yönlendirme birimini bulan IP lookup (karşılaştırma,arama-bulma) eylemidir. Bu eylemin IP yönlendirme işlemini darboğazasokmayacak bir hızda yapılabilmesi gerekir.Bu çalışmada IPv6' ya geçiş sürecinde aynı omurga üzerinden hem IPv6 hem deIPv4 paketlerini yönlendirmek ilave olarak bunu kabul edilebilir maliyetlere sahipyazılım ve donanım tasarımları ile gerçekleştirmek gerekeceği fikrinden hareketle,IPv4 ve IPv6 yönlendirici tasarımında çekirdek olabilecek bir arama-bulmaalgoritması incelenmiş ve çift IP destekleyen bir yönlendirici yazılımıkurgulanmıştır. Kurgulanan yazılımın çeşitli platformlarda çalışmasından elde edilenperformans değerleri analiz edilmiştir. The main input to decide the IProuting is IP lookup process which uses IP address to find the related routing unit.This process have to be done with enough speed which will not create a bottleneckfor IP routing job.In this study, a dual routing model has been proposed based on the clue of necessityof routing IPv4 and IPv6 packets together with reasonable cost software andhardware designs, a lookup algorithm which can be core of such design wasinspected and a dual IP supporting routing software was simulated. The performancevalues from simulation running on different platforms were analyzed. As a result, itis concluded that the high performance criteria which is necessary for IP routing andother possible IP based data lookup processes in the future can be achieved. 51

IV ÖZET Süslü AE, Obstrüktif uyku apneli hastaların değerlendirilmesinde kullanılabilecek parametreler. Hacettepe Üniversitesi Tıp Fakültesi KBB Anabilim Dalı, Uzmanlık Tezi, Ankara, 2004. Obstrüktif uyku apnesi sendromunun (OUAS) teşhisinde altın standart olan polisomnografinin kullanımı, pahalı olması ve özel teknik ekipman desteği nedeniyle yaygınlaşamamıştır. Bu çalışmada vücut kitle indeksi ve boyun çevresi gibi antropometrik ölçümler, bioelektrik impedans analizi (BİA) ile ölçülen vücut yağ kütlesi, bilgisayarlı tomografi (BT) ile vücut yağ dağılımı ve sefalometrik analiz yapılmış ve elde edilen parametrelerle, OUAS derecesi ve apne- hipopne indeksi (AHİ) arasındaki ilişkiler değerlendirilmiştir. Hacimsel vücut yağ dokusu, vücut yağ kütlesi, boyun çevresi, kesitsel karın viseral yağ dokusu ve yaş ile AHİ arasında anlamlı ilişki olduğu bulunmuştur. AHl'yi tahmin etmek için hangi parametrelerin daha önemli rolü olduğunu göstermek amacıyla adımsal regresyon modeli uygulanmış, sonuç olarak BT ile ölçülen hacimsel vücut yağ dokusu en kuvvetli, BİA ile ölçülen vücut yağ kütlesi ise ikinci en kuvvetli faktör olarak tespit edilmiştir. AHİ'yi tahmin etmek için vücut yağ kütlesinin kullanıldığı, istatistiksel olarak anlam ifade eden bir denklemde bu analiz sonucunda geliştirilmiştir. UPPP ameliyatından fayda görebilecek adayların tespitinde kullanılan iki endoskopik muayene yöntemi olan Müller manevrası ve uykuda nazofaringoskopi birbirleriyle karşılaştın I m ıştır. Sonuçların uyumsuz olduğu görülmüş, Müller manevrasına oranla özellikle yumuşak damak seviyesinde, uykuda nazofaringoskopi ile daha fazla obstrüksiyon izlendiği dikkat çekmiştir. Bu bulgular, Müller manevrası ile oluşturulan obstrüksiyonun gerçeği yansıtmadığını düşündürmüştür. Çalışmamızda bir grup hastadan, palatoglossus biyopsileri alınmıştır. Bazı OUAS hastalarında, myopati bulgularına rastlanmış olmasına rağmen, biyopsi sonucu tamamen normal olan OUAS hastaları da tespit edilmiştir. Bu sonuçlar, OUAS'ın multifaktöryel etiyolojiye sahip olduğuna işaret etmektedir. Although polysomnography is the gold standard for the diagnosis of obstructive sleep apnea (OSA), there are limitations because of its expense and dependence to complex technical support. In this study, anthropometric measures including body mass index and neck circumference, body fat mass by bioelectric impedans analysis (BIA), body fat distribution by computed tomography (CT) and cephalometric analysis were performed to determine their relationships with the degree of OSA and apnea-hypopnea index (AHI). Volumetric body fat tissue, body fat mass, neck circumference, sectional abdominal visceral fat tissue, and age were found to be significantly correlated with AHI. To identify parameters that would best predict the AHI a regression model was calculated. The only significant predictors were volumetric body fat tissue by CT and body fat mass by BIA. Also a statistically significant equation was created with the body fat mass to predict the AHI. The two endoscopic examination method to select the good UPPP responders - Müller maneuver and sleep nasendoscopy - were compared in this study. Sleep nasendoscopy revealed greater degree of occlusion especially in the level of soft palate when compared to Müller maneuver. It was suggested that the degree of obstruction might be underestimated by Müller maneuver and this method did not properly reflect the real obstruction during normal sleep. Palatoglossus biopsies were performed in a group of patients. Although some OSA patients had the signs of myopathy in their biopsies, there were OSA patients with a completely normal muscle biopsies. According to the results it was indicated that not single but multifactorial etiologies play role in the development of OSA.
