Background: Vaccination against severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) is the most desired solution to combat COVID-19. We examined the willingness to accept the vaccine and reasons for vaccine hesitancy, and identified some factors associated with the vaccine hesitancy among the socio-economically disadvantaged urban population from Delhi, India. Methods: We conducted a cross-sectional survey of a randomly selected sample of 1539 households from 31 urban clusters. Data on socio-demographics, health beliefs, and willingness to accept the SARS-CoV-2 vaccine were collected through a face-to-face interviewer-administered, pre-tested questionnaire from an adult member. Vaccine acceptance/hesitancy was analysed by various socio-demographic and health belief variables. Multinomial regressions were carried out to identify the factors associated with the vaccine hesitancy. Results: Overall, 64.9% (95% CI: 62.5 to 67.3) of the respondents would accept the vaccine, 17.4% (95% CI: 15.6 to 19.4) were undecided, and 17.7% (95% CI: 15.8 to 19.7) would not accept the vaccine. The reasons for not accepting the vaccine were: belief that they had immunity (12.9%), the corona was a hoax (11.8%), the vaccine was not necessary (7.4%), and did not want to disturb the natural bodily systems by the vaccine (5.6%). The undecided group mainly would like to wait and see (37.7%), decide when the vaccine become available (11.6%), will take if everyone in their community takes (10.4%). Multinomial logistic regression identified older age, low perceived susceptibility of contracting COVID-19, low perceived severity of COVID-19, low self-efficacy to protect against COVID-19, and unawareness and non-use of Arogyasetu App as significant predictors of vaccine hesitancy. Conclusions: Two-thirds of Delhi’s low-income groups would accept the vaccine against SARS-CoV-2. Vaccine hesitancy was associated with older age, low perceived susceptibility, low perceived severity, and low self-efficacy to protect themselves from COVID-19. Hence, efforts are needed to address these issues and vaccine concerns to increase the vaccine uptake.

accreditation ,customer satisfaction ,laboratory ,national accreditation board for testing and calibration laboratories ,Internal medicine ,RC31-1245 - Abstract
Laboratory accreditation is a process by which an authoritative body gives formal recognition of technical competence for specific tests or measurements usually following international standards such as ISO/IEC 15189/17025. The process checks on inaccuracy or imprecision that may crop up during the preanalytical, analytical, or postanalytical phases, which could have an impact on the patient's reports and thereby the treatment aspects. The process itself helps the personnel of the laboratory to be alert, be quality conscious, and provide the best to the customers, namely patients and health-care providers, thereby helping the health-care providers to give the best of their services to the patients. This enhances the quality of the health-care services on the whole due to quality management system being in place in laboratories. The accreditation agency which takes interest in the laboratories of India and ensures quality in them is National Accreditation Board for Testing and Calibration Laboratories. The developed countries already have the system in place, and it is mandatory for every lab to be accredited before releasing patient reports. We are not far behind in this process, and shortly it may be considered to be mandatory for all labs to obtain the accreditation to run the tests. However, there are several challenges one needs to address in the process of accreditation, with the most important being the finances, team work, and planning.

Background: Disparities in healthcare access to internal migrants exist, and the gaps may widen further if appropriate steps are not taken. Innovative approaches are needed to better align the healthcare services with the migrants’ needs. Aim: The aim was to develop and test a supportive strategy of healthcare, which would achieve the desired level of access and delivery of maternal healthcare services to internal migrants living in nine Indian cities. Methods: This intervention with the quasi-experimental design was conducted with pre- vs post-intervention comparisons within the interventional groups and with the control group. The intervention was implemented with an inclusive partnership approach. Advocacy and community mobilization were the main intervention components. Findings: An increased proportion of women sought antenatal care during the intervention. More women initiated seeking antenatal care in the first trimester. Due to intervention, health workers’ prenatal (41.7% in the post- against 14.7% in the pre-interventional phase) and postnatal home visits increased (11.6% to 34.7%) considerably. Conclusions: Interventions with inclusive partnership would improve healthcare access to vulnerable communities such as migrants. Hence, efforts to strengthen the government healthcare system through novel strategies are crucial to provide better healthcare to migrants.

LEPTOSPIROSIS , *SEROPREVALENCE , *ZOONOSES , *AGGLUTINATION tests , *TERTIARY care - Abstract
Leptospirosis is an anthropozoonotic disease, which remains under diagnosed in most of the regions of our country due to the lack of awareness, atypical presentation and lack of proper diagnostic facility and set up. Diagnosis usually depends on Microscopy, Culture and Serological tests like ELISA, MAT (Microscopic agglutination test). MAT is the gold standard diagnostic test but it requires live Leptospires and it can only be done in a reference laboratory. Hence, ELISA is the most preferred cost-effective serological method and is genus specific with both sensitivity and specificity of 95%. Aim of the present study is to estimate the seroprevalence of Leptospirosis among suspected Acute Febrile illness (AFI) cases presented to Government General Hospital Kurnool. The study was conducted from August 2023 to March 2024 on 260 AFI cases. Detection of IgM antibody was done by using Recombilisa Leptospira IgM ELISA kit. Out of 260 clinically suspected cases of Leptospirosis, 30 (11.53%) were positive. Majority of cases belonged to 21-40 years age group. Seroprevalence was found to be higher in Males (14.1%) as compared to Females (8.5%). A high seroprevalence of Leptospirosis was noted in AFI cases, so a programmatic approach for prevention, control and management of such emerging zoonotic diseases through NOHP-PCZ (National One Health Program for Prevention and Control of Zoonoses) will be a very useful step towards achieving optimal health. Renewable energy sources ,TJ807-830 - Abstract
When simulating a wind turbine, the lowest eigenmodes of the rotor blades are usually used to describe their elastic deformation in the frame of a multi-body system. In this paper, a finite element beam model for the rotor blades is proposed which is based on the transfer matrix method. Both static and kinetic field matrices for the 3-D Timoshenko beam element are derived by the numerical integration of the differential equations of motion using a Runge–Kutta fourth-order procedure. In the general case, the beam reference axis is at an arbitrary location in the cross section. The inertia term in the motion differential equation is expressed using appropriate shape functions for the Timoshenko beam. The kinetic field matrix is built by numerical integration applied on the approximated inertia term. The beam element stiffness and mass matrices are calculated by simple matrix operations from both field matrices. The system stiffness and mass matrices of the rotor blade model are assembled in the usual finite element manner in a global coordinate system accounting for the structural twist angle and possible pre-bending. The program developed for the above-mentioned calculations and the final solution of the eigenvalue problem is accomplished using MuPAD, a symbolic math toolbox in MATLAB®. The natural frequencies calculated using generic rotor blade data are compared with the results proposed from the FAST and ADAMS software.

Zinc oxide nanoparticles (ZnO-NPs) have in recent times shown effective adsorption capability for the confiscation of colour contaminants from aqueous environments (aquatic ecosystems or water bodies) due to the fact that ZnO contains more functional groups. Direct blue 106 (DB106) dye was selected for this present study as a model composite due to its wide range of uses in textiles (cotton and wools), woods, and paper industries, as well as their therapeutic applications, along with its potential for impairments. This study therefore focuses on the use of DB106 dye as a model composite due to its wide range of uses in textiles (cotton and wools), woods, and paper industries, as well as their therapeutic applications and their potential for impairments. Furthermore, the surface functionalization, shape, and composite pore size were revealed by TEM, FTIR, UV, and BET techniques. The current study uses green synthesis method to prepare ZnO-NPs as an adsorbent for the DB106 dye molecules adsorption under various conditions using the batch adsorption process. The adsorption of DB106 dye to the ZnO-NPs biosorbent was detected to be pH-dependent, with optimal adsorption of DB106 (anionic) dye particles observed at pH 7. DB106 dye adsorption to the synthesized ZnO-NPs adsorbent was distinct by means of the linearized Langmuir (LNR) and pseudo-second-order (SO) models, with an estimated maximum adsorption capacity (Qm) of 370.37 mg/g. Solanum ferox Linn is one of the local agricultural products. Its local name is “terong asam”, the plant's shape is like eggplant but the fruit is round, orange with ripe and sour taste. The problem is that the fruit is easily damaged if left in its fresh. One of the innovations to increase the shelf life is to process it into processed dry sweetmeats. However, the amount of sugar that produces the best physicochemical characteristics of “terong asam,” dry sweetmeats is currently unknown. This study aims to obtain the concentration of sugar that produces the best physicochemical and sensory characteristics of “terong asam,” dry sweetmeats. The research design used was a randomized block design with one treatment, namely concentration of sugar, which consisted of 4 treatments and 6 replications. Analysis of physicochemical data using ANOVA (Analysis of Variance) and Honest Significant Difference (HSD) Test with a level of 5% significantly. Sensory data were analyzed by Kruskal Wallis test. Determination of the best treatment using the effectiveness index test. The results showed that the production “terong asam,” dry sweetmeats is recommended by using a 50% sugar concentration which the best sensory physicochemical characteristics of the sweets, namely water content (13.28%), pH (5.08), yield (26.49%), vitamin C (8.7mg/100g) with sensory characteristics of colour (3.17), taste (2.67), aroma (3.30), texture (3.20).

The study aims to assess the counselling services provided to prevent mother to child transmission of HIV (PMTCT) under the Indian programme of prevention of parent-to-child transmission of HIV (PPTCT). Five hospitals in Delhi providing PMTCT services were randomly selected. A total of 201 post-test counselled women were interviewed using a modified version of the UNICEF-PPTCT evaluation tool. Knowledge about HIV transmission from mother-to-child was low. Post-test counselling mainly helped in increasing the knowledge of HIV transmission; yet 20%–30% of the clients missed this opportunity. Discussion on window period, other sexually transmitted diseases and danger signs of pregnancy were grossly neglected. The PMTCT services during the antenatal period are feasible and agreeable to be provided; however, certain aspects, like lack of privacy, confidentiality of HIV status of the client, counsellor’s ‘hurried’ attitude, communication skills and discriminant behaviour towards HIV-positive clients, and disinterest of clients in the counselling, remain as gaps. These issues may be addressed through refresher training to counsellors with an emphasis on social and behaviour change communication strategies. Addressing attitudinal aspects of the counsellors towards HIV positives is crucial to improve the quality of the services to prevent mother-to-child transmission of HIV.

General Medicine - Abstract
The only method for gauging the effectiveness of a teacher's lessons and the learning results of students is examination. Creating the questions and then randomly assigning them to the test are processes in the manual production of test papers. It takes a lot of time and effort to manually create test papers that match the standards of a good test paper. Mainly generating the test papers to evaluate the student technical skills is more time taking and the recruiters always want to know the students in better way by conducting the tests in the technical concepts. Our system helps in generating the questions automatically from the database which consists of the questions of the various technical concepts. To store the data and access the information from the database we are using MYSQL. To generate the test paper with the questions based on the skills known by the candidates we are using the classification algorithm like Naive Bayes Classifier, Random Forest Classifier and the Decision Tree. Naive Bayes, Random forest and Decision tree ,all are supervised algorithms. Giving the questions depending upon the candidate skills in the test paper will effectively evaluate the knowledge of the candidate in the skills that they know rather than testing their knowledge in completely unknown domain, the recruiters will also get the chance to know the candidate capabilities accurately and they can use the skills of the candidates in the better way and they can allocate the work to the candidates based on their performance in the particular skill. To know the performance of the candidate in the test , we have provided the graphical user interface (GUI) which was designed using the HTML and CSS concepts, where the questions are generated depending upon the classified results of the candidates.

Mutations in katG gene are often associated with isoniazid (INH) resistance in Mycobacterium tuberculosis strain. This research was perfomed to identify the katG mutation in clinical isolate (L8) that is resistant to INH at 1 μg/ml. In addition to characterize the catalase-peroxidase of KatG L8 and perform the ab initio structural study of the protein to get a more complete understanding in drug activation and the resistance mechanism. The katG gene was cloned and expressed in Escherichia coli, then followed by characterization of catalase-peroxidase of KatG. The structure modelling was performed to know a basis of alterations in enzyme activity. A substitution of A713G that correspond to Asn238Ser replacement was found in the L8 katG. The Asn238Ser modification leads to a decline in the activity of catalase-peroxidase and INH oxidation of the L8 KatG protein. The catalytic efficiency (Kcat/KM) of mutant KatGAsn238Ser respectively decreases to 41 and 52% for catalase and peroxidase. The mutant KatGAsn238Ser also shows a decrease of 62% in INH oxidation if compared to a wild type KatG (KatGwt). The mutant Asn238Ser might cause instability in the substrate binding site of KatG, because of removal of a salt bridge connecting the amine group of Asn238 to the carboxyl group of Glu233, which presents in KatGwt. The lost of the salt bridge in the substrate binding site in mutant KatGAsn238Ser created changes unfavorable for enzyme activities, which in turn emerge as INH resistance in the L8 isolate of M. tuberculosis.

This study reports the awareness, access, and utilization of health insurance by the urban poor in Delhi, India. The study included 2998 households from 85 urban clusters spread across Delhi. The data were collected through a pretested, intervieweradministered questionnaire. Logistic regression was performed for determinants of health insurance possession. Only 19% knew about health insurance; 18% had health insurance (Employees State Insurance Scheme – ESIS – 8%; Central Government Health Scheme – CGHS – 1.4%; Rashtriya Swasthya Bima Yojana (RSBY) – 9.4% of the eligible households). In case of health needs, 95% of CGHS, 71% ESIS beneficiaries, and 9.5% of RSBY beneficiaries utilized the schemes for episodic and chronic illnesses. For hospitalization needs, 54% of RSBY, 86% of ESIS, 100% CGHS utilized respective services. Residential area, migration period, possession of ration card, household size, and occupation of the head of the household were significantly associated with possession of RSBY. RSBY played a limited role in meeting the healthcare needs of the people, thus may not be capable of contributing significantly in the efforts of achieving equity in healthcare for the poor. Relatively, ESIS and CGHS served the healthcare needs of the beneficiaries better. Expansion of ESIS to the informal workers may be considered.

Introduction: CoNS are gaining importance due to increase in resistance rates to betalactam antibiotics and multi drug resistance. Although specific virulence factors are not as clearly established, it seems clear that factors such as bacterial polysaccharide components, and ability to form biofilm are involved in attachment and/or persistence of bacteria on foreign materials. Biofilms usually result in persistent infections that cannot be easily resolved with standard antibiotic treatments; therefore, the biofilm formation ability and the resistance to antimicrobial therapy can be intimately related. Materials and Methods: A prospective cross-sectional study was done on purely isolated CoNS from various clinical samples from both out patients and inpatients. All the test strains were subjected to antimicrobial susceptibility testing. The ability to produce biofilm was detected by tube adherence method. Results: Among 193 CoNS isolates 156 were from inpatients and 37 were from out patients. Methicillin resistant was seen in 80.31%. Of the total, 40.41% showed moderate biofilm formation by tube adherence method. 23.32% of isolates did not form biofilm. All the isolates from blood samples showed moderate (20/26) and strong (6/26) biofilm formation. Among non biofilm producers 66.67% were MS CoNS isolates and 33.33% were MRCoNS. 94.59% of biofilm producers were MRCoNS and 5.41% were MSCoNS. Production of biofilm was relatively more (1.16) among CoNS isolates of IPD than OPD. Conclusions: As Coagulase negative Staphylocooci are exhibiting multi drug resistance and are able to form biofilm, these organisms causing a major challenge for the physicians. Hence, such problems can be prevented by detection of biofilm producers and appropriate antibiotic doses modification. The issue of antibiotic resistance among CoNS needs to be addressed through a more rational use of existing antibiotics as well as the development of new antimicrobial agents. Keywords: CoNS, Antim

Macro studies reveal that internal migration is of considerable extent in India, however disaggregated studies on specific migrant groups are lacking from India. The present study is based on a scientifically drawn sample of 10,428 migrant households living in Delhi and the objective is to present socio-economic profile of the socioeconomically disadvantaged migrants living in Delhi. Migration is predominantly male in character and is dominated by younger ages. Migrants form a considerable proportion of the city’s population and are indispensable for the city’s growth. The other characteristics of migrants reveal that migrants are marginalized in the city and have denied access to basic amenities and compromise to live in such conditions in order to earn a livelihood and better incomes. Since migrants are an indispensable population in the cities, they should be recognized as a specific vulnerable group for the provision of basic services and there is a need for policy formulation ensuring rights of the migrants.

Background and Objectives: Hypertension control remains a major global challenge. Behavioral approaches recommended for blood pressure (BP) reduction include: stress reduction, increased exercise and healthy dietary habits. Some study findings suggest that yoga exerts beneficial cardiovascular effects including blood pressure alterations. This study was done to assess the effect of trataka, a yogic cleansing technique, on blood pressure and heart rate variables in patients with hypertension and thereby substantiate the clinical understanding of its effect on blood pressure. Material and methods: A total of 60 subjects with primary hypertension (age 25–45 years) were randomized to the study. The yoga group practiced trataka for 30 minutes with guided instructions and the control group was asked to sit calmly for 30 minutes. Blood pressure and heart rate were recorded before and after the practice. Results: The individuals who practiced trataka showed significant reduction in systolic blood pressure (SBP) (p < 0.05), mean heart rate (HR) (p < 0.05), and mean RR (p < 0.05) after the procedure. There were no significant changes observed in the control group as compared to the yoga group. Conclusion: The practice of trataka leads to significant reduction in blood pressure and heart rate in patients with primary hypertensive. Though trataka is considered as a cleansing technique, it induces calmness similar to a mental state during meditation. Evidently, both techniques bear the potential to reduce stress and, henceforth, blood pressure and heart rate.

Emphasis must be given to local food development, especially in major tourist destinations, including Geopark Bromo Tengger Semeru (BTS). Consumers perceive local food in tourist areas as more enticing once it incorporates functional, safe, and halal aspects. Halal food affirms the materials, equipment, and production. One of the central aspects that will be covered is halal traceability. Pokak is a local drink readily available in the Geopark BTS area that is often sold in syrup, the production is dominated by women in local communities. This study aims to identify the Halal Control Point (HCP) traceability model of pokak to support the development of local food at Geoparks BTS. The results were obtained from the market’s four most renowned pokak syrup brands. A descriptive-analytic method was used to collect data from the literature on standard pokak syrup recipes. The method is based on HAS 23301 about HAS Criteria for processing industries. The HCP was identified on granulated, palm sugar, sodium benzoate, and water. The HCP (haram) risk of granulated sugar is in the refining (ion-exchange resins) and bleaching processes (activated carbon). Palm sugar dan sodium benzoate should comply with the material and process document’s halal requirements. The basic processes of pokak syrup use water, so the water separation process includes the HCP. The HCP ingredients and equipment do not meet halal standards and should be substituted with halal material and equipment alternatives. Implementing halal traceability ensures that local food at Geopark BTS complies with the halal standard.

Renewable Energy, Sustainability and the Environment - Abstract
The difference between physical activations (by sonications) and chemical activations (by ammonia) on sawdust biochar has been investigated in this study by comparing the removal of Cu(II) ions from an aqueous medium by adsorption on sawdust biochar (SD), sonicated sawdust biochar (SSD), and ammonia-modified sawdust biochar (SDA) with stirring at room temperature, pH value of 5.5–6.0, and 200 rpm. The biochar was prepared by the dehydrations of wood sawdust by reflux with sulfuric acid, and the biochar formed has been activated physically by sonications and chemically by ammonia solutions and then characterized by the Fourier transform infrared (FTIR); Brunauer, Emmett, and Teller (BET); scanning electron microscope (SEM); thermal gravimetric analysis (TGA); and energy-dispersive spectroscopy (EDX) analyses. The removal of Cu(II) ions involves 100 mL of sample volume and initial Cu(II) ion concentrations (conc) 50, 75, 100, 125, 150, 175, and 200 mg L−1and the biochar doses of 100, 150, 200, 250, and 300 mg. The maximum removal percentage of Cu(II) ions was 95.56, 96.67, and 98.33% for SD, SSD, and SDA biochars, respectively, for 50 mg L−1Cu(II) ion initial conc and 1.0 g L−1adsorbent dose. The correlation coefficient (R2) was used to confirm the data obtained from the isotherm models. The Langmuir isotherm model was best fitted to the experimental data of SD, SSD, and SDA. The maximum adsorption capacities (Qm) of SD, SSD, and SDA are 91.74, 112.36, and 133.33 mg g−1, respectively. The degree of fitting using the non-linear isotherm models was in the sequence of Langmuir (LNR) (ideal fit) > Freundlich (FRH) > Temkin (SD and SSD) and FRH (ideal fit) > LNR > Temkin (SDA). LNR and FRH ideally described the biosorption of Cu(II) ions to SD and SSD and SDA owing to the low values ofχ2andR2obtained using the non-linear isotherm models. The adsorption rate was well-ordered by the pseudo-second-order (PSO) rate models. Finally, chemically modified biochar with ammonia solutions (SDA) enhances the Cu(II) ions’ adsorption efficiency more than physical activations by sonications (SSD). Response surface methodology (RSM) optimization analysis was studied for the removal of Cu(II) ions using SD, SSD, and SDA biochars.

Background: Air conditioners (AC) have become indispensable in the contemporary World. However, their effects on respiratory health need to be explored further using dynamic and static lung functions. Objectives: The objectives were: 1. To evaluate and compare dynamic parameters (FEV1, FVC, FEV1/FVC, PEFR, and FEF25-75) in AC users and non-users of Bengaluru. 2. To evaluate and compare airway resistance and specific airway conductance as additional parameters. Materials and Methods: After applying ATS-ERS criteria for lung disorders, 30 AC users and non-users of Bengaluru, aged 18-40 yrs were assessed using body plethysmography. A significant association of restrictive pattern was seen in AC users (9 AC users out of 60 participants, Chi-square 8.37, P = 0.0038) having an Odd's ratio of 26.95 (CI: 1.4876 to 488.3558, Z = 2.229, P = 0.0258). Airway resistance (Raw) was comparable in both groups further indicating a non-obstructive pathology. Conclusion: AC users had a significant decrease in flow rates (PEFR and FEF25-75) as well as an association with restrictive lung disorder suggesting the need for proper ventilation and hygiene. It is crucial to develop policies to maintain indoor air quality. [ABSTRACT FROM AUTHOR]

Background India is endemic for dengue. The present formative study reports awareness and practices of people regarding dengue and its prevention, personal protection behaviours and peoples’ exposure to dengue-related health promotion activities. Methods This is a cross-sectional study of 1194 households randomly selected from 20 slum clusters in Delhi. Data were collected from an adult member of the household through an interviewer-administered, pre-tested questionnaire. An observation checklist was used to identify potential breeding sources. Results People were aware that dengue is transmitted through mosquitoes and sources of breeding (clean, stagnant water -60.1%, domestic water containers -18.7%, coolers -15.0%). Each house had at least one potential source of mosquito breeding. Using mosquito repellents (83.3%) was the most common personal protection behaviour. Peoples’ participation is limited in the mosquito prevention and health promotion activities carried out by the local municipality. Still, participation resulted in positive behavioural change. Conclusions While people were aware of dengue transmission, preventive measures against mosquito breeding were not satisfactory. People could not relate their existing knowledge on dengue to their household surroundings, and their awareness had not been translated to behavioural change. Community participation is limited, but is significant in the prevention and control of dengue.

Globally, sickle cell disease (SCD) is one of the major public health problems. In India, it is more prevalent in tribal communities. Tribal communities are socio-economically disadvantaged and constitute 8.6% of India's population. The health and health care seeking of these communities is very poor. Though efficacious interventions are available to manage SCD, they are not reaching these people and no comprehensive programme is in place. The objective of this analysis is to demonstrate the burden of SCD among the tribes in two Indian states of Andhra Pradesh and Telangana, as a case and to highlight the need for public health intervention and health systems strengthening in the country to prevent and manage SCD. One in 10 persons of tribal population of these states carries Hb S gene. A substantial number of children are born every year with the condition. Mostly, the research is limited to screening. Hence, a programme with early detection and an appropriate referral system should be developed. The primary health care system should be strengthened to screen and manage SCD persons with good disease management practices and appropriate community mobilisation activities. The programme should partner with traditional healers and community leaders. People should be encouraged to seek treatment; and counselling for prevention. The study warrants human-centric approaches during the interventions to address the possible threat of fear of being stigmatised. Thus, the transition of evidence-based interventions into the programme and practice can improve the lives of people with SCD, particularly the tribal population.

Aim: The main aim of this study is to estimate the incidence of switching to second line Anti-Retroviral Therapy (ART) among HIV patients. Objective: The primary objective of this study is to determine the reason for switching to second line ART. The secondary objective of the study is to estimate the rate of treatment failure among HIV patients receiving first line ART. To determine the factors responsible for first line treatment failure. Methodology: A Retrospective Observational study is conducted at a tertiary care hospital, Ongole, Prakasam District, Andhra Pradesh. Incidence of switching to second line ART among 4,187 HIV patients is assessed. Results: In our study, we have collected ART cards of 4,187 patients living with HIV/AIDS. Out of which 3,419 patients are excluded from the study. 768 patients are included for the study who met the inclusion criteria. Of these patients, 739 members are receiving first line regimen and 29 patients are switched to second line Anti-Retroviral Therapy (3.77%). Conclusion: Our study concluded a low incidence of switching to second line ART with an incidence rate of 1.01 per 100 persons a year. Out of 29 patients, failure of first line treatment is majorly observed with AZT+3TC+NVP (Zidovudine+Lamivudine+Nevirapine) among 13 patients (44.82%). Reasons for failure are determined majorly as a result of immunological and virological failure (37.93%). These reasons are associated with decreased adherence of 80-90% among 21 patients (72.41%).

From 1985 to 2016, the prevalence of underweight decreased, and that of obesity and severe obesity increased, in most regions, with significant variation in the magnitude of these changes across regions. We investigated how much change in mean body mass index (BMI) explains changes in the prevalence of underweight, obesity, and severe obesity in different regions using data from 2896 population-based studies with 187 million participants. Changes in the prevalence of underweight and total obesity, and to a lesser extent severe obesity, are largely driven by shifts in the distribution of BMI, with smaller contributions from changes in the shape of the distribution. In East and Southeast Asia and sub-Saharan Africa, the underweight tail of the BMI distribution was left behind as the distribution shifted. There is a need for policies that address all forms of malnutrition by making healthy foods accessible and affordable, while restricting unhealthy foods through fiscal and regulatory restrictions., Wellcome Trust, Medical Research Council, peer-reviewed
