Abstract

In laboratory medicine, external quality assessment (EQA) schemes have become versatile tools for detecting analytical flaws. However, EQA schemes are lacking for pediatric sex steroid levels. We aimed to investigate the suitability of different estradiol and testosterone immunoassays in a pediatric setting in comparison with clinical liquid chromatography-tandem mass spectrometry (LC-MS/MS) assays. The study was conducted by staff and the advisory group on endocrinology at Equalis, the Swedish provider of EQA schemes for laboratory medicine. The test material consisted of five pooled serum samples from children who were either prepubertal or in puberty. Clinical laboratories enrolled in Equalis EQA schemes for estradiol and testosterone were invited to participate, as were clinical laboratories using LC-MS/MS-assays. Samples were analyzed by either routine immunoassays (n=18) or in-house LC-MS/MS assays (n=3). For estradiol, LC-MS/MS assays showed a high degree of conformity with interlaboratory coefficients of variation (CV) below 24.2 %. Reported levels were between 4.9 ± 1.2 and 33.9 ± 1.6 pmol/L (group mean ± standard deviation). The direct immunoassays had lower precision; their CVs were up to 81.4 %. Reported concentrations were between 25.3 ± 18.1 and 45.7 ± 19.4 pmol/L, an overestimation compared to LC-MS/MS. Testosterone LC-MS/MS also showed a high degree of conformity, CVs were below 13.4 %, and reported concentrations were from 0.06 ± 0.00 to 1.00 ± 0.11 nmol/L. The direct immunoassays had a larger discrepancy between results; CVs were up to 95.8 %. Concentrations were between 0.12 ± 0.11 and 0.85 ± 0.23 nmol/L. For the safe diagnosis and determination of sex steroids in children, analysis with mass spectrometry-based methods is recommended. [ABSTRACT FROM AUTHOR]

12. The diagnostic value of salivary cortisol and salivary cortisone in patients with suspected hypercortisolism

Abstract

Background: Diagnosing endogenous hypercortisolism remains a challenge, partly due to a lack of biochemical tests with good diagnostic accuracy. Objectives: To evaluate the diagnostic value of salivary cortisol and cortisone in patients with suspected hypercortisolism. Methods: Retrospective study including 155 patients with adrenal incidentaloma, and 54 patients with suspected Cushing's syndrome (CS). Salivary samples were collected at home, at 11 p.m., and at 8 a.m. following an over-night dexamethasone suppression test (DST). Salivary cortisol and cortisone were measured with liquid chromatography-tandem mass spectrometry. Results: Ten of 155 patients with adrenal incidentaloma were considered to have autonomous cortisol secretion (ACS). Using previously established cut-offs, all patients with ACS had elevated plasma-cortisol (>50 nmol/L) following DST, 9/10 had elevated late-night salivary cortisone (>15 nmol/L) whereas only 4/10 had elevated late-night salivary cortisol (LNSC; >3 nmol/L) compared to 35%, 9% and 8%, respectively, of the 145 patients with non-functioning adrenal incidentaloma. Six (60%) patents with ACS had elevated salivary cortisol and cortisone at 8 a.m. following DST compared to 9% and 8%, respectively, of patients with non-functioning adrenal incidentaloma. One of 6 patients with overt CS had a normal LNSC and one had normal late-night salivary cortisone, while all had increased salivary cortisol and cortisone following DST. Conclusion: LNSC is not sufficiently sensitive or specific to be used for screening patients with suspected hypercortisolism. Instead, late-night salivary cortisone seems to be a promising alternative in patients with adrenal incidentaloma and salivary cortisone at 8 a.m. following DST in patients with suspected CS. Larger studies are needed to confirm these findings.

22. Low Progesterone and Low Estradiol Levels Associate With Abdominal Aortic Aneurysms in Men.

Abstract

Context: Male sex is a major risk factor for abdominal aortic aneurysms (AAA) but few studies have addressed associations between sex hormone levels and AAA. Objective: We aimed to describe the associations between serum sex steroids and early, screening-detected AAA in men. Methods: We validated a high-sensitivity liquid chromatography–tandem mass spectrometry assay for comprehensive serum sex hormone profiling. This assay was then employed in a case-control study including 147 men with AAA (infrarenal aorta ≥ 30 mm) and 251 AAA-free controls recruited at the general population-based ultrasound screening for AAA in 65-year-old Swedish men. Outcomes included associations between dehydroepiandrosterone, progesterone, 17α-hydroxyprogesterone, androstenedione, estrone, testosterone, dihydrotestosterone, and estradiol and AAA presence. Results: Dehydroepiandrosterone, progesterone, 17α-hydroxyprogesterone, testosterone, and estradiol, but not the other hormones, were lower in men with AAA. In models with adjustments for known AAA risk factors and comorbidity, only progesterone (odds ratio per SD decrease 1.62 [95% CI, 1.18-2.22]) and estradiol (1.40 [95% CI, 1.04- 1.87]) remained inversely associated with the presence of AAA. Progesterone and estradiol contributed with independent additive information for prediction of AAA presence; compared with men with high (above median) levels, men with low (below median) levels of both hormones had a 4-fold increased odds ratio for AAA (4.06 [95% CI, 2.25-7.31]). Conclusion: Measured by a high-performance sex steroid assay, progesterone and estradiol are inversely associated with AAA in men, independent of known risk factors. Future studies should explore whether progesterone and estradiol, which are important reproductive hormones in women, are protective in human AAA. [ABSTRACT FROM AUTHOR]

44. Mass spectrometric investigation of protein nitration in inflammation and neurodegeneration

Abstract

The nitration of tyrosine seems to be very common in human diseases and at least forty different diseases have been reported to be associated with an increased nitration of proteins. The nitration of proteins results in a modification of tyrosine, 3-nitrotyrosine (3-NT), which is stable. This modification is believed to cause toxic effects to the cell and has e.g. been shown to cause increased degradation of proteins and decreased activity of enzymes. The principal methods for investigation of this type of protein modification are assays based on antibody-based techniques, e.g. immunohistochemistry. The majority of all investigations have used such methods. However, these methods often lack necessary information for interpretation of the validity of the results. In this work we describe how 3-NT can be measured in biological samples with an assay using gas chromatography and mass spectrometry. The assay is based on the reduction of the nitrotyrosine in the samples to aminotyrosine and subsequent derivatization to a di-O-methyl-di-N-heptafluorobutyryl derivative. The method was shown to have a limit of detection of 0.03 nM and a limit of quantification of 0.3 nM in human plasma. The method was used to measure free 3-NT and nitration of proteins in a model of zymosan-induced peritonitis. The results from these investigation show increases of 3-NT in serum from day one, together with increases of protein nitration in the cerebral cortex at day eight after the zymosan treatment. However, when applied to CSF samples from healthy controls and patients with either amyotrophic lateral sclerosis or Alzheimer s disease, no obvious difference between the groups could be seen.

Abstract

Purpose Amoxicillin is commonly used in oral surgery for antimicrobial prophylaxis against surgical-site infection and bacteremia because of its effect on oral streptococci. The aim of this study was to determine whether amoxicillin reaches the break-point concentrations in saliva and has any effect on the salivary microbiota, colonizing bacteria on mucosal membranes and on the gingival crevice after a single dose of amoxicillin. Material and Methods Twenty subjects received 2 g of amoxicillin, per os. The facultative and strictly anaerobic microflora, as well as the streptococcal microflora specifically, were followed from baseline and after 1, 4, and 24 hours. Samples were taken for microbial analysis from saliva, the dorsum of the tongue, and the gingival crevice, and were inoculated and cultured. Plasma samples and saliva samples were analyzed for amoxicillin concentrations (free and protein bound) using liquid chromatography and mass-spectrometry. Results Amoxicillin was detected in concentrations over the break-point (>2 μg/mL) of amoxicillin in plasma after 1 and 4 hours but not after 24 hours. The dose had a significant effect on the streptococci in the gingival crevice. Conclusion A single dose given as prophylaxis to prevent a surgical-site infection results in a significant reducing effect on the oral streptococcal microflora in the gingival crevice and may have an impact on bacteria spreading into tissues and the bacteremia of streptococci. Abstract

This symposium will explore the practical/societal impact opportunity and potential of academic conferences in theory and practice. The symposium offers insights on this topic based on empirical research on the practical impact of bringing managers and professionals into the ""machine room"" of the academy examplified by practitioner study trips to the Academy of Management Annual Meeting. Researchers and management/HR practitioners will discuss research end-user participation in academic conferences as a pathway of practical/societal impact of research, the nature of practical/societal impact from practitioner participation in research conferences as well as the circumstances under which knowledge uptake can take place. Abstract

This symposium explores the practical/societal impact opportunity and potential of academic conferences in theory and practice. The symposium offers new insights on the practical value-added of research end-user particiption in academic conferences based on concrete cases and new empirical research on the practical impact of research including a practitioner study trip to the Academy of Management Annual Meeting 2019 explored through survey data, participant and participating observation, interviews, written statements from practitioners as well as practitioner presentations. The symposium debates academic conferences as a pathway of practical/societal impact of research, the nature of practical/societal impact from practitioner participation in research conferences, the circumstances under which knowledge uptake can take place as well as the transfer of knowledge from a conference setting to everyday managerial practice [ABSTRACT FROM AUTHOR]
