4. Prospective Safety Evaluation of a Cardiovascular Health Dietary Supplement in Adults with Prehypertension and Stage I Hypertension

Objective: To prospectively examine the long-term safety of a cardiovascular health dietary supplement by assessing a comprehensive set of safety measures. Design: Single-arm, open-label study. Location: National University of Natural Medicine, Portland, OR. Subjects: Thirty adults with screening blood pressure readings consistent with prehypertension or stage I hypertension. Intervention: One caplet per day of a dietary supplement for 6 months. The investigated herbal-mineral supplement contains several ingredients, most notably Rauwolfia serpentina. Outcome measures: Primary measures included b-type natriuretic peptide (NT-proBNP), aspartate aminotransferase (AST), alanine aminotransferase (ALT), estimated glomerular filtration rate (EGFR), electrolytes, and the Patient Health Questionnaire (PHQ-9). Exploratory measures included physical vital signs, cholesterol levels, high-sensitivity cardiac troponin-I, cystatin C, endothelin, interleukin (IL)-6, IL-17a, tumor necrosis factor-α, high-sensitivity C-reactive protein, blood counts, and the Patient Reported Outcome Measure Information System (PROMIS) Sleep Disturbance Short Form 8b. Results: NT-proBNP, AST, ALT, EGFR, sodium, calcium, magnesium, PHQ-9 score, and the majority of exploratory measures did not change. However, serum potassium increased (p < 0.05), systolic blood pressure decreased (p < 0.0001), and diastolic blood pressure decreased (p < 0.0001). There were no serious adverse events, but 30% of participants withdrew citing potential side effects, most commonly nasal congestion or fatigue; most participants who reported nasal congestion also reported concomitant seasonal allergies. Adherence to the supplement was 90.9%. Conclusions: The findings of this study suggest that the investigated dietary supplement is safe for long-term use in adults with prehypertension and stage I hypertension. Additional results of this study, particularly the increase in serum potassium and decreases in systolic and diastolic blood pressure, are promising and suggest that future research on this dietary supplement, or its ingredients, should further explore effects on blood pressure and biologic mechanisms of action, which may involve potassium-sparing and diuretic effects.

18. Effects of soil properties on the uptake of pharmaceuticals into earthworms

Pharmaceuticals can enter the soil environment when animal slurries and sewage sludge are applied to land as a fertiliser or during irrigation with contaminated water. These pharmaceuticals may then be taken up by soil organisms possibly resulting in toxic effects and/or exposure of organisms higher up the food chain. This study investigated the influence of soil properties on the uptake and depuration of pharmaceuticals (carbamazepine, diclofenac, fluoxetine and orlistat) in the earthworm Eisenia fetida. The uptake and accumulation of pharmaceuticals into E. fetida changed depending on soil type. Orlistat exhibited the highest pore water based bioconcentration factors (BCFs) and displayed the largest differences between soil types with BCFs ranging between 30.5 and 115.9. For carbamazepine, diclofenac and fluoxetine BCFs ranged between 1.1 and 1.6, 7.0 and 69.6 and 14.1 and 20.4 respectively. Additional analysis demonstrated that in certain treatments the presence of these chemicals in the soil matrices changed the soil pH over time, with a statistically significant pH difference to control samples. The internal pH of E. fetida also changed as a result of incubation in pharmaceutically spiked soil, in comparison to the control earthworms. These results demonstrate that a combination of soil properties and pharmaceutical physico-chemical properties are important in terms of predicting pharmaceutical uptake in terrestrial systems and that pharmaceuticals can modify soil and internal earthworm chemistry which may hold wider implications for risk assessment.

BACKGROUND: Antimicrobial resistance remains a growing and significant concern in human and veterinary medicine. Current laboratory methods for the detection and surveillance of antimicrobial resistant bacteria are limited in their effectiveness and scope. With the rapidly developing field of whole genome sequencing beginning to be utilised in clinical practice, the ability to interrogate sequencing data quickly and easily for the presence of antimicrobial resistance genes will become increasingly important and useful for informing clinical decisions. Additionally, use of such tools will provide insight into the dynamics of antimicrobial resistance genes in metagenomic samples such as those used in environmental monitoring. RESULTS: Here we present the Search Engine for Antimicrobial Resistance (SEAR), a pipeline and web interface for detection of horizontally acquired antimicrobial resistance genes in raw sequencing data. The pipeline provides gene information, abundance estimation and the reconstructed sequence of antimicrobial resistance genes; it also provides web links to additional information on each gene. The pipeline utilises clustering and read mapping to annotate full-length genes relative to a user-defined database. It also uses local alignment of annotated genes to a range of online databases to provide additional information. We demonstrate SEAR's application in the detection and abundance estimation of antimicrobial resistance genes in two novel environmental metagenomes, 32 human faecal microbiome datasets and 126 clinical isolates of Shigella sonnei. CONCLUSIONS: We have developed a pipeline that contributes to the improved capacity for antimicrobial resistance detection afforded by next generation sequencing technologies, allowing for rapid detection of antimicrobial resistance genes directly from sequencing data. - Abstract
Introduction: Diffuse systemic sclerosis is associated with high mortality; however, the pathogenesis of cardiac death in these patients is not clear. Case presentation: A 56-year-old Caucasian female patient presented with dyspnea and requested to donate her body to science in order to improve understanding of diffuse systemic sclerosis pathogenesis. She had extensive testing for dyspnea including pulmonary function tests, an echocardiogram, cardiac magnetic resonance imaging, and right heart catheterization to characterize her condition. Her case highlights the morbidity seen in this disease, including the presence of extensive skin thickening, digital ulcerations, and scleroderma renal crisis. Conclusion: In this case report, we present the finding of cardiac tissue metabolomics, which may indicate a problem with vasodilation as a contributor to cardiac death in diffuse systemic sclerosis. The use of autopsy and tissue metabolomics in rare disease may help clarify disease pathogenesis.

- Abstract
The 21st annual symposium of the International Isotope Society's United Kingdom Group took place at the Møller Centre, Churchill College, Cambridge, UK, on Friday 12th October 2012. The meeting was attended by around 60 delegates from academia and industry, the life sciences, chemical, radiochemical and scientific instrument suppliers. Delegates were welcomed by Dr Ken Lawrie (GlaxoSmithKline, UK, chair of the IIS UK group). The subsequent scientific programme consisted of oral and poster presentations on isotopic chemistry and applications of labelled compounds or of chemistry with potential implications for isotopic synthesis. Both short-lived and long-lived isotopes were represented, as were stable isotopes. The symposium programme was divided into a morning session chaired by Professor Chris Willis (University of Bristol, UK) and afternoon sessions chaired by Mr Mike Chappelle (Quotient Biosciences, UK) and by Dr Sofia Pascu (University of Bath, UK). The UK meeting concluded with remarks from Dr Ken Lawrie (GlaxoSmithKline, Stevenage, UK).

The shapes of jets with transverse energies, EjetT, up to 45 GeV produced in neutral- and charged-current deep inelastic e+p scattering (DIS) at Q2 > 100 GeV2 have been measured with the ZEUS detector at HERA. Jets are identified using a cone algorithm in the η-φ plane with a cone radius of one unit. The jets become narrower as EjetT increases. The jet shapes in neutral- and charged-current DIS are found to be very similar. The jets in neutral-current DIS are narrower than those in resolved processes in photoproduction and closer to those in direct-photon processes for the same ranges in EjetT and jet pseudorapidity. The jet shapes in DIS are observed to be similar to those in e+e- interactions and narrower than those in p̄p collisions for comparable EjetT. Since the jets in e+e- interactions and e+p DIS are predominantly quark initiated in both cases, the similarity in the jet shapes indicates that the pattern of QCD radiation within a quark jet is to a large extent independent of the hard scattering process in these reactions.

The production of forward jets has been measured in deep inelastic ep collisions at HERA. The results are presented in terms of single differential cross sections as a function of the Bjorken scaling variable (x_{Bj}) and as triple differential cross sections d^3 \sigma / dx_{Bj} dQ^2 dp_{t,jet}^2, where Q^2 is the four momentum transfer squared and p_{t,jet}^2 is the squared transverse momentum of the forward jet. Also cross sections for events with a di-jet system in addition to the forward jet are measured as a function of the rapidity separation between the forward jet and the two additional jets. The measurements are compared with next-to-leading order QCD calculations and with the predictions of various QCD-based models., Comment: 27 pages, 10 figures, 4 tables

The DIS diffractive cross section, dσdiffγ*p→XN/dMx, has been measured in the mass range Mx < 15 GeV for γ*p c.m. energies 60 < W < 200 GeV and photon virtualities Q2 = 7 to 140 GeV2. For fixed Q2 and Mx, the diffractive cross section rises rapidly with W, dσdiffγ*p→X N (Mx, W, Q2)/dMx ∝ Wadiff with adiff = 0.507 ± 0.034 (stat) +0.155-0.046 (syst) corresponding to a t-averaged pomeron trajectory of ̄αℙ = 1.127 ± 0.009 (stat) +0.039-0.012 (syst) which is larger than ̄αℙ observed in hadron-hadron scattering. The W dependence of the diffractive cross section is found to be the same as that of the total cross section for scattering of virtual photons on protons. The data are consistent with the assumption that the diffractive structure function FD(3)2 factorizes according to cursive greek chiℙFD(3)2(cursive greek chiℙ, β, Q2) = (cursive greek chi0//cursive greek chiℙ)nFD(2)2(β, Q2). They are also consistent with QCD based models which incorporate factorization breaking. The rise of cursive greek chiℙFD(3)2 with decreasing cursive greek chiℙ and the weak dependence of FD(2)2 on Q2 suggest a substantial contribution from partonic interactions.

The cross section for the photoproduction of events containing three jets with a three-jet invariant mass of M_3J > 50 GeV has been measured with the ZEUS detector at HERA. The three-jet angular distributions are inconsistent with a uniform population of the available phase space but are well described by parton shower models and O(alpha alpha_s^2) pQCD calculations. Comparisons with the parton shower model indicate a strong contribution from initial state radiation as well as a sensitivity to the effects of colour coherence., 13 pages including 5 figures

This paper presents the first analysis of diffractive photon dissociation events in deep inelastic positron-proton scattering at HERA in which the proton in the final state is detected and its momentum measured. The events are selected by requiring a scattered proton in the ZEUS leading proton spectrometer (LPS) with xL > 0.97, where xL is the fraction of the incoming proton beam momentum carried by the scattered proton. The use of the LPS significantly reduces the contamination from events with diffractive dissociation of the proton into low mass states and allows a direct measurement of t, the square of the four-momentum exchanged at the proton vertex. The dependence of the cross section on t is measured in the interval 0.073 < \t\ < 0.4 GeV2 and is found to be described by an exponential shape with the slope parameter b = 7.2 ± 1.1(stat.)+0.7-0.9(syst.) GeV-2. The diffractive structure function FD(4)2 is presented as a function of xIP, ≃ 1 - xL and β, the momentum fraction of the struck quark with respect to xIP, and averaged over the t interval 0.073 < \t\ < 0.4 GeV2 and the photon virtuality range 5 < Q2 < 20 GeV2. In the kinematic range 4 x 10-4 < xIP < 0.03 and 0.015 < β < 0.5, the xIP dependence of FD(4)2 is fitted with a form (1/xIP)a , yielding a = 1.00 ± 0.09 (stat.)+0.11-0.05(syst.). Upon integration over t, the structure function FD(3)2 is determined in a kinematic range extending to higher xIP and lower β compared to our previous analysis; the results are discussed within the framework of Regge theory.

- Abstract
A small electromagnetic sampling calorimeter, installed in the ZEUS experiment in 1995, significantly enhanced the acceptance for very low x and low Q2 inelastic neutral current scattering, e+p → e+X, at HERA. A measurement of the proton structure function F2 and the total virtual photon-proton (γ*p) cross-section is presented for 0.11 ≤ Q2 ≤ 0.65 GeV2 and 2 × 10-6 ≤ x ≤ 6 × 10-5, corresponding to a range in the γ*p c.m. energy of 100 ≤ W ≤ 230 GeV. Using the ZEUS detector at HERA, we have searched for heavy excited states of electrons, neutrinos, and quarks in $e^+p$ collisions at a center-of-mass energy of 300~GeV. With an integrated luminosity of 9.4~pb$^{-1}$, no evidence was found for electroweak production and decay of such states. Limits on the production cross section times branching ratio and on the characteristic couplings, $f/\Lambda$, are derived for masses up to 250~GeV. For the particular choice $f/\Lambda = 1/M_{f^*}$, we exclude at the 95% confidence level excited electrons with mass between 30 and 200~GeV, excited electron neutrinos with mass between 40 and 96~GeV, and quarks excited electroweakly with mass between 40 and 169~GeV., Comment: 39 pages including 11 figures

The reaction gamma p -> J/Psi p has been studied in ep interactions using the ZEUS detector at HERA. The cross section for elastic J/Psi photoproduction has been measured as a function of the photon-proton centre of mass energy W in the range 40 < W < 140 GeV at a median photon virtuality Q^2 of 5*10^{-5} GeV^2. The photoproduction cross section, sigma_{gamma p -> J/Psi p}, is observed to rise steeply with W. A fit to the data presented in this paper to determine the parameter $\delta$ in the form sigma_{gamma p -> J/Psi p} \propto W^{\delta} yields the value \delta = 0.92 \pm 0.14 \pm 0.10. The differential cross section dsigma/d|t| is presented over the range |t| < 1.0 GeV^2 where t is the square of the four-momentum exchanged at the proton vertex. d\sigma/d|t| falls exponentially with a slope parameter of 4.6 \pm 0.4 (+0.4-0.6) GeV^{-2}. - Abstract
A role for Lyn kinase as a positive regulator of immunoglobulin (Ig)E-dependent allergy has long been accepted. Contrary to this belief, Lyn kinase was found to have an important role as a negative regulator of the allergic response. This became apparent from the hyperresponsive degranulation of lyn-/- bone marrow-derived mast cells, which is driven by hyperactivation of Fyn kinase that occurs, in part, through the loss of negative regulation by COOH-terminal Src kinase (Csk) and the adaptor, Csk-binding protein. This phenotype is recapitulated in vivo as young lyn-/- mice showed an enhanced anaphylactic response. In vivo studies also demonstrated that as lyn-/- mice aged, their serum IgE increased as well as occupancy of the high affinity IgE receptor (FcepsilonRI). This was mirrored by increased circulating histamine, increased mast cell numbers, increased cell surface expression of the high affinity IgE receptor (FcepsilonRI), and eosinophilia. The increased IgE production was not a consequence of increased Fyn kinase activity in lyn-/- mice because both lyn-/- and lyn-/- fyn-/- mice showed high IgE levels. Thus, lyn-/- mice and mast cells thereof show multiple allergy-associated traits, causing reconsideration of the possible efficacy in therapeutic targeting of Lyn in allergic disease.

OBJECTIVES The study replicated methods used in an earlier study to determine the prevalence of dissociative disorders among patients with substance use disorders and to examine demographic characteristics and history of childhood abuse among patients with and without dissociative comorbidity. METHODS A total of 100 inpatients who were completing a substance abuse treatment program at a VA medical center were interviewed using the Dissociative Disorders Interview Schedule, which diagnoses dissociative disorders. Two additional screening measures of dissociative symptoms were used, as well as an instrument to measure IQ. RESULTS Fifteen percent of the sample were diagnosed as having a dissociative disorder. Compared with patients without a dissociative disorder, the patients with a dissociative disorder had significantly higher median scores on the two screening measures, indicating more dissociative experiences and the presence of five distinct symptom clusters. The two groups did not differ in history of childhood abuse or IQ. CONCLUSIONS The results support earlier findings suggesting that patients with substance abuse disorder should be routinely screened for dissociative symptoms and disorders.

Rationale: Pulmonary arterial hypertension (PAH) is a proliferative arteriopathy associated with glucose transporter-1 (Glut1) up-regulation and a glycolytic shift in lung metabolism. Glycolytic metabolism can be detected with the positron emission tomography (PET) tracer (18)F-fluorodeoxyglucose (FDG).Objectives: The precise cell type in which glycolytic abnormalities occur in PAH is unknown. Moreover, whether FDG-PET is sufficiently sensitive to monitor PAH progression and detect therapeutic regression is untested. We hypothesized that increased lung FDG-PET reflects enhanced glycolysis in vascular cells and is reversible in response to effective therapies.Methods: PAH was induced in Sprague-Dawley rats by monocrotaline or chronic hypoxia (10% oxygen) in combination with Sugen 5416. Monocrotaline rats were treated with oral dichloroacetate or daily imatinib injections. FDG-PET scans and pulmonary artery acceleration times were obtained weekly. The origin of the PET signal was assessed by laser capture microdissection of airway versus vascular tissue. Metabolism was measured in pulmonary artery smooth muscle cell (PASMC) cultures, using a Seahorse extracellular flux analyzer.Measurements and Main Results: Lung FDG increases 1-2 weeks after monocrotaline (when PAH is mild) and is normalized by dichloroacetate and imatinib, which both also regress medial hypertrophy. Glut1 mRNA is up-regulated in both endothelium and PASMCs, but not airway cells or macrophages. PASMCs from monocrotaline rats are hyperproliferative and display normoxic activation of hypoxia-inducible factor-1α (HIF-1α), which underlies their glycolytic phenotype.Conclusions: HIF-1α-mediated Glut1 up-regulation in proliferating vascular cells in PAH accounts for increased lung FDG-PET uptake. We assessed the relationship between U.S. News and World Report 2008 rankings of clinical psychology doctoral programs and scores earned by graduates on the Examination for Professional Practice in Psychology (EPPP). For the top 25 programs, relationship between ranking and EPPP scores was not significant, rS= -.28. EPPP scores were computed by region (i.e., West, Midwest, South, and Northeast) to determine if geographic differences existed in terms of test performance, but none were found. The number of applications received and acceptance rates were computed for schools by region. Programs in the Midwest received significantly fewer applications, but reported significantly higher acceptance rates. The implications of this finding are discussed. [ABSTRACT FROM AUTHOR]

46. Polybrominated diphenyl ethers (PBDEs) in U.S. mother's milk.

- Abstract
No previous reports exist on polybrominated diphenyl ether (PBDE) congeners in human milk from individual U.S. mothers. This article on PBDEs is an extension of our previous studies on concentrations of dioxins, dibenzofurans, polychlorinated biphenyls, and other chlorinated organic compounds in human milk in a number of countries. PBDE commercial products are used as flame retardants in flexible polyurethane foam (penta-BDE), in acrylonitrile-butadiene-styrene resins (octa-BDE), and in high-impact polystyrene resins (deca-BDE). Their use is permitted in the United States but is banned in some European countries because of presumed toxicity, demonstrated persistence, and bioaccumulation. Different commercial products can be found in various consumer products such as television sets, computers, computer monitors and printers, carpets, and upholstery. Analyses of human levels of these compounds suggest low but rising levels in European human milk, which may have peaked, at least in Sweden, in the late 1990s. Very few data exist on levels of PBDEs in humans in the United States, and none from milk from individual nursing mothers. To address this issue, we analyzed 47 individual milk samples from nursing mothers, 20-41 years of age, from a milk bank in Austin, Texas, and a community women's health clinic in Dallas, Texas. Up to 13 PBDE congeners were measured. The concentrations of the sum of PBDE congeners varied from 6.2 to 419 ng/g (or parts per billion) lipid, with a median of 34 ng/g and a mean of 73.9 ng/g lipid. The PBDE levels in breast milk from Texas were similar to levels found in U.S. blood and adipose tissue lipid from California and Indiana and are 10-100 times greater than human tissue levels in Europe. Their detection in breast milk raises concern for potential toxicity to nursing infants, given the persistence and bioaccumulative nature of some of the PBDE congeners. These results indicate a need for more detailed investigation of the levels of PBDE in people and food, as well as determining if animal fat in food is the major route of exposure of the general U.S. population. Congenital right diaphragmatic hernia is a potentially lethal defect if not properly diagnosed and repaired. Even when it is suspected clinically, recognition may be difficult as initial plain roentgenograms will perhaps appear normal. This report of four cases of herniation of the liver through the right diaphragm stresses the importance of serial roentgenograms and sometimes opaque studies in the diagnosis of this serious congenital defect. Incidence The rarity of congenital right-sided diaphragmatic hernia has been pointed out by several authors. Eppinger (1) presented 635 cases of herniation of the diaphragm; of these only 30 were congenital and on the right side. Kerr and Steinberg (2) reported 13 cases of diaphragmatic hernia with only 3 congenital through the right diaphragm. The shape of jets produced in quasi-real photon-proton collisions at centre-of-mass energies in the range 134-277 GeV has been measured using the hadronic energy flow. The measurement was done with the ZEUS detector at HERA. Jets are identified using a cone algorithm in the η - φ plane with a cone radius of one unit. Measured jet shapes both in inclusive jet and dijet production with transverse energies EjetT > 14 GeV are presented. The jet shape broadens as the jet pseudorapidity (ηjet) increases and narrows as EjetT increases. In dijet photoproduction, the jet shapes have been measured separately for samples dominated by resolved and by direct processes. Leading-logarithm parton-shower Monte Carlo calculations of resolved and direct processes describe well the measured jet shapes except for the inclusive production of jets with high ηjet and low EjetT. The observed broadening of the jet shape as ηjet increases is consistent with the predicted increase in the fraction of final state gluon jets.
