Correct placement of side dress nitrogen (N) fertilizer could increase nitrogen use efficiency (NUE) and maize yield production. Field studies were established to evaluate application of midseason (V8 to V10), variable liquid urea ammonia nitrate (28%), N rates (0, 45, 90 and 134 kg N ha−1) and different application distances (0, 10, 20 and 30 cm) away from the maize row on grain yield and NUE at Haskell and Hennessey in 2009, Efaw in 2010 and Lake Carl Blackwell, Oklahoma in 2009 and 2010. A randomized complete block design with three replications was used throughout the study. Results indicated that maize grain yield in sites with adequate rainfall increased significantly (p < 0.05) with N rate, and poor N response was recorded in sites with low rainfall. Across sites and seasons, varying side dress N application distance away from the maize row did not significantly (p < 0.05) influence maize grain yield and NUE even with no prep-plant applied. Environments with adequate rainfall distribution had better maize grain yields when high side dress N rates (90 and 134 kg N ha−1) were applied 0 to 10 cm, and a higher NUE when 45 kg N ha−1 was applied 0 to 20 cm away from the maize row. For low N rates (45 kg N ha−1), increased maize grain yield and NUE were achieved when side dress N was applied 0 to 20 cm away from the maize row at locations with low rainfall distribution. Across sites and seasons, increasing side dress N to 134 kg N ha−1 contributed to a general decline in mean NUE to as low as 4%, 35%, 10%, 51% at Hennessey, Efaw, LCB (2009) and LCB (2010) respectively. Background: Gastroschisis is a common birth defect with < 5% mortality in high income countries, but mortality in sub Saharan Africa remains high. We sought to compare gastroschisis management strategies and patient outcomes at tertiary pediatric referral centers in the United States and Kenya., Methods: This retrospective chart review examined uncomplicated gastroschisis patients treated at Riley Hospital for Children in Indianapolis, USA (n = 110), and Shoe4Africa Children's Hospital in Eldoret, Kenya (n = 75), from 2010 to 2018. Analyzed were completed using Chi square, Fisher's exact, and independent samples t tests and medians tests at the 95% significance level., Results: Survival in the American cohort was double that of the Kenyan cohort (99.1% vs 45.3%, p< 0.001). Sterile bag use for bowel containment was lower in Kenya (81.3% vs 98.1%, p< 0.001), but silo use was comparable at both institutions (p = 0.811). Kenyan patients had earlier median enteral feeding initiation (4vs 10 days, p< 0.001) and accelerated achievement of full enteral feeding (10vs 23 days, p< 0.001), but none received TPN. Despite earlier feeding, Kenyan patients displayed a higher prevalence of wound infections (70.8% vs 17.1%, p< 0.001) and sepsis (43.9% vs 4.8%, p< 0.001). In Kenya, survivors and non survivors displayed no difference in sterile bag use, hemodynamic stability, all cause infection rates, or antibiotic free hospital days. Defect closure (p< 0.001) and enteral feeding initiation (p< 0.001) were most predictive of survival., Conclusion: Improving immediate response strategies for gastroschisis in Kenya could improve survival and decrease infection rates. Background: Familial Combined Hyperlipidemia is an inherited disorder affecting cholesterol and triglycerides metabolism, well known myocardial infarction risk factors. The FCHL clinical presentation is usually silent until the third decade although children can be affected, and the more recent opinion is that precocious diagnosis is mandatory in preventing complications. Aim of this study is to examine the effectiveness of the diet therapy (Step-One-Diet) in a group of 13 children affected by Familial Combined Hyperlipidemia., Methods: The patients have been submitted to a normocaloric diet, 30% fat of the total caloric daily intake according with the Dietary Intervention Study in Children (Step-One-Diet). The patients then have been submitted to a two year-follow-up and lipoprotein levels (total cholesterol, LDL-cholesterol, triglycerides and apolipoprotein B), nutritional status (macro- and micro-nutrients) as well as anthropometric data (height, weight, BMI) have been monitored., Results: Results showed a 10% total cholesterol and 30% triglycerides decrease, Iron and Calcium intake show increased levels approaching to the normal ones after controlled diet, while cholesterol intake was correct on both regimen. The growth parameters show a decrease in weight only in two obese and two overweight patients., Conclusions: The present study confirmed the effectiveness and safety of the Step-One-Diet in children patients, allowing triglycerides normalization in 60% of the patients, and a 10% cholesterol decrease, in agreement with the complex genetic inheritance of the disease.
