15. Black farce in Jacobean and 1960s theatre

Abstract

This thesis examines black farce as a genre of simultaneous and equally (though precariously) balanced farce and horror, characterised by excess, physicality, taboo-breaking, ambivalence and disharmony. Black farce provokes an uncertain audience response, in which a combined sense of shock and laughter leads to feelings of discomfort and disorientation. Building upon this definition of the genre, I argue that the reason for its appearance in the Renaissance and its re-emergence in the 1960s lies in a paradoxical sense of exhilaration and horror felt by playwrights in response to bewilderingly rapid social change and the dismantling of many established structures and codes. In the first section of the thesis, the genre of black farce is defined and studied through its correspondences with and distinctions from closely related genres with which it coincides and overlaps. The precise nature of the relationship between the plays of the two periods is then demonstrated through the analysis of their blackly farcical treatment of four broad, central themes: madness, violence, death and sex. As well as revealing some close specific parallels between particular Jacobean and 1960s plays, this analysis locates the dramatic constructions, of those themes within their respective sociological contexts, thus indicating the various factors than gave rise to black farce's particular combination of exhilaration and despair in societies more than three and a half centuries apart. In the final section, a detailed examination of the professional revivals of Jacobean plays during the 1960s shows how the fruitful contact between new and old highlighted and enriched the relationship between them.

Abstract

Methods of Determining Cell of Origin (COO) From a panel of genes involved in lymphocyte development and activation, Alizadeh et al created a unique DNA microarray (termed the Lymphochip) and [...], Diffuse large B-cell lymphoma (DLBCL) is curable in about two-thirds of patients. Research has focused on determining which patients have less favorable prognoses so that they can be considered for novel targeted-treatment strategies. In 2000, gene expression profiling was used to define two principal DLBCL molecular subtypes, germinal center B-cell--like (GCB) and activated B-cell--like (ABC). Patients with GCB DLBCL have more favorable outcomes than those with ABC DLBCL when treated with standard immunochemotherapy. Alternate strategies to characterize molecular subtype include approximation with immunohistochemistry algorithms, and more recently the NanoString gene expression platform. Numerous studies have investigated novel agents in DLBCL with respect to GCB and ABC (or non-GCB) subtypes, but R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone) remains the standard of care for most patients. Here we review the methods of determining cell of origin (COO); use of COO in clinical practice; clinical trials in DLBCL according to COO; and future directions of tailoring treatment, including alternate categorization of genetic subtypes or clusters in DLBCL.

Abstract

Bromodomain and extraterminal (BET) proteins are transcriptional activators for multiple oncogenic processes in diffuse large B-cell lymphoma (DLBCL), including MYC, BCL2, E2F, and toll-like receptor signaling. We report results of a phase 1b dose-escalation study of the novel, subcutaneous BET inhibitor RO6870810 (RO) combined with the BCL-2 inhibitor venetoclax, and rituximab, in recurrent/refractory DLBCL. RO was delivered for 14 days of a 21-day cycle, whereas venetoclax was delivered continuously. A 3 + 3 escalation design was used to determine the safety of the RO+venetoclax doublet; rituximab was added in later cohorts. Thirty-nine patients were treated with a median of 2.8 cycles (range, 1-11). Dose-limiting toxicities included grade 3 febrile neutropenia, grade 4 diarrhea, and hypomagnesemia for the doublet; and grade 3 hyperbilirubinemia and grade 4 diarrhea when rituximab was added. The doublet maximum tolerated dose (MTD) was determined to be 0.65 mg/kg RO+600 mg venetoclax; for RO+venetoclax+rituximab, the MTDs were 0.45 mg/kg, 600 mg, and 375 mg/m2, respectively. The most frequent grade 3 and 4 adverse events were neutropenia (28%) and anemia and thrombocytopenia (23% each). Responses were seen in all cohorts and molecular subtypes. Sustained decreases in CD11b on monocytes indicated pharmacodynamic activity of RO. Overall response rate according to modified Lugano criteria was 38.5%; 48% of responses lasted for ≥180 days. Complete response was observed in 8 patients (20.5%). Optimization of the treatment schedule and a better understanding of predictors of response would be needed to support broader clinical use. 32. Unmet mental health needs in patients with advanced B-cell lymphomas.

Abstract

Context: Existing research on psychological distress and mental health service utilization has focused on common types of solid tumor cancers, leaving significant gaps in our understanding of patients experiencing rare forms of hematologic cancers.Objective: To examine distress, quality of life, and mental health service utilization among patients with aggressive, refractory B-cell lymphomas.Method: Patients (n = 26) with B-cell lymphomas that relapsed after first- or second-line treatment completed self-report measures of distress (Hospital Anxiety and Depression Scale) and quality of life (Short-Form Health Survey, SF-12). Patients also reported whether they had utilized mental health treatment since their cancer diagnosis.Results: Approximately 42% (n = 11) of patients reported elevated levels of psychological distress. Of patients with elevated distress, only one quarter (27.2%; n = 3) received mental health treatment, while more than half did not receive mental health treatment (54.5%; n = 6), and 18.1% (n = 2) did not want treatment. Patients with elevated distress reported lower mental quality of life than patients without elevated distress [F (1, 25) = 15.32, p = 0.001].Significance Of the Results: A significant proportion of patients with advanced, progressive, B-cell lymphomas may experience elevated levels of distress. Yet, few of these distressed patients receive mental health treatment. Findings highlight the need to better identify and address barriers to mental health service utilization among patients with B-cell lymphoma, including among distressed patients who decline treatment.
