Abstract

Las úlceras por presión (UPP) son las lesiones más frecuentes de las relacionadas con la dependencia y tienen un gran impacto en la calidad de vida de quienes las sufren. Disponer de herramientas en español específicas para evaluar la calidad de vida percibida por los pacientes con UPP se considera un elemento indispensable para la toma de decisiones sobre los cuidados de salud. El objetivo de este estudio fue traducir al español y adaptar culturalmente el cuestionario Pressure Ulcer Quality of Life Questionnaire (PU-QOL) para la medición de la calidad de vida relacionada con la salud en pacientes con úlceras por presión., Pressure ulcers (PU) are the most prevalent of the dependency-related injuries, affecting the quality of life of the patients who suffer them. However, there are no instruments adapted to the Spanish context to evaluate this quality of life. The use of specific tools in Spanish to evaluate the quality of life perceived by patients with PUs is considered an indispensable element for healthcare decisions. The aim of this paper was to translate and culturally adapt the Pressure Ulcer Quality of Life Questionnaire (PUQOL) into Spanish for the measurement of health-related quality of life in patients with pressure ulcers., Proyecto de investigación Análisis de resultados en Salud en Atención Primaria. 18. Foundational Principles in the Development of AdH-SW3, the Three-Dimensional Shallow Water Hydrodynamics and Transport Module within the Adaptive Hydraulics/Hydrology Model

Abstract

This report details the design and development of the three-dimensional shallow water hydrodynamics formulation within the Adaptive Hydraulics/Hydrology model (AdH-SW3) for simulation of flow and transport in rivers, estuaries, reservoirs, and other similar hydrologic environments. The report is intended to communicate principles of the model design for the interested and diligent user. The design relies upon several layers of consistency to produce a stable, accurate, and conservative model. The mesh design can handle rapid changes in bathymetry (e.g., steep-sided navigation channels in estuaries) and maintain accuracy in density-driven transport phenomena (e.g., thermal, or saline stratification and intrusion of salinity).

19. Improving the patient-reported outcome sections of clinical trial protocols: a mixed methods evaluation of educational workshops

Abstract

INTRODUCTION: Failure to incorporate key patient-reported outcome (PRO) content in trial protocols affects the quality and interpretability of the collected data, contributing to research waste. Our group developed evidence-based training specifically addressing PRO components of protocols. We aimed to assess whether 2-day educational workshops improved the PRO completeness of protocols against consensus-based minimum standards provided in the SPIRIT-PRO Extension in 2018. METHOD: Annual workshops were conducted 2011-2017. Participants were investigators/trialists from cancer clinical trials groups. Although developed before 2018, workshops covered 15/16 SPIRIT-PRO items. Participant feedback immediately post-workshop and, retrospectively, in November 2017 was summarised descriptively. Protocols were evaluated against SPIRIT-PRO by two independent raters for workshop protocols (developed post-workshop by participants) and control protocols (contemporaneous non-workshop protocols). SPIRIT-PRO items were assessed for completeness (0 = not addressed, 10 = fully addressed). Mann-Whitney U tests assessed whether workshop protocols scored higher than controls by item and overall. RESULTS: Participants (n = 107) evaluated the workshop positively. In 2017, 16/41 survey responders (39%) reported never applying in practice; barriers included role restrictions (14/41, 34%) and lack of time (5/41, 12%). SPIRIT-PRO overall scores did not differ between workshop (n = 13, median = 3.81/10, interquartile range = 3.24) and control protocols (n = 9, 3.51/10 (2.14)), (p = 0.35). Workshop protocols scored higher than controls on two items: 'specify PRO concepts/domains' (p = 0.05); 'methods for handling missing data' (p = 0.044). CONCLUSION: Although participants were highly satisfied with these workshops, the completeness of PRO protocol content generally did not improve. Abstract

European Strategy Update | Laboratory Directors Group Editor: Nicolas Mounet The 2020 update of the European Strategy for Particle Physics emphasised the importance of an intensified and well-coordinated programme of accelerator R&D, supporting the design and delivery of future particle accelerators in a timely, affordable and sustainable way. This report sets out a roadmap for European accelerator R&D for the next five to ten years, covering five topical areas identified in the Strategy update. The R&D objectives include: improvement of the performance and cost-performance of magnet and radio frequency acceleration systems; investigations of the potential of laser/plasma acceleration and energy-recovery linac techniques; and development of new concepts for muon beams and muon colliders. The goal of the roadmap is to document the collective view of the field on the next steps for the R&D programme, and to provide the evidence base to support subsequent decisions on prioritisation, resourcing and implementation.

Abstract

The prediction of a difficult cholecystectomy has traditionally been based on certain pre-operative clinical and imaging factors. Most of the previous literature reported small patient cohorts and have not used an objective measure of operative difficulty. The aim of this study was to develop a pre-operative score to predict difficult cholecystectomy, as defined by a validated intra-operative difficulty grading scale. Two cohorts from prospectively maintained databases of patients who underwent laparoscopic cholecystectomy were analysed: the CholeS Study (8755 patients) and a single surgeon series (4089 patients). Factors potentially predictive of difficulty were correlated to the Nassar intra-operative difficulty scale. A multivariable binary logistic regression analysis was then used to identify factors that were independently associated with difficult laparoscopic cholecystectomy, defined as operative difficulty grades 3 to 5. The resulting model was then converted to a risk score, and validated on both internal and external datasets. Increasing age and ASA classification, male gender, diagnosis of CBD stone or cholecystitis, thick-walled gallbladders, CBD dilation, use of pre-operative ERCP and non-elective operations were found to be significant independent predictors of difficult cases. Abstract

Aims: Background: Upper gastrointestinal (UGI) cancers (pancreatic, oesophageal and stomach) have a dismal prognosis, with five-year survival of only 7.7-28.5% even when detected in their early stages and treated with a complete resection. UGI cancers are characterised by a high symptom burden. It is critical that patients with these cancers receive the best possible symptomatic control, while the search continues for treatments to improve survival. Recent research has shown that patient-reported outcomes (PROs), integrated into clinical practice in a meaningful way to drive action, are associated with improved patient health outcomes and decrease burden to the healthcare system. Aim(s): To determine whether the systematic measurement, identification and management of PROs in clinical practice, coupled with information support to patients, improves health outcomes for patients with UGI cancers. Methods and Design: A registry-based randomised, multicentre, controlled trial will be designed to assess whether, compared to usual practice, the UGI Cancer Care Coordination (UGICCC) intervention is effective in improving health-related quality of life in patients diagnosed with UGI cancers, as the primary outcome. The UGICCC intervention involves: (1) collection of PROs to allow patients to track and report their symptoms over time; (2) provision of real-time feedback to patients for management of symptoms; and (3) alerts to a care-coordinator of patients reporting severe or worsening symptoms to allow for triage and follow-up as well as provision of real-time reports to clinicians. The secondary outcome measures are to assess whether patients assigned to the intervention arm demonstrate: reduced health service use and emergency department visits; lower level of information needs; timely referral to palliative care services; and increased median survival. Abstract

BACKGROUND: In order to overcome inconsistencies in the reporting of outcomes in clinical trials, core outcome sets (COS) have been developed in many clinical areas and the awareness of this concept is growing steadily. The Outcomes for Pressure Ulcer Trials (OUTPUTs) project aims to improve the quality of evidence on pressure ulcer prevention trials by developing a COS. OBJECTIVES: As an initial step in the COS process we aimed to identify and classify outcomes as well as concepts that represent potential outcomes for future trials that have been reported in pressure ulcer prevention research. METHODS: A review was conducted in twelve major databases covering the literature indexed until 2016. Outcomes and relevant concepts reported in primary studies and/or reviews on pressure ulcer prevention in adult patients were extracted as presented in the articles, and afterwards inductively grouped into outcome domains. The domains were then categorized according to the outcome domain taxonomy recently proposed by the Core Outcome Measures in Effectiveness Trials Group. RESULTS: 332 studies were included and 68 outcome domains were identified, covering multiple aspects of pressure ulcer prevention. Pressure ulcer occurrence was reported in 71% of all included studies representing the most frequent outcome, followed by costs (22% of all studies), and acceptability of intervention and comfort (18% of all studies). CONCLUSION: A plethora of different outcomes is applied in pressure ulcer prevention research and substantial variations in definitions and reporting of similar outcomes were observed. Abstract

이 지침서의 목적은 ‘멸종위기에 처한 야생동식물 종의 국제 거래에 관한 협약(Convention on International Trade in Endangered Species of Wild Fauna and Flora, 이하 ‘CITES’)’ 및 ‘유럽 야생동물 무역 규정(European Wildlife Trade Regulations, 이하 ‘EU WTR’)’에 따라 국제 거래를 규제 받는 다육식물 종과 그 부분물과 파생물을 소개하고 CITES 부속서 등재, 적용할 수 있는 기술, 단속 상의 문제, 추가 지원이나 정보 확보를 위한 자료 등을 설명하는데 있다. 이 지침서는 물을 저장하거나 손실을 줄일 수 있는 하나 이상의 기관을 가진 다육식물과 건조한 환경을 견디어 생육할 수 있 는 식물에 대하여 다룬다. 전 세계적 분포를 고려할 때, CITES에 등재된 다육식물은 18개 과에 해당하며, 아주 작은 식물에 서 큰 나무의 형태까지 다양하다. 선인장, Aloe속 및 대극속 식물이 주로 거래되는 다육식물이다. 건조한 상황에서 살아남을 수 있는 식물의 능력과 난방 장치의 발달로 자생지 외에서도 재배가 가능해지면서 다육식물은 실 내에서 기르는 식물로 인기가 높아졌다. 또한 건조한 환경에 강한 식물로 조경을 하는 건식 조경(xeriscaping)에서 필수 요 소로 인식되며, 야생에서 보이는 독특한 성장형태 또는 희귀성으로 인해 많은 식물 수집가들의 각광을 받고 있다. 이 지침 서는 다육식물의 거래와 관련한 여러 가지 정보를 제공하며, 특히 비전문가에게 어려울 수 있는 종 식별법과 다육식물 그룹 간의 차이를 보여준다. 다육식물의 분류 방법이나 학명은 복잡할 수 있으며 CITES 부속서에는 개별 종이 기재되기도 아니 면, 속이나 과 명칭으로 기재되기도 한다. 다육식물을 다루는 경우, 부속서 목록과 관련 문제를 확실히 이해하기 위하여 가 능한 식물 또는 분류 전문가에게 연락하는 것을 추천한다. CITES 부속서 목록 및 이 지침서에서 사용된 용어의 정의는 100 쪽의 용어 해설을 참조하면 된다.

Abstract

PURPOSE:Given the high survival rate of cervical cancer patients, understanding women's health-related quality of life (HRQL) during and after treatment is of major clinical importance. We conducted a systematic review to synthesize all available evidence about the effects of each contemporary treatment modality for cervical cancer on all dimensions of women's HRQL, including symptoms, functioning, and global HRQL. METHODS:We searched four electronic databases from January 2000 to September 2019, cross-referenced and searched by author name for studies of patients treated for cervical cancer that reported patient-reported outcomes (PROs) before treatment and with at least one post-treatment measurement. Two independent reviewers applied inclusion and quality criteria and extracted findings. Studies were categorized by treatment to determine specific treatment effects on PROs. Results were narratively summarized. RESULTS:We found twenty-nine papers reporting 23 studies. After treatments with curative intent for early or locally advanced disease, lymphedema, diarrhea, menopausal symptoms, tight and shorter vagina, pain during intercourse, and sexual worries remained long-term problems; however, sexual activity improved over time. HRQL and psychological distress were impacted during treatment with also worsening of global HRQL but improved 3-6 months after treatment. In patients with metastatic or recurrent disease, pain improved during palliative treatment or remained stable, with no differences in global HRQL found over time. CONCLUSION:Whereas most symptoms worsen during treatment and improve in the first 3 months after completing treatment, symptoms like lymphedema, menopausal symptoms, and sexual worries develop gradually and persist after curative treatment. These findings can be used to inform clinical practice and facilitate communication and shared decision-making. More research is needed in very early cervical cancer and the impact of fertility sparing therapy on PROs.

38. Views of healthcare professionals about the role of active monitoring in the management of ductal carcinoma in situ (DCIS): Qualitative interview study

Abstract

BACKGROUND: Ductal carcinoma in situ (DCIS) is an in-situ (pre-cancerous) breast malignancy whereby malignant cells are contained within the basement membrane of the breast ducts. Increasing awareness that some low-risk forms of DCIS might remain indolent for many years has led to concern about overtreatment, with at least 3 clinical trials underway internationally assessing the safety of active monitoring for low-risk DCIS. This study aimed to understand healthcare professionals' (HCPs) views on the management options for patients with DCIS. METHODS: Qualitative study using semi-structured interviews with HCPs involved in the diagnosis and management of DCIS in Australia and New Zealand. Interviews were audio-recorded, transcribed and analysed thematically using Framework Analysis method. RESULTS: Twenty-six HCPs including 10 breast surgeons, 3 breast physicians, 6 radiation oncologists, and 7 breast care nurses participated. There was a strong overall consensus that DCIS requires active treatment. HCPs generally felt uncomfortable recommending active monitoring as a management option for low-risk DCIS as they viewed this as outside current standard care. Overall, HCPs felt that active monitoring was an unproven strategy in need of an evidence base; however, many acknowledged that active monitoring for low-risk DCIS could be appropriate for patients with significant co-morbidities or limited life expectancy. They believed that most patients would opt for surgery wherever possible. CONCLUSIONS: This study highlights the important need for robust randomised controlled trial data about active monitoring for women with low-risk DCIS, to provide HCPs with confidence in their management recommendations and decision-making.

39. Meta -analysis of average change in laboratory-measured HbA1c among people with type 1 diabetes mellitus using the 14 day Flash Glucose Monitoring System

Abstract

AIM: FreeStyle Libre™ Flash Glucose Monitoring System (Flash GM), a novel, sensor-based, factory-calibrated system has been compared with self-monitoring of blood glucose in a well-controlled adult type 1 diabetes mellitus (T1D) population (HbA1c ≤ 7.5%, 58 mmol/mol), in a randomized controlled trial (RCT). The need for RCTs to recruit homogenous patients and for a well-controlled environment may not necessarily reflect use of a new technology in real clinical practice. METHODS: A random effects meta-analysis of all identified studies in T1D was performed to investigate changes in laboratory-measured HbA1c following introduction of Flash GM. RESULTS: Flash GM introduction showed a mean change from baseline to longest follow-up timepoint of -0.41% ([95% CI -0.51%, -0.31%]; P < 0.001; -4.5 [95% CI -5.6, -3.3] mmol/mol) in HbA1c in the random effects meta-analysis (34 studies comprising 5,466 participants). When the Flash GM arms of the two RCTs were excluded, there was a similar change in HbA1c of -0.41% ([95% CI -0.50%, -0.32%], P < 0.001; -4.5 [95% CI -5.4, -3.5] mmol/mol) in the 32 uncontrolled studies. Considerable heterogeneity was shown in all meta-analyses (I2 values > 85%), likely due to the inclusion of diverse populations and variations in study protocols, meaning random effects meta-analyses should be strongly preferred. CONCLUSIONS: In people with T1D, use of Flash GM for 2 to 24 months was associated with an estimated HbA1c reduction from baseline of 0.4%. A similar reduction occurred in uncontrolled studies where baseline HbA1c was generally higher compared with Flash GM arms of well-controlled studies.
