Although toxicity from excessive exogenous vitamin D supplementation is rare, a range of symptoms can occur, most of which result from hypercalcemia. We report a novel case of posterior reversible encephalopathy syndrome (PRES) in a young child who required intensive care after presenting with hypercalcemia, hypertensive emergency, acute kidney injury, and hypercarbic respiratory failure, which ultimately were attributed to vitamin D toxicity (VDT). We report a young child who developed PRES in association with VDT. Our report informs pediatric outpatient, hospitalist, and intensivist providers about rare but life-threatening complications from hypervitaminosis D, adds VDT to the differential diagnosis for children with similar presentations, and highlights the importance of vitamin supplementation safety guidance for families., (Copyright © 2024 by the American Academy of Pediatrics.)

Background: For decades, parental self-efficacy (PSE), or parents' belief in their ability to influence their child in a healthy and success-promoting manner, has been understood as a key factor in promoting healthy functioning for parents and their children. In that time, an extensive collection of research examining the specific impact of PSE on parents and their children has developed. However, to the authors' knowledge, no comprehensive and systematic review of the outcomes linked to this factor exists, and the two most closely related non-systematic reviews were published over 10 years ago., Methods: Therefore, by utilizing an iteratively optimized set of search terms applied across four databases, the current review sought to systematically collect, synthesize, and present the extant literature concerning the role of PSE in parent and child well-being., Results: This search strategy yielded a total of 115 studies, the results of which were organized into three broad thematic categories relating to: the parent and child relationship, parental mental health, or child development., Conclusions: These results recapitulate the clinical relevance of PSE, and provide an updated and comprehensive understanding of both the role PSE plays in the welfare of parents and children, as well as the gaps in the literature as it currently stands., (© 2019 John Wiley & Sons Ltd.)

Renal cell carcinoma is a common neoplasia of the adult kidney that accounts for about 3% of adult malignancies. Clear cell renal carcinoma is the most frequent subtype of kidney cancer and 20-40% of patients develop metastases. The absence of appropriate biomarkers complicates diagnosis and prognosis of this disease. In this regard, small noncoding RNAs (microRNAs), which are mutated in several neoplastic diseases including kidney carcinoma, may be optimal candidates as biomarkers for diagnosis and prognosis of this kind of cancer. Here we show that patients with clear cell kidney carcinoma that express low levels of miR501-5p exhibited a good prognosis compared with patients with unchanged or high levels of this microRNA. Consistently, in kidney carcinoma cells the downregulation of miR501-5p induced an increased caspase-3 activity, p53 expression as well as decreased mTOR activation, leading to stimulation of the apoptotic pathway. Conversely, miR501-5p upregulation enhanced the activity of mTOR and promoted both cell proliferation and survival. These biological processes occurred through p53 inactivation by proteasome degradation in a mechanism involving MDM2-mediated p53 ubiquitination. Our results support a role for miR501-5p in balancing apoptosis and cell survival in clear cell renal carcinoma. In particular, the downregulation of microRNA501-5p promotes a good prognosis, while its upregulation contributes to a poor prognosis, in particular, if associated with p53 and MDM2 overexpression and mTOR activation. Thus, the expression of miR501-5p is a possible biomarker for the prognosis of clear cell renal carcinoma.

Renal infarction secondary to thromboembolism is usually a sequela of cardiac disease, the heart being the source of systemic arterial emboli in up to 94% of cases; the three major causes are: atrial fibrillation, myocardial infarction and rheumatic mitral stenosis. Renal infarction is often confused with other conditions due to similar presenting symptoms. This leads to delay in initiating treatment and significantly decreases the chances of renal salvage. We report a sonographic diagnosis in 39-year-old man, with risk factors for thrombosis, without a prior history of thromboembolism.

Introduction: The purpose of this study was to evaluate how the age factor might influence the tolerance biopsy examination, and then choose the modality of a local or general anesthetic technique., Materials and Methods: This study evaluated 248 consecutive patients undergoing transrectal ultrasound-guided prostate biopsy for the first time. Each patient was treated under local anesthesia with lidocaine spray (10gr/100ml). Patients were divided into two groups according to age: Group A) 112 with age range ≤65 years, Group B) 136 with age range >65 years. Our first aim was to obtain a schedule of 14 biopsy samples in both groups. After the procedure each patient was given a verbal numeric pain scale (VNS), which was designed with 0 representing absence of pain and 10 the maximum pain they perceived in life., Results: The average pain assessed with VNS was around 4.2 (2-8) and 2.8 (0-6) for the patients in Group A and Group B, respectively. The two groups appeared to be homogeneous in terms of pain perception regarding PSA value and gland volume, and appeared to have different pain scores with regard to age. In fact, the first group of patients, with the same anesthetic technique used, perceived more intense pain sensations related to acute biopsy., Conclusions: In conclusion, we can say that a local anesthetic in clinical practice in the course of prostate biopsy is more sensitive if performed in a person aged 65 years.

Introduction: The process of organizing a ultrasound service nowadays can be improved by properly managing the user's request, the speed of response and safety, the standardization of methods and skills., Materials and Methods: The outpatients at our uro-andrologic ultrasound clinic (O.U. of Urology in Ferrara) received a questionnaire each; we administered a total of 640 questionnaires. The number of questionnaires collected was 532. Patients were asked to give an assessment of services using a qualitative method according to a 4-parameter response scale: very satisfied, satisfied, dissatisfied, very dissatisfied., Results: The identification of indicators to be monitored by the user's perceived quality was accomplished by establishing the correlation coefficient between different parameters of analysis and an overall rating of the sample. Some of these parameters were: the relationship with the practitioner, the availability of doctors, the ability of doctors for reassurance, the completeness of information and facilities hygiene conditions. When these parameters vary, positively or negatively, also the citizen's overall opinion changes., Conclusions: The customer satisfaction is an important component of the quality of care, it represents both an indicator of the effectiveness of health intervention and the ability to meet quality requirements of the health service organization. The objective of an ultrasound service should be to provide, within a reasonable timeframe, the supply of high quality with qualified personnel, with adequate tools and procedures.

Objectives: The purpose of this study is to evaluate the current role of the Ultrasound associated with the color-Doppler in the diagnosis of acute pyelonephritis (APN) and to compare ultrasound images with CT images in order to reduce the amount radiation absorbe without significant loss of diagnostic efficacy, since this disease in most cases affects young adults., Material and Methods: We studied 38 patients (aged 17-65 years) who presented from September 2007 to March 2010 to the emergency department with suspected diagnosis of APN. All patients underwent first to an ultrasound study, then to abdominal CT. Renal, perirenal and extrarenal tomographic findings usually associated with acute pyelonephritis were analyzed, in an attempt to identify what are the differences with respect to the images obtained with an ultrasound study. All patients then performed ultrasonography and/or abdominal CT evaluation one month later, 25 patients repeated both examinations, while the other 13 repeated only ultrasound., Results: In 38 subjects with suspected APN, CT assessed the presence in 79% and in 21% the absence of the disease. Ultrasonography in 68% of cases diagnosed APN, by an increase in kidney size related to the presence of hypoechoic areas associated to edema, blurred margins and reduction of the color-Doppler vascularity. Ultrasound associated with the use of color-Doppler revealed a sensibility of 76% and specificity of 75%. Color and power-Doppler have better diagnostic accuracy than basic gray scale ultrasound, in the diagnosis of focal pyelonephritis., Conclusions: Therefore the combined use of ultrasound and color-Doppler can obtain useful information about the diagnosis and follow-up of the disease, with an improvement in terms of cost, without significantly altering the diagnostic efficacy and reducing the amount of radiation absorbed.

Four novel antiviral WIN compounds, that contain a methyl tetrazole ring as well as isoxazole, pyridazine or acetylfuran rings, have had their structures determined in human rhinovirus serotype 14 at 2.9 A resolution. These compounds bind in the VP1 hydrophobic pocket, but are shifted significantly towards the pocket pore when compared to previously examined WIN compounds. A putative water network at the pocket pore is positioned to hydrogen bond with these four WIN compounds, and this network can account for potency differences seen in structurally similar WIN compounds.

The authors report their initial results in ESWL of urinary calculi with the Storz Modulith SL 20 lithotriptor, a multipurpose device for both urinary and biliary stone treatment. During 176 sessions, a total of 144 cases (81 renal and 63 ureteral) were treated and then, followed up. The over-all rate free of stones was 86.8% (90.1% for kidney and for ureteral stones). The treatment rate was 1.23 per patient. Auxiliary procedures before ESWL were performed in 7.9% of the sessions: insertion of "double-J" stent or nephrostomy tube; all ureteral stones received "in situ" treatment. Mean number of shock waves per session was 1836.1 (range 300-3020). The maximum voltage on the average reached was 17.1 kv. Sedo-analgesia by Diazepam and Ketoprofen was given in 75% of the cases. No serious complications were observed. The experience with this machine is discussed in regard to performance; the advantages of its fluoroscopy and ultrasound guided system of focusing are stressed.
