Abstract

Targeted therapeutic interventions utilizing low-inten\-sity ultrasound (LIUS) exhibit substantial potential for hindering the proliferation of cancer stem cells. This investigation introduces a multiscale model and computational framework to comprehensively explore the therapeutic LIUS on poroelastic tumor dynamics, thereby unraveling the intricacies of mechanotransduction mechanisms at play. Our model includes both macroscopic timescales encompassing days and rapid timescales spanning from microseconds to seconds, facilitating an in-depth comprehension of tumor behavior. We unveil the discerning suppression or reorientation of cancer cell proliferation and migration, enhancing a notable redistribution of cellular phases and stresses within the tumor microenvironment. Our findings defy existing paradigms by elucidating the impact of LIUS on cancer stem cell behavior. This endeavor advances our fundamental understanding of mechanotransduction phenomena in the context of LIUS therapy, thus underscoring its promising as a targeted therapeutic modality for cancer treatment. Furthermore, our results make a substantial contribution to the broader scientific community by shedding light on the intricate interplay between mechanical forces, cellular responses, and the spatiotemporal evolution of tumors. These insights hold the promising to promote a new perspective for the future development of pioneering and highly efficacious therapeutic strategies for combating cancer in a personalized manner.

Abstract

Mechanotherapy is a groundbreaking approach to impact carcinogenesis. Cells sense and respond to mechanical stimuli, translating them into biochemical signals in a process known as mechanotransduction. The impact of stress on tumor growth has been studied in the last three decades, and many papers highlight the role of mechanics as a critical self-inducer of tumor fate at the in vitro and in vivo biological levels. Meanwhile, mathematical models attempt to determine laws to reproduce tumor dynamics. This review discusses biological mechanotransduction mechanisms and mathematical-biomechanical models together. The aim is to provide a common framework for the different approaches that have emerged in the literature from the perspective of tumor avascularity and to provide insight into emerging mechanotherapies that have attracted interest in recent years. • This work discusses the therapeutic potential of some known mechanotransduction signaling pathways in cancer treatment. • The article describes and mechanically classifies the forces that are exerted on a growing tumor and its microenvironment. • We review avascular tumor growth models that account for mechanotransduction. • The review provides insights into emerging mechanotherapies that are gaining interest in recent years. Abstract

Background: Venous thromboembolism (VTE) is a major health problem, with over one million events every year in Europe. However, there is a paucity of data on the current management in real life, including factors influencing treatment pathways, patient satisfaction, quality of life (QoL), and utilization of health care resources and the corresponding costs. The PREFER in VTE registry has been designed to address this and to understand medical care and needs as well as potential gaps for improvement. Methods/design: The PREFER in VTE registry was a prospective, observational, multicenter study conducted in seven European countries including Austria, France Germany, Italy, Spain, Switzerland, and the UK to assess the characteristics and the management of patients with VTE, the use of health care resources, and to provide data to estimate the costs for 12 months treatment following a first-time and/or recurrent VTE diagnosed in hospitals or specialized or primary care centers. In addition, existing anticoagulant treatment patterns, patient pathways, clinical outcomes, treatment satisfaction, and health related QoL were documented. The centers were chosen to reflect the care environment in which patients with VTE are managed in each of the participating countries. Patients were eligible to be enrolled into the registry if they were at least 18 years old, had a symptomatic, objectively confirmed first time or recurrent acute VTE defined as either distal or proximal deep vein thrombosis, pulmonary embolism or both. After the baseline visit at the time of the acute VTE event, further follow-up documentations occurred at 1, 3, 6 and 12 months. Follow-up data was collected by either routinely scheduled visits or by telephone calls. Results: Overall, 381 centers participated, which enrolled 3,545 patients during an observational period of 1 year. Conclusion: The PREFER in VTE registry will provide valuable insights into the characteristics of patients with VTE and their acute and mid-term management, as well as into drug utilization and the use of health care resources in acute first-time and/or recurrent VTE across Europe in clinical practice. Abstract

Background: Venous thromboembolism (VTE) is a major health problem, with over one million events every year in Europe. However, there is a paucity of data on the current management in real life, including factors influencing treatment pathways, patient satisfaction, quality of life (QoL), and utilization of health care resources and the corresponding costs. The PREFER in VTE registry has been designed to address this and to understand medical care and needs as well as potential gaps for improvement. Methods/design: The PREFER in VTE registry was a prospective, observational, multicenter study conducted in seven European countries including Austria, France Germany, Italy, Spain, Switzerland, and the UK to assess the characteristics and the management of patients with VTE, the use of health care resources, and to provide data to estimate the costs for 12 months treatment following a first-time and/or recurrent VTE diagnosed in hospitals or specialized or primary care centers. In addition, existing anticoagulant treatment patterns, patient pathways, clinical outcomes, treatment satisfaction, and health related QoL were documented. The centers were chosen to reflect the care environment in which patients with VTE are managed in each of the participating countries. Patients were eligible to be enrolled into the registry if they were at least 18 years old, had a symptomatic, objectively confirmed first time or recurrent acute VTE defined as either distal or proximal deep vein thrombosis, pulmonary embolism or both. After the baseline visit at the time of the acute VTE event, further follow-up documentations occurred at 1, 3, 6 and 12 months. Follow-up data was collected by either routinely scheduled visits or by telephone calls. Results: Overall, 381 centers participated, which enrolled 3,545 patients during an observational period of 1 year. Abstract

Abstract: Fibre-reinforced composites are often selected for high-responsibility structural applications due to their high specific stiffness and strength, and corrosion resistance. These materials have a rather good rating as regards to life time in fatigue, but the same does not occur with the number of cycles to initial damage or with the evolution of damage. Due to material heterogeneity, random nature of fatigue loading and random environment conditions, the damage phenomenon is a cumulative evolutionary stochastic process in essence, characterized by changes in laminate compliance with time. As a consequence, it is more appropriately examined within a probabilistic framework. An approach that considers stochastic damage accumulation by means of discrete time Markov chains is proposed in this work to analyze damage evolution of composite laminates. Additionally a unitary time transformation by means of monotonic cubic Hermite splines has been introduced to take into account the nonstationary effects with a reduced set of model parameters. An inverse procedure is proposed to find the optimal stochastic model parameters together with the time transformation parameters.A numerical comparative study of the above-proposed technique is presented. Abstract

Abstract: Anticipate and characterize damages in layered carbon fiber-reinforced polymers (CFRP) is a challenging problem. Non-destructive evaluation using ultrasonic signals is a well-established method to obtain physically relevant parameters to characterize damages in isotropic homogeneous materials. However, due to their structural complexity, composites require special care in signal interpretation. Some enhancements on the inversion and interpretation are proposed by adapting classical signal processing techniques, including spectral analysis, homomorphic transformations, linear prediction, and by defining weighted residue in the inverse procedure scheme. This study aims to evaluate the feasibility of the signal-processing based inverse problem to reconstruct experimental data sets at different levels of damage, and to present a comparative study between the above-mentioned techniques. Abstract

Piezoelectric ceramics have recently become one of the most used materials in all kinds of electromechanical systems. However, the presence of defects in such materials prevents them from fulfilling their function. A number of numerical, analytical and experimental works are recently being developed to understand the behaviour of piezoelectrics with presence of damage, but very few aimed at locating defects. One of the current challenges in monitoring piezoelectrics is the correct interpretation of the readings from sensors, in order to reliably recover the defect characteristics minimizing uncertainties due to noise and model. An inverse problem strategy is proposed for this reconstruction, starting from the electromechanical response measurement as input data, and incorporating a numerical model that simulates that response. This model is solved using a Boundary Element Method (BEM), whose formulation is developed for the 2D static case. The damage identification inverse problem is solved using genetic algorithms for the minimization of the discrepancy or cost functional. The effect of noise on measurements and uncertainties in the model is studied in detail through a sensitivity analysis for some simple cases of defect.

Abstract

The understanding of changes in the viscoelastic properties of cervical tissue during the gestation process is a challenging problem. In this work, we explore the importance of considering the multilayer nature (epithelial and connective layers) of human cervical tissue for characterizing the viscoelastic parameters from torsional waves. For this purpose, torsional wave propagations are simulated in three multilayer cervical tissue models (pure elastic, Kelvin–Voigt (KV) and Maxwell) using the finite difference time domain method. High-speed camera measurements have been carried out in tissue-mimicking phantoms in order to obtain the boundary conditions of the numerical simulations. Finally, a parametric modeling study through a probabilistic inverse procedure was performed to rank the most plausible rheological model and to reconstruct the viscoelastic parameters. The procedure consist in comparing the experimental signals obtained in human cervical tissues using the Torsional Wave Elastography (TWE) technique with the synthetic signals from the numerical models. It is shown that the rheological model that best describes the nature of cervical tissue is the Kelvin–Voigt model. Once the most plausible model has been selected, the stiffness and viscosity parameters have been reconstructed of the epithelial and connective layers for the measurements of the 18 pregnant women, along with the thickness of the epithelial layer. Overlap exists between the risk factors for coronary artery disease and venous thromboembolism (VTE). However, a paucity of data is available on the incidence of major acute cardiovascular events (MACE) and major adverse limb events (MALE) among patients presenting with VTE. Moreover, it is unknown whether the rate of cardiovascular outcomes differs among patients with unprovoked vs provoked VTE.
