3. Ruptura forniksa bubrega kao posljedica opstruktivne uropatije – prikaz slučaja.

- Abstract
Aim: To present a case of a renal forniceal rupture as a result of obstructive ureterolithiasis and to give insight into diagnostic and treatment methods of this acute state. Case report: A 34-year-old female patient was admitted to the emergency department for a recurrent left-sided flank pain. The percussion of the left kidney was positive, and blood analysis showed increased creatinine levels and decreased estimated glomerular filtration rate. Aditionally, urine analysis was positive for leukocyte esterase and hemoglobin. Abdominal ultrasound showed grade 2 hydronephrosis and fluid collection in the left perirenal space which indicated forniceal rupture. Non-contrast CT images obtained through abdomen and pelvis revealed obstructive stone in the ureter and perirenal fluid. Post-contrast CT images confirmed the presence of water attenuation fluid around the kidney consistent with a forniceal rupture and urine leak. The patient received analgesics and antibiotics. The Double J-stent and urinary catheter were used to decrease renal pressure and to reduce symptoms. With proper instructions, the patient went home after a successful recovery and regression of symptoms. Two months later, the patient underwent a successful ureteroscopy with a stone extraction. The postoperative course was uneventful. Conclusion: The clinical presentation of renal forniceal rupture is non-specific. The most common cause is obstruction caused by stones, and imaging confirms the diagnosis. Drainage of the urinary tract is the first step of treatment, which can be conservative and/or surgical. El propósito de este estudio fue mostrar la dinámica de reconfiguración de la identidad en docentes universitarios a partir de sus experiencias de vida en el contexto del COVID-19. Teóricamente, se realizó una intersección entre la perspectiva de la psicología cultural semiótica y el abordaje de la teoría del self dialógico. Metodológicamente, esta fue una investigación cualitativa idiográfica con tres docentes universitarios participantes, quienes, a través de la entrevista en profundidad y la técnica de la foto-palabra, mostraron la dinámica de cambios que trajo consigo la pandemia. Si bien los tres casos reflejaron sus experiencias particulares, fue interesante reconocer que en todos se crearon significados comunes sobre la pandemia y el estallido social como parte fundamental de las vivencias. Los resultados llevaron a reconocer cómo las rupturas y las transiciones permiten comprender los ajustes psicológicos entre la persona y su entorno sociocultural.

Introduction. Unlike traumatic rupture of the spleen, which is the most common consequence of blunt abdominal trauma and is well documented in the literature, and unlike the atraumatic rupture of the spleen, which is less common but also well described, spontaneous rupture of the spleen in a patient with malaria is a rare complication, with a potentially fatal outcome. Case report. A soldier aged 46 years, was hospitalized, with the clinical picture of abdominal colic and diarrhea, previously treated for a primary attack of acute malaria. During hospitalization, deterioration of health condition occurred, with the development of a clinical picture of an acute abdomen due to spontaneous rupture of the spleen which was successfully resolved by splenectomy. Conclusion. Splenectomy is the method of choice for patients with unstable clinical presentation due to atraumatic rupture of the spleen. [ABSTRACT FROM AUTHOR]

- Abstract
Esophageal perforation is a rare but important complication of the modern neonatal intensive care setting. Female patient of 31 weeks gestation is presented. Appropriate weight for gestational age. Respiratory distress syndrome secondary to hyaline membrane, early neonatal sepsis. At 24 hours of life she presented respiratory deterioration, thoracoabdominal radiography was performed showing left pneumothorax. He required endotracheal intubation and chest tube for the management of the pneumothorax. At 4 days of extrauterine life in phase III of ventilation, with pleural tube, the control thoracoabdominal radiography shows: anomalous trajectory of the polyvinyl orogastric tube towards the left pleural cavity, the contrasted study and direct exploration with laryngoscopy confirms esophageal perforation in the upper third of the esophagus. The patient was kept fasting for 6 days, initiating enteral trophic enteral stimulation by orogastric tube, every 3 hours, gradually increasing until nutritional requirements were reached, He is discharged due to improvement at 41 years of age. Iatrogenic causes of esophageal perforation include: difficult tracheal intubation, Non-surgical treatment is the first option in esophageal perforation. Pharyngoesophageal perforation occurs after traumatic placement of a polyvinyl feeding tube. The use of a silastic feeding tube instead of polyvinyl probes could probably prevent iatrogenic esophageal perforation. [ABSTRACT FROM AUTHOR]

This paper outlines the concept of helplessness through Freudian metapsychology and by doing so imply a new way of thinking about politics. Helplessness is not conceived simply as the newborn's vulnerability, but also as the lack of response to internal excitements. Such an estate is radicalized by Freud's concept of death drive, which generates processes of collapsing, ruptures, unplugging and dispossession that are essential for turning life complex. In this sense, we propose helplessness as a main political affection when referring to transformation. By the collapse of oneself and the radical opening to contact with others, one may experience other ways of experiencing and of being affected and, consequently, the construction of political bodies leading to the radical transformation of politics. We of the modern world tell stories about being modern, becoming modern. We ask where modernity is going. Two metaphorical complexes dominate these stories: We favour metaphors of life and growth; modernity has a life of its own. Or we prefer metaphors of motion and direction; modernity is a journey that takes many paths. The two complexes coexist uneasily even as they feed on each other; together they mark the modern conceit that modernity has left tradition behind. Those whom modern society has victimized, uprooted or abandoned may resist both complexes, often seeking to retrieve the metaphors of a lost, broken, misremembered or invented past. Most beneficiaries of modernity favour the metaphorical complex of life and growth--or merely take it for granted. Scholars with a critical attitude toward modernity often favour many paths and thus the metaphorical complex of motion and direction--without realizing it. Seven metaphors reveal these tendencies: boundary, break, juncture, limit, rupture, stage, transition. They also hint at a third, distinctively modern metaphorical complex. In our stories about modernity, we deploy plural versions of spatial metaphors sequenced in time: frames, boxes, compartments, or containers, and mark the sequence with metaphorical signposts: age, stage, wave, or period. 17. Utilización de ligamento sintético para resolución de ruptura del tendón de Aquiles de paciente canino

El objetivo de este trabajo es presentar la utilización de un ligamento sintético para reparación de ruptura crónica del tendón de Aquiles. Se presenta un caso de un canino adulto, mestizo, de talla grande, con apoyo plantígrado del miembro posterior derecho. Luego de realizar una exploración física detallada de la zona, correlacionar con anamnesis y toma de placas radiográficas para descartar compromiso óseo en la lesión se arriba al diagnóstico de ruptura del tendón de Aquiles. Se decide instaurar tratamiento quirúrgico utilizando un ligamento sintético biocompatible junto con un sistema de fijación externa para lograr restituir la funcionalidad del miembro.

Objetivos: descrever o tratamento cirúrgico de uma ruptura de tendão distal do bíceps braquial em membro não dominante, que evoluiu satisfatoriamente sem complicações relacionadas ao sítio cirúrgico, lesão neurovascular ou ossificação heterotópica, mostrando que esta pode ser uma boa alternativa para o reparo destas lesões. Metodologia: relatamos o caso de um paciente masculino, 37 anos, portador de uma ruptura crônica de tendão distal do bíceps braquial, após trauma típico em membro não dominante, e que foi submetido a reparação da lesão com enxerto tricipital e fixação com túneis transósseos. Resultados: após o primeiro mês de pós-operatório, o paciente se encontrava com arco de movimento mantido, força de flexão do cotovelo e supinação do antebraço preservadas, sem déficit neurológico. Conclusão: Apesar de não haver consenso sobre a melhor forma de tratamento dessa ruptura, a abordagem cirúrgica com a técnica apresentada mostra-se como uma vantajosa opção terapêutica para esse tipo de lesão, visto os excelentes resultados clínicos obtidos no seguimento pós-operatório.

Background: Non-marital romantic relationship dissolutions (RRDs) are common among emerging adult students (EAS) and may result in severe distress and suicidality. However, studies on RRDs in youth are limited to mental health sequelae of depression and prolonged grief. Little is known about the association between RRDs and posttraumatic stress symptoms (PTSS), and how this compares to posttraumatic stress symptoms following a traumatogenic event. Objective: We aimed to determine the association between RRDs and PTSS in an EAS sample; and how this compared to the association between posttraumatic stress symptoms and a Diagnostic and Statistical Manual 5th edition (DSM-5) traumatic event. Method: University students (N = 2,022; female = 71.1%; 18–25 years) completed a demographic and relationship questionnaire, the Life Events Checklist, the Adverse Childhood Experiences questionnaire, and the Posttraumatic Stress Checklist (PCL). We compared EAS with an RRD (n = 886) or a DSM-5 criterion A traumatic event (n = 592) against a control group (n = 544) exposed to a non-traumatic stressful life event. Utilising ANOVAs and Pearson's correlations we determined demographic and clinical variables associated with PTSS. ANCOVA and stepwise hierarchical regression analyses were used to determine between-group differences in PTSS. Results: Total trauma exposure and adverse childhood experiences, sex, monthly income, sexual orientation, and attachment style were significantly associated with PTSS. The RRD group had significantly higher PCL scores compared to the DSM and control groups. The mean PCL scores for both the RRD and DSM groups were above the cut-off score of 33, consistent with a probable posttraumatic stress disorder diagnosis. Significantly more RRD participants (72.9%) scored above the cut-off score of 33 than DSM-5 Trauma Group participants (55.4%). Conclusion: An RRD is a potentially traumatic event and is significantly associated with PTSS, similar to a posttraumatic stress disorder diagnosis. Students may experience non-marital romantic relationship breakups as very painful, with various negative psychological, physical, and academic consequences. However, their painful experiences are often trivialised as something to 'just get over' and not acknowledged as potentially traumatic. Yet, using an attachment theory framework, breakups can be reconceptualised as subjectively traumatic. We explored the differences in posttraumatic stress symptoms of students who experienced a breakup, a traumatic event (as defined in psychiatry), and other non-traumatic life events. Students who experienced a breakup reported significantly highly posttraumatic stress symptoms. Acknowledging students' pain following a self-defined traumatic breakup can increase help-seeking behaviour and reduce the risk of intergenerational attachment trauma. O gerenciamento de risco de barragens identifica, avalia, trata e monitora riscos, tendo como objetivo minimizar ou mesmo eliminar a possibilidade de impactos negativos desse tipo de empreendimento na vida de pessoas e do meio ambiente. Neste estudo, uma revisão bibliográfica analítica foi realizada para identificar a classificação dos riscos das barragens do Paraná, Brasil, com base em uma análise quantitativa e qualitativa desses riscos e na análise do gerenciamento de segurança dessas barragens. Nas diversas literaturas encontradas, foram verificadas informações atualizadas sobre: fiscalização e classificação das barragens paranaenses realizadas por órgãos como o Instituto de Água e Terra (IAT), a Agência Nacional de Águas (ANA), a Agência Nacional de Energia Elétrica (ANEEL) e a Agência Nacional de Mineração (ANM); a existência de diversos métodos quantitativos e qualitativos que podem ser utilizados para determinar falhas em barragens, sendo a Análise dos Modos de Falha e seus Efeitos (Failure Mode and Effect Analysis – FMEA) a técnica que se destaca; e a identificação de práticas adequadas para um gerenciamento eficaz. Essa revisão possibilitou concluir que a identificação, classificação e fiscalização das barragens no estado do Paraná são realizadas pelo IAT, que apresenta relatórios anuais das condições das barragens para conhecimento público e segurança da população. Além disso, pode-se concluir que a aplicação de uma metodologia apropriada para análise de riscos é a forma mais adequada de prevenir uma catástrofe com possíveis perdas de vida.

Introduction: the mortality and morbidity rate due to abdominal aortic aneurysm rupture (RAAA) still remains high. In order to improve the final outcome surgical decision-making involves quickly evaluating. To enhance the objectivity and efficacy of surgical treatment, a prediction of RAAA outcomes is crucial. Several models have been developed to predict death after intervention in patients with an RAAA. Glasgow Aneurysm Score is one of the most commonly used models. Aim: to evaluate the accuracy of the most frequently used GAS system in predicting mortality and its practical use in clinical decision-making. Materials and methods: a retrospective, comparative cohort study was conducted in all 86 consecutive surgically treated patients with an RAAA in the Clinic of Cardiovascular Surgery of the Clinical Center University of Sarajevo in the four years period, from June 2019 to June 2023. Glasgow Aneurysm Score (GAS) was calculated and compared with final score. Results: the results show that total mortality rate was 23.3%, the mean GAS for the 86 patients who underwent surgery was 100.33 +/- 17.57. No statistically significant difference in GAS values was found between patients who die d and patients who survived surgery and in the 30-day postoperative period. Conclusion: our research has shed light on the limitations of existing scoring systems, including the Glasgow Aneurysm Score system. [ABSTRACT FROM AUTHOR]

In 2006 Andrew Coppola introduced the Q-Abstract Elementary Class concept as a generalization of the class of models of a sentence in the logic Lω1, ω (Q) [5]. We present the RUPTURA code () as a free and open-source software package (MIT license) for (1) the simulation of gas adsorption breakthrough curves, (2) mixture prediction using methods like the Ideal Adsorption Solution Theory (IAST), segregated-IAST and explicit isotherm models, and (3) fitting of isotherm models on computed or measured adsorption isotherm data. The combination with the RASPA software enables computation of breakthrough curves directly from adsorption simulations in the grand-canonical ensemble. RUPTURA and RASPA have similar input styles. IAST is implemented near machine precision but we also provide several explicit mixture prediction methods that are non-iterative and potentially faster than IAST. The code supports a wide variety of isotherm models like Langmuir, Anti-Langmuir, BET, Henry, Freundlich, Sips, Langmuir-Freundlich, Redlich-Peterson, Toth, Unilan, O'Brian & Myers, Asymptotic Temkin, and Bingel & Walton. The isotherm model parameters can easily be obtained by the fitting module. Breakthrough plots and animations of the column properties are automatically generated. In addition to highlighting the code, we also review all the developed techniques from literature for mixture prediction, breakthrough simulations, and isotherm model fitting, and provide a tutorial discussing the workflows. The loss of a child is an event that starts a lengthy and painful process of grief to a family. When it comes to stillborn children, the studies of the mourning process of the ones who expected eagerly for a birth are still very scarce. The death of a baby and the break of positive expectations related to the pregnancy give place to a specific mourning process. Thus, this article had the intention to discuss the real and symbolic grief of a mother from the loss of a stillborn child, using semiotic-cultural psychology as theoretical framework. A multiple case study applying the narrative interview method was used in two attendees. The participants needed to fulfill the following criteria: have been a mother of a stillborn and passed by this experience by, at least, a year ago. This amount of time was established from studies that pointed this period as the most stressful for the parents. The results support the main findings of the literature reviews which state that this mourning process has its own peculiarities, and occurs in the real and symbolic spheres. The speech of the interviewed showed that the no recognition of the grief by social environment and the lack of institutional support were the main factors that turned the experience even more painful. However, the therapy was an important resource to help them handle the loss. Further studies are suggested that also include the narratives of parents and the support network regarding the experience of having a stillborn baby, in order to deepen the repercussions of the loss for the entire context that involves it. 28. Acute kidney injury after open surgical treatment of ruptured aortic aneurysm.

Introduction: the research is concerned with acute renal insufficiency (AKI) occurring after the rupture of an abdominal aortic aneurysm. Aim: to determine the incidence rate of AKI in the total sample, and the two groups comparatively, and to determine independent predictors of AKI. Materials and methods: the sample consisted of 43 patients surgically treated at the Clinic of Cardiovascular Surgery of the Clinical Center University of Sarajevo in the period from 1 January 2021 to 3 1 March 2023, due to an abdominalaortic aneurysm rupture. The research was designed as an observational prospective analysis modeled on a typical cohort study. The sample was divided into two groups by the criterion determining the degree of hemorrhagic shock. Results: the incidence of survival after aortic trunk reconstruction due to abdominal aortic aneurysm rupture was 67.4%. In the total sample, 79.16% of patients had at least one form of AKI. A statistically significant disproportion between the groups is evident for AKI stage 0, as there were no patients without renal symptoms in Group 2 (p=0.004). Patients with hypertension more often showed severe types of hemorrhagic shock and consequent AKI (p<0.001). Preoperatively elevated values of serum creatinine denote a significant predictor of the occurrence of AKI (p<0.046). Conclusion: the occurrence of AKI after urgent surgical reconstruction of the aorta due to aneurysm rupture is a significantly frequent problem. The research determined intraoperative hemodynamic instability, blood loss exceeding 1500ml, and preoperative elevated serum creatinine levels as risk factors for postoperative AKI. Preoperatively elevated serum creatinine level is also a safe independent predictor of acute kidney function disorder in the postoperative period. [ABSTRACT FROM AUTHOR]

Kidney tumors in childhood and adolescence represent 7% of neoplasias. Its incidence is recorded between two and four years of age, with 80% of cases presenting before five years of age, and it rarely appears after 10 years of age, although cases have even been described in adults. The clinical manifestations presented by children with Wilms tumor are: palpable abdominal mass (75%), abdominal pain (28%), hypertension (26%), gross hematuria (18%), microscopic hematuria (24%), and fever (22%). Below is the case of a 13-year-old female patient with acute abdominal pain, associated with the presence of a palpable mass on the left flank; who presented tumor rupture and was approached as an oncological emergency, the need to consider it as a differential diagnosis in the presence of acute-onset abdominal pain in a pediatric patient stands out. Feridas e lacerações são bem comuns em equinos. Destas, a ruptura de tendões, sobretudo o tendão digital extensor, é uma das formas que muito acarreta preocupações pois, compromete a locomoção do animal além de poder, na maioria dos casos, causar a hiper flexão permanente ou intermitente do membro envolvido. Assim, devem ser bem tratadas e medicadas de acordo com cada fase do processo de cicatrização. Logo o objetivo do trabalho é relatar o caso de uma potra que teve uma laceração do tendão extensor digital comum, mostrando o manejo e métodos que foram utilizados para seu tratamento e reabilitação. Além disso, procura descrever os procedimentos utilizados para tratamento da ruptura total do tendão extensor digital comum devido à queimadura por corda; atentando-se para a anatomia do membro pélvico. Para fundamentar a temática tratada neste estudo buscou-se por uma revisão de literatura acerca da anatomia dos tendões em equinos, a ruptura do tendão extensor digital assim como o diagnóstico e tratamento do animal comprometido pela ruptura. A pesquisa possui caráter qualitativo ao descrever os procedimentos utilizados desde o contato com o animal até sua reabilitação estruturados pelo estudo de caso. O material consultado baseou-se em livros, artigos científicos, consultados em sua maioria, na internet em sites confiáveis como revistas universitárias e manuais técnicos. Pelo relato de caso foi possível observar desde o atendimento inicial da potra, com vista ao diagnóstico e tratamento até sua reabilitação. Percebeu-se que o exame físico, nesse caso, é a melhor forma para diagnosticar a laceração do tendão digital extensor e as formas de tratamento variam, sendo o uso do gesso e caminhadas controladas, alternativas positivas para a reabilitação. Ao final do estudo foi possível concluir que os procedimentos adotados foram satisfatórios para cicatrização da ferida e reabilitação da potra.

El presente texto señala algunas de las categorías fundamentales de la crítica de arte de Octavio Paz entre 1950 y 1967 como aparición y poesía, para demostrar que la noción de “ruptura” se retoma en la década de los sesenta para realizar un cuestionamiento profundo a la modernidad mexicana, sugiriendo una teoría de la posmodernidad y de la neovanguardia local. Además del texto llamado “Tamayo en la pintura mexicana” (1950), se hará referencia a otros cinco ensayos escritos en la década del sesenta, en los que se puede encontrar un programa y una conceptualización de su crítica, sustentada en lecturas que realiza de Kant, Heidegger y del estructuralismo, entre otros, cuya influencia es evidente. Además, se señalará que la crítica de arte de Paz es constitutiva de su pensamiento general.

Las rupturas de pareja han sido objeto de interés recurrente, desde una perspectiva sociológica, para la comunidad científica. Sin embargo, hasta el momento, el foco de atención se ha situado en las rupturas con vínculos legales, ya sean separación o divorcio y se ha estudiado a nivel nacional. El objetivo de esta investigación es analizar cómo es la ruptura de todo tipo de parejas y las razones que las motivan, centrándonos en el caso Andaluz. Para ello se utiliza metodología cuantitativa descriptiva mediante la explotación de datos procedentes de la Encuesta Social General Española (ESGE, 2018). Los resultados muestran como la mayor parte de las rupturas que se producen en Andalucía no se insertan en las modalidades de divorcio o separación, existiendo ligeras diferencias según el componente etario. En cuanto a las motivaciones para poner fin a una relación se encuentran ligadas a factores de tipo subjetivo, apreciando sutiles diferencias según sexo y la iniciativa suele tomarla, en general, una parte de la pareja.

From 1959 to 1968, Juan García Ponce published some of his most important art criticism, as well as collections of essays, in a context of the tension between the so-called Mexican School of Painting and young abstract painters such as Manuel Felguérez and Vicente Rojo; tension that became visible due to the Inter-American Biennials of 58 and 60, the Esso Salon of 1965, and the exhibition Confrontación 66. In his critiques, García Ponce moves to develop fundamental concepts for his writing such as abstraction, rupture, people, and culture, many of them based on ideas from authors such as Worringer, Marcuse, and Bataille. In this article I propose to show that from these concepts and authors, the writer raises an idea of rupture that will lead to a critique of modernity in Mexico, in order to propose a theory of criticism based on artistic freedom as a necessity in all creation. 39. Osteotomia niveladora do platô tibial para tratamento de ruptura do ligamento cruzado cranial: Relato de caso

O objetivo deste trabalho foi apresentar um caso de ruptura do ligamento cruzado cranial (RLCCr) um cão SRD de 5 anos, pesando 29 quilos, com histórico de claudicação do MPE (membro pélvico esquerdo), tratado cirurgicamente através da técnica de osteotomia niveladora do platô tibial (TPLO). Ao exame físico constatou-se que o animal não apresentava dor à palpação abdominal e que o membro afetado apresentou sensibilidade à palpação. Realizando-se o teste de compressão tibial e teste de gaveta, ambos positivos, diagnosticou-se então a presença de ruptura completa do ligamento cruzado cranial. Foi realizado tratamento cirúrgico pela técnica de TPLO, para redução do ângulo do platô tibial e estabilização da articulação. No pós-operatório foi indicado fisioterapia e acompanhamento quinzenal para avaliar retorno da função do membro.

La interposición de una denuncia o la ruptura de la relación son situaciones que van a influir en la dinámica relacional de una pareja, pudiendo hacer que aparezcan conductas violentas, incrementando dichas conductas, en caso de que ya existieran, o incluso dando lugar a violencia mortal. En este trabajo se analizan las distancias temporales entre (1) la primera denuncia, (2) la última denuncia, (3) la denuncia más reciente y (4) la ruptura de la relación con el feminicidio. Se emplearon dos submuestras, una formada por 40 feminicidios con denuncia previa; y otra con 76 feminicidios con ruptura. Los análisis mostraron que el tiempo entre denuncias y feminicidio es muy grande, aunque el nivel de riesgo de las víctimas, así como las conductas de control y violentas influyen en que pase más o menos tiempo. En el caso de la ruptura, parece existir una conexión más clara entre esta y el feminicidio, ya que en más de la mitad de los casos pasan menos de tres meses hasta que se acaba con la vida de la víctima, influyendo, también, la existencia de conductas de control y violentas.

O sistema urinário de cães e gatos podem ser acometidos por diversas patologias, das quais umas das mais frequentes está a ruptura da vesícula urinaria, que pode ser ocasionada por diversos fatores, o qual destaca-se os traumáticos como mais frequentes, como consequência do extravasamento da urina para a cavidade, diversos sinais clínicos são observados, sendo que quando não tratados, assim como a causa primaria, podem levar o paciente a óbito. O diagnóstico para tal enfermidade vai desde os exames físicos aos complementares e tem como tratamento definitivo a cistorrafia. O presente trabalho relata um caso de uma ruptura de bexiga em cão ocasionada por um trauma automobilístico, o paciente em questão após o acidente, foi atendido em clínica particular, onde foi diagnosticado com tal enfermidade mediante exames físicos, em seguida foi encaminhado para o hospital veterinário universitário da universidade federal do Piauí para que fosse submetido ao procedimento cirúrgico de cistorrafia, todos os procedimentos adotados bem como a experiência do cirurgião foram de grande importância para a recuperação do paciente, que hoje se encontra totalmente recuperado.

- Abstract
El stent es un dispositivo que tiene como función mantener permeable un conducto generalmente vaso sanguíneo. Su estructura en la mayoría de los casos es metálica y con estructura de mallado que le confiere ciertas propiedades en lo que respecta a su adaptabilidad y en muchos de ellos flexibilidad. Las estructuras biológicas no son estáticas, por una parte, por estar compuestas por tejidos vivos que mantienen una serie de cambios en consonancia con el organismo que forman parte y por otro, hay estructuras y muy especialmente las del aparato locomotor que sufren modificaciones en su posicionamiento. El stent puede sufrir por los cambios del lugar de implantación modificaciones y una muy relevante puede ser la ruptura del mismo, que condicionaría su función. Esta posibilidad es lo que ha marcado su diseño con el fin de que esta posibilidad no tenga lugar. Por otro lado, el sector arterial fémoro-popliteo es un segmento vascular donde la arteria por los movimientos del miembro inferior sufre diversos cambios que van desde la elongación, retracción, torsión, plicatura a la rotación, entre otros que le muestran como un segmento crítico y muy válido, para la valoración en la valoración que nos ocupa de los stent intravasculares. Se ha realizado un estudio para valorar las posibles fracturas de stent de diferentes tipos de dispositivos, incluyéndose los más utilizado en la práctica clínica. Se analizan diferentes aspectos y se sacan conclusiones con objeto de tener conocimiento de sus propiedades para su aplicación práctica de la información obtenida., Grado en Ingeniería Biomédica

- Abstract
El objetivo de este trabajo consiste en incorporar lo monstruoso –estética y metafísicamente–a la mirada del ser humano. El monstruo representa la parte grotesca, trágica y ridícula de la realidad. Y el hombre vive en un intento constante de ocultarla, tratando de ser lo que no es, en una inútil pretensión de aparentar perfección, cuando su esencia es caótica y absurda. Esta parte sombría de la vida genera una ruptura existencial, puesto que perturba e incomoda como abrir los ojos ante un abismo: uno se queda sin palabras, sin certezas a las que agarrarse. Pero, si el humano pretende vivir con profundidad, ha de valorar la suciedad que habita en el mundo, ha de apreciar la belleza que emerge de lo monstruoso: la belleza trascendental, The aim of this work is to incorporate the monstrous –aesthetically and metaphysically– into the gaze of the human being. The monster represents the grotesque, tragic, and ridiculous nstant attempt to hide it, trying to be what he is not, in a useless pretense of appearing perfect, when his essence is chaotic and absurd. This dark part of life generates an existential rupture, since it disturbs and uncomforts like opening one's eyes to an abyss: one is left without words, without certainties to hold on to. 48. Jeringas de mayor versus menor calibre para prevenir ruptura del PICC en neonatos: una revisión sistemática

Objetivo: Comparar efectividad del uso de jeringas de mayor calibre versus menor calibre, para prevenir la ruptura del PICC (Catéter Central de Inserción Periférica), en la UCIN (Unidad de Cuidados Intensivos Neonatales), a través de una revisión sistemática. Metodología: Revisión sistemática, con base a la declaración PRISMA. Bases de datos consultadas: SciELO, LILACS, Dialnet, PubMed, Elsevier, BVS. Se consideraron estudios en neonatos de UCIN con PICC instalados, complicaciones y recomendaciones de uso (desde 1998 hasta el 2016). Lectura crítica realizada con herramienta CASPe. Grado de evidencia y recomendación asignados por criterios Oxford. Resultados: Seis artículos describen como principales complicaciones del PICC, la obstrucción, ruptura e infección. Algunas recomendaciones para su mantenimiento se relacionan con el calibre de las jeringas. Conclusión: La ruptura, es una de las principales complicaciones durante el uso y manejo del PICC; asimismo, existen recomendaciones específicas, como el uso de jeringas de mayor calibre (por encima de los 5 cc), para evitar presiones excesivas en el catéter.

Se presentan dos observaciones de equinococosis peritoneal primitiva heterotópica poco frecuentes. En una de ellas, por el volumen del parásito parido al peritoneo que simulaba una ascitis. La otra, por tratarse de la ruptura de un quiste hidático de bazo que provocó una hemorragia peritoneal importante y el parto de la hidátide sana. La hemorragia quedó enquistada en la región subfrénica izquierda con la membrana indemne en el interior del hematocele.

50. Complication after the reconstruction of the old patellar tendon rupture.

Introduction. Chronic patellar tendon rupture (PTR) occurs rarely; its frequency and prevalence are unknown. There are very little data on the late patellar tendon reconstruction in rheumatoid arthritis and its complications. Case report. We presented a surgical repair of a PTR with early postoperative rupture of the contralateral patellar tendon for a 21-year-old woman with a past medical history of juvenile rheumatoid arthritis (treated with corticosteroids) who sustained initial injury 11 months prior to the presentation. The contralateral side was used for autograft harvesting. We used bone-tendon-bone (BTB) autograft and allografts followed by double-wire loop reinforcement and immediate postoperative mobilization. The patient was followed for 2 years, and the function of both knees was restored completely, with a full active range of motion. In this case, reconstruction of an 11-month-old chronic PTR (with complete resorption of the tendon and completely separated infrapatellar pads, complicated by the contralateral PTR) with BTB autograft and allografts and double wire loop reinforcement gave an excellent functional result. Two years after the surgical treatment, the extensor function of both knees was completely restored with a full range of movements. The patient reported satisfying outcomes and was able to return to all pre-injury activities without the assistance of orthopedic devices. Conclusion. This case report highlights the importance of the early diagnosis and describes operative techniques used in chronic PTR repair and treatments of the early postoperative complications such as rupture of the contralateral tendon.
