Problems related to equine large colon sand accumulations have been known for at least a century, but evidence-based medicine on the subject remains scarce. The current medical treatments are largely based on case reports and small studies on research horses with artificial sand accumulation or without confirmed accumulation. Therefore, clinical studies on naturally acquired sand accumulation are needed. The main objectives of the present studies were to define clinical signs related to the presence and size of large colon sand accumulations and to determine the optimal way to medically treat horses with these accumulations. A questionnaire study was performed to determine whether certain clinical signs are linked to the amount of sand in the colon and to clarify associations between equine large colon sand accumulations and individual traits and management-related factors. Owners or caretakers of 447 horses that had been radiographed for suspected large colon sand accumulation, responded to an internet based survey on clinical signs and management of their horses. The most common clinical signs of large colon sand accumulation noted by the owner were colic, poor performance and changes in faecal consistency (diarrhoea, increased faecal water). Colic (odds ratio (OR) 2.03, confidence interval (CI) 1.42-2.91) and poor performance (OR 1.89, CI 1.33-2.68) alone were associated with the size of sand accumulation. Horses with combinations of clinical signs, such as diarrhoea and poor performance or colic and diarrhoea also had a higher probability of a large sand accumulation; OR 2.12, CI 1.42-3.44 for diarrhoea and poor performance, and OR 2.64, CI 1.69-4.13 for colic and diarrhoea. Dominant position in the herd was protective for large colon sand accumulation (Spearman rho 0.170, P=0.002), while greediness predisposed the horse to the condition (Spearman rho 0.184, P

This paper explores the value of a research-based student inventory from the quality assurance point of view in two culturally different European higher education institutions for veterinary education. Perceived heavy workload is a well-known problem in veterinary studies and is a challenge to the quality of learning. First- and third-year students in both institutions responded to an inventory consisting of items regarding their approaches to learning, self-efficacy, study workload and the teaching-learning environment. There were differences in students’ approaches to learning and perceived workload between the two institutions. In both contexts, the strongest predictor of the workload turned out to be the surface approach to learning. Self-efficacy showed a positive correlation with the deep approach to learning and organised studying. The strengths of the teaching-learning environment varied between the institutions. Moreover, the present study discusses how the gained information could be used in improving the teaching-learning environment and students’ learning.

This dissertation empirically explored interest as a motivational force in university studies, including the role it currently plays and possible ways of enhancing this role as a student motivator. The general research questions were as follows: 1) What role does interest play in university studies? 2) What explains academic success if studying is not based on interest? 3) How do different learning environments support or impede interest-based studying? Four empirical studies addressed these questions. Study 1 (n=536) compared first-year students explanations of their disciplinary choices in three fields: veterinary medicine, humanities and law. Study 2 (n=28) focused on the role of individual interest in the humanities and veterinary medicine, fields which are very different from each other as regards their nature of studying. Study 3 (n=52) explored veterinary students motivation and study practices in relation to their study success. Study 4 (n=16) explored veterinary students interest experience in individual lectures on a daily basis. By comparing different fields and focusing on one study field in more detail, it was possible to obtain a many-sided picture of the role of interest in different learning environments. Questionnaires and quantitative methods have often been used to measure interest in academic learning. The present work is based mostly on qualitative data, and qualitative methods were applied to add to the previous research. Study 1 explored students open-ended answers, and these provided a basis for the interviews in Study 2. Study 3 explored veterinary students portfolios in a longitudinal setting. For Study 4, a diary including both qualitative and quantitative measures was designed to capture veterinary students interest experience. Qualitative content analysis was applied in all four studies, but quantitative analyses were also added. The thesis showed that university students often explain their disciplinary choices in terms of interest. Because interest is related to high-quality learning, the students seemed to have a good foundation for successful studies. However, the learning environments did not always support interest-based studying; Time-management and coping skills were found to be more important than interest in terms of study success. The results also indicated that interest is not the only motivational variable behind university studies. For example, future goals are needed in order to complete a degree. Even so, the results clearly indicated that it would be worth supporting interest-based studying both in professionally and generally oriented study fields. This support is important not only to promote high-quality learning but also meaningful studying, student well-being, and life-long learning. Opettajat ja vanhemmat ovat tienneet pitkään, että oppilaat ovat kiinnostuneempia joistakin oppiaineista kuin toisista ja että mikäli kiinnostusta löytyy, oppiminenkin sujuu usein paremmin. Muutaman viimeisen vuosikymmenen aikana kiinnostuksen merkitystä on alettu aktiivisemmin tutkia erilaisissa akateemisissa ympäristöissä. Tulosten perusteella tutkijat ovat yhtä mieltä siitä, että kiinnostus on yhteydessä etenkin oppimisen laatuun. Se on oppimisen taustalla oleva voima, joka selittää opintomenestystä ja asiantuntijaksi kehittymistä. Yliopisto-opiskelijat selittävät usein koulutusalansa valintaa nimenomaan kiinnostuksella. Jos itse opiskelu pohjautuu kiinnostukseen, opiskelijoilla on hyvät lähtökohdat laadukkaaseen oppimiseen. PsM Johanna Mikkosen väitöskirja kuitenkin osoittaa, ettei yliopisto aina tue opiskelijoiden henkilökohtaiseen kiinnostukseen pohjautuvaa opiskelua. Väitöskirjan tutkimuskysymykset olivat: 1) Millainen rooli kiinnostuksella on yliopisto-opinnoissa? 2) Mikä selittää opinnoissa etenemistä ja menestymistä, jos opiskelu ei pohjaudu kiinnostukseen? ja 3) Miten erilaiset oppimisympäristöt tukevat kiinnostukseen pohjautuvaa opiskelua? Tutkimusta varten koottiin aineistoa Helsingin yliopiston eläinlääketieteellisen ja humanistisen tiedekunnan opiskelijoilta. Opiskelu näissä kahdessa tiedekunnassa on luonteeltaan hyvin erilaista: eläinlääketiedettä opiskellaan vuosikursseittain valmiin lukujärjestyksen pohjalta kun taas humanistisessa opiskelu on perinteisesti vapaampaa. Tulosten perusteella vahva henkilökohtainen kiinnostus lievitti sitä tulevaisuuteen liittyvää epävarmuutta, joka hankaloittaa humanistisen alan opiskelijoiden sitoutumista opintoihinsa. Eläinlääketieteessä opiskelijoilla puolestaan oli selkeä tavoite valmistua eläinlääkäriksi. Opintomenestystä selitti kuitenkin pikemminkin hyvä ajanhallinta ja sopeutuminen oppimisympäristöön kuin kiinnostus. Kova työmäärä ja valmiiksi pureskeltu opetusohjelma ohjasi opiskelijoita ulkokohtaiseen suorittamiseen. Mikkosen väitöskirjan tulokset osoittavat, että kiinnostuksen roolia yliopisto-opiskelussa on tärkeää tukea. Vaikka akateemiset tutkinnot eivät voi rakentua ainoastaan opiskelijoita henkilökohtaisesti kiinnostavista sisällöistä, tehokkuusvaatimukset eivät saisi johtaa ulkokohtaisen suorittamisen ylikorostumiseen. Hyvin kehittynyt henkilökohtainen kiinnostus on ominaista sellaisille akateemisille asiantuntijoille, jotka eivät ainoastaan tunne syvällisesti omaa alaansa vaan jotka kykenevät uuden tiedon tuottamiseen ja oman alansa rajojen rikkomiseen. Siksi kiinnostukseen pohjautuva opiskelu on tarpeen sekä ammattiorientoituneilla että yleisemmin orientoituneilla aloilla. Opiskelijoiden henkilökohtaisen kiinnostuksen kehittymistä voi tukea ohjaamalla heitä pohtimaan omia kiinnostuksen kohteitaan. Heille voi tarjota mahdollisuuksia omiin valintoihin ja aktiiviseen työskentelyyn kiinnostuksen kohteiden parissa. Tällaisen omaehtoisen opiskelun tukeminen ei ole tärkeää vain laadukkaan oppimisen näkökulmasta; kiinnostus liittyy myös siihen, mikä on opiskelijalle omakohtaisesti tärkeää ja merkityksellistä. Siksi kiinnostukseen pohjautuva opiskelu on yhteydessä mielekkääseen opiskeluun, opiskelijoiden hyvinvointiin ja elinikäiseen oppimiseen.

The aim of this dissertation was to explore factors which affect first-year law students study success. A modified Biggs s 3P model was used as the theoretical framework. The model includes both personal and contextual factors in student learning. The participants were first-year law students from the academic years 2005-2008, and the data were collected through interviews, open-ended question and electronic questionnaires. Study I explored first-year law students spontaneous descriptions of their learning activities at the beginning of their studies as well as fast study pace law students who had already finished their first year. Even though, law students are selected through a demanding entrance examination, some of the beginner students mentioned using only one or very few learning activities, which were mainly non-generative strategies. On the other hand, it was typical for the fast study pace students to mention generative strategies and elements of organised effort in studying. Study II analysed the relationship between approaches to learning and study success in terms of earned study credits and grade point average among first-year law students in three years. Organised students and students applying a deep approach earned the highest number of credits and the highest grades, whereas students applying a surface approach and unorganised students applying a deep approach received the lowest number of credits and the lowest grades. The study confirms previous findings that organised students constitute the largest cluster among first-year law students. Study III explored factors affecting the study pace of law students during their first academic year. The factors mentioned by the students were classified into four categories of self-regulation: motivation, behaviour, cognition and context. The group of fast study pace students turned out to have good skills in all areas of self-regulation. Respectively, the slow study pace group showed more individual variation, and had weaknesses in one or more areas of self-regulation. In addition, students experienced, that other activities such as working affected their study pace, this could be constitute a fifth category. However, the slow and fast study pace students felt differently about work. According to the slow study pace students, work impeded their studying because it took up too much time. For their part, the fast study pace students were able to allocate their time effectively and felt working to be useful and a counterbalanced to their studying, as well as an opportunity to apply knowledge in practice. Study IV analysed differences in law students perceptions of their teaching-learning environments after three learning periods. The students perceptions were compared with pharmacy and veterinary students perceptions of their teaching-learning environments. The results showed that the law students experienced their teaching-learning environment more negatively than the pharmacy and veterinary students. The law students experienced that alignment, teaching for understanding, staff enthusiasm and support, along with constructive feedback were areas that could be developed at the Faculty. Together the four studies indicate that both law students learning skills and the teaching-learning environment could be further developed. The results imply that managing in the demanding teaching-learning environment of law requires student to effectively employ qualitative learning activities: organised studying and a deep approach to learning and good self-regulation skills. In addition to student counselling, it is important for students study success to direct the teaching-learning environment towards a more learning-focused than content-focused approach to teaching. Väitöstutkimuksessa selvitettiin oikeustieteen ensimmäisen vuoden opiskelijoiden opintomenestykseen vaikuttavia tekijöitä. Tutkimuksen teoreettisena viitekehyksenä käytettiin muokattua Biggsin 3P-mallia, jossa oppimista tarkastellaan sekä opiskelijan henkilökohtaisten ominaisuuksien että opetus- ja opiskeluympäristön näkökulmasta. Kohdejoukkona olivat oikeustieteen ensimmäisen vuoden opiskelijat lukuvuosina 2005-2008, ja aineisto kerättiin haastatteluiden, kirjallisen vastausaineiston sekä sähköisen kyselylomakkeen avulla. Osatutkimus I tutki sekä opintonsa aloittavien että ensimmäisen vuoden opinnoissaan nopeasti edenneiden opiskelijoiden käyttämiä opiskelumenetelmiä. Tulokset osoittivat, että vaikka oikeustieteen opiskelijat valitaan tiedekuntaan vaativan valintakokeen kautta, niin osa opintonsa aloittavista opiskelijoista käytti vain yhtä tai hyvin rajattua määrää opiskelumenetelmiä, ja nämä olivat useimmiten tietoa toistavia strategioita. Sen sijaan nopeasti edenneet opiskelijat käyttivät pääsääntöisesti tietoa muokkaavia ja tehokkuuteen pyrkiviä opiskelustrategioita. Osatutkimus II analysoi kolmen peräkkäisen vuosikurssin opiskelijoiden käyttämien oppimisen lähestymistapojen yhteyttä ensimmäisen vuoden opintomenestykseen. Järjestelmällisten opiskelijoiden ja syväsuuntautunutta lähestymistapaa käyttävien opiskelijoiden opintomenestys, sekä opintopisteiden määrällä että arvosanojen keskiarvolla arvioiden, oli merkitsevästi parempi kuin pintasuuntautunutta ja ei-järjestelmällistä syväsuuntautunutta lähestymistapaa käyttävien opiskelijoiden. Tutkimus vahvisti aikaisemman tuloksen, jonka mukaan huomattava osa oikeustieteen opiskelijoista kuuluu järjestelmällisten opiskelijoiden ryhmään. Osatutkimus III tutki hitaasti ja nopeasti edenneiden opiskelijoiden opintojen etenemisnopeuteen vaikuttaneita syitä. Aineistosta nousi neljä itsesäätelytaitoja kuvaavaa luokkaa: motivaatio, kognitio, käyttäytyminen sekä opetus- ja opiskeluympäristö. Nopeasti edenneillä opiskelijoilla oli hyvät itsesäätelytaidot kaikilla neljällä itsesäätelyn alueella. Vastaavasti hitaasti edenneiden ryhmä oli heterogeenisempi, ja heillä oli puutteita yhdellä tai useammalla itsesäätelyalueella. Lisäksi aineistosta nousi esille viides luokka, muut syyt, johon sisältyi muun muassa työssäkäynti, mutta sen vaikutus näkyi eri tavalla hitaasti ja nopeasti edenneiden ryhmissä. Hitaasti edenneet opiskelijat nimesivät työssäkäynnin opintoja haittaavaksi tekijäksi, koska sen vuoksi heille ei jäänyt riittävästi aikaa opiskeluun. Vastaavasti nopeasti edenneet opiskelijat osasivat suunnitella ajankäyttönsä tehokkaasti ja kokivat työssäkäynnin hyödyllisenä vastapainona opiskelulle, ja työssäkäynti tarjosi heille mahdollisuuden opittujen tietojen soveltamiseen. Osatutkimus IV analysoi oikeustieteen ensimmäisen vuoden opiskelijoiden kokemuksia opetus- ja opiskeluympäristöstään kolmen opiskeluperiodin jälkeen. Näiden opiskelijoiden vastauksia verrattiin farmasian tiedekunnan ja eläinlääketieteellisen tiedekunnan opiskelijoiden vastauksiin. Oikeustieteen opiskelijat kokivat opetus- ja opiskeluympäristönsä negatiivisemmin kuin farmasian ja eläinlääketieteen opiskelijat. Opiskelijoiden kokemuksen mukaan oikeustieteellisessä tiedekunnassa on kehitettävää etenkin opetuksen linjakkuudessa ja ymmärtämistä tukevassa opetuksessa sekä opettajilta saatavassa tuessa ja palautteessa. Osatutkimukset osoittivat, että kehitettävää on sekä opiskelijoiden opiskelutaidoissa että opetus- ja opiskeluympäristössä. Tulosten mukaan vaativassa oikeustieteen opetus- ja opiskeluympäristössä menestyminen vaatii opiskelijalta kykyä käyttää useita laadukkaita opiskelumenetelmiä, järjestelmällisyyttä ja syväsuuntautuneiden oppimisen lähestymistapojen käyttöä sekä hyviä itsesäätelytaitoja. Opiskelijoiden opintomenestyksen kannalta on tärkeää, että ohjauksen tehostamisen lisäksi oikeustieteen opetus- ja opiskeluympäristössä panostetaan opetuksen suunnittelussa sisältöjen kattavuuden sijaan oppimislähtöiseen lähestymistapaan.

The primary aim of this thesis was the evaluation of the perfusion of normal organs in cats using contrast-enhanced ultrasound (CEUS), to serve as a reference for later clinical studies. Little is known of the use of CEUS in cats, especially regarding its safety and the effects of anesthesia on the procedure, thus, secondary aims here were to validate the quantitative analyzing method, to investigate the biological effects of CEUS on feline kidneys, and to assess the effect of anesthesia on splenic perfusion in cats undergoing CEUS. -- The studies were conducted on healthy, young, purpose-bred cats. CEUS of the liver, left kidney, spleen, pancreas, small intestine, and mesenteric lymph nodes was performed to characterize the normal perfusion of these organs on ten anesthetized, male cats. To validate the quantification method, the effects of placement and size of the region of interest (ROI) on perfusion parameters were investigated using CEUS: Three separate sets of ROIs were placed in the kidney cortex, varying in location, size, or depth. The biological effects of CEUS on feline kidneys were estimated by measuring urinary enzymatic activities, analyzing urinary specific gravity, pH, protein, creatinine, albumin, and sediment, and measuring plasma urea and creatinine concentrations before and after CEUS. Finally, the impact of anesthesia on contrast enhancement of the spleen was investigated by imaging cats with CEUS first awake and later under anesthesia on separate days. -- Typical perfusion patterns were found for each of the studied organs. The liver had a gradual and more heterogeneous perfusion pattern due to its dual blood flow and close proximity to the diaphragm. An obvious and statistically significant difference emerged in the perfusion between the kidney cortex and medulla. Enhancement in the spleen was very heterogeneous at the beginning of imaging, indicating focal dissimilarities in perfusion. No significant differences emerged in the perfusion parameters between the pancreas, small intestine, and mesenteric lymph nodes. -- The ROI placement and size were found to have an influence on the quantitative measurements of CEUS. Increasing the depth or the size of the ROI decreased the peak intensity value significantly, suggesting that where and how the ROI is placed does matter in quantitative analyses. --- A significant increase occurred in the urinary N-acetyl-β-D-glucosaminidase (NAG) to creatinine ratio after CEUS. No changes were noted in the serum biochemistry profile after CEUS, with the exception of a small decrease in blood urea concentration. The magnitude of the rise in the NAG/creatinine ratio was, however, less than the circadian variation reported earlier in healthy cats. Thus, the changes observed in the laboratory values after CEUS of the left kidney did not indicate any detrimental effects in kidneys. Heterogeneity of the spleen was observed to be less and time of first contrast appearance earlier in nonanesthetized cats than in anesthetized ones, suggesting that anesthesia increases heterogeneity of the feline spleen in CEUS. ---- In conclusion, the results suggest that CEUS can be used also in feline veterinary patients as an additional diagnostics aid. The perfusion patterns found in the imaged organs were typical and similar to those seen earlier in other species, with the exception of the heterogeneous perfusion pattern in the cat spleen. Differences in the perfusion between organs corresponded with physiology. Based on the results, estimation of focal perfusion defects of the spleen in cats should be performed with caution and after the disappearance of the initial heterogeneity, especially in anesthetized or sedated cats. Finally, these results indicate that CEUS can be used safely to analyze kidney perfusion also in cats. Future clinical studies are needed to evaluate the full potential of CEUS in feline medicine as a tool for diagnosing lesions in various organ systems. Väitöskirja käsittelee kissan vatsaontelon elinten (maksa, munuainen, perna, haima, ohutsuoli, suoliliepeen imusolmukkeet) ultraäänivarjoainekuvantamista. Ultraäänivarjoainekuvantaminen on diagnostinen tutkimusmenetelmä, jolla kuvannetaan vatsaontelon elimiä ja niiden kudostason verenkiertoa ja verenkierron muutoksia. Ultraäänivarjoainekuvantamista ei ole aikaisemmin systemaattisesti tutkittu kissoilla ja menetelmä on käytössä uusi myös muilla eläinlajeilla (lähinnä koira) ja ihmisillä Suomessa. Tutkimuksen avulla löydettiin tutkimusprotokolla kissapotilaille, ja varmennettiin menetelmän munuaisturvallisuutta. Lisäksi tutkimuksessa määritettiin rauhoituksen vaikutusta pernan ultraäänivarjoainekuvaukseen ja selvitettiin ns. normaaliarvot useiden vatsaontelon elimien verenkierrolle ultraäänivarjoainekuvauksessa. Tuloksien perusteella voidaan todeta menetelmän soveltuvan myös kissapotilaiden tutkimiseen ja olevan munuaisille turvallinen. Tulokset viittaavat kuitenkin siihen, että rauhoitettujen kissojen pernamuutosten diagnostiikka ei ole yksiselitteistä. Usein eläinlääkärin vastaanotolla tutkittavat kissapotilaat ovat vanhoja ja sairaita ja siksi niiden diagnostiikkaan tarvitaan turvallisia tutkimusmenetelmiä, jotka eivät vaaranna potilaiden terveydentilaa. Ultraäänivarjoainekuvantaminen on menetelmänä turvallinen, jopa munuaissairailla potilailla. Munuaissairaudet ja munuaisten toimintaa heikentävät muut sairaudet, kuten sokeritauti (diabetes) ovatkin yleisiä erityisesti vanhoilla kissapotilailla. Monet kissapotilaat joudutaan usein kevyesti rauhoittamaan eläinlääkärin suorittamien tutkimusten ajaksi ja siksi onkin tärkeää tuntea rauhoitusaineiden vaikutus ultraäänivarjoainekuvantamiseen. Ultraäänivarjoainekuvantamisessa potilaalle annostellaan laskimonsisäisesti mikrokuplina kaasua sisältävää, spesifisesti verisuonissa pysyvää varjoainetta. Varjoaineen avulla vahvistetaan verestä saatavia kaikuja huomattavasti useiden minuuttien ajaksi. Veren aiheuttama kaiku on liian heikkoa havaittavaksi perinteisellä ultraäänitutkimuksella, eli veri nähdään kuvassa kaiuttomana kuten muukin neste. Tavallisesti veren virtausta on ultraäänilaitteella tarkasteltu Doppler-tekniikan avulla. Dopplerilla ei kuitenkaan pystytä havaitsemaan virtauksia, joiden volyymi tai virtausnopeus on hyvin pieni. Ultraäänivarjoaineen avulla lisätään verestä saatavaa signaalin intensiteettiä niin paljon, että pienikin verenvirtaus kudoksissa kuten kapillaaritason mikroverenkierto on havaittavissa. Ultraäänivarjoaineiden avulla saadaan lisätietoa kudostason verenkierrosta. Erilaiset patologiset sairaustilat, kuten infarktit, tulehdusreaktiot ja kasvaimet vaikuttavat kudosten verenkiertoon eri lailla: muuttamalla kudoksiin menevää veren määrää, veren virtausta ja/tai verisuonitusta. Kudosten verenkiertoa tarkastelemalla pystytään paremmin havaitsemaan ja määrittelemään parenkyymin muutoksia. Esimerkiksi useimmat kasvaimet erottuvat muusta kudoksesta ultraäänivarjoainekuvauksessa poikkeavan verisuonituksensa ansiosta - pienten verisuonten määrän, laadun tai sijainnin mukaan.
