Gender-bias in COVID-19 severity has been suggested by clinical data. Experimental data in cell and animal models have demonstrated the role of sex hormones, particularly estrogens, in viral infections such as in COVID-19. SARS-CoV-2 uses ACE2 as a receptor to recognize host cells, and the protease TMPRSS2 for priming the Spike protein, facilitating virus entry into cells. However, the involvement of estrogenic receptors in SARS-CoV-2 infection are still being explored. Thus, in order to investigate the role of estrogen and its receptors in COVID-19, the estrogen receptors ERα, ERβ and GPER1 were overexpressed in bronchial BEAS-2B cell, and then infected with SARS-CoV-2. Interestingly, the levels of ACE2 and TMPRSS2 mRNA were higher in SARS-CoV-2-infected cells, but no difference was observed in cells with estrogen receptors overexpression. GPER1 can be involved in virus infection or replication, since its higher levels reduces SARS-CoV-2 load. On the other hand, pharmacological antagonism of GPER1 enhanced viral load. Those data suggest that GPER1 has an important role in SARS-CoV-2 infection.

This study evaluated SARS-CoV-2 replication in human cell lines derived from various tissues and investigated molecular mechanisms related to viral infection susceptibility and replication.SARS-CoV-2 replication in BEAS-2B and A549 (respiratory tract), HEK-293 T (kidney), HuH7 (liver), SH-SY5Y (brain), MCF7 (breast), Huvec (endothelial) and Caco-2 (intestine) was evaluated by RT-qPCR. Concomitantly, expression levels of ACE2 (Angiotensin Converting Enzyme) and TMPRSS2 were assessed through RT-qPCR and western blot. Proteins related to autophagy and mitochondrial metabolism were monitored in uninfected cells to characterize the cellular metabolism of each cell line. The effect of ACE2 overexpression on viral replication in pulmonary cells was also investigated.Our data show that HuH7, Caco-2 and MCF7 presented a higher viral load compared to the other cell lines. The increased susceptibility to SARS-CoV-2 infection seems to be associated not only with the differential levels of proteins intrinsically related to energetic metabolism, such as ATP synthase, citrate synthase, COX and NDUFS2 but also with the considerably higher TMPRSS2 mRNA expression. The two least susceptible cell types, BEAS-2B and A549, showed drastically increased SARS-CoV-2 replication capacity when ACE2 was overexpressed. These modified cell lines are relevant for studying SARS-CoV-2 replication in vitro.Our data not only reinforce that TMPRSS2 expression and cellular energy metabolism are important molecular mechanisms for SARS-CoV-2 infection and replication, but also indicate that HuH7, MCF7 and Caco-2 are suitable models for mechanistic studies of COVID-19. Moreover, pulmonary cells overexpressing ACE2 can be used to understand mechanisms associated with SARS-CoV-2 replication.

Simple Summary Medullary thyroid cancer (MTC) is often discovered in its advanced stage. Although a rare disease, advanced MTC cases have poor prognosis and the treatment is often palliative. Several studies have reported the existence of an association between copy number alterations (CNAs) burden and cancer progression. Moreover, the accumulation of broad CNAs, which contribute to intra-tumor heterogeneity, might be required for immune evasion. The identification of the recurrent CNAs associated with tumor phenotype aided in discovering new therapeutics options in several cancer types. To our knowledge, CNA is not well characterized in MTC. We analyzed recurrent focal CNAs on MTC. Our analysis provides a novel insight on MTC biology and may help in uncovering novel potential therapeutic targets. Abstract Medullary thyroid carcinoma (MTC) is a malignant tumor originating from thyroid C-cells that can occur either in sporadic (70–80%) or hereditary (20–30%) form. In this study we aimed to identify recurrent copy number alterations (CNA) that might be related to the pathogenesis or progression of MTC. We used Affymetrix SNP array 6.0 on MTC and paired-blood samples to identify CNA using PennCNV and Genotyping Console software. The algorithms identified recurrent copy number gains in chromosomes 15q, 10q, 14q and 22q in MTC, whereas 4q cumulated losses. Coding genes were identified within CNA regions. The quantitative PCR analysis performed in an independent series of MTCs (n = 51) confirmed focal recurrent copy number gains encompassing the DLK1 (14q32.2) and AIFM3 (22q11.21) genes. Immunohistochemistry confirmed AIFM3 and DLK1 expression in MTC cases, while no expression was found in normal thyroid tissues and few MTC samples were found with normal copy numbers. The functional relevance of CNA was also assessed by in silico analysis. CNA status correlated with protein expression (DLK1, p = 0.01), tumor size (DLK1, p = 0.04) and AJCC staging (AIFM3 p = 0.01 and DLK1 p = 0.05). These data provide a novel insight into MTC biology, and suggest a common CNA landscape, regardless of if it is sporadic or hereditary MTC.

The aim of this study was to investigate the influence of Bisphenol A (BPA) exposure on Neuroglobin (Ngb) and Cytoglobin (Cygb) as well as oxidative stress gene expression in the cerebellum, hippocampus, hypothalamus and cortex. Male Wistar rats were randomly divided into 3 groups: Control and two groups receiving 2 different daily BPA dosages, 5 or 25 mg/kg from postnatal day 50 (PND50) through PND90 and they were euthanized at PND105. In the cortex, we found an increase in Ngb gene expression and also in superoxide dismutase 1 and Catalase (Cat). In the cerebellum, we found an increase in Ngb and Cat, in the hypothalamus, there was a decrease in Cygb and an increase in glutathione peroxidase and Cat and in hypoxia-inducible factor 1 alpha (Hif1α) at the low dosage and a decrease in Hif1α at the high BPA dosage. Finally, in the hippocampus, we observed a decrease in Ngb and Cygb and an increase in Hif1α. In summary, BPA promotes the modulation of both Ngb and Cygb, but such changes occur by different mechanisms depending on the exposure dose and anatomical area.

To evaluate the risk of malignancy in thyroid nodules through clinical, laboratory, ultrasonographic and cytological aspects.741 nodules of 407 patients.The cytology was benign (60,5%), indeterminate (23,3%), malignant (8,3%) or nondiagnostic (7,6%). The prevalence of cancer in indeterminate citology was 18,5% (16% in follicular lesions, 44% in suspicious). The diagnosis of malignancy was 17,2% (n = 70). The frequency of cancer in women (15,2%) was lower than in men (27,9%). There was an inverse relation between age and cancer risk. There was no statistical significance in the prevalence of cancer according to number, size of nodules or TSH levels. Hypoechogenicity and microcalcifications on ultrasound were risk factors.The risk of malignancy was higher in men, hypoechoic nodules, with microcalcifications and was inversely related to age. The TSH level was not an independent factor predictive of malignancy.

INTRODUÇÃO: Não há tratamento ideal para o bócio multinodular (BMN). A cirurgia é o método mais utilizado, por proporcionar uma redução imediata do volume tiroidiano (VT), dos sintomas compressivos e permitir um diagnóstico histológico definitivo. O iodo radioativo (I¹³¹) é considerado uma excelente opção terapêutica em situações de contra-indicação cirúrgica ou recusa do paciente a este método. Entretanto doses elevadas de I¹³¹ são utilizadas para obtenção de resultados satisfatórios na redução do VT. Esta estratégia leva a uma maior exposição dos pacientes à radiação ionizante e a um maior custo devido à necessidade de internações. Administração prévia do TSH humano recombinante (rhTSH) ao I¹³¹ no tratamento do BMN, permite a utilização de doses menores deste radiofármaco, obtendo resultados satisfatórios na redução do VT e em menor tempo. Porém esta associação pode acentuar efeitos colaterais descritos neste tipo de terapia. OBJETIVOS: Avaliar a efetividade e segurança de pequenas e diferentes doses do estímulo prévio do rhTSH ao I¹³¹ em dose fixa, no tratamento do BMN. PACIENTES e MÉTODOS: Trinta portadores de BMN foram divididos em três grupos de 10 pacientes. Os pacientes do grupo I foram estimulados previamente com 0,1 mg de rhTSH ; Os pacientes do grupo II com 0,01 mg e o grupo III foi caracterizado como controle. Todos os participantes do estudo foram submetidos à mesma dose terapêutica de I¹³¹. As variáveis medidas foram: captação do I¹³¹ às 24 horas; VT e a menor área seccional transversal da traquéia (MATT) avaliados através da ressonância magnética (RM), antes, 2, 7, 180 e 360 dias, após o tratamento; dosagens seriadas de T3 total, T4 livre, TSH e anticorpos anti-tiroidianos. RESULTADOS: Após seis meses da terapia actínica houve redução no VT de 30,3 ± 16,5 % no grupo I, 22,6 ± 14,5 % no grupo II e 5,0 ± 14,6 % no grupo controle (p=0,01). Aos 12 meses de seguimento o VT reduziu 39,2 ± 16,9 % no grupo I, 38,8 ± 24,4% no grupo II e 23,4 ± 23,59 % no grupo controle (p=0,205). Houve elevação dos níveis dos hormônios tiroidianos e consequente redução das medidas de TSH nos primeiros trinta dias de maneira similar entre os grupos, porém em valores não considerados como tireotoxicose na maioria dos pacientes. Após doze meses do I¹³¹, oito pacientes desenvolveram hipotiroidismo, sendo três no grupo I, três no grupo II e dois no grupo placebo, não havendo diferença estatística entre os grupos. Os níveis de anticorpos antitireoglobulina (AATg) elevaram-se no sexto mês do estudo e retornaram aos níveis basais aos doze meses. Os níveis de anticorpos antitireoperoxidase (ATPO) foram mais elevados no grupo I que nos demais ao final do estudo. Não houve alteração significativa do VT e da MATT durante a primeira semana após o I¹³¹, em nenhum dos grupos estudados. CONCLUSÕES: O estímulo prévio do rhTSH a uma dose fixa de I¹³¹ levou a uma redução maior e mais significativa do bócio aos 6 meses de tratamento, que o uso isolado do I¹³¹. Após 12 meses esta tendência se manteve, entretanto sem significância estatística. BACKGROUND: There is not an optimal treatment for multinodular goiter (MNG). Surgery is the main therapeutic option because it decreases thyroid volume, reduces compression symptoms and provide histological diagnosis. Radioiodine (131I) is an efficient therapeutic option for the treatment of MNG mainly when surgery is not indicated or when the patient refused it. However, high activities of 131I are frequently required for clinically significant results. This procedure increases the body radiation exposure and the hospitalization costs. Recombinant human TSH (rhTSH) allows a reduction in the administered activity of ¹³¹ I with effective thyroid volume (TV) reduction. However, this combination therapeutic can increase collateral effects. OBJECTIVE: To evaluate the efficacy and safety of low and intermediate doses of rhTSH compared to placebo, associated with a fixed activity of 131I in MNG treatment. PATIENTS and METHODS: Thirty patients with MNG received 0.1 mg of rhTSH (group I, n=10), 0.01 mg of rhTSH (group II, n=10), or placebo (control group, n=10). After 24 hours, 30 mCi of 131I was given to all patients. Radioactive iodine uptake (RAIU) was determined before and 24 hours after rhTSH. Before and 2, 7, 180 and 360 days after the TV was measured by magnetic resonance image (MRI). The smallest cross-sectional area of traqueal lumen (Scat) was also measured with MRI before, 2 and 7 days after treatment. Antithyroid antibodies, TSH, T3 and free T4 were assessed regularly. RESULTS: After 6 months, the decrease in TV was more significant in groups I (30.3±16.5%) and II (22.6±14.5%), than in control group (5.0±14.6%; p=0.01). After 12 months, TV decreased more in group I (39.2±16.9%) and group II (38.8±24.4%) than in group III (23.4±23.59%) but it was not statistically significant (p=0.205). During the first 30 days,total T3 and free T4 increased, without reaching thyrotoxic levels and TSH decreased. After 12 months, 8 patients developed hypothyroidism (3 in group I, 3 in group II and 2 in group III). Anti-thyroglobulin antibodies (anti-TG) titers increased after 6 months and returned to basal levels after 12 months similarly in all groups. There was more patients with anti-thyroperoxidase antibodies ( anti-TPO) titers incresead after 12 months on group I. There was not TV increase after ¹³¹I on the first week ,with or without rhTSH. CONCLUSION: The previous stimulus with rhTSH using a fixed 131I activity lead to a greater and more significant goiter reduction after six months of treatment than 131I alone. After twelve months this tendency was maintained, but without statistic relevance.

O desenvolvimento de uma abordagem de estratificação de risco para a doença arterial coronária é certamente uma ferramenta de grande utilidade para o cardiologista clínico ou clínico geral, assim como para o planejamento de saúde pública e organização de ações de saúde pública mais eficazes. No entanto, esse conhecimento deve ser discutido dentro de um cenário de custo-efetividade e de acordo com seu potencial como objeto de valor econômico. O presente estudo tem como objetivo a identificação de fatores de risco genético de eventos cardiovasculares na população brasileira e o desenvolvimento de um algoritmo preditor que utilize essas informações para o diagnóstico. Esse trabalho encontra-se expositivamente dividido em dois módulos. No primeiro, procuramos exemplificar através de um estudo de associação genética nossa capacidade atual de encontrar e caracterizar variantes genéticas com poder de estratificação populacional com relação ao risco cardiovascular. Utilizamos dados obtidos a partir de pacientes com doença coronária multiarterial, analisando a relevância do polimorfismo C242T do gene da p22phox, subunidade protéica da NADPH oxidase, na predição de mortalidade desta população. Nossos dados permitem não apenas associar esse marcador genético a um risco aumentado de mortalidade nessa população, mas também fornecem informações a respeito do provável mecanismo molecular através do qual esse marcador genético age. No segundo módulo, procuramos detalhar as limitações da abordagem previamente exemplificada, avaliando a informação gerada para o paciente individual. Aqui propomos uma nova abordagem de estratificação de risco desta população, capaz de fornecer de maneira individualizada uma estimativa de risco com maior sensibilidade, especificidade e, conseqüentemente, acurácia. Através de uma abordagem analítica de redução de dimensionalidade obtivemos um algoritmo preditor com acurácia maior do que aquela encontrada utilizando-se apenas os fatores de risco clássicos ou fatores de risco genéticos analisados individualmente. O entendimento das bases genéticas do desenvolvimento de doenças cardiovasculares facilitará não apenas o diagnóstico precoce, possibilitando o surgimento de abordagens terapêuticas mais específicas e desenhadas a atender suscetibilidades individuais, mas também poderá levar à identificação de novas vias eficazes de intervenção. The development of risk stratification approaches in coronary artery disease is certainly an important tool for the clinical cardiologist or internist. It also helps in the planning of public health policies and in the development of effective disease management algorithms. Nevertheless, these tools necessarily have to be developed in a cost-effective scenario and in close relationship with its intrinsic economic value. The present research project aims at the identification of genetic risk factors for cardiovascular events in the Brazilian population and the development of an algorithm with high predictive value for the diagnosis of these events. This thesis is, didactically, divided into 2 modules. Firstly, we have exemplified the used paradigm through the development of a genetic association study conducted in the Brazilian population. Here, we were able to describe and characterize genetic variants with the capacity of risk stratify populations into high and low risk groups. This section was possible with the use of data derived from patients with multi-vessel coronary artery disease and the analysis of the C242T gene variant of the p22phox gene, a subunit of the NADPH oxidase protein complex. Our data show not only a major signal of association between this genetic variant and overall mortality in this population, but also shed light on the potential molecular mechanism of this finding. Secondly, we have described the potential limitations of this approach analyzing information derived for the individual patient. Here, we propose a new risk stratification algorithm for this population with the capacity to provide individual risk with increased sensitivity and specificity. Through the use of a dimensionality reduction analytical approach we were able to find a predictive algorithm with higher accuracy than the one derived with the use of only classical cardiovascular risk factors and no genetic information. The understanding of the genetic basis for cardiovascular disease will improve not only the early diagnosis of these disorders, facilitating the rise of therapeutic approaches more specific and tailored to ones particular genetic susceptibility, but also lead to the identification of new pathways for effective intervention

Introdução: As disfunções tireoidianas subclínicas são comuns na prática clínica, particularmente entre mulheres de meia idade. Existem algumas evidências de que as disfunções tireoidianas subclínicas podem afetar o risco cardiovascular de forma negativa, além de afetar a qualidade de vida e produzir sintomas somáticos e psicológicos. Entretanto ainda existe muita controvérsia sobre se o tratamento destas disfunções afeta positivamente algum desfecho clínico e se estaria indicado realizar um rastreamento populacional destas disfunções. Objetivo: Este estudo teve como objetivo determinar a freqüência das disfunções tireoidianas subclínicas e sua associação com fatores de risco cardiovasculares tradicionais e com alguns fatores psico-sociais em mulheres com 40 anos ou mais em seu local de trabalho. Métodos: Estudo transversal de rastreamento com funcionárias da Universidade de São Paulo com 40 anos ou mais. Todas as participantes foram entrevistadas e responderam a quatro questionários específicos validados [um questionário sobre características sócio-demográficas, o questionário de angina de Rose, o Short Form Health Survey - 36 (SF-36) e o Self-Report Questionnaire (SRQ-20)], foram submetidas a mensuração de medidas antropométricas e da pressão arterial e tiveram uma amostra de sangue colhida para avaliação de função tireoidiana (TSH e T4-livre) e anticorpos antitireoperoxidase (anti-TPO), glicemia de jejum e colesterol total, LDL-colesterol e HDL-colesterol. Em uma subamostra do estudo também foi dosada a proteína C ultra-sensível (hsCPR). As mulheres foram analisadas de acordo com seu estado funcional tireoidiano. Resultados: Das 736 funcionárias com 40 anos ou mais convidadas a participar, 314 (42,7%) aceitaram o convite. As freqüências de hipotireoidismo e hipertireoidismo subclínico foram, respectivamente, 7,3% e 5,1%. Anticorpos anti-tireoperoxidase positivos foram encontrados em 51 mulheres (16,2%). Níveis de TSH < 10 mIU/l estavam presentes em 78,3% das mulheres com hipotireoidismo subclínico. Não houve diferença nas características gerais, nos fatores de risco para doença cardiovascular, nos fatores psico-sociais nem, de uma forma geral, na qualidade de vida comparando-se as mulheres de acordo com sua função tireoidiana. Conclusão: Não se encontrou nenhuma associação entre disfunção tireoidiana subclínica e fatores de risco para doença cardiovascular. Não se encontrou nenhuma associação de disfunção tireoidiana subclínica e fatores psico-sociais (qualidade de vida, sintomas somáticos e psicológicos). Os resultados deste estudo transversal não suportam a prática de rastreamento rotineiro de disfunção tireoidiana subclínica. Rational: Subclinical thyroid dysfunction is very common in clinical practice, particularly among middle-aged women. There is some evidence that subclinical thyroid dysfunction may affect cardiovascular risk in a negative fashion, and also affect quality of life and produce somatic and psychological symptoms. There remains much controversy as whether there should be a population based screening for these dysfunctions and whether treatment of these dysfunctions have any positive impact on clinical outcomes. Objective: The aim of this study was to determine the approximate frequency of subclinical thyroid dysfunction and its association with traditional cardiovascular risk factors as well as some psychosocial factors in women 40 years of age or older at the worksite. Methods: Cross-sectional screening study with women 40 years of age or older, working at the University of São Paulo. All the women answered four specific questionnaires [a questionnaire on socio-demographic characteristics, the Rose Angina Questionnaire, the Short Form Health Survey -36 (SF-36) and the Self- Report Questionnaire (SRQ-20)], had antropometric variables and blood pressure measured, and blood analyzed for total-cholesterol, LDL-cholesterol, HDL-cholesterol, tryglicerides, fasting glucose, thyroid-stimulating hormone (TSH), free-thyroxine (free-T4) and anti-thyreoperoxidase antibodies (anti-TPO). In a sub-sample of these women high-sensitive C reactive protein (hsCRP) was measured. Women were analyzed according to their thyroid function status. Results: Of the 736 women invited to participate, 314 (42.7%) accepted the invitation. The frequencies of subclinical hypothyroidism and subclinical hyperthyroidism were, respectively, 7.3% and 5.1%. Positive antibodies against thyreoperoxidase were present in 51 women (16.2%). TSH levels < 10 mIU/l were present in 78.3% of women with subclinical hypothyroidism. There was no difference in general characteristics, cardiovascular risk factors, psychosocial factors nor, in a general way, in quality of life among these women according to their thyroid function status. Conclusion: No association between cardiovascular risk factors and subclinical thyroid dysfunction was found. No association between subclinical thyroid dysfunction and psychosocial factors (quality of life, somatic and psychological symptoms) was found either. The results of this cross-section study do not support the routine screening of subclinical thyroid dysfunction.

CONTEXTO: A secreção de prolactina (PRL) e a expressão de seu gene são inibidas pela dopamina. Prolactinomas são os adenomas hipofisários funcionantes mais freqüentes, sendo que os agonistas dopaminérgicos são a primeira escolha para seu tratamento. No entanto, uma porcentagem dos pacientes é resistente aos agonistas dopaminérgicos. OBJETIVO: Como os mecanismos envolvidos na resistência aos agonistas dopaminérgicos não são totalmente compreendidos, o objetivo deste estudo foi obter mais informações no que diz respeito às alterações moleculares entre os prolactinomas sensíveis e resistentes aos agonistas dopaminérgicos. PACIENTES: O tecido tumoral de 22 pacientes com prolactinomas foram coletados e classificados como sensíveis ou resistentes (incluindo aqueles com crescimento tumoral) de acordo com sua resposta clínica e laboratorial aos agonistas dopaminérgicos. MÉTODOS: A expressão de 7 genes foi avaliada por Real Time polymerase chain reaction: gene do receptor de dopamina tipo 2 (DRD2), do fator de crescimento neural beta (NGF?) e de seu receptor (NGFR), dos receptores de estrógeno alfa (ESR1) e beta (ESR2), do pituitary tumor transforming gene (PTTG) e da metalotioneína 3 (MT3). RESULTADOS: A expressão mediana de DRD2 e de NGFR nos pacientes sensíveis foi significativamente maior quando comparada aos resistentes (p= 0.016 e p= 0.009, respectivamente). Além disso, ambas expressões estiveram significativamente correlacionadas positivamente com a redução da PRL durante o tratamento (r= ?0.66; p= 0.002 e r= 0.57; p= 0.017; respectivamente). Uma correlação positiva foi encontrada entre a mediana de expressão do NGF? e do DRD2 (r=0.53; p=0.023) e entre a mediana de expressão do PTTG e do ESR2 (r=0.66; p=0.008). também houve correlação entre a os valores de PRL sérica antes do tratamento e a mediana de expressão do gene do ESR2 (r=0.53, p=0.04). Não foi encontrada nenhuma correlação entre a expressão do gene da MT3 e a sensibilidade ou resistência aos agonistas dopaminérgicos. CONCLUSÕES: A expressão do gene do DRD2 e do NGFR estão relacionadas com a sensibilidade dos prolactinomas aos agonistas dopaminérgicos, enquanto a expressão do gene do PTTG e do ESR2 podem ter alguma relação com a agressividade tumoral. A resposta dos prolactinomas aos agonistas dopaminérgicos deve ser vista como um espectro variando do tumor mais sensível ao mais resistente com crescimento CONTEXT: Prolactin (PRL) secretion and its gene expression are inhibited by dopamine. Prolactinomas are the most common secreting pituitary adenomas, with dopamine agonists being the first choice for their treatment. However, a subset of patients is resistant to dopamine agonists. OBJECTIVE: As the mechanisms involved in dopamine agonists resistance are not fully understood, the aim of this study was to get new insights regarding the molecular differences between prolactinomas responsive and resistant to dopamine agonists. PATIENTS: Tumor tissue of 22 patients harboring prolactinomas were collected and classified as responsive or resistant (including the ones with tumor growth) according to their clinical and laboratorial response to dopamine agonists. METHODS: The expression of 7 genes was evaluated by Real Time polymerase chain reaction: dopamine receptor type 2 (DRD2), nerve growth factor beta (NGF?) and its receptor (NGFR), estrogen receptor alpha (ESR1), beta (ESR2), pituitary tumor transforming gene (PTTG) and metallothionein 3 (MT3). RESULTS: Median DRD2 and NGFR expressions of responsive patients were significantly higher compared to the resistant ones (p= 0.016 and p= 0.009, respectively). Moreover, both expressions were positively correlated with PRL decrease during treatment (r= ?0.66; p= 0.002 and r= 0.57; p= 0.017; respectively). A positive correlation was found between NGFB and DRD2 (r= 0.53; p= 0.023) and PTTG and ESR2 expressions (r= 0.66; p= 0.008). There was also a correlation between serum PRL levels before treatment and ESR2 expression (r= 0.53, p= 0.04). It was not observed correlation between MT3 and responsiveness or resistance to dopamine agonists. CONCLUSIONS: DRD2 and NGFR expressions are related to prolactinoma responsiveness to dopamine agonists whereas PTTG and ESR2 may have a role in tumor aggressiveness. The response of prolactinomas to dopamine agonists should be view as a spectrum ranging from responsive to resistant with tumor growth

INTRODUÇÃO: As proteínas desacopladoras de prótons (UCPs: uncoupling proteins) pertencem à família dos transportadores mitocondriais H+/ácidos graxos e têm distribuição diferenciada nos tecidos. Sabe-se que a UCP1 é responsável pela termogênese, mas o exato papel fisiológico da UCP2 e UCP3 ainda não está completamente estabelecido. Os hormônios tireoideanos (T3 e T4) estimulam a expressão da UCP3 em músculo cardíaco e esquelético, no entanto o mecanismo pelo qual exercem esse efeito não é conhecido. Este projeto visa avaliar se as alterações na expressão gênica da UCP3 são relacionadas a efeito primário do T3 ou são secundárias à estimulação do sistema renina-angiotensina ou do sistema ?-adrenérgico. MÉTODOS: Para a realização do estudo, criou-se um modelo animal de hipertireodismo, em ratos machos Sprague-Dawley, através da 3 administrações de 100 ?g/100 g peso corpóreo de LT3, em dias alternados, associado ou não à captopril (1 mg/100 g de peso corpóreo), ?-bloqueador propranolol (1 mg/100g de peso corpóreo) ou ?2-agonista clenbuterol (0,04 mg/100 g de peso corpóreo). A expressão do mRNA da UCP3 foi semi-quantitativamente determinada por Northern blot em amostras de músculo ventricular cardíaco e músculo esquelético (gastrocnemius e soleus). A expressão da proteína UCP3 foi avaliada por Western blot em músculo esquelético (quadríceps). Os resultados foram expressos em unidades arbitrárias de densitometria óptica. O excesso de iodo inibe a proliferação celular e secreção hormonal, enquanto retarda os efeitos oncogênicos da ativação de RET/PTC3 na célula folicular tiroidiana. A mutação BRAF (T1799A) é a mais prevalente no câncer de tiroide, e modelo transgênico desenvolve câncer que progride para histotipo agressivo. Altos níveis de microRNAs (miRNAs) do cluster miR-17-92 estão associados a histotipos agressivos de câncer de tiroide e modulam a tradução de mRNAs alvo componentes de vias de sinalização oncogênicas. Neste estudo, avaliamos a influência da alta dose de iodo sobre miRNAs frente ativação do oncogene BRAF e seu efeito na biologia da célula folicular tiroidiana. A indução de BRAFT1799A ativa uma robusta expressão de miR-17-92 enquanto alta dose de iodo bloqueia este efeito na célula tiroidiana. miR-19 inibe a tradução de Smad4 e bloqueia a transdução do sinal de TGFb, efeito revertido pelo iodo. O iodo interfere na expressão de miR-17-92 por bloquear ativação da sinalização Notch induzida por BRAF. Estes resultados indicam que o iodo protege a célula folicular tiroidiana durante a indução de BRAFT1799A, revertendo a ativação dos miRNAs oncogênicos do cluster miR-17-92 e restaurando os níveis protéicos de Smad4 por interferir na via de sinalização Notch. Iodine excess blocks cell proliferation and inhibits hormone synthesis, while delays oncogenic effects of RET/PTC3 activation in thyroid follicular cells. BRAF mutation (T1799A) is the most prevalent genetic alteration in thyroid cancer, and transgenic mice model for BRAF develops thyroid cancer that progress to aggressive histotypes. High levels of microRNAs (miRNAs) of miR-17-92 cluster are associated to aggressive thyroid cancer and modulate translation of target mRNAs in oncogenic signaling pathways. In this study, we evaluated the influence of high dose iodine in miRNAs under BRAF oncogene activation, and its effects in thyroid follicular cell biology. BRAF induction induces high expression of miR-17-92 while high dose iodine blocks this effect in thyroid follicular cells. miR-19 inhibits Smad4 translation and impairs TGFb signaling transduction, effect reverted by iodine. Iodine modulates miR-17-92 expression by interfering in BRAF-induced Notch signaling activation. These results indicate that iodine protects thyroid follicular cell during BRAF induction, reverting oncogenic miR-17-92 activation and restoring protein levels of Smad4 by Notch signaling modulation.

A neoplasia endócrina múltipla Tipo 2 (MEN-2) é uma síndrome hereditária que compreende: carcinoma medular da tireoide (CMT), hiperparatireoidismo primário, feocromocitoma e outras doenças não-endócrinas. Está subdividida em três desordens fenotípicas relacionadas: MEN-2A, MEN-2B e apresentação única do carcinoma medular familiar da tireoide (FMTC). A MEN-2A compreende CMT (95% a 100% dos casos), feocromocitoma (30% a 50%) e a hiperplasia daparatireoide (10% a 30%). A MEN-2B apresenta CMT (95% a 100%),feocromocitoma (30% a 50%), neurofibromatose de mucosas e do tratogastrointestinal e anomalias músculo-esqueléticas em 100% dos casos. A MEN-2 está associada a mutações no proto-oncogene RET (10q11.2), que determinam a ativação permanente do receptor RET . O proto-oncogene RET gene codifica cerca de 10 isoformas, sendo as isoformas RET 9 e RET 51, as mais importantes no desenvolvimento da MEN-2. Para a determinação da correlação genótipo-fenótipo eisoformas da proteína RET (RET 9, RET 51 e resíduo Tyr Y1062) foram estudados 58 indivíduos residentes no Estado de Minas Gerais (33 casos de CMT isolados e 25 familiares). Foram utilizadas as técnicas de sequenciamento genético do sangue periférico e imunoistoquímica dos blocos de parafina provenientes do tratamento cirúrgico a que haviam sido submetidos os pacientes. O fenótipo mais prevalente foi o FMTC e a mutação observada foi a L790F. Vinte e três pacientes apresentaram oSNP L790F, seis pacientes o SNPG691S e quatro pacientes o SNP A764A, não relatado na literatura. O número reduzido de pacientes com MEN-2 dos quais se obteve material parafinizado não permitiu a interpretação da correlação entre o fenótipo, o genótipo e a expressão das isoformas da proteína RET. Acredita-se que o trabalho tenha relevância clínica e laboratorial por ser a primeira tentativa de investigação e registro dos pacientes com CMT esporádico e CMT familiar (MEN-2) no Estado de Minas Gerais. The multiple endocrine neoplasia Type 2 (MEN-2) is a hereditary syndrome comprising: medullary thyroid carcinoma (MTC), primary hyperparathyroidism, pheochromocytoma and other non-endocrine diseases. Is subdivided into three phenotypic related disorders: MEN-2A, MEN-2B and familial medullary thyroid carcinoma (FMTC). The MEN-2A comprises MTC (95% to 100% of cases), pheochromocytoma (30% to 50%) and parathyroid hyperplasia (10% to 30%). The MEN-2B presents MTC (95% to 100%), pheochromocytoma (30% to 50%), ganglioneuromatosis of the gastrointestinal tract, and an asthenic "Marfanoid" body habitus. in 100% of cases. The MEN-2 is associated with mutations in proto-oncogene RET (10q11), which determine the standing of the RET receptor activation. The protooncogene RET gene encodes around 10 isoformas, and isoformas RET 9 and RET 51, the most important in the development of MEN-2. For the determination of correlation genotype-phenotype and isoformas protein RET (RET 9, RET 51 and residue Tyr Y1062) were studied 58 individuals resident in the State of Minas Gerais (33 cases of MTC isolated and 25 family). Were used genetic sequencing techniques of peripheral blood and immunohistochemistry of paraffin blocks from surgical treatment that hadundergone patients. The most prevalent phenotype was FMTC and mutation observed was the L790F. Twenty-three patients presented the SNP L790F, six SNPG691S patients and four patients the SNP A764A, not reported in the literature. The small number of patients with MEN-2 from whom the paraffin blocks were obtained has not allowed the interpretation of the correlation between genotype and phenotype, and theexpression of isoformas protein ret. The work has relevance clinical and laboratory for being the first attempt to research and patient record with CMT sporadic and CMT family (MEN-2) in the State of Minas Gerais.

Neste estudo, geramos ensaios de espectrometria de massa para detecção de 111 mutações nos genes RET, BRAF, NRAS, HRAS, KRAS, PIK3CA e AKT1 e avaliamos inúmeras linhagens celulares e tumores tiroidianos. Mostramos que as mutações dos genes BRAF e RAS refletem prognósticos distintos e que as mutações BRAF são altamente prevalentes em câncer metastático. Mutações dos genes PIK3CA e AKT1, esta última sendo reportada pela primeira vez no câncer de tiróide, são relativamente frequentes neste câncer. Avaliamos ainda a função do microRNA let-7 neste câncer. Mostramos que a ativação do rearranjo RET/PTC3 em células de tiróide PCCl3 reduz a expressão de let-7. Além disso, a transfecção deste microRNA em células TPC-1, que apresentam o rearranjo RET/PTC1, inibe a fosforilação de ERK, o crescimento celular e modula a expressão de genes do ciclo celular e diferenciação. Estes dados contribuem na aplicação de terapias dirigidas a efetores das vias PI3K e MAPK no câncer de tiróide, além de salientar o envolvimento do miRNA let-7 como um gene supressor tumoral nesta doença. In this study, we designed an assay panel for genotyping 111 mutations by mass spectrometry in RET, BRAF, NRAS, HRAS, KRAS, PIK3CA, AKT1 and other related genes in many thyroid cancer cell lines and tumors. We show that patients with BRAF and RAS mutations have distinct prognosis and that BRAF mutations are highly prevalent in metastatic thyroid cancers. Mutations of PIK3CA and AKT1, the latter not previously described in this disease, are comparatively frequent in thyroid cancers. In addition, we evaluated the role of let-7 microRNA in this cancer. We show that RET/PTC3 activation in PCCl3 thyroid cells reduces let-7 expression. Moreover, transfection of let-7 in TPC-1 cells, which harbor RET/PTC1 rearrangement, inhibits MAPK activation, reduces cell growth and modulates the expression of cell cycle and differentiation genes. These findings may contribute to the design of therapies directed at MAPK and PI3K effectors and to highlight the function of let-7 as tumor suppressor gene in thyroid cancer.

Ainda não é claro o papel dos hormônios tireoideanos (HT) no desenvolvimento do esqueleto fetal. Para responder a questão, induzimos hipotireoidismo materno e fetal em camundongos prenhes através da administração de metimazol e perclorato de sódio. O esqueleto fetal apresentou discretas morfológicas até 16,5 dias de idade embrionária (E). Apenas no final da gestação, em 18,5E, foram observadas a redução significativa da zona hipertrófica, do número de condrócitos hipertróficos, desorganização e diminuição da quantidade dos condrócitos proliferativos, além da redução da expressão do colágeno I, X e osteocalcina. Os TRs, assim como LAT1, LAT2 e MCT8 foram detectados em todas as idades estudadas. A alta expressão gênica da D3, principal inativadora do hormônio tireoideano, em 14,5E e sua redução significativa durante o desenvolvimento, até atingir níveis indetectáveis no período pós-natal indicam que a D3 seja responsável por manter baixos níveis de HT no esqueletono início da gestação, garantindo um desenvolvimento ósseo normal. Thyroid hormone (TH) plays a key role on post-natal bone development and metabolism, while its relevance during fetal bone development is uncertain. To study this, pregnant mice and fetuses were made hypothyroid. The skeleton morphology was preserved up to 16.5 embryonic days (E). Only at E18.5, the hypothyroid fetuses exhibited a reduction in femoral type I and type X collagen and osteocalcin mRNA levels, in the length and area of the proliferative and hypertrofic zones, in the number of chondrocytes per proliferative column, and in the number of hypertrophic chondrocytes. This suggests that up to E16.5, thyroid hormone signaling in bone is kept to a minimum. D3 mRNA was readily detected as early as E14.5 and its expression decreased markedly at E18.5, and even more after birth. The expression levels of D3 gene during early bone development along with the absence of a hypothyroidism-induced bone phenotype at this time suggest that its expression keeps thyroid hormone signaling in bone to very low levels at this early stage of bone development.

O Brasil, que conta hoje com 7% de idosos, será a sexta maior população de idosos do Planeta em 2020. A doença cardiovascular aumenta significativamente com a idade e evidências indicam a importância do endotélio e da hipercolesterolemia na aterosclerose. Contudo, não está bem estabelecido, que o tratamento da dislipidemia em idosos melhora a função vasomotora. Objetivos: Analisar a resposta vasomotora e parâmetros metabólicos em idosos com hipercolesterolemia ao tratamento com estatina. Delineamento: Ensaio Clínico Randomizado Duplo Cego. Casuística e Métodos: 72 idosos com idade 65 anos [75 ± 7,7 (65 a 92)], 64% mulheres, excluídos diabéticos, hipertensos graves, obesos, corticoterapia, reposição hormonal, doença em estágio terminal; todos realizaram avaliação clínica, laboratorial (glicose, lípides, TGO, TGP, CPK, Proteina C reativa ultra-sensível (PCR)) e função vasomotora por ultra-sonografia de alta resolução da artéria braquial, com equipamento ATL HDI 5000 e transdutor multifreqüencial de 7 a 10 MHz, segundo Diretrizes. Utilizou-se hiperemia reativa para resposta vasomotora dependente do endotélio (DMF) e nitrato sub-lingual para a independente do endotélio (DNT). Destes, 47 pacientes apresentaram LDL 130 mg/dL e foram randomicamente designados para grupo placebo (GP) e grupo tratamento (GT); todos realizaram avaliação antes e depois da intervenção (placebo ou atorvastatina 20mg oral por 30 dias). Para a análise estatística, utilizaram-se testes paramétricos e não-paramétricos, valores de p 0,05 foram considerados estatisticamente significantes. Resultados: Os grupos foram semelhantes em suas características basais; a DMF não apresentou diferença estatisticamente significante entre o GP e GT após intervenção [6,2 ± 6,2 (-4,8 23,7) e mediana = 6,4 vs. 5,0±5,6 (-5,1 18,9) e mediana= 3,5 p = 0,551], o mesmo ocorrendo para a DNT [7,1 ± 4,7 (-1,3 21,2) e mediana = 6,8 vs. 8,6±5,0 (1,4 21,6) e mediana = 6,8 p = 0,373). A análise estratificada sugere potencial modificador de efeito das co-variáveis estratificadas sexos, estratos etários (65 years old [75 ± 7.7 (65 to 92)], 65% of women, excluding those suffering from diabetes, serious hypertension, obesity, steroid use, hormone replacement, end-stage disease; all of them underwent clinical evaluation, laboratorial evaluation (glucose, lipids, liver enzymes, CPK, high sensitivity CRP (CRP). Vasomotor response was evaluated by means of ATL HDI 5000 high resolution ultrasound scan of the brachial artery and multifrequency transduction of 7 to 10 MHz. Reactive hyperemia was used for endothelium-dependent flow response (DMF) and sublingual nitrate for the endothelium-independent dilation (DNT). Out of those patients, 47 presented LDL 130 mg/dl and were ramdonly divided into placebo (n = 23) and treatment group (n = 24); all of them had underwent evaluation before and after intervention (placebo or atorvastatin 20 mg, orally, during 30 days); for statistic analysis parametric and non-parametric tests were used and p 0.05 values were considered statistically significant. Results: The groups were similar in their basal characteristics; DMF did not present any statistically significant difference between placebo and treatment group after intervention [6.2 ± 6.2 (-4,8 - 23.7) and median = 6.4 vs. 5.0 ± 5.6 (-5.1 - 18.9) and median = 3.5 p = 0.551], the same occurred to DNT [7.1 ± 4,7 (-1.3 - 21.2) and median = 6.8 vs. 8.6 ± 5.0 (1.4 - 21.6) and median = 6.8 p = 0.373). Stratified analysis suggests effect modifying factor of the gender, age strata (< 80 and 80 years of age) and LDL (< 160 mg/dL e 160 mg/dL). CRP diminished 64% in the treatment group (median in the placebo group = 0.3 mg/dL vs. treatment group = 0.1 mg/dL p = 0.014). The reduction of total cholesterol was 27%, LDL = 41%, VLDL = 20% and triglycerides = 34%; HDL raised 12% in the treatmente group in relation to the placebo group. The highest enzymatic elevation did not surpass 3 times the limit and there were no myalgia. Conclusions: In spite of a significant decrease in lipids, there were no significant changes in vasomotion response with statin therapy, suggesting intrinsic alterations of the vessel associated with ageing. However, CRP reduction suggested an anti-inflammatory effect
