Background Survival prediction models are clinical tools that help professionals make the best treatment decisions. In the treatment of critical limb ischemia, several scoring methods have emerged; however, many are limited because they are not reproducible in different populations. This study analyze the Bypass versus Angioplasty in Severe Ischemia of the Leg survival prediction model (BASIL SPM), exclusively in patients who underwent infrapopliteal (InfraPo) revascularization for the treatment of critical limb ischemia. Methods In this retrospective cohort, patients who underwent InfraPo interventions during a 4-year period (2009–2013) were consecutively included, according to the primary intervention (endovascular or open repair), irrespective of combined treatment in the femoropopliteal and InfraPo segments. Performance of the BASIL SPM was determined using the area under the curve of the receiver-operating characteristic curve. Additionally, secondary patency, limb salvage, and overall survival were analyzed by the Kaplan–Meier method. Results The mean follow-up duration was 32.7 ± 23.8 months. In the total patient group (n = 134), the mean age was 72 ± 8.6 years (range, 46–91 years), and the main associated comorbidities were hypertension (85.8%) and diabetes mellitus (76.8%). Regarding differences between the groups, patients in the endovascular group (n = 100) were older (73 vs. 69.5; P = 0.033) and more frequently stratified as high risk (61% vs. 38.2%; P = 0.030) than the open repair group (n = 34). The area under the curve (95% confidence interval [CI]) using the BASIL SPM at 6, 12, and 24 months was 0.499 (95% CI: 0.344–0.657), 0.508 (95% CI: 0.353–0.629), and 0.549 (95% CI: 0.420–0.678), respectively. In the total patient group, the 36-month secondary patency, limb salvage, and overall survival were 31.5%, 81.1%, and 65.9%, respectively. Conclusions The BASIL SPM was a poor predictor of life expectancy in this patient cohort.

Left ventricular hypertrophy (LVH) is an independent predictor of cardiovascular risk, and its characterization and prevalence in chronic renal disease (CRD) should be further studied.To establish the diagnosis of LVH in patients with stage-5 CRD using six different electrocardiographic criteria, and to correlate them with left ventricular mass index (LVMI) as obtained by echocardiography.Cross-sectional study including 100 patients (58 men and 42 women, mean age 46.2 + or - 14.0 years) with CRD of all causes undergoing hemodialysis (HD) for at least six months. Electrocardiography (ECG) and echocardiography were performed in all patients, always up to one hour after the end of the HD sessions.LVH was detected in 83 patients (83%), of whom 56 (67.4%) had the concentric pattern and 27 (32.6%) the eccentric pattern of LVH. Diagnostic sensitivity, specificity and accuracy of all the electrocardiographic methods studied were higher than 50%. Using Pearson's linear correlation for LVMI, only the Sokolow-Lyon voltage criterion did not show aor = 0.50 coefficient. Calculation of the likelihood ratio, in turn, showed that ECG has a discriminatory power for the diagnosis of LVH in the population studied, with emphasis on the Cornell-product and Romhilt-Estes criteria. No correlation was observed between LVMI and QTc and QTc dispersion.ECG is a useful, efficient, and highly reproducible method for the diagnosis of LVH in HD patients. In this population, the Cornell-product proved to be the most reliable criterion for the detection of LVH.

It would be useful to have a clinical test that increases the suspicion of white coat hypertension (WCH) during the medical consultation.To evaluate the Slow Breathing Test (SBT) when differentiating hypertension from WCH.101 hypertensive patients selected at triage had their medication withdrawn for 2-3 weeks. The blood pressure (BP) was measured before and after the SBT at two consultations at the office. The test consisted in breathing for 1 minute at the frequency of one respiratory cycle every 10 seconds. Two diagnostic criteria were compared: 1--decrease in diastolic BPor = 10% in at least one visit or 2--decrease in BP to normal levels (140/90 mm Hg) in at least one visit. The ambulatory blood pressure monitoring (ABPM) was performed while blinded to the clinical measurements.71 women and 30 men, with a mean age of 51+/-10 years, with mean pre and post-test BP of 152+/-17/ 99+/-11 and 140+/-18/ 91+/-11 mm Hg were assessed. Nine patients had normal clinical and ambulatory measurements. Of the 92 patients, 28 (30%) were classified as having WCH; 15 had a positive test for Criterion 1 and 21 for the Criterion 2. Among 64 (70%) hypertensive individuals, 14 tested positive for Criterion 1 and 12 for Criterion 2. Sensitivity and specificity (95% CI): 0.54 (0.36-0.71) and 0.78 (0.67-0.87) for Criterion 1; 0.75 (0.57-0.87) and 0.81 (0.70-0.89) for Criterion 2.The SBT showed an increase in the clinical suspicion of WCH in two visits when using the BP normalization criterion. This finding suggests that the test can help in the optimization of ABPM requests for suspected cases.

The left ventricular hypertrophy (LVH) is an important and independent cardiovascular risk factor. There is a scarcity of studies in Brazil designed to test the efficacy of the electrocardiogram (ECG) in the diagnosis of this important pathological process.To evaluate a new electrocardiographic score for the diagnosis of LVH by ECG: the sum of the highest amplitude of the S wave and the highest amplitude of the R wave on the horizontal plane, multiplied by the result of the QRS duration [(S+R) X QRS)] and comparing it with the classic electrocardiographic criteria.The echocardiograms and ECG of 1,204 hypertensive patients receiving outpatient care were evaluated. The left ventricular mass index (LVMI) was assessed by the echocardiogram, with a diagnosis of LVH when the LVMI wasor = 96 g/m(2) for women andor = 116 g/m(2) for men. Four classic criteria of LVH were analyzed at the ECG, in addition to the new score to be tested.In general, the studied ECG-LVH criteria showed significant statistical correlation to the echocardiographic LVMI. The (R+S) X QRS index, using 2.80 mm.s as the cutoff value, provided test accuracy regarding sensibility and specificity of 35.2% and 88.71%, respectively, representing the best correlation to LVMI (r=0.564) when compared to the other indexes: Romhilt-Estes (r=0.464); Sokolow-Lyon (r=0.419); Cornell voltage (r=0.377); Cornell product r=0.444).All the electrocardiographic criteria used for the assessment of the LV mass presented low sensitivity. The new score presented the best correlation with LVMI when compared to the other indexes.

INTRODUÇÃO: as doenças cardiovasculares (DCV) são as principais responsáveis pela carga global das doenças crônicas não transmissíveis, sobretudo pelo impacto do infarto agudo do miocárdio e do acidente vascular encefálico (AVE). Dentre os fatores de risco potencialmente modificáveis associados a esses eventos, a hipertensão arterial sistêmica (HAS) se destaca, configurando-se como a principal causa de morbimortalidade cardiovascular no mundo contemporâneo. O presente estudo tem como objetivos determinar a força da associação da HAS em relação à ocorrência do AVE (e seus subtipos) e investigar o risco atribuível populacional (RAP) da doença associado ao evento e seus subtipos, na população brasileira. CASUÍSTICA E MÉTODOS: trata-se de um estudo caso-controle, no qual foram avaliados participantes do INTERSTROKE Study incluídos no Brasil. As características clínicas e demográficas da população do estudo foram avaliadas e as variáveis com significância clínica ou estatística associadas à ocorrência de AVE foram incluídas no modelo multivariado para identificar a força da associação entre HAS e AVE (e seus subtipos). RESULTADOS: no Brasil, 734 indivíduos foram incluídos no INTERSTROKE Study, sendo 367 casos e 367 controles. A média de idade dos participantes foi de 64,8 anos e a prevalência do sexo masculino foi de 55,0%. O grupo caso, em relação ao controle, apresentou maior prevalência de HAS (80,9% versus 56,9%, p < 0,0001), tabagismo ativo (19,9% versus 10,1%, p = 0,0003), sedentarismo (91,0% versus 77,9%, p < 0,0001), fatores psicossociais (estresse e/ou depressão) (42,9% versus 27,2%, p < 0,0001), etilismo elevado ou libação alcoólica (6,5% versus 0,8%, p = 0,0004), bem como valor médio mais elevado da relação cintura/quadril (0,95 versus 0,93, p = 0,0118) e da relação ApoB/ApoA1 (0,81 versus 0,79, p = 0,0094), além de avaliação qualitativa desfavorável do padrão dietético pelo Índice de Alimentação Saudável Alternativo modificado (porcentagem no terceiro tercil de 44,4% versus 54,0%, p = 0,0196). Na análise multivariada, observou-se razão de chances (RC) da HAS para a ocorrência de AVE (independentemente do subtipo) de 3,34 (IC = 95,0%, 2,13 - 5,24), com risco atribuível populacional (RAP) de 56,72% (IC = 95,0%, 42,88 - 70,56). Para o AVE isquêmico, identificou-se RC de 3,19 (IC = 95,0%, 2,00 - 5,09), com RAP de 55,60% (IC = 95,0%, 40,30 - 70,91). Para o AVE hemorrágico, constatou-se RC de 6,54 (IC = 95,0%, 2,07 - 20,69), com RAP de 68,59% (IC = 95,0%, 14,62 - 122,56). Para o sexo masculino, na análise multivariada, foi evidenciada RC de 3,34 (IC = 95,0%, 1,80 - 6,20), com RAP de 56,72% (IC = 95,0%, 32,53 - 80,90) do fator para o AVE total, e RC de 3,17 (IC = 95,0%, 1,66 - 6,05), com RAP de 55,41% (IC = 95,0%, 27,68 - 83,13) para o AVE isquêmico. Já para o sexo feminino, foi identificada RC de 4,21 (IC = 95,0%, 2,06 - 8,60), com RAP de 61,74% (IC = 95,0%, 31,50 - 91,98) da HAS para o AVE total, e RC de 3,88 (IC = 95,0%, 1,97 - 7,97), com RAP de 60,09% (IC = 95,0%, 27,24 - 92,94) para o AVE isquêmico. CONCLUSÃO: A HAS foi identificada como preditor independente e de alto impacto clínico associado à ocorrência de primeiro AVE na população brasileira. A associação observada foi mantida para os subtipos do evento, com maior força no grupo AVE hemorrágico. Em relação ao sexo, entre as mulheres, foi observada maior RC da HAS para ocorrência de AVE (total e isquêmico). Estratégias de saúde pública devem ser desenvolvidas para a identificação e o controle efetivo desse importante fator de risco cardiovascular na população brasileira. INTRODUCTION: cardiovascular diseases (CVD) are the main cause for the global burden of chronic non-communicable diseases, especially due to its association with acute myocardial infarction and stroke. Among the potentially modifiable risk factors associated with these events, systemic arterial hypertension (SAH) stands out, being considered the main cause of cardiovascular morbidity and mortality in the contemporary world. The current study aims to determine the strength of the association of SAH in relation to the occurrence of stroke (and its subtypes) and to evaluate the clinical impact through the population attributable risk (RAP) of the disease associated with the event and its subtypes, in the Brazilian population. CASUISTICS AND METHODS: this is a case-control study, in which participants from the INTERSTROKE Study included in Brazil were evaluated. The clinical and demographic characteristics of the study population were assessed and variables with clinical or statistical significance associated with the occurrence of stroke were included in the multivariate model to identify the strength of the association between SAH and stroke (and their subtypes), and its clinical impact. RESULTS: in Brazil, 734 individuals were included in the INTERSTROKE Study, with 367 cases and 367 controls. The average age of the participants was 64.8 year and the male prevalence was 55.0%. Cases had a higher prevalence of SAH compared to controls (80.9% versus 56.9%, p < 0.0001), current smoking (19.9% versus 10.1%, p = 0.0003), sedentary lifestyle (91.0% versus 77.9%, p < 0.0001), psychosocial factors (stress and / or depression) (42.9% versus 27.2%, p < 0.0001), high alcohol consumption or binge (6.5% versus 0.8%, p = 0.0004), in addition to presenting a higher waist / hip ratio (0.95 versus 0.93, p = 0.0118) and ApoB/ApoA1 ratio (0.81 versus 0.79, p = 0.0094) and worse qualitative assessment of the dietary pattern by the modified Alternative Healthy Eating Index (percentage in the third tertile of 44.4% versus 54.0%, p = 0.0196). In the multivariate analysis, the odds ratio (OR) of SAH for the occurrence of stroke (regardless of subtype) was 3.34 (95.0% CI, 2.13 - 5.24), with a population attributable risk (PAR) of 56.72% (95.0% CI, 42.88 - 70.56). For ischemic stroke, an OR of 3.19 (95.0% CI, 2.00 - 5.09) was identified, with PAR of 55.60% (95.0% CI, 40.30 - 70.91). For hemorrhagic stroke, OR of 6.54 was found (95.0% CI, 2.07 - 20.69), with PAR of 68.59% (95.0% CI, 14.62 - 122.56). Male gender presented OR of 3.34 (95.0% CI, 1.80 - 6.20), with PAR of 56.72% (95.0% CI, 32.53 - 80.90) for total stroke and OR of 3.17 (95.0% CI, 1.66 - 6.05), with PAR of 55.41% (95.0% CI, 27.68 - 83.13) for ischemic stroke. For women, an OR of 4.21 (95.0% CI, 2.06 - 8.60) was identified, with PAR of 61.74% (95.0% CI, 31.50 - 91.98) for total stroke and OR of 3.88 (95.0% CI, 1.97 - 7.97), with PAR of 60.09% (95.0% CI, 27.24 - 92.94) for ischemic stroke. CONCLUSION: SAH was identified as an independent predictor with high clinical impact associated with the occurrence of a first stroke in the Brazilian population. The observed association was maintained for the subtypes of the event, with greater strength in the hemorrhagic stroke group. In relation to sex, a greater OR of SAH was observed among women for the occurrence of stroke (total and ischemic). Public health strategies must be developed to identify and effectively control this important cardiovascular risk factor in the Brazilian population.

Introdução: Inúmeros estudos estabeleceram correlações entre o índice tornozelo-braço (ITB), um marcador de aterosclerose subclínica, e o prognóstico cardiovascular em diferentes populações. No entanto, poucos estudos avaliaram a correlação entre os valores do ITB e lesões cardiovasculares e renais, exclusivamente, em pacientes com hipertensão arterial e diabetes. Objetivo: Estudar a prevalência de alterações morfofuncionais cardíacas, carotídeas, retinianas e renais de acordo com a presença ou não de valores de ITB alterados (ITB 1,4) em pacientes hipertensos com diabetes tipo 2. Métodos: Foram incluídos no estudo 99 pacientes hipertensos diabéticos com idade entre 50 e 80 anos. A aferição do ITB foi realizada em todos os pacientes por método validado e estes foram classificados em Grupo 1 (ITB normal, n = 49) ou Grupo 2 (ITB alterado, n =50). Todos os pacientes foram submetidos, em até 06 meses, à realização de ecodopplercardiograma, ultrassonografia de carótidas, retinografia colorida, aferição da taxa de filtração glomerular (TFG) e da albuminúria de 24h. Os pacientes foram analisados para a ocorrência ou não de um desfecho-composto ecocardiográfico que incluiu alterações morfológicas e funcionais cardíacas relevantes para a prática clínica. Os pacientes dos grupos 1 e 2 foram também comparados quanto à prevalência de placas carotídeas com ou sem repercussão hemodinâmica, TFG < 60 ml/mim/m2, albuminúria de 24h > 30mg e presença ou não de retinopatia. Por fim, foram comparadas as frequências médias das seguintes lesões de órgãos-alvo de ambos os grupos, considerando-se valor unitário para a presença de cada uma delas: hipertrofia do ventrículo esquerdo, retinopatia hipertensiva, TFG < 60 ml/min/m2 e estenose da artéria carótida interna > 50% do seu diâmetro. Resultados: A média de idade dos pacientes foi 65,4 ± 7 anos, sendo 61,6% deles do sexo feminino. A presença de níveis elevados de pressão arterial sistólica (153,4 ± 18 versus 170 ± 26 mmHg), de albuminúria de 24h > 30mg (55,3% versus 82,6%) e de TFG < 60 ml/min/m2 (12,8% versus 33,3%) foi significativamente maior (p < 0.05) entre os pacientes do Grupo 2. O desfecho-composto ecocardiográfico foi mais prevalente no grupo 2 (84,0% versus 59,2%; p = 0,006) e a frequência média de lesões de órgãos-alvo também foi maior nos pacientes do grupo 2 (0,36 ± 0,31 versus 0,19 ± 0,19; p = 0,001). Análise por regressão logística binária revelou que o ITB foi uma das variáveis preditoras independentes para o desfecho-composto ecocardiográfico (OR = 3,43; IC 95% = 1,07 - 11,0; p = 0,04). A partir da análise por regressão linear obteve-se um modelo final no qual o ITB foi uma das três variáveis preditoras independentes para a estimativa da frequência média de lesões de órgãos-alvo com coeficiente beta = 13,22 (1,81 - 24,63), ao lado da idade e do infarto prévio. Conclusão: Nossos dados mostram que valores de ITB alterados estão associados à maior prevalência de lesões em órgãos-alvo, principalmente alterações ecocardiográficas, em pacientes com hipertensão arterial e diabetes Introduction: A lot of studies have established strong correlations between the ankle-brachial index (ABI), a marker of subclinical atherosclerosis and cardiovascular prognosis in different populations. However, few studies have assessed the correlation between the values of the ABI and cardiovascular and renal lesions in patients with hypertension and diabetes. Objective: To study the prevalence of cardiac, carotid, renal and retinal morphological and functional changes according to the presence or not of altered ABI values (ABI 1.4) in hypertensive patients with type 2 diabetes. Methods: It was included 99 diabetic hypertensive patients aged between 50 and 80 years. The measurement of the ABI was performed in all patients by validated method and they were classified in Group 1 (normal ABI, n = 49) or group 2 (altered ABI, n = 50). All patients were submitted, up to 6 months, to Doppler echocardiography, carotid ultrasound, color retinography, assessment of glomerular filtration rate (GFR) and 24h albuminuria. Patients were analyzed for the occurrence or not of a composite echocardiographic outcome which included morphological and functional cardiac alterations relevant to clinical practice. Patients in groups 1 and 2 were compared regarding the prevalence of carotid plaques with or without hemodynamic repercussion, TFG < 60 ml/min/m2, 24h albuminuria > 30 mg and the presence or not of retinopathy. Finally, we compared the prevalence of mean frequency of the following end-organ lesions of both groups, considering unit value for each one: left ventricular hypertrophy, hypertensive retinopathy, TFG < 60 ml/min/m2 and internal carotid artery stenosis > 50%. Results: The mean age of the patients was 65.4 ± 7 years, with 61.6% of them female. The presences of elevated levels of systolic blood pressure (153.4 ± 18 versus 170.0 ± 26 mmHg), of 24h albuminuria > 30 mg (55.3% versus 82.6%) and TFG < 60 ml/min/m2 (12.8% vs. 33.3%) were significantly greater (p < 0.05) among the patients of Group 2. The composite echocardiographic outcome was more prevalent in Group 2 (84.0% versus 59.2%, p = 0.006) and the average frequency of subclinical injury of target organs was also greater in patients of Group 2 (0.36 ± 0.31 versus 0.19 ± 0.19; p = 0.001). Binary logistic regression analysis revealed that the ABI was one of the independent predictors of composite echocardiographic outcome (OR = 3.43; IC 95% = 1.07 - 11.0; p = 0.04). From the linear regression analysis it was obtained a final model in which the ABI was one of three independent predictors for the estimation of the average frequency of end-organ damage with ? coefficient = 13.22 (1.81-24.63), besides age and previous myocardial infarction. Conclusion: Our data demonstrates that changed ABI values are associated with higher prevalence of subclinical end-organ lesions, principally changes in echocardiographic parameters, in patients with hypertension and diabetes

As propriedades funcionais e estruturais das grandes artérias tem sido foco de atenção nos últimos anos para o melhor entendimento das alterações fisiopatológicas associadas à hipertensão arterial e ao envelhecimento. Muitas destas alterações como a rigidez arterial, tem sido consideradas marcadores independentes de risco cardiovascular. A avaliação destas propriedades não foi estudada em hipertensos mais graves, onde o risco de complicações é maior. O objetivo do estudo foi avaliar, em pacientes com hipertensão estágio 3, as alterações vasculares estruturais e funcionais em grandes artérias, seus principais determinantes e suas correlações com lesões de órgãos-alvo. Foram avaliados 48 pacientes (idade média 53,6 ± 8 anos; 75% brancos; 71% mulheres), com hipertensão arterial estágio 3, recebendo o mesmo tratamento anti-hipertensivo por um mês. A avaliação dos parâmetros carotídeos (diâmetro, espessura e distensão) foi realizada pelo ultrassom de radiofrequência e a rigidez arterial pela medida da velocidade de onda de pulso (VOP) e pelo Índice Ambulatorial de Rigidez Arterial (IARA). Realizamos ecocardiograma e perfil bioquímico para análise das variáveis e avaliação de lesão de órgãos-alvo. Foram realizadas correlações das variáveis bioquímicas, antropométricas e de lesões em órgãos-alvo com os métodos de avaliação vascular, além da correlação entre os métodos. Observamos uma elevada rigidez arterial tanto pela medida da VOP (12,4 m/s) quanto pelo IARA (0,39), e metade dos pacientes apresentou valores de VOP considerados de pior prognóstico (> 12 m/s). Houve correlação significativa do IARA com a medida da VOP. A distensão da carótida foi menor nos pacientes com diabetes. O principal determinante independente da VOP foi a idade e do IARA os níveis de glicemia. Observou-se correlação significativa e positiva entre a distensão de carótida e o índice de massa de ventrículo esquerdo. Em conclusão, pacientes com hipertensão estágio 3 apresentam alterações importantes das propriedades funcionais de grandes artérias, que são agravadas pelo envelhecimento e pela associação de diabetes, e uma delas está associada à hipertrofia ventricular esquerda presente nestes pacientes. Functional and structural properties of large arteries have been matter of attention for the better understanding of physiopathological modifications associated to arterial hypertension and aging. Most of these alterations, as arterial stiffness, have been considered independent markers of cardiovascular risk. The evaluation of these properties has not yet been studied in severe hypertensives where the risk for complications is high. The aim of our study was to evaluate, in patients with stage 3 arterial hypertension, vascular changes of large arteries, its major determinants and the correlations with end-organ damage. We evaluated 48 patients (mean age 53,6 ± 8 years; 75% white; 71% women), with stage 3 arterial hypertension, under the same antihypertensive treatment for one month. Carotid parameters (diameter, wall thickness, distension) were evaluated by radiofrequency ultrasound and arterial stiffness by measurement of pulse wave velocity (PWV) and ambulatory arterial stiffness index (AASI). It was performed echocardiogram and biochemical profile to evaluation of other variables and end-organ damage. We did correlations among biochemical, anthropometrical variables and end-organ damage with vascular parameters. We observed increased arterial stiffness measured either by PWV values (12,4 m/s) or AASI (0,39), and half of patients had PWV values considered as a poor prognostic marker (> 12 m/s). It was observed a significant correlation between PWV and AASI. Carotid distension was lower in diabetic patients. The main independent determinant of PWV was age while glycemia was the main determinant of AASI. It was noticed a positive and significative correlation between carotid distension and left ventricle mass index. In conclusion, patients with stage 3 hypertension had important modifications of functional properties of large arteries that are impaired by aging and association of diabetes, and one of them is associated to left ventricle hypertrophy present in these patients

Apneia obstrutiva do sono (AOS) é um fator de risco independente para o desenvolvimento de hipertensão arterial sistêmica, principalmente hipertensão arterial resistente, e doenças cardiovasculares. Hipertensão arterial resistente está associada à AOS, níveis elevados de aldosterona e dieta rica em sal. O objetivo do presente estudo foi determinar se a quantidade de sal na dieta e os níveis de aldosterona estão associados com a intensidade da AOS em pacientes com hipertensão arterial resistente. Noventa e sete pacientes com hipertensão arterial resistente foram avaliados com polissonografia noturna assistida e dosagem de aldosterona e sódio em urina coletada por 24h durante a dieta habitual. Hiperaldosteronismo foi definido como atividade da renina plasmática < 1 ng/mL/h e aldosterona urinária >= 12 ?g/24h. Na população total estudada, a média de idade foi 55±9 anos e 47,4% dos pacientes eram do sexo masculino. Em média, os pacientes estavam em uso de 4,3±1,1 medicamentos anti-hipertensivos e a pressão arterial foi 156,3±22,4/88,9±13,3 mmHg. A prevalência de AOS foi de 77,3% na população total estudada. Vinte e oito (28,9%) pacientes foram diagnosticados como portadores de hiperaldosteronismo. A prevalência de AOS foi maior nos pacientes com do que nos pacientes sem hiper-aldosteronismo (82,1% vs 75,4%), mas sem significância estatística. A análise univariada na população total demonstrou que o índice de apneia-hipopneia correlacionou-se positiva e significantemente com a circunferência do pescoço (rho=0,4362, p= 12 ?g/24 h. Overall, mean age was 55±9 years and 47.4% were males. Mean clinic blood pressure was 156.3±22.4/88.9±13.3 mmHg on an average 4.3±1.1 antihypertensive medications. Enquanto há inúmeros trabalhos evidenciando a participação do território arterial na fisiopatologia da Hipertensão Arterial Sistêmica (HAS), pouco ainda se conhece da real participação do território venoso nessa doença. Estudos prévios demonstraram menor complacência venosa até mesmo em pacientes hipertensos limítrofe, e esta alteração não pode ser explicada como sendo apenas conseqüente a alteração do sistema simpático. Acrescidos a estas alterações, foi demonstrada disfunção endotelial no território venoso em pacientes com fatores de risco cardiovascular, incluindo HAS. Entretanto, ainda existem poucas informações sobre a correlação da disfunção endotelial venosa e/ou da capacitância e complacência venosas e seu impacto funcional na HAS. Neste protocolo foram avaliados 27 indivíduos do Grupo Controle (GC) (idade de 36,8±9,2 anos, 13 homens, IMC de 24,6±4,6 Kg/m2) e 31 pacientes do Grupo Hipertenso (GH) (idade de 38,2±10,5 anos, 15 homens e IMC de 26,1±3,1 Kg/m2). Curvas de pressão arterial (PA) foram obtidas de forma não invasiva com o Finometer®, durante 10 minutos de repouso na posição supina (basal) e durante 10minutos em manobra de modulação de volume (Tilt test). A hipertensão arterial freqüentemente está associada com obesidade, alterações metabólicas e cardiovasculares. Embora haja estudos mostrando a correlação dessas alterações com alterações cardíacas estruturais e funcionais, não há dados nesse sentido em pacientes hipertensos graves. O objetivo desse estudo foi avaliar, em pacientes hipertensos graves, variáveis cardíacas estruturais e funcionais e se há associação entre elas e variáveis metabólicas, antropométricas e com a rigidez arterial. Foram avaliados 41 pacientes com hipertensão arterial grave recebendo o mesmo tipo de tratamento anti-hipertensivo durante 30 dias. O ecocardiograma foi usado para avaliar a estrutura e função do coração. A determinação da distensibilidade da aorta foi obtida pela velocidade de onda de pulso. Amostras de sangue venoso foram coletadas em jejum de 12 horas para a avaliação das variáveis bioquímicas. A circunferência de cintura foi obtida com fita métrica flexível de acordo com recomendações internacionais e nacional. Foram realizadas correlações das variáveis ecocardiográficas com as bioquímicas, antropométricas e com a velocidade de onda de pulso. As variáveis bioquímicas que tiveram melhor correlação com dados estruturais e funcionais do coração foram o ácido úrico e triglicérides. O ácido úrico apresentou correlação positiva com massa, índice de massa, diâmetro diastólico do ventrículo esquerdo, espessuras diastólicas do septo e da parede posterior do ventrículo esquerdo, diâmetro da raiz da aorta, e negativa com a variável funcional fração de ejeção. Triglicérides correlacionou-se com as espessuras diastólicas do septo e da parede posterior do ventrículo esquerdo, diâmetros diastólico e sistólico do ventrículo esquerdo, diâmetro da raiz da aorta e do átrio esquerdo, massa e índice de massa do ventrículo esquerdo de forma positiva e velocidade protodiastólica máxima do fluxo mitral e sua razão com a velocidade telediastólica máxima de forma negativa. As correlações persistiram após correção para idade e índice de massa corpórea. A circunferência de cintura e a velocidade da onda de pulso apresentaram correlação com dados estruturais e funcionais do coração. Valores de glicose, HDL-colesterol e LDL-colesterol também foram associados com diferentes variáveis estruturais e funcionais cardíacas. Foi realizado modelo de regressão linear múltipla que considerou como variáveis dependentes os parâmetros do ecocardiograma e como independentes, pressão arterial, os dados bioquímicos, a idade e o índice de massa corpórea. Triglicérides, ácido úrico e índice de massa corpórea tiveram o maior número de associações com as variáveis estruturais cardíacas. O HDL-colesterol e a pressão arterial sistólica mostraram o maior número de associações com as variáveis diastólicas. Outra análise multivariada selecionou idade, pressão arterial sistólica e velocidade da onda de pulso como variáveis independentes e essa última se associou somente com o tempo de relaxamento isovolumétrico. Em pacientes hipertensos graves, os níveis de triglicérides e do ácido úrico apresentaram boa correlação com dados estruturais e funcionais do coração. Glicose, HDLcolesterol e LDLcolesterol também tiveram correlação com dados estruturais e funcionais do coração, porém em menor intensidade. A circunferência de cintura apresentou correlação com alguns dados estruturais e funcionais do coração. Although there are studies correlating these alterations with cardiac structure and function alterations, there is no data regarding this issue in severe hypertensive patients. The aim of this study was to evaluate in severe hypertensive patients structural and functional cardiac variables, and whether there is an association between them and anthropometric, metabolic variables, and pulse wave velocity. Forty-one patients with severe hypertension were evaluated after 30 days taking the same anti-hypertensive medication. Cardiac evaluation was performed with the echocardiogram. Aortic distensibility was obtained by pulse wave velocity. Blood sample was drowning after 12-hours fasting for biochemistry variables. Waist circumference was measured with a flexible metric strip according to international and national recommendations. Correlation between echocardiographic parameters, biochemistry variables, waist circumference, and the pulse wave velocity were done. Uric acid and triglycerides showed better correlation with cardiac structural and functional parameters. Uric acid showed a positive correlation with left ventricle mass, left ventricle mass index, left ventricle diastolic diameter, left ventricle septal and posterior wall thickness, aortic root and a negative correlation with ejection fraction. Triglycerides showed a positive correlation between diastolic septal and posterior wall thickness, systolic and diastolic left ventricle diameters, aortic root and left atrium diameter, left ventricle mass and mass index, and also a negative correlation between peak velocity of early diastolic filling of mitral inflow and your ratio with the peak velocity of late diastolic filling. The correlation persisted after correction for age and body mass index. The waist circumference and the pulse wave velocity also showed correlation with cardiac structural and functional data. Glucose, HDL-cholesterol and LDL-cholesterol values also were associated with myocardial structural and functional different parameters. In a linear regression multiple model, considering echocardiogram parameters as dependent variable and blood pressure, biochemistry data, age, and body mass index independent variables was done. Triglycerides, uric acid, and body mass index showed higher number of association with cardiac structural parameters. HDL-cholesterol and systolic blood pressure showed greater number of association with diastolic variables. In another multivariate analyzes, age, blood pressure, and pulse wave velocity were selected as independent variables and the last one showed association with isovolumic relaxation time. In patients with severe hypertension, triglycerides levels and uric acid showed a good correlation with cardiac structural and functional parameters. Glucose, HDL-cholesterol, and LDL-cholesterol also showed correlation with cardiac structural and functional measures, however it was less strong. The waist circumference showed correlation with cardiac structural and functional parameters.
