Using cross-sectional and longitudinal analyses, we assessed the impact of malnutrition risk on healthcare utilization and costs in a cohort of older adults living in Spanish community.Patients and Methods: Data from 1660 older (range 66– 98 years), community-living adults participating in the Toledo Study on Healthy Ageing, waves 2 (year 2011– 2013) and 3 (year 2015), were analyzed. Nutritional status categories were defined according to the Global Leadership Initiative on Malnutrition (GLIM) criteria, using a two-step approach. First, screening for malnutrition risk. Once positive, individuals were classified as malnourished according to some phenotypic (body mass index, grip strength, and unintentional weight loss) and etiologic (disease burden/inflammation and reduced food intake or assimilation) criteria. Outcomes assessed included healthcare resources (hospital admissions, number of hospitalizations, length of hospital stay per hospitalization, and number of medications).Results: Fifteen percent of the population was found to be at risk of malnutrition, while 12.6% was malnourished. Overall, patients from both groups were older, had lower functional status, and had more comorbidities compared to well-nourished counterparts (p< 0.05). Results of our cross-sectional analysis showed that being at-risk/malnourished was associated with greater medication utilization, higher rates of hospital admission and longer stays, and higher hospitalization costs. However, when adjusting for covariates, malnutrition/risk was associated only with higher hospitalization costs (range: 11– 13%). Longitudinal analysis results indicated that malnutrition/risk was significantly associated with more frequent hospitalizations, longer lengths of stay, higher hospitalization costs, and polypharmacy at follow-up.Conclusion: Malnutrition or its risk, found in over one of four older adults in the Toledo community, was associated with higher healthcare resource use and increased costs. Such findings suggest that malnutrition risk-screening for older adults, and provision of nutrition counseling and care when needed, hold potential to improve their health and to lower costs of care in the Spanish healthcare system.Keywords: malnutrition prevalence, healthcare resource use, costs, oral nutritional supplements, ONS, older adults, community, Spain

Abstract

In this work we made an analysis of the current status of velocity dependent (bound) field components in the framework of the standard electromagnetic theory. Preliminary discussions of the structure of the magnetic field due to an idealized oscillating magnetic dipole provided us with the quantitative insights on the relative contribution of velocity dependent (bound) and acceleration dependent (radiation) terms into the resultant magnetic field. According to this analysis we defined the methodological scheme based on a generalized (time-dependent) Biot-Savart law capable to test the applicability of the standard retardation condition on bound field components. In the second part of this work we made the theoretical analysis of the finite size multi-section antennas, confirming the validity of the methodological scheme conceived for an idealized magnetic dipole. The use of multi-section antennas is fully justified by a substantial rise of the ratio of bound-to-radiation field strength. Finally, we effected numerical calculations taking into account particular experimental settings and compared them with experimentally obtained data that unambiguously indicate on the non-applicability of the standard retardation condition to bound magnetic fields. In addition, experimental observations show a striking coincidence with the predictions of a new generalized Biot-Savart law which implies the spreading velocity of bound fields highly exceeding the velocity of light.

9. On Mathematical Defect in Demonstration of Convection Theorem: Implications For Continuum Mechanics and Other Classical Field Theories

Abstract

Serious mathematical defect in the important kinematics theorem known in continuum mechanics as Convection (or Transport) Theorem is reported. We claim that the traditional demonstration does not take into account a special constraint on integrand functions given in Lagrangian representation. Thus, we put in doubt that the traditional procedure for the transition from integral formulations of physical laws of some classical field theories to their differential form is mathematically rigorous. Reconsidered formulation shows the way how the system of fundamental differential equations of continuum mechanics and some other field theories could be modified without any change of the original integral formulation. The continuity equation and the differential form of the equation of motion for continuous media are discussed as examples of modification.

10. On Essential Incompleteness of Hertz's Experiments on Propagation of Electromagnetic Interactions

Abstract

The historical background of the 19th century electromagnetic theory is revisited from the standpoint of the opposition between alternative approaches in respect to the problem of interactions. The 19th century electrodynamics became the battle-field of a paramount importance to test existing conceptions of interactions. Hertz's experiments were designed to bring a solid experimental evidence in favor of one of them. The modern scientific method applied to analyze Hertz's experimental approach as well as the analysis of his laboratory notes, dairy and private letters show that Hertz's "\textit{crucial}" experiments cannot be considered as conclusive at many points as it is generally implied. We found that alternative Helmholtz's electrodynamics did not contradict any of Hertz's experimental observations of transverse components as Maxwell's theory predicted. Moreover, as we now know from recently published Hertz's dairy and private notes, his first experimental results indicated clearly on infinite rate of propagation. Nevertheless, Hertz's experiments provided no further explicit information on non-local longitudinal components which were such an essential feature of Helmholtz's theory. Necessary and sufficient conditions for a decisive choice on the adequate account of electromagnetic interactions are discussed from the position of modern scientific method.

11. On Two Complementary Types of Total Time Derivative in Classical Field Theories and Maxwell's Equations

Abstract

Close insight into mathematical and conceptual structure of classical field theories shows serious inconsistencies in their common basis. In other words, we claim in this work to have come across two severe mathematical blunders in the very foundations of theoretical hydrodynamics. One of the defects concerns the traditional treatment of time derivatives in Eulerian hydrodynamic description. The other one resides in the conventional demonstration of the so-called Convection Theorem. Both approaches are thought to be necessary for cross-verification of the standard differential form of continuity equation. Any revision of these fundamental results might have important implications for all classical field theories. Rigorous reconsideration of time derivatives in Eulerian description shows that it evokes Minkowski metric for any flow field domain without any previous postulation. Mathematical approach is developed within the framework of congruences for general 4-dimensional differentiable manifold and the final result is formulated in form of a theorem. A modified version of the Convection Theorem provides a necessary cross-verification for a reconsidered differential form of continuity equation. Although the approach is developed for one-component (scalar) flow field, it can be easily generalized to any tensor field. 14. On Two Complementary Types of Directional Derivative and Flow Field Specification in the Classical Field Theory

Abstract

We discuss a general definition of directional derivative of any tensor flow field and its practical applications in physics. It is shown that both Lagrangian and Eulerian descriptions as complementary types of flow field specifications adopted in modern theoretical hydrodynamics, imply two complementary types of directional derivatives as corresponding mathematical constructions. One of them is the Euler substantive derivative useful only in the contexts of initial Cauchy problem and the other, called here as the local directional derivative, arises only in the context of so-called final Cauchy problem. The choice between Lagrangian and Eulerian specifications is demonstrated to be equivalent to the choice between space-time with Euclidean or Minkowski metric for any flow field domain, respectively. Mathematical consideration is developed within the framework of diffeomorfic transformations for general 4-dimensional differentiable manifold. The analytical expression for local directional derivative is formulated in form of a theorem. Although the consideration is developed for ideal one-component (scalar) flow field, it can be easily generalized to any tensor field. Abstract

The question how to introduce thermal fluctuations in the equation of motion of a magnetic system is addressed. Using the approach of the fluctuation-dissipation theorem we calculate the properties of the noise for both, the fluctuating field and fluctuating torque (force) representation. In contrast to earlier calculations we consider the general case of a system of interacting magnetic moments without the assumption of axial symmetry. We show that the interactions do not result in any correlations of thermal fluctuations in the field representation and that the same widely used formula can be used in the most general case. We further prove that close to the equilibrium where the fluctuation-dissipation theorem is valid, both, field and torque (force) representations coincide, being different far away from it.

Abstract

This article discusses a case report of a 20-year-old patient with cervical agenesis and upper third vaginal atresia. The patient experienced cyclical pelvic pain and was previously diagnosed with partial vaginal atresia. Preoperative imaging revealed various abnormalities, including a thick transverse vaginal septum, a partial septate uterus, and a completely obstructed uterine outflow tract. Intraoperative ultrasound was used to confirm the absence of the cervix and guide the surgical management, which involved a laparoscopic hysterectomy with bilateral salpingectomy. The article highlights the value of intraoperative ultrasound in improving the characterization of complex congenital abnormalities. [Extracted from the article]

Resumen El riesgo de orientación suicida es un estado que antecede a la ideación suicida que puede conducir a un intento de suicidio y posiblemente a un suicidio consumado; para la Salud Pública es un tema de interés por su magnitud y por la presión que ejerce sobre los sistemas de salud. Objetivo: el estudio buscó determinar la asociación del riesgo de orientación suicida con variables sociodemográficas y los indicadores de salud mental: dominios de depresión, funcionamiento familiar, vulnerabilidad familiar y consumo de sustancias psicoactivas en adolescentes escolarizados de la zona noroccidental de Medellín, 2010. Metodología: se realizó un estudio de corte transversal con una muestra aleatoria de 604 estudiantes. Se utilizaron los instrumentos: Inventario de orientación suicida (iso); Inventario de depresión en niños y adolescentes (cdi-la); ¿Cómo es tu Familia?; Entrevista Diagnóstica Internacional Compuesta (cici ii) y el Apgar familiar. Se realizaron un análisis factorial confirmatorio y una regresión logística. Resultados: existe asociación del riesgo de orientación suicida en un adolescente escolarizado con los indicadores: acumulación de tensiones en la familia, escasa búsqueda de apoyo, mal funcionamiento familiar, vulnerabilidad en valores morales y la presencia de sintomatología depresiva. Conclusión: el buen funcionamiento de la familia es una estrategia inmediata muy importante para la salud mental del adolescente escolarizado. Abstract The risk of suicide orientation is a state preceding suicidal ideation that can lead to a suicide attempt and, possibly, to a successful suicide. Given its magnitude and the pressure it exerts on health systems, suicide is considered a topic of interest for public health. Objective: the study sought to determine the association of suicide orientation risk with: sociodemographic variables and mental health indicators such as: domains of depression, family functioning, family vulnerability and psychoactive substance consumption among adolescent students in the northwest area of Medellin, 2010. Methodology: the researchers performed a cross-sectional study with a random sample of 604 students. The following instruments were used: Inventory of Suicide Orientation (iso 30), Children's Depression Inventory (cdi-la), ¿How is your family? Composite International Diagnostic Interview (cidi ii) and the Apgar family test. Similarly, a confirmatory factor analysis and a logistic regression were conducted. Results: there is association between suicide orientation risk among adolescent students and the following indicators: accumulation of stress in the family, poor search for support, poor family functioning, vulnerability in their moral values and presence of depression symptoms. Conclusion: good family functioning is a very important immediate strategy for achieving good mental health among adolescent students.

Abstract

Este artículo forma parte de un proyecto de investigación sobre la toma de decisiones emergentes en la Universidad Politécnica Territorial Andrés Eloy Blanco (UPTAEB) desde la contabilidad gerencial. Para ello, se analizaron algunos elementos integrativos de la Teoría General de la Contabilidad y los Principios de Contabilidad Generalmente Aceptados (PCGA), la Teoría de los Sistemas Sociales, la Teoría Gerencial de Drucker, el sustento para la toma de decisiones respecto a la Teoría de la Probabilidad Subjetiva de Ramsey y la Teoría Prospectiva de Kahneman y Tversky. La recolección de la información se realizó atendiendo al principio de la saturación teórica; se aplicó una entrevista a profundidad a la Coordinación Administrativa y al Jefe de Presupuesto de la institución del estudio, posteriormente se categorizó, codificó y se buscaron las convergencias, divergencias y complementariedad de categorías en el marco de los elementos identificadores del fenómeno indagado. Se seleccionaron, fragmentaron e integraron los conceptos emergentes y se hace mención a la visión ontológica, epistemológica y axiológica. Finalmente y considerando el carácter utópico de la toma de decisiones emergentes basadas en la Contabilidad Gerencial presente en la UPTAEB, en las reflexiones se presentan algunas consideraciones sobre la metáfora: voluntad espiritual de práctica, al someter el proceso de la toma de decisiones emergentes hacia el discernimiento fundamentado en la orientación de Dios.

Item does not contain fulltext, The intestinal microbiota plays a major role in infant health and development. However, the role of the breastmilk microbiota in infant gut colonisation remains unclear. A systematic review was performed to evaluate the composition of the breastmilk microbiota and evidence for transfer to/colonisation of the infant gut. Searches were performed using PUBMED, OVID, LILACS and PROQUEST from inception until 18th March 2020 with a PUBMED update to December 2021. 88 full texts were evaluated before final critique based on study power, sample contamination avoidance, storage, purification process, DNA extraction/analysis, and consideration of maternal health and other potential confounders. Risk of skin contamination was reduced mainly by breast cleaning and rejecting the first milk drops. Sample storage, DNA extraction and bioinformatics varied. Several studies stored samples under conditions that may selectively impact bacterial DNA preservation, others used preculture reducing reliability. Only 15 studies, with acceptable sample size, handling, extraction, and bacterial analysis, considered transfer of bacteria to the infant. Three reported bacterial transfer from infant to breastmilk. Despite consistent evidence for the breastmilk microbiota, and recent studies using improved methods to investigate factors affecting its composition, few studies adequately considered transfer to the infant gut providing very little evidence for effective impact on gut colonisation.

Abstract

Background: A considerable body of evidence accumulated especially during the last decade, demonstrating that early nutrition and lifestyle have long-term effects on later health and disease (“developmental or metabolic programming”). Methods: Researchers involved in the European Union funded international EarlyNutrition research project consolidated the scientific evidence base and existing recommendations to formulate consensus recommendations on nutrition and lifestyle before and during pregnancy, during infancy and early childhood that take long-term health impact into account. Systematic reviews were performed on published dietary guidelines, standards and recommendations, with special attention to long-term health consequences. In addition, systematic reviews of published systematic reviews on nutritional interventions or exposures in pregnancy and in infants and young children aged up to 3 years that describe effects on subsequent overweight, obesity and body composition were performed. Experts developed consensus recommendations incorporating the wide-ranging expertise from additional 33 stakeholders. Findings: Most current recommendations for pregnant women, particularly obese women, and for young children do not take long-term health consequences of early nutrition into account, although the available evidence for relevant consequences of lifestyle, diet and growth patterns in early life on later health and disease risk is strong. Interpretation: We present updated recommendations for optimized nutrition before and during pregnancy, during lactation, infancy and toddlerhood, with special reference to later health outcomes. These recommendations are developed for affluent populations, such as women and children in Europe, and should contribute to the primary prevention of obesity and associated non-communicable diseases.
