A growing interest in teacher identity and decision-making has emerged in language teaching; however, studies where English language teaching (ELT) educators reflect on their own practice are limited. We engaged in a self-study to understand how our experiences influenced our ELT educator identity at a public university in Colombia. A constructivist paradigm enabled us to focus on intra and interpersonal reflection as we created artifacts and met to discuss our teaching experiences. Using collaborative analysis, we developed our main themes represented by a red poppy. Based on the findings, our teaching identity is shaped by our families and teachers (roots), teaching misconceptions (leaves), new teaching experiences (new blossoms), other identities (petals), world views about education (cotton soul), and social justice agenda (seeds of change).

BackgroundMany health care facilities in low- and middle-income countries are inadequately resourced. COVID-19 has the potential to decimate surgical health care services unless health systems take stringent measures to protect health care workers from viral exposure and ensure the continuity of specialized care for patients. Among these measures, the timely diagnosis of COVID-19 is paramount to ensure the use of protective measures and isolation of patients to prevent transmission to health care personnel caring for patients with an unknown COVID-19 status or contact during the pandemic. Besides molecular and antibody tests, chest computed tomography (CT) has been assessed as a potential tool to aid in the screening or diagnosis of COVID-19 and could be valuable in the emergency care setting. ObjectiveThis paper presents the protocol for an umbrella review that aims to identify and summarize the available literature on the diagnostic accuracy of chest CT for COVID-19 in trauma surgery patients requiring urgent care. The objective is to inform future recommendations on emergency care for this category of patients. MethodsWe will conduct several searches in the L·OVE (Living Overview of Evidence) platform for COVID-19, a system that performs automated regular searches in PubMed, Embase, Cochrane Central Register of Controlled Trials, and over 30 other sources. The search results will be presented according to PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analysis). This review will preferentially consider systematic reviews of diagnostic test accuracy studies, as well as individual studies of such design, if not included in the systematic reviews, that assessed the sensitivity and specificity of chest CT in emergency trauma surgery patients. Critical appraisal of the included studies for risk of bias will be conducted. Data will be extracted using a standardized data extraction tool. Findings will be summarized narratively, and the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) approach will be used to grade the certainty of evidence. ResultsEthics approval is not required for this systematic review, as there will be no patient involvement. The search for this systematic review commenced in October 2020, and we expect to publish the findings in early 2021. The plan for dissemination is to publish the findings in a peer-reviewed journal and present our results at conferences that engage the most pertinent stakeholders. ConclusionsDuring the COVID-19 pandemic, protecting health care workers from infection is essential. Up-to-date information on the efficacy of diagnostic tests for detecting COVID-19 is essential. This review will serve an important role as a thorough summary to inform evidence-based recommendations on establishing effective policy and clinical guideline recommendations. This study delves into the total factor productivity growth in OECD countries, focusing on the crucial role of technological advancement and environmental management. By utilizing the Malmquist-Luenberger index, the paper encompasses both positive and negative outputs, such as pollution, providing a comprehensive productivity analysis by breaking it down into efficiency and technical change. Data from 36 OECD countries from 2000 to 2021 were examined to uncover trends and patterns in productivity growth and its unintended environmental consequences. The results emphasize the dominant influence of technological progress, particularly after 2006, as the primary driver of productivity growth, surpassing improvements in technical efficiency. A significant increase in technical change (1.56 in 2021) compared to technical efficiency (1.05) underscores the importance of sustained investment in research and development (R&D), which correlates positively with patent generation and technological advancement. The study also illustrates that OECD countries have effectively integrated eco-efficient practices, aligning with global trends in environmentally conscious productivity analyses. By integrating environmental outputs such as PM2.5 pollution, the analysis demonstrates that countries mitigating these adverse effects achieve higher productivity growth. These findings challenge conventional productivity models, where productivity diminishes when environmental aspects are considered. The analysis emphasizes the necessity for tailored policy approaches to address disparities in R&D investments, technological adoption, and eco-efficiency among countries. Countries with more significant R&D investments consistently demonstrate superior technological advancement in patents (0.745). Policymakers are urged to prioritize long-term strategies that foster technological innovation and environmental sustainability to ensure sustained productivity growth and economic resilience.

Introduction: Nephroblastoma, or Wilms’ tumor, is a malignant renal neoplasm commonly found in children, is extremely rare in adults representing only 0.5% of all renal neoplasms. Adult Wilms tumor is rare, to our knowledge fewer than 300 cases have been reported in the English literature to date. However, in older adults after 60 years of age, only less than 45 cases have been reported. For this reason, treatment guidelines in adults still are lacking. Prognosis in nephroblastoma for adult patients is found to be worse than in children. Case Presentation: We report the case of a 65-year-old female with lumbar fossa mass, flank pain and hematuria, and pathologic diagnosis of Wilms tumor. We performed nephrectomy. No adjuvant treatment was given. Our patient remains asymptomatic and without evidence of recurrence 12 months after the surgery. Conclusion: Nephroblastoma in the elderly presents different clinical behavior and prognosis compared to nephroblastoma in children.

Abstract Background Ticks are obligate hematophagous ectoparasites involved in transmitting viruses of public health importance. The objective of this work was to identify the Jingmen tick virus in hard ticks from the Colombian Caribbean, an arbovirus of importance for public health. Methods Ticks were collected in rural areas of Córdoba and Cesar, Colombia. Taxonomic identification of ticks was carried out, and pools of 13 individuals were formed. RNA extraction was performed. Library preparation was performed with the MGIEasy kit, and next-generation sequencing (NGS) with MGI equipment. Bioinformatic analyses and taxonomic assignments were performed using the Galaxy platform, and phylogenetic analyses were done using IQ-TREE2. Results A total of 766 ticks were collected, of which 87.33% (669/766) were Rhipicephalus microplus, 5.4% (42/766) Dermacentor nitens, 4.2% (32/766) Rhipicephalus linnaei, and 3.0% (23/766) Amblyomma dissimile. Complete and partial segments 1, 2, 3, and 4 of Jingmen tick virus (JMTV) were detected in the metatranscriptome of the species R. microplus, D. nitens, and A. dissimile. The JMTVs detected are phylogenetically related to JMTVs detected in Aedes albopictus in France, JMTVs detected in R. microplus in Trinidad and Tobago, JMTVs in R. microplus and A. variegatum in the French Antilles, and JMTVs detected in R. microplus in Colombia. Interestingly, our sequences clustered closely with JMTV detected in humans from Kosovo. Conclusions JMTV was detected in R. microplus, D. nitens, and A. dissimile. JMTV could pose a risk to humans. Therefore, it is vital to establish epidemiological surveillance measures to better understand the possible role of JMTV in tropical diseases. Graphical Abstract

Abstract Background In Colombia and worldwide, breast cancer (BC) is the most frequently diagnosed neoplasia and the leading cause of death from cancer among women. Studies predominantly involve hereditary and familial cases, demonstrating a gap in the literature regarding the identification of germline mutations in unselected patients from Latin-America. Identification of pathogenic/likely pathogenic (P/LP) variants is important for shaping national genetic analysis policies, genetic counseling, and early detection strategies. The present study included 400 women with unselected breast cancer (BC), in whom we analyzed ten genes, using Whole Exome Sequencing (WES), know to confer risk for BC, with the aim of determining the genomic profile of previously unreported P/LP variants in the affected population. Additionally, Multiplex Ligation-dependent Probe Amplification (MLPA) was performed to identify Large Genomic Rearrangements (LGRs) in the BRCA1/2 genes. To ascertain the functional impact of a recurrent intronic variant (ATM c.5496 + 2_5496 + 5delTAAG), a minigene assay was conducted. Results We ascertained the frequency of P/LP germline variants in BRCA2 (2.5%), ATM (1.25%), BRCA1 (0.75%), PALB2 (0.50%), CHEK2 (0.50%), BARD1 (0.25%), and RAD51D (0.25%) genes in the population of study. P/LP variants account for 6% of the total population analyzed. No LGRs were detected in our study. We identified 1.75% of recurrent variants in BRCA2 and ATM genes. One of them corresponds to the ATM c.5496 + 2_5496 + 5delTAAG. Functional validation of this variant demonstrated a splicing alteration probably modifying the Pincer domain and subsequent protein structure. Conclusion This study described for the first time the genomic profile of ten risk genes in Colombian women with unselected BC. Our findings underscore the significance of population-based research, advocating the consideration of molecular testing in all women with cancer.

BACKGROUND: Intracranial pressure (ICP) monitoring is widely practiced, but the indications are incompletely developed, and guidelines are poorly followed. OBJECTIVE: To study the monitoring practices of an established expert panel (the clinical working group from the Seattle International Brain Injury Consensus Conference effort) to examine the match between monitoring guidelines and their clinical decision-making and offer guidance for clinicians considering monitor insertion. METHODS: We polled the 42 Seattle International Brain Injury Consensus Conference panel members ICP monitoring decisions for virtual patients, using matrices of presenting signs (Glasgow Coma Scale [GCS] total or GCS motor, pupillary examination, and computed tomography diagnosis). Monitor insertion decisions were yes, no, or unsure (traffic light approach). We analyzed their responses for weighting of the presenting signs in decision-making using univariate regression. RESULTS: Heatmaps constructed from the choices of 41 panel members revealed wider ICP monitor use than predicted by guidelines. Clinical examination (GCS) was by far the most important characteristic and differed from guidelines in being nonlinear. The modified Marshall computed tomography classification was second and pupils third. We constructed a heatmap and listed the main clinical determinants representing 80% ICP monitor insertion consensus for our recommendations. CONCLUSION: Candidacy for ICP monitoring exceeds published indicators for monitor insertion, suggesting the clinical perception that the value of ICP data is greater than simply detecting and monitoring severe intracranial hypertension. Monitor insertion heatmaps are offered as potential guidance for ICP monitor insertion and to stimulate research into what actually drives monitor insertion in unconstrained, real-world conditions.

Los consumidores suelen asociar sus preferencias con sabores específicos según la procedencia de los alimentos y bebidas; sin embargo, no siempre se consideran estas diferencias cuando se exportan dichos alimentos. Esta investigación presenta los resultados de un estudio diseñado para evaluar diferencias interculturales en la percepción de los consumidores mexicanos y colombianos de características organolépticas (gusto general, frescura, intensidad de color, intensidad de aroma, acidez) entre el arándano cultivar biloxi cultivado en México y Colombia. Se diseñó un estudio experimental basado en un cuestionario en el que 397 consumidores de ambos países (192 en México, 205 en Colombia) evaluaron distintos atributos sensoriales de dos muestras: un arándano mexicano y uno colombiano, ambos de la variedad biloxi. Adicionalmente, se realizó un análisis de microexpresiones faciales para complementar los resultados del experimento con indicadores de valencia emocional. Se demostró que ciertas correspondencias entre las propiedades organolépticas son consistentes en ambas culturas. Específicamente, en ambas nacionalidades, se evidencia un efecto significativo en los atributos acidez, amargura y dulzura. El arándano colombiano fue percibido más dulce, mientras que el arándano mexicano fue evaluado como más amargo y ácido. Cuando el arándano evaluado fue clasificado como más ácido, la calificación de gusto general fue más baja. Estos resultados son relevantes para los productores, comercializadores y exportadores de arándanos, sabiendo que aquellas variedades más dulces serán las que tendrán mayor aceptación por parte de los consumidores colombianos y mexicanos.

Objetivo: el coronavirus del síndrome respiratorio agudo grave de tipo 2 (SARS-CoV-2) causa un gran espectro de síntomas, dentro de los que se incluyen manifestaciones gastrointestinales como náuseas, vómito, diarrea y dolor abdominal. El objetivo del presente estudio fue evaluar si la presencia o no de dichos síntomas gastrointestinales podía estar relacionada con una progresión más grave de la enfermedad, entendida como muerte, necesidad de manejo en unidad de cuidados intensivos, ventilación mecánica o coinfección bacteriana. Pacientes y métodos: se realizó un estudio observacional analítico de tipo cohorte con pacientes con covid-19 confirmado con síntomas gastrointestinales y un grupo comparador sin síntomas gastrointestinales. Los pacientes incluidos debían ser mayores de 18 años, que acudieran por sintomatología sugestiva de covid-19 y que se les confirmara el diagnóstico mediante una prueba de reacción en cadena de la polimerasa. Resultados: se analizó a un total de 414 pacientes que cumplieron con los criterios de selección y se encontró que la mortalidad solo está influenciada por la edad, mientras que la ventilación mecánica y la necesidad de manejo en la unidad de cuidados intensivos están relacionadas con el perfil de función hepática. Sin embargo, no se encontró ninguna diferencia en cuanto a la gravedad con respecto a los pacientes que sí presentaron síntomas gastrointestinales frente a los que no tenían síntomas. Conclusiones: los síntomas gastrointestinales de manera individual no representan un riesgo de progresión de la enfermedad por SARS-CoV-2; sin embargo, sí se debería incluir dentro de la valoración inicial de estos pacientes los niveles de alanina-aminotransferasa (ALT), aspartato-aminotransferasa (AST) y bilirrubina directa (BD), pues aportan un valor en cuanto al pronóstico según los resultados de nuestro estudio.

The collective action of MOCAO, Movimiento en resistencia contra las agresiones oculares del ESMAD (Escuadrón Móvil Antidisturbios) is presented as a social strategy to demand access to justice and the fulfilment of guarantees of reparation and non-repetition in Colombia. A brief account is given of significant events in our trajectory as a social movement, together with our letter of petitions to the national government as victims and survivors of ocular aggressions in the framework of police violence. Although ESMAD today has been reformed under the name of the Unit for Dialogue and Maintenance of Order (UNDMO), we consider that there have not yet been structural changes to ensure that its function is related to protecting the constitutional right to social protest.

El propósito del presente artículo se centra en una revisión documental de los planteamientos de los diferentes autores e investigadores sobre cómo el desempeño académico de los estudiantes se fortalece mediante el desarrollo de competencias emocionales. Para su realización se utiliza la técnica de revisión documental planteada por Hurtado (2008) conocida como Raceer, de esta manera se analizaron treinta (30) fuentes teóricas de: Rendón y Coronel (2019), De la Rosa et al. (2019), Martín et al. (2018), Rodríguez Rosero et al. (2021), García-Ancira (2020), Bisquerra Alzina & Chao Rebolledo (2021), García y Rivera (2019), Sánchez, et al. (2018) entre otras; a partir del estudio de estas se construye una matriz crítica con los alcances de cada uno de los autores en sus diversos planteamientos (nivel hermenéutico), de igual forma, se realiza el contraste de los mismos (nivel paradigmático), hallando como resultado principal que el país más interesado en realizar estudios sobre estas dos variables es España, quienes muestran preocupación por desarrollar acciones que conlleven a la formación integral de los estudiantes y por ende mejorar la calidad educativa y de vida de sus habitantes. Se concluye en el presente artículo que el desarrollo de competencias emocionales es necesario para obtener un buen desempeño académico.

Spatial soil databases can help model complex phenomena in which soils are a decisive factor – for example, evaluating agricultural potential or estimating carbon storage capacity. The Latin America and Caribbean Soil Information System, SISLAC, is a regional initiative promoted by the Food and Agriculture Organization's (FAO) Latin America and the Caribbean Soil Partnership to contribute to sustainable management of soil. SISLAC includes data from 49 084 soil profiles distributed unevenly across the continent, making it the region's largest soil database. In addition, there are other soil databases in the region with about 40 000 soil profiles that can be integrated into SISLAC and improve it. However, some problems hinder its usages, such as the quality of the data and their high dimensionality. The objective of this research is evaluate the quality of the SISLAC data and the other available soil databases to generate a new improved version that meets the minimum quality requirements to be used for different purposes or practical applications. The results show that 15 % of the existing soil profiles had an inaccurate description of the diagnostic horizons and 17 % of the additional profiles already existed in SISLAC; therefore, a total of 32 % of profiles were excluded for these two reasons. Further correction of an additional 4.5 % of existing inconsistencies improved overall data quality. The improved database consists of 66 746 profiles and is available for public use at https://doi.org/10.5281/zenodo.7876731 (Díaz-Guadarrama and Guevara, 2023). This revised version of SISLAC data offers the opportunity to generate information that helps decision-making on issues in which soils are a decisive factor. It can also be used to plan future soil surveys in areas with low density or where updated information is required.
