Abstract

Ever since the first publication of the standard communication protocol for computerassisted electrocardiography (SCP-ECG), prENV 1064, in 1993, by the European Committee for Standardization (CEN), SCP-ECG has become a leading example in health informatics, enabling open, secure, and well-documented digital data exchange at a low cost, for quick and efficient cardiovascular disease detection and management. Based on the experiences gained, since the 1970s, in computerized electrocardiology, and on the results achieved by the pioneering, international cooperative research on common standards for quantitative electrocardiography (CSE), SCP-ECG was designed, from the beginning, to empower personalized medicine, thanks to serial ECG analysis. The fundamental concept behind SCP-ECG is to convey the necessary information for ECG re-analysis, serial comparison, and interpretation, and to structure the ECG data and metadata in sections that are mostly optional in order to fit all use cases. SCP-ECG is open to the storage of the ECG signal and ECG measurement data, whatever the ECG recording modality or computation method, and can store the over-reading trails and ECG annotations, as well as any computerized or medical interpretation reports. Only the encoding syntax and the semantics of the ECG descriptors and of the diagnosis codes are standardized. We present all of the landmarks in the development and publication of SCP-ECG, from the early 1990s to the 2009 International Organization for Standardization (ISO) SCP-ECG standards, including the latest version published by CEN in 2020, which now encompasses rest and stress ECGs, Holter recordings, and protocol-based trials. publishedVersion

Abstract

It is estimated that 100 million electrocardiogram (ECG) recordings are made each year in the US. Over half of these are interpreted with the help of computer analysis. While there have been a few studies that compared the usefulness of some programs, there has been no comprehensive evaluation of the performance of computer programs in interpreting ECGs. A total of 1,220 electrocardiograms were analyzed, representing 382 control patients and patients with a variety of heart disorders including heart attacks and heart failure. The ECG recordings were analyzed by eight cardiologists and nine computer programs. Both sets of diagnoses based on the ECGs were compared with diagnoses made on the basis of clinical findings independent of the electrocardiographic results. The study revealed that the computer programs fell short of the cardiologists, but not by much. The computer programs correctly identified 91.3 percent of the cases, compared with 96 percent for the cardiologists. The performance of the computer programs varied for different heart problems; the sensitivity of the programs for diagnosing right ventricular hypertrophy was 31.8 percent and 46.6 percent for the cardiologists. Similarly, the sensitivity of the programs for diagnosing myocardial infarction was 77.1 percent and 84.9 percent for the cardiologists. It should be emphasized that there was considerable variation among the computer programs in the analysis of ECG recordings. International audience; New medical devices and services enabling citizen‚Äôs health care anywhere and anytime are expected in the near future. However, to become a reality these devices must be supported by personalised services which satisfy user needs. In this paper we propose a general approach to manage the complexity of ambiguously related information for providing enhanced, user-specific services in the self-care domain. Abstract

This project aims to demonstrate the feasibility of run-time mission-oriented assessment of assurance and tradeoff in a distributed system. Distributed mission support systems today incorporate various QoS and security mechanisms, but there is no clear way to judge whether the system is delivering QoS and security assurance at the level needed for mission success. Security is treated as an all or nothing quality--which leads to missed opportunities as well as undue risk taking. Measuring security is difficult. QIAAMU took an approach similar to QoS management where assurance is measured against requirements specified by mission stakeholders. This approach enables us to treat QoS and security in a uniform way as well as consider QoS-Security tradeoffs when the system fails to deliver QoS and Security at the required level. Starting with the metrics to observe and measure, this project developed a foundation for run time assessment and tradeoff including an assessment scheme, an innovative distributed tradeoff algorithm and usability support, and instantiated the assessment and tradeoff framework within a security architecture, which underwent a final demonstration and evaluation phase in December 2012. Abstract

Geckos, as well as many species of insect, have evolved a robust reversible adhesion mechanism, enabling them to traverse rough, smooth, vertical or inverted surfaces. The extraordinary climbing ability of geckos has been attributed to the fine structure of their toe pads which contain arrays of thousands of micronsized setal stalks which are in turn terminated by millions of finger-like spatular pads having nano-scale dimensions. The hierarchical structure of the adhesive hairs provides several levels of compliance and enables them to come into close enough contact with rough surfaces to exploit the normally weak van der Waals force for strong adhesion. MEMS fabrication techniques are capable of producing micrometer and sub-micron features in a massively parallel fashion with a high degree of repeatability. This paper describes the use of such batch processing techniques to produce a synthetic reversible dry adhesive. A potential application for a controllable adhesive system capable of both strong attachment and rapid detachment clearly exists in the design of climbing robotic systems., See also ADM002187. 40. Ambient Intelligence and Pervasive Architecture Designed within the EPI-MEDICS Personal ECG Monitor

Abstract

Recent years have witnessed a growing interest in developing personalized and nonhospital based care systems to improve the management of cardiac care. The EPI-MEDICS project has designed an intelligent, portable Personal ECG Monitor (PEM) embedding an advanced decision making system. We present two of the ambient intelligence models embedded in the PEM: the neural-network based ischemia detection module and the Bayesian-network risk stratification module. Ischemia detection was expanded to take into account the patient ECG, clinical data, and medical history. The neural-network ECG interpretation module and the Bayesian-network risk factors module collaborate through a fuzzy-logic-based layer. We also present two telemedicine solutions that we have designed and in which the PEM is integrated. The first telemedical architecture was created to allow the collection of medical data and their transmission between healthcare providers to get an expert opinion. Abstract

The ITUA project began with the research goals of using Byzantine fault-tolerant protocols to coordinate adaptation to attacks and exploring the use of unpredictability in adaptive responses to confuse and delay the attacker. These main factors distinguish the ITUA approach; 1) dynamic adaptation - intrusions cause charges in the system, and a survivable system must cope with these changes, 2) a defense-enabled application that has an application and mission specific defense strategy, and 3) defense enabling builds the defense in middleware, intermediate between the application and the networks and operating systems on which the applications run. The ITUA approach was to respond and adapt to the effects of a malicious attack while it is in progress. If the defense-enabled application continues correct processing in spite of the attached, the defense was considered to be successful. The report reviews the motivation for the project and its historical context, then summarizes the ITUA research goals, progress toward achieving those goals, and lessons learned from the research. Abstract

The goal of the APOD project was to give software applications an increased resistance against malicious attack even when they run in an environment that is not completely secured. We call any such application "defense enabled". Note that defense enabling is less ambitious than building a secure system: rather than protect the entire system, defense enabling concentrates on the survival and integrity of essential applications, possibly sacrificing other parts of the system to the attacker. Defense enabling also gives priority to some security properties over others: we are much more concerned with defending the integrity of an application's data than its confidentiality. Defense enabling is representation of a relatively recent trend in computer security, often called survivability or 3rd generation security. Several factors distinguish the APOD approach to survivability from others. First, dynamic adaption is a key theme of our approach. Intrusions cause changes in the system, and a survivable system much cope with these changes. As a consequence, defense enabled applications must be very agile and will make use of the flexibility possible in today's dynamic, networked environments. Second, a defense enabled application has a defense strategy that is typically application an mission specific. Such strategies complement and go beyond traditional approaches to security in which protection mechanisms are typically not aware of the applications they aim to protect. Third, defense enabling builds the defense in middleware, intermediate between the application and the networks and operating systems on which the application runs.
