3. The natural history and surgical outcome of patients with scimitar syndrome: a multi-centre European study

Abstract

Aims Treatment decisions in patients with scimitar syndrome (SS) are often challenging, especially in patients with isolated SS who are often asymptomatic and who might be diagnosed accidentally. We queried a large multi-institutional registry of SS patients to evaluate the natural history of this condition and to determine the efficacy of surgical treatment in terms of survival and clinical status. Methods and results We collected data on 485 SS patients from 51 institutions; 279 (57%) patients were treated surgically (STPs) and 206 (43%) were clinically monitored (CMPs). Median age at last follow-up was 11.6 years (interquartile range 4-22 years). Abstract

Background: The optimal management and prognostic factors of postsurgical pulmonary vein stenosis remain controversial. We sought to determine current postsurgical pulmonary vein stenosis outcomes and prognostic factors in a multicentric study in the current era. Methods: Seventy-five patients with postsurgical pulmonary vein stenosis who underwent 103 procedures in 14 European/North American centers (2000-2012) were included retrospectively. A specific pulmonary vein stenosis severity score was developed on the basis of the assessment of each pulmonary vein. End points were death, pulmonary vein reintervention, and restenosis. A univariate and multivariate risk analysis was performed. Results: Some 76% of postsurgical pulmonary vein stenosis occurred after repair of a total anomalous pulmonary venous return. Sutureless repair was used in 42 of 103 procedures (41%), patch veinoplasty was used in 28 procedures (27%), and endarterectomy was used in 16 procedures (16%). Overall pulmonary vein restenosis, reintervention, and mortality occurred in 56% (n = 58/103), 49% (n = 50/103), and 27% (n = 20/75), respectively. Sutureless repair was associated with less restenosis (40% vs 67%; P =.007) and less reintervention (31% vs 61%; P =.003). Mortality after sutureless repair (20%; 7/35) tends to be lower than after nonsutureless repair (33%; 13/40) (P =.22). A high postoperative residual pulmonary vein stenosis score at the time of hospital discharge was an independent risk factor for restenosis (hazard ratio [HR], 1.55; P < 10−4), reintervention (HR, 1.33; P < 10−4), and mortality (HR, 1.37; P < 10−4). The sutureless technique was an independent protective factor against restenosis (HR, 0.27; P =.006). Conclusions: Postsurgical pulmonary vein stenosis still has a guarded prognosis in the current era. The sutureless technique is an independent protective factor against restenosis. The severity of the residual disease evaluated by a new severity score is an independent risk factor for poor outcomes regardless of surgical technique.

Abstract

Obturator artery bleeding is rare. During the acute phase of a severe pelvic trauma, massive bleeding can occur immediately. The pelvic fracture often leads to an arterial injury by the broken bone fragments. Our case was unusual due to the absence of any pelvic bone fracture and the late onset of an isolated groin haematoma. The duplex ultrasound and CT scan performed at the admission did not show any acute bleeding but a small groin haematoma. One month later the patient has a major haematoma secondary to an obturator artery pseudo-aneurysm. We successfully performed a selective obturator artery pseudo-aneurysm coil embolization. © 2005 Elsevier Ltd. All rights reserved.

44. Ross procedure in congenital patients: results and left ventricular function.

Abstract

From April 1990 to August 1995, 121 patients (median age 42 years) underwent aortic valve replacement with allografts (69 patients) or autografts (52 patients). In this latter group, 24 Ross procedures have been performed in congenital patients since November 1991 (median age 10 years, range five months to 27 years): aortic incompetence (n = 17), isolated aortic stenosis (n = 5), small stenotic prosthesis (n = 2). Transthoracic echocardiography was obtained preoperatively in all patients and serially after surgery with the aim of measuring aortic and pulmonary annuli and evaluate gradients and incompetence and to study the left ventricular function. Intraoperative transoesophageal echocardiography was routinely used. Complete root replacement was performed in all patients.

Abstract

From November 1980 to November 1986, 63 patients aged 4 months to 13 years (mean 3.4 years) underwent repair of anomalies of ventriculo-arterial connection with ventricular septal defect and pulmonary outflow tract obstruction, using a technique (REV) first described by us in 1982. The selection of patients was based on preoperative criteria, namely the measurement of the distance between the tricuspid and the semilunar valves. These measurements enabled us to select from patients with an abnormal ventriculo-arterial connection, those in whom the anomaly could be repaired by intra-ventricular partition alone. In the remaining cases, REV was indicated in the presence of pulmonary stenosis. The principles of the technique are: (1) resection of the infundibular septum creating a large, direct and subarterial communication between the left ventricle and the aorta; (2) construction of a straight left ventricle to aorta tunnel by intraventricular partition; (3) direct anastomosis of the pulmonary trunk to the right ventricle. There were 12 hospital deaths (19%). The mean follow-up was 32 months. One patient died suddenly 1 year after repair. Six patients required reoperation. All survivors are in NYHA class I, except for 3 patients who are in class II. No stenosis of the left ventricular outflow tract was found but 5 patients had a significant pressure gradient at the pulmonary outflow tract level. Our present experience suggests that in properly selected patients, REV allows anatomic repair in a wide variety of anomalies of the ventriculo-arterial connection associated with VSD and pulmonary outflow tract obstruction with an acceptable rate of mortality and morbidity.

48. To switch or not to switch? The Senning alternative.

Abstract

We analyzed the fate of 100 consecutive patients with simple arterial transposition who were admitted within the first 2 weeks of life to the Hospital for Sick Children during the period 1978-1986. In contrast to current practice, these patients were managed with the objective of performing a Senning operation as the definitive procedure. Five of these 100 patients died on the first day of life of sequelae of perinatal hypoxia, four died within the first month of life (often of complications of catheterization and septostomy), and three died later than the first month but before definitive operation (12% preoperative mortality). There were four operative deaths, and two late deaths have occurred to date. Actuarial survival was 84% at 5 years and 81% at 9 years. The postoperative attrition rate was 0.4 deaths per 100 patient-years. Actuarial survival of all patients treated for simple arterial transposition seems to be the best index for comparing arterial switch with interatrial repair protocols, because each protocol is associated with different patterns of preoperative, perioperative, and postoperative mortality.
