Statement of problem Rehabilitation of edentulous patients with severe maxillary atrophy is a clinical challenge. Although several treatment options are available, the implant-supported overdenture has been widely advocated. However vertical insertion implant-supported overdentures have drawbacks, including wear, aging, and the continuous maintenance of retention systems. Whether an implant-supported overdenture with a horizontal insertion pathway can overcome these problems is unclear. Purpose The purpose of this observational study was to determine the clinical and biological behavior of a new type of implant-supported overdenture with a horizontal insertion pathway over a mean clinical follow-up period of 6 years. Material and methods Ten study participants with an atrophic maxilla underwent rehabilitation with horizontal overdentures supported by 6 implants. Follow-up visits were scheduled at 3 months, 6 months, and annually thereafter. The variables analyzed were implant and prosthetic survival, peri-implant tissue behavior, mechanical complications, and patient satisfaction. Results The follow-up period ranged from 5 to 8 years, mean ±standard deviation 6.3 ±1.2 years. A total of 60 implants and 10 horizontal overdentures were placed. The clinical survival of the implants was 98.3% (mean peri-implant bone loss, 1.3 ±0.4 mm) and was 80% of the prostheses. Overall, mean patient satisfaction as assessed with a visual analog scale was 7.4, with 8.5 for esthetics and 9.5 for function. Conclusions The horizontal implant-supported overdenture provided a functional alternative to conventional prostheses.

Introduction: Today’s dentistry frequently employs bonded partial restorations, which are usually fabricated in ceramic materials. In the last decade, hybrid materials have emerged that attempt to combine the properties of composites and ceramics. Objectives: To evaluate in vitro, by means of a microtensile test, the bond strength between CAD-CAM restorative materials and the cement recommended by their manufacturer. Material and Method: From blocks of CAD-CAM restorative material bonded to composite blocks (Filtek 500®), beams with a bonding area of approximately 1 mm2 were made and divided into four groups: EMAX (IPS e.max CAD® lithium disilicate), VE (VITA Enamic® polymer-infiltrated ceramic matrix), LUA (Lava Ultimate® nano-ceramic resin with sandblasting protocol) and LUS (Lava Ultimate® nano-ceramic resin with silica coating protocol). In each group, perimeter (external) or central (internal) beams were differentiated according to the position in the block. The samples were tested on the LMT 100® microtensile machine. Using optical microscopy, the fractures were categorized as adhesive or cohesive (of the restorative material or composite), and the data were analysed with parametric tests (ANOVA). Results: The LUS group had the highest results (42 ± 20 MPa), followed by the LUA group (38 ± 18 MPa). EMAX had a mean of 34 ± 16 MPa, and VE was the lowest in this study (30 ± 17 MPa). In all groups, the central beams performed better than the perimeter beams. Both EMAX and VE had the most adhesive fractures, while LUA and LUS had a predominance of cohesive fractures. Conclusions: Lava Ultimate® nanoceramic resin with the silica coating protocol obtains the best bond strength values.

Statement of problem Anterior veneered zirconia partial fixed dental prostheses (FDPs) have substituted for metal-ceramic to improve esthetics and biocompatibility. However, the material is susceptible to aging or hydrothermal degradation and to chipping of the feldspathic veneer. Whether these susceptibilities limit the clinical performance of anterior veneered zirconia FPDs is unclear. Purpose The purpose of this prospective clinical study was to analyze the mechanical and biologic behavior of zirconia partial FDPs in the anterior region over a 12-year follow-up period. Material and methods Twenty-seven 3- to 6-unit FDPs fabricated from zirconia veneered with feldspathic porcelain were placed in the anterior region and examined clinically at 1 month, 6 months, and then annually for 12 years, recording mechanical and biologic outcomes. Raw complication rates and time-to-event Kaplan-Meier analysis was conducted and compared as per the partial fixed dental prosthesis type. Results Five FDPs had biologic complications (3 with secondary caries, 1 with periapical pathology, 1 with periodontal disease) and 9 had mechanical complications (7 with chipping and 2 with decementation). Chipping was the most prevalent complication and increasingly found with longer-span fixed partial prostheses (P=.007). Five FDPs had to be replaced because of complete failure, the most frequent cause of failure being secondary caries (P=.003). The mean survival rate (no failures) was 11 years 4 months (95% confidence interval, 10.3-12.7). Conclusions Zirconia FDPs had an 81.5% survival rate over 12 years. A higher incidence of complications took place during the first 3 years, the most common being chipping.

STATEMENT OF PROBLEM Print orientation may affect the manufacturing accuracy of vat-polymerized diagnostic casts. However, its influence should be analyzed based on the manufacturing trinomial (technology, printer, and material) and printing protocol used to manufacture the casts. PURPOSE The purpose of this in vitro study was to measure the influence of different print orientations on the manufacturing accuracy of vat-polymerized polymer diagnostic casts. MATERIAL AND METHODS A standard tessellation language (STL) reference file containing a maxillary virtual cast was used to manufacture all specimens using a vat-polymerization daylight polymer printer (Photon mono SE. LCD 2K) and a model resin (Phrozen Aqua Gray 4K). All specimens were manufactured using the same printing parameters, except for print orientation. Five groups were created depending on the print orientation: 0, 22.5, 45, 67.5, and 90 degrees (n=10). Each specimen was digitized using a desktop scanner. The discrepancy between the reference file and each of the digitized printed casts was measured using the Euclidean measurements and root mean square (RMS) error (Geomagic Wrap v.2017). Independent (unpaired) sample t tests and multiple pairwise comparisons using the Bonferroni test were used to analyze the trueness of the Euclidean distances and RMS data. STATEMENT OF PROBLEM Digital systems including intraoral scanners (IOSs) and optical jaw tracking systems can be used to acquire the maxillomandibular relationship at the centric relation (CR). However, the discrepancy of the maxillomandibular relationship recorded at the CR position when using digital methods remains uncertain. PURPOSE The purpose of this clinical study was to compare the accuracy of the maxillomandibular relationship recorded at the CR position using a conventional procedure, 4 different IOSs, and an optical jaw tracking system. MATERIAL AND METHODS A completely dentate volunteer was selected. A Kois deprogrammer (KD) was fabricated. Six groups were created based on the technique used to obtain diagnostic casts and record the maxillomandibular relationship at the CR position: conventional procedures (CNV group), 4 IOS groups: TRIOS4 (TRIOS4 group), iTero Element 5D (iTero group), i700 wireless (i700 group), Primescan (Primescan group), and a jaw tracking system (Modjaw) (Modjaw group) (n=10). In the CNV group, conventional diagnostic stone casts were obtained. A facebow record was used to mount the maxillary cast on an articulator (Panadent). The KD was used to obtain a CR record for mounting the mandibular cast, and the mounted casts were digitized by using a scanner (T710) to acquire the reference scans. In the TRIOS group, intraoral scans were obtained and duplicated 10 times. The KD was used to obtain a bilateral virtual occlusal record at the CR position. To acquire the specimens of the iTero, i700, and Primescan groups, the procedures in the TRIOS4 group were followed, but with the corresponding IOS. In the Modjaw group, the KD was used to record and export the maxillomandibular relationship at the CR position. Articulated virtual casts of each group were exported. Thirty-six interlandmark linear measurements were computed on both the reference and experimental scans. The distances obtained on the reference scan were used to calculate the discrepancies with the distances obtained on each experimental scan. The data were analyzed by using 1-way ANOVA followed by the pairwise comparison Tukey tests (α=.05). RESULTS The trueness and precision of the maxillomandibular relationship record were significantly affected by the technique used (P.05). The i700 group obtained the lowest trueness and precision values (0.40 ±0.22 mm) of all groups tested, followed by the Primescan grop (0.26±0.13 mm); however, the i700 and Primescan groups had significantly lower trueness and precision than only the iTero group (P

BACKGROUND The efficiency of the mandibular advancement device (MAD) in patients with obstructive sleep apnoea syndrome (OSAS) has been demonstrated. Nevertheless, the behaviour of the upper airway once MAD is placed and titrated, and its correlation with the apnoea-hypopnoea index (AHI) is still under discussion. OBJECTIVES To analyse the morphological changes of the upper airway through a bi- and three-dimensional study and correlate it with the polysomnographic variable, AHI. METHODS Patients were recruited from two different hospitals for the treatment of OSAS with a custom-made MAD. A cone-beam computer tomography and a polysomnography were performed at baseline and once the MAD was titrated. RESULTS A total of 41 patients completed the study. Treatment with MAD reduced the AHI from 22.5 ± 16.8 to 9.2 ± 11.6 (p ≤ .05). There was a significant increase of the total airway volume with MAD from 21.83 ± 7.05 cm3 to 24.19 ± 8.19 cm3 , at the expense of the oropharynx. Moreover, the correlation between the improvement of the AHI and the augmentation of the volume of the upper airway was not statistically significant. CONCLUSIONS The oral device used in this prospective study increased the mean upper pharyngeal airway volume and significantly reduced the AHI. Future studies that measure the muscular tone are needed to completely understand the association between the AHI and the physiological and anatomical response of the upper airway.

Statement of problem Evidence of the behavior of the periodontal tissues around anterior teeth restored with the biologically oriented preparation technique (BOPT) is available. However, outcomes of this technique in posterior teeth restored with fixed partial dentures (FPDs) are lacking. Purpose The purpose of this randomized controlled clinical trial was to evaluate the clinical, mechanical, and biological behavior of posterior 3-unit FPDs placed on teeth prepared with BOPT. Material and methods Forty participants received a 3-unit zirconia FPD in the posterior region of the mandible or maxilla. Twenty FPDs were placed on teeth prepared with BOPT (study group) and 20 on teeth with a horizontal chamfer finishing line (control group). Follow-up examinations were performed 1, 3, and 5 years after treatment to evaluate periodontal responses around the prepared teeth by means of the following parameters: plaque index, gingival index, probing depth, and marginal stability (MS). Mechanical behavior was also assessed, as were any complications. Results After the 5-year follow-up, 57.9% of the control group and 35% of the BOPT group presented a plaque index of 1. The gingival index was 1 in 68.4% of the control group and 30% of the BOPT group after the follow-up period. In the analysis of probing depth, 26.3% of teeth in the control group had pockets of more than 3 mm in depth, whereas the BOPT group had only 10%. Marginal stability appeared in 100% of the BOPT group, whereas only 10.5% of the control group exhibited gingival stability. Complications during the follow-up period were similar, 20% in the control group and 15% in the BOPT group. Conclusions Posterior FPDs prepared by using BOPT had a good clinical response over a 5-year follow-up, with a low gingival index, a small increase in pocket depth, and a 100% marginal stability of the surrounding tissues. High survival rates after 5 years indicated that the technique produced predictable outcomes.

This report describes a prosthetically-driven implant planning method, guided by the alignment procedures between the cone beam computed tomography, intraoral digital scans, and digitized maxillary and mandibular interim complete dentures using intraoral composite resin markers as a common reference. The markers were attached to the keratinized oral mucosa of the edentulous ridges using cyanoacrylate and kept in place during the digitizing procedures. The technique provides a simpler and more economical alternative to conventional prosthetically-driven static implant planning methods.

Tooth isolation is essential to isolate a prepared tooth from saliva, oral humidity, and gingival fluids and facilitate the removal of excess subgingival cement in adhesive cementation procedures. However, the isolation of prepared teeth can be challenging, especially with vertical preparations. A technique for achieving relative isolation by using polytetrafluoroethylene tape is described for adhesive cementation with vertical tooth preparations.

Objective: The main objective of this study aimed to evaluate the response to treatment in implants with peri-implantitis regarding the presence or absence of keratinized mucosa. Materials and Methods: One hundred implants with peri-implantitis were treated non-surgically at the Prosthodontics and Occlusion Teaching Unit of the University of Valencia. Records were registered at day zero (recession, bleeding on probing, suppuration, probing depth, bone loss and plaque index), at 1, 3 and 6 months. Results: In both groups, a slight increase in recession and disappearance of suppuration took place, and no bone loss was observed during the following 6 months. However, after an initial reduction, probing depth, bleeding on probing and plaque index increased again in the group without keratinized mucosa (KM). Conclusions: Implants with KM presented better results after non-surgical treatment as well as in their medium-term evolution, although it would be advisable to increase the number of samples in order to achieve greater scientific evidence and standardization in the treatment protocol. Clinical relevance: The role of keratinized mucosa in the development of peri-implantitis has been mentioned in many publications, but less has been emphasized regarding its influence on the success of the treatment of this pathology. The presence of keratinized mucosa has been found to play a key role in the evolution of the non-surgical treatment of peri-implantitis. The amount of keratinized mucosa should be considered in the treatment of peri-implantitis, as well as when planning connective tissue grafts accompanying the decontamination of implants in the absence of keratinized mucosa.

Purpose To evaluate the clinical behavior and survival of full coverage monolithic zirconia crowns on posterior teeth over a 5-year follow-up. Methods Fifty patients were recruited and underwent restoration with a Lava Plus monolithic zirconia crown (Lava™ Frame Zirconia, 3M Espe, Germany) on premolars or molars. Patients were monitored over a 5-year follow-up (2014-19), recording any biological and/or mechanical complications; these data were used to estimate the crowns' success rate. Periodontal clinical parameters were recorded (pocket probing depth (PPD), plaque index (PLI), bleeding on probing (BOP), and gingival recession (GR)). Wear to the zirconia crowns and antagonist teeth were also evaluated with Geomagic software (3D Systems, U.S.A.). Patients' satisfaction with treatment was evaluated in a questionnaire. Results For the 50 monolithic zirconia crowns analyzed, the survival rate was 98% after 5 years. Only 6% of the crowns presented some type of complication (two debonding and one root fracture). No fracture or fissures were detected. GR and BOP were the only clinical parameters found to be significantly greater around teeth restored with crowns. The monolithic zirconia crowns suffered less wear than the enamel of antagonist teeth. Patient's general satisfaction with treatment was high. Conclusions Monolithic zirconia crowns on posterior teeth are a highly predictable treatment option, with a high survival rate. The single treatment failure was due to a biological complication arising from root fracture. This treatment may be recommended as a treatment that conserves dental structure, and requires minimal dental preparation.

This clinical report describes a chemical abrasive protocol and whitening techniques to quantify modifications to the color and volume produced when eliminating white spots associated with developmental defects in enamel. Chemical (oxidative-erosive) abrasive treatment (whitening + 6.6% hydrochloric acid) resolves white spots up to a depth of 0.2 mm. At greater depths, infiltration with 16% hydrochloric acid followed by composite resin infiltration is recommended.

To assess the influence of implant angulation, humidity, and interimplant distance on the accuracy and scanning time of complete-arch implant scans.A definitive cast with 4 parallel implant abutment analogs (P group), and another cast with 4 angulated (up to 30 degrees) implant abutment analogs (NP group) were digitized by using a scanner (7Series) (reference scans). Two subgroups were created: dry (D subgroup) and wet (W subgroup). For the D subgroup, the casts were digitized without altering the surface humidity by using an intraoral scanner (IOS) (TRIOS 3). For the W subgroup, the cast surface was humidified with artificial saliva and digitized by using the same IOS. The interimplant distance discrepancies were assessed by computing linear and angular measurements. Trueness data was analyzed using 3-way ANOVA followed by the pairwise comparison Tukey tests. The Bartlett test, followed by the pairwise comparison tests, was used to assess the precision (α=.05).Regarding the trueness, implant angulation (P.001) and inter-implant distance measurement (P.001) influenced the linear discrepancies. Implant angulation (P=.002), humidity conditions (P.001), and inter-implant distance (P=.001) influenced the angular discrepancies. Regarding the precision, significant differences in the variance of linear and angular measurements and inter-implant distances were found. Humidity conditions (P.001) influenced the scanning time.Implant angulation, humidity, and interimplant distance influenced the accuracy and scanning time of complete-arch implant scans. Parallel implants resulted in higher trueness and precision values. Dry conditions resulted in slightly higher scanning trueness and precision and shorter scanning time.Drying the surface being scanned increases intraoral scanning accuracy and decreases intraoral scanning time.

(1) Background: Facial scanners are used in different fields of dentistry to digitalize the soft tissues of the patient’s face. The development of technology has allowed the patient to have a 3-dimensional virtual representation, facilitating facial integration in the diagnosis and treatment plan. However, the accuracy of the facial scanner and the obtaining of better results with respect to the manual or two-dimensional (2D) method are questionable. The objective of this clinical trial was to evaluate the usefulness and accuracy of the 3D method (a dual-structured light facial scanner) and compare it with the 2D method (photography) to obtain facial analysis in the maximum intercuspation position and smile position. (2) Methods: A total of 60 participants were included, and nine facial landmarks and five interlandmarks distances were determined by two independent calibrated operators for each participant. All measurements were made using three methods: the manual method (manual measurement), the 2D method (photography), and the 3D method (facial scanner). All clinical and lighting conditions, as well as the specific parameters of each method, were standardized and controlled. The facial interlandmark distances were made by using a digital caliper, a 2D software program (Adobe Photoshop, version 21.0.2), and a 3D software program (Meshlab, version 2020.12), respectively. The data were analyzed by SPSS statistical software. The Kolmogorov–Smirnov test revealed that trueness and precision values were normally distributed (p > 0.05), so a Student’s t-test was employed. (3) Results: Statistically significant differences (p ≤ 0.01) were observed in all interlandmark measurements in the 2D group (photography) to compare with the manual group. The 2D method obtained a mean accuracy value of 2.09 (±3.38) and 2.494 (±3.67) in maximum intercuspation and smile, respectively. On the other hand, the 3D method (facial scanner) obtained a mean accuracy value of 0.61 (±1.65) and 0.28 (±2.03) in maximum intercuspation and smile, respectively. There were no statistically significant differences with the manual method. (4) Conclusions: The employed technique demonstrated that it influences the accuracy of facial records. The 3D method reported acceptable accuracy values, while the 2D method showed discrepancies over the clinically acceptable limits.

Objective: The aim of the systematic review and meta-analysis carried out was to evaluate the effects of changing the prosthetic platform on peri-implant tissue after 1 year of prosthetic loading. Material and methods: In November 2020, an electronic search was carried out in PubMed, EMBASE, Web of Science, and Scopus databases with the aim of obtaining all the randomized clinical trials that had been published in the preceding 10 years comparing the effects on the peri-implant tissue of implants with a prosthetic platform change and implants with a conventional platform for at least 1 year after prosthetic loading. Randomized model meta-analyses of the selected studies were performed to compare the results of the two implant groups in terms of vertical maintenance of bone level and increased probing depth. Results: Nine studies were included, summing up a total of 475 implants with prosthetic platform exchange and 462 implants with a conventional platform. Implants with prosthetic platform exchange had less peri-implant bone loss than implants with a conventional platform (mean difference of 0.255 mm, statistically significant) but suffered a greater increase in probing depth (mean difference of 0.082 mm, not statistically significant). However, the probing depth from One Study Remove revealed a statistically significant increase of 0.190 mm in the prosthetic platform exchange group compared to the conventional platform group. Conclusion: Implants with platform switching suffer less peri-implant bone loss after 1 year of loading than implants with a conventional platform. Further long-term studies are required to observe how these differences vary over time.

Cutting off and rescanning procedures have been shown to affect the accuracy of intraoral scanning; however, the clinical impact of tooth cutting off and rescanning of mesh holes on accuracy remains unclear.The purpose of this clinical study was to evaluate the influence of the tooth location of the rescanned mesh holes (with or without modifying the preexisting intraoral digital mesh with the rescanning procedures) on intraoral scanning accuracy.A maxillary right quadrant digital scan was acquired (control scan) on a dentate participant by using an intraoral scanner (TRIOS 4). The control scan was duplicated 240 times and distributed among 4 groups depending on the location of the rescanned mesh hole: first molar (M group), second premolar (PM group), canine (C group), and central incisor (I group). Each group was divided into 2 subgroups: one subgroup contained overlapping rescanning modifications (WO subgroup), and the other blocked the preexisting digital scan to avoid further modifications when rescanning (NO subgroup) (n=30). A software program (Geomagic) was used to assess the discrepancy between the control and the experimental meshes by using the root mean square (RMS) error calculation. The Shapiro-Wilk test showed that data were not normally distributed. The Kruskal-Wallis test and post hoc Dunn test with Bonferroni correction were used to analyze the RMS mean discrepancies (α=.05). The Levene test was used to analyze the equality of the variances.Trueness ranged from 15 to 17 μm with a precision of 4 μm among the subgroups in which the existing digital scan was blocked, but the trueness ranged from 42 to 72 μm and the precision ranged from 15 to 47 μm among the subgroups in which the rescanning procedures allowed the modification of the existing digital scan. Significant trueness differences were found among the groups tested (P.05). Significant differences in the RMS values were computed between the WO and NO subgroups for each group (M (P.001): PM (P.001); C (P.001), and I (P.001) groups), but the effect of the tooth mesh hole location demonstrated no significant difference either among the WO (P=1.00) or NO subgroups (P=1.00). Furthermore, the NO groups showed markedly better precision than the WO groups for each tooth location. The I-WO group showed better precision than the groups C-WO, PM-WO, and M-WO. However, when no overlapping was allowed, no difference was found in precision between the different tooth locations tested.Rescanning procedures influenced intraoral scanning accuracy. Allowing further modification of the preexisting intraoral digital scan demonstrated a significantly decreased scanning accuracy. However, tooth location of the rescanned mesh hole did not impact scanning accuracy.

Different vertical facial patterns may present different bone and gingival thicknesses at the molar level and can be influenced by the dental compensations that manifest in the presence of transverse bone discrepancies. A retrospective analysis was made of 120 patients divided into three groups according to their vertical facial patterns (mesofacial, dolichofacial or brachyfacial). Each group in turn was divided into two subgroups according to the presence or absence of transverse discrepancies assessed by cone-beam computed tomography (CBCT). The bone and gingival measurements were made integrating a CBCT-3D digital model of the patient dentition. In the brachyfacial patients, the distance from the palatine root to the cortical bone corresponding to the right upper first molar was significantly greater (1.27 mm) than in the dolichofacial (1.06 mm) and mesofacial (1.03 mm) (p < 0.05) patients. The brachyfacial and mesofacial patients with transverse discrepancies presented a greater distance from the mesiobuccal root of the left upper first molar and from the palatine root to the cortical bone, while in the dolichofacial individuals the distances were shorter (p < 0.05); The presence of transverse bone discrepancies in brachyfacial and mesofacial patients without posterior cross-bite implies a better dentoalveolar expansion prognosis than in dolichofacial individuals.

Background and Purpose: Provisional prostheses in restorations over several implants with immediate loading in completely edentulous patients increase the risk of frequent structural fractures. An analysis was performed of the resistance to fracture of prosthetic structures with cantilevers using graphene-doped polymethyl methacrylate (PMMA) resins and CAD-CAM technology. Methods: A master model was produced with four implants measuring 4 mm in diameter and spaced 3 mm apart, over which 44 specimens representing three-unit fixed partial prostheses with a cantilever measuring 11 mm were placed. These structures were cemented over titanium abutments using dual cure resin cement. Twenty-two of the 44 units were manufactured from machined PMMA discs, and 22 were manufactured from PMMA doped with graphene oxide nanoparticles (PMMA-G). All of the samples were tested in a chewing simulator with a load of 80 N until fracture or 240,000 load applications. Results: The mean number of load applications required for temporary restoration until the fracture was 155,455 in the PMMA-G group versus 51,136 in the PMMA group. Conclusions: Resistance to fracture under cyclic loading was three times greater in the PMMA-G group than in the PMMA group.

Background: Available knowledge about malocclusion and cephalometric variables and their connection with an increased risk of condylar displacement (CD) is scarce. This article aims to present current information on the relationship between centric relation-maximum intercuspal position discrepancies and maxillofacial morphology and malocclusion in patients seeking orthodontic treatment as well as to identify those who require expanded diagnostic evaluation for this disorder. Methods: This review analyzed the PubMed, Cochrane Library, Web of Science, and Scopus electronic databases up to February 2022. Keywords and additional manual searches were performed. Literature selection was based the PRISMA-ScR checklist. The JBI Critical Appraisal Tool assessed the methodological quality of included studies. Results: The databases search provided 2321 studies. A total of 10 studies were included in this review after eligibility criteria and JBI assessment. This review was separated into five parts that evaluated CD correlations depending on the following: maxillofacial structure in different vertical and sagittal skeletal patterns, vertical, horizontal, and transverse malocclusions. Conclusions: A hyperdivergent facial skeletal structure is a risk factor for increased CD, particularly in the vertical dimension. The condylar processes are usually displaced in a posteroinferior direction. Further studies are warranted to elucidate the relationship among remaining skeletal and dental malocclusions and the occurrence of CD.

Background. The effectiveness of mandibular advancement devices has been solidly demonstrated in the past. They are considered a valid alternative treatment to continuous positive airway pressure for patients with obstructive sleep apnea. Nevertheless, the relationship between polysomnographic parameters and the increase in the volume of the upper airway in patients with obstructive sleep apnea syndrome has not been clearly established so far. This study aimed to determine the impact of these oral appliances upon the volume of the airway after the device titration phase and correlate it with the degree of mandibular advancement and the improvement of polysomnographic parameters. Methods. All patients were diagnosed by polysomnography and were treated with a customized, titratable mandibular advancement device. Three-dimensional volumetric measurements were performed using cone beam computed tomography. Results. The present study included 45 patients diagnosed with obstructive sleep apnea hypopnea syndrome (mild in 23 patients, moderate in 11 and severe in 11). Forty-four percent of the patients presented with an apnea hypopnea index &lt, 5/h at the end of treatment. The volume of the upper airway increased an average of 4.3 ± 5.9 cm3, this represents a percentage increase of 20.9%, which was significantly correlated with an apnea hypopnea index and a minimum oxygen saturation improvement. Conclusions. The mandibular advancement device used was found to be effective in improving polysomnographic parameters. Moreover, the oral appliance was able to significantly increase the tridimensional dimensions of the upper airway. Moreover, this finding was correlated with a reduction in the apnea hypopnea index (p = 0.007) and an increase on minimum oxygen saturation (p = 0.033).

Purpose To evaluate the clinicaland biological behavior of full coverage restorations on teeth prepared without finish line during a 4-year follow-up. Methods This prospective study included 149 teeth treated using biologically oriented preparation technique (BOPT). The sample (149 teeth) was divided into two groups: Seventy four teeth restored with crowns, and 75 teeth supporting fixed partial dentures (FPD). Restorations were fabricated with zirconium oxide cores and ceramic coverings. Patients attended regular annual check-ups when probe depth, presence of inflammation with bleeding on probing, presence of plaque, gingival thickness, marginal stability, biological or mechanical complications, and the patient’s level of satisfaction were registered over a 4-year follow-up. Results After the 4-year follow-up, 2.1% of teeth underwent increases in probing depth; 12% of the sample presented inflammation and bleeding on probing; 20% of the restored teeth presented plaque; gingival thickening increased a 32.5%; 98.6% of teeth presented marginal stability; the restoration survival rate was 96.6%, with 2% of biological complications and 1.4% of mechanical complications. General satisfaction score was 80.73. Conclusions Restorations placed on teeth prepared using BOPT present good periodontal behavior, increase of gingival thickening, and marginal stability over a 4-year follow-up. High survival rates after 4 years show that the technique produces predictable outcomes. Clinical significance The BOPT technique is a good treatment option in cases where replacement of an old restoration is required; presenting good periodontal behavior, gingival thickening, and marginal stability.

PURPOSE To analyze peri-implant bone loss around six different types of implant-prosthetic connections over a 2-year follow-up period. MATERIALS AND METHODS A total of 120 implants were divided into six groups (n = 20), three with crestally and three with supracrestally placed implants, with different internal connections, placed in posterior sectors (molar and premolar), and bearing one-piece CAD/CAM restorations screwed directly to each implant's prosthetic platform. Bone height was measured from parallelized periapical radiographs taken at the moment of loading and 2 years later. RESULTS The patient sample included 61 subjects, 36 men and 25 women. Significant differences were found in bone loss between groups, the implant with internal hex and convergent transmucosal collar showing the least bone loss (P < .001) in comparison with the other designs. Supracrestally placed implants underwent less bone loss than crestally placed implants (P = .025). CONCLUSION Peri-implant bone loss is influenced by the level of implant placement in relation to the bone crest (crestal or supracrestal) and by the morphology of the prosthetic platform.

bjectives. The aim of this study was to analyze the behavior of the periodontal tissues around teeth in the anterior region when restored with zirconia single crowns, using a biologically oriented preparation technique (BOPT), over a 6-year follow-up. Methods. The study investigated tooth-supported single crowns in the anterior region that were fabricated with a zirconia core and feldspathic ceramic covering, in 34 patients. Follow-up analysis took place annually for 6 years, assessing periodontal responses by evaluating the following variables: plaque index (PI), probing depth (PD), gingival index (GI), gingival thickness adjacent to the restoration, and stability of the gingival margin (MS). Any (biological and mechanical) complications were also recorded, as well as the patients’ satisfaction with the treatment. Results. After 6 years’ follow-up, a low mean plaque index was obtained, probing depth was stable, and gingival thickness and margin stability had increased. Complications (biological and mechanical) did not present a statistically significant incidence and a crown survival rate of 97.2% was achieved. Patients’ satisfaction obtained a mean VAS score of 9.04 under 10. Conclusion. Teeth that are prepared with BOPT in the anterior region present good periodontal behavior around the restored teeth, particularly in terms of the stability of the gingival margin and increased gingival thickness. Single crowns prepared with BOPT obtain an excellent clinical survival rate, as well as a high score in patients’ satisfaction after 6 years.

Purpose: This systematic review set out to investigate the influence of chemical composition and specimen thickness of monolithic zirconia on its optical and mechanical properties. Meta-analysis and meta-regression analyzed the effects of variations in percentages of yttrium, aluminum, and specimen thickness of monolithic zirconia. Study selection: The review followed recommendations put forward in the PRISMA checklist. An electronic search for relevant articles published up to October 2019 was conducted in the Pubmed, Cochrane, Scopus, Scielo, and Web of Science databases, with no language limits and articles published in the last 10 years. From 167 relevant articles; applying inclusion criteria based on the review’s PICO question, 26 articles were selected for qualitative synthesis (systematic review) and 24 for quantitative synthesis (meta-analysis). Experimental in vitro studies published were selected and their quality was assessed using the modified Consort scale for in vitro studies of dental materials. Introduction: The aim of this retrospective study was to analyze the radiographic peri-implant bone loss of bone level implants and tissue level implants with a convergent neck in screw-retained single crowns and in screw-retained fixed partial prostheses, after two years of functional loading. Materials and methods: The sample was divided into two groups according to their type: Group I: supracrestal implants with convergent transmucosal neck, Group II: crestal implants. In each group we distinguish two subgroups according to the type of prosthetic restoration: single crowns and a three-piece fixed partial prosthesis on two implants. To quantify bone loss, parallelized periapical radiographs were analyzed at the time of implant placement and after two years of functional load. Results: A total of 120 implants were placed in 53 patients. After statistical analysis it was observed that for each type of implant bone loss was 0.97 ± 0.91 mm for bone level and 0.31 ± 0.48 mm for tissue level. No significant differences were found regarding the type of prosthesis and the location (maxilla or mandible) of the implants. Conclusions: Tissue level implants with a convergent transepithelial neck exhibit less peri-implant bone loss than bone level implants regardless of the type of prosthesis.

Background The aim of this study was to compare the influence of two novel reciprocating movements on the cyclic fatigue resistance of endodontic reciprocating files. Methods 30 Procodile® (Komet Medical, Lemgo, Germany) files were selected in this study and distributed according to the following study groups depending on the movements to be performed: ReFlex Dynamic (n = 10), ReFlex Smart (n = 10) and Reciproc (n = 10) reciprocating movement. These files were fixed to a specific dynamic cyclic fatigue device designed and manufactured by 3D impression to simulate the pecking motion performed by the operator. The time to failure and the number of cycles of in-and-out of the endodontic files was registered. The results were analyzed by ANOVA and Weibull statistics. Results Statistically significant differences were found when the number of cycles of in-and-out movement and the time to failure of ReFlex Dynamic and Reciproc reciprocating movement (p p p = 0.253). Conclusions The ReFlex Smart reciprocating movement increased the cyclic fatigue resistance of endodontic reciprocating files compared with traditional reciprocating movement.

Statement of problem One of the most frequent complications in participants with fixed partial dentures (FPDs) is the apical migration of the gingival margin, which may be associated with factors such as fit, gingival margin location, or tooth preparation type. The prevalence of the complication in participants restored with FPDs prepared by using the biologically oriented preparation technique (BOPT) is unclear. Purpose The purpose of this prospective clinical trial was to evaluate the clinical and biologic outcomes of FPDs on teeth prepared by using the BOPT, over a 6-year follow-up period. Material and methods Tooth-supported zirconia FPDs in the anterior region prepared by using the BOPT were evaluated. Each participant was monitored annually for 6 years by evaluating plaque index, probing depth, vestibular gingival thickness, and gingival margin stability. Biologic and/or mechanical complications were also recorded. Patient satisfaction was measured by using a visual analog scale (VAS). Results A total of 25 FPDs supported by 70 teeth in 24 participants were analyzed. Low plaque index values and stable probing depths were observed, whereas the gingival index was 0 for most of the teeth. Teeth treated by using the BOPT presented significant increase in gingival thickness, and the gingival margin was found to be stable in 100% of the treatments. FPD survival was 100%. Conclusion Tooth supporting FPDs prepared by using the BOPT presented good periodontal health and gingival margin stability, without recession and with a 100% survival rate during a 6-year follow-up.

To measure the accuracy (trueness and precision) of a facial scanner depending on the alignment method and the digitized surface area location.Fourteen markers were adhered on a head mannequin and digitized using an industrial scanner (GOM Atos Q 3D 12 M; Carl Zeiss Industrielle Messtechnik GmbH). A control mesh was acquired. Subsequently, the mannequin was digitized using a facial scanner (Arc4; Bellus3D) (n = 30). The control mesh was delineated into 10 areas. Based on the alignment procedures, two groups were created: reference best fit (RBF group) and landmark-based best fit (LA group). The root mean square was used to calculate the discrepancy between the control mesh and each facial scan. A 2-way ANOVA and Tukey pairwise comparison tests were used to compare trueness and precision between the 2 groups across 10 areas (α = .05).Both alignment algorithms (P = .007) and digitized area (P .001) were significant predictors of trueness with a significant interaction between the two predictors (F (9, 580) =25.13, P .001). Tukey pairwise comparison showed that there was a significant difference between mean trueness values of RBF (mean=0.53 mm) and LA (mean=0.55 mm) groups. Moreover, a significant difference was detected among the trueness values across surface areas. The A9-area (left tragus area) had the highest and A5-area (right cheek area) had the lowest mean trueness. Both alignment algorithm (P .001) and digitized surface area (P .001) were significant predictors of precision with a significant interaction between the two predictors (F (9, 580) =14.34, P .001). Tukey pairwise comparison showed that there was a significant difference between mean precision values of RBF (mean=0.38 mm) and LA (mean=0.35 mm) groups. Moreover, a significant difference was detected among the precision values across surface areas. Comparing the surface areas, A9-area had the highest and A10-area (forehead area) had the lowest mean precision.Alignment procedures influenced on the scanning trueness and precision mean values, but the facial scanner accuracy values obtained were within the clinically acceptable accuracy threshold of less or equal than 2 mm. Furthermore, the scanning accuracy (for both trueness and precision) depended on the location of the scanned surface area, being more accurate on the middle of the face than on the sides of the face.

Background: The objective of this case series was to evaluate the efficacy of miniscrews to upright the lower second molar as a result of early loss of the lower first molar in order to permit rehabilitation by means of an implant-supported single crown. Methods: The case series included ten patients who each received a miniscrew placed distal of the second molar in order to straighten the tooth, the prosthetic space gained (sufficient to allow implant placement in the edentulous space) and the change in angulation obtained were analyzed. The changes produced by miniscrews were evaluated in radiographs. Results: Statistical analysis identified significant improvements in angulation (p = 0.005) and significant amounts of space gained (p = 0.005) as well as a strong correlation between these two parameters (r = &minus, 0.93, p &lt, 0.001). Conclusions: The use of miniscrews alone without the intervention of other orthodontic methods to straighten a mesialized lower second molar in order to replace a missing first molar with an implant significantly improves both the angulation of the molar and the prosthetic space available.

This study aimed to analyze a novel technique to make screws with greater untightening resistance and to solve screw loosening in implant-supported single crowns. Thirty grade IV titanium straight abutments were screwed onto 30 external hex implants using grade IV titanium screws (30 Ncm). They were exposed to cyclic loading (300,000 cycles, 200 N). Samples were divided into 4 groups (15 samples per group): new screws (SCREW group) (control), reused screws (rSCREW group), new screws wrapped with polytetrafluoroethylene (PTFE) tape (PTFE group), and reused screws wrapped with PTFE tape (rPTFE group). Reverse torque values (RTVs) were recorded with a digitally calibrated implant motor. Mean RTVs observed were 14.46 N (&plusmn, 1.10 N) for the control group, 14.42 N (&plusmn, 1.22 N) for the rSCREW group, 19.97 N (&plusmn, 1.16 N) for the PTFE group, and 19.13 N (&plusmn, 2.38 N) for the rPTFE group. Statistically significant differences were found between RTVs of both groups employing screws without PTFE tape (SCREW and rSCREW groups) compared with those using screws wrapped with PTFE tape (PTFE and rPTFE) (p &lt, 0.001). These results suggest that wrapping the implant&ndash, abutment screw with PTFE tape may effectively lower the risk of loosening and even constitute a solution when this complication occurs in implant single crowns.

Objective: To evaluate the influence of implant length on marginal bone loss, comparing implants of 4 mm, 6 mm, and &gt, 8 mm, supporting two splinted crowns after 36-month functional loading. Materials and Methods: this retrospective clinical trial evaluated the peri-implant behavior of splinted crowns (two per case) on pairs of implants of the same length placed in the posterior maxilla (molar area). Implants were divided into three groups according to length (Group 1: extra-short 4 mm, Group 2: short 6 mm, Group 3: conventional length &gt, 8 mm). Marginal bone loss was analyzed using standardized periapical radiographs at the time of loading and 36 months later. Results: 24 patients (19 women and 5 men) were divided into three groups, eight rehabilitations per group, in the position of the maxillary first and second molars. The 48 Straumann&reg, Standard Plus (Regular Neck (RN)/Wide Neck (WN)) implants were examined after 36 months of functional loading. Statistical analysis found no significant differences in bone loss between the three groups (p = 0.421). No implant suffered biological complications or implant loss. Long implants were associated with less radiographic bone loss. Conclusions: extra-short (4 mm), short (6 mm), and conventional length (&gt, 8 mm) implants in the posterior maxilla present similar peri-implant bone loss and 100% survival rates in rehabilitation, by means of two splinted crowns after 36 months of functional loading. Implants placed in posterior positions present better bone loss results than implants placed in anterior positions, regardless of the interproximal area where bone loss is measured. Conventional length (&gt, 8 mm) implants show better behavior in terms of distal bone loss than short (6 mm) and extra-short (4 mm) implants.

A systematic review and meta-analysis was performed to analyze the survival of onlay restorations in the posterior region, their clinical behavior according to the material used (ceramic reinforced with lithium disilicate, conventional feldspathic ceramic or reinforced with leucite; hybrid materials and composite), possible complications, and the factors influencing restoration success. The systematic review was based on the preferred reporting items for systematic reviews and meta-analyses (PRISMA) statement, without publication date or language restrictions. An electronic search was made in the PubMed, Scopus, Embase, and Cochrane databases. After discarding duplicate publications and studies that failed to meet the inclusion criteria, the articles were selected based on the population, intervention, comparison, outcome (PICO) question. The following variables were considered in the qualitative and quantitative analyses: restoration survival rate (determined by several clinical parameters), the influence of the material used upon the clinical behavior of the restorations, and the complications recorded over follow-up. A total of 29 articles were selected for the qualitative analysis and 27 for the quantitative analysis. The estimated restoration survival rate was 94.2%. The predictors of survival were the duration of follow-up (beta = −0.001; p = 0.001) and the onlay material used (beta = −0.064; p = 0.028). Composite onlays were associated with a lower survival rate over time. Onlays are a good, conservative, and predictable option for restoring dental defects in the posterior region, with a survival rate of over 90%. The survival rate decreases over time and with the use of composite as onlay material.

Short implants are an increasingly common alternative to other surgical techniques in areas where bone availability is reduced. Despite the advantages they offer, a variety of biological repercussions have been described in the literature that can even lead to the loss of these. The aim of this systematic review and meta-analysis was to analyze the impact of the use of short implants on their survival and on peri-implant bone loss, evaluating the influence that length, diameter, and crown-to-implant ratio (C/I) have on these parameters. This systematic review was based on guidelines proposed by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). An electronic search was conducted using terms related to the use of short implants in partially or totally edentulous patients. A total of five databases were consulted in the literature search: PubMed, Embase, Cochrane, Scopus, and Web of Science. After eliminating the duplicate articles and assessing which ones met the inclusion criteria, 15 articles were included for the qualitative analysis and 14 for the quantitative study. Through meta-analysis, the percentage of implant loss and peri-implant bone loss was estimated. Relating these parameters to length, diameter, and C/I ratio, no significant differences have been found for implant loss (values of p = 0.06, 0.10, and 0.9, respectively for length, diameter, and C/I ratio), nor for peri-implant bone loss (values of p = 0.71, 0.72, and 0.36, respectively for length, diameter, and C/I ratio). In conclusion, the use of short implants does not seem to have a significant influence on marginal bone loss or the survival rate of implants.

(1) Background: To analyze the fracture resistance of endodontically upper premolar teeth restored with glass fiber reinforced posts, glass fiber elastic posts, conventional composite resin (CR) and glass fiber reinforced composite (FRC) resins as restorations. (2) Methods: Seventy premolars were submitted to root canal treatment and restored with the following restorative materials (n = 10): A. FRC posts restored with resin, B. Elastic FRC posts restored with resin, C. FRC posts restored with FRC resin, D. Elastic FRC posts restored with FRC resin, E. Direct restoration with resin, F. Direct restoration with FRC resin, G. Untreated teeth. The teeth were embedded in an epoxy resin model, thermal cycling fatigued in distilled water and mechanical cycling fatigued inducing 80 N load. Loading was applied axially on the center of the occlusal surface with a vertical displacement. The fracture was produced by a universal machine at a crosshead speed of 0.5 mm/s with a 5000 N load cell. The results were analyzed by ANOVA and Tukey's test and Weibull characteristic strength and modulus were calculated. (3) Results: The group that obtained the greatest fracture resistance was D (3620 &plusmn, 470 N) and the least resistant was group A (2420 &plusmn, 1010 N). Statistically significant differences were observed between the groups restored with Elastic FRC posts-CR versus FRC post-CR and only CR (p = 0.043 and p = 0.008). (4) Conclusions: The glass fiber reinforced restorative materials increase the fracture resistance of endodontically treated teeth.

Purpose: The aim of this comparative in vitro study was to assess the bond strength and mechanical failure of carbon-fiber-reinforced composites against cobalt&ndash, chrome structures with ceramic veneering. Materials and methods: A total of 24 specimens (12 per group) simulating dental prosthetic frameworks were fabricated. The experimental specimens were subjected to a thermocycling aging process and to evaluate bond strength. All specimens were subjected to a three-point bending test to fracture using a universal testing machine. Results: The cobalt&ndash, chrome/ceramic group yielded a bond strength value of 21.71 &plusmn, 2.16 MPa, while the carbon-fiber-reinforced composite group showed 14.50 &plusmn, 3.50 MPa. The failure assessment reported statistical significance between groups. Although carbon-fiber-reinforced composite group showed lower bond strength values, the chipping incidence in this group was as well lower. Conclusions: The chrome&ndash, cobalt/ceramic group showed greater bonding strength compared to the carbon-fiber-reinforced composite, most of the fractures within the cobalt&ndash, chrome/ceramic group, had no possibility of direct clinical repair.

Background: The aim of this study was to analyze the effects of the time of use (TU) and sterilization cycles (SC) of endodontic reciprocating files on cyclic fatigue resistance. Methods: One-hundred-and-twenty (120) Procodile NiTi endodontic reciprocating instruments were selected at random and distributed into the following study groups: A: 0 sterilization cycles/0s time of use (n = 10), B: 0/60 (n = 10), C: 0/120 (n = 10), D: 1/0 (n = 10), E: 1/60 (n = 10), F: 1/120 (n = 10), G: 5/0 (n = 10), H: 5/60 (n = 10), I: 5/120 (n = 10), J: 10/0 (n = 10), K: 10/60 (n = 10), and L: 10/120 (n = 10). A dynamic cyclic fatigue device was designed using computer-aided design/computer-aided engineering (CAD/CAE) technology and created with a 3D printer to simulate the pecking motion performed by the clinician. Failure of the endodontic rotary instrument was detected by a light-emitting diode-light-dependent resistor (LED-LDR) system controlled by an Arduino driver complex and management software. The results were analyzed using the ANOVA test. Results: All pairwise comparisons presented statistically significant differences between the time to failure, number of cycles to failure and number of cycles of in-and-out movement for the time of use study groups (p &lt, 0.001), but not in the number of sterilization cycles (p &gt, 0.05). Conclusions: The time of use of NiTi endodontic reciprocating files negatively affects dynamic cyclic fatigue resistance. Dynamic cyclic resistance is not affected by the number of sterilization cycles.

Background: Dental bleaching has become an everyday procedure for treating teeth presenting discoloration, as a therapy on its own or as a part of restorative dentistry. The most widely used bleaching products available for vital teeth are hydrogen peroxide and carbamide peroxide, marketed as a range of products adapted to different treatment modes. The purpose of the study was to evaluate the clinical whitening efficacy of four high-concentration bleaching products for vital teeth applied in single in-office sessions. Methods: This clinical trial included 40 patients divided into four groups according to the concentration of the hydrogen peroxide-based (HP) product used (Group 1: HP 35%, Group 2: HP 37.5%, Group 3: HP 38%, Group 4: HP 40%). Each patient received one in-office whitening treatment of maxillary and mandibular anterior teeth (incisors, canines, and premolars), making a total of 791 teeth. The color changes produced in central incisors and canines were measured. Results: All groups underwent significant color changes from initial to final evaluations for color parameters L*, a*, and b*. No significant differences between the four groups were found for either the L* or a* parameters. Analysis of the b* parameter found significant differences between the HP 38% group and the HP 35% group. No significant differences in &Delta, E were found between the four groups (ANOVA p = 0.174). HP 38% obtained the highest &Delta, E (4.63), while HP 40% obtained 4.01. Conclusions: A single in-office whitening session, regardless of the bleaching product used, modifies tooth color effectively. All four products achieved increases in the L* parameter and significant reductions in parameters a* and b*. The &Delta, E did not show significant differences between the four groups. In-office whitening with high concentration hydrogen-peroxide-based products was found to be an effective treatment for moderate and severe dental discoloration.

Background: The aim of this systematic review and meta-analysis was to determine the wear sustained in the natural antagonist tooth in cases of full-coverage fixed-base prosthetic restorations or monolithic zirconia tooth-supported crowns, as well as to determine the wear in the restoration itself, both in the short- and medium-term and considering the factors that may influence wear. Material and methods: A systematic literature review and meta-analysis, based on Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) recommendations, of clinical studies that evaluated wear in antagonist teeth in relation to fixed-prosthesis monolithic zirconia crowns. A total of 5 databases were consulted in the literature search: Pubmed-Medline, Cochrane, Scopus, Embase and Web of Science (WOS). After eliminating duplicated articles and applying the inclusion criteria, eight articles were selected for the qualitative analysis and four for the quantitative analysis. Results: Mean maximum wear of the antagonist tooth in relation to monolithic zirconia crowns of magnitude 95.45 µm (CI at 95% 79.57–111.33) was observed. By using a meta-regression model (R2 = 0.92) the significant effect of time in maximum wear rate (p < 0.001) was observed, estimated at 6.13 µm per month (CI at 95% 3.99–8.27). Furthermore, monolithic zirconia crowns are subject to a mean maximum wear of 58.47 µm (CI 95% 45.44–71.50). By using a meta-regression model (R2 = 0.53) the significant effect of time in the maximum wear value was observed (p = 0.053), estimated at 3.40 µm per month (CI al 95% −0.05–6.85). Conclusions: Monolithic zirconia crowns lead to a progressive maximum wear of the antagonist tooth over time which is greater than the maximum wear sustained in the crown itself. It is not possible to establish an objective and quantitative objection in relation to natural enamel wear or metal–ceramic crowns.

Background and Objectives: To evaluate in vitro the fracture resistance and fracture type of computer-aided design and computer-aided manufacturing (CAD-CAM) materials. Materials and Methods: Discs were fabricated (10 &times, 1.5 mm) from four test groups (N = 80, N = 20 per group): lithium disilicate (LDS) group (control group): IPS e.max CAD&reg, zirconium-reinforced lithium silicate (ZRLS) group: VITA SUPRINITY&reg, polymer-infiltrated ceramic networks (PICN) group: VITA ENAMIC&reg, resin nanoceramics (RNC) group: LAVA&trade, ULTIMATE. Each disc was cemented (following the manufacturers&rsquo, instructions) onto previously prepared molar dentin. Samples underwent until fracture using a Shimadzu&reg, test machine. The stress suffered by each material was calculated with the Hertzian model, and its behavior was analyzed using the Weibull modulus. Data were analyzed with ANOVA parametric statistical tests. Results: The LDS group obtained higher fracture resistance (4588.6 MPa), followed by the ZRLS group (4476.3 MPa) and PICN group (4014.2 MPa) without statistically significant differences (p &lt, 0.05). Hybrid materials presented lower strength than ceramic materials, the RNC group obtaining the lowest values (3110 MPa) with significant difference (p &lt, 0.001). Groups PICN and RNC showed greater occlusal wear on the restoration surface prior to star-shaped fracture on the surface, while other materials presented radial fracture patterns. Conclusion: The strength of CAD-CAM materials depended on their composition, lithium disilicate being stronger than hybrid materials.

Objective: The aim of this study was to analyze the peri-implant bone loss of infracrestal, supracrestal, and crestal implants from the day of placement and up to 1 year of prosthetic loading. Material and methods: A retrospective clinical study was carried out. The sample consisted of 30 implants placed on 30 patients. It was divided into three groups: infracrestal (n = 10), crestal (n = 10), and supracrestal (n = 10) implants. Results: Following the statistical analysis, it was observed that, 4 months after implant placement, the mean values of total peri-implant bone loss were 0.04 mm in infracrestal implants, 0.26 mm in crestal implants, and 0.19 mm in supracrestal implants. At the end of one year of prosthetic loading, the peri-implant bone loss was 0.12 mm in infracrestal implants, 1.04 mm in crestal implants, and 0.27 mm in supracrestal implants. It was determined that peri-implant bone loss in crestal implants was significantly higher than in supracrestal implants, and these in turn were significantly higher than in infracrestal implants. Conclusions: The implants that obtained a better biological behavior on peri-implant bone tissue were the infracrestal implants with a converging transmucosal abutment.

Epidermolysis bullosa belongs to a group of genetic diseases that present with skin disorders and is characterized by generalized blister formation in response to mechanical trauma. This article reports on the management of a recessive dystrophic epidermolytic patient with four remaining periodontally compromised maxillary teeth. Treatment involved placement of four maxillary implants and use of computer-aided design/computer-assisted manufacture techniques to fabricate a fixed full-arch implant-supported prosthesis. The prescribed dental treatment protocol led to a successful short-term prosthodontic outcome.

PURPOSE To compare the effectiveness of different methods of fractured screw fragment retrieval from dental implants, comparing a conventional method (use of an exploration probe and ultrasonic tips) with two mechanical retrieval kits. MATERIALS AND METHODS One hundred thirty-five screws in implant internal connections were fractured to test fragment retrieval comparing three methods: conventional method with the use of an explorer probe and ultrasonic tips (group CE), extraction with a mechanical kit with a guide cylinder (group MK1), and extraction with a mechanical kit without a guide cylinder (group MK2). Extraction success and retrieval times for each method, as well as success and retrieval times in relation to fracture depth-coronal, middle, or apical-were analyzed by means of chi-square, Fisher exact, Kruskal-Wallis, and Mann-Whitney tests (P < .05). The integrity of implant internal threads was also analyzed. RESULTS The extraction success rates for groups MK2, CE, and MK1 were 93.3%, 73.3%, and 20%, respectively. The mean ± SD extraction times were 2 minutes, 56 seconds ± 2 minutes, 9 seconds; 3 minutes, 15 seconds ± 1 minute, 48 seconds; and 13 minutes, 34 seconds ± 7 minutes, 25 seconds for groups MK2, CE, and MK1, respectively. Statistically significant differences were found in the number of fractured screws extracted and in the retrieval time. No significant differences were found for retrieval time or extraction success in relation to the depth of the fracture. CONCLUSION The mechanical kit without the guide cylinder achieved the best results with respect to the number of screw fragments retrieved, retrieval time, and preservation of implant internal threads.

Background and objectives: To compare the medium- to long-term mechanical behavior of overdentures with two different retention systems: overdentures with Locator&reg, axial retention, and vertical insertion overdentures with bar retention, used to rehabilitate edentulous maxillar. Material and Methods: This prospective study assessed patients presenting complete maxillary edentulism, rehabilitated by means of implant-supported overdentures (n = 20), 10 with Locator&reg, axial retention (ODA group) and 10 with overdentures on bars (ODB group). Patients also completed a questionnaire to determine their satisfaction with treatment. Results: The mean follow-up time in both groups was 11.4 years, with follow-up times in both groups ranging from 5 to 14 years. The ODA group suffered mechanical complications such as retention loss, need for nylon retention insert changes, resin fracture, and need for relining. In the ODB group, prosthetic dental wear, screw loosening, and complete prosthetic failure were more common. A total of 19 implants failed (23.8%), of these, 11 were in the ODA group (failure rate = 27.5%) and eight in the ODB group (failure rate = 20%). The patient satisfaction questionnaire obtained a mean score of 7.9 out of 10 in the ODA group, and 9.75 in the ODB group. Conclusions: in rehabilitations of edentulous maxillar by means of implant-supported overdentures, both the systems assessed were shown to be effective in the medium to long term. Patients expressed a high level of satisfaction with the treatments received.
