Disorders of mineral metabolism, including secondary hyperparathyroidism, are thought to contribute to extraskeletal (including vascular) calcification among patients with chronic kidney disease. It has been hypothesized that treatment with the calcimimetic agent cinacalcet might reduce the risk of death or nonfatal cardiovascular events in such patients.In this clinical trial, we randomly assigned 3883 patients with moderate-to-severe secondary hyperparathyroidism (median level of intact parathyroid hormone, 693 pg per milliliter [10th to 90th percentile, 363 to 1694]) who were undergoing hemodialysis to receive either cinacalcet or placebo. All patients were eligible to receive conventional therapy, including phosphate binders, vitamin D sterols, or both. The patients were followed for up to 64 months. The primary composite end point was the time until death, myocardial infarction, hospitalization for unstable angina, heart failure, or a peripheral vascular event. The primary analysis was performed on the basis of the intention-to-treat principle.The median duration of study-drug exposure was 21.2 months in the cinacalcet group, versus 17.5 months in the placebo group. The primary composite end point was reached in 938 of 1948 patients (48.2%) in the cinacalcet group and 952 of 1935 patients (49.2%) in the placebo group (relative hazard in the cinacalcet group vs. the placebo group, 0.93; 95% confidence interval, 0.85 to 1.02; P=0.11). Hypocalcemia and gastrointestinal adverse events were significantly more frequent in patients receiving cinacalcet.In an unadjusted intention-to-treat analysis, cinacalcet did not significantly reduce the risk of death or major cardiovascular events in patients with moderate-to-severe secondary hyperparathyroidism who were undergoing dialysis. Multiple endocrine neoplasia type 1 (MEN1) is a rare autosomal dominant hereditary disease due to a mutation in the MEN1 tumor suppressor gene. The risk of the disease in first-degree relatives of MEN1 mutation carriers is 50%. MEN1 gene mutations are not identified in 10-30% of familiar MEN1 patients and in 60-80% of sporadic MEN1 cases, which can be explained by mutations in the noncoding regions of the MEN1 gene, large gene deletions or mutations in other yet unknown genes. Molecular genetic testing can exclude the diagnosis of MEN1 in patients who do not harbor the MEN1 mutation, thus revealing a MEN1 phenocopy. This obviates the need for annual screening for the early detection of other remaining components of the disease and its risk in progeny.

Aims: this study evaluates the most common associations of symptoms and complications in patients with Cushing’s syndrome (CS) in order to choose a potential population to be screened for CS and estimates the diagnostic accuracy of first line screening tests (cortisol, ACTH) to differentiate ACTH-ectopic CS from Cushing’s disease. Materials and Methods: The clinical data of 259 patients with proven CS during 2001–2011 was analyzed. The clinical presentations of 197 patients (159 Cushing’s disease, 28 ACTH-ectopic CS and 10 cases of benign cortisol-secreting adrenal adenoma) were compared according to the cause of hypercortisolism. ROC-analysis was performed to estimate the diagnostic accuracy of the first line tests (cortisol, ACTH) to suggest ACTH-ectopic CS. A threshold for the test with the highest area under the curves was chosen based on the maximum sum of the sensitivity and specificity. Results: The most frequent complaints were related to fatigue, muscle weakness, weight gain and changes in appearance (facial plethora and fullness, striae). Among the complications of CS the most frequent were being overweight or obese (71%), hypertension (63%), dislipoproteinemia (41%), low traumatic fractures (43%) and steroid-induced diabetes (31%). In women, 16% were older than 50, in those who were younger amenorrhea was registered in 43%. The patients with ACTH-ectopic CS had higher rate of low traumatic fractures (p=0.04), increased serum late-night cortisol, 24 hours urinary free cortisol, morning and evening ACTH and lower levels of potassium (p0.01 for all parameters). Plasma late-night ACTH measurements showed the highest AUC (0,811 (95% CI 0,712–0,909)) to differentiate ACTH-ectopic CS from Cushing’s disease. A cut off value of 108.9 pg/ml for late-night ACTH yielded a sensitivity of 60,7% and a specificity of 79%. Conclusions: patients with a coexistence of obesity, muscle weakness, fatigue, some components of metabolic syndrome and especially low traumatic fractures should be screened for CS. High plasma late night ACTH values in patients with proven CS value suggest ACTH-ectopic syndrome.

Background— Patients with kidney disease have disordered bone and mineral metabolism, including elevated serum concentrations of fibroblast growth factor-23 (FGF23). These elevated concentrations are associated with cardiovascular and all-cause mortality. The objective was to determine the effects of the calcimimetic cinacalcet (versus placebo) on reducing serum FGF23 and whether changes in FGF23 are associated with death and cardiovascular events. Methods and Results— This was a secondary analysis of a randomized clinical trial comparing cinacalcet to placebo in addition to conventional therapy (phosphate binders/vitamin D) in patients receiving hemodialysis with secondary hyperparathyroidism (intact parathyroid hormone ≥300 pg/mL). The primary study end point was time to death or a first nonfatal cardiovascular event (myocardial infarction, hospitalization for angina, heart failure, or a peripheral vascular event). This analysis included 2985 patients (77% of randomized) with serum samples at baseline and 2602 patients (67%) with samples at both baseline and week 20. The results demonstrated that a significantly larger proportion of patients randomized to cinacalcet had ≥30% (68% versus 28%) reductions in FGF23. Among patients randomized to cinacalcet, a ≥30% reduction in FGF23 between baseline and week 20 was associated with a nominally significant reduction in the primary composite end point (relative hazard, 0.82; 95% confidence interval, 0.69–0.98), cardiovascular mortality (relative hazard, 0.66; 95% confidence interval, 0.50–0.87), sudden cardiac death (relative hazard, 0.57; 95% confidence interval, 0.37–0.86), and heart failure (relative hazard, 0.69; 95% confidence interval, 0.48–0.99). Conclusions— Treatment with cinacalcet significantly lowers serum FGF23. Treatment-induced reductions in serum FGF23 are associated with lower rates of cardiovascular death and major cardiovascular events. Clinical Trial Registration— URL: http://www.clinicaltrials.gov . Hypercortisolism is a severe endocrine disorder that leads to all types of metabolic disturbances. The most frequent symptom of hypercortisolism is a specific weight gain and body fat distribution - "cushingoid" obesity which clinically reflects the pathological changes in protein, carbohydrate and lipids metabolism. We provide a clinical case of a patient with Cushings disease. This clinical case represents challenges in the differential diagnosis of this rare endocrine disorder and metabolic syndrome, and specific approach in further evaluation and treatment of Cushing's disease that requires all the latest technology in this field to be employed.

Acromegaly is a severe neuroendocrine disease that develops when the pituitary gland produces excess growth hormone (GH). Acromegaly is not only a cosmetic problem. Hypersecretion of GH leads to variety complications, decreased quality of life, shortening the expected lifespan due to development of severe cardiovascular complications. Timely diagnosis and adequate treatment can reduce mortality by 2-5 times and medical therapy deservedly plays an important role in treatment of patients with acromegaly when radical surgical treatment cannot be performed. We present a first Russian experience of acromegaly patient treatment with new preparation - Somatulin Autogel, with analysis of its favorable effects on clinical and metabolic manifestations of the disease.

Chronic fatigue syndrome is a disease of unexplained feeling of profound fatigue lasting for more than 6 months. This fatigue is not relieved even after prolonged rest and is exacerbated after physical or mental work. More than 3000 scientific studies had proved that chronic fatigue syndrome is not a form of depression or hypochondria. It is a real somatic illness that results in professional, social and individual desadaptation. This article summarizes the contemporary etiological conceptions of this condition.

Резюме. Пролактиномы редко выявляются у лиц детского возраста, но зачастую характеризуются более агрессивным ростом. И, несмотря на то, что подходы к лечению этих опухолей у взрослых хорошо изучены и в настоящее время предпочтение отдается медикаментозной терапии агонистами дофамина, применение этих препаратов у детей ограничено из-за отсутствия данных о безопасности и эффективности их применения на этой группе пациентов. Каберголин в качестве медикаментозной терапии является препаратом выбора в лечении пролактином у взрослых пациентов. Представленный нами клинический случай демонстрирует высокую эффективность и хорошую переносимость каберголина у мальчика 14 лет с гигантской пролактиномой, осложненной зрительными, неврологическими и метаболическими нарушениями, что позволило на данном этапе воздержаться от оперативного лечения, а также нивелировать или уменьшить проявления осложнений заболевания. Resume. Prolactinomas are very rare, but usually more aggressive among children, than in adults. Although treatment strategies for prolactinomas in adults are well established and at present time the preference is given to dopamine agonists, the use of these drugs in pediatric patients is limited due to lack of information on safety and efficiency in this group of patients. Cabergoline is a drug of choice for treatment adult patients with prolactinomas. This clinical case that we report here demonstrates high efficiency and tolerability of cabergoline in a 14-year patient with giant prolactinoma, complicated by visual, neurological and metabolic disturbances, which allowed us to refrain from surgical treatment and to reduce the degree of complications.

Bone metabolism and changes in bone mineral density (BMD) are very important in patients with multiple endocrine neoplasia (MEN) type 1 syndrome. Case report: we present a clinical case of primary hyperparathyroidism (PHPT) in a patient with a familal MEN 1 syndrome and severe PHPT debuted at a young age w. Pituitary Cushing syndrome was diagnosed in 16 years old patient. He had marked osteoporosis due to hypercorticism and deficiency of sex hormones (-12% from the age norm). There was significant positive dynamics in BMD (+7% by Z-score) after remission of hypercorticism by two courses of radiation treatment and effective hormone replacement therapy of hypogonadism. PHPT with marked decrease in BMD at three sites was revealed at age of 19 years. Obvious increase in BMD was observed during a year after effective surgical treatment and antiosteoporotic therapy. In conclusion, this clinical case demonstrates the combined influence of various factors on bone metabolism in patients with MEN type 1 syndrome, which requires integrated approach to successful therapy.

Central diabetes insipidus (CDI) - a severe disease with disturbance of arginine vasopressin secretion, with excretion of large amounts of hypotonic urine, which leads to dehydration and thirst. Desmopressin, a synthetic analog of arginine vasopressin, is used to compensate a water-electrolyte balance in patients with CDI is available in forms of intranasal spray and tablets. Herein we present a clinical case of a patient with longstanding CDI due to Langerhans cell hystiocytosis in childhood who received different medications for treatment of CDI symptoms that demonstrates variable efficiency and tolerance of these drugs.
