14. Characterization of rib fracture development following liver directed stereotactic body radiation therapy

Abstract

PURPOSE: Rib fractures are a well-described complication following thoracic stereotactic body radiation therapy (SBRT). However, there are limited data in the setting of liver-directed SBRT. METHODS: Patients who underwent liver SBRT from 2014 to 2019 were analyzed. Logistic regression models were used to identify the demographic, clinical, and dosimetric factors associated with the development of rib fractures. RESULTS: Three hundred and forty-three consecutive patients were reviewed with median follow-up of 9.3 months (interquartile range [IQR]: 4.7–17.4 months); 81% of patients had primary liver tumors and 19% had liver metastases. Twenty-one patients (6.2%) developed rib fractures with a median time to diagnosis of 7 months following SBRT (IQR: 5–19 months). Of those patients, 11 experienced concomitant chest wall pain, while 10 patients had an incidental finding of a rib fracture on imaging. On univariate analysis, female gender (odds ratio [OR]: 2.29; p = 0.05), V30 Gy (OR: 1.02; p < 0.001), V40 Gy (OR: 1.08; p < 0.001), maximum chest wall dose (OR: 1.1; p < 0.001), and chest wall D30 cm(3) (OR: 1.09; p < 0.001) were associated with an increased probability of developing a rib fracture. On multivariate analysis, maximum chest wall dose (OR: 1.1; p < 0.001) was associated with developing a rib fracture. Receipt of more than one course of SBRT (p = 0.34), left versus right sided lesion (p = 0.69), osteoporosis (p = 0.54), age (p = 0.82), and PTV volume (p = 0.55) were not significant. CONCLUSIONS: Rib fractures following liver SBRT were observed in 6.2% of patients with the majority being asymptomatic. To mitigate this risk, clinicians should minimize dose delivery to the chest wall. Female patients may be at increased risk.

Abstract

Background: Deficits in list of words or history recall performance is associatedwith regional declines in brain metabolism, neurodegeneration andamyloid deposition. However, little is known regarding the neurocorrelatesof these tests in early stages of AD. Here we aimed to investigate the associationbetween these tests and synaptic depletion and amyloid depositionmeasured by [18 F]FDG and [18 F]AV45 respectively. Methods: We analyzeda subsample of participants from ADNIGO & ADNI2 who hadclinical, neuropsychological, and [18F]AV45/[18F]FDG data collectedin a single visit. Diagnosis of cognitively normal (CN), early mild cognitiveimpairment (EMCI), late mild cognitive impairment (LMCI) and ADwas adjusted using ADNI2 guidelines. Scores on the Rey Auditory VerbalLearning Test 30 min delay (RAVLT30), total recall (RAVLTT), sum oftrials (RAVLTST), Logical Memory 30 min delay (LM) were obtainedfrom the ADNI database. RAVLTST was calculated by adding the 5 initialtrials of the Rey Auditory Verbal Learning Test. T1 MRIs underwentnon-uniformity correction, were skull-stripped and nonlinearly registeredto MNI152 space. After registration to MRI, PET uptake ratios were calculateddividing [18F]AV45 and [18F]FDG scans by the median counts ofcerebellar-gm and pons, respectively. PET images were subsequently registeredto MNI152 space. Voxel-based (age corrected) regression betweenPET images and LM, RAVLT30, RAVLTT and RAVLST were calculatedwith PET-UR resampled to MNI space and blurred with a 6mm Gaussian filter. Results: Demographics are summarized in table 1,. [18 F]FDG uptakecorrelated positively (Figure 1,) with RAVLT30 and RAVLTST in the hippocampusof EMCI. Hypometabolism correlated with LM (left hippocampus)in LMCI and with RAVLTST (temporal lobe) in AD. [18 F]AV45 uptake(Figure 2) correlated negatively with LM (temporal and parietal) andRAVLTT (parietal) in controls and with RAVLT30, RAVLTTand RAVLTSTin the frontal and parietal lobes of EMCI. For all groups collapsed, all testscorrelated with brain hypometabolism and [18 F]AV45 uptake in temporal,parietal and frontal areas. Conclusions: RAVLT30 and RAVLTST conveyneurodegeneration in EMCI while LM reflects neurodegeneration inLMCI. LM30 correlates with amyloid deposition only in controls, whileRAVLT30, RAVLTT and RAVLTST reflect amyloidosis in EMCI.

Abstract

Background: The dynamic biomarker hypothesis predicts that fluid biomarkerabnormalities precede brain morphological changes observed inAD by many years. However, the link between early CSF abnormalitiesand late structural changes remains under debate. Here we investigatedthe association between regional cortical thinning (CT) measured by MagneticResonance Imaging (MRI) and brain amyloidosis (measured by CSFAb 1-42 concentrations), or tau hyperphosphorylation (tau 181; p-tau) inAlzheimer’s Disease (AD) and Mild Cognitive Impairment (MCI) patients.We test the hypothesis that the association between cortical thinning, amyloidosisor tau hyperphosphorylation depends on cortical regions and clinicalstages of AD. Methods: T1-weighed MRIs and associated CSF markersfrom individuals with mild cognitive impairment (MCI; 16), Alzheimer Disease(AD; n¼7) and age-matched cognitively normal subjects (CN; n¼8)were obtained from the Alzheimer’s Disease Neuroimaging Initiative(ADNI) cohort. Cortical surface reconstruction and group registration were generated using Freesurfer. A general linear model was used to conductregressions between CSF markers and cortical thickness. Results: Correlationanalyses in the MCI group showed a positive correlation betweenCT and Ab 1-42 measures predominantly in the temporo-parietal regions,namely the precuneus (peak r¼0.67; pr¼0.85; pclusters in the frontal cortex (peak r¼0.94; p¼0.0001). In both MCIand AD groups, significant clusters of negative correlations between CTandp-tau were mainly found in medio-temporal and dorso-lateral prefrontalareas. In the CN group, no relationship was observed between cortical thicknessand either CSF biomarkers. Conclusions: Although limited by samplesizes, our results suggest a posterior-anterior gradient of structural vulnerabilityto A b 1-42. Alternatively, weaker temporo-parietal effect between CTand Ab 1-42 in AD relative to MCI, likely results from a ceiling effect of Ab1-42 levels in the ADgroup. Conversely, a frontal effect can only be found inthe AD group, which possibly corresponds to a later stage of Ab 1-42-driven neurodegeneration.

Abstract

Purpose: Metabotropic glutamate receptor type 5 (mGluR5) is a G protein-coupled receptor that has been implicated in several psychiatric and neurological diseases. The radiopharmaceutical [C]ABP688 allows for in vivo quantification of mGluR5 availability using positron emission tomography (PET). In this study, we aimed to detail the regional distribution of [C]ABP688 binding potential (BP) and the existence of age/sex effects in healthy individuals. Methods: Thirty-one healthy individuals aged 20 to 77 years (men, n = 18, 45.3 ± 18.2 years; females, n = 13, 41.5 ± 19.6 years) underwent imaging with [C]ABP688 using the high-resolution research tomograph (HRRT). We developed an advanced partial volume correction (PVC) method using surface-based analysis in order to accurately estimate the regional variation of radioactivity. BP was calculated using the simplified reference tissue model, with the cerebellum as the reference region. Surface-based and volume-based analyses were performed for 39 cortical and subcortical regions of interest per hemisphere. Results: We found the highest [C]ABP688 BP in the lateral prefrontal and anterior cingulate cortices. The lowest [C]ABP688 BP was observed in the pre- and post-central gyri as well as the occipital lobes and the thalami. No sex effect was observed. Associations between age and [C]ABP688 BP without PVC were observed in the right amygdala and left putamen, but were not significant after multiple comparisons correction. Conclusions: The present results highlight complexities underlying brain adaptations during the aging process, and support the notion that certain aspects of neurotransmission remain stable during the adult life span.
