High intra-abdominal pressure (IAP) during laparoscopic surgery is associated with reduced splanchnic blood flow. It is uncertain whether a low IAP prevents this reduction. We assessed the effect of an individualized low-pneumoperitoneum-pressure strategy on liver perfusion. This was a single-center substudy of the multicenter 'Individualized Pneumoperitoneum Pressure in Colorectal Laparoscopic Surgery versus Standard Therapy II study' (IPPCollapse–II), a randomized clinical trial in which patients received an individualized low-pneumoperitoneum strategy (IPP) or a standard pneumoperitoneum strategy (SPP). Liver perfusion was indirectly assessed by the indocyanine green plasma disappearance rate (ICG–PDR) and the secondary endpoint was ICG retention rate after 15 min (R
15 ) using pulse spectrophotometry. Multivariable beta regression was used to assess the association between group assignment and ICG–PDR and ICG–R15 . All 29 patients from the participating center were included. Median IAP was 8 (25th–75th percentile: 8–10) versus 12 (12,12) mmHg, in IPP and SPP patients, respectively (p < 0.001). ICG–PDR was higher (OR 1.42, 95%-CI 1.10–1.82; p = 0.006) and PDR–R15 was lower in IPP patients compared with SPP patients (OR 0.46, 95%-CI 0.29–0.73; p = 0.001). - Abstract
Background: We aimed to examine whether using a high fraction of inspired oxygen (FIO2) in the context of an individualised intra- and postoperative open-lung ventilation approach could decrease surgical site infection (SSI) in patients scheduled for abdominal surgery. Methods: We performed a multicentre, randomised controlled clinical trial in a network of 21 university hospitals from June 6, 2017 to July 19, 2018. Patients undergoing abdominal surgery were randomly assigned to receive a high (0.80) or conventional (0.3) FIO2 during the intraoperative period and during the first 3 postoperative hours. All patients were mechanically ventilated with an open-lung strategy, which included recruitment manoeuvres and individualised positive end-expiratory pressure for the best respiratory-system compliance, and individualised continuous postoperative airway pressure for adequate peripheral oxyhaemoglobin saturation. The primary outcome was the prevalence of SSI within the first 7 postoperative days. The secondary outcomes were composites of systemic complications, length of intensive care and hospital stay, and 6-month mortality. Results: We enrolled 740 subjects: 371 in the high FIO2 group and 369 in the low FIO2 group. Data from 717 subjects were available for final analysis. The rate of SSI during the first postoperative week did not differ between high (8.9%) and low (9.4%) FIO2 groups (relative risk [RR]: 0.94; 95% confidence interval [CI]: 0.59-1.50; P=0.90]). Secondary outcomes, such as atelectasis (7.7% vs 9.8%; RR: 0.77; 95% CI: 0.48-1.25; P=0.38) and myocardial ischaemia (0.6% [n=2] vs 0% [n=0]; P=0.47) did not differ between groups. Conclusions: An oxygenation strategy using high FIO2 compared with conventional FIO2 did not reduce postoperative SSIs in abdominal surgery. No differences in secondary outcomes or adverse events were found.

- Abstract
Background The effects of individualised perioperative lung-protective ventilation (based on the open-lung approach [OLA]) on postoperative complications is unknown. We aimed to investigate the effects of intraoperative and postoperative ventilatory management in patients scheduled for abdominal surgery, compared with standard protective ventilation. Methods We did this prospective, multicentre, randomised controlled trial in 21 teaching hospitals in Spain. We enrolled patients who were aged 18 years or older, were scheduled to have abdominal surgery with an expected time of longer than 2 h, had intermediate-to-high-risk of developing postoperative pulmonary complications, and who had a body-mass index less than 35 kg/m(2). Patients were randomly assigned (1: 1: 1: 1) online to receive one of four lung-protective ventilation strategies using low tidal volume plus positive end-expiratory pressure (PEEP): open-lung approach (OLA)-iCPAP (individualised intraoperative ventilation [individualised PEEP after a lung recruitment manoeuvre] plus individualised postoperative continuous positive airway pressure [CPAP]), OLA-CPAP (intraoperative individualised ventilation plus postoperative CPAP), STD-CPAP (standard intraoperative ventilation plus postoperative CPAP), or STD-O-2 (standard intraoperative ventilation plus standard postoperative oxygen therapy). Patients were masked to treatment allocation. Investigators were not masked in the operating and postoperative rooms; after 24 h, data were given to a second investigator who was masked to allocations. The primary outcome was a composite of pulmonary and systemic complications during the first 7 postoperative days. We did the primary analysis using the modified intention-to-treat population. The current status of pain in our hospital, is vital to plan and develop strategies in order to follow a Pain free Hospital.Methods:An observational cross-sectional study was performed for 7 consecutive days to all patients admitted to a tertiary hospital of 300 beds. acute pain was collected by visual analog scale (VAS) at rest and in motion, VAS measurements were performed every 8 hours to all patients admitted, which generated more than 1350 VAS measurements of pain. Other variables of interest as the specific analgesic treatment (locoregional techniques),rescue analgesic medication administered, as well as sex, age, reason for admission, and specialty care was collected. Data was processed by SPSS v22 (IBM).Results:246 patients (149 surgical and 97 medical) were evaluated, the prevalence of episodes of moderate to severe pain (VAS> 4) was 53% (132/246) of all patients admitted.69% of surgical patients and 30% of medical patients underwent moderate-severe pain episodes.The average number of episodes (VAS> 4) occurred 1.45 u00b12.0 times.The results showed an average pain VAS of 1.69 u00b1 1.96 IC95{0,5.6} at rest and 2.42 u00b1 2.49 IC95{0,7.4} in motion.Rescue medication was administered to 38% of the observed patients (20% medical and 25% of surgical patients) .Being acetaminophen the most used drug.7.3% of all hospitalized patients received specific analgesic techniques 18/246. De Andres1Hospital General Universitario de Valencia - Valencia (Spain)Background and Goal of Study:In some patient, procedures(anesthetic or surgical) that could produce phrenic nerve palsy should be avoided because it produces ipsilateral hemidiaphragm palsy, this significantly reduces FVC and FEV1 by 30%.Currently, the use of diaphragmatic ultrasound applied duringweaning in critical care patients has shown that after unilateral phrenic nerve injury, extubation was possible when there was a contralateral health hemidiaphragm with at least diaphragmatic thickening Fraction (DTF)>30%(1) and diaphragmatic excursion (DE)>25mm. Diaphragm evaluation using DTF and DE during the perioperative period can be useful in detecting patients at risk of suffering postoperative pulmonary complications(PPC) if hemidiaphragm palsy occurs.Materials and Methods: We conducted an observational study in 6 patients with ipsilateral hemidiaphragm palsy due to interscalenic block(ISB) for shoulder surgery at post-anesthesia care unit (PACU). In all the patients DTF and DE were obtained pre and postoperative. DTF was calculated measuring diaphragm thick at TLC and FRC in aposition zone(5-8th intercostal space at axillary line) with a linear 12Mhz probe. DE measures diaphragm movement with a convex 3-5 Mhz probe at subcostal space; we recorded SaO2/FiO2,FR, and PPC at PACU.Results and Discussion: The echographic evaluation of the diaphragm lasted 8u00b13 minutes. 83% of patients (5/6) has postoperative complete palsy of ipsilateral hemidiaphragm after ISB. But only 16% of patients (1/6) has postoperative pulmonary complications. Patient ID2 has Dyspnoea, tachypnoea of 34 rpm, and SaO2/FiO2 reduction from 466 to 247 at PACU. Diaphragm evaluation of ID2 shows DTF=28% and DE=14mm. In this small case report series study, patients with ipsilateral hemidiaphragm palsy after ISBwere common, this fact didnu00b4t produce PPC because contralateral healthy hemidiaphragm has DTF>30% and DE>25mm. the only patient with PPC has DTF

- Abstract
Background: Postoperative pulmonary and non-pulmonary complications are common problems that increase morbidity and mortality in surgical patients, even though the incidence has decreased with the increased use of protective lung ventilation strategies. Previous trials have focused on standard strategies in the intraoperative or postoperative period, but without personalizing these strategies to suit the needs of each individual patient and without considering both these periods as a global perioperative lung-protective approach. The trial presented here aims at comparing postoperative complications when using an individualized ventilatory management strategy in the intraoperative and immediate postoperative periods with those when using a standard protective ventilation strategy in patients scheduled for major abdominal surgery. Methods: This is a comparative, prospective, multicenter, randomized, and controlled, four-arm trial that will include 1012 patients with an intermediate or high risk for postoperative pulmonary complications. The patients will be divided into four groups: (1) individualized perioperative group: intra-and postoperative individualized strategy; (2) intraoperative individualized strategy + postoperative continuous positive airway pressure (CPAP); (3) intraoperative standard ventilation + postoperative CPAP; (4) intra-and postoperative standard strategy (conventional strategy). The primary outcome is a composite analysis of postoperative complications. Discussion: The Individualized Perioperative Open-lung Ventilatory Strategy (iPROVE) is the first multicenter, randomized, and controlled trial to investigate whether an individualized perioperative approach prevents postoperative pulmonary complications.

Máster Internacional en GESTIÓN PESQUERA SOSTENIBLE (4ª edición: 2011-2013) Con el objetivo general de valorar si el uso de anzuelos circulares contribuiría a la reducción de capturas incidentales de tortugas marinas en la pesquería de palangre que se realiza en la ZEE de la República de Panamá, el programa creado con ese objetivo por un grupo de organizaciones entre estas ASOEXPEBLA, CIAT y WWF, propuso a la flota palangrera, realizar intercambios de anzuelos tradicionales (J y circular tamaño 14/0) por anzuelos circulares de mayor tamaño (15/0 y 16/0). A través de la implementación de un programa de observadores pesqueros a bordo se pudo experimentar (entre 2005 – 2009) con diversos tamaños de anzuelos circulares y se recopiló la información referente a las capturas de especies objetivas de la pesca y no objetivas. Se analizó la información pesquera de 796 lances en donde se usaron un total de 508,553 anzuelos en la pesca dirigida a la captura del Atún aleta amarilla (Thunnus albacares) y del Dorado (Coryphaena hippurus), probando 8 combinaciones de anzuelos. El 98,5 % de las tortugas marinas capturadas incidentalmente fueron recuperadas con vida al momento de la virada del palangre. Los análisis muestran diversos resultados para cada combinación de anzuelos analizada. En la pesca del Atún aleta amarilla el anzuelo C16 reduce significativamente la CPUE de L. olivacea con respecto al anzuelo tradicional J, y el C15 presenta una reducción de CPUE marginalmente significativas frente al J. En la pesca del Dorado, el anzuelo C16 reduce significativamente la CPUE de tortugas marinas con respecto al anzuelo C15 (p-valor 0,003). Las reducciones en la CPUE de las especies Chelonia mydas y C. mydas agassizii se detectaron en las combinaciones C15 vs C14 y C15 vs C13 y fueron significativamente menores con los anzuelos de menor tamaño C14 (p-valor 0,036) y C13 (p-valor 0,05) frente al anzuelo C15.
