Abstract

The ANATOMIX beamline at Synchrotron SOLEIL, operational since 2018, is dedicated to hard X-ray full-field tomography techniques. Operating in a range of photon energies from approximately 5 to 50 keV, it offers both parallel-beam projection microtomography, in absorption and phase contrast, and nanotomography using a zone-plate transmission X-ray microscope. With these methods, the beamline covers a range of spatial resolution from 20 nm to 20 $\mu$m, expressed in terms of useful pixel size. The variable beam size of up to 40 mm allows users to image large objects. Here we describe the microtomography instrumentation of the beamline., Comment: 6 pages, 3 figures. 4. Are Faba Bean and Pea Proteins Potential Whey Protein Substitutes in Infant Formulas? An In Vitro Dynamic Digestion Approach

Abstract

Infant formulas (IFs) are used as substitutes for human milk and are mostly based on cow milk proteins. For sustainability reasons, animal protein alternatives in food are increasingly being considered, as plant proteins offer interesting nutritional and functional benefits for the development of innovative IFs. This study aimed to assess how a partial substitution (50%) of dairy proteins with faba bean and pea proteins influenced the digestibility of IFs under simulated dynamic in vitro digestion, which were set up to mimic infant digestion. Pea- and faba bean-based IFs (PIF and FIF, respectively) have led to a faster aggregation than the reference milk-based IF (RIF) in the gastric compartment, that did not affect the digesta microstructure at the end of digestion. The extent of proteolysis was estimated via the hydrolysis degree, which was the highest for FIF (73%) and the lowest for RIF (50%). Finally, it was apparent that in vitro protein digestibility and protein digestibility-corrected amino acid score (PDCAAS)-like scores were similar for RIF and FIF (90% digestibility, 75% PDCAAS), but lower for PIF (75%, 67%). Therefore, this study confirms that faba bean proteins could be a good candidate for partial substitution of whey proteins in IFs from a nutritional point of view, provided that these in vitro results are confirmed in vivo.

Abstract

1000 profiles chosen randomly from an in-house database of 6000 profiles were searched against the database for matches with at least one shared allele per locus. The database contains profiles that have been analyzed with Identifiler Plus (15 markers) for biological relationship and DNA identification purposes and both true and false matches are expected to be obtained. 100 pairs of at least one true paternity match and one false match were selected and were initially supplemented with 5 additional STRs representing the new Core CODIS set. Study of the LR value showed that when false matches were treated as paternity matches, the expansion of the marker set severely diminished the LR values obtained compared to true matches and the false positive ratio of familial database searching. When false matches were treated as full-sibling matches, the expansion to 20 STRs also diminished the number of false matches and the corresponding LR values compared to true full-sibling cases, but the effect was less dramatic. Addition of the SE33 marker, further promoted distinction between true and false matches both in paternity and full-sibling cases. Counting the number of shared alleles presented improved distinction efficiency between true and false matches after STR expansion to20 and 21 STRs but remains a less valuable method of familial DNA database searching compared to LR.

Abstract

 Premise of the study: In abiotically severe habitats, intraspecifi c aggregations can increase species ’ fi tness by ameliorating stressful environmental factors. However, the diffi culty of identifying individual plants in some growth forms makes the measurements of intraspecifi c aggregation, and therefore the assessment of intraspecifi c facilitation, problematic. In this study, we examined the genotype composition within cushions of Azorella selago , a sub-Antarctic cushion plant, to investigate the potential extent of intraspecifi c facilitation.  Methods: The study was performed on Marion Island, South Africa. Two to eight samples were collected from 42 A. selago cushions, comprising eight different growth forms. Samples were genotyped using seven microsatellite markers.  Key results: We showed that all cushion shapes, with the exception of small cushions, may be comprised of more than one genetically distinct individual.  Conclusions: Under harsh sub-Antarctic conditions, intraspecifi c aggregation between A. selago individuals appears common and may be driven by the positive impacts of environmental amelioration.

Abstract

Scholars are divided as to the origins of the settlers in the highlands of Palestine, during Iron Age I. Some (e.g. Rainey) are convinced that they came from the east, while others (e.g. Dever) are of the opinion that they came from the coastal plain. Irrespective of their origins, however, in this article it is proposed that the migration to the highlands was due, at least in part, to the fact that Palestine, from the late 13th century to about 900 B.C.E., became significantly drier.

19. A GENERALIZED NONLINEAR GUIDING CENTER THEORY FOR THE COLLISIONLESS ANOMALOUS PERPENDICULAR DIFFUSION OF COSMIC RAYS

Abstract

The original nonlinear guiding center (NLGC) theory by Matthaeus et al. was a breakthrough in establishing a theory that promised to reproduce for the first time the anomalous perpendicular diffusion results from test particle trajectory calculations in prescribed static magnetic field turbulence dominated by a two-dimensional component. The assumptions used in this approach guaranteed anomalous diffusion to be a classical process (the variance [?]Dx 2[?] [?] (Dt)a with a = 1). However, Shalchi & Kourakis showed that similar calculations can be even better reproduced within the context of a generalized compound diffusion model for anomalous perpendicular diffusion whereby anomalous diffusion is nonclassical (0 < a < 1 (subdiffusion) or 1 < a < 2 (superdiffusion)). In this paper, it is shown how NLGC theory can be generalized to model compound diffusion conditions consistent with the generalized compound diffusion model in terms of a nonuniform plasma medium. Such a medium is assumed to generate a distribution of cosmic-ray pitch-angle scattering times for cosmic rays interacting resonantly with a minor small-scale slab turbulence component. This suggests that the anomalous perpendicular diffusion from the test particle simulations might be explained best within the framework of anomalous diffusion in a nonuniform plasma medium. It is argued that, during intermediate times when the magnetic field appears to be static in the quiet solar wind near Earth, generalized NLGC theory predicts possibly subdiffusive anomalous perpendicular transport for intermediate-energy (E [?] 400-500 MeV) cosmic rays because they experience nonuniform scattering conditions. These conditions are suggested to be a product of stochastic wave growth of small-scale slab turbulence resulting intermittently in large patches of intense slab turbulence where scattering times are reduced and cosmic rays are trapped. Classical anomalous diffusion is expected to be restored at high cosmic-ray energies (E [?] 400-500 MeV) in accordance with original NLGC theory because they encounter more uniform scattering conditions due to weaker stochastic wave growth. Magnetic turbulence observations near Earth during 2003 were used to show that the solar wind can be characterized as a nonuniform medium sustaining a power-law distribution of cosmic-ray pitch-angle scattering times. However, application of generalized NLGC theory to the 2003 observations produced unexpectedly a negative exponent for the variance of the anomalous diffusion (a < 0). This indicates that particle bunching occurs as cosmic rays are trapped in plasma regions associated with strong particle scattering. Such effects can plausibly be explained by the intermittent presence of strong compressive turbulence downstream of shocks, stream, and interaction regions.

20. TIME-DEPENDENT ACCELERATION OF INTERSTELLAR PICKUP IONS AT THE HELIOSPHERIC TERMINATION SHOCK USING A FOCUSED TRANSPORT APPROACH

Abstract

A time-dependent focused transport approach to modeling diffusive shock acceleration of interstellar pickup ions at the termination shock is discussed. By taking into account time variations in the magnetic field angle, and thus by implication in the shock obliquity and injection speed at the termination shock using Voyager 1 observations as guide, we show that unaccelerated core interstellar pickup protons can be accelerated by the nearly perpendicular termination shock at the heliospheric nose. Many features of anomalous cosmic rays, observed by the Voyager spacecraft at energies below the big spectral dip starting at [?]3 MeV, can be successfully reproduced with this approach. This includes multiple power-law spectral slopes with stable breaking points, power-law spectral slopes that are harder than predicted by standard diffusive shock acceleration, large pitch-angle anisotropies with strong time variations upstream that converge to steady-state isotropy in the heliosheath, upstream pitch-angle anisotropies that peak at [?]1 MeV, episodic strongly anisotropic intensity spikes at the termination shock, and strong spectral volatility upstream that is reduced downstream and almost completely disappears farther downstream.

21. The Effects of a ?-Distribution in the Heliosheath on the Global Heliosphere and ENA Flux at 1 AU

Abstract

In this paper we investigate heliosheath energetic neutral atom (ENA) fluxes at keV energies, by assuming that the heliosheath proton distribution can be approximated using a k-distribution. The choice of the k-parameter derives from observational data of the solar wind (SW). Our work has direct applications to the upcoming IBEX mission, since we generate all-sky ENA maps within the IBEX energy range (10 eV-6 keV), as well as ENA energy spectra in several directions. We find that the use of k, as opposed to a Maxwellian, gives rise to greatly increased ENA fluxes above 1 keV, while medium-energy fluxes are somewhat reduced. We show how IBEX data can be used to estimate the spectral slope of the proton distribution in the heliosheath, and that the use of k reduces the differences between ENA maps at different energies. We also investigate the effect that introducing a k-distribution has on the global interaction between the SW and the local interstellar medium (LISM), and find that there is generally an increase in energy transport from the heliosphere into the LISM, due to the modified profile of ENA energies. This results in a termination shock that moves out by 4 AU, a heliopause that moves in by 9 AU, and a bow shock 25 AU farther out, in the nose direction.

22. Nonlinear Cosmic-Ray Diffusive Transport in Combined Two-dimensional and Slab Magnetohydrodynamic Turbulence: A BGK-Boltzmann Approach

Abstract

Particle simulations revealed that standard quasi-linear theory cannot adequately describe the parallel and perpendicular diffusion of cosmic rays in three-dimensional magnetohydrodynamic solar wind turbulence, when the turbulence is modeled approximately as a combination of a dominant two-dimensional component and a minor slab component. Recent nonlinear theories based on a Taylor-Green-Kubo formalism, such as the nonlinear guiding center (NLGC) theory, and the weakly nonlinear theory (WNLT) proved to be much more successful. Instead, we follow a BGK-Boltzmann approach to investigate this matter. Going beyond current NLGC theory and WNLT in scope, we derived a complete cosmic-ray transport theory that includes expressions not only for parallel and perpendicular diffusion, but also for drift, convection, adiabatic energy change, and momentum diffusion. The BGK-Boltzmann approach enables one to derive tractable yet complicated expressions for the transport coefficients in both the weak and strong particle scattering limits. We show that the WNLT expressions can be recovered by combining these two limits in such a way that there is weak particle scattering along the magnetic field but strong scattering across the field. It is shown that the complex WNLT expressions can be reduced to simple analytical expressions for parallel and perpendicular diffusion that reproduce approximately the rigidity dependence of particle simulations at low to medium rigidities. These expressions also prove to be consistent with well-known expressions for perpendicular diffusion in the literature. It is discussed how large-scale gradient and curvature drifts get modified by turbulence and how stochastic particle acceleration changes when two-dimensional turbulence is dominant.

23. A Focused Transport Approach to Pickup Ion Shock Acceleration: Implications for the Termination Shock

Abstract

The discovery of low-energy suprathermal ions coming directly from the solar wind termination shock by the Voyager 1 spacecraft revealed strong beaming of these particles along the direction of the large-scale magnetic field. Since such large anisotropies exclude the use of standard cosmic-ray transport theory for nearly isotropic particle distributions in modeling the acceleration of these particles at the termination shock, we developed a shock acceleration model based on the numerical solution of the standard focused kinetic transport equation. After investigating the appropriateness of the physical content of this equation for shock acceleration, and applying the model to shocks of varying shock obliquity, we find that (1) this approach is a viable alternative to more sophisticated particle codes such as hybrid codes for simulating the shock acceleration of pickup ions and (2) that the model has the potential to reproduce many of the interesting features of energetic ions observed by the Voyager spacecraft at the termination shock.

24. Compound and Perpendicular Diffusion of Cosmic Rays and Random Walk of the Field Lines. I. Parallel Particle Transport Models

Abstract

A Chapman-Kolmogorov equation description of compound transport of cosmic rays due to random walk of the magnetic field lines, and for a range of models for particle transport along the field, is developed. The probability distribution, Pp, for the particle propagation along the field corresponds to either (1) a ballistic or scatter-free model, (2) a parallel diffusion model, or (3) a telegrapher equation model. The probability distribution function (pdf) describing the magnetic field statistics, PFRW, is assumed to be Gaussian. These models are used to discuss features of the dropout events in the low-energy, solar cosmic-ray intensity observed by Mazur et al. We show that the Chuvilgin and Ptuskin transport equation for compound diffusion, at sufficiently late times, can be written as a fractional Fokker-Planck equation, involving ordinary diffusion parallel to the mean magnetic field and compound diffusion of the particles normal to the field. The Green's function solution of the equation and the corresponding spatial moments of the particle transport, both parallel and perpendicular to the field, are obtained. The two-dimensional pdf for compound diffusion across the field is obtained as an inverse Laplace transform, or as a real integral.

25. Nonlinear Energetic Charged Particle Transport and Energization in Enhanced Compressive Wave Turbulence near Shocks

Abstract

During solar maximum, interplanetary coronal mass ejections and associated interplanetary shocks occur frequently. These structures are often accompanied by high levels of low-frequency compressive wave turbulence, which requires a nonlinear extension of standard quasi-linear theory to properly describe energetic particle transport in their vicinities. The same might be true for the solar wind termination shock. We present a nonlinear diffusive kinetic theory for suprathermal particle transport and stochastic acceleration along the background magnetic field in strong compressive dynamic wave turbulence to which small-scale Alfven waves are coupled. Our theory shows that the standard cosmic-ray transport equation must be revised for low suprathermal particle energies to accommodate fundamental changes in spatial diffusion (standard diffusion becomes turbulent diffusion), as well as modifications to particle convection and adiabatic energy changes. In addition, a momentum diffusion term, which generates accelerated suprathermal particle spectra with a hard power law, must be added. Such effective first-stage acceleration possibly leads to efficient injection of particles into second-stage diffusive shock acceleration, as described by standard theory.

26. Energetic Charged Particle Transport and Energization in Dynamic Two-dimensional Turbulence

Abstract

Quasi-linear theory has been used extensively to study the interaction of energetic particles with MHD fluctuations in the solar wind. However, in recent years there has developed a view that solar wind MHD turbulence can be modeled approximately as a dominating incompressible two-dimensional turbulence component combined with a minor one-dimensional parallel-propagating Alfven wave component (a one-dimensional slab component in the static limit). Here a quasi-linear theory is developed to investigate the effect of dynamical two-dimensional MHD turbulence in the solar wind on low-energy charged particle pitch-angle scattering and momentum diffusion. Stochastic acceleration by transverse two-dimensional turbulence electric field fluctuations is also considered, yielding finite momentum diffusion coefficients. We find significant effects by energy-containing-scale, dynamic two-dimensional turbulence on low-energy particles in the vicinity of 1 keV energies, and overall dominance of parallel-propagating Alfven waves at higher energies.

28. The Injection Problem for Anomalous Cosmic Rays

Abstract

The precise mechanism by which some interstellar pickup ions are selected to be accelerated up to anomalous cosmic-ray (ACR) energies at the heliospheric termination shock is a central puzzle in the physics of the outer heliosphere. Observations by Gloeckler et al. of the mass dependence of the efficiency with which low-energy pickup ions are accelerated at an interplanetary shock is in the opposite sense of related ACR observations made by Cummings and Stone. Furthermore, the rapidity with which ions must be accelerated at the termination shock requires that the scattering of these ions be weak, an effect which requires that relatively energetic ions exist initially. It is shown here that these three sets of observations can be reconciled by using a multiply reflected ion acceleration mechanism to provide a preaccelerated population of pickup ions at the termination shock. The precise fraction of preenergized pickup ions that can subsequently be diffusively shock accelerated depends on the ion scattering strength at the termination shock. It is shown that the assumption of weak scattering yields an anomalously low injection efficiency for pickup H+ compared to those of He+, O+, and Ne+, and those inferred for C+ and N+. The predicted injection efficiencies of C+, N+, O+, and Ne+ are clustered together closely whereas He+ is a little less efficiently injected. Computed ACR termination shock and modulated fluxes compare well to those observed and inferred by Cummings and Stone. Finally, strong scattering of ions at the termination shock is found to eliminate the mass dependence of the ACR injection efficiency.

Abstract

International audience; OBJECTIVE: Admission in the intensive care unit of the old patient with coronavirus disease 19 raises an ethical question concerning the scarce resources and their short-term mortality. METHODS: Patients aged over 60 from 7 different intensive care units admitted between March 1, 2020 and May 6, 2020, with a diagnosis of coronavirus disease 19 were included in the cohort. Twenty variables were collected during the admission, such as age, severity (Simplified Acute Physiology Score [SAPS] II), several data on physiological status before intensive care unit comorbidities, evaluation of autonomy, frailty, and biological variables. The objective was to model the 30-day mortality with relevant variables, compute their odds ratio associated with their 95% CI, and produce a nomogram to easily estimate and communicate the 30-day mortality. The performance of the model was estimated with the area under the receiving operating curve. RESULTS: We included 231 patients, among them 60 (26.0%) patients have died on the 30th day. The relevant variables selected to explain the 30-day mortality were Instrumental Activities of Daily Living (IADL) score (0.82 [0.71-0.94]), age 1.12 (1.07-1.18), SAPS II 1.05 (1.02-1.08), and dementia 6.22 (1.00-38.58). A nomogram was computed to visually represent the final model. Area under the receiving operating curve was at 0.833 (0.776-0.889). CONCLUSIONS: Age, autonomy, dementia, and severity at admission were important predictive variables for the 30-day mortality status, and the nomogram could help the physician in the decision-making process and the communication with the family.

Abstract

International audience; Oxaliplatin, a pivotal drug in the management of colorectal cancer, causes chemotherapy-induced peripheral neuropathy (CIPN) in a third of cancer survivors. Based on a previous cross-sectional study assessing oxaliplatin-related sensory CIPN in colorectal cancer survivors, a secondary analysis was designed to explore the possibility that different clusters of patients may co-exist among a cohort of patients with oxaliplatin-related CIPN. Other objectives were to characterize these clusters considering CIPN severity, anxiety, depression, health-related quality of life (HRQOL), patients’ characteristics and oxaliplatin treatments. Among the 96 patients analyzed, three clusters were identified (cluster 1: 52, cluster 2: 34, and cluster 3: 10 patients). Clusters were significantly different according to CIPN severity and the proportion of neuropathic pain (cluster 1: low, cluster 2: intermediate, and cluster 3: high). Anxiety, depressive disorders and HRQOL alteration were lower in cluster 1 in comparison to clusters 2 and 3, but not different between clusters 2 and 3. This study underlines that patients with CIPN are not a homogenous group, and that CIPN severity is associated with psychological distress and a decline of HRQOL. Further studies are needed to explore the relation between clusters and CIPN management.

Local antibody responses in the nasopharyngeal epithelium, that are likely to determine the course of infection, have not been analysed so far nor their correlation with antibody responses in serum.MethodsThe IgG and IgA antibody responses were analysed in the plasma as well as in nasopharyngeal swabs (NPS) from the first four COVID-19 patients confirmed by RT-qPCR in France. Two were pauci-symptomatic while two developed severe disease. Taking advantage of a comprehensive series of plasma and nasopharyngeal samples, we characterized their antibody profiles from the second week post symptoms onset, by using an in-house ELISA to detect anti-SARS-CoV-2 Nucleoprotein (N) IgG and IgA.ResultsAnti-N IgG and IgA antibodies were detected in the NPS of severe patients. Overall, the levels of IgA and IgG antibodies in plasma and NPS appeared specific to each patient.ConclusionsAnti-N IgG and IgA antibodies are detected in NPS, and their levels are related to antibody levels in plasma. The two patients with severe disease exhibited different antibody profiles that may reflect different disease outcome. For the pauci-symptomatic patients, one showed a low anti-N IgG and IgA response in the plasma only, while the other one did not exhibit overt serological response.

Abstract

International audience; BACKGROUND: For many years, researchers have been interested in investigating airflow and aerosol deposition in the nasal cavities. The nasal airways appear to be a complex geometrical system. Thus, in vitro experimental studies are frequently conducted with a more or less biomimetic nasal replica. AIM: This study is devoted to the development of an anatomically realistic nose model with bilateral nasal cavities, i.e. nasal anatomy, airway geometry and aerodynamic properties as close as possible to in vivo behaviour. METHODS: A specific plastination technique of cephalic extremities was developed by the Anatomy Laboratory at the Saint-Etienne University in the last 10 years. The plastinated models obtained were anatomically, geometrically and aerodynamically validated using several techniques (endoscopy, CT scans, acoustic rhinometry and rhinomanometry). RESULTS: Our plastination model exhibited a high level of anatomic quality, including a very good mucosa preservation. Aerodynamical and geometrical investigations highlighted a global behaviour of plastinated models perfectly in accordance with a nasal decongested healthy subject. CONCLUSIONS: The present plastination model provides a realistic cast of nasal airways, and may be a useful tool for nasal flow, drug delivery and aerosol deposition studies.

Abstract

International audience; To relate exposure to adverse health effects, it is necessary to know where particles in the submicron range deposit in the respiratory tract. The possibly higher vulnerability of children requires specific inhalation studies. However, radio-aerosol deposition experiments involving children are rare because of ethical restrictions related to radiation exposure. Thus, an in vivo study was conducted using three baboons as a child respiratory tract model to assess regional deposition patterns (thoracic region vs. extrathoracic region) of radioactive polydisperse aerosols ([d16-d84], equal to [0.15 μm-0.5 μm], [0.25 μm-1 μm], or [1 μm-9 μm]). Results clearly demonstrated that aerosol deposition within the thoracic region and the extrathoraic region varied substantially according to particle size. High deposition in the extrathoracic region was observed for the [1 μm-9 μm] aerosol (72% ± 17%). The [0.15 μm-0.5 μm] aerosol was associated almost exclusively with thoracic region deposition (84% ± 4%). Airborne particles in the range of [0.25 μm-1 μm] showed an intermediate deposition pattern, with 49% ± 8% in the extrathoracic region and 51% ± 8% in the thoracic region. Finally, comparison of baboon and human inhalation experiments for the [1 μm-9 μm] aerosol showed similar regional deposition, leading to the conclusion that regional deposition is species-independent for this airborne particle sizes.

41. Geriatric determinants of curative radiotherapy scheme choice for older adults with breast cancer treatment compliance and tolerance: Results from the GERABEL study.

Abstract

Introduction: Chronological and functional aging complicates care in older patients, and therapeutic decisions need to consider individual needs to minimise morbidity and mortality. Therapeutic decisions should be guided by a multidisciplinary geriatric assessment, allowing a complete assessment of physical and functional performance. In this context, the GERABEL study aimed to orientate the irradiation strategy based on a detailed oncogeriatric assessment in women more than 70 years old with breast cancer., Materials and Methods: This was a multicentre clinical trial conducted between January 2017 and June 2021. The oncogeriatric assessment comprised seven questionnaires (activities of daily living [ADL], instrumental ADL, Mini-Mental State Examination, mini Geriatric Depression Scale, Mini-Nutritional Assessment, Cumulative Illness Rating Scale-Geriatric, and Timed Up and Go) to determine a predictive score. Irradiation regimen was assigned according to oncogeriatric score: normofractionated - NF (score range, 160-200), hypofractionated - HF (score range, 159-120), or high-dose hypofractionated - HDHF (score ≤ 119). Endpoints were the overall tolerance to treatment, using the oncogeriatric score as a proxy at six months post-treatment, and compliance., Results: After oncogeriatric assessment at baseline, 177 patients were treated with a NF regimen, 24 with a HF regimen, and four with a HDHF regimen. Tolerance was excellent in the three groups, as no decrease of more than 20 % in the oncogeriatric score was noted at six months post-treatment. More generally, 68 % and 73 % of patients reported an improvement of the oncogeriatric score at two months and six months post-treatment, respectively. Only four treatment interruptions were observed and quality-of-life was well-conserved. Abstract

Introduction: Chronic inflammatory dermatoses (CIDs) can significantly affect patients' lives. The Observatory of Chronic Inflammatory Skin Diseases (OMCCI) cohort was initiated to quantify the impact and disease evolution of four CID over 4 years' follow-up; at least 1,000 patients per CID are planned to be enrolled. The objective of this study was to present baseline characteristics of patients included in the OMCCI cohort between December 2020 and September 2022., Methods: This French, prospective, multicenter registry included adult patients treated in daily practice for moderate-to-severe psoriasis (PS), atopic dermatitis (AD), hidradenitis suppurativa (HS), or chronic urticaria (CU) starting or modifying a systemic treatment. At the inclusion visit and then every 6 months during 4 years, patient-reported outcomes and data on these diseases and their treatments are recorded., Results: A total of 2,058 patients from 24 centers were included: 1,137 PS, 413 AD, 301 HS, and 207 CU. Of these, 1,950 patients started or changed systemic treatment, and 108 reduced the dose of existing systemic treatment. Disease impact was qualified as debilitating by 80.1% (PS), 90.5% (AD), 90.5% (HS), and 89.4% (CU), affecting daily, family, and professional life. According to the SF-12 Survey, the impact of all four diseases was borderline pathological for physical health and severe for mental health. At inclusion, 20.4% of patients were receiving a conventional systemic or biologic treatment. After the first visit, this percentage raised to 83.3%. During the 6 months preceding study inclusion, 17.7% (PS), 27.9% (AD), 43.1% (HS), and 43.6% (CU) of patients missed work due to their illness, and 26.3% of patients with HS had been admitted to hospital (vs. 8.1%, 5.8%, and 13% of patients with PS, AD, or CU, respectively)., Conclusion: These CIDs (especially HS) had a major impact on all aspects of patients' quality of life. The low baseline use of systemic drugs and the high burden of these CIDs suggest that these agents are underused. Abstract

Background: Posttraumatic stress disorder (PTSD) after a stay in the intensive care unit (ICU) can affect one in five ICU survivors. At the beginning of the coronavirus disease 2019 (COVID-19) pandemic, admission to the ICU for COVID-19 was stressful due to the severity of this disease. This study assessed whether admission to the ICU for COVID-19 was associated with a higher prevalence of PTSD compared with other causes of ICU admission after adjustment for pre-ICU psychological factors., Methods: This prospective observational comparative cohort study included 31 ICUs. Eligible patients were adult ICU survivors hospitalized during the first wave of COVID-19 pandemic in France, regardless of the reason for admission. The prevalence of presumptive diagnosis of PTSD at 6 months was assessed using the PTSD Checklist for DSM-5 (PCL-5). Sociodemographics, clinical data, history of childhood trauma (Childhood Trauma Questionnaire [CTQ]), and exposure to potentially traumatic events (Life Events Checklist for DSM-5 [LEC-5]) were assessed., Results: Of the 778 ICU survivors included during the first wave of COVID-19 pandemic in France, 417 and 361 were assigned to the COVID-19 and non-COVID-19 cohorts, respectively. Fourteen (4.9%) and 11 (4.9%), respectively, presented with presumptive diagnosis of PTSD at 6 months (p = 0.976). After adjusting for age, sex, severity score at admission, use of invasive mechanical ventilation, ICU duration, CTQ and LEC-5, COVID-19 status was not associated with presumptive diagnosis of PTSD using the PCL-5. Only female sex was associated with presumptive diagnosis of PTSD. However, COVID-19 patients reported significantly more intrusion and avoidance symptoms than non-COVID patients (39% vs. 29%, p = 0.015 and 27% vs. 19%, p = 0.030), respectively. The median PCL-5 score was higher in the COVID-19 than non-COVID-19 cohort (9 [3, 20] vs. 4 [2, 16], p = 0.034)., Conclusion: Admission to the ICU for COVID-19 was not associated with a higher prevalence of PTSD compared with admission for another cause during the first wave of the COVID-19 pandemic in France. Abstract

Background: The determination of cytokines in the postoperative drainage (POD) fluid could be a method for early detection of the development of a pharyngocutaneous fistula (PCF)., Materials and Methods: We conducted a prospective two-center study involving 28 patients. PODs were collected on Day 1 (D1) and Day 2 (D2) postoperatively for determination of a cytokine panel and cytobacteriological examination., Results: Eleven (39%) patients presented with PCF on average 13 ± 5.5 days after surgery. Patients with PCF had higher IL-10 (121 vs. 40.3, p = 0.04, effect size (ES) = 0.98 [0.16, 1.79]) and TNFα level (21.2 vs. 2.2, p = 0.02, ES = 0.83 [0.03, 1.63]) on D2. An IL-10 threshold of 72 pg/mL on D2 was diagnostic of the occurrence of PCF with a sensibility of 70%, specificity of 88%., Conclusion: The determination of cytokines in POD fluid on D2 is a reliable tool for predicting the development of a PCF after total laryngectomy., (© 2023 Wiley Periodicals LLC.)

Abstract

The impact of immunosuppressive therapy (IS) strategies after kidney transplant failure (KTF) on potential future new grafts is poorly established. We assessed the potential benefit of calcineurin inhibitor (CNI)-based IS maintenance throughout the dialysis period on the outcome of the second kidney transplant (KT). We identified 407 patients who underwent a second KT between January 2008 and December 2018 at four French KT centers. Inverse probability of treatment weighting was used to control for potential confounding. We included 205 patients with similar baseline characteristics at KTF: a total of 53 received at least CNIs on the retransplant day (G-CNI), and 152 did not receive any IS (G-STOP). On the retransplant date, G-STOP patients experienced a longer pretransplant dialysis time, were more often hyperimmunized, and underwent more expanded-criteria donor KTs than G-CNI patients. During the second KT follow-up period, rejection episodes were similar in both groups. The 10-year survival rates without death and dialysis were 98.7% and 59.5% in G-CNI and G-STOP patients, respectively. In the multivariable analysis, CNI-based IS maintenance was associated with better survival (hazard ratio: 0.08; 95% confidence interval: 0.01-0.58, p = 0.01). Abstract

Background: The optimal calorie and protein intakes at the acute phase of severe critical illness remain unknown. We hypothesised that early calorie and protein restriction improved outcomes in these patients, compared with standard calorie and protein targets., Methods: The pragmatic, randomised, controlled, multicentre, open-label, parallel-group NUTRIREA-3 trial was performed in 61 French intensive care units (ICUs). Adults (≥18 years) receiving invasive mechanical ventilation and vasopressor support for shock were randomly assigned to early nutrition (started within 24 h after intubation) with either low or standard calorie and protein targets (6 kcal/kg per day and 0·2-0·4 g/kg per day protein vs 25 kcal/kg per day and 1·0-1·3 g/kg per day protein) during the first 7 ICU days. The two primary endpoints were time to readiness for ICU discharge and day 90 all-cause mortality. Key secondary outcomes included secondary infections, gastrointestinal events, and liver dysfunction. The trial is registered on ClinicalTrials.gov, NCT03573739, and is completed., Findings: Of 3044 patients randomly assigned between July 5, 2018, and 8 Dec 8, 2020, eight withdrew consent to participation. By day 90, 628 (41·3%) of 1521 patients in the low group and 648 (42·8%) of 1515 patients in the standard group had died (absolute difference -1·5%, 95% CI -5·0 to 2·0; p=0·41). Median time to readiness for ICU discharge was 8·0 days (IQR 5·0-14·0) in the low group and 9·0 days (5·0-17·0) in the standard group (hazard ratio [HR] 1·12, 95% CI 1·02 to 1·22; p=0·015). Proportions of patients with secondary infections did not differ between the groups (HR 0·85, 0·71 to 1·01; p=0·06). The low group had lower proportions of patients with vomiting (HR 0·77, 0·67 to 0·89; p<0·001), diarrhoea (0·83, 0·73 to 0·94; p=0·004), bowel ischaemia (0·50, 0·26 to 0·95; p=0·030), and liver dysfunction (0·92, 0·86-0·99; p=0·032)., Interpretation: Compared with standard calorie and protein targets, early calorie and protein restriction did not decrease mortality but was associated with faster recovery and fewer complications., Funding: French Ministry of Health., Competing Interests: Declaration of interests We declare no competing interests., (Copyright © 2023 Elsevier Ltd. Abstract

Imlifidase recently received early access authorization for highly sensitized adult kidney transplant candidates with a positive crossmatch against an ABO-compatible deceased donor. These French consensus guidelines have been generated by an expert working group, in order to homogenize patient selection, associated treatments and follow-up. This initiative is part of an international effort to analyze properly the benefits and tolerance of this new costly treatment in real-life. Eligible patients must meet the following screening criteria: cPRA ≥ 98%, ≤ 65-year of age, ≥ 3 years on the waiting list, and a low risk of biopsy-related complications. The final decision to use Imlifidase will be based on the two following criteria. First, the results of a virtual crossmatch on recent serum, which shall show a MFI for the immunodominant donor-specific antibodies (DSA) > 6,000 but the value of which does not exceed 5,000 after 1:10 dilution. Second, the post-Imlifidase complement-dependent cytotoxicity crossmatch must be negative. Patients treated with Imlifidase will receive an immunosuppressive regimen based on steroids, rATG, high dose IVIg, rituximab, tacrolimus and mycophenolic acid. Abstract

Introduction: Sleep disorders are still often underestimated in patient care management even though they are present in the criteria of the American College of Rheumatology for the diagnosis of fibromyalgia syndrome (FMS). The objective of this study will be to assess the current situation of sleep disorders in patients with FMS in France and to estimate its prevalence., Methods and Analysis: The FIBOBS study is a multicentred, prospective, observational trial performed by 46 specialised chronic pain structures in France. Patients with FMS visiting for a first consultation or follow-up (if they have already been followed up for less than a year with a pain management service) will be included after giving their informed consent. Data will be collected through the physician questionnaire filled during the inclusion visit. Patient self-questionnaires will be completed from home. The primary outcome of the study will be to estimate the prevalence of sleep disorders classified into three categories: (a) poor sleep quality in general, (b) sleep apnoea syndrome and (c) restless legs syndrome, using self-administered questionnaires., Ethics and Dissemination: This protocol is approved by the ethics committee Comité de Protection des Personnes ' Ile de France II' in accordance with French regulations.
