Abstract Childhood adversity is associated with various clinical dimensions in psychosis; however, how genetic vulnerability shapes the adversity-associated psychopathological signature is yet to be studied. We studied data of 583 First Episode Psychosis (FEP) cases from the EU-GEI FEP case-control study, including Polygenic risk scores for major depressive disorder (MDD-PRS), bipolar disorder (BD-PRS) and schizophrenia (SZ-PRS); childhood adversity measured with the total score of the Childhood Trauma Questionnaire (CTQ); and positive, negative, depressive and manic psychopathological domains from a factor model of transdiagnostic dimensions. Genes and environment interactions were explored as a departure from a multiplicative effect of PRSs and total CTQ on each dimension. Analyses were adjusted for age, sex, 10 PCA, site of recruitment and for medication. A childhood adversity and PRS multiplicative interaction was observed between A) the CTQ and MDD-PRS on the predominance of positive (β = 0.42, 95% CI = [0.155, 0.682], p = 0.004); and depressive (β = 0.33, 95% CI = [0.071, 0.591], p = 0.013) dimensions; B) between the CTQ and BD-PRS on the positive dimension (β = 0.45, 95% CI = [0.106, 0.798], p = 0.010), and C) with the CTQ and SZ-PRS on the positive dimension (β = −0.34, 95% CI = [−0.660, −0.015], p = 0.040). Bonferroni corrected p-value of significance was set at 0.0125. In conclusion, despite being underpowered, this study suggests that genetic liability for MDD and BD may have a moderating effect on the sensibility of childhood adversity on depressive and positive psychotic dimensions. This supports the hypothesis of an affective pathway to psychosis in those exposed to childhood adversity.

Objective: To ascertain whether sociodemographic and health-related characteristics known from previous research to have a substantive impact on recovery from depression modified the effect of a digital intervention designed to improve depressive symptoms (CONEMO). Methods: The CONEMO study consisted of two randomized controlled trials, one conducted in Lima, Peru, and one in São Paulo, Brazil. As a secondary trial plan analysis, mixed logistic regression was used to explore interactions between the treatment arm and subgroups of interest defined by characteristics measured before randomization – suicidal ideation, race/color, age, gender, income, type of mobile phone, alcohol misuse, tobacco use, and diabetes/hypertension – in both trials. We estimated interaction effects between the treatment group and these subgroup factors for the secondary outcomes using linear mixed regression models. Results: Increased effects of the CONEMO intervention on the primary outcome (reduction of at least 50% in depressive symptom scores at 3-month follow-up) were observed among older and wealthier participants in the Lima trial (p = 0.030 and p = 0.001, respectively). Conclusion: There was no evidence of such differential effects in São Paulo, and no evidence of impact of any other secondary outcomes in either trial. Objectives: Gene-environment interactions increase the risk of psychosis. The objective of this study was to investigate gene-gene and gene-environment interactions in psychosis, including single nucleotide variants (SNVs) of dopamine-2 receptor (D2R), N-methyl-d-aspartate receptor (NMDAR), and cannabinoid receptor type 1 (CB1R), lifetime cannabis use, and childhood trauma. Methods: Twenty-three SNVs of genes encoding D2R (DRD2: rs1799978, rs7131056, rs6275), NMDAR (GRIN1: rs4880213, rs11146020; GRIN2A: rs1420040, rs11866328; GRIN2B: rs890, rs2098469, rs7298664), and CB1R (CNR1: rs806380, rs806379, rs1049353, rs6454674, rs1535255, rs2023239, rs12720071, rs6928499, rs806374, rs7766029, rs806378, rs10485170, rs9450898) were genotyped in 143 first-episode psychosis patients (FEPp) and 286 community-based controls by Illumina HumanCoreExome-24 BeadChip. Gene-gene and gene-environment associations were assessed using nonparametric Multifactor Dimensionality Reduction software. Results: Single-locus analyses among the 23 SNVs for psychosis and gene-gene interactions were not significant (p > 0.05 for all comparisons); however, both environmental risk factors showed an association with psychosis (p < 0.001). Moreover, gene-environment interactions were significant for an SNV in CNR1 and cannabis use. The best-performing model was the combination of CNR1 rs12720071 and lifetime cannabis use (p < 0.001), suggesting an increased risk of psychosis. Conclusion: Our study supports the hypothesis of gene-environment interactions for psychosis involving T-allele carriers of CNR1 SNVs, childhood trauma, and cannabis use.

Abstract Higher levels of interleukin (IL)-6 and elevated neutrophil counts are consistently reported in the blood of patients with schizophrenia. Stressors during childhood and/or adolescence are major socioenvironmental risk factors for schizophrenia and may contribute to immune dysregulation. Previous studies using blood cytokines to stratify patients with schizophrenia suggest that only a subset presents a low-grade inflammatory state. However, these studies have not addressed whether environmental factors such as childhood maltreatment contributed to identifying inflammatory clusters. Moreover, a neutrophil-related mechanism (Neutrophil Extracellular Traps; NETs) central to both the initiation and chronicity of autoimmune and inflammatory diseases has never been investigated in psychiatry. Elevated NETs in schizophrenia may predispose patients to inflammatory and autoimmune diseases resulting in reduced life expectancy. We, therefore, investigated NETs as a novel mechanism and biological target in early schizophrenia and their role together with IL-6 and childhood maltreatment in identifying cluster subgroups. We found increased NETs in the plasma of patients with early schizophrenia (n = 78) compared to both their unaffected siblings (n = 25) and community controls (n = 78), irrespective of sex, body mass index, psychoactive drug use, or tobacco smoking. Increased NETs in patients were unrelated to antipsychotic treatment, which was further tested in vitro using fresh neutrophils. By applying unsupervised two-step clustering analysis, we integrated values of NETs, IL-6, and childhood maltreatment scores. We identified two main clusters; childhood maltreatment scores and NETs were the most important variables contributing to cluster separation (high-CL1 and low-CL2), while IL-6 was the least contributor. Patients allocated in the high-CL1 (61.5%) had significantly higher childhood maltreatment scores, NETs, and IL-6 levels than the remaining groups (patients low-CL2, siblings, and controls high-CL1 and low-CL2). We complemented these findings with a rat model based on stress exposure during adolescence that results in several schizophrenia-like changes in adulthood. We found that adolescent stressed rats had higher NETs and IL-6 levels in serum compared to non-stressed rats with a tendency to produce more NETs from the bone marrow. Altogether, this study brings a novel cellular-based mechanism in schizophrenia that, combined with early-stress, could be useful to identify subgroups for more personalised treatments.

Abstract: Background Psychosis rates are higher among some migrant groups. We hypothesized that psychosis in migrants is associated with cumulative social disadvantage during different phases of migration. Methods We used data from the EUropean Network of National Schizophrenia Networks studying Gene-Environment Interactions (EU-GEI) case-control study. We defined a set of 3 indicators of social disadvantage for each phase: pre-migration, migration, and post-migration. Results 249 cases and 219 controls were assessed. Pre-migration (OR 1.61, 95%CI 1.06-2.44, p=0.027) and postmigration social disadvantages (OR 1.89, 95%CI 1.02-3.51, p=0.044), along with expectations/achievements mismatch (OR 1.14, 95%CI 1.03-1.26, p=0.014) were all significantly associated with psychosis. We found a dose-response effect between number of adversities across all phases and odds of psychosis (≥6: OR 14.09, 95%CI 2.06-96-47, p=0.007). Conclusions The cumulative effect of social disadvantages before, during and after migration was associated with increased odds of psychosis in migrants, independently of ethnicity or length of stay in the country of arrival. Public health initiatives that address the social disadvantages that many migrants face during the whole migration process and post-migration psychological support may be reduce the excess of psychosis in migrants. BackgroundDepression is one of the most prevalent mental disorders and a leading cause of disability, disproportionately affecting specific groups, such as patients with noncommunicable diseases. Over the past decade, digital interventions have been developed to provide treatment for these patients. CONEMO (Emotional Control in Spanish) is an 18-session psychoeducational digital intervention delivered through a smartphone app and minimally supported by a nurse. CONEMO demonstrated effectiveness in reducing depressive symptoms through a randomized controlled trial (RCT) among patients with diabetes, hypertension, or both, in Lima, Peru. However, in addition to clinical outcomes, it is important to explore users’ experiences, satisfaction, and perceptions of usability and acceptability, which can affect their engagement with the intervention. ObjectiveThis study aimed to explore the RCT participants’ experiences with CONEMO in Peru, complemented with information provided by the nurses who monitored them. MethodsIn 2018, semistructured interviews were conducted with a sample of 29 (13.4%) patients from the 217 patients who participated in the CONEMO intervention in Peru and the 3 hired nurses who supported its delivery. Interviewees were selected at random based on their adherence to the digital intervention (0-5, 10-14, and 15-18 sessions completed), to include different points of view. Content analysis was conducted to analyze the interviews. ResultsParticipants’ mean age was 64.4 (SD 8.5) years, and 79% (23/29) of them were women. Most of the interviewed participants (21/29, 72%) stated that CONEMO fulfilled their expectations and identified positive changes in their physical and mental health after using it. Some of these improvements were related to their thoughts and feelings (eg, think differently, be more optimistic, and feel calmer), whereas others were related to their routines (eg, go out more and improve health-related habits). Most participants (19/29, 66%) reported not having previous experience with using smartphones, and despite experiencing some initial difficulties, they managed to use CONEMO. The most valued features of the app were the videos and activities proposed for the participant to perform. Most participants (27/29, 93%) had a good opinion about the study nurses and reported feeling supported by them. A few participants provided suggestions to improve the intervention, which included adding more videos, making the sessions’ text simple, extending the length of the intervention, and improving the training session with long explanations. ConclusionsThe findings of this qualitative study provide further support and contextualize the positive results found in the CONEMO RCT, including insights into the key features that made the intervention effective and engaging. The participants’ experience with the smartphone and CONEMO app reveal that it is feasible to be used by people with little knowledge of technology. In addition, the study identified suggestions to improve the CONEMO intervention for its future scale-up. Abstract Diagnostic categories do not completely reflect the heterogeneous expression of psychosis. Using data from the EU-GEI study, we evaluated the impact of schizophrenia polygenic risk score (SZ-PRS) and patterns of cannabis use on the transdiagnostic expression of psychosis. We analysed first-episode psychosis patients (FEP) and controls, generating transdiagnostic dimensions of psychotic symptoms and experiences using item response bi-factor modelling. Linear regression was used to test the associations between these dimensions and SZ-PRS, as well as the combined effect of SZ-PRS and cannabis use on the dimensions of positive psychotic symptoms and experiences. We found associations between SZ-PRS and (1) both negative (B = 0.18; 95%CI 0.03–0.33) and positive (B = 0.19; 95%CI 0.03–0.35) symptom dimensions in 617 FEP patients, regardless of their categorical diagnosis; and (2) all the psychotic experience dimensions in 979 controls. We did not observe associations between SZ-PRS and the general and affective dimensions in FEP. Daily and current cannabis use were associated with the positive dimensions in FEP (B = 0.31; 95%CI 0.11–0.52) and in controls (B = 0.26; 95%CI 0.06–0.46), over and above SZ-PRS. We provide evidence that genetic liability to schizophrenia and cannabis use map onto transdiagnostic symptom dimensions, supporting the validity and utility of the dimensional representation of psychosis. In our sample, genetic liability to schizophrenia correlated with more severe psychosis presentation, and cannabis use conferred risk to positive symptomatology beyond the genetic risk. Our findings support the hypothesis that psychotic experiences in the general population have similar genetic substrates as clinical disorders.

Abstract Background Task-shifting and technology in psychological interventions are two solutions to increasing access to mental health intervention and overcoming the treatment gap in low and middle-income countries. The CONEMO intervention combines a smartphone app with support from non-specialized professionals, aiming to treat depression in patients with diabetes and/or hypertension. The aim of this paper is to describe the process of recruitment, training and supervision of the non-specialized professionals who participated in the CONEMO task-shifting intervention in Brazil and Peru. Methods We described and analyzed data related to the recruitment, training and supervision of 62 nurse assistants from the health system in Sao Paulo, Brazil, and three hired nurses in Lima, Peru. The data were collected from information provided by nurses and nurse assistants, supervisor records from supervision meetings and the CONEMO platform database. Results We found that task-shifting was feasible using existing resources in Sao Paulo and additional human resources in Lima. Training and supervision were found to be crucial and well received by the staff; however, time was a limitation when using existing human resources. Ensuring technological competence prior to the start of the intervention was essential. Group supervision meetings allowed non-specialized professionals to learn from each other’s experiences. Conclusion Carefully considering recruitment, training and supervision of non-specialized professionals is important for effective task-shifting when delivering an mHealth intervention for depression. Opportunities and challenges of working in different health systems are described, which should be considered in future implementation, either for research or real settings. Abstract Investigations of plasma amino acids in early psychosis and their unaffected siblings are rare. We measured plasma amino acids involved in the co-activation of dopaminergic, GABAergic, glutamatergic, and serotoninergic neurotransmitters in first-episode psychosis (FEP) patients (n = 166), unaffected siblings (n = 76), and community-based controls (n = 166) included in a cross-sectional study. Plasma levels of glutamic acid (GLU), glutamine, glycine, proline (PRO), tryptophan (TRP), tyrosine, serine and GABA were quantified by gas-chromatography-mass spectrometry. Resumen: Introducción: Los futuros profesionales de Medicina y Salud Pública se enfrentarán a rápidos cambios en el conocimiento científico y tecnológico derivados del proceso de la globalización. La formación universitaria debe reconocer que la internacionalización de sus actividades es imprescindible para la trasferencia de conocimiento. Los Colegios Tordesillas de Doctorado nacieron con el propósito de crear una red sólida de cooperación académico-científica entre universidades brasileñas, portuguesas y españolas. Métodos: La Universidad Miguel Hernández coordina el Colegio Doctoral Tordesillas de Salud Pública e Historia de la Ciencia compuesto por las Facultades de Medicina de las Universidades de Porto y Miguel Hernández, y de Medicina y Salud Pública de São Paulo desde julio de 2017. Su objetivo principal es dotar a los programas de posgrado de nuevos contenidos docentes y potenciar la innovación en la investigación a través de la cooperación internacional. En febrero de 2018 se celebró el primer seminario internacional para establecer alianzas en investigación y docencia. Resultados: Se encontraron intereses comunes importantes de colaboración en investigación y docencia de posgrado en las siguientes áreas: en Salud Pública en estudios sobre políticas públicas, evaluación de la innovación en pruebas diagnósticas, relaciones entre los ámbitos político y científico y conflictos de intereses; en derechos de reproducción y en la creciente medicalización de la sociedad; en Historia de la Ciencia, en estudios sobre el análisis de las reformas sanitarias en países europeos y latinoamericanos en el siglo XX. Discusión: La internacionalización universitaria es una pieza clave en la transferencia de conocimiento científico. Los Colegios Tordesillas de Doctorado ofrecen un contexto más flexible para superar obstáculos legales y administrativos para esta internacionalización. University education must recognize that the internationalization of its activities is essential for the transfer of knowledge. The Tordesillas Doctoral Schools were born with the purpose of creating a solid network of scientific academic cooperation between Brazilian, Portuguese and Spanish universities. Methods: The University Miguel Hernández has coordinated The Tordesillas Doctorate School of Public health and History of Science (July 2017) composed by the Faculty of Medicine, from Porto University, Miguel Hernández, and the Faculties of Medicine and Public Health of the University of São Paulo, whose aim is to provide postgraduate programs with new teaching content and enhance innovation in research trough international cooperation. In February 2018, the first International seminar of this The Tordesillas Doctorate School was held to establish partnerships in research and postdoctoral teaching. Results: Important common interest for research and teaching collaboration was found: for Public Health in public policies evaluation studies, assessment of innovation in diagnostic tests, relations between the political and scientific agenda and conflict of interests, studies on health reproduction rights, and the increasing medicalization of society in all contexts. For History of Science interest was shown in analysis of sanitary reforms carried out in European and Latin American countries in the 2nd half of the 20th century. Discussion: The university internationalization is as a key piece in the transfer of scientific knowledge. The Tordesillas Doctoral Schools offer a more flexible context to overcome legal and/or administrative obstacles for internationalization of university activities which might be found in each Institution.

BackgroundMobile health interventions provide significant strategies for improving access to health services, offering a potential solution to reduce the mental health treatment gap. Economic evaluation of this intervention is needed to help inform local mental health policy and program development. ObjectiveThis paper presents the protocol for an economic evaluation conducted alongside 2 randomized controlled trials (RCTs) to evaluate the cost-effectiveness of a psychological intervention delivered through a technological platform (CONEMO) to treat depressive symptoms in people with diabetes, hypertension, or both. MethodsThe economic evaluation uses a within-trial analysis to evaluate the incremental costs and health outcomes of CONEMO plus enhanced usual care in comparison with enhanced usual care from public health care system and societal perspectives. Participants are patients of the public health care services for hypertension, diabetes, or both conditions in São Paulo, Brazil (n=880) and Lima, Peru (n=432). Clinical effectiveness will be measured by reduction in depressive symptoms and gains in health-related quality of life. We will conduct cost-effectiveness and cost-utility analyses, providing estimates of the cost per at least 50% reduction in 9-item Patient Health Questionnaire scores, and cost per quality-adjusted life year gained. The measurement of clinical effectiveness and resource use will take place over baseline, 3-month follow-up, and 6-month follow-up in the intervention and control groups. We will use a mixed costing methodology (ie, a combination of top–down and bottom–up approaches) considering 4 cost categories: intervention (CONEMO related) costs, health care costs, patient and family costs, and productivity costs. We will collect unit costs from the RCTs and national administrative databases. The multinational economic evaluations will be fully split analyses with a multicountry costing approach. We will calculate incremental cost-effectiveness ratios and present 95% CIs from nonparametric bootstrapping (1000 replicates). We will perform deterministic and probabilistic sensitivity analyses. Finally, we will present cost-effectiveness acceptability curves to compare a range of possible cost-effectiveness thresholds. ResultsThe economic evaluation project had its project charter in June 2018 and is expected to be completed in September 2021. The final results will be available in the second half of 2021. ConclusionsWe expect to assess whether CONEMO plus enhanced usual care is a cost-effective strategy to improve depressive symptoms in this population compared with enhanced usual care. This study will contribute to the evidence base for health managers and policy makers in allocating additional resources for mental health initiatives. It also will provide a basis for further research on how this emerging technology and enhanced usual care can improve mental health and well-being in low- and middle-income countries. Background: Altered sensorimotor gating has been demonstrated by Prepulse Inhibition (PPI) tests in patients with psychosis. Recent advances in signal processing methods allow assessment of neural PPI through electroencephalogram (EEG) recording during acoustic startle response measures (classic muscular PPI). Simultaneous measurements of muscular (eye-blink) and neural gating phenomena during PPI test may help to better understand sensorial processing dysfunctions in psychosis. In this study, we aimed to assess simultaneously muscular and neural PPI in early bipolar disorder and schizophrenia patients. Method: Participants were recruited from a population-based case-control study of first episode psychosis. PPI was measured using electromyography (EMG) and EEG in pulse alone and prepulse + pulse with intervals of 30, 60, and 120 ms in early bipolar disorder (n = 18) and schizophrenia (n = 11) patients. As control group, 15 socio-economically matched healthy subjects were recruited. All subjects were evaluated with Rating Scale, Hamilton Rating Scale for Depression, and Young Mania Rating Scale questionnaires at recruitment and just before PPI test. Wilcoxon ranked sum tests were used to compare PPI test results between groups. Results: In comparison to healthy participants, neural PPI was significantly reduced in PPI 30 and PPI60 among bipolar and schizophrenia patients, while muscular PPI was reduced in PPI60 and PPI120 intervals only among patients with schizophrenia. Conclusion: The combination of muscular and neural PPI evaluations suggested distinct impairment patterns among schizophrenia and bipolar disorder patients. Simultaneous recording may contribute with novel information in sensory gating investigations.

Life stressors during critical periods are reported to trigger an immune dysfunction characterised by abnormal production of inflammatory cytokines. Despite the relationship between early stressors and schizophrenia is described, the evidence on inflammatory biomarkers remains limited. We aimed to investigate whether an imbalance between pro- and anti-inflammatory cytokines in the brain is reflected in the peripheral blood of rats submitted to post-weaning social isolation (pwSI), a model with validity to study schizophrenia. We evaluated pro- and anti-inflammatory cytokines (IL-6, TNF-α, and IL-10) simultaneously at blood, prefrontal cortex and hippocampal tissues (Milliplex MAP), including the respective cytokines gene expression (mRNA) (qRT-PCR TaqMan mastermix). We also performed a correlation matrix to explore significant correlations among cytokines (protein and mRNA) in blood and brain, as well as cytokines and total number of square crossings in the open field for isolated-reared animals. Male Wistar rats (n = 10/group) were kept isolated (n = 1/cage) or grouped (n = 3–4/cage) since weaning for 10 weeks. After this period, rats were assessed for locomotion and sacrificed for blood and brain cytokines measurements. Prolonged pwSI decreased IL-10 protein and mRNA in the blood, and IL-10 protein in the hippocampus, along with decreased IL-6 and its mRNA expression in the prefrontal cortex. Our results also showed that cytokines tend to correlate to one-another among the compartments investigated, although blood and brain correlations are far from perfect. IL-10 hippocampal levels were negatively correlated with hyperlocomotion in the open field. Despite the unexpected decrease in IL-6 and unchanged TNF-α levels contrast to the expected pro-inflammatory phenotype, this may suggest that reduced anti-inflammatory signalling may be critical for eliciting abnormal behaviour in adulthood. Altogether, these results suggest that prolonged early-life adverse events reduce the ability to build proper anti-inflammatory cytokine that is translated from blood-to-brain.

RESUMO: Objetivo: Descrever a prevalência do autorrelato de diagnóstico médico prévio de depressão na população adulta brasileira (18 anos ou mais) segundo fatores sociodemográficos. Métodos: Foram utilizados dados da Pesquisa Nacional de Saúde 2013, inquérito de base populacional. Foi investigado o autorrelato do diagnóstico médico prévio de depressão, recebido em algum momento da vida. Foram calculadas as prevalências e seus respectivos intervalos de confiança de 95% (IC95%), estratificados por sexo, faixa etária, escolaridade, raça/cor da pele, para Brasil, local de residência, grandes regiões, unidades federativas e capitais. Resultados: A prevalência de autorrelato de diagnóstico de depressão em adultos no Brasil foi de 7,6% (IC95% 7,2 - 8,1), sendo maior em mulheres (10,9%; IC95% 10,3 - 11,6) e em pessoas entre 60 e 64 anos (11,1%; IC95% 9,1 - 13,1). Ainda, as maiores prevalências foram em indivíduos sem instrução ou com fundamental incompleto, 8,6% (IC95% 7,9 - 9,3), para aqueles com ensino superior completo, 8,7% (IC95% 7,5 - 9,9); e para aqueles que se autodeclararam brancos (9,0%; IC95% 8,3 - 9,6). Por local de residência, esse autorrelato foi maior em indivíduos residentes na região urbana (8,0%; IC95% 7,5 - 8,4) e na região Sul (12,6%; IC95% 11,2 - 13,9). Conclusão: A análise deste estudo revela a importância de conhecer o acesso ao diagnóstico de depressão no Brasil. É necessário aprimorar o acesso aos serviços de saúde com qualidade em todo o território nacional para abranger as populações mais desfavorecidas. Reduzir as disparidades no acesso aos serviços de saúde é fundamental para garantir que direitos sociais sejam equânimes e universais.

In Brazil and in most low- and middle-income countries (LMICs), information about how prepared the health care system is for the rapid aging of the population is scarce. We investigated the prevalence of disability and areas of life affected by disability among elders of the public primary health care in São Paulo and Manaus, Brazil. We investigated whether people with disability visited a primary care professional more frequently, the individual characteristics associated with disability, and differences by city. We randomly selected participants aged ≥60 years (n = 1375). The main outcome was disability, evaluated with the 12-item World Health Organization Disability Assessment Schedule (WHODAS 2.0). Exposure variables were consultation with a family physician, sociodemographic characteristics, health status, social support, and lifestyle. The prevalence of global disability was higher in Manaus (66.2% vs. 56.4% in São Paulo). In both cities, participation and mobility were the areas of life most affected by disability. The number of consultations with a family physician was not associated with disability. The high prevalence of disability and associated risk factors indicates that public primary health care is not meeting the needs of elders in both cities. It is warning because most elders in LMICs live in more underserved communities compared to Brazil.

Prepulse inhibition (PPI) test has been widely used to evaluate sensorimotor gating. In humans, deficits in this mechanism are measured through the orbicularis muscle response using electromyography (EMG). Although this mechanism can be modulated by several brain structures and is impaired in some pathologies as schizophrenia and bipolar disorder, neural PPI evaluation is rarely performed in humans. Since eye blinks are a consequence of PPI stimulation, they strongly contaminate the electroencephalogram (EEG) signal. This paper describes a method to reduce muscular artifacts and enable neural PPI assessment through EEG in parallel to muscular PPI evaluation using EMG. Both types of signal were simultaneously recorded in 22 healthy subjects. PPI was evaluated by the acoustical startle response with EMG and by the P2-N1 event-related potential (ERP) using EEG in Fz, Cz, and Pz electrodes. In order to remove EEG artifacts, Independent Component Analysis (ICA) was performed using two methods. Firstly, visual inspection discarded components containing artifact characteristics as ocular and tonic muscle artifacts. The second method used visual inspection as gold standard to validate parameters in an automated component selection using the SASICA algorithm. As an outcome, EEG artifacts were effectively removed and equivalent neural PPI evaluation performance was obtained using both methods, with subjects exhibiting consistent neural as well as muscular PPI. This novel method improves PPI test, enabling neural gating mechanisms assessment within the latency of 100–200 ms, which is not evaluated by other sensory gating tests as P50 and mismatch negativity.

ABSTRACT OBJECTIVE To present the development and validation of the WebAd-Q Questionnaire, a self-report instrument to monitor adherence to antiretroviral therapy in HIV/AIDS centers in Brazil. METHODS The WebAd-Q is an electronic questionnaire that has three questions about the use of antiretrovirals in the last week. It was constructed from interviews and focus groups with 38 patients. Its validity was tested in a study with a sample of 90 adult patients on antiretroviral therapy for at least three months. We used electronic monitoring bottles, pill counting, and self-report interview to compare adherence. The WebAd-Q was answered on the sixtieth day, twice, with at least one hour of interval. The viral load of the patients was obtained from the service records. We have analyzed the agreement between the answers to the WebAd-Q, the associations, and the correlations with viral load and performance compared to other measures of adherence. RESULTS Among the invited patients, 74 (82.2%) answered the WebAd-Q. No difficulties were reported to answer the questionnaire. The average answer time was 5 min 47 sec. The set of three questions of the WebAd-Q obtained agreement of 89.8%, with Kappa of 0.77 (95%CI 0.61–0.94). The non-adherence answers of the WebAd-Q were associated with detectable viral load. We obtained moderate viral load correlations with the non-adherence scale according to the WebAd-Q. For the three questions of the WebAd-Q, patients with non-adherence answers were also reported as less adherent according to the other measures of adherence. CONCLUSIONS The WebAd-Q answered all the issues considered relevant in the validation of questionnaires, was well understood by patients, was associated with viral load, and obtained good agreement and good performance compared to the other measures. The feasibility analysis of its implementation still depends on a national study on its applicability.

To examine whether religious and spiritual interventions (RSIs) can promote physical health and quality of life in individuals.The following databases were used to conduct a systematic review: PubMed, Scopus, Web of Science, EMBASE, PsycINFO, Cochrane, and Scielo. Randomized controlled trials that evaluated RSIs regarding physical health outcomes and/or quality of life in English, Spanish or Portuguese were included. RSI protocols performed at a distance (i.e. intercessory prayer) or for psychiatric disorders were excluded. This study consisted of two phases: (a) reading titles and abstracts, and (b) assessing the full articles and their methodological quality using the Cochrane Back Review Group scale.In total, 7,070 articles were identified in the search, but 6884 were excluded in phase 1 because they were off topic or repeated in databases. Among the 186 articles included in phase 2, 140 were excluded because they did not fit the inclusion criteria and 16 did not have adequate randomization process. Thus, a final selection of 30 articles remained. The participants of the selected studies were classified in three groups: chronic patients (e.g., cancer, obesity, pain), healthy individuals and healthcare professionals. The outcomes assessed included quality of life, physical activity, pain, cardiac outcomes, promotion of health behaviors, clinical practice of healthcare professionals and satisfaction with protocols. The divergence concerning scales and protocols proposed did not allow a meta-analysis. RSIs as a psychotherapy approach were performed in 40% of the studies, and the control group was more likely to use an educational intervention (56.7%). The results revealed small effect sizes favoring RSIs in quality of life and pain outcomes and very small effects sizes in physical activity, promotion of health behaviors and clinical practice of health professionals compared with other complementary strategies. Other outcomes, such as cardiac measures and satisfaction with the protocols, revealed no evidence for RSIs. Regarding the quality of the selected articles according to the Cochrane Back Review Group Scale, the average score was 6.83 (SD = 9.08) on a scale of 11, demonstrating robustness in the studies.Clinical trials on RSIs demonstrated that they had small benefits compared with other complementary health therapies by reducing pain and weight, improving quality of life and promoting health behaviors. The lack of clinical trials that included biological outcomes and the diversity of approaches indicate a need for more studies to understand the possible mechanisms of action of RSIs and their roles in health care.

O contexto social pode ter papel importante na etiologia dos transtornos mentais e na sua prevalência. O objetivo do presente estudo foi investigar fatores de risco que contribuem para a prevalência de transtornos mentais comuns (TMC), considerando distintos níveis contextuais: indivíduo, domicílio e setor censitário. Para isso, utilizou-se uma amostra de base populacional de 2.366 indivíduos participantes do São Paulo Ageing & Health Study. A presença de TMC foi identificada pelo instrumento SRQ-20. Sexo, idade, escolaridade e ocupação foram características individuais associadas à prevalência de TMC. Modelos de regressão logística multinível mostraram que parte da variância na prevalência de TMC foi associada ao nível do domicílio, com associações entre aglomeração, renda familiar e prevalência de TMC, mesmo após controle para características individuais. Esses resultados sugerem que características do ambiente onde as pessoas vivem contribuem para sua saúde mental.

INTRODUCTION: The prevalence of violence in intimate relationships, suffered or perpetrated, was estimated among undergraduate students, be it physical, psychological and sexual, describing the overlap between them. This is an original study and there is scarce literature on this matter in Brazil. Men and women were investigated and some issues on gender violence were discussed. METHODS: The study is part of the multicenter survey "International Study of Violence in Dating - IDVS" conducted in 2002 - 2003, using its standardized instrument. A total of 362 students, from two universities in the state of São Paulo, one public and one private, answered the self-reported questionnaire, being 37% male and 63% female, mean age of 20 years old. Suffered and perpetrated intimate partner violence during life was examined. RESULTS: Among all interviewees, 75.9% suffered and 76.4% perpetrated some kind of violence throughout life. Psychological violence was the most prevalent type, followed by sexual violence, both suffered and perpetrated. The great superposition between suffered and perpetrated violence (83.9%) reflects the reciprocity of aggression, with no observed difference between men and women. The results of this study are consistent with previous literature on dating violence, which shows high prevalence of suffered and perpetrated violence, besides reciprocity for both men and women. CONCLUSIONS: Intervention actions at this stage of intimate relationships may potentially impact on subsequent marital partnership situations.

Objective: To perform a systematic review of cross-sectional studies on the prevalence of insufficient physical activity (IPA) based on a WHO-defined cutoff point (< 60 min/d of moderate and vigorous physical activity). Methods: The search was carried out using online databases (BioMed Central, CINAHL, EMBASE, ERIC, PsycInfo, PubMed MEDLINE, SCOPUS, SPORT-Discus), and included articles published from the beginning of the databases until February18th, 2012, as well as references cited by the retrieved articles and information provided by the authors. Only original articles using questionnaires in the diagnosis were considered. Results: Of 2,384 papers initially retrieved, fifteen studies met the inclusion criteria, of which seven were conducted in Brazil. The prevalence of IPA varied from 18.7% to 90.6%, with a median of 79.7%. In all surveys, the prevalence was higher among girls than boys and the developing countries have higher prevalence. Conclusions: We concluded that the prevalence of IPA is high among adolescents and that the definition adopted in this study is rarely used in the literature. These results suggesting that is necessary the development of interventions for increasing physical activity levels among adolescents.

OBJECTIVE: To estimate the association between antenatal and postnatal depression and to examine the role of socioeconomic conditions in the risk of postnatal depression. METHODS: A prospective cohort study, conducted between May 2005 and January 2006, with 831 pregnant women recruited from primary care clinics in the public sector in the city of São Paulo, Brazil. The presence of antenatal and postnatal depression was measured with the Self Report Questionnaire (SRQ-20). Sociodemographic and socioeconomic characteristics and obstetric information were obtained through a questionnaire. Crude and adjusted risk ratios (RR), with 95% CI, were calculated using a Poisson regression. RESULTS: The prevalence of postnatal depressive symptoms was 31.2% (95%CI: 27.8-34.8%). Among the 219 mothers who had depressive symptoms, nearly 50% had already shown depressive symptoms during pregnancy. Women who had antenatal depression were 2.4 times more likely to present with postnatal depression than were women who did not have such symptoms during pregnancy. In the multivariate analysis, higher scores for assets (RR: 0.76, 95% CI 0.61-0.96), higher education (RR: 0.75 95%CI 0.59-0.96), daily contact with neighbors (RR: 0.68, 95%CI 0.51-0.90) and antenatal depression (RR: 2.44, 95%CI 1.93-3.08) remained independently associated with postnatal depression. CONCLUSIONS: Antenatal and postnatal depression are highly prevalent in the primary care setting.OBJETIVO: Estimar a associação entre depressão pré-natal e pós-natal, e examinar o papel das condições sócioeconômicas sobre o risco de depressão pós-parto. MÉTODOS: Estudo de coorte prospectivo, realizado entre maio de 2005 e janeiro de 2006, com 831 gestantes recrutadas de clínicas de cuidados básicos, do setor público, na cidade de São Paulo, Brasil. Presença de depressão pré-natal e pós-natal foi medida com o Self Report Questionnaire (SRQ-20). Características sócio-demográficas e socioeconômicas, e informações obstétricas foram obtidas através de um questionário. Riscos relativos (RR), bruto e ajustado, com IC de 95%, foram calculados usando Regressão de Poisson. RESULTADOS: A prevalência de sintomas depressivos pós-natal foi de 31,2% (IC95% 27,8-34,8%). Entre as 219 mães que tinham sintomas depressivos, quase 50% já haviam mostrado sintomas depressivos durante a gravidez. Mulheres que tiveram depressão pré-natal tiveram risco 2,4 vezes maior de apresentar depressão pós-parto do que as mulheres que não tiveram tais sintomas durante a gravidez. Na análise multivariada, maior escore de bens (RR: 0,76; IC95% 0,1-0,96), maior escolaridade (RR: 0,75; IC95% 0,59-0,96), contacto diário com vizinhos (RR: 0.68; IC95% 0,51-0,90) e depressão pré-natal (RR:2,44; IC95% 1,93-3,08) permaneceram independentemente associadas com depressão pós-parto. CONCLUSÕES: Depressão pré-natal e pós-natal são altamente prevalentes na atenção primária, e profissionais de saúde devem ser treinados para realizar intervenções simples e eficazes, o mais precocemente possível.

FUNDAMENTO: A fibrilação atrial é um fator de risco controverso para demência. OBJETIVO: O objetivo deste estudo é avaliar a associação entre fibrilação atrial e demência em participantes do São Paulo Ageing & Health. MÉTODOS: O São Paulo Ageing & Health é um estudo transversal, de base populacional, de idosos residentes em um uma região de baixa renda da cidade de São Paulo, Brasil. Diagnóstico de demência foi realizado de acordo com o protocolo do grupo 10/66, com base em critérios do Manual de Diagnóstico e Estatística das Perturbações Mentais (DSM-IV). O diagnóstico de fibrilação atrial foi feito por eletrocardiograma de 12 derivações, avaliado por dois cardiologistas. Dados demográficos e de fatores de risco cardiovasculares também foram obtidos. RESULTADOS: A demência foi diagnosticada em 66 (4,3%) e fibrilação atrial em 36 (2,4%) de 1.524 participantes com um eletrocardiograma válido. A razão de chances bruta para demência em participantes com fibrilação atrial foi 2,8 (intervalo de confiança [IC] 95%: 1,0-8,1; p = 0,06) em comparação com indivíduos sem fibrilação atrial. Relação positiva foi encontrada em mulheres (RC 4,2; IC 95%: 1,2-15,1; p = 0,03). Após ajuste para idade, no entanto, essa associação tornou-se não significativa (RC 2,2, IC 95%: 0,6-8,9; p = 0,26). CONCLUSÃO: Não houve associação independente entre a fibrilação atrial e demência nessa amostra. A prevalência da fibrilação atrial pode ser baixa nesta população em virtude da mortalidade cardiovascular prematura.

ABSTRACT OBJECTIVE To describe the prevalence of common mental disorders in Brazilian adolescent students, according to geographical macro-regions, school type, sex, and age. METHODS We evaluated 74,589 adolescents who participated in the Cardiovascular Risk Study in Adolescents (ERICA), a cross-sectional, national, school-based study conducted in 2013-2014 in cities with more than 100,000 inhabitants. A self-administered questionnaire and an electronic data collector were employed. The presence of common mental disorders was assessed using the General Health Questionnaire (GHQ-12). We estimated prevalence and 95% confidence intervals of common mental disorders by sex, age, and school type, in Brazil and in the macro-regions, considering the sample design. RESULTS The prevalence of common mental disorders was of 30.0% (95%CI 29.2-30.8), being higher among girls (38.4%; 95%CI 37.1-39.7) when compared to boys (21.6%; 95%CI 20.5-22.8), and among adolescents who were from 15 to 17 years old (33.6%; 95%CI 32.2-35.0) compared to those aged between 12 and 14 years (26.7%; 95%CI 25.8-27.6). The prevalence of common mental disorders increased with age for both sexes, always higher in girls (ranging from 28.1% at 12 years to 44.1% at 17 years) than in boys (ranging from 18.5% at 12 years to 27.7% at 17 years). We did not observe any significant difference by macro-region or school type. Stratified analyses showed higher prevalence of common mental disorders among girls aged from 15 to 17 years of private schools in the North region (53.1; 95%CI 46.8-59.4). CONCLUSIONS The high prevalence of common mental disorders among adolescents and the fact that the symptoms are often vague mean these disorders are not so easily identified by school administrators or even by health services. The results of this study can help the proposition of more specific prevention and control measures, focused on highest risk subgroups.

BACKGROUND:The prevalence of depression has been widely studied in high-income countries and in large cities of low-income countries; however, little is known about the prevalence and treatment gap of depression in remote areas of the Amazonian region in Brazil. OBJECTIVES:The objectives of this study are to estimate the prevalence of depression in adults registered with the Family Health Strategy in two remote cities in the Brazilian Amazon and to investigate the proportion of individuals with depression that received mental health care. METHODS:This is a cross-sectional study of an adult population registered with primary care clinics in the cities of Coari and Tefé, State of Amazon, Brazil. Depression was defined as a score of ≥10 on the Patient Health Questionnaire-9. Depression care was evaluated by asking participants with depression if they received antidepressants and/or had been seen by a health professional at a community mental health center in the three months prior to the interview. Poisson regression was used to examine the unadjusted and adjusted associations between depression and exposure variables. RESULTS:The overall prevalence of depression was 19.1% (95% CI: 17.2-21.1), with 22.2% (95% CI: 19.3-25.0) among women and 16.0% (95% CI: 13.4-18.5) among men. The prevalence of depression in Coari and Tefé were 18.3% (CI 95% 15.7-21.0) and 19.9% (95% CI:17.2-22.7), respectively. Being a woman, lacking social support, increasing exposure to stressful life events and having a higher number medical comorbidities were consistently associated with depression. Lower educational attainment and income, tobacco use, and risky alcohol use were also associated with depression in the unadjusted analyses. Only 11.5% of those with depression were receiving antidepressants and/or visited the mental health care facility during the three months prior to the interview. CONCLUSION:Approximately one in five adults in our sample had depression. A high proportion of participants presented indicators of social disadvantage and other risk factors previously associated with depression worldwide. There was a large treatment gap for depression in the Amazonian region, which demonstrates the need for innovative models of depression care in primary care settings in Brazil.

INTRODUCTION: Reductions in the prevalence of hepatitis B virus (HBV) infection and carriage, decreases in liver cancer incidence, and changes in patterns of liver dysfunctions are described after hepatitis B vaccination. METHODS: We conducted a population-based seroprevalence study aimed at estimating the HBV prevalence and risk of infection in the rural area of Lábrea following nineteen years of HBV vaccination. RESULTS: Half of the subjects showed total anti-HBc of 52.1% (95% CI 49.6-54.7). The HBsAg prevalence was 6.2% (95% CI 5.1-7.6). Multivariate analysis showed an inverse association between HBV infection and vaccination (OR 0.62; 95% CI 0.44-0.87). HBsAg remained independently associated with past hepatitis (OR 2.44; 95% CI 1.52-3.89) and inversely to vaccination (OR 0.43; 95% CI 0.27-0.69). The prevalence of HBeAg among HBsAg-positive individuals was 20.4% (95% CI 12.8-30.1), with the positive subjects having a median age of 11 years (1-46) p=0.0003. CONCLUSIONS: We demonstrate that HBV infection is still an important public health issue and that HBV vaccination could have had better impact on HBV epidemiology. If we extrapolate these findings to other rural areas in the Brazilian Amazon, we can predict that the sources of chronic infected patients remain a challenge. Future studies are needed regarding clinical aspects, molecular epidemiology, surveillance of acute cases, and risk groups.

O crescimento da população idosa brasileira fez aumentar a prevalência de doenças crônicas e o número de pessoas sofrendo de diversos sintomas ao final da vida. Este estudo objetivou entrevistar cuidadores de idosos falecidos, investigando a presença, intensidade e duração de sintomas no último ano, e se os mesmos foram tratados. Arrolaram-se idosos por inquérito domiciliar de base populacional em setores de baixa renda do Butantã, São Paulo. Eram elegíveis os idosos falecidos durante o período de dois anos de seguimento. Oitenta e um cuidadores foram entrevistados entre três e 16 meses pós-óbito e responderam um questionário sobre onze sintomas. Os sintomas mais referidos foram dor (78%), fadiga (68%), dispnéia (60%), depressão e anorexia (58% cada). Dor, dispnéia e fadiga foram os mais intensos. Dor, fadiga e depressão duraram 6 meses ou mais. Ficaram sem tratamento 79% dos idosos com depressão, 77% daqueles com incontinência urinária e 67% daqueles com ansiedade. É necessário implementar conceitos e ações de cuidados paliativos para dar aos idosos mais dignidade e qualidade ao final da vida.The Brazilian population of elders is growing, resulting in high prevalences of chronic diseases and people facing distressing symptoms in their last years. This study aimed at interviewing family caregivers of deceased elders to investigate the presence, severity and duration of common symptoms in the last year, as well as whether the symptoms were managed or not. Elders were enrolled in a population-based study in deprived areas of Butantã, São Paulo. After a two-year follow-up, any death was eligible for the study. Eighty-one caregivers were interviewed between three and sixteen months after elder's death and answered a questionnaire on eleven symptoms. The most commonly reported symptoms were pain (78%), fatigue (68%), dyspnea (60%), depression, and anorexia (58% each). Pain, dyspnea and fatigue were the most severe. Pain, fatigue and depression lasted 6 months or more. No treatment was received for depression (79%), urinary incontinence (77%) and anxiety (67%). The implementation of palliative care concepts and actions is mandatory to provide Brazilian elders with dignity and better quality at the end of life.

This study aimed to estimate prevalence of thyroid disorders in the São Paulo Ageing & Health Study, an epidemiological study addressing several health-adverse outcomes among elderly people living in a poor area of São Paulo, Brazil. All participants answered a questionnaire and had a blood sample collected to assess levels of tireotropic hormone and free-thyroxine. Among 1,373 people (60.8% women), prevalence rates (95% confidence interval) for thyroid dysfunction (%) were: overt hyperthyroidism, 0.7% (0.2-1.1)[women: 0.8% (0.2-1.5); men: 0.4% (0.01-0.9)]; overt hypothyroidism, 5.7% (4.5-6.9) [women: 5.9% (4.3-7.5); men: 5.4% (3.5-7.3)]; subclinical hyperthyroidism, 2.4% (1.6-3.2) [women: 2.8% (1.6-3.9); men: 1.9% (0.7-3.0)]; and subclinical hypothyroidism, 6.5% (5.2-7.8) [women: 6.7% (5.0-8.4); men: 6.1% (4.1-8.2)]. There was no difference in prevalence rates according to gender, but almost 40% of women were diagnosed and under treatment compared to 9% of men. The burden of thyroid disorders in this sample is high and most participants were not aware of them.O objetivo da pesquisa foi avaliar a prevalência de doenças da tireóide no São Paulo Ageing & Health Study, estudo epidemiológico focado em desfechos de saúde adversos em amostra de idosos moradores de São Paulo, Brasil. Todos os participantes responderam a questionário e colheram sangue para dosagem de hormônio tireotrópico e tiroxina-livre. Entre os 1.373 participantes (60,8% mulheres), a prevalência (intervalo de 95% de confiança) de hipertireoidismo clínico foi de 0,7% (0.2-1,1) [mulheres: 0,8% (0,2-1,5); homens: 0,4% (0,01-0,9)]; hipotireoidismo clínico, 5,7% (4,5-6,9) [mulheres: 5,9% (4,3-7,5); homens: 5,4% (3,5-7,3)]; hipertireoidismo subclínico, 2,4% (1,6-3,2) [mulheres: 2,8% (1,6-3,9); homens: 1,9% (0,7-3,0)]; e hipotireoidismo subclínico, 6,5% (5,2-7,8) [mulheres: 6,7% (5,0-8,4); homens: 6,1% (4,1-8,2)]. Não houve diferença na prevalência de doenças da tireóide por sexo. Quarenta por cento das mulheres tinham diagnóstico e estavam tratando, comparadas a 9% dos homens. A prevalência de disfunção tireoidiana foi elevada e a maioria dos participantes desconhecia o diagnóstico.

OBJETIVO: Estimar os fatores socioeconômicos e sociodemográficos associados ao consumo diário de cinco porções de frutas e hortaliças por idosos residentes em áreas de baixa renda, identificando as principais frutas e hortaliças que compõem a dieta desta população. MÉTODOS: Estudo transversal de base populacional com 2.066 idosos (>60 anos) de baixa renda residentes na cidade de São Paulo, SP, em 2003-2005. Para a avaliação do consumo de frutas e hortaliças foi aplicado questionário de freqüência alimentar. As respostas foram transformadas em consumo diário e comparadas às recomendações da Organização Mundial da Saúde (consumo de cinco ou mais porções diárias). A relação entre consumo recomendado de frutas e hortaliças e variáveis socioeconômicas foi avaliada mediante modelos de regressão logística. RESULTADOS: Dos participantes, 60,5% eram mulheres e 39,5% homens. Cerca de um terço dos idosos (n=723; 35,0%) não consumia diariamente nenhum tipo de fruta ou hortaliça e 19,8% relataram consumo diário de cinco ou mais porções de frutas e hortaliças. Este consumo esteve positivamente associado à renda e à escolaridade. CONCLUSÕES: O consumo de frutas e hortaliças de idosos de baixa renda do município de São Paulo mostrou-se insuficiente em relação às recomendações da Organização Mundial da Saúde e está associado a condições socioeconômicas desfavoráveis.OBJETIVO: Estimar los factores socioeconómicos y sociodemográficos asociados al consumo diário de cinco porciones de frutas y hortalizas por ancianos residentes en áreas de baja renta, identificando las principales frutas y hortalizas que componen la dieta de esta población. MÉTODOS: Estudio transversal de base poblacional con 2.066 ancianos (>60 años) de baja renta residentes en la ciudad de São Paulo, Sureste de Brasil, en 2003-2005. Para la evaluación del consumo de frutas y hortalizas fue aplicado cuestionario de frecuencia alimentaria. Las respuestas fueron transformadas en consumo diário y comparadas con las recomendaciones de la Organización Mundial de la Salud (consumo de cinco o más porciones diárias). La relación entre consumo recomendado de frutas y hortalizas y variables socioeconómicas fue evaluada mediante modelos de regresión logística. RESULTADOS: De los participantes, 60,5% eran mujeres y 39,5% hombres. Cerca de un tercio de los ancianos (n=723; 35,0%) no consumia diariamente ningún tipo de fruta u hortaliza y 19,8% relataron consumo diário de cinco o más porciones de frutas y hortalizas. Este consumo estuvo positivamente asociado con la renta y la escolaridad. CONCLUSIONES: El consumo de frutas y hortalizas de ancianos de baja renta del município de São Paulo se mostró insuficiente con relación a las recomendaciones de la Organización Mundial de la Salud y está asociado a condiciones socioeconómicas desfavorables.OBJECTIVE: To estimate the socioeconomic and sociodemographic factors associated with the daily intake of five servings of fruit and vegetables by elderly individuals living in low income areas, identifying the main fruits and vegetables which compose the diet of this population. METHODS: This is a cross-sectional population-based study with 2,066 low income elderly individuals (>60 years) living in the city of São Paulo, Southeastern Brazil, in 2003-2005. To assess the fruit and vegetable intake a Food Frequency Questionnaire was administered. The answers were transformed into daily intake and compared with the recommendations of the World Health Organization (five or more servings per day). The relationship between recommended fruit and vegetable intake and socioeconomic variables was analyzed using logistic regression models. RESULTS: Of the participants, 60.5% were women and 39.5% were men. Approximately one third of the elders (n=723; 35.0%) did not consume any kind of fruit or vegetable on a daily basis and 19.8% reported a daily intake of five or more servings of fruits and vegetables. This intake was positively associated with income and years of schooling. CONCLUSIONS: The fruit and vegetable intake of low income elderly individuals in the city of São Paulo was insufficient according to the recommendations of the World Health Organization and is associated with unfavorable socioeconomic conditions.

OBJECTIVE: We set out to assess the feasibility, reliability and convergent validity of the Camberwell Assessment of Needs for the Elderly Scale in older residents of a Brazilian urban elderly population of low socioeconomic status. METHOD: We identified 32 older users of community health services from a population-based catchment area survey. We administered the Brazilian version of the Camberwell Assessment of Needs for the Elderly Scale to the older person and to an informal caregiver, and tape-recorded the assessments. Interviewers made a rating. Tape-recordings were independently co-rated. RESULTS: Items contributing to older person and caregiver reports of needs and unmet needs had a high internal consistency. Inter-rater reliability was excellent for all needs, and fair to good for unmet needs. Older person and caregiver's reports, and interviewer ratings were highly mutually consistent. Convergent validity was supported by associations, as hypothesized, between needs and disability, and needs and dementia. CONCLUSIONS: The Brazilian version of the Camberwell Assessment of Needs for the Elderly Scale is a feasible, reliable and, to the extent assessed, valid assessment of unmet needs in a disadvantaged low and middle income countries setting. Its practical utility as a clinical tool remains to be assessed.OBJETIVO: O objetivo deste estudo foi o de avaliar a aplicabilidade, a confiabilidade e a validação convergente da Escala Camberwell de Avaliação de Necessidades em Idosos em uma população de baixa renda residente na cidade de São Paulo. MÉTODO: O estudo incluiu 32 participantes com 65 anos ou mais, usuários de serviços de saúde local que fizeram parte do estudo de levantamento de base populacional. A Escala Camberwell de Avaliação de Necessidades em Idosos foi administrada ao participante e ao seu cuidador, todas as entrevistas foram gravadas. Assistentes de pesquisa pontuaram as entrevistas. As gravações foram pontuadas por um pesquisador independente. RESULTADOS: Itens que contribuíram para a pontuação positiva de necessidades atendidas ou não atendidas pelos participantes e seus cuidadores obtiveram alto coeficientes de consistência interna. Confiabilidade entre examinadores foi excelente para todas as necessidades, e para necessidades não atendidas oscilou entre razoável e boa. As respostas de participantes e cuidadores, e as pontuações dos entrevistadores foram mutuamente consistentes. Validade convergente foi mantida pelas associações entre necessidades e incapacidade, e necessidades e demência. CONCLUSÕES: O estudo de confiabilidade da versão brasileira da Escala Camberwell de Avaliação de Necessidades em Idosos mostrou que a mesma é um instrumento de pesquisa prático, confiável e válido para avaliar necessidades em países em desenvolvimento menos favorecidos.

OBJECTIVE: To investigate the recognition of depressive symptoms of major depressive disorder (MDD) by general practitioners. INTRODUCTION: MDD is underdiagnosed in medical settings, possibly because of difficulties in the recognition of specific depressive symptoms. METHODS: A cross-sectional study of 316 outpatients at their first visit to a teaching general hospital. We evaluated the performance of 19 general practitioners using Primary Care Evaluation of Mental Disorders (PRIME-MD) to detect depressive symptoms and compared them to 11 psychiatrists using Structured Clinical Interview Axis I Disorders, Patient Version (SCID I/P). We measured likelihood ratios, sensitivity, specificity, and false positive and false negative frequencies. RESULTS: The lowest positive likelihood ratios were for psychomotor agitation/retardation (1.6) and fatigue (1.7), mostly because of a high rate of false positive results. The highest positive likelihood ratio was found for thoughts of suicide (8.5). The lowest sensitivity, 61.8%, was found for impaired concentration. The sensitivity for worthlessness or guilt in patients with medical illness was 67.2% (95% CI, 57.4-76.9%), which is significantly lower than that found in patients without medical illness, 91.3% (95% CI, 83.2-99.4%). DISCUSSION: Less adequately identified depressive symptoms were both psychological and somatic in nature. The presence of a medical illness may decrease the sensitivity of recognizing specific depressive symptoms. CONCLUSIONS: Programs for training physicians in the use of diagnostic tools should consider their performance in recognizing specific depressive symptoms. Such procedures could allow for the development of specific training to aid in the detection of the most misrecognized depressive symptoms.

OBJETIVO: Estimar a prevalência da síndrome do esgotamento profissional e de transtornos mentais comuns em agentes comunitários de saúde, identificando fatores associados. MÉTODOS: Estudo transversal realizado com 141 agentes comunitários atuantes há pelo menos seis meses em unidades básicas de saúde do município de São Paulo (SP), em 2006. Os participantes responderam a três questionários: um sobre características sociodemográficas, socioeconômicas, da saúde e do trabalho; o Self Reporting Questionnaire; e o Maslach Burnout Inventory, que permite medir três dimensões da síndrome do esgotamento profissional: exaustão emocional, despersonalização e decepção. A regressão logística multivariada foi usada para verificar associações entre variáveis. RESULTADOS: No total, 24,1% dos entrevistados apresentaram síndrome do esgotamento profissional. Níveis moderados ou altos de esgotamento profissional foram observados em 70,9% dos participantes para exaustão emocional, em 34% para despersonalização e em 47,5% para decepção. A prevalência de transtornos mentais comuns foi 43,3%. Foram observadas associações positivas entre as dimensões de esgotamento profissional. Presença de transtorno mental comum associou-se independentemente com maiores níveis de exaustão emocional e decepção. CONCLUSÕES: A alta freqüência de níveis intensos de esgotamento profissional e a elevada ocorrência de transtornos mentais comuns encontradas entre os agentes comunitários de saúde suscitam a necessidade de estratégias de intervenção no cotidiano desses indivíduos e de novas investigações sobre a dimensão e determinantes do esgotamento profissional.OBJETIVO: Estimar la prevalencia del síndrome de agotamiento profesional y de trastornos mentales comunes en agentes comunitarios de salud, identificando factores asociados. MÉTODOS: Se hizo estudio transversal con 141 agentes comunitarios activos por al menos seis meses en unidades básicas de salud del municipio de Sao Paulo (Sureste de Brasil), en 2006. Los participantes respondieron a tres encuestas: una sobre características sociodemográficas, socioeconómicas, de la salud e del trabajo; el Self Reporting Questionnaire; y el Maslach Burnout Inventory, que permite medir tres dimensiones del síndrome del agotamiento profesional: fatiga emocional, despersonalización y decepción. La regresión logística multivariada fue usada para verificar asociaciones entre variables. RESULTADOS: De los encuestados el 24,1% presentaron síndrome de agotamiento profesional. Niveles moderados o altos de agotamiento profesional fueron observados en 70,9% de los participantes con fatiga emocional, en 34% con despersonalización y en 47,5% con decepción. La prevalencia de trastornos mentales comunes fue de 43,3%. Se observaron asociaciones positivas entre las dimensiones de agotamiento profesional. La presencia de trastorno mental común se asoció independientemente con mayores niveles de fatiga emocional y decepción. CONCLUSIONES: La alta frecuencia de niveles intensos de agotamiento profesional y la elevada ocurrencia de trastornos mentales comunes encontrados entre los agentes comunitarios de salud promueven la necesidad de estrategias de intervención en el cotidiano de estos individuos y de nuevas investigaciones sobre la dimensión y determinantes del agotamiento profesional.OBJECTIVE: To estimate the prevalence of the burnout syndrome and of common mental disorders among community-based health agents, also identifying associated factors. METHODS: A cross-sectional survey was carried out in 2006 with 141 community-based health agents who had been working for at least six months in primary care units in the city of São Paulo (Southeastern Brazil). The participants answered three questionnaires: one about sociodemographic, health- and work-related characteristics, the Self Reporting Questionnaire, and the Maslach Burnout Inventory, which allows the assessment of three dimensions of the burnout syndrome: emotional exhaustion, depersonalization and reduced personal accomplishment. Multivariate logistic regression was used to examine associations between variables. RESULTS: Overall, 24.1% of the interviewees presented burnout syndrome. Moderate or high levels of emotional exhaustion, depersonalization and reduced personal accomplishment were observed in 70.9%, 34.0% and 47.5% of the participants, respectively. The prevalence of common mental disorders was 43.3%. Positive correlations between the three dimensions of burnout, were observed. Presence of common mental disorder was independently associated with higher levels of emotional exhaustion and reduced personal accomplishment. CONCLUSIONS: The high frequency of intense levels of burnout and the high prevalence of common mental disorders among community-based health agents point to the need of intervention strategies in these individuals' daily lives and of further studies to better understand the actual picture and the determinants of burnout.

OBJECTIVES: to investigate patterns of sexual activity among teenagers. METHODS: a cross-sectional study was conducted between July 1998 and September 2000, among 117 sexually active female adolescents from a private clinic, in the city of the Osasco, State of São Paulo, Brazil. They were divided into two groups: one pregnant group (PG) comprised 62 adolescents that were either pregnant (46) or had previously been pregnant (16); another group of 55 female adolescents that had never been pregnant (NPG). During consultations with these subjects, a physician conducted a semi-structured interview. Knowledge, attitudes and practices relating to sexual activity were evaluated. The comparison between the two groups was carried out using Student's t test, the chi-square test or Fisher's exact test. RESULTS: the two groups showed considerable similarities in terms of sexual behaviour, having engaged in the first sexual intercourse at the age of 15 and having had an average number of sexual partners of 1.5. Nevertheless, adolescents in the PG group had initiated sexual life earlier and tended to use less contraceptive methods during the first intercourse. Despite widespread knowledge of contraception, a large number of the adolescents did not use any contraceptive method during first sexual intercourse. In their current sexual life, an average of 81% of the participants referred to attaining orgasm. CONCLUSIONS: knowledge about contraceptive techniques is not enough to avoid unplanned pregnancies, suggesting the importance of investigating other psychosocial aspects of motherhood and maternal identity among teenagers.OBJETIVOS: investigar padrões de atividade sexual em adolescentes. MÉTODOS: estudo transversal realizado entre julho de 1998 e setembro de 2000, com 117 mulheres adolescentes sexualmente ativas oriundas de clínica privada, em Osasco, São Paulo, Brasil. Foram constituídos dois grupos: um grupo de grávidas (PG) com 62 adolescentes que estavam grávidas (46) ou tiveram gravidez prévia (16); outro grupo não grávidas (NPG), com 55 adolescentes que nunca engravidaram. Uma entrevista semi-estruturada foi conduzida pelo médico durante consulta clínica. Foram avaliados o conhecimento, atitudes e práticas acerca da atividade sexual. A comparação entre os grupos foi feita por meio do teste t de Student, qui-quadrado e teste exato de Fisher. RESULTADOS os dois grupos eram similares quanto aos aspectos sexuais, sendo a coitarca na idade de 15 e a média do número de parceiros, 1,5. Entretanto, adolescentes no PG iniciaram vida sexual mais cedo e usaram menos métodos contraceptivos na primeira relação sexual. Apesar do grande conhecimento sobre contracepção, parte considerável das adolescentes não usou nenhum método durante a coitarca. Na vida sexual atual, cerca de 81% das participantes refere atingir orgasmo. CONCLUSÕES: o conhecimento sobre técnicas contraceptivas não é suficiente para evitar gestações não planejadas, sugerindo a importância de investigar outros aspectos psicosexuais da maternidade e identidade materna entre adolescentes.

OBJETIVO: Avaliar a influência das condições socioeconômicas na associação entre transtornos mentais comuns, uso de serviços de saúde e de psicofármacos. MÉTODOS: Estudo transversal populacional conduzido na cidade de Botucatu, SP, com amostragem probabilística, estratificada e por conglomerados. Foram realizadas entrevistas domiciliares com 1.023 sujeitos de 15 anos ou mais de idade, entre 2001 e 2002. Transtorno mental comum foi avaliado utilizando o Self Reporting Questionnaire (SRQ-20). O uso de serviços foi investigado com relação à quinzena anterior à entrevista e uso de psicotrópicos, nos três dias anteriores. Utilizou-se regressão logística para análise multivariável, considerando o efeito do desenho. RESULTADOS: No total da amostra, 13,4% (IC 95%: 10,7;16,0) procuraram serviços de saúde na quinzena anterior à entrevista. A procura de serviços de saúde se associou ao sexo feminino (OR=2,0) e à presença de transtorno mental comum (OR=2,2). Na amostra 13,3% (IC 95%: 9,2;17,5) referiram ter usado ao menos um psicotrópico, destacando-se os antidepressivos (5,0%) e os benzodiazepínicos (3,1%). Na análise multivariável, sexo feminino e presença de transtorno mental comum mantiveram-se associados ao uso de benzodiazepínicos. Renda per capita mostrou-se direta e independentemente associada ao uso de psicofármacos, conforme aumento da renda. CONCLUSÕES: Menor renda associou-se à presença de transtorno mental comum, mas não ao uso de psicotrópicos. A associação entre transtorno mental comum e uso de psicotrópicos e maior renda reforça a hipótese da existência de iniqüidades no acesso à assistência médica na população estudada.OBJETIVO: Evaluar la influencia de las condiciones socioeconómicas en la asociación entre trastornos mentales comunes, uso de servicios de salud y de psicofármacos. El uso de servicios fue investigado con relación a la quincena anterior a la entrevista y uso de psicotrópicos, en los tres días anteriores. Se utilizó regresión logística para análisis multivariable, considerando el efecto del diseño. RESULTADOS: En el total de la muestra, 13.4% (IC 95%: 10.7;16.0) buscaron servicios de salud en la quincena anterior a la entrevista. La búsqueda de servicios de salud se asoció al sexo femenino (OR=2.0) y la presencia de trastorno mental común (OR=2.2). En la muestra 13.3% (IC 95%: 9.2; 17.5) se refirieron a haber usado al menos un psicotrópico, destacándose los antidepresivos (5.0%) y los benzodiazepínicos (3.1%). En el análisis multivariable, sexo femenino y presencia de trastorno mental común se mantuvieron asociados al uso de benzodiazepínicos. Renta per capita se mostró directa e independientemente asociada al uso de psicofármacos, de acuerdo al aumento de la renta. CONCLUSIONES: Menor renta se asoció a la presencia de trastorno mental común, pero no al uso de psicotrópicos. La asociación entre trastorno mental común y uso de psicotrópicos y mayor renta refuerza la hipótesis de la existencia de injusticias en el acceso a la asistencia médica en la población estudiada.OBJECTIVE: To evaluate the influence of socioeconomic conditions on the association between common mental disorders and the use of health services and psychoactive drugs. METHODS: This was a population-based cross-sectional study conducted in the city of Botucatu, Southeastern Brazil. The sample was probabilistic, stratified and cluster-based. Interviews with 1,023 subjects aged 15 years or over were held in their homes between 2001 and 2002. Common mental disorders were evaluated using the Self-Reporting Questionnaire (SRQ-20). The use of services was investigated in relation to the fortnight preceding the interview and the use of psychotropic drugs, over the preceding three days. Logistic regression was used for multivariable analysis, and the design effect was taken into consideration. RESULTS: Out of the whole sample, 13.4% (95% CI: 10.7;16.0) had sought health services over the fortnight preceding the interview. Seeking health services was associated with female gender (OR=2.0) and the presence of common mental disorders (OR=2.2). 13.3% of the sample (95% CI: 9.2;17.5) said they had used at least one psychotropic drug, especially antidepressives (5.0%) and benzodiazepines (3.1%). In the multivariable analysis, female gender and the presence of common mental disorders remained associated with the use of benzodiazepines. Per capita income presented a direct and independent association with the use of psychoactive drugs: the greater the income, the greater the use of these drugs was. CONCLUSIONS: Lower income was associated with the presence of common mental disorders, but not with the use of psychotropic drugs. The association of common mental disorders and the use of psychotropic drugs in relation to higher income strengthens the hypothesis that inequality of access to medical services exists among this population.

OBJECTIVE: To translate and adapt the Brazilian version of the Camberwell Assessment of Needs schedule for research, and to assess its inter-rater reliability in a sample of first-episode psychosis cases in São Paulo, Brazil. METHOD: A sub-sample of participants included in a study of first-episode psychosis in São Paulo was assessed utilizing the 22 domains of the Brazilian version of the Camberwell Assessment of Needs. The Camberwell Assessment of Needs was applied, tape-recorded and rated by research assistants. An independent rater gave scores to the same participants, based on the recorded interviews. The kappa coefficient and the intraclass correlation coefficient were used to assess inter-rater reliability. RESULTS: Fifty-two subjects were included. For seven domains the inter-rater reliability was almost perfect to total (range: 0.81 to 1.0). The domains with the lowest agreement were transport and benefits. The inter-rater reliability for the total number of needs was excellent (ICC = 0.95). CONCLUSIONS: The inter-rater reliability of the Camberwell Assessment of Needs was similar to what has been found in previous studies. The Camberwell Assessment of Needs showed to be easy to use and reliable with first-episode psychosis individuals. The use of standardized instruments to assess needs of care in Brazil will contribute to the assessment of the effectiveness of treatment and to the planning of individualized care for individuals with mental illnesses.OBJETIVO: Traduzir e adaptar a versão para pesquisa da escala Camberwell de Avaliação de Necessidades e avaliar sua confiabilidade entre avaliadores em uma amostra de casos incidentes de psicoses funcionais em São Paulo, Brasil. MÉTODO: Uma amostra de participantes incluídos em um estudo de casos incidentes de psicoses funcionais em São Paulo foi avaliada utilizando-se os 22 domínios da versão brasileira da escala Camberwell de Avaliação de Necessidades. A escala Camberwell de Avaliação de Necessidades foi aplicada, gravada e pontuada por assistentes de pesquisa. Um avaliador independente pontuou os mesmos participantes, baseando-se nas gravações das entrevistas. O coeficiente kappa e o coeficiente de correlação intraclasse foram utilizados para avaliar a confiabilidade entre avaliadores. RESULTADOS: Cinqüenta e dois indivíduos foram incluídos. Para sete domínios, a confiabilidade entre avaliadores foi de quase perfeita a total (k = 0,81 a 1,00). Os domínios com a menor concordância entre avaliadores foram transporte e benefícios. O coeficiente de correlação intraclasse para o total de necessidades foi excelente (ICC = 0,95). CONCLUSÕES: A confiabilidade entre avaliadores da escala Camberwell de Avaliação de Necessidades foi similar aos resultados de estudos anteriores. A escala Camberwell de Avaliação de Necessidades mostrou-se confiável e fácil de aplicar em indivíduos com psicoses incidentes. O uso de instrumentos padronizados para a avaliação de necessidades de cuidado no Brasil contribuirá para a avaliação da efetividade de tratamentos e para o planejamento de cuidados personalizados para indivíduos com transtornos mentais.

PURPOSE: To carry out a systematic review and meta-analysis of the efficacy of chemonucleolysis in the treatment of lumbar disc herniation. METHODS: Clinical trials were selected from 3 electronic databases (The Cochrane Controlled Trials Register, MEDLINE, and EMBASE). Data were analyzed with the software STATA, using the meta command. RESULTS: Twenty-two clinical trials were eligible. For chemonucleolysis versus placebo, the summary risk ratio estimate for pain relief as outcome was 1.51 (95% CI: 1.27-1.80). The summary estimate was 1.07 (95% CI: 0.95-1.20) for the comparison between chymopapain and collagenase. Regarding chemonucleolysis with chymopapain versus surgery, the fixed-effect summary estimate of effect for pain relief was 0.93 (95% CI: 0.88-0.98) with surgery as the reference group. In this case, heterogeneity was statistically significant. CONCLUSIONS: Chemonucleolysis with chymopapain was superior to placebo and was as effective as collagenase in the treatment of lumbar disc prolapse. Results for studies comparing chemonucleolysis with surgery were heterogeneous, making it difficult to interpret the summary measure of effect.OBJETIVO: Avaliar a eficácia da quimonucleólise no tratamento da hérnia de disco lombar por meio de uma metanálise de ensaios clínicos. MÉTODOS: Os ensaios clínicos foram selecionados de três bases de dados eletrônicas( Cochrane, MEDLINE, e EMBASE). Os dados foram analisados por intermédio do aplicativo STATA, com o comando meta. RESULTADOS: trabalhamos com 22 ensaios clínicos. Para a comparação entre quimonucleólise e placebo, a estimativa da razão de riscos, tendo melhora da dor como desfecho, foi de 1,51 (I 95% C: 1,27-1,80). Aquela medida foi de 1,07 (I 95% C: 0,95-1,20) para a comparação entre quimopapaína e colagenase. Em um modelo de efeitos fixos, a razão de risco, para melhora da dor, foi 0,93 (I 95% C: 0,88-0,98), tendo a discectomia como grupo de referência. Nesse caso, um teste de heterogeneidade foi considerado estatisticamente significante. CONCLUSÕES: a eficácia da quimonucleólise foi superior à do placebo e semelhante à da colagenase. Os resultados dos estudos referentes à comparação entre quimonucleólise e cirurgia foram heterogêneos, o que implica interpretação não-trivial da medida de efeito.

OBJETIVOS: estimar a prevalência de ansiedade puerperal (AP) e fatores de risco associados, em amostra de mulheres de clínica privada. MÉTODOS: foi realizado estudo de corte transversal com 299 mulheres, atendidas em consulta ginecológica de rotina, durante o período de agosto de 2000 a maio de 2003. Foram utilizados o STAIT (Spielberger State-Trait Anxiety Inventory), para avaliação de ansiedade puerperal, e um questionário com dados sociodemográficos e obstétricos. Os critérios de inclusão foram: puérperas sem história atual ou passada de depressão ou tratamento psiquiátrico, alcoolismo ou abuso de drogas e cujos filhos estavam vivos. As prevalências de AP-traço e AP-estado que avaliam, respectivamente, características de personalidade e ansiedade transitória, segundo o STAIT, foram estimadas, conjuntamente com o intervalo de confiança (IC) 95%. Estimaram-se os odds ratios (OR) e os intervalos de confiança de 95%, na avaliação da associação entre AP e as variáveis explicativas. Utilizou-se o teste do chi2 ou chi2 de tendência, quando as categorias foram ordenadas para análise estatística. Valor de pPURPOSE: postpartum anxiety (PPA) is highly prevalent and has important consequences on mother and newborn. The aim of the present study was to estimate the prevalence of PPA and its risk factors, in a sample of women attending a private setting. METHODS: a cross-sectional study was performed with 299 women, at a routine gynecological visit, from August 2000 to May 2003. The Spielberger State-Trait Anxiety Inventory (STAIT) and a questionnaire with sociodemographic data and obstetric data were used. Inclusion criteria were: women with no past or present history of depression, psychiatric treatment, alcohol or drug abuse and whose children were alive. The prevalences of PPA-trace and PPA-state, that evaluate characteristics of personality and transitory anxiety, respectively, were estimated with 95% confiance intervals (CI). Odds ratios and 95% CI were used to examine the association between PPA and exposure variables. OBJECTIVE: To investigate the social adjustment of individuals with severe mental illness living in the community in a large urban center of a developing country, and the characteristics associated with poor social functioning. METHOD: A cross-sectional study was performed in the city of Sao Paulo. Eligible subjects were residents of a defined geographic area, aged between 18 and 65, with a diagnosis of functional psychosis who had had contact with any public psychiatric service during a defined period. Structured assessments were used to obtain information on social-demographic characteristics, diagnosis (ICD-10), psychiatric symptoms (PANSS), and social adjustment (DAS). RESULTS: One hundred and eighty-eight subjects were included, of whom, 120 (63.8%) had some degree of impairment in social functioning. The most frequently affected areas of social functioning were work performance and sexual role. Twenty-four patients (12.8%) showed poor or very poor social adjustment in the month prior to the interview. Negative symptoms, number of previous admissions and general symptoms showed statistically significant associations with global social adjustment scores. CONCLUSIONS: The proportion of patients showing any degree of impairment in social adjustment was as high as in more developed societies. In order to successfully implement the new mental health policy in Brazil, better provision of community-based mental health services for those with severe mental illnesses is needed.OBJETIVO: Investigar o ajustamento social em indivíduos com transtornos mentais graves vivendo na comunidade de um grande centro urbano de um país em desenvolvimento e as características associadas a um ajustamento social pobre. MÉTODO: Um estudo de corte transversal foi conduzido em São Paulo. Indivíduos elegíveis deviam residir em áreas definidas da cidade de São Paulo, ter tido contato com os serviços psiquiátricos do setor público em período de três meses, idade entre 18 e 65 anos e diagnóstico de psicose funcional. Entrevistas estruturadas foram utilizadas para obter dados sociodemográficos, diagnóstico (CID-10), sintomas psiquiátricos (PANSS) e ajustamento social (DAS). RESULTADOS: Cento e oitenta e oito indivíduos foram incluídos e 120 (63,8%) apresentaram algum grau de desajustamento social. As áreas mais afetadas foram performance no trabalho e sexual. Vinte e quatro indivíduos (12,8%) apresentaram ajustamento social pobre ou muito pobre no mês anterior à entrevista. Número de internações anteriores, sintomas gerais e negativos de esquizofrenia mostraram-se associados ao ajustamento social global. CONCLUSÕES: A proporção de indivíduos apresentando qualquer grau de desajustamento social é comparável à encontrada em países mais desenvolvidos. Para a adequada implementação das novas políticas públicas em Saúde Mental no Brasil, há a necessidade de melhor provisão de serviços aos indivíduos com transtornos mentais graves.

Brazil has one of the fastest aging populations in the world and the incidence of cognitive impairment in the elderly is expected to increase exponentially. We examined the association between cognitive impairment and fruit and vegetable intake and associated factors in a low-income elderly population. A cross-sectional population-based study was carried out with 1849 individuals aged 65 or over living in São Paulo, Brazil. Cognitive function was assessed using the Community Screening Instrument for Dementia (CSI-D). The effects of organizational justice on workers' health have been investigated in several areas of work. However, the systematization of available information on the effects related to cardiovascular diseases (CVD) and diabetes is scarce. This article aims to systematically review the association between organizational justice and CVD and metabolic disease in adult workers. The search strategy included the terms organizational justice, coronary heart disease, cerebrovascular disease, systemic arterial hypertension, diabetes mellitus and CVD. This study investigated the following databases: MEDLINE, EMBASE, and LILACS. The quality of the studies was assessed using the instrument developed by the National Institute of Health. Results: This study identified 1,959 titles. After evaluation, eight studies were selected. Individuals with a high perception of organizational justice showed a lower risk of CVD and metabolic disease, whereas low organizational justice presented repercussions for the cardiovascular and metabolic health of workers. The development of strategies to promote organizational justice must be prioritized and thus mitigate its impacts on workers and institutions. [ABSTRACT FROM AUTHOR]

BACKGROUND: Worldwide, a high proportion of HIV-infected individuals enter into HIV care late. Here, our objective was to estimate the impact that late entry into HIV care has had on AIDS mortality rates in Brazil. METHODOLOGY/PRINCIPAL FINDINGS: We analyzed data from information systems regarding HIV-infected adults who sought treatment at public health care facilities in Brazil from 2003 to 2006. We initially estimated the prevalence of late entry into HIV care, as well as the probability of death in the first 12 months, the percentage of the risk of death attributable to late entry, and the number of avoidable deaths. We subsequently adjusted the annual AIDS mortality rate by excluding such deaths. Of the 115,369 patients evaluated, 50,358 (43.6%) had entered HIV care late, and 18,002 died in the first 12 months, representing a 16.5% probability of death in the first 12 months (95% CI: 16.3-16.7). By comparing patients who entered HIV care late with those who gained timely access, we found that the risk ratio for death was 49.5 (95% CI: 45.1-54.2). The percentage of the risk of death attributable to late entry was 95.5%, translating to 17,189 potentially avoidable deaths. Averting those deaths would have lowered the 2003-2006 AIDS mortality rate by 39.5%. Including asymptomatic patients with CD4(+) T cell counts >200 and ≤ 350 cells/mm(3) in the group who entered HIV care late increased this proportion by 1.8%. CONCLUSIONS/SIGNIFICANCE: In Brazil, antiretroviral drugs reduced AIDS mortality by 43%. Timely entry would reduce that rate by a similar proportion, as well as resulting in a 45.2% increase in the effectiveness of the program for HIV care. The World Health Organization recommendation that asymptomatic patients with CD4(+) T cell counts ≤ 350 cells/mm(3) be treated would not have a significant impact on this scenario.

OBJETIVO: Revisar dados da literatura relativos ao diagnóstico de primeiro episódio psicótico no contexto das emergências psiquiátricas. MÉTODO: Revisão de artigos empíricos e de revisão selecionados por meio de busca eletrônica no indexador PubMed. RESULTADOS: Características específicas de atendimento de emergência - avaliação única e breve, em corte transversal e com poucas informações disponíveis - podem dificultar o processo diagnóstico. Essas limitações podem ser contornadas por meio da aplicação adequada de critérios diagnósticos operacionais, do uso de escalas e entrevistas diagnósticas padronizadas e de um tempo mínimo de observação de 24 a 72 horas. Diagnósticosdetranstornobipolar,esquizofrenia,depressãopsicóticaetranstorno delirante elaborados em contexto de emergência apresentam boa estabilidade temporal, não ocorrendo o mesmo com diagnósticos de transtorno psicótico breve, transtorno esquizofreniforme e transtorno esquizoafetivo. Primeiro episódio psicótico pode ocorrer na vigência do uso de substâncias psicoativas, sendo relativamente frequente a manutenção do quadro psicótico mesmo após cessação do uso. A utilização racional de exames complementares pode ajudar no diagnóstico diferencial com episódios psicóticos devido a condições médicas gerais. CONCLUSÃO: Diagnósticos de primeiro episódio psicótico podem ser adequadamente realizados durante emergências psiquiátricas, desde que sejam implementadas rotinas baseadas em evidências científicas.OBJECTIVE: To review the literature regarding the diagnosis of first episode psychosis in the context of emergency psychiatry. METHOD: Review of empirical and review articles selected by electronic search in the database PubMed. RESULTS: Specific features of emergency care - single, brief and cross section assessment, and with little information - may jeopardize the diagnostic process. These limitations can be circumvented by application of operational diagnostic criteria, the use of scales and structured interviews and a short period of observation, between 24-72 hours. Diagnoses of bipolar disorder, schizophrenia, psychotic depression and delusional disorder developed in the context of emergency have good stability, but not the diagnoses of brief psychotic disorder, schizophreniform disorder and schizoaffective disorder. First episode psychosis can occur in the course of the use of psychoactive substances, with relatively frequent maintenance of psychotic symptoms even after cessation of the use of the substance. The rational use of subsidiary tests may help the differential diagnosis of psychotic episodes due to general medical conditions. CONCLUSION: Diagnoses of first psychotic episode can be adequately performed during psychiatric emergencies, if routines are implemented based on scientific evidence.
