Abstract

This research project aims at identifying a new network of routes and historical itineraries for the development and promotion of rural tourism in the Tuscany Region, by promoting forms of sustainable mobility in rural areas, particularly marginal ones. Religious tourism is defined as tourism, which includes visits to religious sites such as shrines, churches, abbeys, hermitages and sacred places; such places are an attraction for their religious essence as well as their artistic and cultural beauty. The ancient obsolete road system, which often survives as an archaeological relic as a precursor of the present road system, especially in rural areas, is more widespread than a layman would think. The paper refers the results of the study aimed at recovering the entire historical road that allowed pilgrims and travelers to reach La Verna (a place where St. Francis received the Stigmata) coming from Florence.

3. A new source for light dark matter isocurvature in low scale inflation

Abstract

Light scalar and pseudoscalar particles are compelling dark matter candidates, with a vast running experimental program to discover them. Previous studies have shown that these light fields can generate sizable isocurvature perturbations in high scale inflationary models. Thereby, dark matter existence and cosmic microwave background measurements impose an upper bound on the inflationary scale. In this work, focusing on the axion case, we point out that light fields present during inflation can generate important isocurvature perturbations also in scenarios of low-scale inflation. In our mechanism, the axion field starts with some non-zero field value during inflation and rolls along its potential. Since inflation has a different duration in different patches of the universe, different regions will then have different values of the axion field, generating cold dark matter isocurvature modes. These modes are fully correlated with the adiabatic ones and share the same spectral index. Abstract

We present the analysis of a new signature for light dark matter detection with superfluid $^4$He: the emission of three phonons. We show that, in a region of mass below the MeV, the kinematics of this process can offer a way to reconstruct the dark matter interaction vertex, while providing background rejection via coincidence requirements and directionality. 22. Long-acting antipsychotic drugs for the treatment of schizophrenia: use in daily practice from naturalistic observations

Abstract

Abstract Background Current guidelines suggest specific criteria for oral or long-acting injectable antipsychotic drugs (LAIs). This review aims to describe the demographic and clinical characteristics of the ideal profile of the patient with schizophrenia treated with LAIs, through the analysis of nonrandomized studies. Methods A systematic review of nonrandomized studies in English was performed attempting to analyze the factors related to the choice and use of LAIs in daily practice. The contents were outlined using the Cochrane methods for nonrandomized studies and the variables included demographic as well as clinical characteristics. The available literature did not allow any statistical analysis that could be used to identify the ideal profile of patients with schizophrenia to be treated with LAIs. Results Eighty publications were selected and reviewed. Prevalence of LAI use ranged from 4.8% to 66%. The only demographic characteristics that were consistently assessed through retrieved studies were age (38.5 years in the 1970’s, 35.8 years in the 1980’s, 39.3 years in the 1990’s, to 39.5 years in the 2000’s) and gender (male > female). Efficacy was assessed through the use of various symptom scales and other indirect measurements; safety was assessed through extrapyramidal symptoms and the use of anticholinergic drugs, but these data were inconsistent and impossible to pool. Efficacy and safety results reported in the different studies yielded a good therapeutic profile with a maximum of 74% decrease in hospital admissions and the prevalence of extrapyramidal symptoms with LAIs consistently increased at 6, 12, 18, and 24 months (35.4%, 37.1%, 36.9%, and 41.3%, respectively). Conclusions This analysis of the available literature strongly suggests that further observational studies on patients with schizophrenia treated with LAIs are needed to systematically assess their demographic and clinical characteristics and the relationships between them and patient outcome. Besides the good efficacy and safety profile of LAIs, health care staff must also take into account the importance of establishing a therapeutic alliance with the patient and his/her relatives when selecting the most appropriate treatment. LAIs seem to be a good choice not only because of their good safety and efficacy profile, but also because they improve compliance, a key factor to improving adherence and to establishing a therapeutic alliance between patients with schizophrenia, their relatives, and their health care providers.

Abstract

Abstract Background Chronic right ventricular (RV) apical pacing may lead to left ventricular (LV) dyssynchrony and LV dysfunction. In heart failure due to RV pacing, upgrading to biventricular stimulation (CRT) can improve NYHA Class and LV function. A proportion of patients do not respond to upgrading. Aim was to assess whether etiology of LV dysfunction accounts for responses to CRT in RV-paced patients. Methods Sixty-two patients treated by CRT, under RV pacing from 50.2 ± 5.4 months, were studied. Cause of LV dysfunction was non-ischemic (NIC) in 28 and ischemic cardiomyopathy (IC) in 34 patients. Clinical and conventional echocardiographic parameters were available within 1 month before RV pacing, within 1 month before CRT and at 12 ± 2 months of follow-up (FU). Results Decreased LVEF (from 37.0 ± 8.8 to 25.6 ± 6.1%, p 10% decrease in LVESD was observed in 24 patients: 5 with IC, 19 with NIC (p < .0.001). The association between cause of LV dysfunction with >10% decrease in LVESD remained highly significant (p < 0.001) adjusting for pre-CRT QRS duration, NYHA Class, LVEF, LVESD, treatment or RV pacing duration. Conclusions CRT improves functional class even after long-lasting pacing. Reverse remodeling is evident in a small population, more likely with NIC.

Abstract Background The immune defects occurring in chronic lymphocytic leukemia are responsible for the frequent occurrence of infections and autoimmune phenomena, and may be involved in the initiation and maintenance of the malignant clone. Here, we evaluated the quantitative defects of newly produced B and T lymphocytes. Methods The output of B and T lymphocytes from the production and maturation sites was analyzed in chronic lymphocytic leukemia patients and healthy controls by quantifying kappa-deleting recombination excision circles (KRECs) and T-cell receptor excision circles (TRECs) by a Real-Time PCR assay that simultaneously detects both targets. T-lymphocyte subsets were analyzed by six-color flow cytometric analysis. Data comparison was performed by two-sided Mann-Whitney test. Results KRECs level was reduced in untreated chronic lymphocytic leukemia patients studied at the very early stage of the disease, whereas the release of TRECs+ cells was preserved. Furthermore, the observed increase of CD4+ lymphocytes could be ascribed to the accumulation of CD4+ cells with effector memory phenotype. Conclusions The decreased number of newly produced B lymphocytes in these patients is likely related to a homeostatic mechanism by which the immune system balances the abnormal B-cell expansion. This feature may precede the profound defect of humoral immunity characterizing the later stages of the disease.

Abstract

Abstract Background One third to two thirds of people with schizophrenia have persistent psychotic symptoms despite clozapine treatment. Under real-world circumstances, the need to provide effective therapeutic interventions to patients who do not have an optimal response to clozapine has been cited as the most common reason for simultaneously prescribing a second antipsychotic drug in combination treatment strategies. In a clinical area where the pressing need of providing therapeutic answers has progressively increased the occurrence of antipsychotic polypharmacy, despite the lack of robust evidence of its efficacy, we sought to implement a pre-planned protocol where two alternative therapeutic answers are systematically provided and evaluated within the context of a pragmatic, multicentre, independent randomised study. Methods/Design The principal clinical question to be answered by the present project is the relative efficacy and tolerability of combination treatment with clozapine plus aripiprazole compared with combination treatment with clozapine plus haloperidol in patients with an incomplete response to treatment with clozapine over an appropriate period of time. This project is a prospective, multicentre, randomized, parallel-group, superiority trial that follow patients over a period of 12 months. Withdrawal from allocated treatment within 3 months is the primary outcome. Discussion The implementation of the protocol presented here shows that it is possible to create a network of community psychiatric services that accept the idea of using their everyday clinical practice to produce randomised knowledge. The employed pragmatic attitude allowed to randomly allocate more than 100 individuals, which means that this study is the largest antipsychotic combination trial conducted so far in Western countries. We expect that the current project, by generating evidence on whether it is clinically useful to combine clozapine with aripiprazole rather than with haloperidol, provides physicians with a solid evidence base to be directly applied in the routine care of patients with schizophrenia. Abstract

Coffee is a key agricultural product in national and international markets. Physiological parameters, such as plant growth indicators, can signal interruptions in these processes. This study aimed to characterize the temperature obtained by a thermal camera embedded in a remotely piloted aircraft (RPA) and evaluate its relationship with the water potential (WP) and stomatal conductance (gs) of an experimental coffee plantation using geostatistical techniques. The experiment was conducted at the Federal University of Lavras, Minas Gerais, Brazil. A rotary-wing RPA with an embedded thermal camera flew autonomously at a height of 10 m and speed of 10 m/s. Images were collected on 26 November 2019 (rainy season), and 11 August 2020 (dry season), between 9:30 am and 11:30 am. Data on gs and WP were collected in the field. The thermal images were processed using FLIR Tools 5.13, and temperature analysis and spatialization were undertaken using geostatistical tools and isocolor maps by Kriging interpolation in R 4.3.2 software. Field data were superimposed on final crop temperature maps using QuantumGIS version 3.10 software. The study found that with decreasing WP, stomatal closure and reduction in gs occurred, increasing the temperature due to water deficit. Abstract

The sustainability of coffee production is a concern for producers around the world. To be sustainable, it is necessary to achieve satisfactory levels of coffee productivity and quality. Pests and diseases cause reduced productivity and can affect the quality of coffee beans. To ensure sustainability, producers need to monitor pests that can lead to substantial crop losses, such as the coffee leaf miner, Leucoptera coffeella (Lepidoptera: Lyonetiidae), which belongs to the Lepidoptera order and the Lyonetiidae family. This research aimed to use machine learning techniques and vegetation indices to remotely identify infestations of the coffee leaf miner in coffee-growing regions. Field assessments of coffee leaf miner infestation were conducted in September 2023. Aerial images were taken using remotely piloted aircraft to determine 13 vegetative indices with RGB (red, green, blue) images. The vegetation indices were calculated using ArcGis 10.8 software. A comprehensive database encompassing details of coffee leaf miner infestation, vegetation indices, and crop data. The dataset was divided into training and testing subsets. A set of four machine learning algorithms was utilized: Random Forest (RF), Logistic Regression (LR), Support Vector Machine (SVM), and Stochastic Gradient Descent (SGD). Following hyperparameter tuning, the test subset was employed for model validation. Remarkably, both the SVM and SGD models demonstrated superior performance in estimating coffee leaf miner infestations, with kappa indices of 0.6 and 0.67, respectively. The combined use of vegetation indices and crop data increased the accuracy of coffee leaf miner detection. The RF model performed poorly, while the SVM and SGD models performed better. This situation highlights the challenges of tracking coffee leaf miner infestations in fields with varying ages of coffee plants, different cultivars, and other environmental variables.
