- Abstract
The analysis of the dependence of landscape patterns on environment was carried out in order to investigate the landscape structure evolution of Spain. The underlying concept was that the dependence between landscape spatial structure and environmental factors could be gradually decreasing over time. Land cover data were recorded from aerial photo interpretation of 206 4 × 4 km 2 samples from three different years: 1956, 1984 and 1998. Geographical variables were taken into consideration together with the purely environmental ones. General Linear Models of repeated measures were then used to segregate environmental from geographical effects on the pattern of the land cover patches of the samples. Aridity, lithology and topography were the environmental factors used to analyse structural indices of landscape. Landscape composition has a higher dependence on environment than configuration. Environmental variables showed higher correlations with landscape composition and configuration than geographical variables. Among them, overall the climatic aridity and topography significantly accounted for more variation than did lithology. There was a high degree of stability in land cover composition over time, with some signif icant exceptions. Nevertheless, the registered increase of fragmentation over time has demonstrated that configuration measures are needed to fully assess landscape change. - Abstract
Background: Uncertainty exists about the optimal schedule of adjuvant treatment of breast cancer with aromatase inhibitors and, to our knowledge, no trial has directly compared the three aromatase inhibitors anastrozole, exemestane, and letrozole. We investigated the schedule and type of aromatase inhibitors to be used as adjuvant treatment for hormone receptor-positive early breast cancer. Methods: FATA-GIM3 is a multicentre, open-label, randomised, phase 3 trial of six different treatments in postmenopausal women with hormone receptor-positive early breast cancer. Eligible patients had histologically confirmed invasive hormone receptor-positive breast cancer that had been completely removed by surgery, any pathological tumour size, and axillary nodal status. Key exclusion criteria were hormone replacement therapy, recurrent or metastatic disease, previous treatment with tamoxifen, and another malignancy in the previous 10 years. Patients were randomly assigned in an equal ratio to one of six treatment groups: oral anastrozole (1 mg per day), exemestane (25 mg per day), or letrozole (2·5 mg per day) tablets upfront for 5 years (upfront strategy) or oral tamoxifen (20 mg per day) for 2 years followed by oral administration of one of the three aromatase inhibitors for 3 years (switch strategy). Randomisation was done by a computerised minimisation procedure stratified for oestrogen receptor, progesterone receptor, and HER2 status; previous chemotherapy; and pathological nodal status. Neither the patients nor the physicians were masked to treatment allocation. The primary endpoint was disease-free survival. The minimum cutoff to declare superiority of the upfront strategy over the switch strategy was assumed to be a 2% difference in disease-free survival at 5 years. - Abstract
Aims and background. We report a collection of data about early breast cancer in male patients from 13 Italian institutions. Methods and study design. We obtained data from patient charts and performed statistical analysis. The primary end points were overall survival and disease-free survival. Results. A total of 205 men with invasive breast cancer was identified, with a median age of 66 years. Pathological characteristics were heterogeneous for T stage, N stage and HER2 status. Histological subtype was predominantly ductal infiltrating carcinoma. Most of them were hormone receptor positive. Mastectomy was the most common strategy. Postsurgical treatment was not standardized. Patients with large tumors were more likely to be treated with chemotherapy. Disease recurrence was associated with an ER+ and PR+ status. Conclusions. We identified a correlation between relapse and hormone receptor expression, as is the case in female breast cancer.

Aims and background. We report a collection of data about early breast cancer in male patients from 13 Italian institutions. Methods and study design. We obtained data from patient charts and performed statistical analysis. The primary end points were overall survival and disease-free survival. Results. A total of 205 men with invasive breast cancer was identified, with a median age of 66 years. Pathological characteristics were heterogeneous for T stage, N stage and HER2 status. Histological subtype was predominantly ductal infiltrating carcinoma. Most of them were hormone receptor positive. Mastectomy was the most common strategy. Postsurgical treatment was not standardized. Patients with large tumors were more likely to be treated with chemotherapy. Disease recurrence was associated with an ER+ and PR+ status. Conclusions. We identified a correlation between relapse and hormone receptor expression, as is the case in female breast cancer.

The results find the operators' strong need to share their work experience. Moreover the whole team is reputed to be important. This work is considered to be a source of human and professional gratification rather than economic. In Austria, Sweden and Great Britain protocols are being used to produce data which can be integrated at the environmentall stratum level. The national protocols for habitat monitoring in Great Britain, Sweden and Austria have been converted into a common format based on General Habitat Categories (GHCs). The conversion tables enable full integration of the datasets. This makes it possible to use national data in estimates for European comparisons and the production European habitat extent. Without common protocols it would be impossible to carry out comparisons between environmental zones and biogeographical regions.

The analysis of the dependence of landscape patterns on environment was carried out in order to investigate the landscape structure evolution of Spain. The underlying concept was that the dependence between landscape spatial structure and environmental factors could be gradually decreasing over time. Land cover data were recorded from aerial photo interpretation of 206 4 × 4 km2 samples from three different years: 1956, 1984 and 1998. Geographical variables were taken into consideration together with the purely environmental ones. General Linear Models of repeated measures were then used to segregate environmental from geographical effects on the pattern of the land cover patches of the samples. Aridity, lithology and topography were the environmental factors used to analyse structural indices of landscape. Landscape composition has a higher dependence on environment than configuration. Environmental variables showed higher correlations with landscape composition and configuration than geographical variables. Among them, overall the climatic aridity and topography significantly accounted for more variation than did lithology. There was a high degree of stability in land cover composition over time, with some significant exceptions. Nevertheless, the registered increase of fragmentation over time has demonstrated that configuration measures are needed to fully assess landscape change., Se ha analizado la evolución de la estructura del paisaje español mediante un análisis de la dependencia de los patrones del paisaje respecto del medioambiente. La hipótesis principal es que la dependencia entre la estructura del paisaje y los factores ambientales está disminuyendo con el tiempo. Los datos de composición y configuración de paisaje fueron tomados mediante interpretación de fotografías aéreas tomadas en los años 1956, 1984 y 1998 en 206 parcelas de 4 × 4 km2. Se han tenido en cuenta variables geográficas junto con las variables puramente ambientales. Se han utilizado modelos lineales generalizados de medidas repetidas para determinar la importancia de los efectos ambientales con respecto a los geográficos en los patrones de paisaje. Las variables ambientales utilizadas para analizar los índices estructurales del paisaje fueron: aridez, litología y topografía. Existe una mayor dependencia ambiental en la composición que en la configuración. Las variables ambientales tuvieron un mayor efecto sobre la estructura del paisaje que las geográficas. Entre aquellas, la aridez climática y la topografía influyeron más que la litología. La composición se mostró en general estable a lo largo del periodo de estudio con algunas excepciones. Sin embargo, se observó una fragmentación significativa que indica la necesidad de analizar los índices de configuración para detectar los cambios de paisaje.

This research has been carried out in the framework of the EC FP7 project EBONE (EC-FP7 Contract ENV-CT-2008-212322) and co-financed by the Dutch Ministry of Economics, Agriculture and Innovation. Project code KB14-002-007., It is important to develop protocols for harmonisation in order to convert habitat categories recorded in monitoring projects in Europe into a format that enables datasets to be integrated. This deliverable demonstrates the success of the procedures for most available projects. The present document delivers conversion tables and summarises the conclusions, which can be drawn from them, including comments on their integration with satellite imagery. In Austria, Sweden and Great Britain the protocols are now being used to produce data which can be integrated at the environmental stratum level. In Spain the work in EBONE has shown that additional survey at the right level of detail will be required. The national protocols for habitat monitoring in Great Britain, Sweden and Austria have been converted into a common format based on General Habitat Categories (GHCs). The conversion tables enable full integration. This makes it possible to use national data in estimates for European comparisons and the production European habitat extent. Without common protocols it would be impossible to carry out comparisons between environmental zones and biogeographical regions. Although conversion at a high level will be possible with the Spanish SISPARES the scale of the Minimum Mappable Units does not fit the detail of the other projects. Further data therefore needs to be collected in the Spanish SISPARES sites to provide detailed mapping for integration. An additional conclusion reached during the preparation of protocols for the Countryside Survey of Great Britain, was that further divisions of the GHCs will be required to improve the relationship with Remote Sensing. These divisions are being made using ancillary data that are available in all existing projects as well as from the EBONE field records. The. quantities and composition of these wastes is described together with their physical, chemical and biological effects on the environment. At the center of the sludge dump the bearing capacity of the waters has been exceeded and an anoxic bottom area devoid of life formed. Over the last 30 years, landscape ecology has developed rapidly, and many studies now require spatial data on habitats. Without rigorous rules, changes from baseline records cannot be separated reliably from background noise. A procedure is described that satisfies these requirements and can provide consistent data for Europe, to support a range of policy initiatives and scientific projects. The methodology is based on classical plant life forms, used in biogeography since the nineteenth century, and on their statistical correlation with the primary environmental gradient. Further categories can therefore be identified for other continents to assist large scale comparisons and modelling. The model has been validated statistically and the recording procedure tested in the field throughout Europe. A total of 130 General Habitat Categories (GHCs) is defined. These are enhanced by recording environmental, site and management qualifiers to enable flexible database interrogation. The same categories are applied to areal, linear and point features to assist recording and subsequent interpretation at the landscape level. The distribution and change of landscape ecological parameters, such as connectivity and fragmentation, can then be derived and their significance interpreted.

- Abstract
The dumping of sewer sludge and dredge spoil in the waters of the New York Bight and the effect of this waste disposal practice on the marine environment is reviewed. The quantities and composition of these wastes are described, together with their physical, chemical and biological effects on the environment. At the center of the sludge dump, the bearing capacity of the waters has been exceeded, and an anoxic bottom area devoid of life formed. Both spoil and sludge contain large quantities of toxic heavy metals, and the spoil also contains large quantities of petrochemicals and pesticides. (Author)

Background: Uncertainty exists about the optimal schedule of adjuvant treatment of breast cancer with aromatase inhibitors and, to our knowledge, no trial has directly compared the three aromatase inhibitors anastrozole, exemestane, and letrozole. We investigated the schedule and type of aromatase inhibitors to be used as adjuvant treatment for hormone receptor-positive early breast cancer. Methods: FATA-GIM3 is a multicentre, open-label, randomised, phase 3 trial of six different treatments in postmenopausal women with hormone receptor-positive early breast cancer. Eligible patients had histologically confirmed invasive hormone receptor-positive breast cancer that had been completely removed by surgery, any pathological tumour size, and axillary nodal status. Key exclusion criteria were hormone replacement therapy, recurrent or metastatic disease, previous treatment with tamoxifen, and another malignancy in the previous 10 years. Patients were randomly assigned in an equal ratio to one of six treatment groups: oral anastrozole (1 mg per day), exemestane (25 mg per day), or letrozole (2·5 mg per day) tablets upfront for 5 years (upfront strategy) or oral tamoxifen (20 mg per day) for 2 years followed by oral administration of one of the three aromatase inhibitors for 3 years (switch strategy). Randomisation was done by a computerised minimisation procedure stratified for oestrogen receptor, progesterone receptor, and HER2 status; previous chemotherapy; and pathological nodal status. Neither the patients nor the physicians were masked to treatment allocation. The primary endpoint was disease-free survival. The minimum cutoff to declare superiority of the upfront strategy over the switch strategy was assumed to be a 2% difference in disease-free survival at 5 years. Primary efficacy analyses were done by intention to treat; safety analyses included all patients for whom at least one safety case report form had been completed. Follow-up is ongoing. This trial is registered with the European Clinical Trials Database, number 2006-004018-42, and ClinicalTrials.gov, number NCT00541086. Findings: Between March 9, 2007, and July 31, 2012, 3697 patients were enrolled into the study. After a median follow-up of 60 months (IQR 46â72), 401 disease-free survival events were reported, including 211 (11%) of 1850 patients allocated to the switch strategy and 190 (10%) of 1847 patients allocated to upfront treatment. 5-year disease-free survival was 88·5% (95% CI 86·7â90·0) with the switch strategy and 89·8% (88·2â91·2) with upfront treatment (hazard ratio 0·89, 95% CI 0·73â1·08; p=0·23). 5-year disease-free survival was 90·0% (95% CI 87·9â91·7) with anastrozole (124 events), 88·0% (85·8â89·9) with exemestane (148 events), and 89·4% (87·3 to 91·1) with letrozole (129 events; p=0·24). No unexpected serious adverse reactions or treatment-related deaths occurred. Musculoskeletal side-effects were the most frequent grade 3â4 events, reported in 130 (7%) of 1761 patients who received the switch strategy and 128 (7%) of 1766 patients who received upfront treatment. Grade 1 musculoskeletal events were more frequent with the upfront schedule than with the switch schedule (924 [52%] of 1766 patients vs 745 [42%] of 1761 patients). All other grade 3â4 adverse events occurred in less than 2% of patients in either group. Interpretation: 5 years of treatment with aromatase inhibitors was not superior to 2 years of tamoxifen followed by 3 years of aromatase inhibitors. None of the three aromatase inhibitors was superior to the others in terms of efficacy. Therefore, patient preference, tolerability, and financial constraints should be considered when deciding the optimal treatment approach in this setting. Funding: Italian Drug Agency.

Healthy lifestyles are associated with better health-related quality of life (HRQoL), favorable prognosis and lower mortality in breast cancer (BC) survivors. We investigated changes in HRQoL after a 12-month lifestyle modification program in 227 BC survivors participating in DEDiCa trial (Mediterranean diet, exercise, vitamin D). HRQoL was evaluated through validated questionnaires: EQ-5D-3L, EORTC-QLQ-C30 and EORTC QLQ-BR23. Baseline changes were tested using analysis of variance. Multiple regression analyses were performed to assess treatment effects on HRQoL. Increases were observed in global health status (p <, 0.001), physical (p = 0.003), role (p = 0.002) and social functioning (p <, 0.001), body image (p <, 0.001), future perspective (p <, 0.001), well-being (p = 0.001), and reductions in fatigue (p <, 0.001), nausea and vomiting (p = 0.015), dyspnea (p = 0.001), constipation (p = 0.049), financial problems (p = 0.012), sexual functioning (p = 0.025), systematic therapy side effects (p <, 0.001) and breast symptoms (p = 0.004). Multiple regression analyses found inverse associations between changes in BMI and global health status (p = 0.048) and between serum 25(OH)D levels and breast symptoms (p = 0.002). A healthy lifestyle treatment of traditional Mediterranean diet and exercise may impact positively on HRQoL in BC survivors possibly through reductions in body weight while vitamin D sufficiency may improve BC-related symptoms. These findings are relevant to BC survivors whose lower HRQoL negatively affects treatment compliance and disease outcomes.
