Abstract

Summary: Prolongation of peripheral nerve blockade by intravenous dexamethasone may be extended by intravenous dexmedetomidine. We randomly allocated 122 participants who had intravenous dexamethasone 0.15 mg.kg−1 before interscalene brachial plexus block for day‐case arthroscopic rotator cuff repair to intravenous saline (62 participants) or intravenous dexmedetomidine 1 μg.kg−1 (60 participants). The primary outcome was time from block to first oral morphine intake during the first 48 postoperative hours. Fifty‐nine participants reported taking oral morphine, 25/62 after placebo and 34/60 after dexmedetomidine, p = 0.10. The time to morphine intake was shorter after dexmedetomidine, hazard ratio (95%CI) 1.68 (1.00–2.82), p = 0.049. Median (IQR [range]) morphine doses were 0 (0–12.5 [0–50]) mg after control vs. 10 (0–30 [0–50]) after dexmedetomidine, a difference (95%CI) of 7 (0–10) mg, p = 0.056. There was no effect of dexmedetomidine on pain at rest or on movement. Intra‐operative hypotension was recorded for 27/62 and 50/60 participants after placebo vs. dexmedetomidine, respectively, p < 0.001. Other outcomes were similar, including durations of sensory and motor block. In conclusion, dexmedetomidine shortened the time to oral morphine consumption after interscalene block combined with dexamethasone and caused intra‐operative hypotension. [ABSTRACT FROM AUTHOR]

Abstract

BackgroundTissue inflammation in inflammatory bowel diseases (IBD) is associated with a decrease in local pH. The gene encoding G-protein-coupled receptor 65 (GPR65) has recently been reported to be a genetic risk factor for IBD. In response to extracellular acidification, proton activation of GPR65 stimulates cAMP and Rho signalling pathways. We aimed to analyse the clinical and functional relevance of the GPR65 associated single nucleotide polymorphism (SNP) rs8005161.Methods1138 individuals from a mixed cohort of IBD patients and healthy volunteers were genotyped for SNPs associated with GPR65 (rs8005161, rs3742704) and galactosylceramidase (rs1805078) by Taqman SNP assays. 2300 patients from the Swiss IBD Cohort Study (SIBDC) were genotyped for rs8005161 by mass spectrometry based SNP genotyping. IBD patients from the SIBDC carrying rs8005161 TT, CT, CC and non-IBD controls (CC) were recruited for functional studies. Human CD14+ cells were isolated from blood samples and subjected to an extracellular acidic pH shift, cAMP accumulation and RhoA activation were measured.ResultsIn our mixed cohort, but not in SIBDC patients, the minor variant rs8005161 was significantly associated with UC. In SIBDC patients, we observed a consistent trend in increased disease severity in patients carrying the rs8005161-TT and rs8005161-CT alleles. No significant differences were observed in the pH associated activation of cAMP production between IBD (TT, CT, WT/CC) and non-IBD (WT/CC) genotype carriers upon an acidic extracellular pH shift. However, we observed significantly impaired RhoA activation after an extracellular acidic pH shift in IBD patients, irrespective of the rs8005161 allele.ConclusionsThe T allele of rs8005161 might confer a more severe disease course in IBD patients. Human monocytes from IBD patients showed impaired pH associated RhoA activation upon an acidic pH shift.

The effects of the central electron cyclotron heating (ECH) and current drive (ECCD) on the spontaneous plasma rotation and on the presence of Tearing Modes (TM), observed in the TCV tokamak[1], were recently investigated as an interplay between the toroidal velocity and NTM onset in absence of sawteeth, ELMs and error fields [2-3]. In a set of reproducible TCV discharges (Ip~ -150 kA, Bt~ -1.4 T, ne,av~1.5 1019 m-3, Te~ 3 keV and Ti~0.25 keV, q95~5.8) with both pure EC heating and current drive the cnt-Ip toroidal velocity was observed to be reduced with subsequent co-Ip appearance of 3/2 and 2/1 modes during the ramp up EC phases. The understanding of the capability of the on-axis EC power to modify the rotation profiles before and after the TM onset and of the sudden disappearance of 3/2 mode when 2/1 starts is the main purpose of this work. The velocity profile modifications are due to a direct effect of the EC absorbed power and also related to some variation of the perpendicular diffusion of the toroidal momentum and to magnetic braking effects of the kind of neoclassical toroidal viscosity (NTV) due to the NTM resonant field perturbations associated to the presence of TM. Numerical investigations are performed using a 1D toroidal momentum balance equation including contributions by external sources, as EC power, and NTV torques. Furthermore, the combined evolution of the 3/2 and 2/1 modes requires considering also coupling effects included in a generalized Rutherford equation for the modelling of the TM time growth. Abstract

Highly localized deposition of ECRH/ECCD is particularly suited for MHD control, in particular when combined with real-time beam orientation and power control capabilities. The powerful (4.5MW) and flexible (7 steerable launcher) EC system on TCV has recently been complemented by an equally flexible digital real-time control system with the aim of developing and testing integrated MHD control methods [1]. Sawtooth pacing is one such method [2]. The crash time of stabilized sawteeth can be precisely controled by removing the EC power at a given time after the last sawtooth crash, causing the crash to occur at a short and reproducible time thereafter. This control strategy is combined with efficient neoclassical tearing mode (NTM) preemption by depositing power at the mode rational surfaces only during a short time synchronized with the island-seeding sawtooth crash. If an NTM appears nevertheless, full power is applied to stabilize the mode. The real-time steerable launchers have also been employed to stabilize fully saturated NTMs and to investigate the precise requirements for deposition localization for full island stabilization. Finally, though ELM dynamics is markedly different, recent results show that ELM pacing is possible using a similar control technique as used for sawtooth pacing. In this case, edge EC power is removed after each ELM, and is reapplied after a programmable time interval. The ELM period can be real-time controlled by adjusting the length of this interval. While the overall trend conforms to the increase of ELM frequency with increasing power, this technique provides a means to significantly regularize the ELM cycle. [ABSTRACT FROM AUTHOR]

Abstract

Simultaneous real-time control of multiple MHD instabilities is experimentally demonstrated in the TCV tokamak. Multiple sources of ECheating and current drive, injected through real-time controlled launchers, are used to stabilize 3/2 and 2/1 neoclassical tearing modes (NTMs) rapidly after their appearance. Control of the sawtooth instability using a new sawtooth-pacing technique is demonstrated, providing precise control of the time of appearance of the sawtooth crash. Efficient NTM preemption can then be performed by applying pulsed power on the mode rational surface at the time of the seed-island generating sawtooth crash. These three elements are combined into one integrated control system which can simultaneously control the sawtooth period, preempt the formation of NTMs and suppress these if they appear.
