Abstract

Cholesterol absorption by the small intestine and the gall bladder is generally a passive process. That is, the concentration of cholesterol in the bile and in the small intestine is sufficiently high that much of it will pass into the organs naturally, without the expenditure of energy. However, when these tissues are poisoned with cyanide, which cripples the energy production of cells, there is a reduction in the amount of cholesterol absorbed. In the case of the gall bladder, cholesterol absorption is reduced by 30 percent, indicating that this portion of cholesterol absorption relies upon the expenditure of energy by the lining of the gall bladder. Utilizing human gall bladders, which had been surgically removed for medical purposes, researchers used an artificial concoction resembling bile to investigate the factors affecting cholesterol absorption. Their results indicate that the composition of bile salts affected the uptake of cholesterol. In particular, substances like chenodeoxycholic acid may increase the uptake of cholesterol, but inhibit its transport through the cell. Consequently, such substances might result in the accumulation of cholesterol. Since chenodeoxycholic acid is used therapeutically to dissolve gallstones, the possibility exists that extended treatment with this drug may result in the accumulation of cholesterol in the mucosa of the gall bladder. Abstract

Infectious and inflammatory diseases have repeatedly shown strong genetic associations within the major histocompatibility complex (MHC); however, the basis for these associations remains elusive. To define host genetic effects on the outcome of a chronic viral infection, we performed genome-wide association analysis in a multiethnic cohort of HIV-1 controllers and progressors, and we analyzed the effects of individual amino acids within the classical human leukocyte antigen (HLA) proteins. We identified >300 genome-wide significant single-nucleotide polymorphisms (SNPs) within the MHC and none elsewhere. Specific amino acids in the HLA-B peptide binding groove, as well as an independent HLA-C effect, explain the SNP associations and reconcile both protective and risk HLA alleles. These results implicate the nature of the HLA-viral peptide interaction as the major factor modulating durable control of HIV infection.

10. Direct versus indirect determination of suspended-sediment-associated metals in a mining-influenced watershed.

Abstract

Published water quality data often include only concentrations of total and dissolved metals, with particulate concentrations assumed equal to the difference between the two. The validity of this assumption for mining-influenced waters was investigated by comparing data determined from the subtraction of total and dissolved metal concentrations (difference method) with data obtained from acid-digestion of the particles retained on the filters used for filtering the dissolved fraction (digestion method). Water samples containing suspended sediment were collected from three sites in the Clear Creek watershed in Colorado, USA, where sulphide mining activities have occurred. The volumes of water filtered ranged from 50 to 250 ml, representative of those typically used for the determination of dissolved metal concentrations. The particulate-associated concentrations of Al, Cu, Fe, Mn, and Zn determined from the difference method compared well with those determined by the digestion method. In extremely oligotrophic Lac Matamec (Quebec, Canada) the ultraplankton (<15 μm) represented 47% of total phytoplankton biomass but accounted for 71% of the primary production and was 2.5 times more photosynthetically efficient than larger phytoplankton. Abstract

A hand-held interactive electronic lung function device and tele-health monitoring system (AirWatch Airway Monitoring System, ENACT Health Management Systems, Inc., Mountain View, California, USA) has been developed that measures, records and reports airway status via a remote computer to the healthcare team of the patient with asthma. This article discusses the contribution of this electronic lung function monitoring system to a comprehensive programme of asthma care of 26 patients with severe asthma (mean age 48.7 ± 17.9 years) followed for a total of 359 patient-months. The total number of emergency room visits decreased from 84 (mean 3.32 ± 8.45 per patient) before beginning the treatment programme to 1 (mean 0.04 ± 0.20 per patient) after participating in the programme (Mann-Whitney U, p < 0.001). The total number of inpatient hospital days decreased from 114 (mean 4.38 ± 7.01 per patient) before beginning the programme to 3 (mean 0.12 ± 0.43 per patient) after the programme (Mann-Whitney U, p < 0.001). Estimated average emergency room charges were reduced from $US2656 to $US32, and average hospital charges were reduced from $US4380 to $US120 for equivalent pre- and post-programme periods. The requirement for prednisone pulses was reduced (t-test, p < 0.008). The mean symptom score (higher score means better function) before entrance into the programme was 0.62 ± 1.20 and improved to a mean of 3.38 ± 1.92 after participation in the programme (Mann-Whitney U, p < 0.0001). Abstract

The paper is all about the restoration of vintage IBM 1401 computer for the computer history museum, in Mountain View, Calif. The 1401 processed numbers in decimal rather than binary form and accepted numbers of varying digit spans. Companies leased it for US $6500 a month, equivalent to $45 000 today, a bargain for an all-transistor design with stored programs. Such features had been offered only in huge systems costing about six times as much. The museum got an even better bargain in 2003, when five enthusiasts gave it an old 1401 they'd bought on the German eBay site for a mere $18000, then about $21000. In part, the low price reflects the water damage that occurred during storage in Hamburg, damage the rehabbers took some 10000 man-hours to repair, typically with old parts. The selling price also reflects the old-tech fervor of the seller, a German engineer who'd bought the machine for his own use in 1972, when IBM stopped supporting the model, and couldn't bear to scrap it when its working life was done. None of the patients started on 500 mg daily showed complete gallstone dissolution. 8 out of 41 patients on 750 mg daily had complete dissolution of their radiolucent gallbladder stones after six or more months, and a further 4 showed partial dissolution. 5 out of 28 patients on 1000 mg daily had complete dissolution of their radiolucent gallbladder stones after at least six months, and a further 9 showed partial gallstone dissolution. The mean duration of therapy was greater on 750 mg than on 1000 mg/day (1·27 vs. 0·58 years), and when results were analysed after the first six months' therapy the total response-rate was significantly greater for the 1000 mg dose (13 out of 28) than for the 750 mg dose (9 out of 41). The individual response of radiolucent gallbladder stones to therapy could not be predicted from stone size, weight of patient, dosage/kg, or change in biliary lipids. Treatment of radiolucent gallstones with chenodeoxycholic acid should start at 1000 mg daily.

Abstract

Giving clofibrate 2 g daily to seven patients significantly increased the biliary cholesterol concentration while the proportion of bile acids fell. Five patients on established clofibrate treatment were given 750 mg of chenodeoxycholic acid (CDCA) daily for one month. Biliary lipid analysis after the CDCA treatment showed a significant fall in the proportion of cholesterol and a rise in that of bile acids. The serum lipid concentrations, which had already been reduced by diet and clofibrate, showed a further significant reduction after the introduction of CDCA.
This study suggests the CDCA may be usefully combined with clofibrate to reverse the tendency towards cholesterol saturation of bile and enhance the effect of lowering serum lipid concentrations. [ABSTRACT FROM AUTHOR]
