2. A new SEIR type model including quarantine effects and its application to analysis of Covid-19 pandemia in Poland in March-April 2020

Abstract

Contact tracing and quarantine are well established non-pharmaceutical epidemic control tools. The paper aims to clarify the impact of these measures in COVID-19 epidemic. A new deterministic model is introduced (SEIRQ: susceptible, exposed, infectious, removed, quarantined) with Q compartment capturing individuals and releasing them with delay. We obtain a simple rule defining the reproduction number $\mathcal{R}$ in terms of quarantine parameters, ratio of diagnosed cases and transmission parameters. The model is applied to the epidemic in Poland in March - April 2020, when social distancing measures were in place. We investigate 3 scenarios corresponding to different ratios of diagnosed cases. Our results show that depending on the scenario contact tracing could have prevented from 50\% to over 90\% of cases. The effects of quarantine are limited by fraction of undiagnosed cases. Taking into account the transmission intensity in Poland prior to introduction of social restrictions it is unlikely that the control of the epidemic could be achieved without any social distancing measures.

7. Conflict diagnostics for evidence synthesis in a multiple testing framework

Abstract

Evidence synthesis models that combine multiple datasets of varying design, to estimate quantities that cannot be directly observed, require the formulation of complex probabilistic models that can be expressed as graphical models. An assessment of whether the different datasets synthesised contribute information that is consistent with each other, and in a Bayesian context, with the prior distribution, is a crucial component of the model criticism process. However, a systematic assessment of conflict suffers from the multiple testing problem, through testing for conflict at multiple locations in a model. We demonstrate the systematic use of conflict diagnostics, while accounting for the multiple hypothesis tests of no conflict at each location in the graphical model. The method is illustrated by a network meta-analysis to estimate treatment effects in smoking cessation programs and an evidence synthesis to estimate HIV prevalence in Poland.

Abstract

Dynamics of the infectious disease transmission is often best understood taking into account the structure of population with respect to specific features, in example age or immunity level. Practical utility of such models depends on the appropriate calibration with the observed data. Here, we discuss the Bayesian approach to data assimilation in case of two-state age-structured model. This kind of models are frequently used to describe the disease dynamics (i.e. force of infection) basing on prevalence data collected at several time points. We demonstrate that, in the case when the explicit solution to the model equation is known, accounting for the data collection process in the Bayesian framework allows to obtain an unbiased posterior distribution for the parameters determining the force of infection. We further show analytically and through numerical tests that the posterior distribution of these parameters is stable with respect to cohort approximation (Escalator Boxcar Train) to the solution. Finally, we apply the technique to calibrate the model based on observed sero-prevalence of varicella in Poland.

The Escalator Boxcar Train (EBT) is a tool widely used in the study of balance laws motivated by structure population dynamics. This paper proves that the approximate solutions defined through the EBT converge to exact solutions. Moreover, this method is rigorously shown to be effective also in computing optimal controls. As preliminary results, the well posedness of classes of PDEs and of ODEs comprising various biological models is also obtained. Abstract

Background People who inject drugs (PWID) are frequently incarcerated, which is associated with multiple negative health outcomes. Aim We aimed to estimate the associations between a history of incarceration and prevalence of HIV and HCV infection among PWID in Europe. Methods Aggregate data from PWID recruited in drug services (excluding prison services) or elsewhere in the community were reported by 17 of 30 countries (16 per virus) collaborating in a European drug monitoring system (2006–2020; n = 52,368 HIV+/−; n = 47,268 HCV+/−). Country-specific odds ratios (OR) and prevalence ratios (PR) were calculated from country totals of HIV and HCV antibody status and self-reported life-time incarceration history, and pooled using meta-analyses. Country-specific and overall population attributable risk (PAR) were estimated using pooled PR. Results Univariable HIV OR ranged between 0.73 and 6.37 (median: 2.1; pooled OR: 1.92; 95% CI: 1.52–2.42). Pooled PR was 1.66 (95% CI 1.38–1.98), giving a PAR of 25.8% (95% CI 16.7–34.0). Univariable anti-HCV OR ranged between 1.06 and 5.04 (median: 2.70; pooled OR: 2.51; 95% CI: 2.17–2.91). Pooled PR was 1.42 (95% CI: 1.28–1.58) and PAR 16.7% (95% CI: 11.8–21.7). Subgroup analyses showed differences in the OR for HCV by geographical region, with lower estimates in southern Europe. Conclusion In univariable analysis, a history of incarceration was associated with positive HIV and HCV serostatus among PWID in Europe. Applying the precautionary principle would suggest finding alternatives to incarceration of PWID and strengthening health and social services in prison and after release (‘throughcare’).

Wydane we wrześniu 2020 roku Zrozumieć COVID-19 miało na celu podsumowanie przebiegu pierwszych miesięcy pandemii i przygotowanie społeczeństwa w Polsce do ciężkiego sezonu jesienno-zimowego 2020/2021. Niniejsze opracowanie powstaje po dwóch latach trwania pandemii, która rozpoczęła się w Wuhan w grudniu 2019 roku. 36. Risk of COVID-19 epidemic resurgence with the introduction of vaccination passes

Abstract

Many countries hit by the COVID-19 epidemic consider the introduction of vaccination passes. So far, no thorough impact assessment of vaccination passes and of lower restrictions for their holders has been conducted. Here, we propose the VAP-SIRS model that accounts for susceptible, infected, and recovered subpopulations, also within the group of vaccinated pass holders. The model accounts for imperfect vaccination effectiveness, revaccinations and waning immunity. Different restrictions for pass holders and the rest of the population result in different scenarios of the epidemic evolution, some of which yield unfavourable COVID-19 dynamics and new waves. We identify critical variables that should be considered by policymakers and show how unfavourable outcomes can be avoided using adaptive policies. In particular, while pass holders could initially be allowed large freedoms, the gradual loss of immunity will require either increased restrictions for pass holders, or accelerated revaccination. In the long-term, common restrictions for both the pass holders and the rest of the population will have to be kept to avoid epidemic resurgence. Such minimum required restrictions depend on vaccination effectiveness, revaccination rate, waning rate and fraction of never-vaccinated population, and, for realistic combinations of these parameters, range between 29% and 69% reduction of contacts.

Abstract

COVID-19 pandemic had a major impact on the healthcare system globally. This work aims to evaluate COVID-19 impact on local treatment in bone sarcoma treated in a single, high-throughput institution. We have analyzed the local outcomes (i.e., possibility of limb sparing surgery) in all bone sarcoma patients treated between January 2016 and November 2022 in the main sarcoma reference center in Poland. Patients treated in the 2016–2019 period were regarded as "pre-pandemic" group, patients treated in 2020–2022 - "pandemic". Mann–Whitney U and Chi-square tests were used in the statistical analysis. No correction for multiple testing was applied. Tests with p < 0.05 were deemed significant. There were 302 eligible patients identified. The group characteristics are presented in table 1. There were no differences in patient-related variables and histological subtypes of tumors between two groups. The tumor size did not differ (p = 0.053), when all tumor grades were considered, but high grade tumors were larger in the "pandemic" group (p = 0.034). This was reflected in the percentage of limb sparing surgeries which dropped from 83.3 % to 68.2 % ("pre-pandemic" vs. "pandemic", p = 0.004). This difference was even more stark in case of high grade tumors - 78 % vs. 54 % respectively (p = 0.001). To our knowledge, this is the first report of the long lasting impact of COVID-19 pandemic on oncologic treatment outcomes in patients with malignant bone tumors. [ABSTRACT FROM AUTHOR]

Abstract

Supplemental Digital Content is available in the text, Objective: HIV cohorts are an important source of clinical data for informing public health policies and programmes. However, the generalizability of cohort findings to the wider population of people diagnosed with HIV in each country remains unclear. In this work, we assessed the representativeness of six large national HIV cohorts within Europe. Design and methods: Individual-level cohort data were provided from national cohorts in France, Germany, Greece, Italy, Spain and the United Kingdom. Analysis focused on new HIV diagnoses reported to The European Surveillance System (TESSy) during three time periods (2000–2004, 2005–2009 and 2010–2013), to allow for temporal changes. Cohort and TESSy records were matched and compared by age, sex, transmission mode, region of origin and CD4+ cell count at diagnosis. The probability of being included in each cohort given demographic characteristics was estimated and used to generate weights inversely proportional to the probability of being included. Results: Participating cohorts were generally representative of the national HIV-diagnosed population submitted to TESSy. However, people who inject drugs, those born in a country other than that reporting the data, those with low CD4+ cell counts at diagnosis, and those more than 55 years were generally underrepresented in the cohorts examined. Conclusion: These European cohorts capture a representative sample of the HIV-diagnosed populations in each country; however some groups may be underrepresented.
