Background: Contrast-induced encephalopathy (CIE) is a rare adverse event linked to intravascular use of iodine-containing contrast media. The prevalence of CIE could increase in the future due to growing numbers of endovascular procedures. We provide insights from a case series of 7 patients. Methods: Cases from 3 centers were collected based on existing academic collaborations, and key factors were extracted to illustrate development and management of CIE. Results: In our retrospective case-series analysis of 7 cases from 3 countries, affected patients had an equal distribution of sex (4 women, 3 men) and a median age of 75 (IQR 63-77). Common risk factors included hypertension (5/7), hyperlipidemia (5/7), previous stroke (3/7), and type 2 diabetes (3/7). CIE developed in 3 cases after endovascular thrombectomy (EVT) for stroke, in 2 cases after aneurysm treatment, in 1 case after cardiac catheterization, and in 1 case after diagnostic computed tomography (CT) angiography without an endovascular procedure. The median procedure time was 48 min (IQR 40-81). All patients received non-ionic, low-osmolar contrast agents with volumes ranging from 100-300 ml. Symptom onset was close to contrast administration, with stroke-like neurological deficits being most common (4/7). Prednisolone was the most frequently used medication to treat the symptoms (4/7). Symptom resolution occurred in 4 out of 7 patients within two to several days, and 1 patient died, but without clear connection to CIE. Conclusion: CIE is a rare and possibly underrecognized condition, but fortunately, with a favorable outcome in most cases.

Background: Given the lack of an ideal endodontic irrigant on the market, this study evaluates the antimicrobial potential of a formulated solution of 2′-fucosyllactose and lacto-N-neotetraose against E. faecalis within infected root canals, and explores any associated impacts related to the duration of irrigation. Methods: 32 single-rooted teeth extracted for periodontal reasons were infected with Enterococcus faecalis, and subsequently subjected to endodontic treatment with two different irrigation systems: sodium hypochlorite or a solution of 2′-fucosyllactose and lacto-N-neotetraose. These samples were then incubated in sterile culture media at 37 °C to observe microbial activity through turbidity. The culture broth of each individual sample was assessed as positive or negative by observing the turbidity or lack of turbidity in the culture at the time of evaluation. Results: the analysis of the results obtained from the comparison of groups irrigated with sodium hypochlorite or a solution of 2′-fucosyllactose and lacto-N-neotetraose demonstrates that the case solution has no bactericidal effect against E. faecalis inoculated in the endodontic system. Conclusions: the HMOs used in this study do not have a bactericidal effect on E. faecalis inoculated in an endodontic system.

Abstract There is a significant gap in cost-effective quantitative phase microscopy (QPM) systems for studying dynamic cellular processes while maintaining accuracy for long-term cellular monitoring. Current QPM systems often rely on complex and expensive voltage-controllable components like Spatial Light Modulators or two-beam interferometry. To address this, we introduce a QPM system optimized for time-varying phase samples using azobenzene liquid crystal as a Zernike filter with a polarization-sensing camera. This system operates without input voltage or moving components, reducing complexity and cost. Optimized for gentle illumination to minimize phototoxicity, it achieves a 1 Hz frame rate for prolonged monitoring. The system demonstrated accuracy with a maximum standard deviation of ±42 nm and low noise fluctuations of ±2.5 nm. Designed for simplicity and single-shot operations, our QPM system is efficient, robust, and precisely calibrated for reliable measurements. Using inexpensive optical components, it offers an economical solution for long-term, noninvasive biological monitoring and research applications.

Abstract Salivary gland squamous cell carcinomas (SG-SCCs) constitute a rare type of head and neck cancer which is linked to poor prognosis. Due to their low frequency, the molecular mechanisms responsible for their aggressiveness are poorly understood. In this work we studied the role of the phosphatase DUSP1, a negative regulator of MAPK activity, in controlling SG-SCC progression. We generated DUSP1 KO clones in A253 human cells. These clones showed a reduced ability to grow in 2D, self-renew in ECM matrices and to form tumors in immunodeficient mice. This was caused by an overactivation of the stress and apoptosis kinase JNK1/2 in DUSP1 −/+ clones. Interestingly, RNAseq analysis revealed that the expression of SOX2, a well-known self-renewal gene was decreased at the mRNA and protein levels in DUSP1 −/+ cells. Unexpectedly, CRISPR-KO of SOX2 did not recapitulate DUSP1 −/+ phenotype, and SOX2-null cells had an enhanced ability to self-renew and to form tumors in mice. Gene expression analysis demonstrated that SOX2-null cells have a decreased squamous differentiation profile -losing TP63 expression- and an increased migratory phenotype, with an enhanced epithelial to mesenchymal transition signature. In summary, our data indicates that DUSP1 and SOX2 have opposite functions in SG-SCC, being DUSP1 necessary for tumor growth and SOX2 dispensable showing a tumor suppressor function. Our data suggest that the combined expression of SOX2 and DUSP1 could be a useful biomarker to predict progression in patients with SG-SCCs.

Key Clinical Message Recommendations advise factor IX desensitization before immune tolerance induction in severe hemophilia B, supported by immunosuppression. A child with inhibitor and anaphylaxis to factor IX showed successful immunosuppression‐free immune tolerance induction using very low and slowly increasing doses of a factor IX extended‐half‐life product. Immune tolerance to factor IX based on this protocol merits further study.

Molecular and cellular characterization of tumors is essential due to the complex and heterogeneous nature of cancer. In recent decades, many bioinformatic tools and experimental techniques have been developed to achieve personalized characterization of tumors. However, sample handling continues to be a major challenge as limitations such as prior treatments before sample acquisition, the amount of tissue obtained, transportation, or the inability to process fresh samples pose a hurdle for experimental strategies that require viable cell suspensions. Here, we present an optimized protocol that allows the recovery of highly viable cell suspensions from breast cancer primary tumor biopsies. Using these cell suspensions we have successfully characterized genome architecture through Hi-C. Also, we have evaluated single-cell gene expression and the tumor cellular microenvironment through single-cell RNAseq. Both technologies are key in the detailed and personalized molecular characterization of tumor samples. The protocol described here is a cost-effective alternative to obtain viable cell suspensions from biopsies simply and efficiently.

Stomata are pores surrounded by a pair of specialized cells, called guard cells, that play a central role in plant physiology through the regulation of gas exchange between plants and the environment. Guard cells have features like cell-autonomous responses and easily measurable readouts that have turned them into a model system to study signal transduction mechanisms in plants. Here, we provide a detailed protocol to analyze different physiological responses specifically in guard cells. We describe, in detail, the steps and conditions to isolate epidermal peels with tweezers and to analyze i) stomatal aperture in response to different stimuli, ii) cytosolic parameters such as hydrogen peroxide (H2O2), glutathione redox potential (EGSH), and MgATP-2 in vivo dynamics using fluorescent biosensors, and iii) gene expression in guard cell–enriched samples. The importance of this protocol lies in the fact that most living cells on epidermal peels are guard cells, enabling the preparation of guard cell–enriched samples.

Introduction and Objectives: Patients with metabolic dysfunction-associated steatotic liver disease (MASLD) are at an increased cardiovascular risk. On the contrary, non-alcoholic fatty liver disease (NAFLD) is highly prevalent in patients with coronary heart disease (CHD). However, it is not known whether patients with significant CHD show a higher frequency of liver fibrosis. This study aimed to determine the frequency of MASLD and liver fibrosis in patients with CHD and to assess whether coronary stenosis is significantly associated with MASLD and fibrosis. Patients and Methods: This observational and analytical study included adult patients without any known liver disease who underwent coronary angiography for suspected coronary artery disease (Jul 2021–Jul 2022). The presence of significant CHD (> 50% stenosis of at least one coronary artery) was determined. Liver elastography (FibroScan®) was performed up to 6 months after the coronary angiographic study to determine liver fibrosis, a measurement of liver stiffness (> 6.5 Kpa). Fisher's test, Mann–Whitney U test, and logistic regression models were used (p < 0.05). Results: The study included 113 patients (76% men, average age: 63 years [standard deviation: 9.9]), of which 72% presented with significant CHD. The prevalence rate of MASLD was 52%. Liver fibrosis was present in 12% of the patients and all patients in the significant CHD group (p = 0.007). An increase in the number of vessels with significant CHD increased the probability of liver fibrosis (odds ratio, 1.79; 95% confidence interval, 1.06–3.04; p = 0.029). Conclusions: MASLD is highly prevalent in patients with significant CHD but without known liver damage. These data suggest that MASLD and liver fibrosis should be investigated in patients with CHD. The presence of confounding variables, especially the presence of type 2 diabetes mellitus, should be evaluated in further studies.

Although the centrality of the works of Giovanni Botero in the political literature of the first half of the seventeenth century is widely recognized, the role that the former Jesuit had in the economic thought of the Catholic Counter-Reformation has not yet been sufficiently analyzed. In fact, Botero’s works reveal that the existence of a mentality favorable to the development of productive and commercial activities was present and widespread even in the post-Tridentine period. In particular the theses expressed in Antonio Serra’s Breve trattato reveals clear affinity with Botero’s thought and in the same period in Spain the literature focused on similar themes in an attempt to counteract the decadence of the Iberian society. This article shows that the analyses by Botero and Serra are complementary and fully consistent with the spirit of the Counter-Reformation, offering a different historiographical perspective on the relations between Catholicism and the genesis of capitalist economy.

Abstract Cisplatin-based chemotherapy has associated clinical disadvantages, such as high toxicity and resistance. Thus, the development of new antitumor metallodrugs able to overcome different clinical barriers is a public healthcare priority. Here, we studied the mechanism of action of the isomers trans and cis-[PtI2(isopropylamine)2] (I5 and I6, respectively) against gastrointestinal cancer cells. We demonstrate that I5 and I6 modulate mitochondrial metabolism, decreasing OXPHOS activity and negatively affecting ATP-linked oxygen consumption rate. Consequently, I5 and I6 generated Reactive Oxygen Species (ROS), provoking oxidative damage and eventually the induction of senescence. Thus, herein we propose a loop with three interconnected processes modulated by these iodido agents: (i) mitochondrial dysfunction and metabolic disruptions; (ii) ROS generation and oxidative damage; and (iii) cellular senescence. Functionally, I5 reduces cancer cell clonogenicity and tumor growth in a pancreatic xenograft model without systemic toxicity, highlighting a potential anticancer complex that warrants additional pre-clinical studies.

Abstract Background An observational study involving patients recovered from COVID-19 was conducted in order to evaluate the presence/absence of vein wall thickness increasing, according to the severity of pulmonary involvement quantified with a CT-scoring system. Methods The venous wall thickness (VWT) of 31 patients (23 males and 8 females) with COVID 19 previously admitted to Federico II University Hospital of Naples was evaluated through ultrasound measurement of the common femoral Vein 1 cm proximal to the saphenous-femoral junction and the popliteal Vein 1 cm distal to the confluence of gemellary veins. Measurements were taken with an automated tool to avoid human error. All patients were evaluated in the supine position. Patients were then stratified into two groups, VWT > 1 mm and VWT 1 mm group was 7.4 (S.D. 4.83), whilst the mean value of the COVID score in the VWT

The gut microbiota (GM) plays a key role in health by influencing several physiological functions, including the digestive process, the immune system, vitamin production, and mental health. Dysbiosis in gut microbial composition and function has been linked to systemic inflammatory response and the development of metabolic diseases, including diabetes mellitus (DM). ''Leaky gut'' resulting from dysbiosis causes endotoxemia, leading to systemic inflammation and insulin resistance, which are pathogenetic agents of type 2 and gestational DM. Moreover, in children, gut dysbiosis has been associated with the immunitary dysregulation with increased risk of autoimmunity and development of type 1 DM. However, dietary changes in the GM and their role in DM are poorly understood. Plant-based diets that are low in fat and high in fiber have been associated with beneficial effects on the GM. Clinical trials of prebiotics and probiotics have shown promising, albeit mixed, results. This narrative review summarizes recent findings on the relationship between the GM, diet, and DM, focusing on the systems in which the microbiota is involved in the pathogenesis of this disease and its potential use as a therapy. In addition, we discuss immune dysfunction associated with gut dysbiosis and its role in type 1, type 2, and gestational DM. Further research is needed to evaluate the GM as a potential therapeutic target for the prevention and treatment of DM.

Introduction: Studies focused on dermoscopic aspects of pigmented Bowen disease (pBD) in Latin American population are scarce and limited to only case reports or small series. Objectives: To report dermoscopic findings in a large series of 147 pBD diagnosed in Ibero-Latin American population. Methods: We conducted a multicentric, retrospective study on 147 histologically proven pBD under the auspices of the Dermoscopy Chapter of the Ibero-Latin American College of Dermatology. Results: The study population consisted of 77 females (52%) and 70 males (48%) with a mean age of 68.6 years. 70.1% of patients had skin phototype 3, 15.6% to skin phototype 2, and 14.3% to skin phototype 4. On clinical examination, near 60% of pBD were flat, 70% presented with scales, and 90% were asymmetric. Under dermoscopy, structureless hypopigmented areas, dots brown and pink color were the most frequently observed. Regarding specific dermoscopic clues to pBD, the most prevalent were structureless hypopigmented areas, vessels arranged in linear fashion at the periphery, and pigmented lines or pigmented dots distributed in a linear fashion. Clustered, coiled, and dotted vessels were observed in 55.8%, 45.6%, and 45.6% of the cases, respectively. Conclusions: We report a large series of cases of pBD in Latin American patients, with most patients being skin phototype 3 and 4. Distinctively in our study, the pigmented structures and the clues derived from the presence of melanin were much more frequent than in previous reports in fair skin.

Severe non-infectious or non-haemorrhagic brain edema surrounding the electrode represents a rare complication of subthalamic nucleus deep brain stimulation (STN-DBS) surgery. The aim of this study is to report three patients with advanced Parkinson’s Disease (PD) who developed symptomatic brain edema after STN-DBS surgery treated with intravenous steroids with a specific profile of reversible cognitive alterations. Patients were both assessed with a comprehensive neuropsychological battery including attention, memory, visuo-spatial and executive tasks. They were also briefly assessed for emotional and behavioural alterations, and for possible limitations in the activities of daily living. Normative data for an Italian population were available for all neuropsychological tests. The patients were firstly assessed before the surgery (baseline) as soon as they became symptomatic for the post-surgery edema and a few more times in follow-up up to ten months. In all patients we observed the resolution of cognitive deficits within six months after surgery with the corresponding reabsorption of edema at brain CT scans. The appearance of post-DBS edema is a fairly frequent and clinically benign event. However, in some rare cases it can be very marked and lead to important clinical—albeit transient—disturbances. These events can compromise, at least from a psychological point of view, the delicate path of patients who undergo DBS and can prolong the post-operative hospital stay. In this setting it could be helpful to perform a brain CT scan in 2–3 days with the aim of detecting the early appearance of edema and treating it before it can constitute a relevant clinical problem.

This paper analyzes the safeguarding process, started in the 1960s, of the first Italian Element inscribed in the UNESCO’s ICH 2003 List, the Opera dei pupi, which became a Masterpiece of Humanity in 2001. This participatory process combines museographic activity, field research and the staging of shows, both traditional and innovative. It was promoted by the Antonio Pasqualino Puppet Museum of Palermo, which is a museum of performance, a museum-narration, a place of ideas and not of things, which goes beyond the simple collection and becomes a meta-story, an “open work”.

The tribological properties of steels used to realise railway wheels play a fundamental role in the performances of both vehicle and infrastructure. In particular, the wear process, caused by the wheel–rail interaction, modifies the shape of wheel and rail profiles, changing the performances of the vehicle. For this reason, research institutes and vehicle manufacturers have worked hard to develop predictive tools able to estimate the evolution of the wheel and rail profiles. The efficiency of these tools is strongly influenced by the tribological properties of the materials, i.e., the wear coefficients, which are used as input data. The characterisation of these properties requires specific tools and long-lasting experimental campaigns, which are usually performed under controlled operating conditions, using twin-disc test benches. These devices usually do not consider the real contact conditions in terms of normal load, contact geometry, and slip velocity, since they are equipped with small-size rollers. The paper proposes an innovative 1:5 scaled twin-disc, which allows the reproduction of the real wheel–rail contact conditions, thanks to Pascal’s scaling technique. The testing device allows the reproduction of a wide range of typical operating conditions of railway vehicles, thanks to high-power independent brushless motors, used to actuate the rollers, and an innovative loading system.

Spontaneous convexity subarachnoid hemorrhage (cSAH) is a vascular disease different from aneurysmal SAH in neuroimaging pattern, causes, and prognosis. Several causes might be considered in individual patients, with a limited value of the patient’s age for discriminating among these causes. Cerebral amyloid angiopathy (CAA) is the most prevalent cause in people > 60 years, but reversible cerebral vasoconstriction syndrome (RCVS) has to be considered in young people. CAA gained attention in the last years, but the most known manifestation of cSAH in this context is constituted by transient focal neurological episodes (TFNEs). CAA might have an inflammatory side (CAA-related inflammation), whose diagnosis is relevant due to the efficacy of immunosuppression in resolving essudation. Other causes are hemodynamic stenosis or occlusion in extracranial and intracranial arteries, infective endocarditis (with or without intracranial infectious aneurysms), primary central nervous system angiitis, cerebral venous thrombosis, and rarer diseases. The diagnostic work-up is fundamental for an etiological diagnosis and includes neuroimaging techniques, nuclear medicine techniques, and lumbar puncture. The correct diagnosis is the first step for choosing the most effective and appropriate treatment.

This study evaluated the postharvest evolution in terms of quality and functional traits of recently introduced mini plum tomato cultivars, differing for fruit pigmentation. The cultivars Santy Naranja (orange fruit) and Melange (dark red, green-striped fruit), along with the well-established ‘Angelle’ (red fruit), were monitored during refrigerated storage (10 °C) for 0, 7, and 14 days (hereafter S0, S7, and S14, respectively) inside closed polylactic acid trays. ‘Melange’ displayed the highest fruit fresh weight loss and total soluble solids (TSS) (9.28 °Bx), along with the lowest firmness (4.69 N), whereas ‘Santy Naranja’ showed the strongest color deviation during the storage period. Titratable acidity (TA) was relatively stable in ‘Melange’, while ‘Angelle’ experienced the highest decrease between S7 and S14, resulting in stronger TSS/TA fluctuations. Regarding the functional traits, ‘Melange’ had the highest total polyphenols content (TPC), whereas ‘Angelle’ excelled in total carotenoid content (TCC), ascorbic acid content (AsA), and fruits’ antioxidant activity, as revealed by the 2,2-diphenyl-1-picrylhydrazyl (DPPH) and ferric reducing antioxidant power (FRAP) assays. With reference to storage time, the O2 concentration in the headspace, TPC, AsA, and DPPH decreased between S0 and S14, whereas CO2 concentration, TCC, and FRAP increased, but these variations were mostly cultivar-dependent. This study provides insights into the postharvest behavior of novel mini plum tomato typologies and their ability to maintain quality over time, providing information for developing strategies to preserve their sensory and nutraceutical characteristics during refrigerated storage.

In the contemporary landscape of construction project management, the strategic integration of Building Information Modeling (BIM) methodology has emerged as a fundamental avenue to improve operational efficiency. This study evaluates the feasibility and effectiveness of the implementation of the BIM methodology in improving the management of construction projects in construction companies in Peru. Using an applied approach underscored by a quantitative and correlational non-experimental design, a comprehensive survey was administered to 415 workers. This survey focused on evaluating the dimensions of BIM (3D, 4D, and 5D), as well as the fundamental facets of project management that encompass planning and execution. The results reveal that 47% of respondents highlighted deficiencies within the 3D BIM dimension, indicating potential challenges in visualizing and understanding the design. Likewise, 49% of participants highlighted deficiencies within the 4D BIM dimension, shedding light on issues related to time coordination and strategic planning. Around 57% expressed reservations about the 5D BIM dimension, highlighting the complexities surrounding accurate cost estimation and financial management. The synthesis of these revelations underscores an urgent call for specific interventions to rectify these specific areas. This involves investing in cutting-edge BIM technology tools and the simultaneous provision of targeted training programs for stakeholders to increase their multi-dimensional view of BIM. Ultimately, the study results drive innovation, thus opening the prospect of a more sustainable and streamlined construction industry.

Addiction-related attentional biases may play a central role in the development and maintenance of drug-seeking and drug-taking behaviors. However, evidence in cocaine dependence is limited and mixed. This study examined the time course and component processes of attentional biases for cocaine-related cues in a sample of 47 outpatients (38 men) with cocaine use disorder (CUD) with varying durations of current abstinence. Reaction times in a visual dot-probe task with two picture exposure durations —500 ms, to assess initial stages of attention, and 2,000 ms, to assess maintained attention— were recorded. We found faster responses to probes replacing cocaine-related vs. matched control pictures in the 500 ms but not in the 2,000 ms condition, indicative of early but not late attentional biases for cocaine cues in abstinent patients with CUD. Further comparisons with a neutral baseline revealed that it was not due to rapid orienting but to delayed disengagement from cocaine-related pictures, being this effect greater the longer the period of current abstinence. Consistent with the incentive-sensitization theory, these data suggest that cocaine-related stimuli maintain the capacity to hold spatial attention in abstinent patients with CUD, even after months of abstinence, highlighting the relevance of carrying out stimulus control to avoid relapses.

Abstract Background Metabolic syndrome (MetS) has become a growing risk factor of some non-communicable diseases. Increase of greenhouse gas emissions affects the planet. Aims To assess the association between MetS severity and amount of carbon dioxide (CO2) emitted in an adult population. Design Cross-sectional study (n = 6646; 55-76-year-old-men; 60-75-year-old-women with MetS). Methods Dietary habits were assessed using a pre-validated semi quantitative 143-item food frequency questionnaire. The amount of CO2 emitted due to the production of food consumed by person and day was calculated using a European database, and the severity of the MetS was calculated with the MetS Severity Score. Results Higher glycaemia levels were found in people with higher CO2 emissions. The risk of having high severe MetS was related to high CO2 emissions. Conclusions Low CO2 emissions diet would help to reduce MetS severity. Advantages for both health and the environment were found following a more sustainable diet. Anderson–Fabry disease (AFD) is a genetic sphingolipidosis involving virtually the entire body. Among its manifestation, the involvement of the central and peripheral nervous system is frequent. In recent decades, it has become evident that, besides cerebrovascular damage, a pure neuronal phenotype of AFD exists in the central nervous system, which is supported by clinical, pathological, and neuroimaging data. This neurodegenerative phenotype is often clinically characterized by an extrapyramidal component similar to the one seen in prodromal Parkinson’s disease (PD). We analyzed the biological, clinical pathological, and neuroimaging data supporting this phenotype recently proposed in the literature. Moreover, we compared the neurodegenerative PD phenotype of AFD with a classical monogenic vascular disease responsible for vascular parkinsonism and cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL). A substantial difference in the clinical and neuroimaging features of neurodegenerative and vascular parkinsonism phenotypes emerged, with AFD being potentially responsible for both forms of the extrapyramidal involvement, and CADASIL mainly associated with the vascular subtype. The available studies share some limitations regarding both patients’ information and neurological and genetic investigations. Further studies are needed to clarify the potential association between AFD and extrapyramidal manifestations.

Antiphospholipid syndrome (APS) is an autoimmune prothrombotic condition characterized by venous thromboembolism, arterial thrombosis, and pregnancy morbidity. Among neurological manifestations, arterial thrombosis is only one of the possible associated clinical and neuroradiological features. The aim of this review is to address from a neurovascular point of view the multifaceted range of the arterial side of APS. A modern neurovascular approach was proposed, dividing the CNS involvement on the basis of the size of affected arteries, from large to small arteries, and corresponding clinical and neuroradiological issues. Both large-vessel and small-vessel involvement in APS were detailed, highlighting the limitations of the available literature in the attempt to derive some pathomechanisms. APS is a complex disease, and its neurological involvement appears multifaceted and not yet fully characterized, within and outside the diagnostic criteria. The involvement of intracranial large and small vessels appears poorly characterized, and the overlapping with the previously proposed inflammatory manifestations is consistent.

The ring finger protein 213 gene (RNF213) is involved in several vascular diseases, both intracranial and systemic ones. Some variants are common in the Asian population and are reported as a risk factor for moyamoya disease, intracranial stenosis and intracranial aneurysms. Among intracranial vascular diseases, both moyamoya disease and intracranial artery dissection are more prevalent in the Asian population. We performed a systematic review of the literature, aiming to assess the rate of RNF213 variants in patients with spontaneous intracranial dissections. Four papers were identified, providing data on 53 patients with intracranial artery dissection. The rate of RNF213 variants is 10/53 (18.9%) and it increases to 10/29 (34.5%), excluding patients with vertebral artery dissection. All patients had the RNF213 p.Arg4810Lys variant. RNF213 variants seems to be involved in intracranial dissections in Asian cohorts. The small number of patients, the inclusion of only patients of Asian descent and the small but non-negligible coexistence with moyamoya disease familiarity might be limiting factors, requiring further studies to confirm these preliminary findings and the embryological interpretation.

The present investigation was conducted to explore the role of potassium nutrition in improving biomass and physio-chemical alterations to reduce the adverse effects of salinity in mungbean. A sand-culture experiment was carried out under different salinity levels (0, 50, and 100 mM NaCl) with two levels of potassium (0 and 50 mM K2SO4) and two mungbean cultivars (NM-92 and Ramzan), and the alterations in mungbean biomass and metabolic activities were investigated. The results suggested that salinity significantly reduced the biomass, nitrate reductase activity (NRA), nitrite reductase activity (NiRA), total soluble proteins, chlorophyll a, chlorophyll b, total chlorophyll, nitrogen, potassium, calcium, magnesium, and phosphorous contents in both mungbean cultivars in comparison to the control. However, K2SO4 at 50 mM significantly improved all the parameters in both mungbean cultivars except for the sodium content. A significant increase in the total free amino acids, carotenoids, and sodium content in both mungbean cultivars was observed due to salt stress. Moreover, principal component analysis and heatmaps were developed individually for both mungbean cultivars to assess the variability and correlation among the studied attributes under all applied treatments. Under saline conditions, the mungbean ‘Ramzan’ showed more marked reductions in almost all the growth parameters as compared to ‘NM-92’. The results suggest that the application of K2SO4 ameliorates the adverse effects of salinity by regulating osmolyte production, NRA, and NiRA, thus promoting plant growth and productivity.

Background and Purpose This study aimed to investigate the effect of endovascular treatment (EVT, with or without intravenous thrombolysis [IVT]) versus IVT alone on outcomes in patients with acute ischemic stroke (AIS) and intracranial large vessel occlusion (LVO) attributable to cervical artery dissection (CeAD). Methods This multinational cohort study was conducted based on prospectively collected data from the EVA-TRISP (EndoVAscular treatment and ThRombolysis for Ischemic Stroke Patients) collaboration. Consecutive patients (2015–2019) with AIS-LVO attributable to CeAD treated with EVT and/or IVT were included. Primary outcome measures were (1) favorable 3-month outcome (modified Rankin Scale score 0–2) and (2) complete recanalization (thrombolysis in cerebral infarction scale 2b/3). Odds ratios with 95% confidence intervals (OR [95% CI]) from logistic regression models were calculated (unadjusted, adjusted). Secondary analyses were performed in the patients with LVO in the anterior circulation (LVOant) including propensity score matching. Results Among 290 patients, 222 (76.6%) had EVT and 68 (23.4%) IVT alone. En la esfera organizacional, la toma de decisiones es un proceso complejo que suele estar asociado a distintas problemáticas, por ejemplo, el manejo completo y adecuado de información; conflictos de intereses entre partes de la organización; la comunicación inadecuada; la escasa participación de actores claves o interesados en el proceso; la rigidez y falta de flexibilidad, entre otros elementos. El objetivo de este trabajo es determinar la relación existente entre toma de decisiones y rendimiento en universidades peruanas. Se realizó un estudio cuantitativo aplicando un cuestionario a 204 miembros de la universidad, adoptando un enfoque no experimental y transversal, y se aplicaron técnicas de análisis descriptivas e inferenciales, como la media, la desviación estándar, la prueba T de Student y la correlación de Pearson. Los resultados evidencian que existe una relación significativa entre ambas variables. Los principales factores que inciden en el rendimiento organizacional son la participación de docentes y estudiantes en la toma de decisiones, la comunicación efectiva, el clima organizacional y el ambiente laboral.

Precise nitrogen (N) supply is an agronomic practice of crucial importance to achieve optimal crop performance without compromising product quality. However, excessive use of synthetic N fertilizers may have deleterious effects on both agroecosystem and human health. Thus, the development and use of strategies aiming to ameliorate the losses caused by water constraints and N deficiency are essential for fostering resilient and sustainable agroecosystems. In this regard, the impact of three drought stress levels (DS) [100%, 80% and 60% of the field capacity (FC)] in combination with four N supply rates (0, 50, 100 and 150 kg ha−1) on sweet basil cultivated in a protected environment was investigated. The interactive biostimulatory action of Kelpstar® seaweed extract (SWE) and Tyson® protein hydrolysate (PH) was also explored. The study focused on the effects of these treatments on yield, physiological attributes, functional traits, and volatile compounds profile. Drought stress led to a reduction in yield by 12.5% and 21.1% under irrigation at 80% and 60% FC, respectively, compared to well-watered plots (100% FC). Furthermore, drought stress levels linearly decreased total leaf area (-15.4% and -26.2% for DS80 and DS60, respectively), stomatal conductance (-14.2% and 34.1% for DS80 and DS60, respectively), nitrogen use efficiency (NUE) (4.0% and 10.0% for DS80 and DS60, respectively), and volatile compounds, such as trans-2-hexanal, 1-octen-3-ol and α-bergamotene. Conversely, an increase in N application rate positively influenced yield (8.6% and 12.2% for N100 and N150, respectively), total leaf area (22.2% and 16.5% for N100 and N150, respectively), specific leaf area (SLA), total chlorophyll (7.7% for N150), nitrate content, and the presence of specific volatile compounds, such as 1-octen-3-ol and α-bergamotene, when compared to no N application. Seaweed extract application caused an upsurge in yield (+17.5%), stomatal conductance (+25.8%), WP (+13.5%), total chlorophyll (+2.3%), nitrate (+3.4%), phenolics (+14.2), ascorbic acid (+28.2), as well as, 1-octen-3-ol, β-cis-ocimene, linalool and eugenol, compared to the control. Similarly, plant protein hydrolysate increased yield (+16.1), stomatal conductance (+10.4), WP (+13.7), total chlorophyll (+4.3), phenolics (+10.7%), ascorbic acid (+9.7%), β-cis-ocimene and eugenol, compared to the control. Notably, the increased yield, improved quality, and enchanced physiological traits observed after biostimulant application, especially under drought stress or N deficiency conditions, underscore the potential role of biostimulants in increasing resilience of basil plants. Thus, the foliar application of SWE and PH offer a valuable strategy for enhancing plant yield and quality under sub-optimal conditions, while simultaneously enhancing water and N use efficiency.

Improving the fermentation performance of starter strains used in the fermentation of table olives is a biotechnological solution of current interest to improve the quality characteristics of the final product. The aim of this study was to evaluate the use of Lactiplantibacillus pentosus OM13 as a starter culture for the fermentation of Seville-type table olives in two different production areas: Italy and Spain. The starter strain L. pentosus OM13 was inoculated into two different table olive varieties: Nocellara del Belice in Italy and Manzanilla in Spain. Lactiplantibacillus plantarum Vege-Start 60 was used as a commercial control, while an additional control production was carried out by spontaneous fermentation. The industrial productions consisted of three different protocols, differing in the type of nutrient and the presence/absence of acclimatisation of the starter strain. All trials were subjected to microbiological monitoring, evaluation of acidification dynamics and sensory analysis of the final product. After 90 days, the pH reached values below 5 in the different treatments. The LAB reached microbial loads varying between 6.5 and 8.7 log CFU/mL throughout the monitoring period. The microbial populations of spoilage and/or potential pathogenic microorganisms were variable depending on the microbial group monitored. However, after 12 days of fermentation, Enterobacteriaceae showed values below the detection limit. In contrast, a fluctuating trend was observed for yeasts, Pseudomonadaceae and Staphylococcaceae. Sensory analyses showed variable differences depending on the technological protocol used. Table olives obtained with L. pentosus OM13 in the presence of nutrient, activator and acclimatisation period achieved higher overall acceptability values compared to the other trials. The use of adjuvants (nutrients and activators) is a strategy used in the production of table olives fermented with L. pentosus OM13 to improve the sensory characteristics of table olives.

ABSTRACTThis study investigates the impact of priming on the resistance of tomato plants to the potato aphid Macrosiphum euphorbiae, focusing on the role of glandular trichomes. Glandular trichomes are specific hairs that provide protection to tomato plants against herbivorous insects. The experimental priming conducted in this study revealed that prior infestation by Spodoptera littoralis caterpillars increased the plant's resistance against M. euphorbiae, pointing at the jasmonic acid (JA) signaling pathway in regulating this plant-aphid interaction. Glandular trichomes type IV were effective against aphids regardless of the previous infestation. Using JA-deficient tomato (spr2), we observed that M. euphorbiae multiplication increased, while the number of aphids on salicylic -deficient NahG plants was lower than in the wildtype Moneymaker. These findings emphasize the crucial role of the JA signaling pathway in tomato plant resistance to aphids and the importance of glandular trichomes to enhance plant defences against pests.

BackgroundPsoriatic arthritis (PsA) is a chronic inflammatory musculoskeletal disease involving several articular and extra-articular structures. Despite the important progresses recently made in all of the aspects of this disease, its management is still burdened by unresolved issues. The aim of this exercise was to provide a set of statements that may be helpful for the management of PsA.MethodsA group of 38 Italian rheumatologists with recognized expertise in PsA selected and addressed the following four topics: “early PsA,” “axial-PsA,” “extra-articular manifestations and comorbidities,” “therapeutic goals.” Relevant articles from the literature (2016–2022) were selected by the experts based on a PubMed search. A number of statements for each topic were elaborated.ResultsNinety-four articles were selected and evaluated, 68 out of the 1,114 yielded by the literature search and 26 added by the Authors. Each of the four topic was subdivided in themes as follows: transition from psoriasis to PsA, imaging vs. CASPAR criteria in early diagnosis, early treatment for “early PsA”; axial-PsA vs. axialspondyloarthritis, diagnosis, clinical evaluation, treatment, standard radiography vs. magnetic resonance imaging for “axial PsA”; influence of inflammatory bowel disease on the therapeutic choice, cardiovascular comorbidity, bone damage, risk of infection for “comorbidities and extra-articular manifestations”; target and tools, treat-to-target strategy, role of imaging for “therapeutic goals.” The final document consisted of 49 statements.DiscussionThe final product of this exercise is a set of statements concerning the main issues of PsA management offering an expert opinion for some unmet needs of this complex disease.

ObjectiveThis study evaluated short- and long-term efficacy and safety of the second-generation somatostatin receptor ligand pasireotide alone or in combination with dopamine agonist cabergoline in patients with Cushing’s disease (CD).Study designThis is an open-label, multicenter, non-comparative, Phase II study comprising 35-week core phase and an optional extension phase. All patients started with pasireotide, and cabergoline was added if cortisol remained elevated. Eligible patients had active CD, with or without prior surgery, were pasireotide naïve at screening or had discontinued pasireotide for reasons other than safety. Primary endpoint was proportion of patients with a mean urinary free cortisol (mUFC) level not exceeding the upper limit of normal (ULN) at week 35 with missing data imputed using last available post-baseline assessments.ResultsOf 68 patients enrolled, 26 (38.2%) received pasireotide monotherapy and 42 (61.8%) received pasireotide plus cabergoline during the core phase. Thirty-four patients (50.0%; 95% CI 37.6–62.4) achieved the primary endpoint, of whom 17 (50.0%) received pasireotide monotherapy and 17 (50.0%) received combination therapy. Proportion of patients with mUFC control remained stable during the extension phase up to week 99. Treatment with either mono or combination therapy provided sustained improvements in clinical symptoms of hypercortisolism up to week 99. Hyperglycemia and nausea (51.5% each), diarrhea (44.1%) and cholelithiasis (33.8%) were the most frequent adverse events.ConclusionAddition of cabergoline in patients with persistently elevated mUFC on maximum tolerated doses of pasireotide is an effective and well-tolerated long-term strategy for enhancing control of hypercortisolism in some CD patients.Clinical trial registrationhttps://clinicaltrials.gov/ct2/show/NCT01915303, identifier NCT01915303.

Analysis of a series of thermal mappings obtained by UAS flights on quiescent volcanoes requires some special techniques to be performed. The main challenge is represented by the difficulty of separating hot and cold pixels in areas where their temperatures are quite similar. This task is indeed much simpler, for example, for lava flows where the temperature differences between the hot lava and the cold soil is rather big. This paper shows various software developed in order to perform this extraction and calculate the trends over time of both the average temperature and the heat flux from the soil. This prototypal implementation used thermal flights performed over a time span of a few years on an area in the Campi Flegrei caldera in southern Italy. Standard image manipulation techniques were used to segmentate and clusterise each thermal mapping in order to reduce the thermal anomalies to some sets of simpler features characterised by their fundamental parameters. The temporal trends of some physical parameters (temperature, heat flux, etc.) were extracted from these sets, and we found interesting results necessary for correlations and for ongoing research with other parameters.

Rare endocrine conditions present specific diagnostic and manage ment challenges for healthcare providers, one of which is the understudied transition of care. Despite the need for guidance regarding transition, consensus on structured and protocolled approaches is lacking. Therefore, we aimed to map the current clinical practice and identify unmet needs regarding transition of care for patients with pituitary disease in the reference centers (RCs) of the European Reference Network on Rare Endocrine Conditions (Endo-ERN). A survey-based, cross-sectional study using the EU Survey tool was performed and completed by 46 physicians (n) from 30 RCs (N). Transition is a common practice among RCs (n = 44/46), usually accomplished by a multidisciplinary team meeting (N = 20/30). Criteria for start and end of transition were defined in half of the RCs, with 16.7% of centers providing dissimilar answers. Transition readiness was assessed by >75% of the RCs, mostly by unvalidated means (e.g. subjective opinions, informal consultations). Pituitary-specific transition assessment tool was applied in one RC only. Transition protocols were present in only 9% of RCs, while in many RCs, tr ansition decisions were taken in combined adult-pediatric meetings or based on clinicians’ personal judgment. A minority of physicians evaluated the effectiveness of transiti on-related interventions (n = 11/46) or medical outcomes (n = 8/46). Patient-reported outcome measures were infrequently used (n = 4/46). Identified unmet needs included the development of guidelines (n = 5/46) and EU-wide approach (n = 2/46). This study exemplifies the unmet needs for a structural definition of the transition period and t ransition management for patients with rare hypothalamic and pituitary conditions from healthcare providers’ perspective.

Abstract Background The aim of this study was to investigate the prognostic role of hematoma characteristics and hematoma expansion (HE) in patients with spontaneous intracerebral hemorrhage (ICH). Methods This multicenter prospective cohort study enrolled consecutive adult patients with non-traumatic ICH admitted to three Italian academic hospitals (Salerno, Padova, Reggio Emilia) over a 2-year period. Early noncontrast CT (NCCT) features of the hematoma, including markers of HE, and 3-month outcome were recorded. Multivariable logistic regression analysis was performed to identify predictors of poor outcome. Results A total of 682 patients were included in the study [mean age: 73 ± 14 years; 316 (46.3%) females]. Pontine and massive hemorrhage, intraventricular bleeding, baseline hematoma volume > 15 mL, blend sign, swirl sign, margin irregularity ≥ 4, density heterogeneity ≥ 3, hypodensity ≥ 1, island sign, satellite sign, and black hole sign were associated with a higher risk of mortality and disability. However, at multivariate analysis only initial hematoma volume (OR 29.71) proved to be an independent predictor of poor functional outcome at 3 months. Conclusion Simple hematoma volume measured on baseline CT best identifies patients with a worse outcome, while early NCCT markers of HE do not seem to add any clinically significant information.

Abstract Background Research related to sustainable diets is is highly relevant to provide better understanding of the impact of dietary intake on the health and the environment. Aim To assess the association between the adherence to an energy-restricted Mediterranean diet and the amount of CO2 emitted in an older adult population. Design and population Using a cross-sectional design, the association between the adherence to an energy-reduced Mediterranean Diet (erMedDiet) score and dietary CO2 emissions in 6646 participants was assessed. Methods Food intake and adherence to the erMedDiet was assessed using validated food frequency questionnaire and 17-item Mediterranean questionnaire. Sociodemographic characteristics were documented. Environmental impact was calculated through greenhouse gas emissions estimations, specifically CO2 emissions of each participant diet per day, using a European database. Participants were distributed in quartiles according to their estimated CO2 emissions expressed in kg/day: Q1 (≤2.01 kg CO2), Q2 (2.02-2.34 kg CO2), Q3 (2.35-2.79 kg CO2) and Q4 (≥2.80 kg CO2). Results More men than women induced higher dietary levels of CO2 emissions. Participants reporting higher consumption of vegetables, fruits, legumes, nuts, whole cereals, preferring white meat, and having less consumption of red meat were mostly emitting less kg of CO2 through diet. Participants with higher adherence to the Mediterranean Diet showed lower odds for dietary CO2 emissions: Q2 (OR 0.87; 95%CI: 0.76-1.00), Q3 (OR 0.69; 95%CI: 0.69-0.79) and Q4 (OR 0.48; 95%CI: 0.42-0.55) vs Q1 (reference). Conclusions The Mediterranean diet can be environmentally protective since the higher the adherence to the Mediterranean diet, the lower total dietary CO2 emissions. Mediterranean Diet index may be used as a pollution level index.

Secondary neurodegeneration refers to the final result of several simultaneous and sequential mechanisms leading to the loss of substance and function in brain regions connected to the site of a primary injury. Stroke is one of the most frequent primary injuries. Among the subtypes of post-stroke secondary neurodegeneration, axonal degeneration of the corticospinal tract, also known as Wallerian degeneration, is the most known, and it directly impacts motor functions, which is crucial for the motor outcome. The timing of its appearance in imaging studies is usually considered late (over 4 weeks), but some diffusion-based magnetic resonance imaging (MRI) techniques, as diffusion tensor imaging (DTI), might show alterations as early as within 7 days from the stroke. The different sequential pathological stages of secondary neurodegeneration provide an interpretation of the signal changes seen by MRI in accordance with the underlying mechanisms of axonal necrosis and repair. Depending on the employed MRI technique and on the timing of imaging, different rates and thresholds of Wallerian degeneration have been provided in the literature. In fact, three main pathological stages of Wallerian degeneration are recognizable—acute, subacute and chronic—and MRI might show different changes: respectively, hyperintensity on T2-weighted sequences with corresponding diffusion restriction (14–20 days after the injury), followed by transient hypointensity of the tract on T2-weighted sequences, and by hyperintensity and atrophy of the tract on T2-weighted sequences. This is the main reason why this review is focused on MRI signal changes underlying Wallerian degeneration. The identification of secondary neurodegeneration, and in particular Wallerian degeneration, has been proposed as a prognostic indicator for motor outcome after stroke. In this review, the main mechanisms and neuroimaging features of Wallerian degeneration in adults are addressed, focusing on the time and mechanisms of tissue damage underlying the signal changes in MRI.

Alzheimer’s disease (AD) is a prevalent neurodegenerative disorder characterized by cognitive decline and neuropathological hallmarks, including β-amyloid (Aβ) plaques, Tau tangles, synaptic dysfunction and neurodegeneration. Emerging evidence suggests that abnormal iron (Fe) metabolism plays a role in AD pathogenesis, but the precise spatial distribution of the Fe and its transporters, such as ferroportin (FPN), within affected brain regions remains poorly understood. This study investigates the distribution of Fe and FPN in the CA1 region of the human hippocampus in AD patients with a micrometer lateral resolution using laser ablation inductively coupled plasma mass spectrometry (LA-ICP-MS). For this purpose, we visualized and quantified Fe and FPN in three separated CA1 layers: stratum molecular–radial (SMR), stratum pyramidal (SP) and stratum oriens (SO). Additionally, chromogenic immunohistochemistry was used to examine the distribution and colocalization with Tau and Aβ proteins. The results show that Fe accumulation was significantly higher in AD brains, particularly in SMR and SO. However, FPN did not present significantly changes in AD, although it showed a non-uniform distribution across CA1 layers, with elevated levels in SP and SO. Interestingly, minimal overlap was observed between Fe and FPN signals, and none between Fe and areas rich in neurofibrillary tangles (NFTs) or neuritic plaques (NP). In conclusion, the lack of correlation between Fe and FPN signals suggests complex regulatory mechanisms in AD Fe metabolism and deposition. These findings highlight the complexity of Fe dysregulation in AD and its potential role in disease progression.

Primary Angiitis of the Central Nervous System (PACNS) is a rare disease and its diagnosis is a challenge for several reasons, including the lack of specificity of the main findings highlighted in the current diagnostic criteria. Among the neuroimaging pattern of PACNS, a tumefactive form (t-PACNS) is a rare subtype and its differential diagnosis mainly relies on neuroimaging. Tumor-like mass lesions in the brain are a heterogeneous category including tumors (in particular, primary brain tumors such as glial tumors and lymphoma), inflammatory (e.g., t-PACNS, tumefactive demyelinating lesions, and neurosarcoidosis), and infectious diseases (e.g., neurotoxoplasmosis). In this review, the main features of t-PACNS are addressed and the main differential diagnoses from a neuroimaging perspective (mainly Magnetic Resonance Imaging—MRI—techniques) are described, including conventional and advanced MRI.

A toxicogenomic approach was used for toxicity evaluation of arsenic in the aquatic environment, and differential gene expression was investigated from 24 h and 96 h water-only acute toxicity tests with the aquatic oligochaete, Tubifex tubifex (Annelida, Clitellata). Several toxicological endpoints (survival and autotomy) of the oligochaete and tissue residues were measured, and dose-response modelling of gene expression data was studied. A reference transcriptome of the aquatic oligochaete, T. tubifex, was reconstructed for the first time, and genes related to cell stress response (Hsc70, Hsp10, Hsp60, and Hsp83), energy metabolism (COX1), oxidative stress (Cat, GSR, and MnSOD), and the genes involved in the homeostasis of organisms (CaM, RpS13, and UBE2) were identified and characterised. The potential use of the genes identified for risk assessment in freshwater ecosystems as early biomarkers of arsenic toxicity is discussed.

(1) Background: Non-stenotic complicated plaques are a neglected cause of stroke, in particular in young patients. Atherosclerosis has some preferential sites in extracranial arteries and the prepetrous segment of the internal carotid artery has been rarely described as site of atheroma in general and of complicated atheroma in stroke patients. The aim of this study is to describe the rate of the prepetrous internal carotid artery’s (ICA) involvement in a single-center case series of young stroke patients. (2) Methods: All patients < 50 years old with acute ischemic stroke admitted to a single-center Stroke Unit during two time periods (the first one from 1 January 2018 to 31 December 2019, and the second one from 1 January 2021 to 30 June 2022), were prospectively investigated as part of a screening protocol of the Searching for Explanations for Cryptogenic Stroke in the Young: Revealing the Etiology, Triggers, and Outcome (SECRETO) study [ClinicalTrials.gov ID NCT01934725], including extracranial vascular examination by using computed tomography (CT) or magnetic resonance imaging (MRI). (3) Results: Two out of ninety-three consecutive patients (2.15%) had a complicated atheroma in the prepetrous ICA as the cause of stroke and both CT angiography and high-resolution vessel wall MRI were applied to document the main features of positive remodeling, cap rupture, ulceration, intraplaque hemorrhage, and a transient thrombus superimposed on the atheroma. The two patients had a different evolution of healing in the first case and a persisting ulceration at 12 months in the second case. (4) Conclusions: The prepetrous ICA is a rarely described location of complicated atheroma in stroke patients at all ages and it represents roughly 2% of causes of acute stroke in this single-center case series in young people.

Over the past decade, multiple studies have suggested that the secondary metabolites produced by plants against herbivorous insects could be used as biopesticides. However, as the molecular mechanism of action of these compounds remains unknown, it is difficult to predict how they would affect non-target insects; thus, their innocuity needs to be clarified. Here, we investigate, from the molecular level to the organism, the responses of a useful parasitic insect Nasonia vitripennis (Walker, 1836) being exposed at the pupae stage for 48 h (up to 6 days) to sublethal doses (5 µg/L and 500 µg/L) of 2-Dodecanone. 2-Dodecanone altered the gene expression of genes related to ecdysone-related pathways, biotransformation, and cell homeostasis. A significant induction of ecdysone response-genes (EcR, usp, E78, Hr4, Hr38) was detected, despite no significant differences in ecdysteroid levels. Regarding the cell homeostasis processes, the gene l(2)efl was differentially altered in both experimental conditions, and a dose-dependent induction of hex81 was observed. 2-Dodecanone also triggered an induction of Cyp6aQ5 activity. Finally, 2-Dodecanone exposure had a significant effect on neither development time, energy reserves, nor egg-laying capacity; no potential genotoxicity was detected. For the first time, this study shows evidence that 2-Dodecanone can modulate gene expression and interfere with the ecdysone signalling pathway in N. vitripennis. This could lead to potential endocrine alterations and highlight the suitability of this organism to improve our general understanding of the molecular effects of plant defences in insects. Our findings provide new insights into the toxicity of 2-Dodecanone that could potentially be explored in other species and under field conditions for plant protection and pest management as a means to reduce reliance on synthetic pesticides.

Primary Angiitis of the Central Nervous System (PACNS) is a rare cerebrovascular disease involving the arteries of the leptomeninges, brain and spinal cord. Its diagnosis can be challenging, and the current diagnostic criteria show several limitations. Among the clinical and neuroimaging manifestations of PACNS, intracranial bleeding, particularly intracerebral hemorrhage (ICH), is poorly described in the available literature, and it is considered infrequent. This review aims to summarize the available data addressing this issue with a dedicated focus on the clinical, neuroradiological and neuropathological perspectives. Moreover, the limitations of the actual data and the unanswered questions about hemorrhagic PACNS are addressed from a double point of view (PACNS subtyping and ICH etiology). Fewer than 20% of patients diagnosed as PACNS had an ICH during the course of the disease, and in cases where ICH was reported, it usually did not occur at presentation. As trigger factors, both sympathomimetic drugs and illicit drugs have been proposed, under the hypothesis of an inflammatory response due to vasoconstriction in the distal cerebral arteries. Most neuroradiological descriptions documented a lobar location, and both the large-vessel PACNS (LV-PACNS) and small-vessel PACNS (SV-PACNS) subtypes might be the underlying associated phenotypes. Surprisingly, amyloid beta deposition was not associated with ICH when histopathology was available. Moreover, PACNS is not explicitly included in the etiological classification of spontaneous ICH. This issue has received little attention in the past, and it could be addressed in future prospective studies.

Home parenteral nutrition (HPN) is increasingly prescribed for patients with advanced cancer. This therapy improves free-fat mass, quality of life and survival, but it is not free from complications, especially catheter-related bloodstream infections (CRBSIs). The use of commercial multichamber bags in HPN has not been extensively explored in oncologic patients and their association with complications is not well known. In this prospective cohort study, we included 130 patients with advanced cancer and HPN. We compared the effects of individual compounded bags (n = 87) vs. commercial multichamber bags (n = 43) on complications. There were no differences in any complication, including thrombosis (p > 0.05). There were 0.28 episodes of CRBSI per 1000 catheter days in the individual compounded bag group and 0.21 in the multichamber bag group (p > 0.05). A total of 34 patients were weaned off HPN, 22 with individual bags and 12 with multichamber bags (p = 0.749). Regarding survival when on HPN, the group with individual bags showed a median of 98 days (95% CI of 49–147), whereas those with multichamber bags showed a median of 88 days (95% CI of 43–133 (p = 0.913)). In conclusion, commercial multichamber bags for HPN in patients with advanced cancer are non-inferior when compared to individual compounded bags in terms of complications.
