Objectives: Biomimetic remineralization strategies offer promising paths for managing caries- affected dentin by restoring its morphology and structural integrity. This pilot study aimed to assess the morphological and structural changes in dentin following two chemical artificial caries-affected dentin (ACAD) protocols, evaluate the effects of three biomimetic remineralization procedures (BRPs) on dentin structure, and determine the most effective ACAD-BRP combination. The BRPs studied were casein phosphopeptide-amorphous calcium phosphate (CPP-ACP), self-assembling P11-4 (SAP-P11-4), and calcium phosphate polymer- induced liquid precursor (Ca/P-PILP). Methods: Seventy-two specimens of healthy human dentin were divided into twelve groups. Half of each group’s samples were analyzed using field-emission gun environmental scanning electron microscope/energy dispersive X-ray, while the other half were analyzed by X-ray diffraction and micro-Raman spectroscopy. Results: Results demonstrate alterations in dentin morphology and structure induced by the ACAD protocols, affirming their effectiveness in mimicking caries-affected dentin. Among the BRPs evaluated, CPP-ACP emerged as the most promising agent for promoting remineralization and restoring dentin structure. The combination of pH cycling with CPP-ACP exhibited superior efficacy in promoting dentin remineralization compared to other combinations. Conclusions: These findings highlight the potential of biomimetic remineralization strategies, particularly CPP-ACP combined with pH cycling, as effective approaches for managing caries-affected dentin and preserving dental health.

Introdução: Com a eclosão da pandemia causada pela COVID-19, o distanciamento físico trouxe à tona a necessidade de novas formas de interação social. Essa nova realidade atinge diversas áreas da vida cotidiana nos âmbitos social, profissional e acadêmico. Sendo assim, a educação remota passou a ser uma nova constante na academia. Um dos efeitos disso foi a organização do PetLive, que inicia em abril de 2020 como uma solução para dar continuidade às ações praticadas pelo Pet Saúde Interprofissionalidade e refletindo sobre o contexto da pandemia. Objetivo: Apresentar o PETLive – Saúde em Pauta enquanto iniciativa que, por meio do preconizado pela educação interprofissional1, fomentou o acesso às informações técnicas de qualidade sobre saúde em tempos de pandemia e distanciamento físico. Metodologia: A plataforma virtual ‘Instagram’ foi utilizada para a realização dos encontros remotos, por meio de “lives” (encontros ao vivo) na internet. Nesses encontros, profissionais da área de saúde foram entrevistados por discentes dos cursos de Saúde Coletiva e Serviço Social, somado a isso foi utilizado o recurso das “caixas de perguntas”, espaço de interação entre os que assistem com os que apresentam, facilitando a interação entre espectador e profissionais por meio de perguntas. Foram convidados profissionais de áreas diversas da saúde para a exposição dos temas, como docentes de Saúde Coletiva e Serviço Social, Fonoaudióloga, Assistentes Sociais e Psiquiatra. Ao total ocorreram cinco “lives” com os temas “História do SUS”, “Distanciamento Físico”, “Atenção Primária em Saúde”, “Serviço Social em tempos de pandemia” e “Saúde Mental”. Os temas foram pré-estabelecidos pelos discentes e divulgadas com antecedência nas redes sociais. Após a transmissão do encontro, os vídeos foram hospedados no ‘Youtube’ e ‘Instagram’ (IGTv) para viabilizar o acesso dos que não conseguiram assistir de forma síncrona. Resultados: As “lives” tiveram um alcance significativo; as transmissões ao vivo, feitas uma vez por semana no Instagram em conjunto com as publicações no Youtube, potencializaram o alcance e a disseminação de informação técnica de qualidade sobre temas atinentes a pandemia. As transmissões duraram 60 minutos e atraíram um público média de 150 pessoas a cada encontro. Considerações Finais: Os discentes são parte essencial para o desenvolvimento dessa iniciativa, portanto, para a continuidade do PetLive será necessário a participação de novos discentes, para uma melhor execução da mesma e pluralidade de ideias e propostas. O projeto inicial, que teve duração de 5 semanas, obteve um resultado positivo, com um bom desempenho nas redes, extrapolando o nicho municipal. Sendo assim, a perspectiva é da organização de novas temporadas para serem abordados outros temas relacionados à Saúde e enfrentamento a COVID-19. Palavras-chave: Coronavírus. Saúde. O artigo analisa a configuração da força de trabalho civil ativa do governo central brasileiro nos mandatos de Fernando Henrique Cardoso (1995-2002) e de Luiz Inácio Lula da Silva (2003-2010). O artigo associa a condição da força de trabalho do governo central brasileiro às mudanças na coalizão governamental. A residual participação do Ministério da Saúde (MS) na prestação direta de serviços públicos influenciou na trajetória declinante da força de trabalho federal. A implantação do SUS foi fortemente afetada pelo ajuste estrutural da força de trabalho no nível federal ao longo das décadas de 1990 e 2000.This article analyzes the configuration of the active civil servant workforce of the Brazilian government during Fernando Henrique Cardoso - FHC - (1995-2002) and Luiz Inácio Lula da Silva - Lula - (2003-2010) mandates. The article associates the condition of the workforce of the Brazilian government with the changes in the government coalition. The residual participation of the Ministry of Health (MOH) in the direct provision of public services influenced the downward trend of the federal workforce.The implementation of the Unified Health System (SUS) was strongly affected by the structural adjustment of the workforce at the federal level during the decades of 1990 and 2000.

Background: Family and meaning-making resources are culturally congruent and help support Latinx coping with cancer. Objectives: To explore Latinx advanced cancer patients' perspectives on the role of family and meaning/purpose in adjustment to advanced cancer. Methods: A qualitative study was conducted. In-depth interviews were performed and transcripts were analyzed using the method from applied thematic analysis. Setting/Subject: Participants were patients with any advanced cancer diagnosis, recruited from Memorial Sloan Kettering Cancer Center (MSKCC), New York; Lincoln Medical Center (LMC), New York; and Ponce Health Science University (PHSU), Puerto Rico. Measurements: Sociodemographic and semistructured interview. Results: N?=?24 in-depth interviews were completed by Latinx advanced cancer patients (Stage III and IV). When evaluating patients' perspectives on the role of family and meaning/purpose in adjustment to advanced cancer, the team generated three categories: (1) family support, (2) family communication, and (3) include support for the family. Many patients reported the importance of family as a source of meaning and social support in the process of cancer diagnosis and treatment. They also reported communication as central to process information and planning. Also, participants describe their desire to incorporate family members into therapy and for encouraging them to seek counseling. Conclusions: Results highlight the need to include syntonic cultural values such as family and meaning for Latinx individuals coping with advanced cancer in psychological interventions.

Este trabalho, com base na consulta a informações sobre as pautas de debate e trabalho e de instituições públicas e movimentos sociais selecionados, identifica subtemas prioritários e estratégias de ação que conformam a agenda da saúde. Detalham-se convergências e dissintonias entre as proposições, concepções e linhas de ação de distintos agentes, bem como contradições da dinâmica dos movimentos sociais e conselhos de saúde. As reflexões sobre as afinidades e lacunas das agendas investigadas, por um lado, sinalizam por um lado a consolidação e preponderância da saúde para o país; por outro, questionam as perspectivas de mudança dos padrões que estruturaram o sistema de saúde brasileiro. A partir dessas interrogações, são apresentados quatro desafios à implementação do SUS.Based on examinations of information on the discussion agendas and work programs of selected social movements and Government institutions, this paper identifies high-priority sub-themes and action strategies that comply with the healthcare agenda. It offers detailed descriptions of convergences and discrepancies among the propositions, concepts and lines of action of various agents, in addition to clashes in the dynamics of social movements and healthcare councils. Reflections on affinities and gaps in the agendas examined indicate the consolidation and preponderance of healthcare for Brazil on the one hand, while on the other they question the standpoints for change in the guidelines structuring the Brazilian healthcare network. Based on these queries, four challenges are presented for the implementation of Brazil's National Health System (SUS).

Fear of recurrence is one of the main affectations of cancer survivors. In Latin-American exist a lack of evidence on the subject and of valid instruments to measure it. The objective of this study was to determine the psychometric properties of the Concerns About Recurrence Scale in Mexican breast cancer survivors. An instrumental study was carried out with 200 women breast cancer survivors with a mean age of 53 years old and a mean survival of 52.5 months. A confirmatory factor analysis of the original version of the scale identified four factors that explained 77 % of the variance and satisfactory goodness of fit indicators: CFI = .967, TLI = .957, SRMR = .035, RMSEA = .071 as well as high internal consistency α = .945. Significant associations were also identified between general fear subscale, anxiety subscale of the HADS and total HADS score (r = .638 and r = .607; p = .01). The CARS presented adequate validity, reliability and factorial structure. It is recommended for usage in clinical care and research with Mexican breast cancer survivors.

In it, it is showed the acting of the emeritus bishop Dom Aldo Mongiano, between the years of 1975 and 1996. Together with the priests missionaries of Consolata, he put his ministry in defense of a cause: the limitation and ratification of Indians territories occupied by farmers and prospectors. The facts narrated and analyzed here, are based on books and scientific articles, as well as documents available at the Roraima Diocese and also an interview done by Professor Jaci Guilherme, with Dom Aldo himself. Therefore, breaking the specific methodology of a bibliographic research, the work is also a documentary inquiry. The analysis of the collected documents, revealed that, there were conflicts between the church and the local power for the Indians cause, as well as the way how the ecclesial institution has contributed to the formation and strengthen of Indian citizenship in a process of boldness which emerges gradually. Este estudo é um ensaio acerca da história da Igreja Católica de Roraima e seu engajamento na luta pelas causas dos povos indígenas. Nele, delineia-se a atuação do bispo emérito, D. Aldo Mongiano, entre os anos de 1975 e 1996. Ele, junto com os padres e missionários da Consolata, se uniu em defesa de uma causa comum: a demarcação e homologação das terras indígenas ocupadas por fazendeiros e garimpeiros. Os fatos aqui narrados e analisados se baseiam em livros e artigos científicos, bem como em documentos disponibilizados pela Diocese de Roraima e por uma entrevista, realizada pelo professor Jaci Guilherme, com D. Aldo. Portanto, indo além de uma pesquisa bibliográfica, ele também se configura como uma pesquisa documental, a partir de cuja análise é possível perceber os conflitos existentes entre a Igreja e o poder local pela causa indígena, bem como a forma como ela tem contribuído para a formação e fortalecimento da cidadania indígena, em um processo de alteridade que emerge paulatinamente.

Eighty-eight consecutive patients (48 men and 40 women; mean age, 58.9 years; range, 16–84 years) with clinically localized cutaneous melanoma involving the trunk, extremities or head and neck underwent lymphatic mapping at our institution. The primary melanoma had a mean thickness of 2.74 mm (range, 0.95 to 9 mm). Patients were divided into two groups: group A (39 patients) underwent only vital blue dye (VBD) mapping, while group B (49 patients) underwent lymphatic mapping with VBD and radio-guided surgery (RGS) combined. In all patients 1-1.5 mL of VBD was injected subdermally around the biopsy scar 10–20 min before surgery. In group B 37 MBq in 150 μL of 99mTc-HSA nanocolloid was additionally injected intradermally 18 h before surgery (3–6 aliquots injected perilesionally). In all lymphatic basins where drainage was noted the sentinel lymph nodes (SNs) were identified and marked with a cutaneous marker. Final identification of the SN was then performed externally by a hand-held gamma probe. After the induction of anesthesia 0.5–1-0 mL of patent blue V dye was injected intradermally with a 25-gauge needle around the site of the primary melanoma. SNs were examined by routine hematoxylin and eosin (H&E) staining and immunohistochemistry. Patients with histologically positive SN(s) underwent standard lymph node dissection (SLND) in the involved lymph node basin. The SN was identified in 37/39 patients (94.9%) of group A and in 48/49 patients (98.0%) of group B. Blue dye mapping failed to identify the SN in 5 of the 88 patients (5.8%), while the radioisotope method failed in only 1 of 49 patients (2.0%). Similar results were obtained with the combined use of the two probes. The average number of SNs harvested was 1.9 per basin sampled, which does not differ significantly from the numbers reported by other authors114. The SN was histologically positive in 18 patients (20.5%). None of the 12 patients with a Breslow thickness less than 1.5 mm had positive SNs, whereas 18 of the 77 patients (23.4%) with a Breslow index exceeding 1.5 mm showed metastatic SNs with H&E or immunohistochemistry. The latter all underwent SLND of the affected basin. In 10 patients (55.6%) the SN was the only site of tumor invasion; eight patients (44.4%) with positive SNs had one or more metastatic lymph nodes in the draining basin.

Background: Anxiety and depression symptoms are known to increase cancer symptom burden, yet little is known about the longitudinal integrations of these among Hispanic/Latinx patients. The goal of this study was to explore the trajectory and longitudinal interactions among anxiety and depression, cancer symptom burden, and health-related quality of life in Hispanic/Latinx cancer patients undergoing chemotherapy. Methods: Baseline behavioral assessments were performed before starting chemotherapy. Follow-up behavioral assessments were performed at 3, 6, and 9 months after starting chemotherapy. Descriptive statistics, chi-square tests, Fisher’s exact tests, and Mann–Whitney tests explored associations among outcome variables. Adjusted multilevel mixed-effects linear regression models were also used to evaluate the association between HADS scores, follow-up visits, FACT—G scale, MDASI scale, and sociodemographic variables. Results: Increased cancer symptom burden was significantly related to changes in anxiety symptoms’ scores (adjusted β^ = 0.11 [95% CI: 0.02, 0.19]. Increased quality of life was significantly associated with decreased depression and anxiety symptoms (adjusted β^ = −0.33; 95% CI: −0.47, −0.18, and 0.38 adjusted β^= −0.38; 95% CI: −0.55, −0.20, respectively). Conclusions: Findings highlight the need to conduct periodic mental health screenings among cancer patients initiating cancer treatment.

21. International adaptation of Meaning-Centered Psychotherapy for Latinos: Providers’ views on pre-implementation

Background This qualitative study aims to identify facilitators of and barriers to the implementation of Meaning-Centered Psychotherapy (MCP) by providers of mental health services to Latinos in the US and Latin America using the practical, robust implementation and sustainability model (PRISM). This information will be used to increase usability and acceptability of MCP for Latino patients with cancer and their providers in Latin America and the US. Methods A total of 14 Latino cancer patient mental health providers completed in-depth semi-structured interviews. Participants were recruited from 9 countries and 12 different sites. They provided feedback about barriers to and facilitators of implementation of MCP at the patient, provider, and clinic levels in their clinical setting. The qualitative data from the interviews was coded according to PRISM domains. Three analysts independently coded the transcripts; discrepancies between analysts were resolved through discussion and consensus. Results Based on PRISM, themes were: clinic environment (protected time for training and supervision), intervention characteristics (adapt the intervention using more simple language, include more visual aids, include more family-oriented content), patient (develop materials for the identification and screening of patients, provide educational materials, increase motivation and knowledge about psychotherapy, assess commitment to psychotherapy, adapt for the inpatient vs. outpatient setting), provider (receive interactive/participatory training, educational materials, ongoing supervision, have flexibility of delivering the intervention in a less structured manner, theoretical framework of the provider) and external environment (work at policy level to integrate services for oncology patients). Conclusion These qualitative data revealed potential facilitators and barriers of this intervention (MCP) on an international scale. Identified cultural, contextual, and healthcare systems factors illustrated the importance of examining pre-implementation needs prior to implementing a trial. We will design and plan a future RCT using the PRISM framework and these pre-implementation data. Plain Language Summary This study integrates frameworks of implementation science and cultural adaptation through the examination of pre-implementation contextual issues at the preparation phase of a cultural adaptation of a psychotherapeutic intervention, Meaning-Centered Psychotherapy (MCP), for Latinos with advanced cancer. By examining implementation needs early in the implementation process, during preparation, the intervention can be adapted in a way that attends to and addresses the providers’ most cited challenges in implementation: having a rigid protocol/structure, complexity of the intervention, disease burden preventing adherence to the intervention, transportation, and competing demands, and limited clinic space to offer the intervention.

Introduction Literature suggests couple-based interventions that target quality of life and communication can lead to positive outcomes for patients with cancer and their partners. Nevertheless, to date, an intervention to address the needs of Latino families coping with advanced cancer has not been developed. Meta-analytic evidence suggests that culturally adapted evidenced-based intervention targeting a specific cultural group is four times more effective. Our goal is to culturally adapt a novel psychosocial intervention protocol entitled ‘Caregivers-Patients Support to Latinx coping advanced-cancer’ (CASA). We hypothesised that combine two evidence-based interventions and adapting them, we will sustain a sense of meaning and improving communication as patients approach the end of life among the patient–caregiver dyad.Methods and analysis To culturally adapt CASA, we will follow an innovative hybrid research framework that combines elements of an efficacy model and best practices from the ecological validity model, adaptation process model and intervention mapping. As a first step, we adapt a novel psychosocial intervention protocol entitled protocol entitled ‘Caregivers-Patients Support to Latinx coping advanced-cancer’ (CASA). The initial CASA protocol integrates two empirically based interventions, meaning-centred psychotherapy and couple communication skills training. This is an exploratory and prepilot study, and it is not necessary for a size calculation. However, based on recommendations for exploratory studies of this nature, a priori size of 114 is selected. We will receive CASA protocol feedback (phase 1b: refine) by conducting 114 questionnaires and 15 semistructured interviews with patients with advanced cancer and their caregivers. The primary outcomes of this study will be identifying the foundational information needed to further the develop the CASA (phase IIa: proof-of-concept and phase IIb: pilot study).Ethics and dissemination The Institutional Review Board of Ponce Research Institute approved the study protocol #1907017527A002. Results will be disseminated through peer-reviewed publications.

In this paper the year 2000 report from the World Health Organization is discussed and the methodology for comparing and classifying the performance of the health systems of the 191 member countries is analyzed. The main conclusions refer to the use of methodologies which are scientifically questionable, to the implementation of indicators which do not evaluate the performance of health systems but rather reveal the existing socio-economic inequalities, to the absence of necessary data for calculating the indicators in the majority of countries, and to the lack of transparency in the procedures adopted for calculating some indicators. En este texto se discute el Informe de la Organización Mundial de la Salud para 2000 y se analiza la metodología utilizada para comparar y clasificar el desempeño de los sistemas de salud de los 191 países miembros. Las principales conclusiones están referidas a la utilización de metodologías de evaluación científicamente cuestionables; a la implementación de indicadores que no evalúan el desempeño de los sistemas de salud sino que expresan las desigualdades socioeconómicas existentes; a la ausencia de datos en la mayoría de los países para el cálculo de los indicadores y a la falta de transparencia en los procedimientos adoptados para el cálculo de algunos indicadores.

Several studies have been published describing the techniques of identification of the “sentinel lymph node'’ (SN). There are marked differences in the techniques used by different investigators. Although agreement exists about the tracer particle size and the volume of injection, it is unknown what is the best site where to inject the tracer or the vital dye. The aim of the present study was to define the influence of different sites of injection on imaging of the lymphatic ducts and their SNs. We performed a pilot study in 30 consecutive patients with breast cancer who underwent SN biopsy by means of radioguided surgery and vital blue dye mapping. The patients were divided into six groups of five patients each; each patient was given a subdermal (ID) or peritumoral (IP) injection of radiotracer (300 μCi in 150 mL of 99mTc-HSA microcolloids; Albures, Amersham Sorin) above the tumor site in order to localize the SN. After the identification of the SN, a second injection of radiotracer was performed, which was different in each patient subset. In some cases more than one lymph node appeared on the lymphoscintigraphic scans after the second injection of radiotracer. When the peritumoral route was used it took longer to visualize the lymphatic pathways. For the ID route, injection at the exact skin projection over the tumor is optimal. Internal mammary lymph nodes were identified by both IP (2) and ID (1) injection, irrespective of the quadrant in which the tracer was injected. Our findings support the hypothesis of a precise topographic correspondence between the primary tumor and its specific SN. The subdermal route is more accurate than the intraparenchymal route, as it allows faster identification of the lymphatic vessels and SN. We believe these observations should be taken into account for the proper selection of the injection site of either vital dye or radiopharmaceuticals.

ObjectiveThe purpose of this study was to describe the preferences, the use, satisfaction of mental health services (MHS) among a sample of Puerto Rican patients with cancer undergoing oncology treatment.MethodsA convenience sample of 120 patients diagnosed with cancer was recruited. Self-report questionnaires assessed socio-demographic and background questions, and the Mental Health Service Preference, Utilization and Satisfaction Questionnaire (MHSPUS). The Socio-demographic and Background Questionnaire inquired about participants' demographic and socioeconomic characteristics, and included questions such as history of psychiatric diagnosis and spiritual practices. Univariate and Chi square analyses were used for descriptive purposes. Logistic regressions were used to explore associations between sociodemographic factors and MHS preferences and use.ResultsThe majority of the sample were females (53.8%), 61 and older (53.8%), and married or living with partner (57.1%), and reported an income equal to or less than $12,000 per year (44.4%), which places them under the US federal poverty line. Most of the participants (66.7%) reported being receptive to seeking services. Findings showed a significant association between living situation and past (p < .05) and lifetime use (p < .05) of MHS and past use of MHS. Participants living alone were more like to have used MHS in the past and during their lifetime. Adjusted logistic regression analyses revealed that living with someone was a protective factor for not using MHS in their lifetime (OR = 0.28; C1 = 0.08-0.95). Participants preferred to receive MHS at the oncology clinic, preferably on the date of their oncology appointments and during morning hours.ConclusionFindings support the integration of mental health services within the oncology practice setting.

This paper will expound on the findings discovered from efforts made by including an occupational therapist on an interprofessional education initiative charged with developing content to enhance knowledge of geriatrics for health care providers, community-dwelling older adults, and caregivers in medically underserved areas. We will describe the actions of the interprofessional team and present data about the impact of collaborative, systematic input on one educational program taught primarily by the occupational therapist. We will then describe the personal and professional growth opportunities experienced by the occupational therapist on this initiative, and impart the challenges faced and provide recommendations to overcome them. Finally, we will discuss practical implications for advancing interprofessional education and occupational therapy at other institutions

El cáncer de mama es el diagnóstico de cáncer más comúnmente diagnosticado entre las mujeres latinas. En la actualidad se plantea la importancia de tomar en consideración los valores tradicionales de la cultura hispana cuando se trabaja con pacientes de cáncer. Estos valores han incluido la impor- tancia de enfatizar la familia e involucrar la misma en el tratamiento. Los objetivos de este artículo son explorar: 1) la reacción de los familiares ante el diagnóstico y tratamiento de la paciente de cáncer de mama, 2) el impacto del apoyo recibido por los familiares en las pacientes y 3) la importancia del involucramiento de la familia en el tratamiento. Los hallazgos de este estudio revelan la necesidad de dirigir los servicios de salud, no sólo a la paciente, sino también al sistema familiar al cual ella pertenece. Es necesario que se ayude a la familia a explorar los sentimientos y miedos relacionados al cáncer, fomentar la comunicación entre éstos, evaluar el impacto de la enfermedad a nivel estructural y fomentar la re-organización del sistema a partir del diagnóstico, durante el tratamiento, post-tratamiento y periodo de sobrevivencia.

En este estudio se exploró los síntomas psicológicos y se analizó la relación e impacto del funcionamiento familiar, desde la perspectiva de los/as paciente/sobreviviente, la pareja cuidador/a y sus hijos), en el funcionamiento psicológico de 51 hijos/as adolescentes de pacientes de cáncer. Los hallazgos revelaron que los/as adolescentes exhibieron sintomatología en niveles clínicos. La expresividad familiar y una relación marital satisfactoria de los pacientes/sobrevvientes son los factores protectores de un mejor ajuste psicológico de los/as hijos/as. Las intervenciones con adolescentes que enfrentan el cáncer de un padre o madre requieren un cernimiento de riesgo del funcionamiento psicológico y familiar e intervenciones orientadas al sistema.

Os Centros de Saúde do Rio de Janeiro são analisados enquanto uma organização estruturada para desempenhar, na área da saúde, parte das funções do aparato burocrático do Estado brasileiro. Seu papel institucional vincula-se à emergência de novos problemas sanitários numa sociedade em processo de urbanização e de consolidação de um Estado nacional. Ao longo de usa trajetória histórica passou por situações distintas, no que se refere ao seu papel no Sistema de Saúde, articuladas a determinantes de natureza técnica e política. Enquanto fenômeno burocrático, esta organização sobreviveu às diversas conjunturas históricas interpondo resistências, que se manifestaram, principalmente, pela lentidão com que eram processadas as mudanças internas propostas. The Health Centers in the city of Rio de Janeiro are analyzed as organizations that have been structured to execute parto of the bureaucratic duties petaining to the brazilian State, as far as health care is concerned. Their role, as institutions, is related to the solution of new problems, in their sanitary aspects, that are emerging in a society in processo f increasing urbanization and inside a process of organizing a National State. Along the course of their history, these organization have overcome various distinctive situations, dependind on technical and political determinants. As bureaucratic phenomena, this organization have survived many historical conjuntures, presenting itself as resistant to the proposed changes, mainly by slow ways of implementing these propositions.

33. The concepts of health regulation in Brazil

- Abstract
Neste trabalho foram identificados os conceitos de regulação em saúde empregados na literatura científica nacional sobre gestão em saúde. Foi elaborada uma tipologia para o conceito de regulação utilizando os conceitos de regulação de outras disciplinas (ciências da via, direito, economia, sociologia e ciência política). Essa tipologia baseou-se no agrupamento das ideias que fundamentam a concepção da regulação naquelas disciplinas. Das diferen-tes ideias relacionadas ao emprego dos conceitos de regulação, destaca-ram-se quatro como fundamentais, quais sejam: controle, equilíbrio, adaptação e direção. A partir dessa tipologia, os conceitos de regulação identificados nos artigos científicos nacionais foram classificados e analisados. Considera-se que a discussão do conceito de regulação em saúde está relacio-nada com a compreensão da inserção do Estado no setor saúde, ou seja, o papel que o Estado exerce nesse setor. Os conceitos de regulação em saúde apresentam uma variedade de significados, abordagens e finalidades. O tema da regulação em saúde está conexo ao da reforma política e administrativa do Estado. No entanto, o que diferencia esses conceitos de regulação é justamente a definição de diferentes formas de intervenção do Estado. Por fim, destaca-se o maior peso para a natureza técnica da regulação (gerencial) dos conceitos de regulação em saúde apresentados nos artigos objeto deste estudo. A natureza política da regulação ficou em segundo plano The purpose of this study was to identify the concepts of health regulation employed in the national scientific literature on healthcare management. The study developed a typology for the concept of regulation using the concepts of regulation from other disciplines (Life science, Law, Economics, Sociology and Political science). This typology was based on the grouping of the ideas underlying the design of regulation in those disciplines. Considering the various ideas related to the use of the concepts of regulation, four were considered as fundamental ideas, namely: control, balance, adaptation and direction. From this typology, the concepts of regulation found in the national papers were classified and analyzed. It is considered that discussion of the concept of health regulation is related to the understanding of the integration of State in the health sector, namely the role that the State has in this sector. The concepts of health regulation offer a variety of meanings, approaches and goals. The theme of health regulation is related to the political and administrative reform of the state. However, what differentiates these concepts of regulation is precisely the definition of different forms of State intervention. Finally, it was highlighted the emphasis to the more technical nature of regulation (management) concepts of health regulation presented in the articles analyzed. O trabalho analisa a experiência de implantação do modelo das Organizações Sociais de Saúde (OSS) no Estado de São Paulo, buscando compreender as questões que se colocam para a adoção de modelos centrados na administração privada de unidades de saúde no âmbito do SUS, seja do ponto de vista sistêmico, seja com relação aos elementos de natureza prática relacionados a sua gestão financeira por parte do Estado. Para tanto, procura-se, inicialmente, identificar as referências teóricas que dão suporte ao modelo OSS no campo da Ciência Econômica e posicionar a experiência em relação às tendências recentes de reforma dos sistemas de saúde. Em seguida, explora-se a importância relativa desse modelo no orçamento do Estado e da Pasta Saúde e os determinantes da formação dos preços dos serviços contratados pelo Estado sob essa forma específica de gestão das unidades públicas de saúde. This work analyses the experience of Social Health Organizations (Organizações Sociais de Saúde - OSS) implemented by the State Government of São Paulo. Its purpose is to discuss the systemic and practical issues involved in strategies of private management of health units in the Brazilian public health system (SUS) from a financial perspective. This inquiry is pursued initially by identifying the theorectical approach supporting the experience and its relation with health systems recent reforms. In paralel, the OSS model is studied in terms of its importance in the state public budget and in respect to the mechanims determining health services prices through it provided.

35. Analysis of a process in construction: the regulation of the supplementary health system in Brazil

Trata-se de uma pesquisa qualitativa composta por análise bibliográfica categorial com foco em reforma do Estado e regulação, por um levantamento da estrutura do mercado e das atas de reunião da Câmara de Saúde Suplementar (CSS) e por entrevistas semi-estruturadas com os principais atores desta câmara, buscando entender posicionamentos e principais pontos em debate, construindo um quadro de referência do setor, visando identificar sucessos e lacunas do processo. A pergunta central é até que ponto a regulação, a partir do modelo de agência adotado no Brasil para o setor de saúde suplementar, não estaria atingindo os objetivos propostos quando da sua criação, ou seja, de defender o interesse público na assistência suplementar à saúde. O mercado de saúde suplementar é composto por 36,9 milhões de beneficiários de planos de saúde em 2006 representando 19,6% da população brasileira. Apesar do seu tamanho e da relação público-privada que permitiu seu desenvolvimento ao longo do século passado ocorreu à margem de um regramento oficial até 1998, com a promulgação das Leis 9.656/98 e 9.961/00, esta última criando a ANS, estendendo o processo de reconfiguração do papel do Estado para o setor de saúde. Entre 01/2000 e 12/2006 a ANS realizou 25 consultas públicas (4,2 consultas ao ano em média), destas 11 trataram de temas financeiros; enquanto questões cadastrais, de definição de produtos e de contratualização foram temas de três consultas cada. Neste mesmo período a ANS emitiu 790 normativos, uma média de 113 normativos por ano. A comparação entre consultas públicas e normativos sugere uma pequena participação externa à agência no processo de regulação. Desde sua criação em 1998 até 09/2006 a CSS se reuniu 44 vezes, considerando presença relativa (ponderada pelo número de convocações) as representações mais presentes foram: prestadores de serviço, medicinas de grupo, seguradoras, reguladores (ANS) e consumidores com 100%. No pólo inverso foram identificados: trabalhadores, governo e gestores com menos de 55% de presença relativa, sugerindo o grau de importância que cada grupo de representação confere à CSS, seja como fórum de debate ou espaço para disseminação de suas posições. Nestas reuniões foram pautados 129 temas, com predominância daqueles ligados a característica e estrutura da regulamentação (35% dos temas), apresentações da ANS (13% das pautas) e programas da ANS (9% dos temas). Temas como a avaliação da ANS pelos atores e discussão quanto a lacunas no processo de regulação, embora pareçam cruciais para a adequação do modelo, foram tratados apenas uma vez cada. Adicionalmente, a elaboração por parte dos atores da CSS de documentos para discussão foi tema em apenas duas das 129 pautas. As entrevistas com os atores da CSS revelam que há consenso quanto a Reforma do Estado ser a origem da regulação via agência, porém discute-se sua autonomia, distanciamento do controle social do SUS, falta de integração com políticas do Ministério da Saúde, interfaces entre os sistemas público e privado, renúncia fiscal, subordinação entre SUS e sistema suplementar, efetividade dos contratos préregulação e participação (ou interferência) do Judiciário no processo. A maioria das xv representações discute saúde suplementar desde o inicio dos anos 90, sofrendo o desgaste em função do longo período de participação e dos resultados aquém dos esperados. A composição heterogênea, não paritária e o caráter consultivo da CSS dificulta a construção do entendimento. A baixa participação de governo, trabalhadores e gestores e a discussão prévia entre ANS e MS dos assuntos relevantes corroboram com a avaliação de baixa produtividade dada à CSS. Há consenso pela busca de sustentabilidade do mercado, mas com divergências quanto às alternativas para atingi-la, com posicionamentos antagônicos quanto a incentivos fiscais, ressarcimento ao SUS, volume de lucro aceitável e metodologia de apuração dos reajustes de preços. Os gargalos apontados pelos atores foram sistemas e recursos humanos, falta de integração entre as diretorias da agência e o volume de normativos, que são apontados como fatores de lentidão e incremento de custos no processo regulatório. Ressaltam ainda a necessidade de maior participação da sociedade e transparência. São reconhecidos poucos sucessos (definição de produtos, direitos e cobertura, saneamento do mercado e programas de qualificação e troca de informação) e diversas lacunas (adequação do marco regulatório, integração interna da ANS, incorporação do prestador de serviços no campo regulado e integração com o SUS). Conclui-se que a regulação em saúde suplementar atingiu uma fase em que nenhum ator está satisfeito, mesmo entendendo ser este um processo em construção e dadas as divergências de interesses e limitações do fórum de discussão, a construção de consensos via CSS é complexa podendo não ocorrer. Adicionalmente, os posicionamentos e lacunas da agência no processo não permitem enxergá-la como efetiva defensora do interesse público em saúde suplementar. This qualitative research is composed by a bibliographical analysis focused on state reform and regulation, the market structure analysis, the Câmara de Saúde Suplementar (CSS) meeting minutes analysis and by semi-structured interviews with CSS actors understanding positioning, discussion points, identifying successes and lacking points of the process. The objectives of this thesis are build a frame of reference for the supplementary health market, through a statistical analysis including analysis of the CSS meeting minutes, of the public consultations and rules issued by ANS; as well as analyzing the impacts of regulation on the several groups of interest represented in the CSS, discussing amplitude and range of regulation and questioning ANS mission achievement, as public interest defender in this market. The Brazilian supplementary health market assists 36.9 million beneficiaries, according to 2006 s data, which represents 19.6% of the population. Despite its size and the public-private relationship which allowed its growth since the beginning of last century, it remained unregulated until 1998. Regulatory activity was undertaken by ANS in 2000, extending State role reconfiguration concept to health field. Between 01/2000 and 12/2006 ANS called 25 public consultations (4.2 per year), from which 11 dealt with financial subjects, while masterfile, product definitions and contractualization were subject of 3 consultations each. During this period ANS issued 790 rules, an average of 113 per year. The comparison between the number of public consultations and of rules issued suggests small external participation. Since its creation in 1998 until 09/2006, CSS held 44 meetings. Considering relative participation, the most present representations were service providers, health maintenance groups, insurers, regulators and consumers with 100% of presence. On the other hand health workers, government and public health managers were present to less than 55% of the meetings, suggesting the relative importance given to CSS by each representation. During these meetings 129 different subjects were discussed. Main topics covered were regulation characteristics and structure (35%), ANS presentations (13%) e ANS programs (9%). Subjects as ANS evaluation and lacking points discussion, despite seeming crucial were dealt only once each. Additionally discussion of documents prepared by CSS actors took place only twice. Interviews seeked for actor s positioning on regulation model, were consensus resides on agency origin from state reform process, but autonomy, distance from SUS social control, lack of integration with Health Ministry policies, public and private systems interfaces, tax relieves, subordination of private system to SUS, effectiveness of preregulation contracts and Justice interference in the process are still points of discussion. The majority of representants have being discussing private health issues since the beginning of the 90s, suffering from the stress of long participation with limited results. The uneven composition and consulting status of the CSS are obstacles towards build understanding. Government s, health workers and public health managers low participation in CSS and pre-meeting discussions of relevant xvii subjects between ANS and MS, induce to a low productivity appraisal of CSS. Pursue sustainability seems to be a consensus, although ways to achieve this are discrepant, varying from definition of new fiscal incentives and reimbursement to SUS policy to definition of admited profit margins. ANS s evaluation by actors indicate botlenecks in IT and human resources, lack of integration within the agency s directorships and the amount of rules issued all of with contribute to increase costs and delay the regulatory process. Transparency and participation on decision processes are also claimed for. Few success examples are identified (product, rights and coverage definition, market clearing and implementation of quality programs) and several lacking points are indicated (adequacy of the regulatory base, internal integration, inclusion of the service providers in the regulated field and integration with SUS). Conclusions indicate that regulation has achieved a stage were none of the actors are satisfied, even recognizing that this is still a process in construction, and given the interest discrepancies between actors and limitations of the discussion arena, build consensus via CSS is complex and may not happen. At the same time, the number of lacking points in the regulation process show the distance for ANS to achieve its mission, and therefore do not allow see ANS as effective public interest defender in this market.
