Este artículo se propone a analizar y contextualizar la formación de enfermeras y enfermeros a nivel de posgrado, y presentar orientaciones estratégicas para ampliar su formación doctoral en América Latina y el Caribe. En esta comunicación se presentan estrategias para avanzar el potencial de la contribución del personal de enfermería a los sistemas de salud y el desarrollo de competencias científicas por medio de los estudios de posgrado -nivel doctorado-, con hincapié en la cooperación e internacionalización, la formación alineada con las necesidades de salud de la población, la transferencia de conocimientos a la práctica y la sostenibilidad de las acciones. El personal de enfermería está bien posicionado para actuar como investigadores, docentes y líderes, y para contribuir a la elaboración de estrategias más eficaces para las futuras acciones y políticas que requieren los sistemas de salud. Sin embargo, será necesaria la inversión y el compromiso de diferentes actores clave para transformar la formación de enfermería.

Este estudo descreve a sobrecarga de cuidadores de idosos demenciados no Brasil por meio de uma revisão integrativa da literatura, no período de 1999 a 2009, publicado em português e em inglês. Os dados foram coletados pelas bases de dados eletrônicas Pubmed, Cinahl e Lilacs. As palavras-chave usadas na CINAHL e na Pubmed foram: dementia ou cognitive impairment, aged ou older people, Brazil, caregiver burden ou caregiver stress ou caregiver strain. Foram encontradas 14 pesquisas. As palavras-chave usadas nos LILACS na categoria DeCS foram idosos”, “cuidadores” e “demência” foram combinados com “e”, que conduziram a 25 estudos. Aproximadamente 39 estudos foram selecionados, os quais foram reduzidos então a vinte, após temas não relacionados, a língua espanhola, pesquisas repetidas, teses/dissertações e livros. As pesquisas foram organizadas em três temas: doze(60%) focalizaram na correlação entre a sobrecarga e as características dos cuidadores e dos idosos; seis (30%) reportaram as experiências e sentimentos dos cuidadores e dois (10%) discutiram a defi nição de sobrecarga. Há necessidade de que os serviços sanitários governamentais brasileiros, as universidades, as organizações não governamentais e outras agências e redes planejem propostas formais e informais de intervenção dirigidas aos cuidadores informais.

Introduction: The World Health Organization (WHO) has proposed to monitor intrinsic capacity (IC) in the older population as a public health strategy through the Integrated Care for Older People (ICOPE) program. Although the program has been developed based on solid concepts, scientific evidence on its practical applicability is still scarce. Objectives: To evaluate IC in Brazilian older adults, its progress over time, and its association with sociodemographic and health factors and outcomes. To evaluate the psychometric properties of the WHO/ICOPE screening tool. Methods: This is a prospective multicenter cohort study with a 36-month follow-up. We will recruit 3838 people aged ≥ 60 years, registered in the health care units included in the study by the participating centers. We will collect sociodemographic and health data and will administer tools to assess IC domains, both those provided for in the ICOPE screening tool and the sequence of confirmatory assessments provided for in the program. Participants will be reassessed every 6 months for 36 months. Expected results: To establish the profile of IC in the study population and to understand its progress and the variables associated with the clinical outcomes of interest. To reveal the diagnostic and psychometric properties of the WHO/ICOPE screening tool. The project is funded by the Brazilian National Council for Scientific and Technological Development (CNPq). Relevance: Understanding the potential use of the ICOPE public health strategy proposed by the WHO within the scope of the Brazilian Unified Health System (SUS) by integrating several research centers in the field of Geriatrics and Gerontology throughout Brazil.

AIM: To analyze the evidence available on foot reflexology with application of caring consciousness to reduce pain in older adults METHODS: Integrative review carried out in Medline/PubMed, CINAHL, EBSCO Host, SCOPUS, Web of Science, and EMBASE databases using the MESH terms “Aged,” “Pain,” “Musculoskeletal manipulations,” “Nursing,” and “Nursing theory.” in English, Spanish, Portuguese, and French published between 2012 and 2022. The selected studies were exported to Rayyan software and evaluated by title and abstract; after reading the full text, were analyzed according to the PRISMA guide checklist. RESULTS: In this study, 497 articles were found, 4 were selected. 75% of the studies were based on the Ingham method of reflexology. The elements that demonstrate caring consciousness and enhance the healing effect of the therapy were foot grooming, use of essential oils, implementation of relaxation techniques, description of pressure points and combining other therapies. In 75% of the cases, foot reflexology was effective to reduce pain. CONCLUSION: Those authors who apply caring consciousness in the technique of reflexology obtained more significant results in the reduction of pain. The incorporation of external elements shows the intention of nursing to enhance the effects of the therapy, as well as the search for connection with the elderly.

Objective: to map the transition of care for the elderly after a stroke from hospital to home. Methods: a scoping review using the JBI methodology. The search was conducted in eight databases, an electronic library, and gray literature sources. Endnote and Rayyan were the tools used to select the studies, following the PRISMA-ScR guidelines. The selection of studies involved two reviewers, with a third for the final analysis. Results: of 1,378 studies, 30 were selected for full reading, resulting in 12 articles published between 2013 and 2023, three of which were qualitative, eight quantitative, and one mixed study. In addition, two book chapters were retrieved from the grey literature to make up the sample, giving a total of 14 studies on the subject. Conclusion: The proposals for a plan need to cover the general conditions of the elderly person in the transition process, but attention should be paid to the environmental context in which the person lives and the availability of resources. Contributions to practice: the evidence described can help build more effective measures to improve the care provided to this individual who needs to transition from hospital to home care.

Background & Aim: Dependent elderly people have difficulty carrying out activities of daily living and need help with essential life-sustaining activities such as bathing. Nursing care includes technique, organization, and care for the patient's physical and emotional well-being. This study aimed to map the scientific evidence on home bathing care for dependent elderly people. Methods & Materials: A scoping review on home bathing care for dependent elderly people according to the methodology proposed by the Joanna Briggs Institute Collaboration. Nine databases were searched, as well as sources of gray literature. Rayyan software was used to select the studies by two reviewers and a third for the final assessment. Results: 24 studies were selected. The most important aspects of bathing were communication, considering the elderly person's preference for bathing, considering conflict resolution, safety, comfort, and privacy, planning the necessary materials and actions, paying attention to the temperature of the water and the environment, cleaning and tidying the bed, as well as hygiene care such as trichotomy, and nail clipping. Conclusion: It was identified that care is related to specific skincare and planning to carry out the steps in body hygiene. The studies show the importance of guidance from nursing professionals in the process of bathing dependent elderly people at home from the perspective of a person-centered approach and their uniqueness.

Introdução: O Diabetes Mellitus ocasiona a diminuição das reservas e capacidades funcionais. Sua associação com a síndrome da fragilidade acarreta declínio gradativo no sistema biológico, causando prejuízos globais à saúde da população idosa e, portanto, comprometendo sua qualidade de vida. Objetivo: analisar a evolução da fragilidade e fatores associados em pessoas idosas com Diabetes Mellitus. Materiais e Métodos: Estudo longitudinal caracterizado por duas avaliações com intervalo de 18 meses, envolvendo 49 participantes com idade ≥ 60 anos e de ambos os sexos com diagnóstico clínico de Diabetes Mellitus. Na análise dos dados foram utilizadas medidas de tendência central, dispersão, proporções, teste não paramétrico de Wilcoxon pareado e análise linear múltipla. Resultados: Na avaliação do seguimento, ocorreu um aumento da fragilidade e comprometimento da capacidade funcional entre as duas avaliações. Fatores associados, como as atividades instrumentais da vida diária e o número total de doenças, impactaram negativamente sobre a fragilidade dos participantes. Discussão: Os resultados encontrados no estudo convergem com a literatura cientifica relacionada à associação de doenças crônicas como a Diabetes Mellitus no aumento da fragilidade. Conclusão: A presença de uma doença crônica como a Diabetes Mellitus pode ocasionar o aumento da fragilidade e comprometer a funcionalidade. A avaliação destas condições nos serviços de saúde para identificação precoce é fundamental para estabelecer estratégias assertivas para a manutenção de um envelhecimento com qualidade de vida.

Resumen Objetivos: Determinar la predicción de las caídas y caídas recurrentes asociadas a factores de riesgo en el adulto mayor que vive en el domicilio. Metodología: Estudio cuantitativo y transversal realizado en La Libertad, Perú. La muestra fue de 1.110 adultos mayores que viven en el domicilio y se utilizó instrumentos como perfil social, enfermedades y medicamentos autorreferidos, caídas, estado cognitivo, síntomas depresivos, actividad básica e instrumental de la vida diaria, y equilibrio estático y dinámico. Se analizó con estadística descriptiva, curvas ROC y el área bajo la curva para evaluar los valores predictivos y la regresión logística. Resultados: La mayoría era mujer, de entre 60 y 79 años, con compañero, viven acompañados y trabajan. La prevalencia de caídas fue del 30,5% y de caídas recurrentes del 14%. Hubo predominio de aquellos sin déficit cognitivo, necesitaban ayuda para las actividades instrumentales e independientes para las básicas, sin síntomas depresivos y con equilibrio estático y dinámico preservado. En la regresión hubo relación entre caídas con ser mujer, vivir solo, número de enfermedades y medicamentos y dependencia para las actividades básicas. Para caídas recurrentes hubo asociación con vivir solo, uso de medicamentos y dependencia para las actividades instrumentales. Conclusiones: La caída está asociada a diferentes factores y su identificación precoz es importante para la creación de planes de cuidados por el equipo multiprofesional evitando problemas físicos y psicológicos en el adulto mayor.

Objetivo: relacionar el Síndrome de la Fragilidad y la sarcopenia en ancianos con y sin diabetes mellitus tipo 2; e identificar los potenciales factores de riesgo para la fragilidad y sarcopenia. Método: estudio epidemiológico descriptivo realizado en 140 ancianos del municipio de Sinop, Mato Grosso, Brasil. Para evaluar el Síndrome de la Fragilidad se utilizó el fenotipo de fragilidad y para la evaluación de la sarcopenia el cuestionario de evaluación física con medida de la circunferencia de la pantorrilla. Resultados: en lo que se refiere al Síndrome de la Fragilidad se obtuvo un mayor porcentaje para ancianos con diabetes mellitus tipo 2, cuando comparados con aquellos sin la enfermedad (p = 0,00). En relación a la presencia de sarcopenia, los ancianos con y sin diabetes mellitus tipo 2 presentaron valores semejantes, sin significación estadística (p = 0,74). El Síndrome de la Fragilidad presentó asociación con: inactividad física (IC95%: 3,29-56,55); intervalo etario superior a 75 años (IC95%: 3,30-27,82); baja renta familiar (IC95%: 1,80-50,98); y, comorbilidades (IC95%: 4,9-5,4). Entre tanto, la sarcopenia fue asociada a la presencia de la inactividad física (IC95%: 1,26-10,44), al bajo peso/eutrófico (IC95%: 3,32- 26,76) y a la desnutrición/riesgo nutricional (IC95%: 1,30-7,70), en los ancianos con y sin diabetes mellitus tipo 2. Conclusión: los ancianos diabéticos tienen mayor vulnerabilidad para desarrollar el Síndrome de Fragilidad, lo que requiere la adopción de medidas preventivas en la atención primaria a la salud.

Background: Low literacy is associated with poor cognitive performance and it is a risk factor for dementia. Recently, research suggests an association between levels of mindfulness and cognitive performance of older adults. This study aims to analyze if levels of mindfulness mediate the relationship between years of education and cognitive performance in older adults with low education. Objetivo: analisar o conhecimento produzido na literatura científica sobre os instrumentos utilizados na avaliação da capacidade funcional, fragilidade e sarcopenia do idoso. Método: revisão integrativa da literatura, nas bases de dados Cinahl, MEDLINE/PubMed, Embase, Web of Science e Scopus, de 2012-2021. Foram extraídos os dados: autores, ano, país de publicação, tipo de estudo, características da amostra, objetivo, resultados e os instrumentos. Ademais, foi realizada a avaliação da qualidade metodológica e nível de evidência. Resultados: a amostra final incluiu 13 artigos. Os instrumentos mais utilizados foram: Índice de Lawton e Brody para avaliação da capacidade funcional; modelo conceitual do fenótipo para fragilidade; e a conceituação e diagnóstico do Grupo de Trabalho Europeu da Sarcopenia. Objective: to analyze the knowledge produced in the scientific literature on the instruments used to assess functional capacity, frailty, and sarcopenia in older adults. Method: integrative literature review, in the Cinahl, MEDLINE/PubMed, Embase, Web of Science and Scopus databases, from 2012-2021. Data were extracted: authors, year, country of publication, type of study, sample characteristics, objective, results, and instruments. In addition, the methodological quality and level of evidence were assessed. Results: The final sample included 13 articles. The most used instruments were Lawton and Brody Index for functional capacity assessment; conceptual model of the frailty phenotype; and the conceptualization and diagnosis of the European Working Group on Sarcopenia. Conclusion: The scientific evidence demonstrated the importance of using instruments to screen for these conditions that can interfere with the health of the elderly population, reinforcing the need to strengthen interprofessional care practices.

Método: revisión bibliográfica integradora, en las bases de datos Cinahl, MEDLINE/PubMed, Embase, Web of Science y Scopus, entre 2012 y 2021. Se extrajeron datos: autores, año, país de publicación, tipo de estudio, características de la muestra, objetivo, resultados e instrumentos. Además, se evaluó la calidad metodológica y el nivel de evidencia. Resultados: La muestra final incluyó 13 artículos. Los instrumentos más utilizados fueron: el índice de Lawton y Brody para evaluar la capacidad funcional; el modelo conceptual del fenotipo de fragilidad; y la conceptualización y diagnóstico del Grupo de Trabajo Europeo sobre Sarcopenia. Conclusión: La evidencia científica demostró la importancia del uso de herramientas para la detección de estas condiciones que pueden interferir con la salud de la población anciana, reforzando la necesidad de fortalecer las prácticas de atención interprofesional.

Resumo Objetivo Analisar o conhecimento produzido na literatura científica sobre a resiliência de pessoas idosas diante da pandemia COVID-19. Método Revisão integrativa da literatura com estudos primários publicados na íntegra, em Portugues, Ingles e/ou Espanhol, entre 2020 e 2022 nas bases de dados PubMed, EMBASE, LILACS, Web of Science e PsycINFO-APA. Os artigos foram exportados para o sistema Rayyan, os estudos selecionados foram lidos e inseridos em um instrumento cujos dados extraídos foram: autores, ano, objetivo,tipo de estudo, amostra, média da idade, resiliência, principais resultados e conclusão. Na avaliação crítica para determinar a qualidade metodológica e nível de evidência, para estudos qualitativos utilizou-se o Guidelines for Critical Review Form: Qualitative Studies , para quantitativos o Guideline Critical Review Form for Quantitative Studies e estudos mistos o Mixed Methods Appraisal Tool . Resultados Correspondem à análise final dos nove artigos publicados. Sobre às estratégias de enfrentamento, destacam-se atividades físicas, sendo estas, caminhadas, Tai-chi , corridas, exercícios de resistência e ciclismo; o uso da tecnologia de comunicação, como jogos, site de videos, teleconferencias, assistir TV e o uso de aplicativos de comunicação e as atividades de lazer, como a leitura, tocar instrumentos musicais, meditação, a arte de costura, jardinagem e pintura e estilo de vida, mantendo-se ocupado, busca de apoio social, se alimentar saudável e a manutenção de rotinas domésticas. Conclusão As pessoas idosas foram relativamente resilientes durante essa pandemia. A resiliência foi fator importante para a manutenção do bem-estar e qualidade de vida dessa população, com destaque para a prática de atividade física e o uso de tecnologias.

Objective: to identify the factors associated with the use of assistive technologies by elders in their home environment. Methods: cross-sectional, analytical, and exploratory study with 127 elders capable of answering the data collection instruments. Analyses were carried out using Student's t, Pearson's correlation, and linear regression. Results: in regard to the type of assistive device used, 52.8% of interviewees used orthoses or prostheses, 48.8% used shower chairs, and 47.2% used wheelchairs. In the comparative analysis of the means, there was a statistically significant association between basic daily life activities and cognitive state. In basic daily life activities (r=0.52), as the score of the elderly in Katz's Index increases, the use of assistive devices also increases. In turn, as the Lawton and Brody Scale (r=-0.279) increases, the number of devices decreases. Conclusion: the prevalence of elders who use assistive devices in their homes was high, showing a direct association with their functional capacity and their performance of basic and instrumental daily life activities. Contributions to practice: this research contributes for the organization of elderly assistance by nursing workers, encouraging elder independence and autonomy.

Resumo Objetivo: sintetizar o conhecimento disponível sobre o impacto de mídias sociais na percepção de solidão e/ou no isolamento social em idosos. Método: revisão integrativa da literatura com estudos primários publicados na íntegra, em português, inglês e espanhol, entre setembro de 2014 e julho de 2020 nas bases de dados American Psychological Association Database, Cumulative Index to Nursing & Allied Health Literature, Literatura Latino-Americana e do Caribe em Ciências de Saúde, Web of Science e PubMed. Resultados: foram incluídos 11 artigos categorizados com base nos tipos de tecnologias: “o uso da internet”, englobando sites de redes sociais, internet e aplicativos; “dispositivos de comunicação”, com uso de smartphones, tablets e iPads; e “tipos de comunicação”, incluindo meios de comunicação interpessoal na era digital, como videochamadas e e-mails. Houve resultados positivos (63,6%) sobre o uso de mídias sociais para minimizar a percepção de solidão e/ou o isolamento social dos idosos. Conclusão: as evidências científicas demonstraram que o uso de mídias sociais digitais pode reduzir a percepção de solidão e/ou o isolamento em idosos. Ademais, a internet pode favorecer maior contato entre idosos e famílias, servir como fonte de apoio e contribuir para maior senso de pertencimento em uma comunidade e redução da solidão.

ABSTRACT Objective: To identify and analyze the sociodemographic and health factors and the social support network of the elderly associated with frailty in the assessments carried out between 2007/2008 and 2018. Methods: This is a longitudinal study with elderly people aged ≥65 years living in the community. The instruments used were those for Demographic Profile, the Mini Mental State Examination, the Functional Independence Measure, Lawton and Brody Scale, Geriatric Depression Scale, Minimum Relationship Map for the Elderly, and Edmonton Frail Scale. Descriptive analysis and linear regression were used, all tests with p < 0.05. Results: Of the 189 elderly in the study period (2007/2008–2018), most were 80 years old and over, with an average of 82.31 years old; they were women, with no partner, who lived with other family members and were retired. In the final analysis, regardless of age and sex, a decrease in functional independence, an increase in depressive symptoms, an increase in the number of self-reported illnesses, and an increase in the frailty score were observed. Conclusion: The factors that were associated with the increase in frailty of the elderly during the study period were age, female sex, and no partner. The health team, which includes nurses, shall be aware of changes and develop care plans to prevent or avoid their progression.

Objective. To identify the physiological changes in older adults’ vision during the aging process. Methods. An exploratory, descriptive study with a qualitative approach was conducted with 20 older adults and six healthcare providers who worked with older adults in João Pessoa, Paraíba (Brazil). The Focus Group Technique was used for data collection, with the collected information subsequently being submitted to Inductive Thematic Analysis using textual analysis software. Results. The physiological changes related to vision were described by both the older adults and healthcare providers using the following words: vision, difficulty; see; cataract; glasses; surgery; more; age; eye; and no. These terms represent declines in vision resulting from advancing age which significantly modify the daily lives of older adults and their families. Conclusion. The perception of the older adults and the healthcare providers who care for them regarding the physiological changes in vision throughout the aging process shows that the loss of visual acuity significantly affects the daily life of older adults and their families.

Resumo Objetivo analisar o conteúdo de documentos propostos por movimentos sociais e entidades de classe para orientar o cuidado em Instituições de Longa Permanência para Idosos (ILPI) durante a pandemia da COVID-19 no Brasil. Método Pesquisa documental, retrospectiva, descritiva e exploratória. Foram identificados quatro websites de movimentos sociais e entidades e analisados seus respectivos documentos para orientar o cuidado à pessoa idosa que vive em ILPI no contexto da pandemia no Brasil. A análise foi do tipo temática, com auxílio do software IRaMuTeQ Resultados Foram selecionados 28 textos para análise. O movimento social Frente Nacional de Fortalecimento à ILPI apresentou o maior quantitativo de produções. Na análise temática emergiram três classes: (1) O cuidado como estratégia de prevenção de contágio pela COVID-19 na ILPI; (2) O suporte dos gestores públicos enquanto direito da pessoa idosa que reside em ILPI; e (3) A preservação do vínculo sociofamiliar do residente da ILPI durante a pandemia. Conclusão Os movimentos sociais e entidades tiveram um lépido desempenho no suporte às ILPI no Brasil por meio da divulgação de documentos que contribuíram para nortear o cuidado à pessoa idosa institucionalizada em situação de vulnerabilidade. Faz-se necessário maior envolvimento dos gestores públicos na proteção e direito à vida dessa população idosa.

ABSTRACT Objective: to analyze the available evidence in the nursing literature about financial-patrimonial elder abuse. Method: integrative review of articles indexed in the databases CINAHL, Scopus, Web of Science, LILACS and MEDLINE, from 2007 to 2017. The combination of controlled and uncontrolled descriptors was used in Portuguese, English and Spanish. Results: 15 studies developed in seven countries were included. In the thematic analysis, three categories emerged: epidemiological data, risk factors and institutions providing services to the elderly. The occurrence of this type of violence concomitantly to the other subtypes was highlighted among the most frequent. Final considerations: the literature points to important epidemiological data, risk factors and characteristics of this type of violence, including within institutions providing services to the elderly, in different countries, essential aspects for structuring and rethinking public policies for protection and appreciation of the elderly.

ABSTRACT Objective: To synthesize the knowledge produced on best nursing practices in long-term care for elderly at home, in order to promote healthy aging. Method: A systematic review, based on the Joanna Briggs Institute’s proposal: without restriction of dates; in the English, Portuguese and Spanish languages; conducted in PubMed, CINAHL, LILACS, Embase and Scopus databases. Results: Among 453 articles identified, 16 were included in the review: seven qualitative and nine quantitative, published between 1996 and 2015. The synthesis of the data identified as best practices identifies a premise of care centered on the elderly and the inclusion of the elderly, family and nurses as agents of this care. Conclusion: According to evidence, good practices in gerontological and nursing home care fundamentally depend on constant planning and reorganization, so that they are indeed comprehensive and contextualized. Thus, providing care will be reasoned by and driven to the elderly, based on their specific and global needs, favoring a process of healthy and active aging.

Objetivo: analisar as matérias sobre o cuidado do idoso durante a pandemia da covid-19 publicadas por três jornais considerados de maior circulação no Brasil. Materiais e métodos: estudo documental, retrospectivo, de caráter exploratório e descritivo, com análise qualitativa. Os dados foram coletados de matérias publicadas on-line nos jornais O Globo, O Estado de S. Paulo e Correio Braziliense, de acesso livre, de 1 de janeiro a 31 de maio de 2020. Resultados: foram identificadas 2.147 notícias com a temática cuidado do idoso no Brasil durante a pandemia da covid-19 e, ao final, selecionadas 25. A análise possibilitou a criação de quatro categorias: i) população idosa e risco de morte por covid-19; ii) internação e sistema de saúde diante da pandemia; iii) medo e medidas de enfrentamento da população diante da pandemia e iv) profissionais de saúde e luta contra a covid-19. Conclusões: identificou-se, nas notícias analisadas, que as informações sobre o cuidado do idoso com covid-19 relacionaram-se ao medo de que isso os levasse à UTI e, consequentemente, a óbito, mesmo diante dos esforços dos profissionais de saúde para assegurar esse cuidado e evitar o colapso do sistema de saúde.

Objective. To identify overload and associated factors among caregivers of adult patients receiving palliative care. Methods. Descriptive, quantitative, and cross-sectional study addressing 40 adults under palliative care and their respective caregivers enrolled in the Home Care System in Ribeirão Preto, Brazil. Data concerning the patients included demographic profile and Mini-Mental State Examination. A form was used to collect the caregivers’ demographic data along with the Zarit Burden Interview Scale, Self-Reporting Questionnaire, Beck Depression Inventory, and Coping Strategies Inventory. Results. Regarding the patients, 84.2% were women, 52.6% were over 80, 65.8% had no partner, and 76.3% presented cognitive impairment. The caregivers were mostly women (84.5%), aged 56.67 years old on average, were the patients’ children (42.5%); had no partner (55%), and lived with the patient (77.5%). The mean score obtained in the burden scale was 28.78 points, 32.5% had stress, and 42.5% depression. Regarding coping strategies, the ones most frequently used were positive reappraisal (12.8), withdrawal (10.2), and problem solution (9.7). A positive and statistically significant correlation was found between time spent with care (days and hours) and escape/avoidance with overload. Objective: to analyze the newspaper articles on hospital care for elderly COVID-19 patients in online newspapers. Method: documentary, retrospective, descriptive and exploratory research. The data were collected from articles published on open-access websites of 12 newspapers from the following countries: Brazil, Spain, United States, France, Italy and Portugal. Results: out of 4,220 newspaper articles identified in this regard, 101 were selected after applying the inclusion criteria, the majority coming from Italy. The data analysis revealed three thematic categories: the care for patients with COVID-19 in the health system; the work process of the health team and its concern with contagion; and ethical dilemma in care for the elderly during hospitalization. Conclusion: the COVID-19 pandemic presented itself quickly and was widely reported in all countries. The health systems need to reorganize for care to the global population, especially the elderly, considering their weaknesses and also the lack of prior professional training to offer care to this population.

Objetivo : analizar los traumas por accidentes de tránsito en ancianos atendidos en Emergencias de hospital municipal de referencia en 2010 y 2011. Metodo : estudio longitudinal, retrospectivo, realizado en hospital de urgencias y en Delegación de Represión a Crímenes de Tránsito sobre población de 524 ancianos. Se efectuó análisis descriptivo para todas las variables. Resultados : se determinó riesgo de participación en accidente, ocurrencia de lesión y fallecimiento utilizando riesgo relativo. Se verificó asociación estadísticamente significativa en accidentes con lesión provocada por atropello de motocicleta. Se constató asociación entre sexo masculino, ocurrencia de accidente, lesión y fallecimiento, particularmente entre ancianos menores. De los 524 ancianos accidentados, prevaleció sexo masculino, casados, con escolarización primaria. El 78,6% presentó lesión, siendo mayoría los peatones. Conclusión : entre ancianos jóvenes, la posibilidad de fallecimiento derivada de lesión se mostró 3,4 veces mayor en 2010.

Objective: to synthesize the knowledge about the association of frailty syndrome and cognitive impairment in older adults. Method: the Joanna Briggs Institute’s systematic review of etiology and risk factors was adopted. The search for the studies was conducted by two independent reviewers in the databases MEDLINE, Embase, CINAHL and LILACS and by manual search was performed by tow reviewers independently. The measures of association Odds Ratio and Relative Risk were used in the meta-analysis. The software R version 3.4.3 and the meta-analysis package Metafor 2.0 were used for figure analysis. Results: three studies identified the association of frailty syndrome and cognitive impairment through Odds Ratio values show that frail older adults are 1.4 times more likely to present cognitive impairment than non-frail older adults. Four studies analyzed the association through the measure of Relative Risk and found no statistical significance, and four studies used mean values. Conclusion: despite of the methodological differences of the studies and the lack of definition of an exact proportion in the cause and effect relationship, most studies indicate Frailty Syndrome as a trigger for Cognitive decline.

Objetivo: compreender os significados e os possíveis fatores de violência segundo o modelo ecológico da violência para idosos que vivem no domicílio. Metodologia: estudo descritivo-exploratório e qualitativo, realizado com 19 idosas. Os dados foram coletados por meio da realização de grupos focais e as falas analisadas baseados em análise temática indutiva, com auxílio do software Iramuteq. Resultados: o modelo teórico e o conceito sobre a violência possibilitaram a construção de quatro categorias: Os fatores socioeconômicos e a violência contra o idoso; Violência e os fatores individuais; Violência e os fatores sociais e Violência e as relações do idoso com a comunidade. Conclusão: para os idosos, a violência está relacionada aos significados construídos acerca do problema, sobretudo na sociedade atual onde não se sentem respeitados como cidadãos plenos de direitos, mas sim excluídos socialmente.

Objetivo: determinar a sobrecarga do cuidado e sua associação com a rede de apoio social e estresse emocional do cuidador principal do idoso atendido no serviço de atenção domiciliar. Metodologia: estudo quantitativo e analítico realizado com 94 idosos e seus respectivos cuidadores. Quanto à coleta de dados, para o idoso foram utilizados os instrumentos de perfil sociodemográfico e morbidades, Mini Exame do Estado Mental, índice de Katz, escala de Lawton e Brody; e para o cuidador, o instrumento de perfil sociodemográfico, escala de sobrecarga de Zarit, Self-Reporting Questionnaire e instrumento de medida da rede e apoio social. Resultados: verificou-se predomínio de idosos do sexo feminino, idade superior a 80 anos, casados e com escolaridade de 1 a 4 anos. Os cuidadores principais são do sexo feminino, menores de 60 anos, casados, escolaridade de 9 a 12 anos; a maioria era filha do idoso e viviam com ele. Observouse uma associação entre a sobrecarga do cuidador e a dependência do idoso para as atividades instrumentais da vida diária, o estresse emocional e o domínio “interação social”. Conclusão: cuidar de idosos pode gerar sobrecarga quando o cuidador principal está sob estresse, não possui uma rede social de apoio e o idoso é dependente, portanto, o profissional precisa traçar um plano de cuidados para o binômio idoso-família.

ABSTRACT Objective: To verify the occurrence and factors associated to social, individual and programmatic vulnerability among older adults. Method: A household and cross-sectional survey conducted with 701 community-dwelling older adults. For evaluation of the individual component, we used the frailty phenotype; for the social component, the social vulnerability index was implemented; and for the programmatic component, the Index of access and use of health services. Descriptive and bivariate statistical analyzes and multinomial logistic regression were also carried out (p≤ 0.05). Results: It was verified that 15.7% of the older adults lived in areas of high social vulnerability, 31.8% were physically frail and had a moderate programmatic vulnerability score. Older people of a lower age, having lower education and income levels were more likely to live in areas of high or very high social vulnerability. The female gender and the high age groups increased the chances of the frailty condition. It was also observed that the older adults in the 70├ 80-year age group and having lower education were more likely to have medium programmatic vulnerability. Conclusion: The importance of primary care professionals to consider the multidimensional aspect of vulnerability in identifying older adults who need to be prioritized in health care is evidenced.

This study analyses the multiple abuse against older adults reported to the elder abuse police units of three Brazilian cities from 2009 to 2013. This is a longitudinal and retrospective study carried out through the analysis of police reports (PRs) in the elder abuse police units of three Brazilian cities: Ribeirão Preto (SP), Teresina (PI), and João Pessoa (PB). Descriptive statistical analysis consisted of absolute and percentage frequencies. The chi-square test, Fisher's Exact test, and Relative Risk (RR) were used to analyze the data, with a 95% Confidence Interval (CI) and a significance of 5%. A total of 2,313 reports of older adult abuse were analyzed, of which 245 (10.6%) were related to reports of multiple abuse, 49.4% in Ribeirão Preto, São Paulo, 22.9% in João Pessoa, Paraíba, and 27.8% in Teresina, Piauí. Most of the victims of recurrent older adult abuse are female and younger elderly. Psychological abuse was the most recurrent, followed by financial abuse, occurring in the victim's own home. In João Pessoa, older women and elderly living with their abusers were at a higher risk of report of multiple abuse acts. In Ribeirão Preto, living with the aggressor was a risk factor for multiple violent acts. In the total population of the study, living without companion and not living with the aggressor were identified as protective factors against recurrent violence. The need to implement social and legal actions to improve safety for the more vulnerable groups is emphasized.

ABSTRACT Objective: To analyze economic-financial and patrimonial cases of elder abuse, recorded in the specialized police station for security and protection of the elderly of a capital city in the northeast region of Brazil. Method: A cross-sectional study was carried out with data extracted from police reports and inquiries recorded in the abovementioned police station. Descriptive and inferential statistics were carried out by means of Pearson’s chi-square test or Fisher’s exact test, and multivariate logistic regression. Results: The sample was made up of 555 police reports. The data showed that financial abuse presented a prevalence of 58.9%, and that older elderly people (67.9%), men (70.4%), and single (75.0%) presented a higher percentage of financial abuse compared with other types of violence, often occurring in public places, with 3.1 more chances of occurrence than at the elderly’s home. Regarding aggressors, women (73.5%), without suspicion of alcohol use (66.4%), and non-family members committed more financial abuse, evidencing 2.97 more chances of practicing it. Conclusion: In the period studied, financial abuse increased in its magnitude when compared with other types of violence, a fact that justifies researching the theme in order to prevent it.

ABSTRACT Objective: to investigate the association between frailty with hospitalization and institutionalization in a follow-up study of elderly residents. Method: the follow-up study was performed in 2008 and 2013 with elderly of both genders, aged 65 years and older who were living in the community-dwelling. The sampling procedure performed was probabilistic, with dual-stage clustering. In 2008, 515 elderly people were interviewed and, in 2013, 262. We used the socioeconomic and demographic data, self-reported morbidity, specific data of hospitalization and institutionalization. Frailty was measured by the Edmonton Frail Scale (EFS), and functional capacity through the Functional Independence Measure. Results: we found the mean gross EFS score was higher among resident elderly who were hospitalized and institutionalized and was statistically significant in both investigated years. Conclusion: the confirmation of association between frailty and hospitalization and institutionalization reinforces the importance of the subject, and highlights frailty as an important tool for risk estimates for these adverse events.

ABSTRACT Objective To identify the prevalence of post stroke depression (PSD) and their socio-demographic and clinical predictors among elderly stroke survivors after hospital discharge to home. Methods In this cross-sectional study, 90 elderly stroke survivors were evaluated 14 days after hospital discharge with the following scales: National Institutes of Health Stroke Scale, Functional Independence Measure, and Geriatric Depression Scale – 15 items (GDS-15). PSD was defined as a score > 5 on GDS-15. After univariate analyses, a multivariate logistic regression model was built to identify independent predictors of PSD. Results Fourteen days after hospital discharge, 27.7% (95%CI 18.1 to 37.2) of elderly stroke survivors had PSD. Functional dependence was the only independent predictor of PSD (OR: 1.04 95%CI: 1.01 to 1.09; p = 0.02). Conclusion After stroke, depressive symptoms are common among elderly survivors. The degree of functional dependency is the main predictor of PSD among elderly stroke patients in Brazil.

Resumo Objetivo: Analisar os acidentes de trânsito envolvendo idosos em que ocorreu desfecho fatal, segundo condição do acidentado, local de ocorrência e causa do óbito, elaborando mapas de distribuição geográfica. Métodos: Estudo transversal realizado na Delegacia de Repressão aos Crimes de Trânsito de Teresina, em uma população de 68 boletins de registro de acidente e inquéritos policiais de idosos nos anos de 2010 e 2011. Na análise espacial utilizaram-se o Índice Local de Moran e a densidade de Kernel. Resultados: Em relação à condição do idoso no momento do acidente, comprovou-se que 57,3% dos óbitos foram de pedestres. Considerando o local de ocorrência e a primeira causa registrada, constatou-se que 49,6% ocorreram no local do acidente, a maior causa o politraumatismo. Conclusão: Os óbitos concentraram-se na área urbana, com aglomerado de bairros com alta ocorrência de acidente, apresentando correlação positiva, evidenciando também a existência de regiões com maior densidade de ocorrências.

Objective: to assess the frail level of the elderly with chronic kidney disease under conservative treatment. Methods: this is a cross-sectional and correlational study performed with 35 elderly. The frailty was assessed using the Edmonton Frail Scale instrument. A descriptive analysis and Spearman correlation coefficients were performed. Results: the frailty showed a minimal variation of 1 and maximum of 14, with a mean score 7.71 (±3.10). Women (8.05±3.551) and illiterates (9.57±2.637) showed a higher mean score of frail. Objective to compare the frailty syndrome among elderly people living at home in two Brazilian cities and to identify factors related to sociodemographic and health-related variables. Method population-based cross-sectional study with 480 elderly individuals from the cities of Ribeirão Preto/SP and João Pessoa/PB, with application of the Mini Mental State Examination instruments and the Edmonton Frailty, Geriatric Depression and Lawton and Brody scales. Descriptive analysis, Chi-square test, Fisher’s test, Student’s t-test, Spermann’s correlation and Logistic regression were used. In all analyzes, the level of significance was set at p≤0.05. Results in relation to frailty, it was verified that living in Ribeirão Preto, presenting advanced age, low schooling, multiple chronic diseases, reduced cognitive status and functional capacity, besides depressive symptoms, are factors associated with the frailty syndrome, in both cities. Conclusion we identified that the frailty syndrome in the elderly of both cities has a relation with the place where the elderly person lives, age, schooling, number of diseases, reduction of cognitive status, functional capacity and presence of symptoms depressive.

To evaluate the chronic pain and analgesic use in elderly patients admitted to a teaching hospital. Cross-sectional, observational, non-experimental study with 100 elderly. Instruments for collection of demographic and health data, the Mc Gill evaluation scale and intensity of pain were used. The mean age was 71.1 years (SD + 7.4) and females prevailed in the sample (52%); 78% of the respondents reported severe pain and 68% reported using painkillers. Pain was characterized as sensitive and stabbing (44%), choking (39%), unbearable (40%) and nauseating (49%). The findings of this study indicate a high chronic pain index and point to the need to create multidisciplinary programs of interventions focused on the evaluation and treatment of this symptom among hospitalized elderly.

ABSTRACT OBJECTIVE To determine the demographic and health factors related to the frailty syndrome in older adults. METHODS This is a longitudinal quantitative study carried out with 262 older adults aged 65 years and older, of both sexes, living at home. Data collection was carried out in Period 1 between October 2007 and February 2008, and in Period 2 between July and December 2013. For data collection, we used the sociodemographic profile instrument, the Edmonton Frail Scale, the Mini-Mental State Examination, the number of falls in the last 12 months, the number of self-reported diseases and used drugs, the Functional Independence Measure, and the Lawton and Brody Scale. We used descriptive statistics for data analysis, in the comparison of the means between periods, the nonparametric Wilcoxon test, and the method of Generalized Estimating Equations, which is considered an extension of the Generalized Linear Models with p ≤ 0.05. RESULTS Of the 515 participants, 262 completed the follow-up, with a predominance of females, older individuals, and those who had no partner; there was an increase in frail older adults. In the Generalized Estimating Equations analysis, frailty score was related to sociodemographic (increase in age, no partner, and low education level) and health variables (more diseases, drugs, falls, and decrease in functional capacity). There was an association between the variables of age (older), marital status (no partner), and loss of functional capacity. CONCLUSIONS Frailty syndrome was associated with increasing age, having no partner, and decreased functional capacity over time, and investments are required to prevent this syndrome and promote quality in aging.

O objetivo deste estudo é determinar a prevalência de queda em idosos que vivem no domicílio e identificar o nível de dependência para as atividades da vida diária, entre os idosos que caíram e não caíram. Estudo transversal, com amostra por conglomerado de duplo estágio de 230 idosos, com idade acima de 70 anos, de ambos os sexos, em Ribeirão Preto, São Paulo, em 2013. Investigaram-se o perfil social, as quedas, a Escala de Lawton e Brody e o Índice de Katz. Foi identificada queda em 37,8% (1-7; dp=1,0) dos entrevistados. Foi mais prevalente no sexo feminino (25,2%), em idosos mais jovens (23%). Não houve associação entre quedas e Atividades Instrumentais da Vida Diária, entretanto houve associação entre queda e Atividades Básicas da Vida Diária, pois os idosos que caíram possuíam mais dependência nestas atividades. Sugere-se que a investigação destas atividades seja utilizada como estratégia para prevenção de quedas. ABSTRACT This cross-sectional study, to determine the prevalence of falls in older adults living at home, and to identify the level of dependence in activities of daily living among those who fell and those who did not, was conducted using a two-stage cluster sample of 230 individuals of both sexes, aged over 70 years, in Ribeirão Preto, São Paulo, in 2013. Social profile, falls, Lawton & Brody scale and Katz Index were investigated. Falls were identified in 37.8% (1-7, sd=1.0) of individuals interviewed. Falls were more prevalent among women (25.2%), and younger individuals (23%). No association was found between falls and Instrumental Activities of Daily Living, although there was an association between falls and Activities of Daily Living, as those who fell were more dependent on these activities. The study of these activities is suggested as a strategy to prevent falls. Antecedentes: La violencia hacia el adulto mayor es un problema social que ocurre dentro de la familia y que por miedo no es denunciado dificultando el trabajo del personal multi-profesional de salud para la detección, denuncia y prevención de estos casos. Objetivo: Identificar la prevalencia, los tipos de violencia intrafamiliar e su asociación con variables socio demográficas y presencia de síntomas depresivos en el adulto mayor que vive en la comunidad. Método: Estudio cuantitativo, descriptivo y transversal con 369 adultos mayores en el distrito de Breña, Lima – Perú. Se utilizó el instrumento de datos sociodemográficos, Canadian Task Force on the Periodic Health Examination y Escala de Depresión Geriátrica. Para el análisis de los datos se aplicó estadística descriptiva y regresión logística. Resultados: Predominó el sexo femenino, adulto mayor más joven, el estado civil casado y la conformación familiar por el cónyuge. La prevalencia de violencia intrafamiliar fue 79,7% y síntomas depresivos 48,2%. Prevaleció la violencia financiera. A la regresión se verificó que aquel que no trabaja tiene mayor chance de sufrir de violencia. Cuanto más son los años de estudio disminuye el riesgo de sufrir de violencia. Asimismo, aquel que no presenta síntomas depresivos no sufre de violencia. Conclusión: Se verificó que el adulto mayor necesita de atención permanente y que el personal de salud debe reconocer síntomas y señales en el adulto mayor que sufre de violencia y que presenta síntomas depresivos ofreciendo información con la importancia de denunciar estos casos favoreciendo un envejecimiento saludable y con calidad.

OBJECTIVES: The Millennium Development Goals are centered around combatting poverty and other social evils all over the world. Thus, this study seeks to identify the Millennium Development Goals as an object of study in theses from Postgraduate Nursing Programs in Brazil scoring 5 (national excellence) and 6 or 7 (international excellence), and evaluate the association between the score for the program and achieving the Millennium Development Goals.METHOD: Exploratory descriptive document research. Data were collected from the Notes on Indicators/Coordination for Higher Education Personnel Improvement for the 15 Postgraduate Nursing Courses scoring between 5 and 7 in the three-year-period of 2010/2012.RESULTS: of the 8 Millennium Development Objectives, 6 were dealt with in the theses. There was an association (Fisher's exact test p-value=0.0059) between the distribution of the theses and the program scores in relation to the Millennium Development Objectives (p-valor=0.0347)CONCLUSION: the doctoral theses were slightly related to the Millennium Development Objectives, covering the population's economic development, health conditions and quality of life. It is recommended that Postgraduate Programs in Nursing pay closer attention to the Millennium Development Objectives..

OBJECTIVE To describe the scientific knowledge produced about trauma in the elderly caused by traffic accidents in healthcare area studies. METHODS Integrative review of studies from 2003 to 2013 searched in LILACS, SciELO, PubMed and CINHAL databases. We used combination of the descriptors injuries, wounds and accidents, in English, Portuguese and Spanish languages. RESULTS 32 studies were selected. In the thematic analysis, three categories emerged: epidemiological data from traffic accidents involving elderly; traffic accidents with elderly pedestrians; and trauma care in the elderly. We observed increased incidence of trauma in most countries and pedestrians represented a large part of the victims. Among these, the elderly are the most vulnerable group. CONCLUSION Studies showed that trauma care in the elderly need protocols and professionals with training in gerontology specialized in trauma care services.

Com o avanço da idade aumenta a ocorrência do diabetes mellitus. Os serviços de saúde necessitam readequar para atender as especificidades desta demanda populacional; para tanto deverá proceder o diagnóstico de saúde da população adstrita. Objetivos: descrever as características de um grupo de idosos portadores e não portadores de diabetes mellitus, segundo as variáveis sóciodemográficas e de saúde, bem como identificar as principais diferenças entre eles. Participaram do estudo 151 sujeitos, com idades acima de 60 anos, com diabetes mellitus (Grupo 1) e 151 sem a referida doença (Grupo 2), emparelhados segundo sexo e faixa etária. Os dados foram coletados através de instrumento estruturado, no domicílio do idoso, processados no programa EPIINFO vs 6. Foi realizada análise descritiva e teste ?2 (p < 0,05). Verificou-se maior percentual para o sexo feminino (73,5%), com 1° grau incompleto (70%), renda de 1 a 2 salários mínimo. Variáveis que apresentaram diferença significativa entre os dois grupos foram: razão de aposentadoria, uso regular de medicamentos, problemas de saúde, procura por serviços públicos de saúde, atendimento e internação. O atendimento ao idoso se torna um desafio, por sua demanda crescente nos serviços de saúde frente às doenças crônicas, dentre elas, o diabetes mellitus.

Objetivou-se identificar o perfil sociodemográfico de idosos vítimas de trauma, caracterizar doenças preexistentes e medicamentos utilizados no domicílio; calcular índices de trauma e desfecho clínico. Estudo retrospectivo e exploratório, com a análise de dados secundários de um banco de dados de um hospital geral terciário, entre 2008 e 2010. Foram estudados 131 idosos, média de idade 69,9 anos, 73,3% homens, 55,1% casados, 54,7% aposentados; 65,6% possuíam doenças preexistentes e 48,9% usavam medicamentos no domicílio. Houve representatividade de quedas (31,3%), seguidas por atropelamento (28,2%), com cabeça/pescoço sendo a região mais acometida (59,5%). Prevaleceu o trauma moderado (44,3%), com condições de sobrevida após o evento (80,2%). Houve associação entre mecanismo do trauma e doença preexistente (p=0,01) e entre mecanismo do trauma e sexo (p=0,03). O conhecimento das variáveis envolvidas com idosos vítimas de trauma possibilita aos profissionais de saúde o planejamento de medidas preventivas, visando aprimorar sua assistência.

The present study aimed at understanding the perception of older adults who are receiving palliative oncological care on self care in relation to different stages of the disease and how such perception affected their lives. This is a qualitative study using oral history conducted with 15 older adults receiving palliative chemotherapy treatment in a health institution. The following categories emerged: social network, perspectives for confronting life, changes and spirituality. It is necessary for nursing staff to understand this process so that the measures implemented take into account all of the implications of the disease and aim at improving quality of life. doi: 10.5216/ree.v16i2.22795.

Pesquisa epidemiológica, descritiva e transversal, que teve como objetivos estimar a prevalência de sobrecarga entre cuidadores familiares de idosos dependentes do município de João Pessoa, Paraíba, e identificar sua relação com condições de saúde, funcionalidade e demanda de cuidado do idoso. A população do estudo foi constituída por 240 idosos residentes nos vinte setores censitários sorteados no referido município. Compuseram a amostra 52 idosos com incapacidade funcional e seus cuidadores familiares . Para a coleta dos dados, aplicou-se um questionário estruturado e as escalas do Miniexame do Estado Mental, Índice de Katz e a Burden Interview. Os resultados demonstraram que 84,6% dos cuidadores apresentaram sobrecarga, a qual apresentou associação com características clínicas e funcionais dos idosos e com a demanda de cuidado. Os achados trazem contribuições relevantes para a elaboração de políticas públicas com vistas ao estabelecimento de suporte formal para cuidadores familiares.
