Abstract

Background: Awake prone position (APP) has been reported to improve oxygenation in patients with COVID-19 disease and to reduce the requirement for invasive mechanical ventilation for patients requiring support with high flow nasal cannula. There is conflicting data for patients requiring lower-level oxygen support. Research question: Does APP reduce escalation of oxygen support in COVID-19 patients requiring supplementary oxygen?The primary outcome was defined as an escalation of oxygen support from simple supplementary oxygen (NP, HM, NRB) to NIV (CPAP or BiPAP), HFNC or IMV; OR from NIV (CPAP or BiPAP) or HFNC to IMV by day30. Study design: Two center, prospective, non-blind, randomised controlled trial. Patients with confirmed or suspected COVID-19 pneumonia requiring ≥ 5 liters/min oxygen to maintain saturations ≥ 94 % were randomised to either APP or control group. The APP group received a 3-h APP session three times per day for three days. Results: Between 9 May and July 13, 2021, 89 adults were screened and 61 enrolled, 31 to awake prone position and 30 controls. There was no difference in the primary outcome, 7 (22.6 %) patients randomised to APP and 9 (30.0 %) controls required escalation of oxygen support (OR 0.68 (0.22–2.14), P = 0.51). There were no differences in any secondary outcomes, in APP did not improve oxygenation. Interpretation: In COVID-19 patients, the use of APP did not prevent escalation of oxygen support from supplementary to invasive or non-invasive ventilation or improve patient respiratory physiology. Abstract

Well-documented natural history allows for thorough study of a species' ecology and evolution. However, inadequate natural history data hamper such studies for many species. For example, diet plays a major role in the evolution of species and traits, but thorough quantitative dietary information is often lacking for many groups. Garter snakes (Thamnophis) have been used as model organisms for numerous studies, including many focused on feeding ecology and evolution, but these studies lack a thorough compilation of dietary records. To fill the substantial natural history gap, we amassed nearly 10,000 dietary records for 32 of 35 Thamnophis species, from more than 150 sources, and classified each species' dietary repertoire. We categorized 15 of 32 species as generalist predators and 17 of 32 species as specialist predators of varying prey groups, presenting novel dietary classifications for 15 species. Additionally, we investigated the hypothesis that generalist predators have larger body size, a pattern shown in other snake communities and lineages. However, we found no significant relationship between body size and dietary specialization in the genus. Our findings add to the knowledge of natural history, ecology, and evolution of Thamnophis and provide access to the largest dietary record repository for the genus to date. [ABSTRACT FROM AUTHOR]

Abstract

Introduction: Ischemic heart disease is frequent, has different manifestation forms, and predominates among diseases leading to patient admission into emergency units and hospital admissions in general.Objective: To deepen the knowledge of patients with ischemic heart disease in a municipal intensive care unit.Method: A descriptive and cross-sectional study was carried out of 528 patients who were admitted into the intensive care unit of Orlando Pantoja Tamayo General Teaching Hospital in Contramaestre Municipality, Santiago de Cuba, with a diagnosis of ischemic heart disease, from January 2016 to June of 2019. The variables used were age group, sex, diagnosis at admission, personal pathological history, hospital stay, and status at discharge. We used percentage to summarize the information, as well as the chi-square test to identify statistical association.Results: There was a predominance of males and ages between 60-70 and 36-59 years. Acute myocardial infarction and the combination of three or more risk factors were more frequent. The highest number of deaths occurred in the first three days after admission and among patients with invasive mechanical ventilation.Conclusions: The characteristics of cardiovascular diseases continues to be a major health concern, as long as they are appearing at increasingly earlier ages. In severe cases, mortality can occur in the first seventy-two hours.

Introduction:Bradyarrhythmias are serious heart diseases that cause severe hemodynamic disorders. Bringing the patient to normal cardiac output may imply that a pacemaker be implanted.Objective:To describe some clinical and epidemiological aspects of patients who received the technique of implanting a pacemaker.Methods:A descriptive and cross-sectional study was carried out with 36 patients who needed a temporary pacemaker, admitted to the intensive care unit of Orlando Pantoja Tamayo General Teaching Hospital in Contramaestrem, in the period from October 2015 to May 2019. The variables used were age group and sex, causes of arrhythmias, and personal pathological history, most used medications prior to admission, time of usage of the temporary pacemaker, and the need for permanent implantation.Results:The male sex and ages 80 and older predominated, the most frequent atrioventricular conduction disorder was the third-degree atrioventricular block. The average time of temporary pacemaker usage was between two and six days.Conclusions:Early implantation in this municipal institution significantly improved patient survival.Keywords: conduction disorders; bradyarrhythmias; temporary pacemaker

Abstract

Introduction:Malnutrition in patients hospitalized in an intensive care room is high, but it is even higher in the critically-ill elderly patient, with repercussions on the respiratory system, prolonged ventilation, and other complications.Objective:To characterize the nutritional status of geriatric patients under mechanical ventilation in an intensive care unit.Methods:A descriptive, prospective and cross-sectional study was carried out during the period from January 2017 to March 2019, in the intensive care unit of Orlando Pantoja Tamayo General Teaching Hospital. 83 patients aged 60 or older were studied; they required mechanical ventilation for more than 24 hours. The nutritional status was evaluated with the use of the nutritional control score.Results:There was a predominance of the male sex, the age group 60-74 years, and of surgical diseases. Only 9.1% of those who received invasive mechanical ventilation were not undernourished. Those who died had moderate or severe undernutrition.Conclusions:In ventilated geriatric patients hospitalized in the intensive care unit, a high degree of malnutrition was found. Moderate or severe malnutrition predominated in the majority of those who received invasive mechanical ventilation and in the group of the deceased. Abstract

Rhipicephalus Boophilus microplus cattle tick is a scourge for livestock production. The infestations produced by this pathogen are incompletely contained by chemical treatments, with the associated environmental pollution risks. Vaccination against cattle ticks has emerged as a feasible and environmentally friendly strategy to control tick-borne diseases. In this setting, Gavac® vaccine has proven effective in decreasing cattle tick populations through antibody responses against the tick Bm86 antigen, as part of an Integrated Control Program. However, animal vaccination programs require easy and ready-to-use screening tests to follow up the immune response in vaccinated animals under field conditions. This study reports the evaluation HeberFast® Line Gavac, a lateral flow immunochromatographic system for the rapid detection of anti Bm86 antibodies in vaccinated cattle. The system was tested on 598 serum samples taken from immunized animals, arranged in three groups according to their anti-Bm86 antibody response in ELISA (209 high, 150 medium or 239 low and 100 samples from non-immunized animals. The HeberFast® Line Gavac system was assessed for sensitivity, specificity, and concordance against the ELISA reference test. Consistency was evaluated among production batches and inter-analyst reading-independent consistency at two moments: ten minutes after completing the test and after strip drying. The system showed high sensitivity (81.6%, 82.2%, and 81%), specificity (96.7, 94.6, and 93.3%), and agreement with the ELISA reference test (75%; 74%, and 71%) for high, medium and low anti-Bm86 sera, respectively. The effectiveness of the diagnosis was 87.6; 87.1; 85.9 for high, medium, and low antibody titers, respectively. Consistency among production batches and analysts was documented, and no significant differences between evaluation times were found. These results indicate that HeberFast® Line Gavac is a valuable tool for the serological surveillance of Gavac vaccinated cattle.

Abstract

Tests of lepton universality in B0?KS0„“+“- and B+?K*+“+“- decays where“ is either an electron or a muon are presented. The differential branching fractions of B0?KS0e+e- and B+?K*+e+e- decays are measured in intervals of the dilepton invariant mass squared. The measurements are performed using proton-proton collision data recorded by the LHCb experiment, corresponding to an integrated luminosity of 9 fb-1. The results are consistent with the standard model and previous tests of lepton universality in related decay modes. The first observation of B0?KS0e+e- and B+?K*+e+e- decays is reported.

Abstract

Esta investigación tiene como propósito analizar la influencia de la administración estratégica de la organización. Es fundamental para el desarrollo efectivo de las actividades empresariales, además la información que se presenta está sustentada teóricamente con escritos de diferentes autores, así como investigadores anteriores con el mismo enfoque. El desarrollo está conformado por cuatro capítulos donde se da a conocer la administración estratégica y la importante para el éxito de una organización en el mercado. También se encuentra la planeación estratégica que sirve fundamentalmente para crear estrategias de acción dentro de las empresas, desarrollar ideas, en todos los procesos de la organización sirve como directriz para medir los resultados. Además, se hace mención de la implementación de estrategias y el proceso de implementación de las estrategias que se pueden aplicar para mejorar el funcionamiento en las empresas. Por último, las fases de control y evaluación donde nos dice que el control es una etapa primordial en la administración. A través de esta investigación se logró el análisis de administración estratégica como herramienta para el desarrollo efectivo de las actividades empresariales. Esta es el conjunto de decisiones y acciones administrativas que determinan el desempeño a largo plazo de la organización. La metodología utilizada servirá para toda la comunidad estudiantil y personas interesadas en el tema, siendo un referente para la profundización de la administración estratégica.

The recognition and the implementation of the Human Rights has generated that his fulfillment is in some cases at the cost of the budget. This article will analyze if the servicial rights requested as human rights, they must be granted without conditions, or if, on the contrary, a mechanism must exist in order that there is no abuse in their exigibility and justiciability., El reconocimiento y la implementación de los Derechos Humanos ha generado que su cumplimiento sea en algunos casos a costa del erario. Este artículo analizará si los derechos prestacionales solicitados como Derechos Humanos deben ser otorgados sin condiciones, o si, al contrario, debe existir un mecanismo para que no haya abuso en su exigibilidad y justiciabilidad., O reconhecimento e a implementação dos Direitos Humanos resultaram no cumprimento, em alguns casos, às custas do tesouro. Este artigo analisará se os direitos de benefícios solicitados como Direitos Humanos devem ser concedidos sem condições ou se, pelo contrário, deve haver um mecanismo para que não haja abuso em sua aplicabilidade e justiciabilidade.
