Abstract Background Tuberculosis (TB) still remains one of the most deadly infectious diseases in the world. Mycobacterium tuberculosis β-ketoacyl-ACP Reductase (MabA) is a member of the fatty acid elongation system type II, providing precursors of mycolic acids that are essential to the bacterial cell growth and survival. MabA has been shown to be essential for M. tuberculosis survival and to play a role in intracellular signal transduction of bacilli. Findings Here we describe site-directed mutagenesis, recombinant protein expression and purification, steady-state kinetics, fluorescence spectroscopy, and molecular modeling for S140T and S140A mutant MabA enzymes. No enzyme activity could be detected for S140T and S140A. Although the S140T protein showed impaired NADPH binding, the S140A mutant could bind to NADPH. Computational predictions for NADPH binding affinity to WT, S140T and S140A MabA proteins were consistent with fluorescence spectroscopy data. Conclusions The results suggest that the main role of the S140 side chain of MabA is in catalysis. The S140 side chain appears to also play an indirect role in NADPH binding. Interestingly, NADPH titrations curves shifted from sigmoidal for WT to hyperbolic for S140A, suggesting that the S140 residue may play a role in displacing the pre-existing equilibrium between two forms of MabA in solution. The results here reported provide a better understanding of the mode of action of MabA that should be useful to guide the rational (function-based) design of inhibitors of MabA enzyme activity which, hopefully, could be used as lead compounds with anti-TB action.

Background: Knowledge regarding the risks associated with Zika virus (ZIKV) infections in pregnancy has relied on individual studies with relatively small sample sizes and variable risk estimates of adverse outcomes, or on surveillance or routinely collected data. Using data from the Zika Brazilian Cohorts Consortium, this study aims, to estimate the risk of adverse outcomes among offspring of women with RT-PCR-confirmed ZIKV infection during pregnancy and to explore heterogeneity between studies., Methods: We performed an individual participant data meta-analysis of the offspring of 1548 pregnant women from 13 studies, using one and two-stage meta-analyses to estimate the absolute risks., Findings: Of the 1548 ZIKV-exposed pregnancies, the risk of miscarriage was 0.9%, while the risk of stillbirth was 0.3%. Among the pregnancies with liveborn children, the risk of prematurity was 10,5%, the risk of low birth weight was 7.7, and the risk of small for gestational age (SGA) was 16.2%. For other abnormalities, the absolute risks were: 2.6% for microcephaly at birth or first evaluation, 4.0% for microcephaly at any time during follow-up, 7.9% for neuroimaging abnormalities, 18.7% for functional neurological abnormalities, 4.0% for ophthalmic abnormalities, 6.4% for auditory abnormalities, 0.6% for arthrogryposis, and 1.5% for dysphagia. This risk was similar in all sites studied and in different socioeconomic conditions, indicating that there are not likely to be other factors modifying this association., Interpretation: This study based on prospectively collected data generates the most robust evidence to date on the risks of congenital ZIKV infections over the early life course. Overall, approximately one-third of liveborn children with prenatal ZIKV exposure presented with at least one abnormality compatible with congenital infection, while the risk to present with at least two abnormalities in combination was less than 1.0%., Competing Interests: Declaration of interests We declare no competing interests.

Introduction: Zika virus (ZIKV) infection during pregnancy is a known cause of microcephaly and other congenital and developmental anomalies. In the absence of a ZIKV vaccine or prophylactics, principal investigators (PIs) and international leaders in ZIKV research have formed the ZIKV Individual Participant Data (IPD) Consortium to identify, collect and synthesise IPD from longitudinal studies of pregnant women that measure ZIKV infection during pregnancy and fetal, infant or child outcomes., Methods and Analysis: We will identify eligible studies through the ZIKV IPD Consortium membership and a systematic review and invite study PIs to participate in the IPD meta-analysis (IPD-MA). We will use the combined dataset to estimate the relative and absolute risk of congenital Zika syndrome (CZS), including microcephaly and late symptomatic congenital infections; identify and explore sources of heterogeneity in those estimates and develop and validate a risk prediction model to identify the pregnancies at the highest risk of CZS or adverse developmental outcomes. The variable accuracy of diagnostic assays and differences in exposure and outcome definitions means that included studies will have a higher level of systematic variability, a component of measurement error, than an IPD-MA of studies of an established pathogen. We will use expert testimony, existing internal and external diagnostic accuracy validation studies and laboratory external quality assessments to inform the distribution of measurement error in our models. We will apply both Bayesian and frequentist methods to directly account for these and other sources of uncertainty., Ethics and Dissemination: The IPD-MA was deemed exempt from ethical review. We will convene a group of patient advocates to evaluate the ethical implications and utility of the risk stratification tool. Introduction: Visceral fat accumulation is associated with several changes, such as, increased production of inflammatory biomarkers, especially, C-reactive protein (CRP) and fibrinogen. Anthropometric measurements for central adiposity evaluation, such as, waist circumference (WC) and sagittal abdominal diameter (SAD) have been highlighted. However, there is no consensus on the best anatomical site for measurement., Objective: To evaluate the reliability of different measurements of WC and SAD and verify their capacity to discriminate changes in inflammatory biomarkers., Method: 130 men (20-59 years) were assessed, having measurements of weight, height, WC and SAD. It was considered as the cutoff point for high-sensitivity CRP (hs-CRP) values ≥ 0.12 mg/dL and for fibrinogen the 50th percentile of the evaluated sample., Results: All measurements presented an intraclass correlation coefficient between 0.998 and 0.999. WC measured at the umbilical level (AUC=0.693 ± 0.049) and the smallest circumference between the thorax and the hips (AUC=0.607 ± 0.050) had greater ability to discriminate changes in concentrations of hs-CRP and fibrinogen, respectively. SAD (umbilical level) showed the better ability to detect changes in concentrations of hs-CRP (AUC=0.698 ± 0.049) and fibrinogen (AUC=0.625 ± 0.049), according to the ROC analysis (p<0.05)., Conclusion: WC (smallest circumference between the thorax and the hips) and SAD (umbilical level) are the anatomic sites of measurement for use in predicting the inflammatory risk in apparently health men., (Copyright AULA MEDICA EDICIONES 2014. Introduction: Epidemiological studies have shown an association between coronary heart disease and emerging cardiovascular risk factors, such as, levels of fibrinogen and high-sensitivity C-reactive protein (hs-CRP)., Objectives: To verify the ability of biochemical indicators in discriminating changes in the levels of hs-CRP and fibrinogen, in apparently healthy adult men., Methods: Were evaluated 130 apparently healthy men (20-59 years), having measurement of weight and height. Biochemical measurements (lipid profile, fasting glucose, uric acid, hs-CRP and fibrinogen) were performed. Body mass index, total cholesterol/HDL-c and LDL-c/HDL-c ratios were calculated. It was considered as the cutoff point for hs-CRP values ≥ 0.12 mg/dL and for fibrinogen the 50th percentile of the evaluated sample., Results: The uric acid showed the best correlation (r = 0.325) and the higher area under the ROC curve (0.704 ± 0.054), showing greater ability to discriminate higher levels of hs-CRP (p < 0.01). The total cholesterol/HDL-c ratio (r = 0.222) and the LDL-c/HDL-c ratio (r = 0.235) showed the best correlations and the higher areas under the ROC curves (0.624 ± 0.049 and 0.624 ± 0.049) in identify higher levels of fibrinogen (p < 0.05)., Conclusion: The uric acid and the total cholesterol/HDL-c and the LDL-c/HDL-c ratios showed greater ability to identify changes in the levels of hs-CRP and fibrinogen, respectively. Introduction: Obesity has been considered a chronic subclinical inflammation. High sensitivity C-reactive protein (hs-CRP) and fibrinogen are increasingly associated with cardiovascular risk., Objectives: To evaluate the ability of anthropometric and body composition indicators in discriminating higher levels of hs-CRP and fibrinogen., Methods: 130 men (20-59 years) were assessed, having measurement of weight, height, waist circumference (WC), hip and thigh circumferences, sagittal abdominal diameter (SAD), coronal diameter (CD) and body composition. Conicity index, waist/height ratio, body mass index, waist/hip ratio, waist/thigh ratio and sagittal index were calculated. It was considered as the cutoff point for hs-CRP values ≥ 0.12 mg/dL and for fibrinogen the 50th percentile of the evaluated sample., Results: Sagittal index (r = 0.280), waist/thigh ratio (r = 0.233) and waist/height ratio (r = 0.233) showed the best correlation with hs-CRP (p < 0.01). Conicity index (r = 0.305) and waist/height ratio (r = 0.279) showed the best correlation with fibrinogen (p < 0.01). In ROC analysis, the SAD (0.698 ± 0.049) and the conicity index (0.658 ± 0.048) had greater ability to discriminate cardiovascular risk through higher levels of hs-CRP and fibrinogen, respectively (p < 0.01). The cutoff points of 30 cm, 89.9 cm and 20.5 cm were the ones that reached largest sum between sensitivity and specificity values for the CD, WC and SAD, respectively., Conclusions: The SAD and the conicity index demonstrated a greater ability to detect higher levels of hs-CRP and fibrinogen, respectively, in apparently healthy adult men., (Copyright © AULA MEDICA EDICIONES 2013. Introduction and Objectives: Mortality from myocardial infarction is declining in high income countries, but the magnitude of this decline could differ between countries. We sought to compare the mortality trends from myocardial infarction between Spain and the United States., Methods: This was an observational retrospective study. Crude data were obtained from public databases. Standardized mortality rates were calculated for the last 17 years available for both countries (1990 to 2006), and stratified by age and sex. Joinpoint regression analysis was used for the trends analysis and projections., Results: There has been a steady decline in mortality from myocardial infarction in both countries from 1990 to 2006. However, the magnitude of this decline was greater in the United States (relative reductions in men: 42.7% [Spain] and 59.7% [United States], and in women: 40% [Spain] and 57.4% [United States]). The estimated annual percentages of decline in mortality were greater in the United States (men: -10.7%, women: -5.1%) than in Spain (men: -1.9%, women: -5.1%). Projections for 2012 suggest that the mortality from myocardial infarction will be lower in men in the United States (53.33/100,000) than in Spain (81.52/100,000), while for women it will be equal (32.56/100,000 in the United States and 33.56/100,000 in Spain)., Conclusions: The decline in mortality from myocardial infarction has been more pronounced in the United States than in Spain, and projections for upcoming years suggest in the United States it will evolve to rates below those expected in Spain for men and equal rates for women., (Copyright © 2012 Sociedad Española de Cardiología. Objectives: To measure waist circumference (WC) and supine abdominal height (SAH) at different anatomic sites and to assess the relationship with cardiometabolic risk factors in women aged >60 years., Methods: The present study included 113 women from Viçosa, Minas Gerais, Brazil. The evaluations comprised anthropometric, biochemical and haemodynamic measurements. Different anatomical sites were used to measure WC: (i) the midpoint between the last rib and iliac crest; (ii) umbilical level; (iii) immediately above the iliac crests; and (iv) the narrowest point between the last rib and the iliac crest. Measurements were also taken at different anatomic sites for SAH: (i) the midpoint between the iliac crests; (ii) umbilical level; (iii) higher abdominal diameter; and (iv) the narrowest point between the last rib and the iliac crest., Results: It was found that 35.4% of women were overweight, and the area (SE) body mass index was 25.8 (4.2) kg/m(2). WC at the umbilical level [area (SE) area under the curve (AUC) = 0.694 (0.079)] and SAH at the midpoint between the iliac crests [AUC = 0.747 (0.076)] showed the largest areas under the receiver operating characteristic curve (P < 0.05) with respect to the identification of cardiometabolic risk factors associated with the metabolic syndrome (MS) where, of the two measures, SAH showed the greatest predictive potential., Conclusions: The results obtained in the present study suggest that, for the assessment of older women, the umbilical level and the midpoint between the iliac crests should used to measure WC and SAH, respectively. Background/aims: Although a variety of classical body measurements have been used to assess adiposity, it is still uncertain which is the best indicator to predict effects arising from the accumulation of body fat (BF) in the elderly. The objective of this study was to analyze different classical anthropometric and body composition measurements and their potential for predicting metabolic syndrome (MS) in elderly women., Methods: There were 113 women (60-83 years old) participating in the study, all of whom had their anthropometric, biochemical, hemodynamic and health conditions evaluated. Statistical analysis consisted of correlation coefficient and receiver operating characteristic (ROC) curves, and calculation of the area under the curve., Results: The waist-hip ratio (WHR) and waist circumference correlated with three (hyperglycemia, hypertriglyceridemia and low concentrations of high density lipoprotein-cholesterol) of the seven cardiometabolic risk factors studied. Body mass index, BF, percentage of BF, and the sum of skinfolds were less related to metabolic risk factors. Among the indicators used to characterize central adiposity, WHR was the index that showed the greatest area under the ROC curve., Conclusions: It is suggested that the WHR, an indicator of abdominal adiposity, should be incorporated into the identification of risk of MS in elderly women., (Copyright © 2012 S. This study aimed to investigate the relationship between stress, working conditions and the nutritional status of 53 military police officers in a Southeast city of Brazil. In order to evaluate the symptomatology and the stress phase, the Inventory of Stress Symptoms Lipp for Adults - ISSL was utilized. The assessment of the working conditions was performed by means of socio-demographic questionnaire, direct observation and interviews. The nutritional and health conditions were assessed through anthropometric measures, biochemical tests, blood pressure measurements and cardiovascular disease risk calculator. The sample is of the male gender (92.5%) and aging below 40 years old (73.6%). From these, 35.8% showed stress and 68.4% were in the resistance phase, with 31.6% almost burned out. Through the calculation of Chi-square we could find positive association between the BMI and tiredness (P = 0.0188), between the BMI and irritation (P = 0.0005) and the BMI and the appearance of nervous system problems or emotional problems (P = 0.0304), indicating that these statuses or problems could be related to work. We can conclude then, the stress is present among military police officers. No case of critical stress was found, and the stress phases identified are still susceptible to intervention.

Background: The criteria for best defining the metabolic syndrome (MS), especially in the elderly population, are still little known, and the understanding is increasingly necessary., Objective: Compare the four MS definition proposals, two official ones (National Cholesterol Education Program's Adult Treatment Panel III/NCEP-ATPIII and International Diabetes Federation/IDF) and two proposed definitions (Metabolic Syndrome - National Cholesterol Education Program's Adult Treatment Panel III - modified/MS-ATPM and Metabolic Syndrome - International Diabetes Federation - modified/MS-IDFM), derived from the changes in the official criteria., Methods: A total of 113 women (60-83 years old) participated in this study; they were submitted to anthropometric, blood pressure, lipid profile, fasting glycemia tests and answered questions related to life style habits and health conditions. Statistical analyses were performed using the chi-square test and Kappa coefficient determination., Results: The frequency of the high pressure levels was similar in the two official definitions (54.8%), with a reduction in the two proposed definitions (33.6%). The homeostasis change of the glucose was higher as per the IDF and MS-IDFM (30.1%). The hypertriglyceridemia and the low levels of HDL-c were similar in all the definitions (35.4%). In relation to the abdominal obesity, the higher occurrence was registered by the IDF criteria (88.5%). The presence of the metabolic syndrome presented higher and lower frequencies as per the IDF proposal (45.1%) and MS-IDFM (22.1%), respectively. Higher agreement was found between the modified definition MS-ATPM with NCEP-ATPIII and MS-IDFM (Kappa: 0.79 and 0.77; p < 0.00001)., Conclusion: The MS-ATPM proposal was found more adequate for the MS detection in the evaluated elderly women.

Background: The fastest-growing age group in Brazil and around the world is the oldest-old group (aged 80 and over). Among these individuals, the prevalence of disability and morbidity is higher than in other groups., Objectives: To investigate the influence of socioeconomic, demographic, biological, health, nutritional, and social factors, as well as perceived health, on the functional status of the oldest old., Methods: This was a cross-sectional population-based study in which the data were collected by means of questionnaires and anthropometric measurements. The functional status was evaluated according to the model developed by Andreotti and Okuma (1999). Univariate and multivariate analyses were used., Results: The independent factors associated with worse functional status were: age 85 years and over (OR=2.91), female gender (OR=0.69), continuous use of five or more medications (OR=2.67), no visits to friends and/or relatives at least once a week (OR=11.91), and worse perceived health relative to peers (OR=4.40)., Conclusions: The results suggest that functional status is associated with a complex web of multidimensional factors. Thus, it is important to develop programs related to the factors that are susceptible to intervention in order to provide a better quality of life to the oldest old.

Antiretroviral (ARV) resistance mutations in HIV-1 may reduce the efficacy of prophylactic therapy to mother-to-child transmission and impact future treatment options. ARV resistance mutations and HIV-1 phylogenetic diversity in protease (PR) and reverse transcriptase (RT) genes were assessed among 77 pregnant women (35 naïve, 42 treated with ARV) from Goiânia/Goiás, central west Brazil. ARV mutations in PR/RT genes were analyzed against the Stanford Database, PR/RT HIV-1 subtypes were assigned by phylogenetic analysis and env/gag subtypes were identified by heteroduplex mobility analysis (HMA). Naïve patients had accessory mutations in the PR gene [A71T (1/6), L10V (2/6), L10I (3/6)] and in the RT gene [V118I (2/6), V179D (1/6), V106I (1/6), K101Q (1/6), H221Y (1/6)]. Seven patients (16.7%) under ARV presented drug resistance mutations, one of them to three ARV classes. Most isolates (67.5%) were subtype B, 11.7% subtype F1 and 3.9% subtype C. Recombinant B(PR)/F1(RT) viruses represented 10.4% while F1(PR)/B(RT) viruses made up 6.5%. HIV-1 envgag/PRRT genes were identified as 66.2% subtype B, 3.9% subtype C, 6.5% subtype F1 and approximately 25% B and F1 viruses. HIV-1 genetic diversity in envgag/PRRT genes indicates the spread and dissemination of BF1 recombinant viruses among a significant proportion of patients from central west Brazil. Moreover, discovery of HIV-1 secondary resistance among a considerable number of pregnant women under ARV therapy indicates the importance of genotypic testing during pregnancy for optimal prophylactic intervention. Background: The correlation between the increase in visceral fat and insulin resistance makes the sagittal abdominal diameter and the waist perimeter as potential tools for the prediction of insulin resistance., Objective: To assess the reproducibility of different measurements of the sagittal abdominal diameter and the waist perimeter and analyze the discriminating power of the measurements when predicting insulin resistance., Methods: A total of 190 adult males were studied. The sagittal abdominal diameter (smallest girth, larger abdominal diameter, umbilical level and midpoint between the iliac crests) and the waist perimeter (umbilical level, smallest girth, immediately above the iliac crest and midpoint between the iliac crest and the last rib) were measured at four different sites. Insulin resistance was assessed by the homeostasis model of assessment-insulin resistance (HOMA-IR) index., Results: All measurements presented an intraclass correlation of 0.986-0.999. The sagittal abdominal diameter measured at the smallest girth (r=0.482 and AUC=0.739+/-0.049) and the waist perimeter measured at the midpoint between the last rib and the iliac crest (r=0.464 and AUC=0.746+/-0.05) presented the highest correlations with the HOMA-IR and the best discriminating power for HOMA-IR according to the ROC analysis (p<0.001)., Conclusion: The sagittal abdominal diameter and waist perimeter showed to be highly reproducible and the sagittal abdominal diameter (smallest girth) and waist perimeter (midpoint between the iliac crest and the last rib) presented the best performance when predicting HOMA-IR. Further studies in other groups of the Brazilian population must be carried out to allow the use of these indicators of insulin resistance in the population as a whole, following standardized procedures.

Purpose: To investigate the effectiveness of biochemical indicators from the plasmatic lipid profile to identify the insulin resistance (IR), assessed by the HOMA-IR index (Homeostasis Model Assessment -Insulin Resistance)., Methods: 138 healthy men (20-59 years) were evaluated. The lipid profile biochemical indicators analyzed were the following: triglycerides (TG), total cholesterol (TC), HDL-C, LDL-C, and TC/HDL-C and TG/HDL-C ratios. The percentile 75 was considered as the cut-off point for IR. Statistical analysis consisted of Spearman correlation coefficient, ROC curves and calculation of the areas under the curve (AUC)., Result: The TG/HDL-C ratio showed the strongest correlation and the greatest AUC (r = 0.334 and AUC = 0.724 +/- 0.046, p < 0.001) respectively, followed by the HDL-C (r = -0.313 and AUC = 0.716 +/- 0.052, p < 0.01), the TG (r = 0.261 and AUC = 0.048 +/- 0.674, p < 0.01) and CT/HDL-C ratio (r = 0.259 and AUC = 0.655 +/- 0.05, p < 0.01). Results for TC and LDL-C showed no statistical significant (p > 0.05)., Conclusion: The TG/HDL-C ratio showed the greatest ability to identify IR, proved to be an alternative and easy access instrument to assess IR in clinical practice, therefore providing diseases prevention for the adult male population.

Objective: To investigate cut-off values for HOMA1-IR and HOMA2-IR to identify insulin resistance (IR) and metabolic syndrome (MS), and to assess the association of the indexes with components of the MS., Methods: Nondiabetic subjects from the Brazilian Metabolic Syndrome Study were studied (n = 1,203, 18 to 78 years). The cut-off values for IR were determined from the 90th percentile in the healthy group (n = 297) and, for MS, a ROC curve was generated for the total sample., Results: In the healthy group, HOMA-IR indexes were associated with central obesity, triglycerides and total cholesterol (p < 0.001). The cut-off values for IR were: HOMA1-IR > 2.7 and HOMA2-IR > 1.8; and, for MS were: HOMA1-IR > 2.3 (sensitivity: 76.8%; specificity: 66.7%) and HOMA2-IR > 1.4 (sensitivity: 79.2%; specificity: 61.2%)., Conclusion: The cut-off values identified for HOMA1-IR and HOMA2-IR indexes have a clinical and epidemiological application for identifying IR and MS in Westernized admixtured multi-ethnic populations.

Objective: To assess the ability of anthropometric and body composition indicators in identifying insulin resistance (IR), determining cut-off points for those showing the best efficacy., Method: 138 men were evaluated. Waist perimeter (WP), sagittal abdominal diameter (SAD), conicity index, body mass index (BMI), body fat percent, sagittal index, and the waist-to-height, waist-to-hip and waist-to-thigh ratios were determined. IR was assessed by the HOMA-IR index. Statistical analysis consisted of Spearman correlation coefficient and ROC (receiver operating characteristic) curves, calculating the area under the curve (AUC)., Results: SAD (r=0.482, AUC=0.746) and WP (r=0.464, AUC=0.739) showed stronger correlations with the HOMA-IR and greater ability to identify IR (p<0.001), being 89.3 cm and 20.0 cm the best cut-offs, respectively., Conclusion: The anthropometric indicators of central obesity, WP and SAD, have shown greater ability to identify IR in men. We encourage studies in women and elderly people in search of the best cut-off points for the entire population.

Objective: To compare the different anatomical sites used in the measurement of waist circumference, as well as the effectiveness of these landmarks to predict the percentage of body fat by tetrapolar bioelectrical impedance analysis., Methods: We evaluated 205 children from 6 to 9 years of age of both sexes. Data on weight, height and waist circumference were collected at three different sites: at the lower abdominal curvature, above the navel and at the midpoint between the last rib and the iliac crest. Nutritional status was assessed through the body mass index (BMI)/age as recommended by the World Health Organization., Results: The sample was considered homogeneous in terms of sex, and the mean age was 7.2+/-1.2 years. Regarding nutritional status, 6.3% of the children had low weight, 75.1% were eutrophic, 7.3% were overweight, and 11.2% were obese. Among males, there was no statistical difference between the different sites of measurement; in females, the measurement above the navel was statistically higher. In the correlation analysis, the midpoint measurement showed the best correlation with percentage of body fat, with values of 0.50 in boys and 0.62 in girls., Conclusion: Waist circumference measured at the midpoint between the last rib and the iliac crest presented the best correlation with percentage of body fat.

This study aimed to describe the demographic and epidemiological characteristics of the elderly population, identify the area of influence of basic care in relation to the area of residence of the elderly and the rate of service use, as well as to map the demand according to the geographic location of the Basic Care Unit. The sample comprised 6,964 male and female subjects aged 60 years or more, who sought any of the outpatient public health services between May 2003 and April 2004. From this total, 64.1% were women and 35.9% men. The results show that cardiovascular diseases account for the greater part of medical consultations in the health system, with a mean of 3,576 consultations per year per elderly. Geographic mapping showed the demand for medical consultations in determinate basic care units to be related with the medical specialty available at these units. These results allow identifying the neighborhoods with the highest concentrations of specific diseases, indicating the need for insertion/redistribution of professionals between the basic care units in order to improve the quality of life of the elderly.

Beta-cell dysfunction and insulin resistance are interrelated metabolic abnormalities in the aetiology of Type 2 Diabetes. In several countries, increases in the prevalence of obesity and diabetes have been observed in association with the presence of insulin resistance. In this context, measurement of insulin resistance and beta-cell function is useful. The HOMA indexes (Homeostasis Model Assessment) have been widely used, representing an alternative for the evaluation of these parameters, particularly as a fast, easy and cheap method. This review discusses the origin and evolution of the HOMA index, as well as details of the method, analyzing features related to its validation and the cutoff limits for its interpretation.

The purpose of the present study was to evaluate, in diabetic type 2 patients, the influence of body weight excess assessed by BMI and the central adiposity assessed by the waist circumference in the lipid and glycemic profile. One hundred and forty five individuals assisted in a unit of health, aged >or= 20 years old, were appraised. The female frequency was 61.4%. Among men, the weight excess was correlated with HDL (r = -0.34, p < 0.05), with the total cholesterol (r = 0.35, p < 0.01), and with the triglycerides (r = 49, p < 0.0001), and among women there was correlation just with the triglycerides (r = 0.24, p < 0,05). Waist circumference was correlated with the fast glycemia women's group (r = 0.3, p < 0.01), with the total cholesterol among men (r = 0.33, p < 0.05) and with triglycerides in both genders (F: r = 0.22, p < 0.05; M: r = 0.49, p < 0.001). Highest glycemic and triglycerides levels were present in patients with excess body weight and central adiposity and lower HDL levels were present in patients with body weight excess. In conclusion, there is association between inadequate metabolic profile and the excess of body weight and/or central obesity, evidencing the need for a nutritional and clinical intervention in the diabetic type 2 patients, in order to reduce the risk of future chronic complications.

Given the difficulties surrounding evaluating food consumption, specifically during adolescence, the goal of the present study was to compare different dietary assessment instruments used for this age group. The study was carried out with a group of 60 female adolescents between 14 and 18 years of age. Three repetitions of 24-Hour Recall, three-day Dietary Records and Purchase List of the adolescents and their families were collected. The 24 Hour Recall and Dietary Records had good repeatability, allowing to infer that only one application of one of these instruments was capable of reflecting the ingestion average (or median) of the study population group. The Purchase List allowed us to know the food availability within the context of the individual's conditions. All dietary assessment instruments may contain errors, therefore the choice of the most adequate method must rely on the objectives of the study being developed, as well as the study population.

The purpose of this work is to assess the current, qualitative and quantitative alimentary consumption of patients detected with hiperglycemia in the "Detection of Diabetes in Suspect Cases Campaign" in Viçosa, MG. 256 patients were detected with hiperglycemia in 2001; however, 156 (60.9%) were assessed in 2004. The remaining people were dead, had changed their address, did not accept to participate in the study or did not have diagnosed diabetes. The hiperglycemic patients answered an alimentary frequency questionnaire for assessing their dietary intake and their usual diet recall, both previously tested in pilot-population. The most daily consumed foods were vegetable oil (99%), bean (94%), rice (90%), artificial sweetener (80%), leaf vegetables (63%), milk (61%), and French bread (54%). Most of the patients said they avoid sugar, sugary foods and animal fats. The largest percentiles of nutrients inadequacy happened for energy (85%), fibers (87%), calcium (94%), carbohydrates (82%), monounsaturated fats (91%) and proteins (58%). These last three happened in relation to total energy intake values. The current data allowed us to know about the alimentary habits of these people and also to stimulate nutrition intakes, being useful to the nutritional counseling, suggesting specific orientations in order to provide a more appropriate feeding, contributing to better health conditions and well-being.

Objective: To evaluate the association between cardiovascular risk factors and the anthropometric values--body mass index (BMI) and waist circumference (WC)., Methods: It was studied 231 employees of Federal University of Viçosa, Brazil, 54.1% of them were men (21-76 years old). Glycemia, total cholesterol, LDL, HDL, triglycerides, blood pressure, BMI, WC, waist-to-hip ratio and body fatness were measured. It was also investigated smoking, alcohol consumption and physical activity., Results: The prevalence of overweight/obesity in this population was high mainly in women. The abdominal obesity was observed in 74% of the women and 46.1% of the men. The average of BMI, body fatness, total cholesterol, HDL and triglycerides were significantly higher in men than in women. (p < 0,05). The sedentary lifestyle was a risk factor for obesity. Smoking and alcohol consumption were more common among men and normal weight volunteers. Most of the correlations between anthropometric indices and risk factors for cardiovascular diseases were significant, but weak and the WC was the index that had the strongest correlation and that associated with the largest number of variables. It was observed that with an increase of the BMI and the abdominal fat, there was also an increase of the glycemia, triglycerides and blood pressure and a decrease of HDL. The metabolic syndrome was more common among men and overweight and obese volunteers., Conclusion: In this study, the frequency of cardiovascular risk factors increased along with BMI and WC.

Objective: To evaluate the educational influence in the relative validation of a semiquantitative food frequency questionnaire (FFQs) for adults in the city of Viçosa, Brazil., Design and Subjects: Four 24-h dietary recalls (reference method) were applied to a sample of 94 adults of both genders, at intervals of 1 month. At the end of the study, an FFQs consisting of 58 food items was also applied. Then, the individuals were divided into two main groups according to their educational level (lower and higher). The dietary data were calculated by the Diet Pro 4.0 software and analyzed according to differences of means or medians and Pearson's correlation coefficients. These coefficients were adjusted by the energy and corrected by the within-person variance for each educational group, considering the extreme quartiles of the data distribution., Results: The intake of energy and nutrients, based on the 24 h dietary recalls, was inferior for the lower educational group (P < 0.05). For the FFQs, just the protein and calcium intakes were statistically different, suggesting interference of the education variable in this assessment. Over estimations in the FFQs were identified in the analyses of means and medians for vitamin C and retinol intakes in the lower education group and for retinol in the higher education one. However, when evaluated by correlation coefficients non-adjusted and adjusted, they were well correlated. On the other hand, lipid (r = 0.34) and calcium (r = 0.13) coefficients of the group with less instruction showed beneath the desirable values, suggesting weak consistence of the estimates provided by the FFQs for these nutrients. Correlation means of r = 0.65 and 0.54 were found for the higher and lower educational groups, respectively., Conclusions: The results indicate that FFQs showed acceptable performance on evaluating the habitual food consumption for most of the nutrients in the studied population. A tendency for better quantifications in the groups with higher education was observed, inferring its influence in the assessment of the dietary intake.
